技术领域
本申请涉及用于接近和紧固组织的外科紧固件和装置,更具体地,涉及用于在内窥镜下缝合组织的缝线锚固器、打结元件及相关装置。 
背景技术

发明内容
在一方面,提供一种用于打结元件和缝线锚固器的组合式施放装置,该施放装置包括:管状针;与所述针可分离地接合的第一打结元件部分;与所述针可分离地接合的第二打结元件部分;邻接所述第二打结元件部分的打结元件展开致动器;以及与所述针的内部连通的锚固器展开致动器;其中所述打结元件展开致动器和所述锚固器展开致动器从所述施放装置的近侧手柄可滑动地操控,并且彼此保持同轴关系。第一打结元件部分可以通过与管状针上的剪切开口接合的剪切柱可分离地接合,第二打结元件部分可以通过与管状 针上的制动件接合的保持柱可分离地接合。

装置一方面的示例性用途 
这里公开的打结元件和缝线锚固器施放装置可以按照下面说明性的而非限制性的方式用于通过内窥镜并置组织。医生可以引导内窥镜器械如胃窥镜和检查管穿过患者的胃肠道。然后,医生可以使用内窥镜器械检查患者的内部组织如胃,并显示受损组织如溃疡区域。为了实现修补,医生可以使用装有多个缝线锚固器580及相连的缝线582的打结元件和缝线锚固器施放装置,如前述的T附件,并将该装置的远侧尖端520和柔性杆510插入器械的工作通道中,引导远侧尖端520到达修补部位。然后,医生可以操控远侧尖端520使管状针530刺穿受损组织附近的组织,并操控缝线锚固器致动器540将第一缝线锚固器580布置到待并置的组织的第一部分的远侧表面上,或者可选择地布置到该第一部分的内部。然后,医生可以操控远侧尖端520使管状针530刺穿受损组织附近的其它组织,并操控缝线锚固器致动器540将第二缝线锚固器580布置到待并置的组织的第二部分的远侧表面上,或者可选择地布置到该第二部分的内部,而不需要将远侧尖端520从内窥镜器械中撤回来交换第二装置或者重新将缝线锚固器580装入管状针530中。因此,医生可以 放置多个缝线锚固器580而不需要撤回装置。优选地,打结元件和缝线锚固器施放装置装有2、3或4个缝线锚固器580及相连的缝线582。在布置好最后一个缝线锚固器580之后,医生可以操控缝线582的近端来并置修补部位的组织。当组织已经正确并置时,医生可以通过操控打结元件致动器550并向第二打结元件部分570施加力来“启动”打结元件,其中如前所述,第二打结元件部分570与管状针530接合。如果施加的力足够,第二打结元件部分570的保持柱574可以变形并与管状针530的制动件538脱离接合,允许第二打结元件部分570向针的远端滑动,直到其邻接第一打结元件部分560,其中如前所述,第一打结元件部分560与管状针530接合。如前所述,第一打结元件部分560和第二打结元件部分570的结合将会“打结”缝线582。继续操控打结元件致动器550将经过第二打结元件部分570将力传递给第一打结元件部分560,将施加的力变换成作用于剪切柱564上的剪切力。如果传递的力足够,剪切柱564将会切向移动并与管状针530的剪切开口534脱离接合,允许第一打结元件部分560和结合的第二打结元件部分570朝向针的远端滑动,直到结合的打结元件部分离开管状针530的远端。医生可以从内窥镜器械中撤回远侧尖端520和柔性杆510,清除该器械的工作通道,为其它程序作准备,或者为撤回器械作准备。

Claims (10)

1.一种用于打结元件和缝线锚固器的组合式施放装置,该组合式施放装置包括:
管状针;
与所述针可分离地接合的第一打结元件部分;
与所述针可分离地接合的第二打结元件部分;
邻接所述第二打结元件部分的打结元件展开致动器;以及
与所述针的内部连通的缝线锚固器展开致动器;
其中,所述打结元件展开致动器和所述缝线锚固器展开致动器能够从所述组合式施放装置的近侧手柄可滑动地操控,并且所述打结元件展开致动器和所述缝线锚固器展开致动器彼此保持同轴关系,
其中,所述第一打结元件部分和所述第二打结元件部分具有相互匹配的形状并且能够相互接合以保持缝线。
2.根据权利要求1所述的组合式施放装置,还包括在所述管状针的远端内携带的多个缝线锚固器。
3.根据权利要求1所述的组合式施放装置,其中,所述管状针包括剪切开口,并且所述第一打结元件部分包括与所述剪切开口接合的剪切柱,由此可以使用作用力使所述第一打结元件部分与所述管状针脱离接合。
4.根据权利要求1所述的组合式施放装置,其中,所述第一打结元件部分通过经过所述第二打结元件部分传递的作用力分离。
5.根据权利要求1所述的组合式施放装置,其中,所述第一打结元件部分包括与所述管状针通过脆弱的粘结剂层接合的剪切柱,由此可以使用作用力使所述第一打结元件部分与所述管状针脱离接合。
6.根据权利要求5所述的组合式施放装置,其中,所述第一打结元件部分通过经过所述第二打结元件部分传递的作用力分离。
7.根据权利要求1所述的组合式施放装置,其中,所述管状针包括制动件,并且所述第二打结元件部分包括与所述制动件接合的保持柱,由此可以使用作用力使所述第二打结元件部分与所述管状针脱离接合。
8.根据权利要求7所述的组合式施放装置,其中,所述第二打结元件部分通过经过所述打结元件展开致动器传递的作用力分离。
9.根据权利要求1所述的组合式施放装置,其中,所述第二打结元件部分包括与所述管状针通过脆弱的粘结剂层接合的保持柱,由此可以使用作用力使所述第二打结元件部分与所述管状针脱离接合。
10.根据权利要求9所述的组合式施放装置,其中,所述第二打结元件部分通过经过所述打结元件展开致动器传递的作用力分离。
