操作模式

图6示出执行本发明方法的排气压力操作型气囊导管系统10,特别地关注排气管14的远端部分,其包括定位在血栓沉淀物或病理部分120部位处的血管118、动脉等中的柔性锥形末端42,那里,血管118和主血栓沉淀物或病理部分120显示在截面图中。多个盐水(或其它合适流体)的高速射流122的喷射流,沿近端方向从流体射流发射器106喷射出而撞击到血栓沉淀物或病理部分120,并带走已被减小为颗粒形式的血栓沉淀物或病理部分。或者,作为该图中所示流体射流发射器106的替代方案,不同结构的其它流体射流发射器也可包括在导管14的远端部分内,以便向近端沿着或靠近导管14的纵向轴线发射或射出一个或多个高速射流122,从而完成与如上描述的流体射流发射器106相同的目的。盐水的高速射流122向外通过流出孔20a-20n沿径向方向流过,形成朝向血管118壁的向外的横向流124,并在流入孔18a-18n处受低压的影响而造成横向流124,以便向远端和圆周地流动而发生撞击、提供曳力和粉碎血栓沉淀物或病理部分120,用夹带方式推动和通过流入孔18a-18n(相对低的压力区域)带走血栓沉淀物或病理部分120的颗粒,流入到高速射流122内,那里,血栓120进一步分离为微小颗粒,由此流入导管腔40内而通过膨胀的气囊16a,并进一步通过内腔40以作其后的排气。流出的排气流被由高速射流122和通过流入孔18a-18n的夹带流体产生的内压所驱动,致使内腔40和气囊16加压,该流出的排气流用作为好几个优点。其一个优点在于,当血的远端由于干预的充气和膨胀的气囊16a的充气而停止流动时,其处于没有流动的情况下,血栓沉淀物或病理部分120的颗粒基本上被滞留,因此可以更加有效地循环、再循环和再稀释,直到所有剩余物是盐水和血栓沉淀物或病理部分120的细小颗粒,其后,通过促使流动通过排气调节器114,沿着近端方向通过导管14的内腔40除去它们。另一优点是使用排气流出流和由高速射流122形成的内压,组合了诸如受排气调节器114影响的对流出流的约束,而造成气囊16自动地膨胀,气囊16强制地撞击和密封抵靠在血管118的内壁上。减小的包括气囊16在内的材料厚度允许气囊16充分地膨胀而变为膨胀的气囊16a,其受到与血管118壁碰撞的限制。可通过在高压流体泵112处控制输入压力的流体和控制排气调节器114处的排气率,来影响充气压力和流动。本发明揭示了排气压力操作型气囊导管系统10,其利用连续形成的充气和膨胀的由与导管(排气管)相同的材料形成的气囊的原理,通过高速射流、横向流等造成的内部加压来自动地进行充气。这样的概念也可应用于其它的血栓切除术导管和系统,诸如,但不局限于,包括快速互换的导管、通过导向丝的导管,以及由流体流源进行加压的导管在内的所有AngioJet

Claims (7)

1.一种用于血管中的血栓切除术的排气压力操作型气囊导管系统,包括:
a.一件式歧管,所述一件式歧管包括:
(1)中心管状体;
(2)高压支管,所述高压支管包括一体的螺纹连接器端口,且所述高压支管从所述中心管状体斜向地延伸;
(3)排气支管,所述排气支管包括一体的螺纹连接器端口,且所述排气支管从所述高压支管斜向地延伸;以及
(4)腔体,所述腔体具有止血螺母组件,且所述腔体从所述中心管状体向近端延伸;
b.由第一材料形成的导管,所述导管包括排气内腔,所述导管从所述歧管向远端延伸通过应变释放结构并朝向位于远端的锥形末端;
c.多个在所述导管上与所述排气内腔连通的、紧靠所述锥形末端的流出孔和流入孔;
d.高压管,所述高压管从所述高压支管连接地延伸通过所述歧管和通过所述导管,延伸到位于所述多个流出孔和流入孔远端的流体射流发射器,流体在高压下从所述歧管朝向远端载送到所述流体射流发射器;
e.由所述导管的所述第一材料形成的柔性、可膨胀和可充气的薄壁部分的气囊,所述气囊与壁厚大于所述气囊壁厚的所述导管形成一体,并连接在所述导管的两个部分之间且与这两个部分对准,且在所述多个流出孔和流入孔的近端处,其中所述多个流出孔和流入孔位于所述流体射流发射器和所述薄壁部分的气囊之间,并且多个流出孔和流入孔通过所述排气内腔与所述薄壁部分的气囊和所述流体射流发射器连通;以及
f.所述导管的所述排气内腔与所述薄壁部分的气囊连通,所述导管的所述排气内腔和所述薄壁部分的气囊由来自所述流体射流发射器的朝向近端的排气流和通过所述多个流出孔流出并以夹带流形式从所述流入孔流入的流体可维持地加压,且所述朝向近端的排气流将所述薄壁部分的气囊充气到膨胀状态。
2.如权利要求1所述的排气压力操作型气囊导管系统,其特征在于,还包括位于所述薄壁部分的气囊远端和近端的相对的辐射透不过的标记带。
3.如权利要求2所述的排气压力操作型气囊导管系统,其特征在于,还包括与所述薄壁部分的气囊相邻的支承环。
4.如权利要求1所述的排气压力操作型气囊导管系统,其特征在于,所述止血螺母组件卡合地和螺纹地配合所述腔体。
5.如权利要求4所述的排气压力操作型气囊导管系统,其特征在于,所述腔体包括所述腔体上向内突出的凹坑,用于与所述止血螺母组件上的环形唇边配合。
6.如权利要求1所述的排气压力操作型气囊导管系统,其特征在于,所述薄壁部分的气囊可围绕所述导管远端区域展开和膨胀,以在与所述排气压力操作型气囊导管系统连接的排气压力装置操作时起作闭塞装置。
7.如权利要求1所述的排气压力操作型气囊导管系统,其特征在于,所述流体射流发射器和所述流入孔和所述流出孔限定了由所述流体射流发射器的流体产生的切除用横向流的位置,其中,所述薄壁部分的气囊位于所述切除用横向流的部位的近端。
