Abstract

技术领域
本发明涉及用于热分解清除人体中的细菌和生物被膜(biofilm)的系统和方法,且更具体而言,涉及牙髓受累的牙齿的牙结构内部和周围的根尖牙周围炎的治疗。
背景技术
多种激光和纤维传输系统已经被提议用于在三维牙根管系统中进行组织消毒或杀菌的明确目的。一般而言,此类系统局限于单向能量传输或局限于在光传输纤维通路远端产生黑体白炽“热尖(hot tip)”。当暴露的“裸”纤维尖端与根管空间内的组织和流体接触时产生这样的能量传输。在这种情形下,碎屑将立即在尖端累积,并且该碎屑将吸收通过相关的光传输纤维传递而来的强烈的红外激光能量。这将立即引起尖端受热和碳化。由于来自红外激光的光子的能量持续被新碳化的尖端吸收,尖端将变得红热(高于726℃)。传导至牙质小管的二次发射的“热尖”能量在口腔组织和液体(包括血液)中引起不期望的局部热和光-生物学事件,即在传输纤维远端末梢附近的牙组织引起不期望的熔化和碳化。因此,在牙髓领域存在通过有效破坏活生物被膜和细菌而不伤害健康的牙组织或其它牙周组织而改善对细菌性原因引起的炎性疾病的治疗的需求。
发明内容
本发明的首要目标是提供用于热分解清除人牙齿根管中的细菌和生物被膜(biofilm)的系统和方法,所述系统和方法涉及使用细长且可弯曲的光学探针以及为该探针提供近红外能量的激光振荡器。优选地,所述光学探针由蓝宝石和锆所组成的类型中的一员组成,并具有向所述光学探针整个横向360°和沿着所述光学探针全长散射光能的光学漫射面(optically diffusive surface)。优选地,所述近红外能量的波长范围在700nm至1100nm,且所述光学探针的长度足够基本上插入牙齿根管的全长。所述光学探针能够在整个根管中从所述探针横向散射所述辐射。以选择性针对整个根管系统中的牙质小管中的细菌或活生物被膜所必需的能量密度和持续时间来提供辐射,立即,由此(1)抑制产生黑体“热尖”,以及(2)在根管空间中诱导激光间隙热疗法(LITT)。主要的光能同时沿着整个根管系统分布,以产生360°三维散射。这允许使用更低的能量和更长的治疗时间,而不产生黑体“热尖”。其结果是根管空间中不出现熔化和碳化以及下文所述的其它优点。

发明详述
感染型根管空间和根尖牙周围炎的现有模型
当活的牙神经(牙髓)的受到致病微生物的感染,将经历不可逆的牙髓炎过程。这见于来自细菌浸润引起的局部炎性产物和组织损伤所导致的牙髓坏死。然后这种级联变为在髓腔和相应的牙根管三维结构中明显的微脓肿。随着感染的进展,牙髓的全部软组织经历液化坏死过程。在硬的牙根管结构中,其导致侧支循环的完全丧失,随后导致牙根管空间内的坏死炎性液体无法获得引流。流体中充满炎症细菌,然后细菌自根顶端离开根管系统,并开始在富含血管的牙周韧带组织的尖周引起大量的炎症反应。尖周是牙根最下面的解剖学区域,并包含根尖牙骨质、牙周韧带和牙槽骨组织。该区域充满血管、淋巴系统和神经纤维。因此,牙根结构或牙根系统的感染将在尖周组织周围产生迅速和复杂的免疫学和炎症作用。即使细菌与高度血管化的牙周上皮细胞直接靠近,也会因为在感染根管空间持续存在病原灶,致使细菌持续生长和繁殖。
牙周韧带的高度血管化性质使得宿主能够产生远超过适当量的免疫学和炎性产物并在局部扩散,以抑制细菌进一步建群和侵入尖周空间。这些免疫学和炎性产物包括溶菌酶、补体、缓激肽、凝血酶、纤维蛋白原、抗体和淋巴细胞。
然而,充满根管的细菌病灶可在这种猛攻中存活下来,并在感染的根管系统这种独特的厌氧小环境中持续生长。这些细菌将在根尖周空间持续繁殖,并加重尖周感染,直到失去牙齿或成功进行根管治疗。前一种情况称为慢性根尖牙周围炎。
当发生强烈的牙尖周组织炎症时,极有可能通过破骨细胞上调而刺激骨吸收活动。如果在一段时间内对受累区域未加处理,由于感染的扩散和骨结构的崩溃,牙根尖周围会出现射线透明区。这就是发生了急性根尖周围脓肿,其是细菌和炎性副产物从根管系统快速扩散至尖周空间的周围骨结构的结果。如果不加处理,该局限性感染将引起严重的后果,包括受累区域的急性骨炎(骨感染)和蜂窝组织炎(软组织感染)。
为了挽救受损牙齿,针对这些病变的传统治疗方法通过对根管空间进行彻底清创去除根尖周的刺激物和它们的生存来源,并及时用根尖封堵物填充根管空间。如果顺利地完成,通过形成纤维蛋白凝块,该根管治疗术可使得尖周组织痊愈。然后该纤维蛋白凝块变成肉芽组织,最终发育成熟为新的骨结构和牙周韧带。
整个齿髓治疗的最终目标是完全密封根管系统的三维区域。如果密封得当,当尖周区域痊愈和再生后,受损牙齿将恢复合适的舒适感和功能。
牙质小管形态学
牙齿中的牙本质由从牙髓延伸到牙根的牙本质-牙骨质-结合部的无数的牙质小管(牙本质中中空充满液体的小管)构成。这些小管的直径约1至3μm,并在牙根结构的整个构造中从牙髓基本上径直延伸至牙本质-牙骨质-结合部。已经计算出牙齿中任一位置每平方毫米牙质小管的量为从4,900至90,000(Mjor and Nordahl,1996)。
感染牙髓并最终在根管系统引起病理性疾病的细菌主要是革兰氏阴性厌氧菌。三十年前,在使用注射用硅酮的新研究中,Davis(Oral Surg OralMed Oral Path,1972)清楚显示了根管系统具有复杂的形态学和结构,以至于即便是经过仔细处理的根管仍存在常规使用的牙髓清创术难于接近的区域。事实上,Sen和Pi′kin(Endod Dent Traumatol,1995)发现,一旦牙髓受到感染,可在包括牙质小管在内的牙齿根管系统的所有区域发现致病菌。
许多研究者已经发现,致病菌在受感染牙齿的牙质小管中处于受感染管壁至牙本质-牙骨质-结合部之间的之间部位。也已经描述了在牙根的根尖部2/3,细菌渗透侵入牙质小管150μm,在牙本质壁内存在细菌内毒素。Perez and Rochd(1993)的体外研究发现,试验接种的牙中的细菌可向牙质小管中渗透至多737μm。
牙质小管系统对于牙髓致病的细菌而言是完美生态环境。小管为恒温37℃,具有完美的湿度和易于获得营养来维持生长和复制。对常规根管疗法最具毒力和抵抗性的生物体是革兰氏阳性生物体,粪肠球菌(Enterococcus faecalis)。粪肠球菌是兼性厌氧菌,对多种抗生素、根管内药物和产氧冲洗剂(irrigant)具有抗性(Sundqvist,Oral Surg.Oral Med.Oral Path.,1998)。
现代根管治疗术须考虑之事项
现代根管治疗术包括根管充填前从牙的三维根管系统去除细菌和患病髓组织,以及用杜仲胶(gutta-percha)建立根尖密封。为去除患病髓组织和细菌以便为填充期做准备,必须进行一系列受控制的机械和化学处理(清除、成形和消毒)。一个前提条件是牙齿中的根管系统一般具有非常复杂的几何学,其在单独小管中具有很多曲线。为充分地克服这种复杂的几何学,已经研发了大量的手工器械(锉刀)和旋转器械(用低速手柄提供动力)并在牙髓操作中用于最初的清洗和成形步骤。这些牙髓器械是可弯曲的,并具有固有的金属记忆性质以有助于对弯曲的三维根管结构进行清洗和成形。这些器械可成功用于“逐步后退法(step-backapproach)”或“冠向下法(crown-down approach)”的机械根管清创术。根管系统彻底清创这一重要步骤通常如上所述以机械方式完成,并在小管壁表面上留下有机和钙化碎屑的“涂层”。
成功的现代根管处理的第二个方面是涉及用冲洗剂次氯酸钠(NaOCl)的5%溶液对三维根管系统进行化学机械清创术。在很多研究中已经发现次氯酸钠在根管系统中具有溶解很多残留有机物质和去除松散的表面碎屑的能力(Svec and Harrison.J.Endod.,1977)。次氯酸钠作为牙髓冲洗液给牙科医师提供很多益处。除了冲洗根管区域,它还是有力的抗微生物剂并具有溶解组织的性质。
然而,牙髓冲洗液例如次氯酸钠不能渗透根管系统的全长也是常识。鉴于根管系统的诸多解剖学复杂性,牙质小管的长度是冲洗液无法完全达到的,并因此可以在该区域留下活的生物被膜和细菌。这主要由根管壁表面的冲洗液的表面张力引起,并且是根管治疗失败的主要原因。Orstavik和Haapasalo(Endod.Dent.Traumatol.,1990)发现次氯酸钠只能保证牙质内至多100μm的抗微生物效果。
因此,人根管系统具有大量的解剖学复杂性,其中很多尚未找出原因,并且因此无法被牙髓锉刀或冲洗液所触及。这种遗漏可最终导致牙髓治疗失败。由于该复杂性以及根管锉刀、旋转装置或次氯酸钠冲洗液无法到达并清洁三维小管系统的所有方面,事实上不可能将感染的牙齿中所有的致病菌和活生物被膜完全破坏和/或去除。
许多证据和临床试验发现,在经过器械处理、冲洗和制备但未闭塞的根管空间中残留的细菌能够在清洗和成形处理后的2至4天内增殖到它们的原始数目。由于残留细菌的快速增殖,许多研究者推荐在进行根管治疗和牙髓治疗之间使用根管内药物(例如CaOH糊)。然而,由于其抗菌活性、可能的抗原活性和难于向隐匿有残留细菌的牙质小管中扩散等,大部分目前使用的管内药物显示出有限的益处。
作为活生物被膜的牙髓细菌病原体
现在已经认识到,细菌能够在牙质小管中存活的原因在于大多数细菌菌落是最近的研究所定义并清楚地认识到的一种受保护的活生物被膜的一部分(Darveau,and Tanner et al,The Microbial Challenge inPeriodontitis,Periodontology 2000 and Chen;Periodontitis as a biofilminfection,J Calif Dent Assoc.2001.)。
Costerton和Lewandowski,J Bacteriology(1994),已经将生物被膜描述为“包含互相粘附和/或粘附于表面或接触面的细菌菌群的基质”。这些研究者还将生物被膜描述为“一种生态群落,其已经进化能够使得该群落作为整体而存活”,具有“位于生物被膜基质中的营养通道(一种简单的循环系统),以促进菌落内的代谢废物的移动”。如果感染牙齿内的牙质小管和三维的管结构转变为上述的生态小环境,而其中隐藏的细菌菌落被视为活生物被膜,则需要实施更有效的处理方法,以清除这些微生物病原体所处根管系统的隐蔽的解剖学复杂性。
对生物被膜的当前理解已经认识到它们中的一些基本性质(Marshand Bradshaw,Physiological approaches to the control of oral biofilms,AdvDent Res 1997)。这些包括,但不限于,不同类型微生物间的群落合作、生物被膜基质中的不同微菌落、细菌菌落周围的保护性基质、不同微生物群落内的不同的微环境、简单的通讯系统、对抗生素、抗微生物剂以及宿主的免疫和炎症反应的独特抗性。
以往进行的用来控制牙髓疾病的大多数尝试是基于在实验条件下获得的对牙髓细菌的理解。然而,牙髓细菌作为活生物被膜,其作用和功能与经典实验室模型所预测的十分不同。与培养基中的牙髓细菌相比,生物被膜中的牙髓细菌产生不同的且更加有害的化学物质和酶。此外,在生物被膜内,通过物种间的相互关系加强了对抗生素耐药性的扩散。生物被膜(一种蛋白性的粘稠基质)本身充当了对许多针对细菌的治疗方案的有效的保护屏障。如果抗微生物剂和管内药物被生物被膜内的抗性酶反应所中和,它们可能甚至无法穿透生物被膜并到达致病细菌。基于对牙髓病形式的这种新理解,可以开发出新的和迄今未尝试过的方法,以抗击隐藏于牙质小管内的细菌和隐匿并保护致病菌的顽固的生物被膜。
以往的治疗牙髓受累的牙齿的经典治疗方法具有最终导致再感染和疾病持续进展的局限性。
完全性牙髓细菌清创术的新逻辑
如果在物理特征方面将生物被膜想象成与未加工的鸡蛋清蛋白类似,就很容易理解单独使用金属锉刀或旋转器械进行根除的困难。事实上不可能用金属器械仅仅通过刮和挫便可从陶瓷面清除破了的鸡蛋。总是有一些粘稠的蛋白性质的清蛋白会留下。
如果在处理活的生物被膜时,在牙质小管深处(机械和化学清创术后)留下粘稠的基质,其中将包含存活的细菌,而这些存活细菌会使得生物被膜在几个小时内重新形成。当前的医疗思路正在试图解决的问题在于,对于牙科医师而言,三维根管系统的一些区域显然是用机械和化学清创术所不可能到达的,且因此活生物被膜和致病菌会持续在其中建群。这就是在实践中需要使用具有能够热分解根管内的细菌和生物被膜作用的特殊末端的可弯曲激光散射尖端来靶向活生物被膜的原因。
借助于使用受控的近红外线激光热分解作用靶向三维牙髓管和牙质小管的活生物被膜,这些经典的牙髓机械和化学清除术方法能够得以增强并给其带来新的应用范围。随着我们对牙髓感染和以活生物被膜的形式隐匿于牙质小管中的细菌的进一步的理解,能够利用受控近红外线光能的分散传输来完全灭活这种残留的粘稠的蛋白性质的基质以及存在于牙质小管和根管系统中的所有细菌和宿主的炎性和破坏性酶。通过该光能在局部转化为热能,本发明的具有特殊末端的根管光散射尖端所发出的激光光子可穿透牙质小管的全长,由此使得生物被膜被凝固(像煎锅中的鸡蛋)。
在进行机械清洁和成形后,通过传输这种光学激光能,残留在牙质小管中的任何生物被膜、细菌和有害酶将转变为变性的灭活固体凝块这一新的物理形式。当机械和化学治疗方案完成后,可以将活生物被膜的热分解作用用作附加的根管治疗。激光增强方法为找出和定向以往的牙髓治疗难于接近的根管区域提供了一种新的途径,并同时通过将小管内的活生物被膜转化为变性并灭活的固体凝块而将其杀死。
现有技术已经确认,相对于受感染牙齿的经典牙髓治疗方法,将激光清创术与根管空间的机械清洁和成形相结合是一种有效的联合治疗方式。然而,现有技术中以往的方法一直难于实施并充满问题。
现有技术中激光根管清创术的描述
在牙髓病学领域有关激光一直存在一个问题:“激光能够提供比常规经典方法更好的治疗结果吗?”美国食品和药品管理局目前批准了几种激光系统用于辅助根管治疗。即便如此,对激光用于齿髓治疗的接受度仍然有限。这是由于在根管空间的狭小范围内进行单向光能传输的固有问题。根管区的机械清洁和成形是经典牙髓病学的首要目标。只有实际上能够有效消融(切割)牙本质的中红外激光能够替代根管锉用于该目的。迄今为止,只有铒激光(中红外消融激光)已经被批准用于该方法。然而,相比于经典机械清除术,中红外线激光的消融速度缓慢,并且光线仅以一个方向(垂直或水平)自传输尖端发射。此外,由于铒激光对水中的发色团具有最高的吸收系数,这种激光的光束在牙本质上每次脉冲将仅穿透2至10μm。这与牙质小管中残留细菌的750μm的渗透距离相比实在是太短了。
根管牙本质的消融不是本发明的焦点或期望的结果。Takeda(Int.Endod.J.,1999)发现,采用经典方法处理三维根管结构之后,可通过激光产生的热量去除涂层并熔化牙本质。现有技术中已经进行的很多研究使用Nd:YAG激光(1064nm)和常规牙科二极管激光(800nm),通过小的可弯曲光纤传输光能,来热杀灭细菌并封闭根管表面。对于被认为在牙质小管中有效热分解细菌的激光能量,最重要的是光能可实际上穿透整个牙根表面渗入牙本质。一些有前景的研究发现,即使减弱光能的强度,其能更深地穿透进入牙本质,其杀菌作用维持在至多1mm的深度(Kline,J.Clin.Laser Med.Surg.,1997)。现在,如果将上面的逻辑与牙质小管充当光能导体(微型波导)(Odor,Int.Ended.J.,1996)且激光辐射能在根管的远端负性影响革兰氏阳性和革兰氏阴性细菌(Moritz,LasersSurg.Med.,2000)等发现相结合,那么将激光用于牙髓细菌和生物被膜的热分解作用的功效将变得明了。
在前一代激光传输装置中,向根管系统的根尖三分之一传输能量,即便不是完全不可能的话,也是非常困难的。使用新型可弯曲光纤,激光能量可以直接传输至根管系统并到达牙根的根尖三分之一处。
在靶组织中具有最大穿透值的用于该功能的理想的激光是800nm至1064nm的近红外激光。然而,现有技术中的纤维传输存在一系列没有克服的问题。光生物学所有方面面临的一个共同问题是每单位时间和每单位面积的功率密度直接影响所发生的激光/组织相互作用的类型。由于高强度激光能量被牙本质(靶组织)吸收,其由于光热相互作用转化为局部热能。在所使用的Nd:YAG或800nm二极管激光器根管激光清创术的现有技术的正常参数中,从光纤尖端的200至400μm的小单位面积传输出巨大密度的光能,其尖端在根管区碳化,并且使近端牙本质熔化并使得邻近骨发生热坏死。下文所述的是一些不良的量子和热相互作用。
近红外激光能的一般光生物学
Niemz(Laser-Tissue Interactions,Fundamentals and Applications,Berlin,Springer,pp 45-80,2002)已经测定了1微秒或更长的脉冲期间的近红外激光波长的所有作用本质上是热。关于这些激光产生的热,要考虑5种因素:
(1)激光的波长和光穿透深度;
(2)暴露的组织的吸收特征;
(3)时间模式(脉冲或持续);
(4)暴露时间;和
(5)激光束的功率密度。
必须要理解的近红外二极管激光的第一个参数是光能的穿透深度。近红外范围的二极管激光在水中具有非常低的吸收系数,因此它们能在含80%水的组织中(包括口腔粘膜和牙质小管)可达到深度的光学穿透。这意味着对于常规的牙齿二极管软组织激光,估计每次脉冲的穿透深度将比Er:YAG硬组织激光高104倍。近红外二极管和Er:YAG激光的短波长在例如黑色素和血红蛋白等分子(发色团)以及黑色色素化的细菌中具有非常高的吸收峰。这将使得激光能以最小的吸收通过水,由于光子被这些色素吸收并通过水,因此在更深的(至多4cm)组织中产生热效应。这种光生物学可实现受控的深层软组织凝固并在牙质小管内传播。
要考虑的下一个参数是待辐射组织基于现有的近红外系统脉冲模式的热效应。对于牙齿的治疗,现有近红外激光对二极管系统以连续波(CW)或门控连续波(门控CW)脉冲发射光子,或对Nd:YAG激光器为自由运行脉冲模式(FRP)。这一点在实践中非常重要,因为组织暴露于激光的光子能的持续时间将决定所达到的热组织相互作用。
在CW或门控CW模式中,激光光子以连续的光束以一个单一功率水平发射。如果光束是门控的,通过位于光路的机械门,基本上是通过激光能量的开和关,光束被间断遮挡。在这种类型激光系统中的开和关的循环期间一般以毫秒级为基础,且“每次脉冲的功率”保持在CW光束的平均功率。在FRP模式中,Nd:YAG激光器能够以微秒级的极短时间间隔产生非常大的激光能量的峰能。
作为一个实例,具有100微秒的短暂脉冲持续时间的这些激光器中的一种以每秒10次传输脉冲(10Hz),意味着激光光子仅以1/1000秒(总时间)击中组织,并且在该秒的其它时间激光器是关闭的。这将在下一次激光能脉冲发射前给组织大量的时间来冷却。脉冲之间更长的间隔时间将有利于该组织的热驰豫时间。CW模式操作总是比脉冲式的能量施加产生更多的热量。
如果脉冲时间持续太长(或在CW中暴露过久),组织中的热驰豫效应将被克服,并且可能对非靶区域造成不可逆的损伤。如果采用足够的冷却时间和适当的暴露时间,则可以避免这些问题。所以,不仅要考虑到在与激光能相互作用的组织中所达到的最终温度,而且还要考虑这种温度增加的持续时间在诱导所需组织效应中发挥所重要作用,并防止不可逆的组织损伤。对于纳秒和皮秒级的脉冲(现今的激光器尚不能达到),激光脉冲过程中的热扩散是可以忽略不计的。
光束的功率密度通过以激光产生的峰功率除以聚焦光束的面积而确定。这意味着用于传输能量的纤维的直径越小(200nm、400nm、600nm),且纤维与组织越靠近(即在不接触组织时斑点的大小越小),则功率密度(每平方毫米光束发射的光子的量)越大,且热相互作用越大。采用非接触式的“清洁”纤维尖端,最重要的两个考虑因素是光束的斑点大小和纤维尖端与组织间的距离。当以“热尖”纤维按照“接触模式”(即施加于整个根管)使用牙科近红外激光时,能量传输以及相应的光生物学将发生显著的变化。
二极管激光装置的尖端的硅纤维除了作为向靶组织传输激光光子的装置以外,如果该尖端发生碳化,即“激活”,其还将充当“热尖”切割或熔化装置。当激活的暴露的纤维尖端与组织和液体相接触时(如将其置于牙神经管中便会发生这种情况),碎屑将立即累积于该尖端。这种碎屑将吸收通过纤维传播而来的强红外激光能,这将导致尖端发热并立即使组织碎屑碳化。由于来自红外激光光子的能量持续被这种新碳化的尖端吸收,尖端将变得红热(温度超过726℃)。Grant,S.等,Degradation-Induced Transmission Losses in Silica Optical Fibers,Lasers inSurgery and Medicine,21:65-71(1997)。一旦发生这种情况,纤维的尖端(已经变为“黑体辐射体”)由于变得白炽和灼热而产生次级的、可见的光发射。由于来自近红外牙科激光器的更多光子持续轰击黑色的碳化尖端并被有机碎屑吸收,尖端的温度会快速升高。(Kuhn,T.,Black BodyTheory and the Quantum Discontinuity,1894-1912,Chicago,The Universityof Chicago Press,1978和Planck,M.,The Theory of Heat)。
正是这种碳化并灼热的纤维尖端的强烈发热被称为二极管激光法的“热尖”。由于牙质小管中存在这种“热尖”,使得光生物学和激光-组织之间的相互作用与使用仅发射初级发射的近红外线光子的非碳化和非接触纤维时完全不同。在现有技术中,纤维的这些现实问题(即“热尖”)无法被克服,且因此必须被医生清楚地认识到,以便能够使用这些激光实现安全和可预测的根管治疗。
“热尖”和黑体辐射体的光生物学
为理解纤维尖端的强发热和随后碳化的热力学和光生物学结果,需要对黑色固体如何吸收然后再发射电磁能进行简短综述。在19世纪中期,Gustav Kirchoff发现“热的不透明物质发射连续辐射光谱”。他观察到黑色固体物体被加热时变得“灼热”并发光。这种现象被称为“黑体发射”。1900年,Max Plank通过研究已有的涉及自高温固体发射热和光的试验数据,描述和揭示了量子力学的一些基本规律(黑体能量释放源自热辐射和原子的热激发)。在根管空间内以接触模式使用二极管激光的牙科医生现在能够理解关于“热尖”(黑体)的一些量子本质,因为“热尖”(黑体)是他们正在使用的根管治疗方法的一部分。
(1)理论上,黑体是吸收所有光的物体(即大部分从激光器发射的红外光子被碳化的尖端吸收)。
(2)由于碳化的尖端持续吸收激光光子,其温度上升(即向“热尖”发射激光的时间越长,或输出的能量越高,尖端就越热)。
(3)发射的光子的能量和峰波长取决于尖端的温度(即尖端变得越热,自尖端发射的总光线、红外线、可见光和紫外线就会越多)。
(4)受热的尖端在红外、可见光和紫外波长以连续光谱发射光(光子)(即不再仅仅是来自激光器初级发射的单一红外波长)。
(5)在所有的波长,越热的物体越明亮。
接触“热尖”的光生物学差异
如上所述,在接触模式中大部分的近红外光子(激光的初级发射)被黑体尖端和引起“热尖”的凝块所吸收。因此,所产生的与尖端相关的凝固带的大小取决于“热尖”在牙质小管和组织中的暴露时间,以及从尖端向这些组织的热传导。Grant等人详细研究了碳化尖端造成的初级激光发射的极大降低(Degradation Induced Transmission Lossesin Silica Optical Fibers,Lasers in Surgery and Medicine,21:65-71(1997)),他们特别研究了接触式激光手术中的“纤维相互作用”。Grant发现,随着纤维尖端的组织沉积物吸收大量激光,其立即发生碳化。
纤维尖端的碳化引起温度的升高,而这会对纤维的光学品质造成明显损害(温度最高超过900℃)。他们还发现,尖端一旦发生碳化,尖端将不再具有合适的光导功能。激光将不再以初级光子进行充分的光凝固,由于尖端的强烈发热,它反而将切割和灼伤组织。
一个重要的问题是,光传输纤维的玻璃部分由两个区域组成:穿行于纤维中央的核心和围绕核心的覆层。覆层具有不同于核心的折光率,并在激光通过纤维传输过程中充当反射物将激光反射回核心。此外,由于纤维尖端持续遭受越来越多的热诱导损伤,更长的持续时间和更高的功率将显著地降低激光辐射的正向功率传输。
当使用830nm二级管激光器以3瓦CW测试360μm的光纤,本发明人(以激光功率计)发现随着组织碎屑造成光纤碳化立即产生了30%的正向功率传输损失。随着辐射时间的持续和组织碎屑的积累,观察到进一步的损失。
Willems和Vandertop在体内测定了该现象。使用预处理的“黑”纤维尖端可安全地进行接触式激光辅助的神经内窥镜术,试验数据发表于Lasers Surg.Med.2001;28(4):324-9。使用二级管和Nd:YAG激光器,在兔大脑组织上比较了常规纤维尖端和涂层纤维尖端的消融效率。使用常规纤维尖端,组织学和热成像表明有害作用深至组织。当使用涂层纤维尖端时,他们报道几乎所有的激光均转化为热能(由于尖端碳化),且立即在尖端自身产生消融温度。而且,他们观察到在低能量和功率(1瓦特持续1秒)下产生消融,而热效应仅限于表面结构。
热效应局限于表面结构似乎是更大部分的激光初级发射被尖端吸收造成激光辐射的正向功率传输衰减的结果。其结果是光传输质量受损。同样重要的是,由于尖端受损导致纤维传输质量下降,能量、焦点和从尖端传输的能量的同质性均受到影响。仍可从尖端正向功率传输出来的初级能量远不足以穿透组织和产生光固定作用。这些就是现有技术中与接触式或非接触式二极管激光器有关的重要的且具有原则性不同的生物学结果。
事实上,基于二极管传输尖端的这些量子热相互作用,并由于高能量密度这一问题,现有技术中已经设定了国际标准。该标准指出,根据现有技术,二极管激光纤维不能在牙齿的根尖终点停留超过1秒,不引起可能造成健康根尖周结构不可逆损伤的温度急剧升高。这是个难以坚持的标准。当用于根管清创术时,牙科二极管和Nd:YAG激光器的标准国际设置如表1所示。

现有技术提倡下列使用方法。进行齿髓治疗的牙齿经常规机械和化学预处理后,用无菌纸签将根管空间充分干燥。将根管的开口扩大至至少ISO 30,使得光纤能够没有摩擦地插入根管空间并弯曲直至达到根尖终点。这一限制旨在防止纤维断裂。一旦激光器被激活,医生仅可以在根尖终点保持1秒的时间,以防止温度的急剧上升和对尖周组织的不可逆损伤。1秒的辐射结束后,将激活的激光纤维以旋转方式自根尖向牙冠部分移动以试图达到所有的内部牙本质结构。一旦该过程完成,按照常规方法用杜仲胶封闭根管。推荐在3周期间进行三次这种治疗以实现充分的细菌热分解作用,然后在用杜仲胶封闭。使用本发明的装置和方法,可使得上述现有技术方法的步骤取得更好的效果。
图1和图2的系统
参照图1和图2,本发明提供了专门的可弯曲光传输探针20,其用于称作激光增强的根管间隙热疗(“LARIT”)的方法。该探针有助于对人牙齿根管22中的细菌和生物被膜进行热分解清除。经光学接头24有效连接于探针的是光缆26、激光振荡器28、定时器30、电源32和手/足控制器34。
优选地,光学探针由蓝宝石和锆所组成的类型中的一员组成,并具有向光学探针整个横向360°并沿着光学探针全长散射光能的光学漫射面;光纤的直径为400至1000μm;光学探针的直径为ISO 20至ISO 70;激光振荡器产生700nm至1100nm的近红外线波长范围的辐射;光纤在激光振荡器和位于光学探针近端的入口之间有效连接。该设计使得光学探针的长度足够基本上插入牙齿根管的全长,并导致来自探针的辐射散射于整个根管,以破坏整个根管系统中的细菌和生物被膜所必需的能量密度和持续时间来散射辐射。
激光根管探针具有沿着探针全长横向发射散射光能的漫射表面。在一个实施方案中,所述表面是粗糙的。在另一个实施方案中,所述表面是磨砂面。该结构能够在传输至根管系统牙本质的光能中实现下列不同的物理特征。
在一种形式中,探针20是逐渐变细的或圆锥状的。如上所示,其沿着已改变的尖端表面的整个跨度以360°放射状散射近红外线光子。使用该逐渐变细或圆锥状的磨砂面的蓝宝石或锆散射探针,能够以均一和可预测的剂量的近红外光能在三维根管空间和牙质小管中实现生物被膜的热分解作用,其中将光子转化为局部的热量。图中所示的(光子的)散射现象在H.Fujii等人,″Light Scattering Properties of a Rough-endedOptical Fibre″,Optical and Laser Technology,February 1984,第40-44页中被广泛讨论。
在图中所示的实施方案中,根管间隙热治疗探针的外表面是有织纹的,来提供毛玻璃效果或磨砂效果,并允许和增强近红外线光子的侧向传输和散射。实践中,探针产生线性漫射体或辐射体。探针本身不受输入模式的限制,即漫射体向外分布光不依赖于偶联模式。使用上述装置,有可能在三维根管系统各处平均地分布照明光学近红外能,以便影响细菌和生物被膜的热分解作用。
(1)可以受控方式沿着探针的纵向全长在根管系统22的整个三维结构(包括牙质小管)中辐射状发射初级激光光子,而探针固定于根管中。
(2)当探针处于根管结构中时,探针的漫射面能够沿探针纵向全长散射光能,而同时控制相邻组织经受辐射的时间。
(3)由于在实践中缺少精确实施的治疗,尚没有可能灼伤或熔化牙齿组织或尖周组织的高密度光能的浓度。具体地,“黑体”的形成受到抑制。
(4)允许牙科医师使用简单的连续波式激光器,以便可以使用更低的激光能,并且能够延长治疗时间,由此扩大附近组织的安全。
该方法避免了对附近组织和健康牙周结构的伤害。主要问题是要避免尖周组织和/或与牙周韧带和骨相连的外根管表面的温度急剧升高。如果任一区域经历温度升高10℃超过一分钟,可能发生牙周韧带坏死、牙齿粘连、和/或牙根再吸收(Eriksson,J.Prosthet.Dent.,1983.)。
治疗窗的机会
为了用近红外牙科激光器实现安全和预测的细菌细胞死亡和活生物被膜的热分解作用,操作者必须认识到激光器与人组织的热相互作用仅提供了非常窄的治疗窗。正常人体温是37℃,其与细菌在牙质小管中的快速生长曲线是对应的。当用近红外牙科激光器将辐射光能施加于口腔组织,受辐射区域(lazed area)的温度立即升高。组织温度每升高10℃均对组织造成伤害性生物相互作用。达到45℃时,残留的软组织变得过热。50℃时细胞酶活性下降并且一些细胞失去活动力。60℃时细胞蛋白质和胶原蛋白变性并且开始凝固。80℃时出现细胞膜的透化作用。100℃时水和生物物质发生汽化。如果牙根周围的尖周或牙周韧带温度升高达到或超过80℃界限达持续相当的时间(5-10秒),便会对骨、牙周和牙齿结构造成不可逆转的有害的伤害。
为了用近红外牙科二极管激光器实现光热分解作用(热诱导死亡)和凝固活生物被膜,需要使靶组织和牙质小管的温度显著升高达特定的时间。对于大多数口腔菌群,在周围牙质小管温度达到50℃之前其持续生长几乎不会减弱。达到该温度后,细菌生长曲线开始减缓。
在60℃,大多数细菌停止生长,除非系统中可能存在的嗜热生物。60℃至80℃这一温度范围被普遍认为能够以时间依赖方式引起显著的细菌死亡和活生物被膜的凝固。活生物被膜发生从粘稠的蛋白性质的基质到固态凝结物这一相移的前提条件是通过熟练控制和传输使得该组织和组织区域短时间达到这一温度范围(60℃-80℃)。要使得近红外牙科激光器产生有效的细菌热分解作用而不过分伤害健康口腔组织,该前提条件必须实现。关键要求是避免牙齿周围软组织的温度急剧升高。使用本发明的激光增强的根管光散射尖端,这是能够做到的。
根据本发明,可成为热分解作用靶向的细菌是那些具体参与牙髓感染的细菌。这些牙髓感染细菌包括,但不限于,梭杆菌属(Fusobacterium)、消化链球菌属(Peptostreptococcus)、真杆菌属(Eubacterium)、普雷沃氏菌属(Prevotella)、乳杆菌属(Lactobacillus)、链球菌属(Streptococcus)、拟杆菌属(Bacteroides)、肠球属菌(Enterococcus)、放线菌属(Actinomyces)、和丙酸杆菌属(Propionibacterium)。
图3的治疗过程
图3描述了用于治疗人牙齿根管的方法,其包括如下步骤:从根管的整个细长空间去除细菌和生物被膜(如36所示);对整个细长空间进行清创术(如38所示);将细长光学探针插入整个细长空间(如40所示);将近红外线辐射纵向传输至光学探针的全长,丙侧向通过细长探针表面进入根管壁的表面(如42所示);以足以破坏所述根管内部及其附近的细菌和生物被膜具的能量密度和时间进行传输;用根尖封堵物填充根管空间(如44所示)。
热力学基本定律表明能量的交换和传递至少可以两种方式发生,其中之一是热传递。本发明所利用的来自吸收光学红外能的热量沉积可具体用作在三维根管空间中根除细菌和凝固活生物被膜的辅助方法。
600nm至1100nm的近红外光光谱固态二极管和Nd:YAG激光器可用于该目的,这是因为它们在牙质小管内具有优良的穿透曲线且没有消融作用。由于这种光谱的红外辐射在水中具有低吸收,这种辐射能可深入穿透生物组织,能达到超过3cm(Nimetz,M.Photobiology2002)。对水中发色团的低吸收和在组织中的深度穿透特征使其成为适合本发明的独特需求的优良光谱。由于具有这些独特的特征,从所述激光增强的根管散射尖端传输至牙质小管的近红外激光能,可实现寻找和靶向三维根管空间中任何位置的细菌和活生物被膜以将其凝固和根除的目的。
目前市售的多种常规近红外牙科激光器均可产生良好的热分解作用。这些激光器易于获得并在810nm、830nm、980nm或1064nm波长发挥作用。由于这些波长基本上均可透过细菌,因此可通过光热转化进行热分解作用和活生物被膜凝固的方法杀死细菌。
可应用于本发明的另一种二极管激光器可使得操作者适当调小功率并更长地增加治疗区的暴露时间,以选择性杀死细菌,同时可伴有或部伴有生物被膜的完全死亡。这是一种双波长(870nm和930nm)二极管激光器。该激光器设计为利用光损伤作用而不是光热作用杀死细菌。这种作用的产生是由于在选定情形下,所述波长(870nm和930nm)不透过活生物被膜中的细菌,而是与细菌的一种或多种细胞内发色团或色素发生致死反应以损伤细菌细胞和诱导死亡。当与根管散射尖端偶联时,该激光穿透活生物被膜的限制而选择性靶向致病细菌,这比任何药物学或机械方法更为有效。这种双波长激光器需要的能量甚至比其它常规近红外激光器更低,且进一步扩展的对抗细菌的治疗窗。由于其已经选择性靶向细菌发色团,因此其同时通过光损伤以及热分解作用和凝固作用杀死细菌。
操作
因此,本发明提供了能够扩展使用目前已有的常规牙科固态二极管和Nd:YAG激光器的治疗窗机会的装置和方法。这种近红外激光能自光学根管尖端散射出来,以简单和可预测的方式热凝固活生物被膜并杀死细菌,并且不损害相邻牙结构。
本发明的装置和方法可使用目前许多牙科医生已经拥有的激光器系统,其通常使用800nm至1064nm波长范围的近红外线进行操作。
已经有大量此类近红外光谱激光系统被用于牙科学和内科学以杀死细菌。紫外光谱也已经用于攻击细菌DNA并杀死细菌。不过,许多紫外波长对人体组织具有有害作用。在近几年中,牙科学已经使用近红外固态二极管和Nd:YAG激光器进行组织切除、烧灼和热分解细菌。四种使用最广泛的牙科近红外波长是810nm、830nm、980nm和1064nm。这些近红外激光在水中具有非常低的吸收曲线,因此具有非常深的组织穿透曲线。由于这些波长几乎可以穿透绝大多数口腔菌群,因此不存在直接的生物体发色团靶向,并且细菌的死亡完全是来自激光的光能局部转化为热能,并将热沉积至细菌和活生物被膜的结果。光吸收引起热沉积,且因此,牙髓激光治疗中过多的功率和能量密度可给患者造成热相关的有害作用。
近红外牙科激光器已经用于杀死口腔中特别是牙髓组织中的致病菌并治疗炎性牙髓病。甚至在牙科学对活生物被膜和通过无光泽的光学传输尖端进行光子散射具有清楚了解前,已经在进行这样的尝试。在所有此类方法中,已经发现独特的时间依赖性的机会性和安全性治疗窗。存在这一狭窄的窗口期的原因在于,制造的机械所产生的红外光能的强烈和深度传输以及常规的近红外牙科激光传输系统的固有的热力学。现有技术的传输一直是通过200或400μm光纤的远端。
以上逻辑进展所证明,本发明的激光增强的根管散射尖端可用于医生可能已经拥有的任何近红外线激光器或与之偶联。
激光诱导的间隙热疗
激光诱导的间隙热疗(“LITT”)涉及热机械现象,其涉及破坏人或动物的疾病或肿瘤部位的不同体积的组织。为了完成LITT,通过选择性吸收激光光子将所涉及的组织的温度升高超过特定的域值温度并持续特定的时间。通过光纤将这些光子传输至组织,所述光纤被设定为在组织中漫射和散射光能。用于传输这种光能的纤维可以被称作间隙热疗纤维(ITT),并通常以玻璃套保护以抵抗来自靶组织的热量。未保护的纤维由于组织碎屑和凝结物而温度显著升高,直至其熔化或断裂。
本发明的意图是将LITT的科学原理应用于可弯曲的粗糙面合成蓝宝石(或其它耐热光学材料)尖端,该尖端与Nd:YAG或二极管激光器通过激光纤维连接器相连,以便通过散射和漫射将光能有效传递至包括牙质小管的整个三维根管系统。在本发明中,在进行常规的机械和化学根管成形和清创术后,将使用LITT根管尖端来实现残留细菌和活生物被膜的凝固作用。
如上所述,本发明的目的是使用特殊设计的LITT可弯曲根管尖端,通过直接靶向活生物被膜和致病细菌,而扩展近红外线牙科激光的机会性治疗窗。该尖端将是粗糙面的或毛玻璃面的合成蓝宝石(或其它坚硬耐热光学材料),其设计为利用LITT的原理和逻辑将光能立即散射至整个根管系统。这些LITT根管尖端的大小将相应于ISO 20、25、30、35、40、45、50、55、60、65、70和75。本发明的LITT根管尖端的不同长度范围是10mm至40mm。
在所述方法中,利用激光增强的根管间隙热疗尖端(LARIT)的直接活生物被膜发色团靶向作用,810nm、830nm、双波长(870nm和930nm)、980nm或1064nm牙科激光器的操作者能够调小功率并增加治疗的时间,以通过凝固作用和热分解作用杀死细菌和获得活生物被膜的相变。在活生物被膜和靶细菌中选择性地将光能转化为热能,可以通过将较少的能量传输至全系统,而引起更快的细菌完全死亡和凝固,由此扩展近红外牙科激光的治疗窗。这使得治疗对牙科患者是更为安全的,并给胶原蛋白、骨和健康软组织提供更多的保护,使之免于受到光能传输的不可逆转的热损伤。
传输激光能可使用商品化的牙科近红外激光器,直径200μm至100m的外科纤维,其通过专用的纤维连接器连接至LARIT设备的锥状尖端。激光能以连续波或脉冲模式传输。激光能以每区域5至120秒传输,而LARIT设备固定于根管系统中,距离根尖1至2mm。在治疗期间,经过LARIT设备从激光中所产生的能量不低于200毫瓦,并且不超过4000毫瓦。
本发明利用了近红外牙科激光的以下重要特征和性质。(1)现在广泛使用的多种近红外线牙科激光器均具有符合用于上述热分解细菌的方法的良好的波长范围。(2)LARIT所用的功率参数,200mw至4000mw,适合于使用现有的牙科激光器。(3)与仅使用常规的用于细菌热分解和活生物被膜凝固的牙科消融激光器相比,将本发明的方法和产品用于近红外牙科激光器更加安全并且更可预测。
Claims (22)
1.用于热分解清除人牙齿根管中的细菌和生物被膜的系统,所述系统包括细长且可弯曲的光学探针、通过至少一根光纤与所述光学探针远端相连接的激光振荡器、有效连接于所述激光振荡器的电源、以及有效连接于所述电源和所述激光振荡器的控制器:
(a)所述光学探针由蓝宝石和锆所组成的类型中的一员组成,并具有向所述光学探针整个横向360°和沿着所述光学探针全长散射光能的光学漫射面;
(b)所述光纤直径为400μm至1000μm;
(c)所述光学探针直径为ISO 20至ISO 70;
(d)所述激光振荡器产生波长为700nm至1100nm的近红外辐射;
(e)所述光纤在所述振荡器和位于所述光学探针近端的入口之间有效连接;
(f)所述光学探针的长度足够基本上插入所述牙齿根管的全长;
(g)所述光学探针造成在整个所述根管中从所述探针横向散射所述辐射;以及
(h)所述控制器给所述激光振荡器提供能量,以便以破坏所述根管中的所述细菌所必需的能量密度和持续时间来散射所述辐射。

(f)以根尖封堵物封闭所述空间。

(e)所述传输具有足够的能量密度和足够的持续时间以破坏所述根管内部和附近的所述细菌和生物被膜的残留物;以及
