Abstract

技术领域
本发明涉及一种用于制备含热稳定氧载体的药物组合物的方法和由所述方法制备的组合物。本发明还涉及所述含热稳定氧载体的药物组合物在人和其他动物的癌症治疗、缺氧病症(oxygen-deprivation disorder)和器官保存中的用途。
背景技术
血红蛋白在大多数脊椎动物中对于血管系统和组织之间的气体交换具有重要作用。其负责经由血液循环将氧从呼吸系统运送至机体细胞,并且还将代谢废物二氧化碳从机体细胞运离至呼吸系统,在呼吸系统二氧化碳被呼出。由于血红蛋白具有此氧运输特性,因此如果其在离体(ex vivo)状态下可以被稳定并且可以在体内(in vivo)使用,则其可以被用作有效的供氧器。
天然存在的血红蛋白是四聚体,当其存在于红细胞内时通常是稳定的。然而,当天然存在的血红蛋白移出红细胞时,其在血浆中变得不稳定,并且分裂成两个α-β二聚体。这些二聚体的每一个的分子量为约32kDa。这些二聚体当通过肾脏过滤和排泄时可能导致显著的肾损伤。四聚体键的断裂还负面地影响了功能性血红蛋白在循环系统中存在的持久性。
为了解决这个问题,近年来在血红蛋白加工方面的发展已经结合了多种交联技术来形成四聚体内的分子内键以及四聚体之间的分子间键,从而形成聚合血红蛋白。现有技术教导聚合血红蛋白是增加血红蛋白的循环半衰期的优选形式。然而,本发明人确定,聚合血红蛋白在血液循环中更容易转变成为高铁血红蛋白。高铁血红蛋白不能结合氧因此不能氧合组织。因此,现有技术教导的导致形成聚合血红蛋白的交联作用是有问题的。在本领域中存在着对允许分子内交联从而形成稳定的四聚体而不同时形成聚合血红蛋白的技术的需求。
现有技术为稳定血红蛋白进行的尝试所存在的更多问题包括:产生的四聚体血红蛋白中包含无法接受的高百分比的二聚体单位;二聚体的存在使得血红蛋白组合物不能令人满意地施用于哺乳动物。血红蛋白的二聚体形式可以导致哺乳动物身体的严重肾损伤;此肾损伤可以严重到足以导致死亡。因此,本领域中存在着对在终产物中形成具有检测不到的二聚体形式的稳定的四聚体血红蛋白的需求。
现有技术血红蛋白产品存在的另一问题是给药后血压的急剧增加。在过去,从上一代基于血红蛋白的氧载体已经记录到血管收缩事件。例如,产品(Biopure Co.,USA)导致与基线(96±10mmHg)相比较高的平均动脉压(124±9mmHg)或高30%,如由Katz等人,2010公开。现有技术解决此问题的尝试依赖于巯基试剂与血红蛋白巯基反应,据称以防止内皮细胞舒血管因子与该巯基的结合。然而,巯基处理的使用增加了加工步骤,导致增加的成本和杂质,所述杂质以后必须从血红蛋白组合物中去除。因此,在本领域中存在着对制备当应用于哺乳动物时不引起血管收缩和高血压的血红蛋白的方法的需求。
现有技术为形成稳定的血红蛋白进行的尝试所存在的更多问题包括:存在蛋白杂质诸如免疫球蛋白G,其可以引起哺乳动物中的变态反应效应。因此,本领域中存在着对可以产生稳定的不含蛋白杂质的四聚体血红蛋白的方法的需求。
除了上述问题以外,在本领域中还存在着对稳定化的四聚体血红蛋白的需求,所述四聚体血红蛋白不含二聚体,不含磷脂并且能够以工业规模生产。
发明内容
本发明提供了一种加工非聚合的、热稳定的、纯化的、交联的四聚体血红蛋白的方法,所述四聚体血红蛋白适合用于哺乳动物,而不会引起严重肾损伤、血管有害作用和严重不良事件包括死亡。本发明去除了二聚体形式的血红蛋白、未交联的四聚体血红蛋白、磷脂和蛋白杂质。另外地,本发明使用(1)用于精确的和受控的低渗裂解的即时细胞裂解设备,(2)流通柱色谱(flowthrough columnchromatography),(3)高温短时(HTST)设备,其用于在纯化过程中热加工血红蛋白溶液,以去除不合乎需要的未稳定化的血红蛋白二聚体,并且去除蛋白杂质,例如免疫球蛋白-G,使得可以避免肾损伤、血管有害作用和其他毒性反应,和(4)气密输液袋包装以避免氧侵入至产品中。

使用超滤滤器进行第一超滤过程,所述超滤滤器被配置成从第一血红蛋白溶液中去除分子量比四聚体血红蛋白高的杂质并且进一步去除任何病毒和残留的废渗余物,从而获得第二血红蛋白溶液。对所述第二血红蛋白溶液进行流通柱色谱以去除蛋白杂质、二聚体血红蛋白和磷脂以形成无磷脂的血红蛋白溶液。使用配置成去除杂质的滤器对所述无磷脂的血红蛋白溶液进行第二超滤过程,产生浓缩的纯化的无磷脂的血红蛋白溶液。
通过富马酸二-3,5-二溴水杨酸酯交联纯化的血红蛋白的至少α-α亚基,从而形成热稳定的交联血红蛋白而不形成聚合血红蛋白,使得得到的非聚合交联四聚体血红蛋白的分子量为60-70kDa。表述“非聚合”当在本文中使用时是指不与其他血红蛋白分子或任何其他非血红蛋白分子诸如PEG分子间交联的四聚体血红蛋白。用合适的生理缓冲液诸如磷酸盐缓冲盐水(PBS)、乳酸盐林格氏溶液、乙酸盐林格氏溶液或Tris缓冲液交换所述交联四聚体血红蛋白。使用切向流过滤去除任何残留的化学物质。
在此步骤后,交联血红蛋白被热处理以去除任何残留的未交联的四聚体血红蛋白和任何未稳定化的血红蛋白(例如,二聚体形式的血红蛋白)和任何其他蛋白杂质。在热处理之前,任选地向所述交联四聚体血红蛋白加入约0.2%的浓度的N-乙酰半胱氨酸以防止高铁血红蛋白的形成。在热处理和冷却后立即加入约0.2%-0.4%的浓度的N-乙酰半胱氨酸以进一步防止高铁血红蛋白的形成。热处理优选是在约70℃-95℃下进行30秒至3小时的高温短时处理,随后冷却至25℃。之后在热处理期间形成的任何沉淀物均通过离心或过滤设备去除以形成清亮的溶液。
不含二聚体、不含磷脂、不含蛋白杂质的、热稳定的、非聚合的交联四聚体血红蛋白然后加入至药学上可接受的载体。
之后,将热稳定的交联四聚体血红蛋白在定制的和气密的聚乙烯、乙烯-乙酸乙烯酯、乙烯-乙烯醇(PE、EVA、EVOH)输液袋中配制和包装。该包装防止氧污染,氧污染导致形成无活性的高铁血红蛋白。
通过上述方法制备的热稳定的交联四聚体血红蛋白用于治疗多种癌症诸如白血病、结直肠癌、肺癌、乳腺癌、肝癌、鼻咽癌和食管癌。破坏癌细胞的机制是改善低氧条件下肿瘤的氧合,从而提高对放射线和化疗剂的敏感性。该热稳定的交联四聚体血红蛋白也用于在移植期间保护器官组织或用于在体内缺少供氧的情况下(诸如在缺氧心脏中)保护心脏。
此外,通过上述方法制备的热稳定的交联四聚体血红蛋白用于减少癌性肿瘤复发和使肿瘤细胞转移最小化。在用于肿瘤切除手术的缺血前和在去除肿瘤后重建血供(再灌注)期间施用所述血红蛋白。所述血红蛋白还可以用于增加癌组织的氧合和减小肿瘤的大小。

具体实施方式
血红蛋白是哺乳动物和其他动物的血液的红细胞中含铁的氧运输蛋白。血红蛋白显示蛋白的三级和四级结构两者的特性。血红蛋白中的大多数氨基酸形成由短的非螺旋段连接的α螺旋。氢键稳定血红蛋白内部的螺旋段,产生其分子内的吸引力,将每条多肽链折叠成特定的形状。血红蛋白分子由四个球状蛋白亚基组装而成。每个亚基由排列成一组α-螺旋结构段的多肽链构成,所述α-螺旋结构段以“肌红蛋白折叠”排列方式与包埋的血红素基团连接。
血红素基团由固定在称为卟啉的杂环中的铁原子组成。铁原子与位于一个平面中的环中心中的所有4个氮原子同等地结合。然后氧能够结合于与卟啉环的平面垂直的铁中心。因此,单个血红蛋白分子具有与四个氧分子结合的能力。
在成人中,最常见类型的血红蛋白是称为血红蛋白A的四聚体,其由称为α2β2的两个α和两个β非共价结合的亚基组成,每个亚基分别由141和146个氨基酸残基构成。α和β亚基的尺寸和结构彼此非常类似。对于总分子量为约65kDa的四聚体,每个亚基的分子量为约16kDa。四条多肽链通过盐桥、氢键和疏水相互作用彼此结合。牛血红蛋白的结构与人血红蛋白类似(α链中的同一性为90.14%;β链中的同一性为84.35%)。不同之处在于牛血红蛋白中的两个巯基位于βCys93处,而人血红蛋白中的巯基分别位于αCys104、βCys93和βCys112处。
在红细胞内部的天然存在的血红蛋白中,α链与其相应的β链的缔合非常强并且在生理条件下不会离解。然而,在红细胞外,一个αβ二聚体与另一个αβ二聚体的缔合相当弱。该结合具有分裂成两个αβ二聚体的倾向,每个二聚体为约32kDa。这些不需要的二聚体足够小从而被肾脏过滤和排泄,结果造成潜在的肾损伤和并导致显著减少的血管内滞留时间。
因此,从功效和安全性两方面来看,稳定在红细胞外使用的任何血红蛋白是必要的。下面概述了制备稳定的血红蛋白的方法;本发明的方法的概况呈现在图1的流程图中。
首先,选择全血来源作为来自红细胞的血红蛋白的来源。选择哺乳动物全血,包括,但不限于,人、牛、猪、马和犬全血。将红细胞与血浆分离、过滤、和洗涤以去除血浆蛋白杂质。
为了从红细胞释放血红蛋白,需裂解细胞膜。尽管多种技术可以用来裂解红细胞,但本发明利用可以以适合于工业规模生产的体积以精确受控的方式在低渗条件下的裂解。为此,图2中所示的即时细胞裂解设备用来裂解红细胞。低渗裂解形成溶胞产物溶液,其包含血红蛋白和废渗余物。为了能够进行工业规模生产,小心地控制裂解作用使得仅红细胞被裂解却不裂解白细胞或其他细胞。在一个实施方案中,选择即时细胞裂解设备的尺寸使得红细胞在2-30秒内或另外地在足以裂解红细胞的时间内(优选30秒内)穿越该设备。所述即时细胞裂解设备包括静态混合器。去离子的和蒸馏的水用作低渗溶液。当然要理解使用具有不同的盐浓度的其他低渗溶液将导致红细胞裂解的时间段不同。由于受控的裂解步骤仅裂解红细胞,不裂解白细胞或细胞性物质,因此其使毒性蛋白、磷脂或DNA从白细胞和其他细胞性物质的释放最小化。在30秒后,即在含有红细胞的溶液已经穿越即时细胞裂解设备的静态混合器部分后,立即加入高渗溶液。得到的血红蛋白与使用其他裂解技术得到的血红蛋白相比具有较高的纯度和较低水平的污染物诸如不合乎需要的DNA和磷脂。分别通过聚合酶链式反应(检出限=64pg)和高效液相色谱(HPLC,检出限=1μg/mL)方法没有在该血红蛋白溶液中检测到来自白细胞的不合乎需要的核酸和磷脂杂质。
进行两个超滤过程:一个过程在流通柱色谱前去除分子量比血红蛋白大的杂质,另一个过程在流通柱色谱后去除分子量比血红蛋白小的杂质。后一超滤过程浓缩血红蛋白。在一些实施方案中,100kDa滤器用于第一超滤过程,而30kDa滤器用于第二超滤过程。
流通柱色谱用来去除纯化血红蛋白溶液中的蛋白杂质诸如免疫球蛋白-G、白蛋白和碳酸酐酶。在一些实施方案中,通过使用一种市售离子交换柱或其组合来进行柱色谱,所述离子交换柱诸如DEAE柱、CM柱、羟基磷灰石柱等。用于柱色谱的典型pH为6-8.5。在一个实施方案中,使用流通CM柱色谱步骤在pH8.0去除蛋白杂质。进行酶联免疫吸附测定(ELISA)以检测从柱色谱洗脱后的样品中残留的蛋白杂质和磷脂。此独特的流通柱色谱分离能够实现连续的分离方案以进行工业规模生产。ELISA结果显示在洗脱的血红蛋白中这些杂质的量非常低(免疫球蛋白-G:44.3ng/mL;白蛋白:20.37ng/mL;碳酸酐酶:81.2μg/mL)。使用不同类型的柱采用不同的pH值去除蛋白杂质的结果显示在下面的表1中。

在柱色谱过程后,血红蛋白通过富马酸二-3,5-二溴水杨酸酯(DBSF)进行交联。为了防止聚合血红蛋白的形成,在脱氧环境(优选小于0.1ppm溶解氧水平)中小心地控制反应,血红蛋白与DBSF的摩尔比为1∶2.5-1∶4.0,反应在环境温度(15-25℃)下,优选在约8-9的pH下持续3-16小时的时间段,使得得到的交联血红蛋白是分子量为60-70kDa的四聚体血红蛋白,证明不存在聚合血红蛋白。DBSF反应的收率高,>99%且终产物中的二聚体浓度低。任选地,本发明的方法不需要如在各种现有技术方法中使用的巯基处理试剂诸如碘乙酰胺来在交联之前与血红蛋白反应。
此时,用磷酸盐缓冲盐水(PBS)——一种生理缓冲液——交换交联溶液,并且通过切向流过滤去除任何残留的化学物质。
在脱氧条件下通过DBSF交联血红蛋白的过程后,本发明提供了在脱氧环境中对交联四聚体血红蛋白溶液的加热加工步骤。在热处理之前,任选地加入N-乙酰半胱氨酸以防止高铁血红蛋白(无活性血红蛋白)的形成。在该加热加工步骤后,将溶液冷却并立即加入N-乙酰半胱氨酸以保持低水平的高铁血红蛋白。如果在热处理之前和之后均加入N-乙酰半胱氨酸,则热处理前加入的量为约0.2%,而在热处理后加入的量为约0.2-0.4%。然而,如果仅在热处理后加入N-乙酰半胱氨酸,则加入的量为0.4%。
在—些实施方案中,将交联四聚体血红蛋白溶液在脱氧环境(小于0.1ppm溶解氧水平)中在50℃至95℃的温度范围内加热0.5分钟至10小时的持续时间。在一些实施方案中,将交联四聚体血红蛋白溶液在70℃至95℃的温度范围内加热30秒至3小时的持续时间。在一些优选的实施方案中,将交联四聚体血红蛋白溶液在80℃加热30分钟。并且还在其他优选的实施方案中,将交联血红蛋白溶液加热至90℃持续30秒至3分钟,然后快速地在约15-30秒内冷却至约25℃,并且如上所述加入N-乙酰半胱氨酸。产生非常低的量的高铁血红蛋白,例如,小于3%。不使用N-乙酰半胱氨酸,形成的高铁血红蛋白的量为约16%,对于制药应用来说是不可接受的高百分比。
之后使用高效液相色谱(HPLC)、电喷雾电离质谱(ESI-MS)、圆二色性(CD)光谱仪和用于p50测量的血氧分析仪(Hemox Analyzer)来分析和表征热稳定的交联四聚体血红蛋白。对于源自牛血来源的血红蛋白,图3显示二聚体形式的血红蛋白在用于血红蛋白(其已经在90℃经历45秒至2分钟或在80℃经历30分钟的热处理)的HPLC系统(检出限:2.6μg/ml或0.043%)中是检测不到的。交联的非聚合四聚体血红蛋白发现在80℃或90℃在一段时间内是热稳定的。加热加工(高温短时,HTST)步骤是使天然的未反应的四聚体形式和二聚体形式的血红蛋白变性的有力步骤。
为了分析此HTST步骤的结果,使用HPLC分析法来检测此加热加工步骤后二聚体的量。用于HPLC分析的流动相含有氯化镁(0.75M),其可以分离二聚体(未被稳定的四聚体)和热稳定的交联四聚体血红蛋白。对于促进血红蛋白解离成二聚体,在相同的离子强度下氯化镁的效力是氯化钠的约30倍。该加热加工步骤还充当变性步骤以大幅度地去除交联的四聚体血红蛋白中那些不需要的蛋白杂质(检测不到免疫球蛋白-G;检测不到白蛋白;碳酸酐酶减少99.99%)。进行酶联免疫吸附测定(ELISA)以检测样品中的蛋白杂质。因此,纯化的热稳定的交联四聚体血红蛋白溶液具有检测不到的水平的二聚体(低于检出限:0.043%)和免疫球蛋白-G,以及非常低的量的白蛋白(0.02μg/ml)和碳酸酐酶(0.014μg/ml)。表2显示了关于通过HTST加热加工步骤去除蛋白杂质和二聚体的实验结果。此HTST加热步骤能够选择性地将热稳定的交联四聚体与不稳定的四聚体和二聚体分离。

在脱氧条件下对交联血红蛋白的加热加工步骤后,热稳定的交联四聚体血红蛋白就可以进行药物配制和包装。本发明描述了在脱氧环境中热稳定的交联四聚体血红蛋白溶液的气密包装步骤。在本发明中热稳定的交联四聚体血红蛋白在脱氧条件下稳定达多于两年的时间。
在本发明中,含氧载体的药物组合物主要意欲用于静脉内注射应用。传统上,现有技术产品使用常规的PVC血袋或Stericon血袋,其具有高氧通透性,将最终缩短产品的寿命,因为产品在氧合条件下会快速地(在几天内)转变成无活性的高铁血红蛋白。
本发明中使用的包装使热稳定的交联四聚体血红蛋白稳定超过两年。EVA/EVOH材料的多层包装用来使气体通透性最小化并且避免形成无活性的高铁血红蛋白。被设计来与本发明的纯化的和热稳定的交联四聚体血红蛋白一起使用的100mL输液袋由厚度为0.4mm的5层EVA/EVOH层压材料制成,其氧通透性为在室温下每个大气压每24小时每100平方英寸0.006-0.132cm3。此材料是VI类塑料(由USP<88>定义),其满足体内生物学反应性测试和物理化学测试并且适合于制造静脉注射用输液袋。此初级袋可特别地用于保护热稳定的交联四聚体血红蛋白溶液免于长期暴露氧,长期暴露氧导致其不稳定并且最终影响其治疗性能。
对于血液产品的二次保护,已经知道使用铝外包装以防止潜在的漏气并且将产品保持在脱氧状态。然而,在铝外包装中有存在针孔的可能性,使其气密性受损并且使产品变得不稳定。因此,本发明使用铝外包装袋作为二次包装,其防止氧合并且还防止暴露于光。外包装袋的组成包括0.012mm聚对苯二甲酸乙二醇酯(PET)、0.007mm铝(Al)、0.015mm尼龙(NY)和0.1mm聚乙烯(PE)。外包装薄膜的厚度为0.14mm且其氧传递速率为在室温下0.006cm3/100平方英寸/24小时/大气压。此二次包装延长了血红蛋白的稳定时间,从而延长了产品保存期限。
本发明的血红蛋白通过多种技术被分析,包括ESI-MS。ESI-MS允许分析非常大的分子。其是一种电离技术,该技术通过电离蛋白质,然后基于质量/电荷比分离电离的蛋白质来分析高分子量化合物。因此,可以准确地确定分子量和蛋白相互作用。在图4中,ES1-MS分析结果表明热稳定的交联四聚体血红蛋白的尺寸为65kDa(非聚合血红蛋白四聚体)。190-240nm的远紫外CD谱揭示了血红蛋白的珠蛋白部分的二级结构。在图5中,纯化的血红蛋白溶液和热稳定的交联四聚体血红蛋白的光谱一致性揭示血红蛋白链即使在90℃热处理后也被正确地折叠。CD结果显示热稳定的交联四聚体血红蛋白具有大约42%的α-螺旋、38%的β-折叠、2.5%的β-转角和16%的无规卷曲。其进一步确定交联的四聚体血红蛋白是热稳定的。
本发明的方法适用于热稳定的交联四聚体血红蛋白的大规模工业生产。另外,热稳定的交联四聚体血红蛋白与药用载体(例如,水、生理缓冲液、以胶囊形式)的组合适合于哺乳动物应用。
本发明还公开了含氧载体的药物组合物在改善组织氧合中、在癌症治疗中、在治疗缺氧病症诸如出血性休克中和在低氧含量环境下的心脏保存(例如,心脏移植物)中的用途。选择剂量使其具有约0.2-1.3g/kg的浓度范围,输液速度小于10ml/小时/kg体重。
对于在癌症治疗中的用途,本发明的含氧载体的药物组合物作为组织氧合剂起作用,以改善肿瘤组织中的氧合,从而提高化学敏感性和放射敏感性。
另外,在本发明中证明了热稳定的交联四聚体血红蛋白改善正常组织中(图6)和极度低氧的肿瘤(图7)(人鼻咽癌(使用CNE2细胞系))中的氧合的能力。沿人CNE2异种移植物的组织轨迹的代表性氧特征曲线显示在图7中。肿瘤块内的氧分压(pO2)通过与微定位系统(DTI Limited)耦联的光纤氧传感器(OxfordOptronix Limited)直接监测。静脉内注射0.2g/kg热稳定的交联四聚体血红蛋白后,中位数pO2值在15分钟内从基线升高至相对平均氧分压的约2倍并且延续到6小时。此外,在输注后24至48小时平均氧水平仍然保持比基线值高出25%-30%的水平。当与在本发明中制备的含氧载体的药物组合物相比,没有一个市售产品或现有技术显示如此高的功效。
对于肿瘤组织氧合,人头颈部鳞状细胞癌(HNSCC)异种移植物(FaDu)的代表性氧特征曲线显示在图7中。静脉内注射0.2g/kg热稳定的交联四聚体血红蛋白后,分别在3小时和6小时观察到多于6.5-倍和5-倍的平均pO2的显著增加(图7)。
对于在癌症治疗中的应用,本发明的含氧载体的药物组合物充当组织氧合剂,以改善肿瘤组织中的氧合,从而提高化学敏感性和放射敏感性。结合X-线照射和热稳定的交联四聚体血红蛋白,肿瘤生长被延缓。在图8A中,代表性曲线显示了鼻咽癌的啮齿动物模型中的显著的肿瘤缩小。携带CNE2异种移植物的裸鼠用单独X-线(2Gy)或与热稳定的交联四聚体血红蛋白联合(2Gy+Hb)治疗。在X-线照射前约3-6小时将1.2g/kg热稳定的交联四聚体血红蛋白经静脉内注射至小鼠,并且导致鼻咽癌异种移植物的部分缩小。
在一个实施方案中,联合化疗剂,在注射所述组合物后观察到显著的肝肿瘤缩小。在图8B中,代表性的图表显示在大鼠原位肝癌模型中显著的肿瘤缩小。携带肝肿瘤原位移植物(CRL1601细胞系)的水牛鼠(buffalo rat)用单独3mg/kg顺铂治疗,或联合0.4g/kg热稳定的交联四聚体血红蛋白治疗(顺铂+Hb)。在顺铂注射前施用热稳定的交联四聚体血红蛋白导致肝肿瘤的部分缩小。
对于在治疗缺氧病症和在心脏保存中的用途,本发明的含氧载体的药物组合物作为将氧提供至靶器官的血液代用品起作用。
在用0.5g/kg的热稳定的交联四聚体血红蛋白治疗后严重出血性休克的大鼠模型中的平均动脉压变化显示在图9中。在严重出血性休克的大鼠模型中,在用热稳定的交联四聚体血红蛋白治疗后,平均动脉压恢复至安全和稳定的水平,并且保持在基线处或基线附近。在用热稳定的交联四聚体血红蛋白治疗后,平均动脉压恢复至正常所需的时间甚至比施用充当阳性对照的自体大鼠血还短。所述结果表明在输注热稳定的交联四聚体血红蛋白后没有发生血管收缩事件。

以描述本发明的具体实施方案的方式提供以下的实施例,但不意在以任何方式限制本发明的范围。
实施例1
过程概况
本发明的方法的示意性流程图显示在图1中。将牛全血采集在封闭的无菌容器/袋中,该容器/袋装有作为抗凝剂的3.8%(w/v)枸橼酸三钠溶液。然后将血液与枸橼酸三钠溶液立即充分混合以抑制血液凝结。通过单采机制(apheresismechamism)将红细胞(RBC)与血浆和其他较小的血细胞分离并收集红细胞。对于此步骤,“细胞洗涤机”与γ灭菌的一次性离心机转筒(centrifuge bowl)一起使用。用等体积的0.9%(重量/体积氯化钠)盐水洗涤RBC。
通过对红细胞的细胞膜进行低渗休克,来裂解洗涤过的红细胞以释放血红蛋白内容物。图2中绘制的用于RBC裂解装置的专用即时细胞裂解设备用于此目的。在RBC裂解后,使用100kDa膜通过切向流超滤将血红蛋白分子与其他蛋白分离。收集滤液中的血红蛋白用于流通柱色谱,并且通过30kDa膜进一步浓缩至12-14g/dL。进行柱色谱法以去除蛋白杂质。
浓缩的血红蛋白溶液首先与DBSF在脱氧条件下反应以形成热稳定的交联四聚体血红蛋白分子。然后在最终的配制和包装前在脱氧条件下在90℃进行热处理步骤30秒-3分钟。
实施例2
时间和受控的低渗裂解和过滤
采集新鲜牛全血并在冷却条件(2-10℃)下运输。经细胞洗涤机将红细胞与血浆分离,随后对红细胞进行0.65μm过滤。在用0.9%盐水洗涤红细胞(RBC)滤液后,通过低渗裂解破裂滤液。通过使用图2中绘制的即时细胞裂解设备来进行低渗裂解。该即时细胞裂解设备包括静态混合器以辅助细胞裂解。含有受控的血红蛋白浓度(12-14g/dL)的RBC悬浮液与4体积的纯净水混合从而产生对RBC细胞膜的低渗休克。控制低渗休克的时期以避免不希望的对白细胞和血小板的裂解。低渗溶液经过该即时细胞裂解设备的静态混合器部分,持续2-30秒,或另外地足以裂解红细胞的时间,优选地30秒。在30秒后当溶胞产物离开静态混合器时通过将溶胞产物与1/10体积的高渗缓冲液混合而终止休克。使用的高渗溶液为0.1M磷酸盐缓冲液,7.4%NaCl,pH7.4。图2的即时细胞裂解设备可以连续地处理50-1000升溶胞产物/小时,优选地至少300升/小时。
在RBC裂解后,红细胞的溶胞产物通过0.22μm滤器过滤以获得血红蛋白溶液。分别通过聚合酶链式反应法(检出限=64pg)和HPLC(检出限=1μg/mL)方法在该血红蛋白溶液中都没有检测到来自白细胞的核酸和磷脂杂质。进行第一100kDa超滤以去除分子量大于血红蛋白的杂质。紧接着进行流通柱色谱以进一步纯化该血红蛋白溶液。然后进行第二30kDa超滤以去除分子量比血红蛋白小的杂质,并用于浓缩。
实施例3
对无基质的(stroma-free)血红蛋白溶液的病毒清除研究
为了证明本发明所述的产品的安全性,我们通过病毒验证研究证明(1)0.65μm渗滤步骤和(2)100kDa超滤步骤的病毒清除能力。这通过在规模缩小形式的这两个过程中故意地添加不同的模型病毒(脑心肌炎病毒(encephalomyocarditis virus)、伪狂犬病病毒(pseudorabies virus)、牛病毒性腹泻病毒(bovine viral diarrhoea virus)和牛细小病毒(bovine parvovims))来进行。在此研究中使用四种类型的病毒(见表3)。这些病毒的生物物理特征和结构特征不同,并且显示在对物理的和化学的试剂或治疗的抗性方面有变化。

流通柱色谱
CM柱(商购自GE healthcare)用来进一步去除任何蛋白杂质。起始缓冲液为20mM乙酸钠(PH8.0),且洗脱缓冲液为20mM乙酸钠、2M NaCl(pH8.0)。用起始缓冲液平衡CM柱后,将蛋白样品加样至柱中。用至少5个柱体积的起始缓冲液洗涤未结合的蛋白杂质。使用8个柱体积的25%洗脱缓冲液(0-0.5M NaCl)进行洗脱。洗脱特征曲线显示在图10中;血红蛋白溶液处于流过级分中。流过级分的纯度通过ELISA分析。结果显示在下面的表6中。

实施例5
热稳定的交联四聚体血红蛋白的制备
(5a)与DBSF的交联反应
在脱氧条件下进行交联反应。将DBSF加入至血红蛋白溶液中以形成交联的四聚体血红蛋白而不形成聚合血红蛋白。DBSF稳定步骤稳定四聚体形式的血红蛋白(65kDa)并且防止解离成二聚体(32kDa),所述二聚体通过肾脏排泄。在此实施方案中,使用1∶2.5的血红蛋白与DBSF的摩尔比,并且pH为8.6。此过程在环境温度(15-25℃)下在氮气惰性气氛中进行3-16小时的时段以防止血红蛋白被氧化形成高铁血红蛋白(其是生理上无活性的)(溶解氧水平维持在小于0.1ppm)。通过使用HPLC测量残留的DBSF来检测DBSF反应的完成情况。DBSF反应的收率很高,>99%。
(5b)HTST加热加工步骤
高温短时(HTST)加工设备显示在图12中。对交联的四聚体血红蛋白进行使用HTST加工设备的加热加工。在此实施例中,热处理的条件为90℃持续30秒至3分钟,并且优选地45-60秒,尽管如上所述可以选择其他的条件,并且相应地改动设备。含有交联的血红蛋白且任选地向其加入0.2%N-乙酰半胱氨酸的溶液以1.0升/分钟的流速被泵入至HTST加工设备(HTST热交换器的第一段被预先加热并维持在90℃)中,在该设备的第一部分的滞留时间为45-60秒,然后使该溶液以相同的流速通过进入到热交换器的维持在25℃的另一部分。冷却所需的时间为15-30秒。冷却至25℃后,立即以0.2%-0.4%优选地以0.4%的浓度加入N-乙酰半胱氨酸。在HTST加热加工后的此化学物质添加对于将高铁血红蛋白(无活性血红蛋白)维持在低水平是非常重要的。容易控制加工设备的设置以进行工业操作。二聚体含量以及温度特征曲线显示在图13中。如果血红蛋白未交联,则其是对热不稳定的,并且在加热步骤后形成沉淀物。然后通过离心或过滤设备将该沉淀物去除以之后形成清亮溶液。
在90℃的HTST加热加工期间,高铁血红蛋白(无活性血红蛋白)增加(显示在图14中)。立即添加N-乙酰半胱氨酸后,可以维持低水平的高铁血红蛋白,大约小于3%。
下面的表7显示蛋白杂质诸如免疫球蛋白-G、白蛋白、碳酸酐酶和不合乎需要的未被稳定的四聚体或二聚体在热处理步骤后被去除。使用ELISA法测量免疫球蛋白-G、白蛋白和碳酸酐酶的量,而通过HPLC法测定二聚体的量。在HTST加热加工步骤后热稳定的交联四聚体血红蛋白的纯度极高,在98.0-99.9%的范围。通过血氧分析仪测量的p50值——氧分压(在此压力下血红蛋白溶液被半饱和(50%))——在整个HTST加热加工步骤中保持在30-40mmHg左右,并且因此,热处理的交联的四聚体血红蛋白在90℃是稳定的。

包装
因为本发明的产品在脱氧条件下是稳定的,因此用于该产品的包装使透气性最小化是重要的。对于静脉内应用,定制设计的100mL输液袋由厚度为0.4mm的5层EVA/EVOH层压材料制成,其氧通透性为在室温下0.006-0.132cm3/100平方英寸/24小时/大气压。此特殊材料是VI类塑料(由USP<88>定义),其满足体内生物学反应性测试和物理化学测试并且适合于制造静脉内注射用容器(需注意也可以由此材料制成其他形式的包装,取决于所需的应用)。二次包装的铝外包装袋也应用于该初级包装输液袋,其提供额外的屏障,使光暴露和氧扩散最小化。该袋的各层包括:0.012mm聚对苯二甲酸乙二醇酯(PET)、0.007mm铝(Al)、0.015mm尼龙(NY)和0.1mm聚乙烯(PE)。外包装薄膜的厚度为0.14mm且其氧传递速率为在室温下0.006cm3/100平方英寸/24小时/大气压。输液袋的示意图绘制在图15中。根据本发明的每个输液袋的总氧通透性为在室温下0.0025cm3/24小时/大气压。
实施例7
氧合的改善
(7a)正常组织中氧合的改善
对于热稳定的交联四聚体血红蛋白对正常组织氧合进行一些研究(图6中所示)。在水牛鼠中进行比较药物动力学和药效学研究。经由雄性近交水牛鼠的阴茎静脉通过推注向该大鼠分别施用0.2g/kg热稳定的交联四聚体血红蛋白溶液或林格氏乙酸盐缓冲液(对照组)。通过HemocueTM光度计在1、6、24、48小时测定血浆血红蛋白的浓度-时间分布曲线并与基线读数比较。该方法基于血红蛋白的光度计测量,其中血红蛋白的浓度以g/dL直接读数。水牛鼠后腿肌肉的氧分压(pO2)通过OxylabTM组织氧合和温度监测仪(Oxford Optronix Limited)直接测量。大鼠通过腹膜内注射30-50mg/kg戊巴比妥溶液进行麻醉,紧接着将氧传感器插入至肌肉中。所有pO2读数通过Datatrax2数据采集系统(World PrecisionInstrument)以实时的方式记录。结果证明在静脉内注射0.2g/kg热稳定的交联四聚体血红蛋白后,平均pO2值在15分钟内从基线升高至相对平均氧分压的约2倍并且延续到6小时。此外,在注射后24至48小时平均氧水平仍然保持在比基线值高出25%-30%(图6B)。
(7b)极度低氧的肿瘤区域中氧合的显著改善
通过人头颈部鳞状细胞癌(HNSCC)异种移植物模型评价极度低氧的肿瘤区域中氧合的改善。下咽鳞状细胞癌(FaDu细胞系)获自美国模式培养物保藏所(American Type Culture Collection)。约1×106癌细胞经皮下注射至4至6周龄近交BALB/c AnN-nu(裸鼠)小鼠中。当肿瘤异种移植物达到8-10mm的直径时,通过OxylabTM组织氧合和温度监测仪(Oxford Optronix Limited)直接监测肿瘤块内的氧分压(pO2)。所有pO2读数通过Datatrax2数据采集系统(World PrecisionInstrument)以实时的方式记录。当pO2读数稳定时,通过小鼠的尾静脉静脉内注射0.2g/kg热稳定的交联四聚体血红蛋白溶液,并测量组织氧合。结果证明在静脉内注射0.2g/kg所述热稳定的交联四聚体血红蛋白后,分别在3小时和6小时观察到多于6.5-倍和5-倍的平均pO2的显著增加(图7)。
实施例8
癌症治疗研究:鼻咽癌的显著肿瘤缩小
在联合X-线照射施用热稳定的交联四聚体血红蛋白溶液后观察到显著的肿瘤缩小(图8A)。采用人鼻咽癌异种移植物模型。将约1x106癌细胞(CNE2细胞系)皮下注射至4-6周龄近交BALB/c AnN-nu(裸鼠)小鼠。当肿瘤异种移植物达到8-10mm的直径,携带肿瘤的小鼠被随机分为如下的三组:
第1组:林格氏乙酸盐缓冲液(对照)
第2组:林格氏乙酸盐缓冲液+X-线照射(2Gy)
第3组:热稳定的交联四聚体血红蛋白+X-线照射(2Gy+Hb)
携带CNE2异种移植物的裸鼠用单独X-照射(第2组)辐照或联合热稳定的交联四聚体血红蛋白(第3组)。对于X-线照射(第2组和第3组),小鼠通过腹膜内注射50mg/kg戊巴比妥溶液麻醉。通过直线加速器系统(Varian MedicalSystems)将2戈瑞(Gray)X-线递送至携带肿瘤的小鼠的异种移植物。对于第3组,在X-线治疗前将1.2g/kg热稳定的交联四聚体血红蛋白通过尾静脉静脉内注射至小鼠中。从治疗第一天开始每隔一天记录肿瘤尺寸和体重。肿瘤重量使用方程1/2LW2计算,其中L和W表示肿瘤块的长度和宽度,在每次测量时通过数显卡尺(Mitutoyo Co,Tokyo,Japan)测量。第1组是未治疗对照组。结果(显示在图8中)证明在用热稳定的交联四聚体血红蛋白溶液联合X-照射治疗的小鼠中观察到CNE2异种移植物的显著缩小(第3组,图8A)。
实施例9
癌症治疗研究:肝肿瘤的显著缩小
另外,在联合顺铂施用热稳定的交联四聚体血红蛋白溶液后观察到显著的肿瘤缩小(图8B)。采用大鼠原位肝癌模型。将约2x106标记有萤光素酶基因(CRL1601-Luc)的大鼠肝肿瘤细胞注射至水牛鼠的肝左叶中。通过Xenogen体内成像系统监测肿瘤生长。注射后2-3周,获取肿瘤组织,切成小块并原位移植到第二组大鼠的肝左叶中。携带肝肿瘤的大鼠被随机分为如下的三组:
第1组:林格氏乙酸盐缓冲液(对照)
第2组:林格氏乙酸盐缓冲液+顺铂(顺铂)
第3组:热稳定的交联四聚体血红蛋白+顺铂(顺铂+Hb)
移植了肝肿瘤组织的大鼠用单独3mg/kg顺铂(第2组)或联合热稳定的交联四聚体血红蛋白(第3组)治疗。对于第2组和第3组,大鼠通过腹膜内注射30-50mg/kg戊巴比妥溶液麻醉,并经由左门静脉施用顺铂。对于第3组,在顺铂治疗前将0.4g/kg热稳定的交联四聚体血红蛋白通过大鼠的阴茎静脉静脉内注射。第1组是未治疗对照组。重要的是,在治疗后3周观察到肝肿瘤的显著缩小(图8B)。
实施例10
大鼠急性严重出血性休克的治疗
热稳定的交联四聚体血红蛋白还在大鼠的急性严重出血性休克模型中用作复苏剂。将50只Sprague-Dawley大鼠根据复苏剂随机地分成3组,每组16-18只大鼠。
第1组:乳酸盐林格氏溶液(阴性对照,16只大鼠)
第2组:动物自体血(阳性对照,16只大鼠)
第3组:热稳定的交联四聚体血红蛋白治疗组(0.5gHb/kg体重,18只大鼠)
通过抽取50%的动物全血,估计为体重的7.4%,造成急性严重出血性休克。造成出血性休克10分钟后,将乳酸盐林格氏溶液、动物自体血、或0.5g Hb/kg的热稳定的交联四聚体血红蛋白输注至动物体内。热稳定的交联四聚体血红蛋白的输液速率设置为5mL/h,之后,对所有试验动物观察24小时。在研究期间观察和分析一组参数,包括存活率、血流动力学、心肌力学、心输出量、心功能、血气、组织氧输送和氧耗量、组织灌注和氧张力(肝、肾和脑)、肝功能和肾功能、血液流变学(血液粘度)、和线粒体呼吸控制速率(肝、肾和脑)。其中,存活率是主要终点指标。观察24小时后,热稳定的交联四聚体血红蛋白治疗组与乳酸盐林格氏溶液或阴性对照组和自体血组相比具有高得多的存活率(显示在下表8中)。

实施例11:预防手术后肝肿瘤复发和转移的方法
手术切除肝肿瘤是肝癌的一线治疗。然而,癌症的手术后复发和转移仍然是这些患者中不利预后的主要特征。例如,以前的研究报道肝切除术与50%的5年存活率相关,但也与70%的复发率相关。对肝细胞癌(HCC)患者的随访研究也揭示在大约15%的HCC患者中检测到来自原发HCC的肝外转移,其中肺是肝外转移的最常见部位。已经提示手术应激特别是在肝脏手术期间引起的缺血/再灌注(IR)损伤是肿瘤进展的主要原因。按照惯例,外科医生通常使用肝血流控制来防止肝切除术期间的大出血。例如,已经使用通过阻断肝门(肝门血流阻断(Pringle maneuver))的入肝血流阻断来使失血最小化并且减少围手术期输血的需求。最近的日本研究显示25%的外科医生常规地采用肝门血流阻断。然而,肝门血流阻断会在残留肝脏中诱发不同程度的缺血损伤并且与癌症复发和转移相关。
IR损伤和肿瘤进展的相关性也得到以前的动物研究的支持。首先,在采用原位肝癌模型的最近一项研究中证明了IR损伤和肝切除对肝癌复发和转移的作用。肝脏IR损伤和肝切除术导致肝脏肿瘤的显著复发和转移。在结直肠肝转移小鼠模型中获得了类似的结果,其中IR损伤的引入加速了结直肠肝转移的生长。
以前,已经研究了几种保护性策略用于减少切除期间的IR损伤。例如,在延期阻断前采用短时段的缺血,被称为缺血预适应(IP),被提议用来触发肝细胞防御机制并且已经被用来减少肝切除期间的IR损伤。其他人采用间歇阻断(IC)操作,该操作允许多个周期的入肝血流阻断,紧接着再灌注。两者方法均被提议有效地保护经历肝脏大手术的非肝硬化患者免受手术后肝损伤。然而,在肿瘤的情况下,动物研究还显示IP不能保护肝脏免于IR损伤诱发的加速肿瘤生长。另外,一些小组尝试使用抗氧化剂诸如α-生育酚和抗坏血酸来保护肝脏免于IR损伤,由此防止肝转移。然而,两种抗氧化剂均不能限制由IR刺激的肝内肿瘤生长。
从机制上,不同的证据提示由于以下的多种原因,低氧与肿瘤复发和转移相关联:(1)研究显示低氧的肿瘤对放射疗法和化学疗法耐受性较大,在治疗中存活的肿瘤细胞易于复发;临床证据还提示具有较多的低氧肿瘤区域的患者具有较高的转移率;(2)在低氧条件下,癌细胞通过低氧诱导因子-1(HIF-1)途径的激活变得更有侵袭性。这又触发了补充反应,该补充反应涉及促血管生成因子血管内皮细胞生长因子(VEGF)和受体诸如c-Met和CXCR4,其增强细胞运动性和至特异的远处器官的归巢;(3)近年来的研究还证明循环癌细胞(CTC)在低氧条件下变得更有侵袭性。在癌症患者的外周血中检测到的循环肿瘤细胞显示是具有远处转移的患者中疾病侵袭的指标,而低氧使得这些细胞具有更具侵袭性的表型并且使凋亡潜能消失。特别地,在氧水平降低的情况下对放射线较为耐受的癌症干细胞群在脑肿瘤中富集。
因此,鉴于上面的观察结果和研究,本发明的非聚合的交联四聚体血红蛋白用于防止肝切除后的手术后肝肿瘤复发和转移。建立了大鼠原位肝癌模型。肝细胞癌细胞系(McA-RH7777细胞)用来在水牛鼠(雄性,300-350g)中建立原位肝癌模型。图16显示了总结了手术和血红蛋白产品施用操作的示意图。将McA-RH7777细胞(3×105/100μl)注射至水牛鼠的肝包膜中以诱发实体肿瘤生长。2周后(此时肿瘤体积达到约10×10mm),收集肿瘤组织并将其切成1-2mm3方块并植入至新的一组水牛鼠的肝左叶中。原位肝肿瘤植入后2周,使大鼠经历肝切除(携带肝肿瘤的左叶)和部分肝IR损伤(在右叶上缺血30分钟)。
两组带有植入的肿瘤组织的大鼠用于比较肿瘤复发和转移。在第1组中,大鼠用戊巴比妥麻醉并在缺血前1小时静脉内施用0.2g/kg本发明的非聚合的热稳定的交联四聚体血红蛋白。通过用动脉夹夹闭肝脏门静脉的右支和肝动脉在肝脏的右叶中引起缺血。随后,在左肝叶进行结扎,紧接着切除携带肝肿瘤的左肝叶。在缺血后30分钟时,通过下腔静脉注射额外的0.2g/kg热稳定的交联四聚体血红蛋白,紧接着再灌注。在第2组中,以相同的操作注射作为载体对照的林格氏乙酸盐缓冲液。在肝切除术操作后4周处死所有大鼠。
为了检查肿瘤生长和转移,在缺血/再灌注和肝切除术操作后4周时将水牛鼠的肝脏和肺取样用于形态学检查。获取组织,对其进行石蜡包埋并切片,紧接着进行苏木素和伊红(H&E)染色。通过组织学检查确认局部复发/转移(肝内)和远处转移(肺)。表9总结了观察结果。

为了检查非聚合的热稳定的交联四聚体血红蛋白对肝肿瘤复发和转移的保护效应,在肝切除术和IR操作后4周处死所有大鼠。收获肺和肝组织;在两组中比较肝肿瘤复发/转移和肺中的远处转移。结果显示血红蛋白治疗减少两个器官中的复发和转移的发生率。
图17显示了在肝切除术和缺血/再灌注操作后IR损伤组大鼠中诱发的肝内肝癌复发和转移以及远处肺转移以及使用本发明的热稳定的交联四聚体血红蛋白的保护作用的代表性实例。在图17A中,在IR损伤组中观察到广泛的肝内肝癌复发/转移。在同一只大鼠中还发生了远处肺转移(由实心箭头指示)。在图17B中,在IR损伤组的另一例中观察到肝内肝癌复发/转移(由虚线箭头指示)。在同一例中观察到广泛的肺转移(由实心箭头指示)。相反,图17C显示了在用本发明的热稳定的交联四聚体血红蛋白治疗的大鼠中保护免于发生肝内肝癌复发/转移和远处肺转移的代表性实例。
图18显示了在肝切除和IR损伤操作后4周时在两组中的组织学检查。在IR损伤组和血红蛋白治疗组两组中进行肝脏和肺组织的组织学检查(H&E染色)以确认肿瘤结节的性质。显示了示出血红蛋白治疗组(T3)和IR损伤组(T1和T2)中肝内复发/转移的代表性视野。包括在治疗组中显示出正常的肝脏整体结构(N1)的组织学检查用于比较。另外,在IR损伤组的同一只大鼠中发现肺中的远处转移(M)。在治疗组中示出没有转移灶的肺组织(N2)用于比较。
为了进一步确认非聚合的热稳定的交联四聚体血红蛋白对肿瘤复发和转移的保护效应,研究了缺血/再灌注和肝切除术操作后肿瘤的复发率和复发肿瘤的大小。再次,如图16中所述,通过如上所述注射McA-RH7777细胞制备的带有植入的肿瘤组织的大鼠在缺血前和在肝切除操作后再灌注时用大约0.2-0.4g/kg本发明的非聚合的热稳定的交联四聚体血红蛋白或作为阴性对照的林格氏乙酸盐(RA)缓冲液经静脉内治疗。测试总共24只大鼠,其中11只大鼠用本发明的血红蛋白治疗且13只是阴性对照大鼠,阴性对照大鼠仅用RA缓冲液治疗。在肝切除术和IR操作后4周处死所有大鼠,检查测试大鼠的肝脏和肺的肿瘤复发/转移并且测量复发肿瘤的相对尺寸。
图19A显示测试大鼠中的肝肿瘤复发和各复发肿瘤的体积。13只未治疗的对照大鼠中有9只发生肝肿瘤复发/转移,而11只经治疗的大鼠中仅有4只经历肿瘤复发/转移。还明显的是在观察到肿瘤复发的情况下,用本发明的血红蛋白治疗的大鼠的复发肿瘤的尺寸显著小于那些未治疗的大鼠。这些结果显示采用本发明的治疗极大地减小了肿瘤复发率并且显著地减小了复发肿瘤的尺寸,如图19B中总结的那样。
图20图示了在使用本发明的非聚合的热稳定的交联四聚体血红蛋白治疗的大鼠和IR损伤(阴性对照)组大鼠在肝切除术和IR操作后4周收获的肝和肺组织的代表性实例。如在未治疗的阴性对照组的代表性实例(大鼠C10和13)中所见,观察到广泛的肝内肝癌复发/转移和远处肺转移灶(圆圈)。另一方面,通过本发明的血红蛋白的治疗防止了肝内肝癌复发/转移和远处肺转移,如大鼠Y9、Y10和Y11中所见。
实施例12:用非聚合的热稳定的交联四聚体血红蛋白治疗减少缺血
如实施例7中所证明,本发明的非聚合的热稳定的交联四聚体血红蛋白对低氧的肿瘤的静脉内注射显著地改善了其中的氧合。因此,研究了在肿瘤切除和IR操作期间本发明的血红蛋白产品的氧合效应。使用如实施例11中所述通过注射McA-RH7777细胞制备的带有植入的肝肿瘤组织的大鼠,并使其接受手术和0.2-0.4g/kg本发明的血红蛋白产品或RA缓冲液施用操作,如图16中所概述的那样。从首次向肝肿瘤施用本发明的血红蛋白产品/RA缓冲液的时间开始并在整个IR操作、肝肿瘤切除期间和再灌注后测量肝脏的氧分压。结果(图21)显示在引入缺血后观察到用本发明的血红蛋白治疗增加了氧合。另外,如图21中所见,再灌注后,已经用本发明的血红蛋白治疗的肝脏的氧分压是未治疗的肝脏的大约3倍。证实与未治疗相比,在缺血前和在肿瘤切除后再灌注时本发明的血红蛋白的治疗显著地改善肝组织的氧合。鉴于本领域中提出的低氧的肿瘤和增加的肿瘤复发/转移的可能性之间的强相关性,如在此实施例中所证明的本发明的血红蛋白产品的巨大氧合效应以及其在肿瘤切除操作期间的使用,本发明的血红蛋白产品减少肿瘤复发和转移的有效性得到明显地证实。
实施例13:用非聚合的热稳定的交联四聚体血红蛋白治疗减少循环内皮祖细胞水平
不同的研究已经证明了癌症干细胞(CSC)和/或祖细胞群在肝癌进展中的重要意义。重要的是,以前的研究显示在HCC患者(包括经历肝切除术的那些患者)中发现明显较高水平的循环内皮祖细胞(EPC)。
因此,通过表面分子诸如CD133、CD34和VEGFR2的表达评价了循环EPC的水平。研究了采用或不采用本发明的血红蛋白产品治疗的肝切除手术和IR操作后的循环内皮祖细胞水平。两组带有植入的肝肿瘤的大鼠接受本发明的血红蛋白或RA缓冲液(对照)的治疗,如上面实施例11和图16中所述,分别在缺血前和在肝切除后再灌注时。然后在肝切除和IR操作后0、3、7、14、21和28天时测量两组大鼠的循环EPC的数目。结果(图22)显示尽管在手术后第0天至第3天期间治疗的和未治疗的组的EPC水平是相当的,但血红蛋白治疗组的EPC水平大大低于那些RA缓冲液治疗的组。这些结果与实施例11的结果是一致的,在实施例11中验证了本发明的血红蛋白减少和最小化肿瘤复发/转移的保护效应。
作为上面研究的结果,得出结论:用本发明的非聚合的、热稳定的交联四聚体血红蛋白治疗对肝肿瘤的复发和其他器官中的转移均具有预防作用。
尽管已经就多个实施方案描述了前述发明,但这些实施方案并非限制性的。本领域普通技术人员将理解大量的变化形式和改型。这样的变化形式和改型被认为包括在下列权利要求的范围内。
Claims (22)
1.含有非聚合的、高度纯化的和热稳定的氧载体的组合物在制备用于在哺乳动物中减少癌性肿瘤复发和/或最小化肿瘤细胞转移的药物中的用途,其中所述组合物包含具有检测不到的二聚体浓度的交联的四聚体血红蛋白。
2.权利要求1所述的用途,其中所述组合物在手术去除肿瘤期间的中断血液供应前和重建血液供应期间被施用于所述哺乳动物。
3.权利要求2所述的用途,其中所述组合物以大约0.2g/kg-1.2g/kg所述哺乳动物的体重的范围施用。
4.权利要求1所述的用途,其中所述癌性肿瘤和肿瘤细胞是肝的或鼻咽的。
5.权利要求1所述的用途,其中所述癌性肿瘤和肿瘤细胞是低氧的。
6.权利要求1所述的用途,其中所述交联的四聚体血红蛋白具有60-70kDa的分子量和0.2-0.4%浓度的N-乙酰半胱氨酸。
7.权利要求6所述的用途,其中所述组合物不含引起血管收缩的杂质和蛋白杂质,是无致热原的、无内毒素的、无磷脂的、无基质的和具有小于5%的高铁血红蛋白水平。
8.含有非聚合的、高度纯化的和热稳定的氧载体的组合物在制备用于治疗癌组织的药物中的用途,其中所述组合物包含具有检测不到的二聚体浓度的交联的四聚体血红蛋白。
9.权利要求8所述的用途,其中所述癌组织通过放射治疗或化疗治疗来治疗。
10.权利要求9所述的用途,其中所述组合物在所述癌组织的放射治疗或化疗治疗前被施用于癌组织。
11.权利要求8所述的用途,其中所述癌组织是鼻咽癌或肝肿瘤。
12.权利要求8所述的用途,其中所述癌组织是低氧的。
13.权利要求8所述的用途,其中所述交联的四聚体血红蛋白具有60-70kDa的分子量和0.2-0.4%浓度的N-乙酰半胱氨酸。
14.权利要求13所述的用途,其中所述组合物不含引起血管收缩的杂质和蛋白杂质,是无致热原的、无内毒素的、无磷脂的、无基质的和具有小于5%的高铁血红蛋白水平。
15.权利要求10所述的用途,其中所述组合物以约0.2-1.2g/kg体重的范围和小于10ml/小时/kg体重的速率通过输注施用。
16.含有非聚合的、高度纯化的和热稳定的氧载体的组合物在制备用于增加癌组织的氧合的药物中的用途,其中所述组合物包含具有检测不到的二聚体浓度的交联的四聚体血红蛋白。
17.含有非聚合的、高度纯化的和热稳定的氧载体的组合物在制备用于减少肿瘤尺寸的药物中的用途,其中所述组合物包含具有检测不到的二聚体浓度的交联的四聚体血红蛋白。
18.一种含有非聚合的、高度纯化的和热稳定的氧载体的组合物,其用于在哺乳动物中减少癌性肿瘤复发和/或最小化肿瘤细胞转移,其中所述组合物包含具有检测不到的二聚体浓度的交联的四聚体血红蛋白。
19.权利要求18所述的组合物,其中所述组合物在手术去除肿瘤期间的中断血液供应前和重建血液供应期间被施用于所述哺乳动物。
20.权利要求19所述的组合物,其中所述组合物以大约0.2g/kg-1.2g/kg所述哺乳动物的体重的范围施用。
21.权利要求18所述的组合物,其中所述交联的四聚体血红蛋白具有60-70kDa的分子量和0.2-0.4%浓度的N-乙酰半胱氨酸。
22.权利要求18所述的组合物,其中所述组合物不含引起血管收缩的杂质和蛋白杂质,是无致热原的、无内毒素的、无磷脂的、无基质的和具有小于5%的高铁血红蛋白水平。
