Description

技术领域
本发明一般涉及校正因眼睛屈光而产生的光学误差。在示范性的实施例中,本发明提供装置、系统、和方法,用于校正眼睛的光学误差,特别适合在原位角膜磨镶术(LASIK)、光性屈光性角膜切除术(PRK)等等时,用于眼睛的治疗。
背景技术
已知的激光眼外科手术,一般采用紫外或红外激光,从眼角膜除去间质组织的微观层。激光通常除去角膜组织已选择好的形状,常常是校正眼睛的屈光误差。紫外激光消融术导致角膜组织的光分解,但一般不会对邻近的和下面的眼组织产生显著的热破坏。受辐射的分子通过光化学过程,直接使分子间的键断开,破裂成更小的挥发性碎片。
激光消融术手术能够为各种目的,例如为校正近视、远视、散光等等,除去对准的角膜基质,以改变角膜的外形。角膜上消融术能量分布的控制,可用各种系统和方法提供,其中包括使用可消融的掩模、固定的和可移动的孔径、可控的扫描系统、眼运动跟踪机构等等。在已知的系统中,激光束常常包括一系列离散的激光能量脉冲,由落在角膜上的激光能量脉冲的形状、大小、位置、和/或数量,确定要除去的组织的总形状和量。可以用各种算法来计算用于角膜整形的激光脉冲模式,以便校正眼睛的屈光误差。已知的系统为使校正起作用,利用了各种形式的激光器和/或激光能量,包括红外激光器、紫外激光器、非秒激光器、波长倍增固体激光器、等等。这些激光系统的激光器,通常在治疗时送出一系列激光脉冲。
结合本发明的工作认为,已知的用于激光消融术治疗的方法可能不太理想。一般要求尽可能快地完成外科手术。但是,如果治疗太快,组织可能过热,有可能导致不希望有的并发症。延长治疗时间,对病人是不舒服的,也消耗外科医师的时间。还有,一些外科医师已经认为,延长治疗时间,能够改变组织的水合作用,从而使病人接受的校正量发生变化。
有鉴于此,需要提供能降低治疗时间,又至少避开一些已知系统限制的外科消融术治疗。
发明内容
本发明提供把脉冲能量照射在眼睛上的系统和方法,其中以脉冲光束照射在眼睛上的能量,在治疗时是变化的。脉冲能量的触发速率(firing rate)可以在治疗时,随照射在眼睛上的激光束脉冲能量作相关的变化。
本发明的第一方面,是提供用激光器治疗眼睛的方法。激光器产生消融的光能的光束。激光束的截面尺寸在治疗眼睛时是变化的。激光器的触发速率,随光束截面尺寸的变化作相关的变化。在某些实施例中,光束截面尺寸与触发速率可以改变和调整,以便在至少一部分治疗时间中,使照射在眼睛上的光束功率,保持在基本恒定的水平上。
在某些实施例中,光束截面尺寸可以在第一量值和第二量值之间变化,以激光束在第一量值上的第一脉冲照射在眼睛上的能量,低于以激光束在第二量值上的第二脉冲照射在眼睛上的能量。触发速率可以在第一量值上的第一触发速率,与在第二量值上的第二触发速率之间变化,而该第一触发速率比第二触发速率更快。

在一个实施例中,把一系列角膜消融术的激光脉冲引向眼睛,第一脉冲有第一量值,而第二脉冲有小于第一量值的第二量值,并且根据量值,改变该系列的触发速率,使第二脉冲产生比在第一脉冲时更快的触发速率。

具体实施方式
本发明对激光眼外科手术提高精度和效率,特别有用,激光眼外科手术,如光性屈光性角膜切除术(PRK)、光性治疗性角膜切除术(PTK)、激光原位角膜磨镶术(LASIK)、等等。最好是,本发明通过改进屈光治疗程序的治疗时间,能提高屈光手术的光学精度。因此,虽然本发明的系统和方法,主要在治疗眼睛角膜的激光眼外科系统方面说明,但应当指出,本发明的技术适合用在另外的眼治疗手术和系统,如在制造隐形眼镜透镜等等。
本发明的技术,能够容易地适合与现有激光系统结合使用。通过提供更快速(从而更不易犯错误)的方法校正眼睛的光学误差,本发明使雕刻角膜变得便利,被治疗的眼睛可以正常地接受需要的光学校正,有了改进的视力,对病人的不舒适度极小。
本文使用的基本恒定的功率水平,包括功率水平稳定在平均功率水平的约25%之内。

激光系统10可以包括另外的部件和子系统,本领域熟练人员是知道的。例如,可以包括空间的和/或时间的积分器,以控制激光束内的能量分布,如在U.S.Patent No.5,646,791中所述,该专利公开的内容收入本文,供参考。激光外科系统的消融术废气排放器/过滤器,以及其他辅助部件,对理解本发明不是必须的,无需为理解本发明而详细说明。
处理器22可以包括(或与之接口的)常规的PC系统,该PC系统包括标准的用户接口装置,如键盘、显示监视器、等等。处理器22通常包括输入装置,如磁盘或光盘驱动器、互连网连接、等等。这些输入装置常常用于从体现本发明任何方法的有形存储器媒体29,下载计算机可执行代码。有形存储器媒体29可以取软盘、光盘、数据磁带、易失性、或非易失性存储器、等等形式,而处理器22将包括存储器板和其他近代计算机系统标准的部件,以便存储和执行这些代码。有形存储器媒体29可以有选择地包含波前传感器数据、波前梯度、波前纵剖图、治疗图、角膜拓扑图、眼睛屈光测量、和/或消融术表。
图1A画出使用扫描激光束的一系列激光脉冲14a-14e对眼睛施行的消融术。该系列脉冲照射在眼睛E角膜C的治疗面积上。如图1A所示,脉冲14e和14d重叠。脉冲14c的截面尺寸,小于脉冲14b的截面尺寸。系列脉冲14a到14e按顺序照射在眼睛E上。
现在参考图2,用于把激光束14引向眼睛E的激光束传送系统16,常常包括许多透镜30,和一个或多个时间积分器32,时间积分器32可以使激光束上的能量分布变平坦(或修整)。激光器12如上所述,常常包括准分子激光器。
在一个示范的实施例中,可变孔径34改变直径和/或狭缝宽度,调整激光束14的分布,它理想的是包括可变直径的光阑和可变宽度的狭缝。棱镜36把激光束14分为多束小光束,这些小光束在眼睛E上可能部分重叠,使消融术边缘变光滑或使激光束每一脉冲的“凹坑”变光滑。现在参考图2和3,偏移模块38包括电机40,电机40改变偏移透镜42的角偏移并改变偏移的轴取向。因此,偏移模块38能有选择地引导激光束14到达角膜需要的侧向区域。使用激光束传送系统16及偏移模块38的结构及方法,在U.S.Patent Nos.6,331,177;6,203,539;5,912,775;和5,646,791中有更完整的说明,这些专利全部公开的内容收入本文,供参考。
现在参考图4,图上按照本发明的原理,示意画出激光系统10的控制系统。处理器22能实现激光系统10的精确控制,按激光治疗表52指定的表面形状雕刻。一般包括PC工作站的处理器22,使用存储在有形媒体29中的计算机程序,产生治疗表52。处理器22包括治疗程序库44,如在U.S.Patent No.6,245,059中所述,本文收入该专利申请全部公开的内容,供参考。在激光系统10内,嵌入与PC工作站进行电子通信的计算机58。或者,可以把PC工作站嵌入激光系统,PC工作站包括嵌入与PC工作站通信的处理器卡,以便指导眼外科。
嵌入的计算机58与多个传感器56及多个电机驱动器60进行电子通信。电机驱动器60与嵌入的计算机58耦合,按照治疗表52,改变传送系统16许多部件的位置及配置。例如,第一和第二扫描轴62、64控制偏移透镜的位置,使小光束在角膜表面移动。光阑电机66控制整个光束的直径,并在某些情形下,通过可变宽度狭缝,控制透射光束的长度。类似地,狭缝宽度电机68控制可变狭缝的宽度。狭缝角驱动器70控制狭缝绕它的轴的旋转。光束角驱动器72如上所述,在时间积分器作用下控制光束的旋转。处理器22在各个光学部件已经就位后,向激光器12发出命令,使它产生激光束14的脉冲,在眼睛E上造出需要的凹坑。治疗表52包括所有要组合的需要的凹坑列表,以便产生治疗作用。
定时器80放在并加在处理器的卡上,是Lab-PC-1200型卡,时标8253/8254。Lab-PC-1200型卡可从National Instruments of Austin,TX购得。在另一个实施例中,定时器50放在处理器22外面。定时器80受处理器22的计算机程序控制,并适合用于测量时间间隔。激光器12在电子学上与处理器22耦合。激光器12按处理器22发出的命令,根据定时器80测量的时间间隔触发。处理器22在至少一部分眼睛E的治疗时间中,改变激光器62触发的速率。
图5画出流程图,示意表明确定角膜治疗程序的方法。治疗程序136可从光学表面134计算,用于清除光学组织规则的(球面的和/或柱面的)和不规则的误差。确定光学表面134的方法和系统,在U.S.Patent Nos.5,163,934和6,271,914中说明,这些专利公开的全部内容收入本文,供参考。通过把治疗程序134与具体激光系统的激光消融术脉冲特征138组合,能够得出一张消融术的脉冲位置、量值、形状、和/或数量的治疗表52。准备该消融术表的示例性方法和系统,在共同待决的U.S.Patent Application No.60/189,633中说明,该专利于2000年3月14日申请,标题为“Generating Scanning Spot Locations forLaser Eye Surgery”,本文收入该申请全文,供参考。治疗表52可供选择地通过把个别脉冲分类来优化,以免在治疗程序被中断等等时局部变热,使不规则消融最小。最好是,把治疗表分类,在初始时用小直径脉冲照射眼睛,接着才用大直径的脉冲。或者,把治疗表分类,在初始时用大直径脉冲照射眼睛,接着才用小直径的脉冲,又,脉冲的顺序甚至可以是大小随机分布的。然后按照治疗表52,用激光消融术142消融眼睛。

对照射眼睛的每一脉冲直径和脉冲能量,图7说明性地概括了直到下一脉冲的延迟。对每一光束直径202列出了延迟206。以mm为单位的光阑直径202,给出在眼睛表面的光束截面面积204。对每一160mJ/cm2的脉冲的平均能量密度210,该激光束脉冲照射在眼睛的能量212,等于面积204与能量密度210的乘积。对每一列出的直径202,照射在眼睛上的功率214,是照射眼睛的每一脉冲212的能量,与激光器以Hertz为单位的触发速率208的乘积。例如,对5mm的光束直径,光束的截面面积是0.196cm2,则每一脉冲照射在眼睛的能量是31.4mJ。激光器12的触发速率208是12.99Hz,则照射在眼睛上的功率214是407.8mW。
如图7所示,对从0.5mm到4mm脉冲治疗的第一部分,激光器的触发速率是恒定的,而对从4mm到6.5mm脉冲治疗的第二部分,激光器的触发速率是可变的。最好限制激光器的触发速率,以改进系统的可靠性和防止系统变热。例如,如图3A所示,激光器的触发速率限制在20Hz。
激光系统10的触发速率可以按治疗表列举的值变化。例如,闭环系统测量若干如上所述的移动单元位置,并延迟激光系统的触发,直到若干移动单元的每一个就位。至少有一个移动单元的定位出现延迟,激光器脉冲也延迟,直到该单元正确就位。同样,眼睛跟踪系统可以根据快速移动的眼睛,或眼睛临时移出限制的范围,例如眼睛的眼球震颤,使治疗脉冲延迟。
图8对使用不同能量,以若干激光束脉冲的治疗,画出有近似恒定功率400mW的激光器照射在眼睛上的的触发速率。可以选择任何功率水平、任何光束直径范围、和任何触发速率范围,把需要的光功率量传送到病人P的眼睛E。如在图8所示,对眼睛上光束直径202为3mm,平均能量密度210为160mJ/cm2,提供的每脉冲能量212为11.3mJ。要向眼睛提供400mW的功率214,那么,在治疗时使用的触发速率是35Hz。对光束直径202为1mm,每脉冲能量212为1.26mJ,则使用的触发速率是318Hz。光束直径范围从约1到3mm,而激光器的触发速率从约35Hz到318Hz。
虽然上面给出本发明具体实施例的完整和精确的说明,但可以立刻对本发明作出若干变化和修改。例如,虽然特别参考了大小变化的激光束,但可以在治疗时用固定光束直径的脉冲激光器与变化的触发速率,在光束脉冲中向治疗区域传送变化的能量。还有,虽然已经说明使用若干种光束直径来治疗,但是,治疗时可变的激光触发速率,与只有两种激光束大小结合,也是需要的。因此,本发明的范围仅受后面的权利要求书限制。

Claims (13)
