Abstract
一种具有销套、在销套远端且可相对其运动一预
定距离而起动的夹紧部件、支承组件以及可相对此支
承组件运动的滑板的外科用夹紧装置,此种装置中设
有器件,能响应滑板相对于支承组件运动一段较上述
预定距离为短的路程,而促使夹紧部件相对于销套运
动过这段用来起动此夹紧部件所需的预定距离。响
应器件包括有皮带之类设备,也可取齿轮形式。此种
外科用夹紧装置便于外科医生操作,可廉价生产,显
著提高机械效益。
Description
本发明涉及用来插入体腔并由手动操作来夹紧病人一部分组织的那种外科用夹紧装置。
手动式外科用夹紧装置属于公知的。一般,是由外科医生在这种装置的近端操纵一根线绳,由后者的运动来控制位于远端的夹紧部件。通常,这类装置包括一设有拇指孔与滑板的支承组件,而此滑板又包括一指孔且可相对于该支承组件运动。上述夹紧装置可封装于一销套内,使得在所述线绳将此夹紧装置拉入销套内时,夹紧装置便可起动而夹住病人的部分组织。这样一类夹紧装置可用于例如支气管窥镜、结肠窥镜、十二指肠窥镜、内窥镜以及胃窥镜等。
在某些情况下,为了起动此种夹紧装置,滑板必须相对于支承组件作较显著的运动。例如在由外科手术从结肠上取下息肉时所用息肉切除术的铉圈套器中,就必须使滑板相对支承组件作多达3.5英寸的运动。这种称之为夹紧装置“推抛”的运动,对外科医生来说常常是不舒适的,有时需要同时用双手来操作这种装置。
本发明的主要目的在于提供所述这种类型的外科用夹紧装置,但其中免除了起动这种装置所需要的推抛运动。
本发明的另一目的在于提供这样一种外科用夹紧装置,它的费用经济,可制成一次性使用装置,且由于省去了起动此种夹紧装置时所需的推抛运动,较可比的同类装置更易使用。
本发明另一个较具体的目的则是提供所述的这样一种外科用夹紧装置,在省除了起动夹紧装置所需的推抛动作的同时,于外观与结构两方面都与先有的这种外科手术装置相似。
简单地说,本发明的外科用夹紧装置包括一支承组件、一个可相对于此支承组件运动的滑板、一个销套以及一通过此销套的线绳。当此线绳相对于销套运动一预定距离后,线绳远端的夹紧部件便开始工作。依据本发明,设置有部件来提高这种装置的机械效益,使得起动此装置时所需的滑板相对于支承组件的位移,大大地小于前述的预定距离。
根据所给的用作说明的几个实施例,起动用的线绳可以耦连到滑板之上,而销套则设计成可动的或可固定到支承组件之上。在一最佳实施例中,此销套是可动的且有一皮带安装在销套之上,而在滑板运动时,它便促使销套逆线绳运动的方向运动。在其它的实施例中,则由齿轮件来提供所需的机械效益。
从以下对本发明的说明性实施例之附图所作的详尽描述中,当可更迅速地理解本发明的上述特点和其它种种特点,在附图中:
图1是本发明一最佳实施例的透视图,表明了未装于销套内的铉圈套器的套圈;
图2是图1所示实施例之部分剖面的顶视图,这里的铉圈套器处于销套之内;

如图1所示,本发明的外科用夹紧装置具有一作为其第一部件的支承组件10,它包括一近侧的拇指环12与轨14。这两条轨14为一沟槽分开,在它们的近端为此拇指环12连接,在其远端则为一整体形成的前方件15所连接。轨14的横剖面呈月牙形(图5)。在支承组件10上沿横向安装有远侧轴16与近侧轴18(图3)。
作为第二组件,乃是一分叉的滑板30,可滑动地位于轨14之上。滑板30包括两个指孔32、一环形体33以及一在轨14下延伸的细长的舌状件34(图4)。环形体33的内部与轨14的横剖面积匹配,从而可允许滑板30在端帽15与拇指环12之间相对于支承组件10作纵向运动。
第三个部件为一线绳组件50,包括杆51,将杆51连至线绳53的连接器部段55(图2),以及将线绳53在其远端连至一套圈式铉圈套器59的连接器57。铉圈套器59在图1中示明于一开放的或未装入套销内的位置。套销60封装着线绳53与铉圈套器59,除非当线绳53处于一伸张位置去打开铉圈套器59成为图1与4所示的套圈形状时。
如图2~4所示,销套60的近端固定在一滑块70之上。由于滑块70是固定于销套60上,因而滑块70在两条轨14之间的运动就将导致销套60作相同的纵向运动。类似地,由于杆51固定在滑板30上,滑板30的纵向运动也将促使杆51有相同的运动。
如图1与2所示,杆51穿过前方件15中的一个孔并在轨14之间通过安装滑块70。杆51置于滑块70的一孔膛(未编号)内,可与之相对作轴向运动。杆51未封装于销套60内而是在轨14之间延伸到环形体33内,并于此由连接器62固定。连接器62通入到环形体内使杆51得以固装到滑板30内至所需程度。销套60的内径大小取定为,可使杆51、线绳53、铉圈套器59以及连接器55与57得以沿着此销套的全长基本上能作自由的纵向运动。
图3是沿图2中3-3线的放大的剖面图,其中所示的皮带80环绕近侧轴18、通过它牢靠地定位于其中的滑块70并绕过远侧轴16。作为滑板16一部分的舌形件34则如图1与4所示,从指孔的环形体33沿轨14的下侧延伸。在舌形件34的远端有一热密封销36,上面安装着皮带80的有孔端,如图3所示。皮带80于此连接着滑板80,通过加热此热密封销36而形成图示的一个头部。皮带80可以通过将滑块70从上方模压到其上而定位到此模块上(图3)。
为了操作图1~5所示的此种装置,使用者可抠住拇指环12与指孔32并将滑板30拉至支承组件10的近端,在此位置,铉圈套器59便撤回于图3所示的销套60之内。
与通常情形相同,在本发明的一种应用中,外科医生将销套60插入病人的结肠中,直至定位于息肉处。然后这名医生可用一只手将滑板30移至支承组件10的远端,后者同时使销套60拉向此装置的近端,且使线绳组件51、53与59推向此装置的远端。这样就使铉圈套器59显露出并且张开,而可用来套合住病人的息肉。例如,若需使此夹紧部件相对于销套远动3英寸而完全脱出铉圈套器59,利用图1~5中所示的外科手术装置时,外科医生便只需使滑板相对支承组件运动1.5英寸。
当此铉圈套器业已环套上待除去的息肉时,这时外科医生便可将滑板后拉至支承组件的远端。于是滑板30便把上述线绳组件与铉圈套器59拉至此夹紧装置的近端,同时,销套60即被推到所述远端,使铉圈套器紧绕息肉。再有,为回撤铉圈套器所需滑板相对于支承组件的移动量,乃是此铉圈套器相对于销套移动量的一半,因而,即使铉圈套器相对于销套的移动量约为4英寸,则外科医生只须使滑板相对于支承组件位移2英寸,而这是简单易行的。
尽管本发明的上述最佳实施例已然是作为铉圈套器予以描述的,但本发明的原理则适用于其它外科手术用夹紧装置,例如夹钳、取活体钳、回收网与机械碎石器等等。此外,要是销套是由电绝缘材料制成,而前述的线绳、杆以及连接器62是由导电材料制成,且有一电源的外部电连接器、一仪表或一测量装置接附到连接器62上(图1),则可在此夹紧装置的夹紧端完成遥控电刺激、烧灼或者其它的电-机械手术或作业。
图6与7以及8、9示明本发明的另三种实施例,其中用齿轮件来提供省去此夹紧装置的推抛运动所获得的机械效益。就这几个实施例来说,有关的夹紧装置用示意图作大略的说明。在此各实施例中,如图1~5所示实施例的情形,连接到铉圈套器或其它夹紧装置上的杆示明于51处,而销套则示明于60处。支承组件则标明于100处,终结于一拇指环102。这里的支承组件100包括两条轨104与106,上面的滑板108可作纵向(轴向)运动。此滑板包括有指孔110,令使用者借一只手抠住指孔110与拇指环102,便得以让滑板108相对于支承组件100运动。
在图6与7所示的实施例中,线绳杆51是定位于滑板108上而随它运动。在销套60上设置有一帽116,装配成可相对此滑板108作轴向滑动。滑板108内以可转动方式支承有两个小齿轮118与120,它们分别啮合于销套帽116上的齿条122与轨104上表面上的齿条124。
当滑板108相对于支承组件100运动时,取决于小齿轮118与120的直径,上述线绳依一个方向而销套便依另一个方向运动。作为一个例子,小齿轮118与120的尺寸可选择成,使滑板关于支承组件的相对运动量为销套60相对于杆(与铉圈套件)51位移量之半。在图6与7的实施例中,当滑板于近侧拉向拇指环102时,线绳51便在同一方向受拉,促使它回至销套内。同时,滑板108的这一近侧运动使小齿轮120顺时针走向转动和使小齿轮118反时针走向转动,而帽116则朝远侧方向运动,由此推进着销套环绕着回撤的铉圈套器(相对于支承组件100)。当滑板108朝远端前进时(朝图6中的左侧),小齿轮120即按反时针走向转动,使齿轮118顺时针走向转动而将销套驱动到右侧。于此同时,定位于滑板上的线绳51则朝远端移动(向左侧)。
在图8的实施例中,滑板108上装有与小齿轮118与120啮合的第三个小齿轮。此外,销套60则固定地定位到支承组件100的远端,使得在这一情形中只是线绳才可以活动。由于增设了此第三个小齿轮。在图8的实施例中,安装帽116与杆51随着滑板108相对于支承组件100的运动而按同向运动。换句话说,要是使用者将指孔110拉向拇指环102,则三个小齿轮118、119与120亦将把杆51拉向拇指环,即朝向支承组件110,而这总是将铉圈套器拉入销套60内的。在此实施例中,当滑板108朝远端行进(朝图8的右侧),小齿轮120顺时针走向转动,小齿轮119反时针走向转动,而小齿轮118顺时针走向转动,这样便将齿条116与线绳51也运动到远端方向(朝右侧)。换言之,线绳是在滑板运动方向中依不同速率运动。
图9与图8类似之处是销套60是不动地定位于支承组件100上,但这时的小齿轮118与120则依共轴方式装配。滑板108朝远端的运动(向右侧)使小齿轮118与120顺时针走向转动,而这又将使齿条116朝远端方向运动(朝右侧),即与滑板同向运动。
在图8与9所示的这两个实施例中,它们与图6与7所示的实施例相同,即小齿轮间的相对尺寸决定着此夹紧装置的机械效益,亦即决定着为起动此夹紧装置所需的推抛运动。预期对于图8与9所示的两实施例来说,可以获得高达4的机械效益,亦即为起动这种夹紧装置所需的推抛运动量将是此夹紧装置相对于销套的实际运动量的1/4。
本发明的外科手术装置可用任何合适材料制成。可以设想此种装置本身可以制成一次使用性的,这时的支承组件10与滑板可由聚碳酸酯或其它惰性塑料之类的适当塑性材料制成。
Claims (11)
1、一种外科用夹紧装置,它包括一销套、在此销套远端且可相对于此销套运动一预定距离而起动的夹紧部件、一支承组件以及一可相对此支承组件运动的滑板,所述滑板至少与上述销套与夹紧部件之一连接而得以使后者运动,改进之处在于它包括有这样的器件,能响应此滑板相对于所述支承组件运动一段小于上述预定距离的路程,而促使夹紧部件相对于销套运动过上述预定距离,用来起动此夹紧部件。
2、如权利要求1所述的外科夹紧装置,其中之前述的响应器件包括有同时使所述夹紧部件朝一个方向而使所述销套朝与之相反方向运动的部件。
3、如权利要求2所述的外科夹紧装置,其中前述的响应器件包括有将所述滑板连至所述夹紧部件的部件,且包括一连接到所说销套与滑板上的皮带。
4、如权利要求1所述的外科夹紧装置,其中所述的响应器件包括安装在前述滑板上且与所述销套或夹紧部件相啮合的可转动的齿轮。
5、一种外科用夹紧装置,它包括:
一支承组件;
一滑板,安装在上述支承组件上且可相对于该组件运动;
一线绳,可随上述滑板相对于所说支承组件运动;
在上述线绳远端的夹紧部件;
封装着上述线绳的一销套,此线绳相对于所述销套运动一段预定距离而使此夹紧部件起动;
器件,它可使滑板相对于支承组件运动一段小于所述预定距离的路程而起动夹紧部件的器件。
6、如权利要求5所述的外科用夹紧装置,其中所说的销套可相对于支承组件运动,而其中所说的器件则包括有可同时使线绳在一个方向而使销套在与此方向相反的方向运动的部件。
7、如权利要求6所述的外科用夹紧装置,其中所述的线绳是定位于滑板上,其中所述的支承组件包括近侧轴与远侧轴,而其中所说的器件包括一两端固定在所述滑板上的皮带,此皮带通过前述两根轴的上方并定位于销套上,由此,所说滑板在一个方向的运动将促使此皮带运动以及此销套朝相反方向运动。
8、如权利要求5所述的外科用夹紧装置,其中所述的销套可相对于支承组件运动,而其中所说之器件包括有连接到所说销套与支承组件上的齿轮,用来使此销套在逆滑板相对于支承组件运动的方向运动。
9、如权利要求5所述的外科用夹紧装置,其中所说之销套固定于前述支承组件上,所说的线绳则可相对于滑板运动,而所说的器件则包括有齿轮件,用来使支承组件运动一段距离,此距离大于滑板运动时滑板相对于支承组件的位移量。
10、如权利要求9所述的外科用夹紧装置,其中所述的齿轮件促使前述线绳在一与滑板相对于支承组件运动方向相反的方向运动。
11、如权利要求9所述的外科用夹紧装置,其中所述的齿轮件促使所说线绳按照与滑板相对于支承组件运动的相同方向运动。
