医学研究人员已经在发现适合于特定的诊断目的超声反差—增强介质的尝试中对固体、气体和液体的使用进行了广泛的研究。已对各种复合物质例如胶质密封的微小气泡、含有气体的脂肪、声定位的局部变性蛋白质和含有高氟化有机化合物的乳胶进行了研究以便开发具有一定的理想性质的介质,这些性质主要是在机体内的稳定性和提供超声图象的显著被增强的反差的能力。
称为“微小气泡”的气体的小气泡在用标准的超声成象技术产生的图象中很容易被检测。当微小气泡被注入血流中或机体的一特定部位时,它增强包含微小气泡的区域和周围组织之间的反差。
涉及反差—增强介质的大部分研究努力已经集中在使用极小的气体气泡方面。研究人员早已知道气体气泡因为具有当它穿过机体中时影响超声能量的独特的物理性质所以是一种高效的反差介质。游离气体气泡呈现的反差增强与液体或固体介质相比的优点将在以下讨论超声诊断技术的上下文中详细地描述。
尽管有已知的优点,但是气体气泡在例如血液或许多含水的静脉内液体这样的溶液中的快速溶解严重限制了它们作为超声反差—增强介质的使用。最重要的局限是微小气泡的尺寸和微小气泡在溶解到溶液中去之前存留的时间。
更仔细地考虑微小气泡的尺寸需求,气体气泡当然必须足够小,使气泡的悬浮不将栓塞的危险带入它们被注入的机体中。同时由通常在超声反差成象中使用的气体组成的极小的游离气体气泡迅速地溶解到溶液中去使得它们的图象增强能力只局限于注入区域的近邻。对于心血管系统的超声成象存在其它的障碍。医学研究人员已经研究了由普通空气、纯氮、纯氧或二氧化碳组成的微小气泡溶解到溶液中去所需的时间。这些气体的小到能够通过肺到达左心、直径小于约8微米的微小气泡其寿命约短于0.25秒。见Meltzer,R.S.Tickner,E.G.Popp,R.L.的“肺为何清除超声反差?”,《医学和生物学中的超声》第6卷第263,267页(1980)。由于血液通过肺需要多于2秒的时间,所以这些气体的微小气泡在通过肺的行程期间将完全溶解,到达不了左心。出处同上。主要是出于对气泡尺寸和寿命之间的折衷,许多研究人员认为游离气体微小气泡作为心血管系统某些部位超声诊断的反差—增强介质是无用的。
但是,这里描述的超声反差—增强介质包括由生物兼容气体——它们的选择也由本发明提供——组成的微小气泡。它们小到能够通过直径约为8微米的肺毛细管,因此可以对心脏的左心室进行反差—增强超声诊断。游离气体微小气泡在血液中的存留时间足够长,使它们可由外部静脉注射,通过右心、肺进入左心室而不溶解到溶液中去。此外,某些这样的介质能长期在溶液中存留并且可以对许多其它器官和组织进行反差—增强。
本发明克服了认为是使用游离气体气泡而存在的固有的许多限制,这是部分地通过提供根据特定的物理标准选择特殊的气体的方法来实现的,使得由这些气体构成的微小气泡并未遇到如在以前被研究的微小气泡所遇到的相同限制。因此,已经发现这里描述的包括由使用生物兼容气体产生的微小气泡的复合物或根据这里揭示的物理和化学参数选择的气体的复合物的超声反差—增强介质能存留足够长的时间并且具有足够小的尺寸,它们在血液中的稳定性使得对在以前认为游离气体微小气泡不能进入的机体的特定组织能够进行增强的超声反差成象。

为了充分理解本发明的主题,描述目前已知的超声成象技术和回顾在这一领域中对改进的超声反差—增强介质的探索是有益的。
用作超声反差介质的材料是由于当它们穿过机体并被反射而产生用于医疗诊断的图象时对超声波产生影响而起作用的。为了试图开发一种有效的图象—反差介质,本领域技术人员认识到不同类型的物质以不同的方式和不同的程度影响超声波。此外,由反差—增强介质产生的某些效应比其它效应更容易测量和观察。因此,当选择一种理想的复合物作反差—增强介质时,人们更喜欢当它们穿过机体时对超声波有显著的影响的那些物质。还有,对超声波的影响应当容易被测量。在超声图象中可被看到的主要反差—增强效应有三种:反向散射、波束衰减和声差速度。这些效应的每一个将依次描述。A.反向散射
当正在通过机体的超声波遇到例如器官或其它机体组织这样的组织时,该组织反射部分超声波。机体内不同的组织以不同的方式和不同的强度反射超声能量。被发射的能量被检测并被用来产生已被超声波穿过的组织的图象。术语“反向散射”指的是具有某种物理性质的物质将超声能量向源方向散向的现象。
很早就认识到由已知能引起大量散射的物质的存在可以增强在超声图象中观察到的反差。当将这样的物质提供给机体的不同部位时,机体这一部分和不包含该物质的周围组织的超声图象之间的反差被增强。众所周知,不同的物质由于它们的物理性质而引起不同程度的反向散射。因此,对反差—增强介质的寻求已经集中在稳定的和无毒的并呈现最大量的反向散射的物质上。
当对物质反射超声能量的方式做一定的假定后已推导出描述反向散射现象的数学公式。利用这些公式,熟练的研究人员根据已知的引起反向散射现象的物理特性能够计算气体、液体和固体反差—增强介质引起反向散射的能力以及特定的物质引起的可测的散射能够与其它物质比较的程度。作为一简单的例子,如果所有其它因素相同而物体A大于物体B,则物体A产生反向散射的能力将大于物体B 。这样,当超声波遇到这两种物体时,较大的物体将散射较大量的超声波。
物质引起超声能量散射的能力还依赖于物质的其它特性,例如它的能被压缩的能力。由于下述的气泡的谐振现象,由气体气泡引起的反向散射的显著增大具有特殊的重要性。当试验不同的物质时,比较被称为“散射截面”的物体引起反向散射的能力的一特定度量是有用的。
特定物质的散射截面正比于散射体的半径,并且还依赖于超声波的波长以及物质的其它物理特性,见J.Ophir和K.J.Parker的“在诊断超声中的反差介质”,《医学和生物学中的超声》第IS卷第4期,第319~323页(1989)。
一个小散射体a的散射截面能够由已知方程来确定: 其中K=2π/λ,λ是波长;a=散射体的半径;Ks=散射体的绝热压缩系数;K=包含散射体的介质的绝热压缩系数,ρs=散射体的密度,ρ=包含散射体的介质的密度。见P.M.Morse和K.U.Ingard的《理论声学》第427页,纽约McGrawHill出版社(1968)。
在估计各种物质作为图象反差介质的实用性时,可以用这一方程来确定哪种介质具有较大的散射截面并由此确定哪种介质在超声图象中提供最大的反差。
参看上述方程,为比较固态、液态和气态散射体起见,可假定上述方程中第一个括以括号的量为常数。可以假定固体微粒的压缩系数比周围介质的压缩系数小得多,该微粒的密度很大。利用这一假定,固体微粒反差—增强介质的散射截面已被计算为1.75。见上述Ophir和Parker的文章第325页。
对于纯液体散射体,散射体Ks的绝热压缩系数和密度很可能和周围介质K的接近相等,根据上述方程将得出液体具有零的散射截面的结果。但是,液体可以呈现一定的反向散射,因为上述方程中第一个括以括号的量中的项a可变得足够大,如果假定存在大体积的液态介质的话。例如,如果一种液态介质从一个非常小的容器流到一个非常大的容器使液体基本上占据了整个容器,则该液体可呈现可测量的反向散射。尽管如此,根据上述方程及其后的描述,本领域的专业人员可以理解,同游离气体微小气泡相比,纯液体是相当差的散射体。
已知在两相,即液/气之间界面处物质的声特性变化是显著的,这是因为在这一界面处超声波反射特性显著地改变。此外,气体的散射截面大大不同于液体或固体的,部分原因是气体气泡比液体或固体的更能被压缩。在溶液中的气体的气泡的物理性质是已知的并且普通空气的压缩系数和密度的标准值能被用于上述方程。利用这些标准值,上述方程中的第二个括以括号的项约为1014,见上述Ophir和Parker的文章第325页,总的散射截面随气泡半径a的改变而改变。此外,液体中的游离气体气泡将呈现振动运动,使得在某些频率处气体气泡将谐振在接近通常用在医疗成象系统中的超声波的频率上。结果是气体气泡的散射截面将比其物理尺寸大一千倍以上。
因此,已认识到气体的微小气泡是超声能量的极佳的散射体和理想的反差—增强介质,如果它们迅速地溶解到溶液中去的障碍能被克服的话。B.声束衰减
能够从某些固体反差—增强介质的存在而被观察到的另一效应是超声波的衰减。由于在某些组织类型之间的局部的衰减差,在传统的成象中已观察到图象反差。见K.J.Parker和R.C.Wang的“在从B—扫描图象中选择的区域内的超声衰减的测量”《IEEE学报Biomed.Enar.BME》第30卷(8期),第431~437页(1983);K.J.Parker.R.C.Wang和R.M.Lerner的“用于组织特性的超声幅值的衰减和频率的相关性”,《放射学》第153卷(3期),第785~788页(1984)。已经假定在注入一种介质之前或之后进行的组织区域的衰减测量可产生增强的图象。但是,根据衰减反差而用作测量液态分质的反差增强的手段的技术尚未得到很好的开发,即使已被充分开发,对于能够采用这一技术的内部器官和组织而言可能受到限制。例如,由于液体反差介质的缘故,不大可能在心血管系统的图象中观察到衰减的损失,这是因为在衰减的显著差别能被测量之前需要在给定的容器中出现的大容积的液体反差介质。
已经完成了由在玻璃试管中的Albunex(分子生物系统,圣地亚哥,加利福尼亚)微球引起的衰减测量并且已经暗示可在体内获得衰减反差的测量。见H.Bleeker,K.Shung,J.Burnhart的“关于用于血液流重测量和心脏灌注评估的超声反差介质的应用”《超声医学杂志》第9期第461~471页(1990)。Albunex是2-4微米的密封的充满空气的微球的悬浮物,这些微球已被观察到具有可接受的稳定性,而且其尺寸足够小,使反差增强能够在左前庭或左室产生。也已经观察到由聚集在肝的双碘苯甲丁二醯乙基酯(IDE)微粒引起的衰减反差。在这样的环境下,反差增强被确信是由在软介质中的密集微粒的存在而引起超声波的衰减造成的。出现能量被微粒吸收是由被称为“相对运动”的机制造成的。由相对运动引起的衰减的变化随微粒的浓度以及微粒和周围介质间的密度差的平方线性地增加,见K.J.Parker等人的“具有用于肝的超声成象的可能性的微粒反差介质”,《医学和生物学中的超声》第13卷第9期,第555~561页(1987)。因此,在出现固体微粒大量聚集的地方,衰减反差可以是用于观察图象反差增强的有生命力的机制,尽管该效应比反向散射现象具有小得多的幅值并且看起来在心血管诊断方面没什么用处。C.声差速度
根据声音速度变化取决于声音穿过的介质这一事实已经提出了增强超声图象中的反差的另外一种可能的技术。因此,如果声音穿过它时声音的速度不同于周围组织的足够大容积的介质能被注入到一目标区域,则可测出通过目标区域的声音速度之差。目前这一技术只是实验性的。
因此,考虑上述用于超声图象的反差增强的三种技术,由游离气体微小气泡引起的反向散射的显著增长是最引入注目的效应,具有这种现象的优点的反差—增强介质是最希望要的,如果它们在溶液中的有限的稳定性这一障碍能被克服的话。
根据对以上描述的各种技术的了解,开发其存在将在超声图象中产生显著的反差并在体内存留足够长的时间使心血管系统的反差—增强成象成为可能的反差—增强介质的努力导致研究广泛的各种物质—气体、液体、固体以及这些物质的结合——作为潜在的反差—增强介质。
A.固体微粒
一般来说,已被研究作为潜在的反差—增强介质的固态物质是被加工成均匀尺寸的极小的颗粒。大量这些微粒能够被注入血流并在其中自由地循环,或者可被注入机体中的特定的组织或区域之中。
IDE微粒是能大量生产的具有约0.5~2.0微米的相当窄的尺寸分布的固体微粒。这些微粒可进行无菌的含盐的注射并且倾向于聚集在肝内。一旦出现大量的聚集,反差增强就由衰减反差或反向散射机制呈现出来。虽然包含这些扩散在液体中的固体微粒的悬浮物可以呈现令人满意的稳定性,但和游离气体气泡相比,反向散射或衰减效应相对很小,并且在观察到超声图象中的显著的反差之前必须出现微粒的大量聚集。因此,这些悬浮物的使用已被限于到某些细胞种类,在这些细胞种类中微粒具有凝结的倾向,因为除非该悬浮物在特定的组织中变得高度集中,否则反差增强将是很小的。
SHU-454(Schering,A.G.,西柏林,德国)是一种粉末状的实验性反差—增强介质,当同糖类稀释剂混合时将形成尺寸为5-10微米的各种扁菱形和多面体形的晶体的悬浮物。虽然这些晶体增强超声反差的精确机制未被完全了解,但是可以推测这些晶体可以在它们的结构中捕获微小气泡或者这些晶体本身可以用尚未确定的机制反向散射超声能量。
B.液体和乳胶
在获取令人满意的介质的另一尝试中,通过将一种与机体组织兼容的化学物品和一种提供高的超声反差增强的物品组合在一起来制备乳胶。欧洲专利申请0231091公开了包含高氟化有机化合物的水包油乳胶,这种乳胶已经在它们可能用作血液替代品方面进行了研究并且还具有提供超声图象的增强的反差的能力。
对包含全氟辛基溴化物(PFOB)的乳胶也已经进行了试验。PFOB乳胶是已知具有输送氧的能力的液态化合物。PFOB乳胶作为超声反差介质已经显示出有限的实用性,因为它有在某些种类细胞中聚集的倾向。虽然机制尚未被完全了解,但由于PFOB乳胶有高的密度和相对较大的压缩系数常数,它们可提供超声反差。
美国专利第4900540号描述使用包含气体或气体先驱物的以磷脂体为基的脂质体作为反差—增强介质。脂质体是一种极微小的球状气泡,包含双层磷脂体和其它两亲的分子以及一内部含水腔,所有这些均同机体的细胞兼溶。在大多数的应用场合,脂质体用作生物密封活性材料。上述参考文献揭示了使用被包含到脂质体核之中的气体或气体先驱物来给被注入机体中的气体提供较长的寿命。生产稳定的脂质体是昂贵的和费时的过程,需要特殊的原料和设备。
C微小气泡
如上所述,用作反差—增强介质的微小气泡必须满足的关键参数是尺寸。大于约8微米的游离气体微小气泡也许还是足够小的,能够避免阻碍血液流动或堵塞血管。但是,当血液流过肺时,大于8微米的微小气泡将从血流中被除去。如上所述,医学研究人员已经在医学文献中指出小到能通过肺的微小气泡溶解得如此之快以致于不可能用游离气体微小气泡对左心图象进行反差增强。见Meltzer,R.S.,Tickner,E.G.,Popp,R.L.的“肺为何清除超声反差”,《医学和生物学中的超声》第6卷,第263,267页(1980)。
但是,由于认识到使用微小气泡作为反差—增强介质得到的好处是由于它们大的散射截面,所以相当多的注意力已经集中在开发包含在溶液中表现出稳定状态的微小气泡的混合物上。提高气体微小气泡的稳定性可以用几种技术来实现。
以下每种技术主要涉及在一种基质中使一群微小气泡悬浮,普通气体的气泡在该基质中比在血流中更稳定。
方法之一是在进行超声诊断时在被注入机体的粘性液体中产生微小气泡。在使用粘性液体背后的理论涉及降低气体溶解到液体中去的速率以及在这之后为气泡提供更加稳定的化学环境,从而延长它们的寿命的尝试。
对这一通用方法的几种变化已有描述。EPO申请第0324938号描述了一种生物兼容性材料的包含微小气泡的粘性溶液,例如人类的蛋白质。通过对粘性蛋白质进行声学处理使微小气泡在溶液中形成。通过减少气泡和溶液之间的表面张力,由化学处理或加热引起的蛋白质部分变性对在溶液中的微小气泡提供了更多的稳定性。
因此,上述方法可被看作是通过使用包含微小气泡的稳定介质提高微小气泡的稳定性的尝试。但是这些方法中没有一个涉及已严重限制游离气体微小气泡在超声诊断中的应用、特别对于心血管系统的气体的主要的物理和化学性质。这些方法中没有一个指出根据精确的标准对气体进行选择将具有产生其尺寸允许肺蒸散反差—增强超声成象的稳定的微小气泡的能力。
在溶液中的微小气泡的行为可根据形成气泡的气体和该气泡所在溶液的某些参数和特性用数学加以描述。根据溶液处于形成微小气泡的气体的饱和状态的程度能计算出微小气泡存留的时间。见P.S.Epstein,M.S.Plesset的“关于在液—气溶液中的气体气泡的稳定性”,《化学物理学杂志》,第18卷,第11期,第1505页(1950)。根据这些计算,当气泡尺寸减小时,气泡和周围溶液之间的表面张力增大是显然的。当表面张力增大时气泡溶解到溶液中去的速率迅速增大,因此气泡的尺寸越来越快地减小。这样一来,随着气泡尺寸的减小,气泡收缩的速率增大。最终的结果是包含普通空气的小的游离气体微小气泡的数目溶解得如此之快以致于反差—增强效应存在的时间极短。利用已知的数学公式计算出直径为8微米的小到可以通过肺的空气的微小气泡根据周围溶液的饱和程度将在190和550毫秒之内溶解。根据这些计算,研究肺排除超声反差介质的方式的医学研究人员已经计算了在人和犬的血液中氧气和氮气的微小气泡的溶解时间,并得出这样的结论:由于微小气泡寿命极短,游离气体微小气泡反差介质不可能对左心室进行反差—增强成象。
以气体气泡或溶解于液体溶液中的气体为特征的系统的物理性质已被详细地研究,这些性质包括在液体的气穴流中形成的空气气泡的扩散和在水中由气体气泡造成的光和声音的散射。
已经不仅在理论上研究了在液—气溶液中气体气泡的稳定性,见Epstein P.S.和Plesset M.S.的“关于在液—气溶液中的气体气泡的稳定性”,《化学物理学杂志》第18卷第1505~1509页(1950),而且还在实验上进行了研究,见Yang WJ的“在纯粹血液和等离子体I中的气体气泡的动力学”,《生物力学杂志》第4期第119~125页(1971);Yang WJ,Echigo R.,Wotton DR和HwangJB的“在纯粹血液和等离子体I中的气体气泡溶解的实验研究:稳定的气泡”,《生物力学杂志》第3期第273~281页(1971);Wang WJ、Echigo R.Wotton DR、Hwang JB的“在纯粹血液和等离子体II中的气体气泡溶解的实验研究:运动的气泡或液体”,《生物力学杂志》第4期第283~288页(1971)。液体和气体的物理和化学性质决定系统的动力学和热力学行为。影响气泡的稳定性、由此影响气泡的寿命的系统的化学性质是消耗、转换或产生气体分子的那些反应的速率和程度。
例如,当二氧化碳存在于水中时,在气体和液体之间被观察到的熟悉的反应就发生。当气体溶解到水溶液中时,由于二氧化碳气体的合水作用产生了碳酸。由于二氧化碳气体在水中极易溶解,因此将很快地扩散到溶液中去,气泡尺寸迅速减小。溶液中存在的碳酸改变水溶液的酸碱的化学性质,而且由于气体的溶解改变了溶液的化学性质,随着溶液变为饱和二氧化碳气体气泡的稳定性也改变。在这一系统中,气体气泡溶解的速率部分地取决于已经溶解在溶液中的二氧化碳气体的浓度。
但是,根据在系统中的特定的气体或液体的不同,气体可以基本上不溶解于液体,气体气泡的溶解将很慢。在这种情况下,已经发现通过检验气体的某些物理参数能够计算在气—液系统中的气泡的稳定性。
已经发现有可能识别极小气体气泡在水溶液中不起反应的化学系统。利用这里公开的方法,本领域的技术人员可以根据气体的物理和化学性质专门选择特定的气体用于超声成象。这些气体能被用来产生也是本发明的主题的反差—增强介质。微小气泡能用某些已有的使用普通空气的技术来产生而且能够如在普通的超声诊断中那样被注入。
属于本发明主题的方法要求按照这里提供的方程,根据气体或液体的内在的物理性质进行计算。具体地说,气体的密度、气体在溶液中的可溶性和又取决于气体的克分子体积和溶液的粘度的气体在溶液中的扩散率都被用于下述的方程中。因此,按照此处揭示的方法能够判断给定的气—液系统的物理性质、能计算气泡消失的速率和程度,并且根据这些计算能够选择有可能形成有效的反差—增强介质的气体。利用现有技术可以产生显著改善了的反差—增强介质,并用该介质来改善超声成象的质量和有效性。
为理解本发明的方法,推导描述气—液系统的参数以及当这些参数的一个或多个的值被改变时出现的对气泡稳定性的影响的数学关系是有用的。假定在初始时刻T0,半径为R0的气体X的一个球状气体气泡被放在一种溶液中,气体X溶解在该溶液中的初始浓度等于零。经过一段时间之后,气体X的气泡将溶解到溶液中去,此时它的半径将等于零。再假定该溶液处于恒温和恒压状态,处于特定气体的饱和状态的溶液的被溶解气体的浓度用Cs来表示。这样,在T0时刻,在溶液中的气体的浓度是零,表示气体尚未溶解,所有存在的气体仍然被包含在半径为R0的气泡之中。
随着时间推移,由于气泡中的气体的浓度和溶液中气体的浓度不同,当气泡中的气体通过弥散过程溶解到液体中去时,气泡将会收缩。在经过一段时间之后,气泡半径从初始半径R0到较小半径R的变化由方程(1)表示: 其中R是在时间T的气泡半径,D是在液体中的特定气体的扩散系数,ρ是形成气泡的特定气体的密度。
随后气泡完全溶解所需的时间T可从方程(1)中通过设R/R0=0,然后对T求解而得出:方程(2)
这一结果定性地指出气泡的稳定性即其寿命可通过增大初始气泡尺寸R0。或者选择高密度ρ,在液相中的较低溶解度Cs或较低扩散系数D的气体来提高。
在液相中的气体的扩散系数D同气体的克分子体积(Vm)和溶液的粘度(η)有关,如已知方程所示:(方程3)
将在方程(3)中给定的D表达式代入方程(2)将显示出气泡的稳定性可通过使用将具有较大的分子量较大克分子体积Vm的气体和具有较大粘度的液体来提高。
作为例子,可以把空气的微小气泡和包含由此处公开的方法专门选择的气体的微小气泡的稳定性作一比较。取在22℃水中的空气的D值为2×10-5cm2sec-1,比值Cs/ρ=0.02(Epstein和Plesset,出处同上),对于在水中(处于空气的不饱和状态)的空气气泡完全溶解所需时间t得到以下数据:

如果从肺毛细管到左心室的血液输送时间为2秒或大于2秒(Hamilton,W.F.编辑的《生物物理学手冊》第二卷第二节“循环”。美国生物物理学会,华盛顿特区,第709页(1963)),并且认为只有接近8微米或更小的微小气泡才小到能够通过肺,那么显然,这些气泡没有一个在溶液中具有足够长的寿命而作为用于左心室超声反差—增强成象的有用的反差介质。
本发明的方法可通过将在下面的描述中称为气体X的任何特定气体的性质同空气相比较来发现可能有用的气体。采用上述方程(2)和(3),对于将描述由在该给定液体中的气体X组成的微小气泡的稳定性的特定气体X,系数Q可以用公式来表示。由这一方法确定的特定气体X的Q系数的值可被用来确定气体X与空气相比作为一种超声反差—增强介质的实用性。
从上述方程(2),可以根据气体X和空气的物理性质写出一个方程,该方程描述在相同溶液温度和粘度的条件下,与相同尺寸的普通空气的气泡相比气体X的气泡完全溶解的时间:方程(4)或者如果气体X的D是已知的,方程(5)为明确表达该方程以便得到Q值,使气体X同空气进行比较,上述方程可以重写:方程(6) Tx=QT空气 其中为了作比较,假定22℃的水的溶液,空气在溶液中的密度,扩散度和溶解度都是已知量,将这些已知量代入上述方程,得出:方程(7) 对于对扩散度Dx未知的气体,将方程(3)代入上述方程,并且假定以下对于22℃的水的粘度项η约等于1.0CP,则得出:方程(8) 这样一来,当知道了气体的密度,溶解度和克分子体积之后,这一方法就可以计算Q系数值。
如果Q小于1,气体X的微小气泡在给定的溶液中的稳定性比空气的微小气泡要差。如果Q大于1,由气体X形成的微小气泡比空气的微小气泡更稳定,存留在溶液中的时间比空气气泡的要长。如果对于给定的微小气泡尺寸有其它性质相同,则气体X的微小气泡完全溶解的时间等于普通空气的微小气泡完全溶解的时间乘以系数Q。例如,如果气体X的系数Q是10,000,如果和空气的微小气泡相比,气体X的微小气泡存留在溶液中的时间要长10,000倍。假如这里标出的所有量都是已知的或能被计算,则对在任何溶液中的任何气体均可确定其Q值。
根据气体的化学结构,也许需要不同的确定或计算密度、扩散度和溶解度的各个参数值的方法。从已知的科学文献,例如由美国化学学会出版的《气体百科全书》或表格中可以找到或可能找不到这些参考数值。大多数气体的密度值很容易从例如CRC出版社第72版的《化学和物理学手册》这样的出处获得。此外,某些气体在水中的溶解度和克分子体积已被准确地测定。然而在许多情况中也许需要用以上描述的方法对克分子体积和溶解度的数值进行计算,以便提供用来确定个别气体系数Q的数值的数据。专门选择的气体的Q值的计算实例说明本发明的方法如何能被用于个别的气体。
一般来说,许多含氟气体在水中呈现出极小的可溶性,相当重的分子量、大的克分子体积和高的密度。为确定某些气体的Q值,各种气体的溶解度、克分子体积和密度被确定,这些值被代入上述方程(7)或(8)。氟碳化合物的气体溶解度的确定
这种用来计算氟碳化合物的气体溶解度的方法使用KabalnovAS,Makarov KN和Scherbakova OV.的实验数据的外推。见“作为确定氟碳化合物乳胶稳定性的关键参数的氟碳化合物在水中的溶解度”,《氟化学杂志》第50卷第271~284页(1990)。这些氟碳化合物的气体溶解度相对于具有4.0×10-6克分子/公升的水溶解度的全氟-正-戊烷来确定。对于无分支的氟碳化合物的同系物系列,气体的溶解度对在分子中存在的附加-CF2-基的数目的每一增大或减小可以通过将该值增大或减小约数值8来进行计算。克分子体积的确定
克分子体积(Vm)是按照Bondi A.的数据计算的,见“Vander Waals体积和半径”,《物理化学杂志》第68卷第441~451(1964)。气体的克分子体积能通过识别原子的数量和类型进行计算,该原子组成所讨论的气体的分子。通过确定存在于分子中的原子的数量和类型以及每个原子如何彼此键联,已知值可被用于每个原子的分子体积。通过考虑每一个单个原子的分布以及其出现的频率可计算特定气体分子的克分子体积。这种计算最好用例子来说明。
已知在烷烃碳—碳键中一个碳分子具有3.3立方厘米/克分子的克分子体积,按烯烃碳—碳键合的一个碳原子具有10.0立方厘米/克分子的克分子体积,当多个氟原子键联到一个烷烃碳时,一个氟原子具有6.0立方厘米/克分子的克分子体积。
考察八氟丙烷,该分子包含三个在烷烃碳—碳键中的碳原子(在3.3立方厘米/克分子中有3个原子)以及6个氟原子键联到烷烃碳(在6.0立方厘米/分子中有6个原子),因此八氟丙烷具有58立方厘米/克分子的克分子密度。
一旦密度、克分子体积和溶解度被确定,Q值即可用上述方程(8)进行计算。
下表列出根据以上详述的计算的几种气体的Q值。

一旦Q值被确定,各种气体用作超声反差—增强介质的实用性就能够通过确定由所讨论的气体组成的不同尺寸的微小气泡的寿命来进行分析,如上述表I中对空气所做的那样。取十氟丁烷的Q值,并考察不同尺寸的气泡在水中溶解所需的时间,只要用十氟丁烷的Q值乘以上述表I中的每一个时间值即能得到下述表III中的值:

注意表III中时间尺度是分而不是如空气的情况时的毫秒。所有十氟丁烷的气泡,甚至小到1微米的气泡都能被皮下注射,在到达左心室所需约10秒的时间内不溶解到溶液中去。类似的计算能够对具有任何系数Q的气体进行。稍大的气泡将能够通过肺并能够存留足够长的时间,使能够对心肌灌注和动态腹部器官成象进行检验。此外,如用这一方法识别的多种气体一样,十氟丁烷在小剂量上呈现低的毒性,因此在通常的超声诊断中作为反差—增强介质将具有显著的优点。
有几种方法可以人工控制产生微小气泡的悬浮物。在此被作为参考的美国专利4832941提供了一种产生具有小于7微米直径的微小气泡的悬浮物的方法,它是用一个三通开关喷溅一种液体、使该液体穿过大量的气体来完成的。虽然在实践中技术可以改变,但是,用三通开关人工控制使大量高Q系数气体悬浮以产生此处描述的反差—增强介质是一种最佳的方法。
使用三通开关装置的一般技术在用于对研究的动物进行免疫的通常的弗罗因德氏佐剂制备方面是熟知的。三通开关通常由一对注射器组成,它们都连接到一个腔。该腔有一个出口,可以直接从这一出口收集或注射悬浮物。
使用三通开关的技术可以不同于美国专利4832941中所描述的技术,因为在这一过程中使用了不同的气体。例如,如果在微小气泡的悬浮物被产生之前系统用普通空气清洗或用其它气体清洗,使用在这里描述的高Q系数气体可更有效。
在本发明的最佳实施例中,40~50%的山梨醇(D-葡糖醇类)溶液同体积约1~10%的高Q系数气体相混合,约5%的气体为最佳值。山梨醇是一种市售的化合物,当混合在水溶液中时将显著增加溶液的粘度。如从上述方程(3)看到的,较高粘度的溶液将延长溶液中微小气泡的存留时间。一旦注射,40~50%的山梨醇最好被保持为团块;在不超过允许的注射压力情况下尽可能保持完整。为了产生微小气泡的悬浮,大量被选择的气体在喷射器中被收集。一定体积的山梨醇溶液可以装在同一喷射器中。大量山梨醇溶液被抽入另一个注射器中,使两体积之和根据所希望的微小气泡的体积百分比产生适当的气体百分比。使用每一个都有非常小的孔径的两个喷射器,液体被喷溅为接近25倍或所需倍数的大气压力的气体以便产生其尺寸分布对于此处所述目的是可接受的微小气泡的悬浮物。当然,这一技术可以随意地稍加改变,只要能在所要求的浓度中获得所需尺寸的微小气泡的悬浮物。微小气泡的尺寸可用已知的方法目测或者用Coulter计数器(Coulter电子公司)来检测。见“用作回波反差介质的微小气泡的产生”,《临床超声杂志》,第14卷408页(1986)。
实例例1.使用十氟丁烷作为微小气泡形成的气体来制备一种超声反差介质。制备的溶液包含:
山梨醇 20.0g
氯化钠 0.9g
大豆油 6.0ml
吐温 20 0.5ml
水 q.S. 100ml
对一种滑腻的、透亮的黄色溶液强力搅拌。该溶液的10ml等分试样被装入一个10ml的玻璃喷射器中。然后该喷射器被接到一三通活塞。第二个10ml的喷射器被接到该活塞,1.0cc的十氟丁烷(PCR公司,Gainesville,FL)被送入空的喷射器。该活塞阀门对含溶液的喷射器打开,液相和气相迅速地混合20~30次。得到了一种奶白的、略有粘性的溶液。例2.在例1中得到的气体乳胶被用水来稀释(1∶10到1∶1000)、被放入血球计数器中并且在使用油浸透镜的显微镜下被检验。该乳胶主要由2-5微米的气泡组成。密度是每毫升原始未稀释配方50-100百万个微小气泡。
例3.制备例1的配方并以犬的模式执行回波心动描记法。用isoflurane将一条17.5公斤的杂种狗麻醉并设置监测器根据Keller,M W,Feinstein,SB和Watson,DD在“在外部静脉注射经声处理的反差介质后成功的左心室浑浊化:实验评估”(Am Heart J 114:570d(1987))中描述的方法测量ECG、血压、心率和动脉血液的气体。

例4如果特定气体的分子量是已知的,能被计算或能被测量,那么用作超声介质的特定气体的稳定性的上述具体的测定能被逼近。这一逼近是根据气体的Q值和分子量的对数值之间的线性关系的推断,如图1所示。根据该图,下述准则能被用来估计Q值:

这些实验指出计算的Q值和实验确定的值之间的良好的一致性。根据这些数据,具有大于5的计算的Q值的气体应当有可能作为超声成象的反差介质。例6.通过比较全氟戊烷和全氟己烷作为超声反差的有效性检测具有高Q系数的给定化学实体的物质状态和它作为超声反差介质的实用性的关系。全氟戊烷(十二氟戊烷)具有207437的计算的Q系数和在标准压强状态下29.5摄氏度的沸点。全氟己烷(PCR公司,Gainsville,FL)具有1659496的计算的Q系数和在标准压强状态下59-60℃的沸点。因此,在人的体温37℃时全氟戊烷是气体而全氟己烷是液体。
在4℃通过强有力的均质化形成全氟戊烷和全氟己烷(2%W/V)的水状扩散液。如在上述例5中指出的那样,在上述每种扩散物的试样加入之前和之后准备一塑料烧杯,盛有约1000ml的37℃的水以便模拟人的血液并被超声扫描。
当小于1.0ml的全氟戊烷扩散物与模拟的血液混合时,它们产生至少存在30分钟的极亮的超声信号。1∶10000稀释后仍然是可检测的。
与此相反,在相同条件下,用超声扫描检测不到全氟己烷扩散物的1.0ml试样,对即使10ml试样(1∶100稀释)也一样。
得出的结论是:根据是本发明的主题的方法,对于将要作为有效的超声反差的物质而言,当处于被扫描的机体的体温时高的Q系数和气态都是必不可少的。
虽然在某些方面参照其特定的最佳实施例描述了本发明,但对本领域的技术人员而言各种变化和改进是显而易见的。因此,不作限制性的解释的权利要求应当被看作包含了本发明的各种变化和改进,它们来源于被公开的发明主题。这样考虑之后,本发明允许本领域的技术人员确定作为超声反差介质的各种化学产品的适用性,其中的密度、溶解度和克分子体积是已知的。
Claims (16)
1.用于超声图象—增强的反差介质,其特征在于,所述的反差介质包含在生物相容含水液体载体中的含有全氟己烷的气体的微小气泡。
2.按照权利要求1的反差介质,其特征在于,所述的微小气泡的直径小于8微米。
3.按照权利要求1或2的反差介质,其特征在于,所述反差介质还包含粘度增强剂。
4.按照权利要求3的反差介质,其特征在于,所述反差介质还包含作为粘度增强剂的山梨醇的水溶液。
