发明领域
本发明通常涉及一种用于处理并显示医疗信息的系统和方法,更具体而言,涉及在网络环境中处理和显示病人数据。
发明背景
在现今的医疗环境中,医疗设备的各个部分被用于在例如医院危急护理或急诊部门监控或管理对病人的护理。例如,医疗设备如呼吸机一般被用于用呼吸气体使病人肺部通气,以在病人靠他或她自己进行呼吸的能力有些削弱时帮助病人。
为了正确管理一件医疗设备如呼吸机,护理者必须首先设置用于呼吸机的各种设置。控制呼吸机的一般所需设置的实例包括:峰值吸气压(PIP)设置—限定吸入空气期间的峰值压力;和正结束呼气压(PEEP)设置—限定呼出空气结束时的峰值压力。许多其它呼吸机设置亦可被控制,这依赖于特定呼吸机的能力。
类似地,医疗设备如呼吸机亦可被配备有各种传感器以使病人护理者可通过呼吸机来监控病人的状况。用于呼吸机的一般监控参数的实例包括:平均气道(airway)压力(MAP)—呼吸循环期间在气道内所测的平均压力;和吸入潮气量(TVi)—正常呼吸期间由病人吸入气体的所测体积。当然,不同类型的医疗设备可监控许多其它不同的病人参数。
此外,医院亦具有专用的实验室,一旦血液已从病人抽出,该实验室用来分析例如病人的血液。血液测试的结果可由实验室技师打印出来并被给予健康护理提供者,如负责病人的医生或护士。护理者然后可分析该结果并选择用于病人治疗的正确疗程。
发明概述
本发明者认识到,随着被用于管理护理和监控病人的医疗设备数量的增加,对处理并显示来自各种医疗设备的大量数据的有效方式的需要亦增加。还有,医疗提供者需要自由、远程地以有效方式输入和/或检索医疗观察、诊断和实验室结果。
另外,本发明者认识到以下愿望:用户能通过例如局域网和/或广域网如互联网来远程收集、处理并显示来自任何位置的一件医疗设备的数据并能使用一般可用的计算设备(如个人计算机,PC)。还有,对于设备,理想的是能以有效和可定制的方式在所选时间范围处理并显示涉及特定病人的病人数据,而不管数据的来源。
因此,用于显示从多个源得到的医疗信息的互联网兼容系统被描述。该系统包括用于从网络上的多个源采集与病人关联的数据的处理器。该系统亦对所采集数据区分优先顺序以便以所需顺序和/或在所需时间范围内显示。菜单生成器被用于生成复合窗口,其用于在第一窗口中显示被排序的所采集数据并在第二窗口中显示用户输入的医疗笔记、医疗实验室结果和呼吸机数据的至少一个。

详述
图1为依照本发明原理的通信网络的示例方块图。如图1中所示,通信网络1由带有被互连在一起的局域和广域网分级结构的IP(互联网协议)兼容网络表示。应指出,尽管本示例医院或医疗网络为IP兼容网络,使用其它计算协议如,但不局限于,例如X.25、帧中继、IBMSNA等的其它类型的网络如,但不局限于光学或无线网络,亦可被使用,如本领域的技术人员可容易理解的。此外,尽管所述示例网络为分层网络,但这不是本发明所要求的。提供网络上设备中的通信连接性的任何类型的网络体系结构可被使用。
如图1中所示,示例分级网络1的第一级包括医疗接口总线(MIB)2。MIB为在本地将医疗设备连接在一起的众所周知的医疗工业标准。如图1中所示,MIB 2典型地被用于互连病人房间内的医疗设备以管理对特定病人的护理并监控特定病人。各种医疗设备可通过MIB 2被连接;图1中所示的实例包括呼吸机6a、IV(Intravenous)泵8或其它医疗设备10。MIB的实例为由Siemens Medical System销售的Infinity PAN。
MIB 2典型地通过接口对接站(IDS)设备12被连接于第二级LAN网络3,从而对接于以太网兼容LAN网络3。较高级LAN 3可以是例如由Siemens Medical System销售的Infinity LAN。尽管不是必须,该较高级LAN 3典型地由医院内的特定部门来使用,如强化护理部门或外科部门等,这依赖于组织的大小。
尽管未在图1中示出,多于一个的MIB可被连接于第二级LAN 3,因此多于一个的病人可通过LAN 3被监控或给予护理。另外,医疗设备可被直接连接于较高级LAN 3。例如,如图1中所示,呼吸机6b和麻醉系统22被直接连接于LAN 3而无需经过MIB。
此外,LAN 3可被互连于医院LAN骨干4,其亦可以是以太网兼容的。该骨干网络4提供医院或医疗组织内各个部门之间的通信连接性;例如,将医院管理系统15与实验室系统17连接在一起。另外,医院LAN 4具有远程接入网关19,其提供通过例如互联网29从例如远程医生办公室23或远程护理地点24到网络1上各种系统和设备的远程安全接入。可选的是,远程地点亦可通过例如拨号电话端口、ADSL或其它类型的私人连接而直接接入远程接入网关19。如本领域众所周知的,取代如图1中所示为独立的,远程接入网关19亦可为待在以下描述的服务器20的一部分。
依照本发明的原理,中心服务器20驻留在LAN 3上,用于收集和处理来自呼吸机和网络1上其它医疗设备的数据以便显示和控制。本领域的技术人员可容易认识到,服务器20可驻留在网络1分级结构的任何级,这是因为在图1中总线或局域网的所有不同级以及远程地点可被互连在一起。服务器20的实例可以是由Siemens Medical System销售的Prometheus服务器。该服务器可由例如能运行Microsoft NT操作系统的计算机系统做主机。
图2以流程图的形式示出可依照本发明来实施的功能。服务器20首先如步骤202中所示建立与网络1上各种设备的通信。这是例如通过使用网络1上每个设备的已知IP设备地址和IP协议以及较高应用层协议来实现的,如本领域众所周知的。
一旦在服务器20和网络1上其它设备之间建立通信,服务器20开始从网络1上的各种设备采集所选病人数据。例如,在步骤204,服务器20可采集如正被监控的呼吸机参数的呼吸机数据和所选的用于每个换气单元(例如,网络1上的6a或6b)的呼吸机设置。服务器亦可从例如网络1上的另一个医疗设备(例如10)采集病人数据。
此外,服务器20可获得包括在图1的实验室系统17中被首先输入和存储的医疗实验室结果的病人数据。还有,服务器20可采集健康护理提供者输入的医疗笔记以便显示。
在步骤206,响应于用户请求的服务器20然后以用于显示的所需顺序和/或所需时间范围对在服务器20中存储的这些所采集病人数据进行区分优先顺序处理。服务器然后将生成复合窗口,以便同时显示,例如第一窗口显示被排序病人数据而第二窗口显示呼吸机数据、医疗实验室结果和用户输入医疗笔记的至少一个,如步骤208和210中所示。
在本发明的一个方面中,用户可使用如图1中所示的MicrosoftWindows兼容PC 26或Windows NT兼容PC 29或者能运行菜单生成程序如网浏览器程序(例如,Microsoft Internet Explorer或NetscapeNavigator等)的任何其它计算机来显示由服务器20获得的病人数据。就是说,只要可建立与服务器20的通信连接,用户可使用任何计算机上的网浏览器通过服务器20做出请求并察看病人数据。这是有利的,因为医生可例如获得从例如远程医师办公室23到网络1上特定呼吸机的接入,而无需接入专用的终端。当然,用户可简单地使用键盘和/或鼠标或任何其它用户接口设备以在用户计算机上输入用户选择或请求,如在本领域所知的。
服务器20因此能格式化从网络1上各种设备采集的病人数据以兼容于例如HTML(超文本置标语言)编程语言,从而在网浏览器上显示数据。服务器亦响应于例如用于做出请求、源自用户网浏览器的HTTP(超文本传输协议)命令。
图3示出依照本发明原理的病人概述屏幕的一个实施例的实例。用户可选择屏幕类型选择条302上的“Summary”标签301以激活来自例如用户计算机26的网浏览器27(图1)的概述屏幕300。概述屏幕300包括三个数据呈现面板305、310和315;各种导航控制图标如,例如307、308、330;光标350以及选择条302和324;时间和日期信息311和312;以及注释入口控制图标309。
图形倾向面板305显示被区分的多个用户指定和区分优先顺序的病人倾向数据。例如,单独的倾向参数用颜色来区分。另外,用于在面板305中所显示的每个参数的高和低标度界限均以相应的所区分颜色和参数标记被呈现的特定顺序而出现。
表格倾向面板310以表格形式显示多个用户所选病人数据。例如,数据被呈现在相应的行中,同时参数或设置标记320在左边而实际的数值数据322延伸到右边。最右列323表示最近采集的病人数据。所显示数据的分辨率由“Scale”选择图标321来确定,其指定从一列到下一列的所呈现数据的时间标度。该标度选择321亦确定面板305中图形倾向数据显示的精度。
在图形倾向面板305和表格倾向面板310两者中,病人数据均可包括来自呼吸机或网络1上任何其它设备的参数和/或设置,这依赖于用户选择。还有,在两个面板中,时间和倾向导航均由日期导航器(navigator)330、光标350、光标时间311和时间滑条352来提供。通过使用日期导航器330,用户可指定应在当前时间312之前的多少天集中图形和表格面板两者中的病人数据。例如,如图3中所示,用户已选择显示在当前日期2000年7月13日之前的5、6和7天已被存储的病人数据。然后用户可使用时间滑条352来聚焦于在日期导航器330中指定的几天内的特定时间段,因此感兴趣的特定时间段可被显示在屏幕上。随着滑条352被移动,光标350亦将被移动以示出时间线385内的所选察看时间。还有,光标时间显示字段311将示出对应于光标350位置的精确察看时间。
在另一个实施例中,用户可简单地在光标时间显示字段311中输入时间和日期以选择面板305和310两者中的病人数据将被集中用于显示的时间。光标350然后将自动被移至对应于在光标时间显示字段311中所输入时间的时间线385上的时间。
此外,注释功能由图3中所示的注释图标309来提供。图标309的选择允许用户在光标时间311处注释用于用户可能想要输入的任何观察的笔记。一旦注释被输入,注释提示图标例如380将被显示于时间线385上,从而使用户知道所注释的笔记存在于相应的时间。为阅读注释,用户可简单地选择注释提示。所选注释的全部文本然后将被显示。
组合数据面板315允许用户通过组合数据面板315顶部的另一个用户选择条324附加地选择以便显示病人医疗笔记、病人实验室结果以及包括呼吸机参数和设置的呼吸机数据的任何一个。
例如,图3示出“Notes”选项314已由用户选择时的实例。在此情况下,组合面板315然后示出为如由例如病人ID 325所示的所选特定病人而输入的医疗笔记。医疗笔记可包括由健康护理提供者输入的观察390和诊断391。医疗笔记信息亦包括医疗信息特定部分被输入的时间和日期以及由谁来输入,如图3中所示。
在另一个实施例中,响应于用户选择在选择条324上的“Labs”图标405,组合面板315可显示用于所选病人的实验室结果,如图4中所示。在此情况下,服务器20(图1)将与例如网络1上的实验室系统17通信以获得例如用于所选病人的最近实验室结果。例如,图4示出以表格形式显示三个最近实验室结果的组合面板315。滑条410被提供给用户以通过向上和向下滑动条410来察看附加的实验室数据。
还有,响应于用户选择在选择条324上的“Vent”505,组合面板315可显示用于所选呼吸机的所选呼吸机参数和/或设置,如图5中所示。所选呼吸机由呼吸机ID显示字段502示出。作为结果,组合面板315可显示例如由用户区分优先顺序的三个最近采集的呼吸机参数和设置,如图5中所示。

Claims (20)

1.在一种用于显示从多个源得到的医疗信息的互联网兼容系统中,设备包括:
用于从多个源的至少一个采集与病人关联的数据的处理器,该处理器对所采集数据区分优先顺序以便以所需顺序显示;以及
用于生成复合窗口的菜单生成器,用于在第一窗口中显示所述被排序的所采集数据并在第二窗口中显示用户输入的医疗笔记、医疗实验室结果和呼吸机数据的至少一个。
2.权利要求1的系统,其中呼吸机数据包括呼吸机设置和呼吸机参数的至少一个。
3.权利要求1的系统,其中处理器进一步对所采集数据区分优先顺序以便在所选时间范围内显示。
4.权利要求3的系统,其中光标被显示以示出所选时间范围内的所选时间。
5.权利要求4的系统,其中时间显示字段显示对应于所选光标时间的时间。
6.权利要求5的系统,进一步包括注释图标,用于允许用户输入用于所选时间段内所选时间的注释。
7.权利要求1的系统,其中菜单生成器响应于用户选择而选择用户输入的医疗笔记、医疗实验室结果和呼吸机数据的至少一个,用于在第二窗口中显示。

11.一种用于显示从多个源得到的医疗信息的方法,包括步骤:
从多个源的至少一个采集与病人关联的数据;
对所采集数据区分优先顺序以便以所需顺序显示;以及
生成复合窗口,其用于在第一窗口中显示所述被排序的所采集数据并在第二窗口中显示用户输入的医疗笔记、医疗实验室结果和呼吸机数据的至少一个。
12.权利要求11的方法,其中呼吸机数据包括呼吸机设置和呼吸机参数的至少一个。
13.权利要求11的方法,进一步包括在用户所选时间范围内显示所采集数据的步骤。
14.权利要求13的方法,其中光标被显示以示出所选时间范围内的所选时间。
15.权利要求14的方法,进一步包括显示对应于所选光标时间的时间的步骤。
