背景技术
关节常常由于各种原因而经历退行性变化。当退化变得严重或不可逆时,可能变得有必要用假体关节置换自然关节。退行性疾病或伤害可能需要使用典型地金属、陶瓷或聚合物材料的合成材料置换患者的关节的全部或一部分。由这些材料制成的包括髋关节植入物、肩关节植入物和膝关节植入物在内的人工植入物在整形外科手术中被广泛地使用以加强或置换正退化的关节。
在初次置换手术中,患者的自然关节被假体植入物置换。植入物设计成与患者的骨一体化并且提供与被置换的自然关节相同的功能和运动。在初次置换之后,可能变得有必要进行第二次或后续的翻修手术,以便用常常比初次置换更大的置换关节来置换假体关节。由于骨的进一步退化或退行性疾病的发展需要用常称为翻修假体的更大的增强的假体关节来置换植入物和患病骨,这样的手术常常变得有必要。例如,骨在髋关节植入物周围的髋臼的边缘周围常常损失,并且因此可提供较小的边缘覆盖率以可靠地保持压力配合的髋臼壳体。较大的壳体可在翻修手术中置于髋臼内以弥补损失的骨。
在初次置换手术和翻修置换手术中,患者的自然骨可具有缺损或空隙,外科医生在插入植入物之前可用骨移植物填充该缺损或空隙。该过程可能是耗时且昂贵的,并且可能使患者面临健康风险。为了有利于在具有缺损或空隙的骨中的成功植入,外科医生可结合标准植入物部件使用植入物增强物来填充空隙。这样的增强物可在自然骨中的骨空隙或缺损的区域中联接或以其它方式附连到植入物的外表面。除了初次手术之外,增强物常常由于在需要翻修手术的退化区域中的骨退化、骨损失或其它缺损而在翻修手术中使用。
植入物增强物中的可变程度可能是所需的,因为自然骨和骨缺损的位置和性质随患者变化。尤其是在骨中的缺损或空隙存在时,优选的是外科医生有能力调整植入物以配合骨,而不是调整骨以配合刚性的植入物。调整植入物或植入物增强物允许保留自然骨,而不需要在准备置入植入物过程中对患者的自然骨进行扩孔或其它移除。保留骨辅助骨对植入物部件所提供的支撑。尤其是在翻修手术中患病骨或破损骨的情况中,为准备植入物而移除的任何骨都可能证明不利于骨的完整性和其承受植入之后的显著力的能力。

发明内容
本文公开了用于提供允许术中可变性和向自然骨内的辅助固定的植入物增强物的系统、设备和方法。通常,系统、设备和方法提供包括一个或多个锁定机构的增强物,该锁定机构接合机械紧固件并将增强物固定在骨中。锁定机构被构造成使得增强物可以多种取向插入,从而允许锁定机构接合处于可能取向中的每一种的机械紧固件。这些增强物提供了充分的骨固定和与机械紧固件的锁定接合的组合,同时仍保持与患者的自然骨和置于该骨中的植入物两者的紧密接触。

开口102a和102b以及锁定翅片105相对于配合面106的取向在螺钉108被插入到开口的第二端部107a或107b时允许增强物100与植入物保持紧密交接。由于翅片105不平行于配合面106,围绕图1A中所示轴线105的旋转以及螺钉108向开口102b的第二端部107b的插入可实现,而不影响配合面106的定位。同样,骨接口表面112被保持在当增强物100围绕轴线115旋转时接触患者的自然骨的位置。类似于配合面106,骨接口表面112在螺钉108被插入穿过开口的第一端部104a或104b或者第二端部107a或107b时与骨保持紧密接触。开口102a和102b的这种对齐允许在外科医生可插入增强物100的取向上的广泛的可变性,同时仍保持配合面106与植入物以及骨接口表面112与患者的骨之间的所需接触,并且还保持将螺钉108锁定在开口内的能力。开口102a和102b以及锁定翅片105的多轴线性质进一步有助于增强物100的广泛可变性。
翅片105相对于配合面106和骨接口表面112的取向在放置增强物过程中提供了可变性,同时保持配合面106与植入物以及骨接口表面112与患者的骨的所需定位。图2进一步示出在增强物100的开口102a的俯视图中的翅片105的取向。如图2所示,翅片105设置在开口102a的内部上,从开口的内表面119朝开口的中心纵向轴线109向内延伸。翅片105基本上垂直于内表面119延伸,并且因此翅片105中的每一个基本上设置在图2中所示的平面114内。平面114垂直于开口102a的纵向轴线109并且示出翅片105相对于配合面106的对齐。如图2所示,平面114与配合面106相交并且不平行于配合面106。在图2所示实施例中,平面114垂直于配合面106,但在其它实施例中,平面可略微歪斜,从而不完全垂直于配合面。
通常,翅片105相对于配合面106的取向允许开口102a以各种角度、位置和方向接纳紧固件。特别地,由于翅片105设置在基本上垂直于配合面106的平面114中,螺钉可由开口102a或者通过开口的第一端部104a(图2中所示)或者通过开口的第二端部107a(图2中未示出)(即,从开口的背面或正面)接纳。这允许外科医生更灵活地选择螺钉插入的位置和方向。它还允许配合面106与植入物保持紧密接触,而不论螺钉插入到开口102a的哪个端部。如果例如开口102a和翅片105平行于配合面106对齐,而不是如图2中所示垂直对齐,这种紧密接触将是不可能的。如果开口和翅片平行于配合面,螺钉将中断配合面的平面。这将阻止配合面和植入物之间的紧密接触,因为在配合面106与植入物紧密接触放置的情况下,螺钉不能在图2中箭头A所示的方向上插入。这种复杂情况通过翅片105和开口102a的取向来避免,因为当配合面106与植入物保持紧密接触时,螺钉可从开口的任一端插入。
由增强物100赋予的可变性为外科医生提供了调整增强物以适合特定手术的具体需求或骨解剖结构的能力。这种可变性允许外科医生在术中做决策并且确定最好地满足周围骨解剖结构的需求的取向。例如,外科医生可观察在植入物置入患者骨内情况下的术野,并且确定用以填充患者的骨内的缺损或空隙的增强物的最佳取向。外科医生接着能够将增强物定位在所需取向并且将螺钉插入开口的不论哪个端部,以便增强物更好地接合患者的骨并将螺钉锁定在锁定构型。备选地,增强物的可变性赋予外科医生在术前决定最好地满足患者的骨的需求的增强物的取向的能力。在一些方法中,这可能需要允许外科医生实现所选取向之一的多个不同的增强物。然而,增强物100的可变性允许外科医生仅使用单个增强物来实现他或她所希望的不论哪个取向。不论决策是在术前还是术中做出,外科医生都能够将增强物定位在最好地满足自然骨的需求的取向,同时保持增强物的配合面与植入物之间的紧密接触、以及增强物的骨接口表面与患者的周围骨之间的紧密接触。所有这些相互作用都得以保持,同时允许螺钉以所选的任一种取向插入和锁定在增强物中。开口和增强物的锁定机构的多轴线性质进一步有助于外科医生根据需要定位增强物和螺钉的能力。
图3A-3C示出突出增强物100的可变性的三种构型。图3A示出第一构型150,其中增强物100与髋臼壳体一起植入。增强物100在图3A中示出为邻接植入的壳体的边缘116,其中边缘116具有外表面118和内表面120。在图3A所示的构型中,增强物100抵靠边缘116的外表面放置。在该取向下,图3A中未示出的两个腿部111a和111b远离顶部表面113向下延伸。例如,如果存在随壳体植入且将壳体保持在患者骨内的螺钉,该取向可能是所需的。通过将增强物100以图3A所示取向插入,两个腿部111a和111b能够经过螺钉的任一侧并且允许植入增强物100而不妨碍螺钉。
不是图3A中所示的增强物取向,外科医生可能希望以备选构型放置增强物100。增强物100的可变性允许外科医生围绕图3A中所示轴线115旋转增强物100,以便实现其它增强物取向,同时保持增强物的配合面106和植入物的外表面118之间的接触。图3B示出一种备选取向,其中,增强物100从图3A中所示取向绕轴线115旋转180度。在图3B中所示取向160中,增强物被放置成使得增强物的顶部表面113定位成向下进入骨中,并且两个腿部111a和111b朝植入物的顶部边缘116面向上。在该取向下,在增强物的配合表面106和植入物的外表面118之间保持紧密接触。另外,骨接口表面112的位置保持背对自然植入物,使得它能够接触患者的自然骨。如果例如外科医生确定了在植入的壳体的下部中存在骨空隙或需要更多支撑,取向160可能是所需的。通过将增强物100定位在取向160,外科医生可将具有更多材料和更多支撑的增强物100的顶部朝需要支撑的自然植入物的底部向下定位。
为了方便以取向160放置增强物100,螺钉161a和161b被插入开口102a和102b的第二端部107a和107b中。这些开口的多轴线和多方向性质允许将螺钉以这种备选取向锁定在开口中。由于开口和锁定机构在多个方向上工作,可以实现取向160,同时保持患者的骨与自然植入物之间的所需接触,并且仍利用增强物100的可用锁定机构。
除了图3A和3B中所示两种取向之外,可能希望的任何中间旋转都可用增强物100实现。图3C示出抵靠自然植入物放置的增强物100的第三取向170。如图3C所示,增强物100从图3B中所示取向绕轴线115旋转90度。这种旋转将植入物定位成使得腿部111a面朝植入物边缘116的顶部。在取向170下,同样保持了在增强物的配合面106和边缘116的外表面118之间的紧密接触。另外,骨接口表面112的定位保持背对边缘116,使得它可以接触患者的自然骨。螺钉171被插入开口102a的第二端部107a中并且延伸穿过开口超出增强物的顶部表面113。由于开口102a的中心轴线109不像图3A和3B中那样延伸远离患者的自然骨,螺钉的放置可能比在取向150和160下的螺钉放置更加困难。开口102a和将螺钉保持在开口102a中的锁定翅片的多轴线性质可有利于螺钉从中心轴线109的偏轴放置,以便仍然允许外科医生将螺钉固定在患者的骨内并且以其与增强物100的取向锁定螺钉。
虽然增强物100示出为具有马蹄形形状以突出增强物的某些优点和应用,但增强物的备选实施例可采取适合于特定应用的任何形式或形状。图4示出具有备选形状的增强物200。增强物具有完整配合面206,其不具有像增强物100的配合面106在腿部111a和111b之间具有的那种间隙。对于其中存在大的骨空隙要填充或者在自然植入物中不存在要避开的螺钉的应用来说,该形状的增强物可能是理想的。增强物200包括其中穿入两个开口202a和202b的顶部表面213。开口202a和202b中的每一个包括从开口的内表面设置的锁定翅片205。类似于上文结合增强物100讨论的开口和锁定翅片,开口202a和202b可通过开口的第一端部204a或204b或第二端部中的任一个接纳紧固件,第二端部在图4中未示出。翅片205设置在不平行于配合面206的平面中。因此,开口202a和202b以及翅片205的多方向、多轴线性质允许增强物200以在配合面206和植入物之间保持紧密接触的各种取向放置,并且还允许外科医生利用开口202a和202b的锁定机构。

Claims (9)

1.一种整形外科设备,包括:
第一表面,其具有构造成与植入物的外表面配合的配合面;
第二表面,其构造成与患者的骨或组织配合;以及
至少一个贯穿开口,其从所述第二表面延伸被构造成接纳紧固件,所述贯穿开口包括设置在不平行于所述配合面的平面中的多个翅片。
2.根据权利要求1所述的设备,其特征在于,所述贯穿开口包括第一和第二端部。
3.根据权利要求2所述的设备,其特征在于,所述贯穿开口被构造成当所述设备处于第一取向时接纳插入所述第一端部中的紧固件,并且被构造成当所述设备处于第二取向时接纳插入所述第二端部中的紧固件。
4.根据权利要求2或3所述的设备,其特征在于,所述翅片设置在所述开口的所述第一和第二端部之间。
5.根据权利要求3所述的设备,其特征在于,所述翅片被构造成当所述设备处于所述第一取向时将紧固件锁定在第一锁定构型中,并且被构造成当所述设备处于所述第二取向时将紧固件锁定在第二锁定构型中。
6.根据权利要求3所述的设备,其特征在于,所述配合面被构造成当所述设备处于所述第一和第二取向中的每一个时与植入物配合。
7.根据权利要求3所述的设备,其特征在于,所述第二表面被构造成当所述设备处于所述第一和第二取向中的每一个时与患者的骨或组织配合。
8.根据权利要求1所述的设备,其特征在于,所述平面基本上垂直于所述配合面。
9.根据权利要求1所述的设备,其特征在于,所述翅片被构造成接合沿多个轴线中的任一个插入的紧固件并且将所述紧固件锁定在对应于所述多个轴线的多个锁定构型中。
