Abstract

发明背景
治疗患者的感染表面或表面下损伤通常是给患者局部或全身施用抗感染剂。抗生素是一类常用于治疗感染的脓肿、损伤、创伤等的抗感染剂。不幸的是,越来越多的传染源例如细菌变得对常规抗生疗法产生耐性。事实上,由公众医疗增加使用抗生素已导致细菌耐药株同样增加,该耐药株对传统的或甚至新开发的抗菌剂不应答。

与表面或下表面感染的常规治疗相关的另一问题是传染源干扰感染区域的血液循环。某些情况下,传染源引起感染区域的毛细血管或其它小血管收缩,其减少了血流量。当血流量减少时,较低水平的抗感染剂可递送到感染区域。另外,当血流被限制在感染区域时,该感染痊愈所需要的时间更长。这增加了需要施用于患者的药物总量,从而增加了使用该类药物的成本。有时局部用药物可用于感染区域。然而,局部用抗感染剂不能渗透到常常存在大部分细菌的皮肤深处。抗感染剂局部治疗在清除感染方面比全身施用(即,口服施用)抗感染药物的效果低。
另外,尽管近来在慢性创伤护理方面有进展,许多下肢溃疡仍不能痊愈。该下肢慢性溃疡是一种重要的公众健康问题。除了用于其治疗的落在卫生保健体系上的繁重财务负担外,它们对患病的生活会造成沉重的代价。随着北美人口的老化和盛行的肥胖症危机,静脉性、糖尿病性和压迫性溃疡有可能比以往更加普遍。在美国大约4百万(人口的1%)人发生慢性小腿溃疡,大多数归类为糖尿病性或静脉性溃疡,而这一数字在老年(>80岁)患者中可升高到4%-5%。
除感染外,许多因素可潜在地影响慢性溃疡的创伤愈合。这包括过量渗出物、坏死组织、弱的组织处理和损伤组织灌流,以及临床状况例如高龄、糖尿病和施用甾族化合物。

在上世纪80年代,研究者们发现,人体内皮组织产生氧化氮(NO),并且NO是一种内源性血管舒张剂,即,一种增加血管内径的物质。NO被普遍地认为是一种环境污染物,其为燃烧产生的副产物。在低浓度下,例如低于100ppm,研究者们发现吸入的NO可用于治疗患者的各种肺病。例如,已研究将NO用于治疗气道阻力(airway resistance)增加的患者,该呼吸道阻力增加是由肺气肿、慢性支气管炎、哮喘、成人呼吸窘迫综合征(ARDS)和慢性阻塞性肺病(COPD)造成的。

发明概要
已经发现,NO会干扰或杀死体外生长细菌的生长,并且已研究了其作为灭菌剂的应用。本申请的发明人之一的于2000年6月2公开的PCT国际申请PCT/CA99/01123中公开了一种通过NO吸入治疗呼吸道感染的方法和装置。

本发明的第六方面为一种通过局部氧化氮暴露治疗创伤的方法,该方法包括下述步骤:提供含有氧化氮的气体源并递送所述含有氧化氮的气体至创伤,从而用氧化氮沐浴所述创伤。优选的,所述治疗方法包括将所述创伤连续暴露于足够高浓度的氧化氮气体中达足够的时间,以杀死创伤部位的微生物,或使其减少2-3log10,而对受试者或对所治疗的受试者的宿主细胞无明显毒性。例如高浓度的氧化氮气体可以为约120ppm~约400ppm,更优选为约200ppm~250ppm。氧化氮暴露的时间可以为5小时~96小时,最佳暴露时间和浓度可根据受试者的个体条件由医生的建议确定。在另一实施方案中,所述治疗方法还可包括在使用高浓度氧化氮气体的第一治疗期之后的第二治疗期,其中用更低(lower)浓度的氧化氮气体治疗所述创伤。优选所述更低浓度的氧化氮气体为1ppm~80ppm,更优选为5ppm~20ppm。所述第二治疗期的暴露时间也可为5小时~96小时,取决于所治疗的受试者的个体条件。在另一实施方案中,所述创伤优选在夜晚患者睡眠期间暴露于200ppm的氧化氮气体中约7-8小时,并且氧化氮暴露可以在白天撤消,或以低浓度(例如,5ppm~20ppm)提供约5-16小时。

在本发明的第八方面,一种通过局部暴露氧化氮清除创伤的方法,该方法包括下述步骤:提供含有氧化氮的气体源、和向所述创伤递送所述含有氧化氮的气体,从而正向调节位于所治疗受试者的创伤部位局部的宿主细胞内源性酶类例如胶原酶和明胶酶的表达。优选的,该治疗方法包括使所述创伤暴露于足够高浓度的氧化氮气体中达足够的时间以正向调节内源性胶原酶表达,而对所治疗的受试者的宿主细胞无明显毒性。例如所述高浓度的氧化氮气体可以为120ppm~400ppm,更优选为约200ppm-250ppm。暴露氧化氮的时间可以为5小时~72小时,最优暴露时间和浓度可根据受试者的个体条件如由医生的建议确定。优选的,胶原酶在宿主细胞中的表达可以通过活组织检查利用本领域的可用的多种技术分析胶原酶mRNA或蛋白的表达来监测,所述技术例如为RNA印迹(Northern blot)、RT-PCR、定量RT-PCR、免疫染色法、免疫沉淀法或ELISA。此外,在利用高浓度的氧化氮气体的治疗期后,所述创伤还可暴露于更低浓度以进行第二治疗期,从而减少胶原酶表达和增加胶原表达。

在本发明的第十方面,一种通过局部氧化氮暴露减少创伤愈合过程中的结疤(scarring)的方法,该方法包括提供氧化氮气体源,将创伤暴露于高浓度的外源性氧化氮气体中一个治疗期,而对受试者或创伤周围的健康细胞不产生毒性;将所述创伤暴露于降低的浓度(decreased)的外源性氧化氮气体中第二个治疗期,足以增加胶原mRNA的表达;和
将所述创伤暴露于第三浓度的外源性氧化氮气体中第三治疗期,其中所述第三浓度介于所述高浓度和所述降低的浓度之间。所述高浓度优选为约200ppm~400ppm,所述降低的浓度优选为约5-20ppm,而所述第三浓度为约20ppm~200ppm。并且,所述第一治疗期优选为一天至少7小时,并且所述第二和第三治疗期各自优选为一天约5~12小时。所述三步骤治疗可进行多天,并且优选至少3~14天。
本发明的一个目的是提供一种递送装置,用于局部递送含有NO的气体至任何皮肤表面或表面下暴露的创伤、或至身体的任何暴露表面例如眼、或至身体的任何暴露内脏器官。该装置的进一步目的是防止含有NO的气体从所述递送装置中泄漏。递送有效量的氧化氮气体至感染或创伤区域杀死细菌和其它病原体并促进愈合过程的方法。

优选实施方案的详细说明
如图1所示,NO递送装置2与患者4相连。一般说来,该NO递送装置2包括流体地(fluidically)连接于NO气体源8的沐浴单元6、流量控制阀22和真空单元10。图1为本发明的一种优选实施方案。
在图1中,NO气体源8是一个含有NO气体的加压钢瓶。使用加压钢瓶是贮藏含NO的气体源8的优选方法,也可采用其它贮藏和递送方法,例如专用供给线(管壁柔软的,wall supply)。通常,该NO气体源8是N2和NO的混合物。虽然通常使用N2稀释加压钢瓶内的NO浓度,但也可使用任意的惰性气体。当该NO气体源8贮藏于加压钢瓶中时,优选加压钢瓶中的NO浓度为约800ppm~约2500ppm。商业氧化氮制造商通常生产的供医疗用的氧化氮混合物为约1000ppm。极高浓度的NO是不需要的,因为这样NO气体意外泄漏更加危险,并且NO的高分压容易造成NO自发降解成氮气。含有低浓度NO(例如,NO低于100ppm)的加压钢瓶也可用于本文公开的装置和方法。当然,使用的NO浓度越低,加压钢瓶的更换就越频繁。
图1还示出了作为NO递送装置2的一部分的稀释气体源14,其用于稀释NO的浓度。该稀释气体源14可含有N2、O2、空气、惰性气体或这些气体的混合物。优选使用N2或惰性气体这样的气体稀释NO浓度,因为这些气体不会像O2或空气那样将NO氧化成NO2。该稀释气体源14显示为贮藏于加压钢瓶中。尽管图1显示使用加压钢瓶作为贮藏该稀释气体源14的方法,但也可采用其它贮藏和递送方法,例如专用供给线(壁柔软的)。
来自NO气体源8的NO气体和来自稀释气体源14的稀释气体优选通过压力调节器16以减小进入NO递送装置2的气体压力。各气体流通过管路18至一任选的气体混合器20。该气体混合器20将NO气和稀释气体混合,得到含NO的气体,其具有降低的NO浓度。优选从气体混合器20输出的所述含NO的气体具有低于约400ppm的浓度,更优选约200ppm。根据具体应用所需要的浓度,如果需要,也可以将从气体混合器20输出的所述含NO的气体的浓度调节至低于约100ppm或低于约40ppm。
从气体混合器20输出的所述含NO的气体经由管路18流至流量控制阀22。该流量控制阀22可包括,例如,以渐进性增加(或减小,在关闭时)的方式打开(或关闭)的比例控制阀。作为另一例子,该流量控制阀22可包括质量流量控制器(mass flow controller)。该流量控制阀22控制输入沐浴单元6的含NO的气体的流速。该含NO的气体经由柔性管24离开流量控制阀22。该柔性管24与沐浴单元6的入口26连接。该入口26可包括任选的单向阀64(见图3),其可防止气体回流入管24。
仍然如图1所示,沐浴单元6密封在患者4的皮肤表面。感染区域30可以是脓肿、损伤、创伤等,该感染区域30被沐浴单元6封入。沐浴单元6优选包括密封部32,该密封部32与患者4的皮肤、或任何其它身体的暴露表面(例如,眼)或暴露的需要治疗的内脏器官形成结实的气密密封。结实的密封是指所述含NO的气体不会以明显量(即,不高于输送至沐浴单元6的含NO的气体的约5%)从沐浴单元6泄漏。该密封部32可包括可膨胀(inflatable)密封部61,例如如图2和3所示,或者该密封部32可包括加固于患者4表面的柔性边缘(flexible skirt)等。该密封部32还可包括附着在患者4皮肤表面的粘接部。在其它预想的实施方案中,密封部32可仅包括沐浴单元6与患者4皮肤表面的界面(interface)。
该沐浴单元6可以根据期望的用途由实际上无限数量的形状和材料制成。该沐浴单元6可形成刚性结构,如图1所示,其置于感染区域30上。可替代的方案是,该沐浴单元6可由柔性的、袋状材料形成,其在感染区域30上是可膨胀的。图2显示了靴子形状的结构,其置于患者4的脚上。图3显示了另一可膨胀的沐浴单元6,其形成戴在患者4手上的连指手套(mitten)或手套(glove)形状。
在本发明一个优选实施方案中,该沐浴单元6包括测量沐浴单元6中的NO气体浓度的NO传感器34。该NO传感器34优选将该信息经由信号线38报告至控制器36。任选的NO2传感器40也可包含于沐浴单元6中。该NO2传感器40优选将NO2浓度经由信号线42报告至控制器36。所述传感器40、42可以是化学发光敏感型(chemilluminesense-type)、电化学单元型(electrochemical cell-type)或分光光度型(spectrophotometric-type)的传感器。
该沐浴单元6也包括用于从沐浴单元6排除气体的出口44。该出口44优选远离气体入口26,这样NO气体进入和离开该沐浴单元6不会很快。优选该入口26和出口44位于沐浴单元6的使NO气体具有相对长的停留时间的区域。柔性管46与该出口44相连,提供用于从沐浴单元6排除气体的管路。
在本发明的一个优选实施方案中,该柔性管46与吸收器单元48流体地连接(in fluid communication)。该吸收器单元48优选吸收或除去从沐浴单元6排出的气流中的NO。还优选该吸收器单元48还吸收或除去从沐浴单元6排出的气流中的NO2。由于这些气体在高水平下是有毒的,优选在气体排放至大气中之前,这些组分已从递送装置2中除去。此外,这些气体可与真空单元10的内部组分反应,干扰递送装置2的运行。
接着洁净的气体从吸收单元48经由管50进入真空单元10。该真空单元10使管50内呈负压,从而从沐浴单元6抽气。优选该真空单元10在供给管50和沐浴单元6的真空或吸力水平方面是可控制的。这方面,与流量控制阀22连接,沐浴单元6中的NO气体量可以被调节。优选该真空单元10经由信号线52与控制器36连接(coupled)。如下面讨论的,该控制器36优选控制真空单元10的输出水平。然后气体从真空单元10到达向大气开放的排气口54。
应当理解,吸收单元48是递送装置2的任选组件。如果不必考虑NO和NO2的局部水平,则含大量NO和NO2的气体不必从气流中除去。例如,该气体可排至NO和NO2不会达到高浓度的外界环境。可替代的方案是,再循环系统(未显示)可用于与沐浴单元6再循环NO。
仍参见图1,递送装置2优选包括能够控制流量控制阀22和真空单元10的控制器36。该控制器36优选为与输入装置56连接的基于微处理器的控制器36。操作者使用该输入装置56调节递送装置的各种参数,例如NO浓度、NO的滞留和暴露时间、沐浴单元6内的压力等。控制器36还可连接任选的显示器58,以显示测定的参数和设置,例如NO浓度设置点、沐浴单元6内的NO浓度、沐浴单元6内的NO2浓度、进入沐浴单元6的气体流速、流出沐浴单元6的气体流速、递送的总时间等。
如果该递送装置2中存在传感器34、40,该控制器36优选从传感器34、40接收关于气体浓度的信号。信号线60、52分别与流量控制阀22和真空单元10连接,用于传送或接收控制信号。
在本发明的另一实施方案中,不包括控制器36。此时,气体进入沐浴单元6的流速和气体流出沐浴单元6的流速是预设的或通过手动调节。操作者可以设置真空输出,其基本上等于经由流量控制阀22进入沐浴单元6的气体流速。这样,NO气体能够沐浴(bathe)感染区域30而无任何该递送装置2的NO或NO2气体的蓄积或泄漏。
图2所示为靴子形状的沐浴单元6,其用于治疗位于患者4腿上的感染区域30。该沐浴单元6包括可膨胀密封部61,该可膨胀密封部61环绕小腿区域与患者4的皮肤形成结实的气密密封。该实施方案显示了固定于沐浴单元6的入口26附近的喷嘴62。该喷嘴62将NO气体喷射至感染区域30。气态NO的喷射有助于NO渗透感染区域30,以杀灭或抑制病原体的生长。图3显示了另一实施方案的连指手套或手套形状的沐浴单元6。沐浴单元6是可膨胀的,并且包含一可膨胀密封部61,其环绕患者4的皮肤形成结实的气密密封。图3还显示了任选的位于入口26中的单向阀64。如图3和4所示,该入口26和出口44的位置相互远离,并且优选在治疗区域的相对侧,以便新递送的NO气体不会过快地从沐浴单元6排出。
为了治疗感染区域30,沐浴单元6置于感染区域30上。在患者4的皮肤与沐浴单元6之间形成气密密封。如果沐浴单元6具有可膨胀结构,该沐浴单元6必须用气体充气。优选该沐浴单元6最初仅用稀释气体充气,以防止NO和NO2从该装置2泄漏。一旦达到气密密封,该装置操作者启动从NO气体源8到沐浴单元6的NO流。如上所述,这可以手动或经由控制器36完成。
一旦沐浴单元6开始填充NO气体,打开真空单元10,并调节至适当的输出水平。对于可膨胀的沐浴单元6,真空单元10的输出水平(即,流速)应低于或等于进入沐浴单元6的NO气体的流速,以避免沐浴单元6收缩。在沐浴单元6是刚性的装置的实施方案中,真空单元10可设置成在沐浴单元4中产生部分真空。此时,部分真空有助于在患者4的皮肤和沐浴单元6之间形成气密密封。当然,真空单元10也可设置成以与气体被递送至沐浴单元6的速度基本相等的速度抽气。将有效量的NO递送至沐浴单元6以杀灭感染区域30的病原体和/或减小病原体的生长速度。病原体包括细菌、病毒和真菌。
图4显示了本发明的另一实施方案,其中所述沐浴单元6包括搅拌器66,该搅拌器66用于在沐浴单元6中产生紊流条件。该搅拌器66优选是扇型机械,但可包括其它的在沐浴单元6中产生紊流条件的方式。该搅拌器66有助于用新供给的NO气体更新感染区域30。
氧化氮的应用例
对于慢性非愈合性创伤例如患糖尿病损伤的病人,多种因素可潜在地影响创伤愈合,包括感染、过量渗出物、坏死组织、不良组织处置和受损组织灌流。氧化氮气体可用于减少创伤的感染或微生物负担。尽管以下讨论的实施例是将氧化氮应用于皮肤,但是氧化氮也可以局部用于身体的其它表面如眼或任何其它的暴露表面,例如肌肉、韧带、腱和由于切口、撕裂或创伤而可能被暴露的身体内脏器官。
为研究气态氧化氮对潜在的病原体的作用,设计了一个自制的气体暴露培养箱(custom gas exposure incubator),并对温度、湿度和气体浓度进行了规定,提供符合微生物培养箱的环境,同时能够控制气体的精确暴露浓度。图5显示了一个特殊的气态氧化氮(gNO)培养室,设计其是为了体外研究gNO暴露对最适生长条件下的哺乳动物细胞培养物和微生物细胞的作用。在全部的体外研究中该gNO室允许控制和调节以下因素:gNO剂量、总空气流、NO2水平、O2水平、CO2水平、温度和湿度。
对于初始的初步研究,根据两个计划用于呼吸道感染和局部应用的gNO的临床应用,选择了两株细菌性病原体。绿脓杆菌主要与肺病相关,但也可能与皮肤感染相关,如在严重烧伤中。金黄色葡萄球菌与表面创伤感染相关。这两种微生物均被选择用于初步研究。
评价gNO对细菌的直接作用过程的第一步骤是设计一个简单研究,以测定何种剂量,如果有,大约是微生物的致死浓度水平。一旦估算了最佳剂量,则应进行研究。对于这些初始研究,将高密度的绿脓杆菌和金黄色葡萄球菌混悬液(108cfu/ml)接种物铺于琼脂平板上。然后,为了评价对菌落生长的作用,将这些平板暴露于暴露装置中的不同浓度的gNO中。
图6和7表明,高于120ppm的gNO水平能够以大于90%的比率减少细菌菌落形成。进一步研究表明,达到该效果所需的时间为8-12小时。这些结果证实,gNO对绿脓杆菌和金黄色葡萄球菌生长具有抑制作用。此外,该数据提供了初步的证据,证明其有时间和剂量关系趋势,杀菌活性随gNO的暴露时间和浓度的增加而增加。即,随着gNO浓度增加,在平板上生长的菌落数减少。
尽管增加gNO至120ppm时,下降的杀菌趋势达到5-10%存活率,但没有原始数据显示100%杀菌作用。一些细菌可能存活,原因是琼脂中的材料和化学物可能与gNO反应而缓冲了该作用。重要的是,观察到在转移至常规培养箱24小时后,细胞菌落保持着同样大小和数目,而对照组的数目和大小增加至不能计数的程度。这强烈表明gNO暴露防止了细菌的生长,并在gNO暴露期间一定程度上杀灭了细菌。相应地,设计了进一步研究gNO杀菌作用的后续研究。
在剂量和时间范围(ranging)研究之后,进行了一系列的试验,以测定用200ppm的gNO有效地诱发杀菌效果所需要的时间,该200ppm为刚刚高于用于剂量范围研究的浓度,该杀菌效果是针对典型的与临床感染相关的革兰氏阳性和革兰氏阴性细菌耐药株的样本的杀菌效果。成功的杀菌效果定义为细菌减少大于3log10 cfu/ml。进一步的,还包括白色念珠菌(C.albicans)、甲氧西林耐药金黄色葡萄球菌(MRSA)、来自囊性纤维病患者的绿脓杆菌耐药株、B组链球菌和齿垢分支杆菌(M.smegmatis),以判断酵母、多重耐药性菌株和放线菌类是否具有相似的反应。耐药细菌代表多种同时造成呼吸器官和创伤感染的病原体。
对于这些试验,选用盐水作为混悬液培养基,原因是它不会掩盖gNO作为杀菌物的直接作用,而完全添加生长培养基可能引入外部的可变因素(例如,缓冲液、或与gNO反应)。其它培养基也可提供代谢产物和补充营养素,其产生保护细菌免受氧化和亚硝基化损害的酶,从而掩盖gNO的作用。此外还表明,盐水环境更真实地代表了细菌在体内典型地暴露的敌对宿主环境。在盐水中,菌落受到抑制(static)但仍保持生存活性。这与Webert和Jean′s应用的动物模型方法相似。Webert KE,等人,(2000),Effects of inhaled nitric oxide in a rat model ofPseudomonas aeruginosa pneumonia,Crit Care Med,28(7):2397-2405和Jean D,等人,(2002)Beneficial effects of nitric oxide inhalation onpulmonary bacterial clearance,Critical Care Medicine.30(2):442-7。
图8为这些实验的结果,其中由方形点标示的线表示对照暴露微生物的存活曲线,由三角形点标示的线表示NO暴露微生物的存活曲线。这些研究显示,200ppm的gNO对所有试验微生物具有完全的杀菌作用。无一例外,用200ppm gNO攻击的所有细菌具有至少3log10的cfu/ml减小,并且所有试验的结果都为:所有细菌的所有细胞死亡。这些结果的另一特征为细菌不受gNO暴露影响的潜伏期(表1)。该潜伏期之后所有细胞突然死亡。革兰氏阴性和革兰氏阴性细菌、抗生素耐药细菌株、酵母和分枝杆菌(mycobacteria)全部对200ppm gNO敏感。重要的是,发现两种耐药细菌株也敏感。因此,这些结果表明,gNO直接显示对多种潜在病原体微生物的非特异性致死效应。
该研究还显示分枝杆菌的迟滞期与所有其它有机体相比明显不同。该迟滞期说明分枝杆菌可能有一种机制,即保护细胞免受gNO的细胞毒作用的时期长于其它细菌。申请人认为,细胞内具有剂量-时间依赖性gNO阈值,此时发生迅速的细胞死亡。有可能当细菌的正常NO解毒途径被压倒时发生该阈值。这些研究显示并证实,超过生理水平的NO(外源性的,例如通过递送120ppm~400ppm的外源性NO)可能对代表性的耐药菌株是杀菌性的,并且对这些细胞的作用呈现为突然的、致死的和非特异性的。

为了获得对广谱微生物的致死作用,优选将200ppm的氧化氮气体在例如患者夜间睡眠时暴露于创伤部位至少连续7小时。较短时间可以使用较高浓度例如400ppm。较长的治疗选择还可以跨越数天。根据受试者情况也可安排治疗间的中断期。
动物模型的体内研究进一步显示了氧化氮气体的有益作用。在动物模型中,在背中线两侧制作全层皮肤创伤(A组:具有8个8.0mm打孔活组织样品的四只兔,和B组:具有2个50×15mm创伤的四只兔),并在第0天用等体积的金黄色葡萄球菌混悬液进行感染。第一天,A和B中的治疗组分别在200和400ppm gNO中暴露共3天。将组A在特殊控制的暴露室中暴露2个4小时,其间有1小时休息。设计一种特殊创伤膏药(patch),对B组动物采用24小时连续递送模式。对照组仅暴露于相应流速的医疗级空气中。在致创伤3天后采集四个随机打孔活组织样品(8.0mm),并分析细菌含量。采集来自创伤和正常皮肤组织的另外四个打孔活组织样品用于成纤维细胞生存力分析和gNO的毒性作用。
图9显示了下述动物研究的数据:创伤连续暴露于200ppm gNO中72小时的细菌含量与仅暴露于医疗级空气中的对照组相比。在被治疗创伤中可观察到显著的细菌减少。与对照组相比,在治疗期间兔表现舒适、放松,并且在被治疗动物的皮肤上未观察到毒性效果或损伤。在任意研究点,NO2未超过由职业安全与卫生管理局设置的安全限度(<4.3±0.3ppm)。图10显示与图9相似的数据,但其中动物创伤暴露于400ppm的gNO中进行治疗。对照组和治疗组比较,发现细菌含量平均下降超过10倍(p<0.05)。
图11显示了氮氧化物的水平(NO2和NO3),氧化氮代谢终产物之一,其由检测采集自间歇地暴露于200ppm gNO中6天后的动物的血清得到。没有样品由于暴露于gNO中而导致NOx水平增加,这表明暴露全层创伤(8个直径为8.0mm)不会增加动物循环系统的氧化氮水平。
图12为6天间断暴露于200ppm gNO中之后动物血中高铁血红蛋白(MetHb)水平。与暴露于空气的对照组相比,治疗组动物未显示MetHB水平增加。这进一步支持了图11所示的数据,表明在开放的创伤上局部使用gNO不会造成循环中氧化氮水平增加,并且在开放的创伤局部使用约200ppm在高铁血红蛋白形成方面没有明显的毒性。
图13表示由治疗组和对照组动物创伤打孔活组织样品制备的组织块的组织学分析。对照组样品显示更严重的中性粒细胞浸润以及更高程度的炎症反应。在用gNO治疗的创伤中可见较低水平的中性粒细胞浓度。用gNO治疗的创伤还显示出一层封闭在创伤上的结痂,但对照组创伤仍然长时间开放。总体的,在用gNO治疗的创伤中观察到较健康的愈合进程。在gNO治疗组中未看到毒性作用(由细胞凋亡产生的细胞碎片)。
由于炎症应答对创伤愈合是必需的,异常的炎症应答被认为是慢性创伤和过量渗出物的一个起因。NO抑制血小板聚集,帮助维持血管紧张度并抑制肥大细胞脱粒作用。Delledonne M,等人,(2003)Thefunctions of nitric oxide-mediated signaling and changes in geneexpression during the hypersensitive response,Antioxid Redox Signal,5:33-41和Hickey MJ.,(2001),Role of inducible nitric oxide synthase inthe regulation of leukocyte recruitment,Clin Sci(Lond),100:1-12。
本质上由内皮细胞产生的NO已显示具有进行性抗炎作用。同前。这可能部分地由于其对血小板聚集的作用。在炎症应答期间iNOS被正向调节。研究表明衍生自iNOS的NO也具有抗炎特点。同上。综上,维持血管紧张度、促进血管发生、缓解炎症和抑制肥大细胞脱颗粒作用,NO可视为一种用于渗出物处理的重要分子。相应地,外源性地应用的氧化氮可加倍和增强内源性氧化氮减少局部炎症应答的作用,以及向下调节全身性炎症应答系统接收到的信息,以增加炎症细胞的传送。这最后可使健康水平的渗出物产生。
图14显示了当暴露在高浓度gNO(200ppm)中的时间增加时,胶原酶mRNA的表达增加。这提示高浓度的氧化氮正向调节可引起胶原的酶性裂解的胶原酶。Witte等人(2002)的独立研究发现MMP-2活性还被NO供体正向调节。Witte MB,等人,(2002)Nitric oxide enhancesinvestigational wound healing in diabetes,Br J Surg.,89:1594-601。因此,申请人相信,NO可以正向调节胶原酶(MMP-I)和明胶酶(MMP-2)的表达,这在保持创伤清洁以免其成为坏死组织而不延长炎症阶段方面可能是重要的。
比将外源性胶原酶应用于坏死组织的酶清创术更好的是,将有坏死组织的创伤暴露于外源性NO气体中以正向调节内源性胶原酶可能更有益。当内源性胶原酶由细胞释放时,它自动地释放TIMP′s(金属蛋白酶(metalloproteinase)组织抑制剂)。这确保了基质降解是协调的,使溶胶原活性产生明显的地理界线,并保护结缔组织区域免受酶活化,相反,使用外源性胶原酶物质以清除创伤对创伤的特定区域无保护,原因是它作用于与其接触的所有细胞,而不论是否需要该作用。氧化氮清除创伤的能力进一步被应用于创伤的高浓度外源性氧化氮可能抑制胶原表达支持,如图14(左面)所示。
优选的,在创伤暴露于高浓度氧化氮气体中的第一治疗期(例如,每天5-8小时)后,坏死组织可以被容易地机械性除去,第二治疗期的氧化氮气体浓度可以减小。递送的用于第二治疗期的低浓度氧化氮气体(例如,5-20ppm)可以正向调节引发新胶原合成以帮助创伤闭合的胶原mRNA的表达。例如,图22表示成纤维细胞暴露于5ppmNO中胶原mRNA表达增加。第二治疗期可以是每天7-16小时。进一步的,用高和低浓度的氧化氮气体的治疗可以反复数天。
对于皮肤上的慢性非愈合性溃疡,在创伤制成后,也可以将天然皮肤组织或合成皮肤组织移植在溃疡上。创伤床制备可包括减少微生物量、清创术和渗出物的处置。
人们认为身体对损伤的自然应答是增加氧化氮的量,以减小损伤部位的细菌量,帮助除去死亡细胞及促进愈合。由损伤部位发送的信息多于损伤部位的细胞产生的氧化氮,这使NO在血液中环绕身体循环。在为愈合准备数天之后,身体减少了氧化氮,其产生新的促进愈合的水平。如果创伤未能愈合或被感染,身体维持高水平的循环氧化氮,并且创伤遇到可防止其愈合的氧化氮浓度。它使创伤愈合处于不可能的状况(“Catch 22”)。将损伤部位沐浴于高浓度的氧化氮气体(例如,120ppm~400ppm)中会向身体发送在损伤部位有充足氧化氮的信息,因此身体可停止其它细胞的额外生产。当接收适当的超生理浓度的氧化氮气体以抑制微生物生长时,能够使局部位置愈合。
附加的安全性研究
除了以上表明对动物模型的开放创伤在体暴露200ppm的氧化氮气体无毒性的研究之外,对成纤维细胞、内皮细胞、角质化细胞、肺泡上皮细胞、巨噬细胞和单核细胞中进行了证实暴露于gNO中的正常宿主细胞成活力的研究,在一些研究中,在平皿和3-D生长模型中进行。这些试验着眼于适当模型的成活力、增殖、迁移(migration)、附着、表达和管形成(tube formation)。
将由进行选择性整形手术的成年患者获得的成纤维细胞在Dulbeco′s改良伊格尔培养基(Dulbeco′s Modified Eagle′s Medium,DMEM)中培养,其中添加了10%胎牛血清(FBS)和抗生素-抗霉菌制品,并分到10个25cm2的通风培养瓶(COSTAR)中。其中4个瓶(治疗组)在特制的NO培养室、在37℃下暴露于20或200ppm加湿的gNO中24和48小时。在研究之前NO暴露室是经过验证的,以清除外界的可变因素并保证为成纤维细胞生长提供最佳条件。另外4个瓶(对照组)置于常规培养箱中,并仅在37℃暴露于周围加湿空气中。两个瓶分别被收集并计数细胞数,作为零时间的细胞数。治疗后,收集成纤维细胞并评价形态、细胞计数、增殖能力和培养基pH。这些试验结果显示暴露于约200ppm的gNO中对成纤维细胞没有有害作用。
图15显示了生存力研究的成纤维细胞形态,其中培养的人成纤维细胞连续暴露于低于200ppm的各种gNO浓度中48小时。48小时后,对照的和治疗的皮肤成纤维细胞的形态外观和附着能力是类似的(comparable)。gNO下的细胞表现健康并附着于培养皿上。未观察到因暴露于gNO中的毒性作用。
图16显示,除了对成纤维细胞无毒性之外,暴露于200ppm NO中还可能有增加成纤维细胞增殖的积极作用,其可进一步帮助创伤愈合过程。
图17显示了成纤维细胞暴露于160ppm gNO中的细胞附着能力结果。在特定时限内细胞再附着于培养皿的能力通常用作细胞在培养基中的存活力的指标。在培养1小时内对照组和治疗组均显示出70%的附着能力。该结果与细胞形态与计数一起支持了在至少100~200ppmgNO的范围内,局部用于哺乳动物皮肤组织的gNO治疗的安全性。
图18显示了生长于3D基质中,并暴露于200ppm NO中3天(每天8小时)的成纤维细胞的迁移量,其与在空气或常规培养箱中的对照细胞相比。从这些结果可见,在这些条件下,未见NO影响(或者更具体地,无干扰)这些成纤维细胞的迁移。
图19显示了生长于3D基质中,并暴露于200ppm NO中3天(每天8小时)的成纤维细胞的增殖量,其与在空气或常规培养箱中的对照细胞相比。再一次的,在这些条件下,未见NO干扰成纤维细胞增殖。
图20显示了生长于基质胶中,并暴露于空气(上面的图)或200ppmNO(下面图)中8小时(左图)或24小时(右图)的人内皮细胞的管形成。再一次的,在暴露于空气和200ppm之间无显著差异。
人体病例研究
本案例研究涉及一位55岁老人,其患有30年的深部和浅表的、与深静脉血栓性静脉炎相关的严重静脉疾病。起初,当患者在20多岁时发生两侧非愈合性静脉小腿溃疡,患者被手术治疗。手术部位愈合,但溃疡继续复发。开始时,患者出现正好在左髁的内踝以下的小溃疡。尽管大小未增加,该溃疡用2年的监护标准治疗而未完全愈合。
在大多数时间,创伤基层被生物膜-粘性的、黄色的、凝胶样物质覆盖。通过使用有刻度的压缩长袜(graduated compressionstockings)维持水肿控制。尝试的抗菌敷料包括Manuka Honey,一种淀粉碘制品(lodosorb,Smith & Nephew,Largo,FL,USA),和胶体银(Aquacel AG,ConvaTec,Princeton,NJ,USA)。为了完全物理地除去生物膜,将患者的创伤经常被清创。由于生物膜经常是在下次就诊是明显地再次存在,一般地说这是无效的。为了引起肉芽组织形成,每隔数天应用20%过氧化苯甲酰洗液;然而,这也是无效的。有时候随着溃疡被新皮肤覆盖而有所改善,但仅几周后就发病。尽管给予适当湿平衡、创伤床制备和潜在疾病的治疗的创伤护理,这种弱的完全闭合的进展是明显的。
创伤闭合的失败对该患者的生活质量有明显的影响。患者整整2年每月就诊至少一次。治疗费用,包括外科医生的时间和治疗材料(数千美元),给卫生保健体系和患者造成了压力,他每次出诊治疗需要数小时。由于以前的治疗被证明是无效的,患者被邀请参与本试验研究。在对试验性治疗和潜在风险作了讨论后,得到知情同意书。
患者到诊所就诊,在此对创伤作了评估并拍照(图22)。对治疗方案作了解释,并对CidaNOx递送系统和靴子的使用作了演示。进行安装以符合患者的家庭,次日安装仪器并对患者反复培训使用该治疗系统。训练包括系统的使用以及使用该气体仪器的安全信息。
应用专门设计用于该研究(PulmoNOx Medical Inc.,Edmonton,Alberta,Canada)的气体稀释递送系统(CidaNOx递送系统),将氧化氮气体(ViaNOx-H,VIASYS Healthcare,Yorba Linda,California,USA)应用于下肢。该CidaNOx递送系统包括用于稀释gNO的内置气体泵和流量控制回路以将NO源气缸中的百万分之800(ppm)稀释降至200ppm的治疗水平。来自该系统中的总流量为1.0L/min并且包括每分钟1/4升(250ml/min)的gNO流。数个内置压力传感器确保稀释流量可操作的并监视该系统。通过没有可被患者改变的外部控制器的机械设置压力调节器和机械流量计,将氧化氮流量限制为250ml/min。递送的氧化氮的浓度是用计量氧化氮分析仪(AeroNOx,Pulmonox Medical Inc.)通过测量CidaNOx输出而确定的,该分析仪已批准用于监测人类患者吸入的NO。
来自CidaNOx递送系统的200ppm gNO流出到使用塑料靴子的单个患者,该塑料靴子覆盖患者的下肢。该靴子在顶部附近有一个可膨胀的袖口,其提供了一个低压密封。来自CidaNOx单元的第二空气出口维持着袖口的膨胀。患者将泵出口连接至袖口连接器,直到它膨胀,然后将该连接器用所提供的夹子密封闭合。然后将gNO流连接至靠近靴子趾部的入口连接器,回路线连接至靠近靴子顶部的连接器。回路线穿过CidaNOx单元,再通过包含吸收氧化氮类的炭和高锰酸钾的清除器出来。该CidaNOx递送系统有两个套环(toggle)位置,一个用于递送gNO,另一个仅用于递送空气。在治疗末期,患者改为仅递送空气,以便在脱下靴子之前清除靴子残留的gNO。
当不接受gNO治疗时,指导患者连续穿戴支持性长筒袜,并在创伤上使用水性纤维(hydrofiber)敷料(Aquacel,Convatec)。gNO治疗期间,脱去支持性长筒袜,并用一多孔的、低粘着性的、先前已显示允许gNO穿过它扩散(数据未显示)的敷料(ETE,Molnlycke Health Care,Sweden)代替Aquacel敷料。
为了由延长使用以探索创伤床制备和促进创伤愈合的潜力,选择了超过3天的治疗方案,并在14天中止该方案,评价短期作用,并探索短期作用改善长期结果的可能性。鼓励患者在每24小时期间尽量常穿戴gNO靴子。由于患者白天工作,确定仅在晚上睡眠时穿上靴子接受gNO治疗是最现实的。患者在数据表中记录日期、时间和各治疗期的持续时间,以及任何有意义的与创伤、治疗或仪器相关的观察结果。创伤大小(cm2)是使用数字摄像术和光密度法技术(Scion Image-4.02,Scion Corp.,Frederick MD,USA)测定的。
患者自我施用连续治疗达14个夜晚。每次夜间治疗持续6.5~9.75小时。在14个治疗期创伤累积暴露于200ppm gNO中的时间为105.25小时。分别在第0天(图22A,预治疗)、第3天(图22B,gNO暴露累积24小时后)和第14天(图22C)对创伤进行评价并照相。还在14天治疗后的第10天(图22D)对创伤进行了评价和拍照,并在完成治疗后第6周和第26周(分别为图22E和22F)对创伤进行了评价和拍照。
积极治疗期间,受试者就CidaNOx系统的使用进行了评价。受试者发现该系统容易在固定位置使用,发现袋子使用舒适。并且从未报道任何与其使用有关的疼痛。患者未经历出血情况。图23A显示了在使用gNO之前溃疡的初始图示。创伤基底被生物膜覆盖,有少量健康的肉芽组织出现,并且未显示新的皮肤从边缘生长。该创伤有恶臭。
在NO暴露24小时(3天,每天8小时)后,在溃疡基底中第一次有健康的肉芽组织。还观察到从边缘生长新皮肤的早期证据。也没有恶臭的气味。同时,存在较少的生物膜(图22B)。在治疗第14天(图22C),溃疡尺寸明显减小。此时上皮几乎完全形成。早在gNO治疗第3天即观察到创伤大小明显减小(p=0.014),在gNO治疗14天结束后创伤区域大约减小75%(图23)。在gNO治疗停止后再对创伤评价10天。尽管认为溃疡未完全愈合,但在此期间创伤未出现任何恶化。与gNO治疗的最后一天相比,可观察到创伤大小无明显的恶化(图23)。6周以后判断约90%愈合,创伤大小或上皮形成无恶化(图22E和图23)。在NO中断26周后,溃疡明显地完全愈合并且重新形成上皮(图22F)。在整个治疗后期,敷料用法未变化,并且未使用其它抗微生物剂或抗生素。
在最佳护理条件下愈合静脉阻塞性疾病(venous stasis disease)引起的溃疡的平均时间为12至16周。我们的患有2年以上的非敏感性溃疡的患者对短暂地暴露于气态的氧化氮表现出积极的应答。患者的创伤大小减小,生成颗粒状基底,并且在2周治疗期间恶臭味消失。一旦生物膜清除,进一步的研究和随机对照试验将能够回答更长久的暴露或不同的浓度是否会对损伤闭合有影响。
虽然已对本发明的实施方案作了说明和描述,在不脱离本发明的范围内可以作多种修饰。例如,用本文描述的方法治疗的具有创伤的组织种类可以包括,但不限于,皮肤、肌肉、腱、韧带、粘膜、骨、软骨、角膜和暴露的内脏器官。该组织可能因外科切口、创伤(机械的、化学的、病毒的、细菌的或自然热)或其它内源性病理过程而受到损伤。因此,本发明除了不应受到以下权利要求的限制外,还不应受到其同等物的限制。
Claims (9)
1.高浓度外源性氧化氮气体和降低的浓度的外源性氧化氮气体在制备药物中的用途,所述药物用于在对所述患者或所述创伤周围的健康细胞不诱发毒性的条件下治疗创伤;其中通过使用降低的浓度增加胶原mRNA的表达。
2.如权利要求1所述的用途,其中所述高浓度氧化氮气体大于200ppm。
3.如权利要求2所述的用途,其中所述高浓度氧化氮气体为200ppm~400ppm。
4.如权利要求1所述的用途,其中所述降低的浓度的氧化氮气体为5ppm~80ppm。
5.氧化氮气体在制备用于正向调节位于所治疗受试者的创伤部位局部的宿主细胞内源性胶原酶和内源性胶原表达的气态药物中的用途。
6.如权利要求5所述的用途,其中在较高浓度下所述药物增加所述细胞内源性胶原酶的表达而对所述细胞不诱发毒性,在较低浓度下所述药物增加所述细胞内源性胶原的表达。
7.如权利要求6的用途,其中所述较高浓度为200ppm~400ppm。
8.如权利要求6的用途,其中所述较低浓度为1ppm~80ppm。
9.氧化氮气体在制备用于创伤治疗的高浓度氧化氮气体药物和低浓度氧化氮气体药物中的用途,其中所述高浓度氧化氮气体帮助清创位于创伤处的坏死组织,所述低浓度氧化氮气体用于闭合所述创伤。
