Abstract

发明背景
1.发明领域
本发明总的来说涉及一种将聚焦能量施加到身体上的系统,该系统有选择地采用单能量施加器或多元微波施加器,以便利用高温来治疗前列腺组织中肉眼可见的肿瘤以及微观恶性和良性细胞。本发明的系统可用于治疗包含未检测到的微观高含水量病理学改变细胞(瘤形成)的健康组织,以防止发生癌变或癌症复发、癌前期或良性前列腺损害。另外,所公开的系统和采用该系统的方法可防止前列腺中肿瘤的生长,同时防止癌细胞向前列腺外扩散。
2.现有技术描述
为了采用高温来治疗前列腺肿瘤,需要对前列腺重要部位进行加热,并保护好前列腺中的健康组织以及包括病人的尿道和直肠壁在内的周围组织。在美国,每年检查出的前列腺癌的病例大约为200,000例,称之为BPH(增大的前列腺)的良性前列腺增生病例约为375,000例。BPH为前列腺非癌变增大,几乎所有男性随着年龄的增长,特别是超过50岁之后均会发生。在BPH情况下,前列腺的增大使得组织过度生长,最终阻塞了膀胱出口,导致排尿困难。在前列腺癌的情况下,最终癌症将突破前列腺囊,导致癌症扩散至骨骼和身体重要器官。尽管BPH的一些症状和前列腺癌相同,但具有BPH不会增加患前列腺癌的机会。然而,具有BPH的病人可能同时患有未检查出来的前列腺癌,或者在将来发展成前列腺癌。

首次公开的报告中所描述的一种用于对深层组织进行高温治疗的非自适应相控阵列为一项理论研究(von Hippel等,Massachusetts Institute ofTechnology,Laboratory for Insulation Research,Technical Report 13,AD-769843,pp.16-19,1973)。Rodler的美国专利申请US3,895,639描述了双通道和四通道非自适应相控阵列高温治疗电路。同样,Turner的美国专利US4,589,423公开了一种非适自应相控阵列高温系统。
Bassen等人在Radio Science,Vol.12,No.6(5),Nov-Dec 1977,pp.15-25中展示:可采用一种电场探头来测量组织中电场的分布,特别是该文章展示了几个例子,其中被测电场在中央组织具有聚焦峰。该报告还论述了一个用于实时测量生物体中的电场的概念。但是,Bassen等人没有将采用电声探头来实时测量电场的概念发展到自适应聚焦相控阵列。

前列腺癌
治疗前列腺癌的医疗护理通用标准包括辐射或神经保守(nerve-sparing)前列腺切除术,其中整个前列腺腺体被手术切除,并且近程放射治疗为将低剂量辐射源永久性植入前列腺腺体中,有效辐射为6至9个月,或者将高剂量辐射源暂时植入前列腺约2天,并结合外界波束放射治疗,以捕获已穿透或可能穿透前列腺囊的微观癌细胞。手术的副作用包括失禁和阳痿。术后癌症复发率5年内高达约35%,10年内高达约60%,特别是当前列腺特异抗原值(后面将讨论)高于10时。放射治疗具有短期副作用,如皮肤反应、疲乏和恶心。另外前列腺放射治疗的长期副作用包括小便失禁(膀胱控制能力丧失)和阳痿,以及损伤周围器官。

活检结果和分期用于预测癌行为和其扩散可能性。1期肿瘤较小,在直肠检查是不会感觉到。2期或更高指的是可能感觉到其中的肿瘤的前列腺。3期癌症可以扩散超出前列腺边缘。在4期中,可以通过成像研究,例如骨扫描、CT或MRI扫描确定,癌症已经扩散进入附近的淋巴腺、骨或身体其它部位。如在医学领域公知的是,癌症越早被检查出,癌症患者的生存机会越多。如果不能在2期检测出,接下来的最好医疗意见将会是保守治疗健康的组织。这样,总的来说需要从还未检查到癌症开始直到其达到2期或更晚期一直对健康组织进行治疗。
前列腺管癌具有四种类型:过渡型细胞癌、管内腺癌、混合型管癌和子宫内膜样癌。过渡型细胞和混合型管癌是浸润性癌,如果发现肿瘤被静静地限制在前列腺中,则需要完全切除前列腺和膀胱。完全切除前列腺和膀胱也是医学上治疗子宫内膜样癌可接受的方法。管内腺癌采用放射性前列腺切除术治疗。因此,需要一种系统来对不需进行外科前列腺切除的癌症进行治疗和防止其生长与扩散。
良性前列腺增生
良性前列腺增生(BPH)最初被描述成为前列腺增大,表现为压迫尿道,导致阻碍了排尿,并且通常困扰着中年和老年男性。65岁以上的男性约50%会具有BPH症状,明显地影响其生活质量。对美国泌尿协会(AUA)症状指数的研究有利于对BPH症状进行分类。AUA分数具有下述分数范围:0至7点——BPH症状被认为是轻度;8-19点——BPH症状被认为是中度;20-30点——BPH症状被认为是重度。但是,许多BPH病人在其AUA分数约为12之前不寻求治疗。
在过去的二十年中,对BPH的治疗数量有所提高,这一点有利有弊。BPH治疗系统的主要类型有:1)经尿道前列腺切除术(TURP),2)经尿道前列腺电蒸汽治疗(TVP),3)药物,4)间质激光凝结,5)RF探头部分切除,6)前列腺微波温热疗法。其它治疗技术也有探究,包括经尿道前列腺切开术,前列腺支架和气囊扩张术,但应用范围较小。
BPH治疗的成功性和实用性可由下述术语衡量:1)效果,2)持久性,3)疼痛程度(在操作中和操作后),4)恢复时间,5)操作复杂程度,6)操作费用,以及7)副作用。BPH治疗的效果通常采用AUA症状指数(SI)和小便流速峰值来定量表示。正常的小便流速约为16ml/sec。其它可选试验如剩余小便量和压力流动有时也用于判断效果。持久性为治疗效果保持的时间长短。疼痛程度主要涉及的是需要全身麻醉还是局部麻醉。恢复时间是用于衡量住院和在家休息的天数的术语。操作的复杂性是操作过程长短、完成该操作的个人(泌尿科医师或技术人员)的培训、所需麻醉类型以及治疗后Foley导管插入所需时间长短的函数。操作费用主要受操作过程长短和复杂性——特别是需要住院时间长短的影响。
直到约1990年,BPH主要治疗(“黄金标准”)为由泌尿科医师实施的经尿道前列腺切除术(TURP)。TURP费用昂贵,恢复时间长,并且具有一些明显的副作用,因而加速了对更好治疗技术的探究。下面将描述治疗BPH的总的途径,包括手术、药物和激光、RF,以及微波应用。
经尿道前列腺切除术(TURP):
BPH治疗“黄金标准”涉及手术操作过程,其中带有电外科回路的精密的经尿道仪器用于采用R F能量切除前列腺组织增大部分(主要为前列腺中央区)。实际操作中,90%的BPH外科操作过程涉及TURP,因为它的效果特别好(85%或更高),90%的病人具有长期持久性(10至15年)。美国每年要进行200,000数量级的TURP。TURP具有一定的弊端:它是十分疼痛的操作,需要住院2-4天和在家休息2-4周。进行TURP操作约需1小时,并且需要全身麻醉。操作过程由泌尿科医师进行。治疗后需要Foley插管约2-3天。TURP一些主要的潜在副作用包括阳痿、失禁、大量失血和逆行性射精。
开放式前列腺切除术:
开放式前列腺切除术主要用于具有较大前列腺的病人,其结果非常好:效果大于95%,持久性与TURP相同(10至15年)。采用任何外科操作,疼痛程度均很高,并且需要全身麻醉。大约需要住院7至10天,外加在家休息3-5周。操作过程需要几个小时,并且必须由泌尿科医师进行。之后的治疗为,需要采用Foley插管2-4天,以排空膀胱。开放式前列腺切除术的费用约为TURP的两倍,并且具有较严重的潜在副作用和综合症,包括大量失血、阳痿和失禁。
经尿道前列腺电蒸汽疗法(TVP):
经尿道前列腺电蒸汽疗法主要是对TURP的改进,采用开槽电外科滚动球电极来引导张开被前列腺组织阻塞的尿道。TVP操作过程与TURP相比更安全,并且副作用最小。TVP效果非常好(85%),但仍是十分疼痛的操作,需要住院2-4天和在家休息1-2周。泌尿科医师进行60分钟的操作,病人为全身麻醉。该操作之后需要使用Foley插管2-4天。操作费用比TURP略低。与TURP相比失血少,但它仍有阳痿、失禁和逆行性射精的潜在副作用。
经尿道前列腺切开术(TUIP):
对具有较小前列腺的病人而言相对较新的操作中,经尿道前列腺切开术的效果约为80%。但是,TUIP对较大前列腺无效果。在该操作中,尽量少地切除前列腺——沿前列腺的整个长度切开一个简单的切口。TUIP操作使得膀胱颈回弹张开,使尿液能够自由流出。持久性如期望那样与TURP类似,但临床研究仍在进行之中。该操作疼痛适度,仅需要住院1至2天,或对一些病人而言是门诊操作。通常在操作之后需要在家休息4-7天。泌尿科医师需要进行60分钟的外科操作,需要使用Foley插管2-4天。TUIP的费用大约与TURP相同。与TURP相比,上述操作的失血较少,但它仍存在阳痿、失禁和逆行性射精的潜在副作用。
气囊扩张术:
对于较小前列腺的病人,在前列腺尿道中使用气囊扩张可用于部分缓解BPH症状。其效果仅约为60%,持久性仅为1至5年。该操作比TURP的费用低,常用于门诊病人,需要在家休息几天即可。需要使用Foley插管2-4天。在该操作中可能会有一些出血,可能的副作用为感染和阳痿。对较大前列腺来说,该操作无效果。
支架:
对于具有较小前列腺的病情很严重的病人来说,采用支架在缓解BPH症状方面可具有良好的效果。持久性不是主要问题,因为这些病人常常同时患有其它疾病而病得很严重。该操作疼痛适度,仅需要局部麻醉,由泌尿科医师进行约30分钟的操作,并且如门诊病人那样在家休息4-5天。该操作的费用比TURP低。一些潜在副作用为有刺激、感染以及在支架上形成有碎屑。
药物:
治疗BPH的药物分为两类。一类药物采用α阻断剂(Hyrin或Cardura)来使前列腺周围肌肉松驰,使得排尿较好。其它类型的药物为还原酶抑制剂(Proscar),它实际上是收缩前列腺腺体。例如Hyrin具有很好的效果(74%),并且能一定程度上快速缓解BPH症状。但是,需要2-3周完全达到效果。临床数据建议,该药物至少具有3-5年的持久性,并且由一般医生简单地开处方即可。费用低于TURP,这取决于治疗年限。该方法可有一些严重的副作用,例如头晕、胸痛、不规则心跳,以及呼吸短促。
Proscar对较大前列腺作用较好,但对较小前列腺无效果。药物达到完全起作用需要约3至6个月,持久性丧失至少为3至5年。药物由一般医生简单地开处方即可,但需要使用至少12个月。药物费用低于TURP。一些公知的副作用为阳痿、嘴唇肿大、射精量减少以及皮疹。
间质(interstitial)激光凝结:
这里,间质激光凝结手术装置沿用户设计的光扩散器长度方向输送辐射性激光能量。扩散器产生椭圆形热损伤图形,将激光能量多向性或均匀地施加到前列腺中最大治疗组织体上。这是一种疼痛适度的外科操作过程,需要住院1或2天,然后在家休息1至2周。这种30分钟的操作必须由泌尿科医师进行,根据病人的情况选择全身麻醉或局部麻醉。这种操作严重的缺陷是需要1至2周较长的时间使用Foley插管来排出膀胱中的尿液。操作费用低于TURP。这种治疗有许多潜在副作用,包括阳痿,失禁,失血和逆行性射精。
RF探头部分切除(经尿道探头部分切除):
这种系统采用两个高能(近似0.47MHz)RF探头穿过尿道插入至前列腺中,从而在数几分钟内消融前列腺组织。在世界范围内,大约已有10,000位病人采用该系统进行了治疗,产生了良好的治疗效果。该系统仅有12个月的持久性数据,因此其长期效果不得而知。该操作疼痛适度(需要局部麻醉),并且作为门诊病人进行该操作需要1-2个月的在家恢复时间。该操作通常由泌尿科医师进行约30分钟。大约40%的病人将需要Foley插管约2-3天。该操作费用低于TURP。这种操作的主要副作用是有排泄刺激,勃起功能不良和逆行性射精。
由于这些公知的BPH治疗方法需要较高费用、伴有疼痛、外科手术或药物伴有潜在的危险的副作用,因此需要一种无痛治疗良性前列腺增生(BPH)的系统;该系统可在门诊基础上进行操作;以及使病人快速恢复其正常功能。另外,需要一种能够采用聚焦能量,在前列腺中形成明显数量的微观肿瘤之前安全治疗前列腺的方法。
发明概述
应用本发明的系统和采用该系统的方法能够解决已知治疗中存在的上述问题。按照本发明的系统和方法采用聚焦或聚集能量如微波能量,通过对前列腺腺体进行加热来安全加热癌前期、癌症、良性前期和良性疾病的前列腺,其中聚焦或聚集能量由相控非相干或相干阵列施加器经尿道和直肠输送,或者采用位于前列腺内的间质施加器。在非相干阵列中,分离的微波振荡器由施加器驱动,并且没有共同相位关系。在相位相干阵列(相控阵列)中,单个微波振荡器由多个具有共同相位关系的施加器驱动。
申请人的解决途径是,在前列腺中形成明显数量的微观肿瘤之前采用聚焦能量,例如微波能量来治疗前列腺腺体。如前所述,所有过去采用的热疗法均用于治疗具有中度至高度PSA值(超过4.0ng/ml)的已形成的前列腺癌,或用于治疗具有中度至重度AUA症状指数分数的BPH。本发明的优选实施例用于在需要检查之前,或在需要进行医学干涉之前进行预防。因此,本发明的方法用于治疗PSA值低于4.0ng/ml的前列腺癌,或治疗BPH症状低于重度或AUA症状指数分数低13的BPH。换言之,本发明的方法对用于预防癌症,或者预防BPH发展成为对病人来说为明显问题(即在严重不利后果出现之前)。
根据本发明,治疗前列腺的优选方法为采用相干自适应相控阵列,包括以下步骤:监视尿道和直肠壁的温度,将两个微波施加器朝向尿道和直肠其中之一,根据受监视的尿道和直肠壁的温度调节输送到前列腺中的微波功率,监视输送到治疗的前列腺的微波能量剂量,并且当所需总微波能量剂量已通过微波施加器输送时自动完成治疗。
非相干阵列或非自适应相控阵列高温治疗系统可用于加热半深度或深度组织,这取决于辐射频率。由于对高含水量组织,例如前列腺肿瘤进行的是非传导性加热,因此有可能采用非相干阵列或非自适应相控阵列来安全加热前列腺肿瘤。
进一步说,本发明的系统和方法可用于如下情形中:没有明确的位置来放置温度反馈传感器,或者希望避免将温度探头插入至前列腺组织中。在单个施加器的情况下,不需要电场探头(或电场传感器),这样本发明的优选系统和方法中不需要介入性探头。本发明的系统和方法可采用由聚焦能量产生的热量来消灭所有前列腺癌前期细胞和癌细胞,或良性损伤,从而避免癌细胞或良性损伤进一步发展。
另外,本发明的方法可用于增强放射治疗效果,或采用或不采用热敏脂质体来进行靶向药物输送和/或目标基因治疗输送,如Fenn的美国专利US5,810,888所述的那样。
本发明的方法消灭前列腺中的癌前期、癌症和良性前期细胞,同时保护正常前列腺组织。这样,本发明的系统和方法实现了热疗前列腺切除,并避免损害健康组织。因此,本发明的方法是前列腺保存技术。
可以采用远离经尿道和经直肠施加器的温度探头传感器来测量尿道和直肠壁的温度,以获得尿道和直肠壁的实际温度。备选的是,可采用本领域公知的外部装置,包括红外、激光、超声、电阻抗断层摄影术、磁共振成像和辐射测定技术来监视组织温度。
备选的是,可将温度探头插入前列腺内适当深度,以监视那里的温度。如后面将描述的那样,温度探头的插入不是优选实施例。
在具有两个或多个能量施加器的实施例中,插入前列腺中的介入式电场探头可用来或不用来测量输送到治疗组织的微波功率,以确定聚焦能量治疗时间的长短。在优选实施例中,介入式电场探头可用于将施加的能量聚焦到插入到前列腺中的电场探头上。
在备选实施例中,对于相干相控阵列来说,两个电场传感器可非介入式放置在前列腺尿道和直肠上,并用于使电场在尿道和直肠中保持为零,将微波辐射有效地聚焦在前列腺组织中。另外,可以对用于经尿道和经直肠施加器的微波相位进行调节,这样微波能量扫描过前列腺区域。
本发明的系统和方法可以使前列腺压缩或不被压缩。在优选方法中,病人的前列腺由于尿道气囊和直肠气囊其中之一膨胀而被压缩。聚焦能量和前列腺的压缩使得前列腺中高含水量的前列腺癌和良性细胞与周围低含水量的正常前列腺组织和前列腺周围组织相比被优先加热。
为了在前列腺中相干聚焦能量,例如微波能量,可借助尿道和直肠气囊压缩病人的前列腺,以及采用用于确定将能量聚焦于病人前列腺具体位置的装置。用于确定能量聚焦位置的装置可以是插入到前列腺中央部分的单个电场探头,或者是两个位于尿道和直肠壁上的非介入式电场传感器。接收信号的探头或传感器可用于测量反馈信号,以调节输送到位于尿道和直肠内的施加器上的能量相位。
根据本发明的另一实施例,压缩前列腺的步骤可保持在微波导入加热步骤之后。这就是说,在微波导入加热步骤之后使前列腺的压缩保持一段时间,以降低前列腺血流,并聚积额外的当量热剂量。可通过保持尿道和直接区域内的气囊插管膨胀加压来实现前列腺压缩。用于减少血流的周期性前列腺压缩可用于在温热疗法期间结合在前列腺区域中进行化学治疗、热敏脂质体包囊化学治疗或基因治疗。
本发明的系统和方法提供的治疗与现有治疗相比的主要优点列表如下:
1.阻止和消灭前列腺肿瘤(包括癌和良性);
2.迅速缓解可能存在的任何BPH症状;
3.持久性长久;
4.仅可能有程度很低的疼痛;
5.门诊操作;
6.局部麻醉;
7.不需Foley插管;以及
8.没有明显的副作用或并发症。
如受让人Celsion公司在1999年做的临床试验证明的那样,组合压缩气囊膨胀和微波加热在前列腺尿道中可以形成生物支架(微波尿道成形术)。其结果是,克服了现有BPH治疗的主要不足之一,即需要使用Foley插管数天,目前不再需要Foley插管,并且病人可感受到BPH症状立即缓解。
如后所述,申请人发明的系统和方法涉及监视输送到治疗前列腺的微波能量剂量,并基于已接收到的总微波能量剂量来完成治疗。这就是说,传统的肿瘤热剂量温度反馈测量可以用输送到相干相控阵列或非相干微波施加器,然后输送到治疗区域上的总微波能量来代替。因此,在目前的发明中,为了替代需要将温度反馈探头插入到前列腺中而进行的温度反馈测量,并解决其固有问题,采用微波能量剂量作为反馈来确定治疗时间长短。在该申请中,术语“微波能量剂量”(焦耳或瓦-秒)类似于放射治疗中采用的剂量,即放射吸收剂量(Rad),是放射剂量单位,定义为每克组织100焦耳能量的沉积。
这样,用于选择加热前列腺癌和良性疾病的瞬时方法避免了癌细胞扩散的危险,因为不再将温度探头插入到前列腺治疗区域(肿瘤床(tumorbed))中。省掉插入的温度探头减少了病人因插入探头而感染的危险。同样,施加到肿瘤上的微波场将不再遭受温度探头,特别是金属探头引起的散射或其它干扰。另外,还节省了插入温度探头所需的时间和费用。
本发明的系统和方法还可用于治疗健康前列腺组织或未检查出的表面上看为健康前列腺组织的高含水量微观癌前期细胞或良性前期细胞,以防止发生或复发前列腺癌变情况。这样,本发明的系统和方法将能够在检查出来之前,将前列腺腺体中与前列腺腺体(例如80%)相比含水量较高的癌前期细胞或良性前期细胞(例如90%)消灭或消融。这将是早期治疗,可以防止前列腺中肿瘤生长和从前列腺扩散,或前列腺增大。在看起来为健康组织的情况下,对前列腺组织进行照射,即将微波能量聚焦到含水量高的微观细胞上,这些细胞已知将受到损害,同时健康的低含水量前列腺组织不受伤害。
如果同时采用经尿道和经直肠施加器,并且同时分别扩张气囊,则优选的实施例具有压缩和固定前列腺的装置,以减少组织穿透深度,并且降低血流。
在备选方法中,采用单个经尿道气囊来压缩前列腺,同时固定前列腺组织,从而降低血流,减少微波辐射所需的穿透深度。压缩气囊由微波可透过塑料材料,如胶乳制成。可采用超声传感器或其它类型的图像导引来实现电场反馈探头在前列腺中的定位。在优选方法中,还可以采用在前列腺中注射局部麻醉药物利多卡因和ephinephrine或抗血管生成药物来实现进一步减少失血。
两个微波施加器(如Sterzer的美国专利US5,007,437所描述的那样,该文献在此通过参考被引入本申请)可经尿道和经直肠定位。采用2个或更多个施加器实现相控阵列。在优选实施例中,将相干915MHz微波功率输送到两个经尿道和经直肠施加器上,在预定功率值下,调节每个通道的移相器,使微波能量最大并且聚焦在电场探头传感器上。插管和气囊中的水冷却器能够使尿道和直肠壁冷却。可在前列腺中另外插入一个间质(interstitial)施加器,以提供由经尿道和经直肠施加器产生的热量。
在高温治疗过程中,输送到每个施加器上的微波功率值可以手动或自动调节,以避免过高的温度会引起尿道或直肠壁的烧伤或起水泡。另外,前列腺压缩量(如果采用压缩的话)可在治疗期间根据需要调节,以便使病人感觉舒适。每次调节前列腺压缩量时,微波能量/相控阵列需重新聚焦,从而使电场探头传感器接收到最大值。输送到微波施加器的总微波能量从开始治疗起在治疗期间受到监视。当所需总微波能量输送到微波施加器时,治疗完成,表明受损细胞明显被消灭(即热缩)或完全被消灭(即前列腺热切除)。
为了确定治疗效果,可在施予微波总能量剂量之前和之后采用X射线,超声和磁共振成象其中之一对前列腺组织进行成像,同时采用针刺活检前列腺组织得到病理结果。
在本发明的备选实施例中,单个介入式电场探头被定位于尿道和直肠的两个电场传感器替代,并且通过使两个传感器接收的单功率或组合功率为最小(零)来对相干阵列进行相位聚焦,从而提供完全的非介入式治疗。在优选实施例中,两个电场传感器包含在附着于压缩气囊外表面上的插管中,其中压缩气囊对尿道和直肠壁提供压力接触。结合电场传感器感应到的反馈信号来应用算法,使尿道和直肠壁外为空白区,从而将施加能量聚焦到内部位置。在完成了空白运算之后,可抽出电场传感器,并可将温度传感器插入,测量尿道和直肠壁温度。
这种总的来说为非介入式的高热治疗,采用电场传感器和温度传感器来监视尿道和直肠壁,提供了消灭前列腺中良性和癌症损伤的有效方法。在采用非相干施加器的实施例中,不需要采用电场聚焦探头和移相器。采用非相干能量时,仅叠加施加器辐射功率,而不采用移相器。
尽管已参考适当的微波相控阵列技术来描述的实施例,但本申请人的系统和方法可通过聚焦能量,总的来说为加热和消融组织区域来实现。聚焦能量可包括电磁波,超声波或在一定辐射频率下的波。这就是说,本申请人发明的系统和方法包括任何可经聚焦来加热和消融组织区域的能量。这种能量,例如微波或超声能量可为相干或非相干的。在另一实施例中,能量可来自于流体或激光施加器。
在采用相干相控阵列的本发明另一实施例中,身体中被治疗的组织(例如前列腺)区域的边缘可计算出,可将电场探头插入到体内,或将至少两个电场传感器放置在尿道和直肠内;并且通道施加器将能量施加到治疗区域。在该实施例中,能量的聚焦可以改变,这样聚点扫过治疗区域。这就是说,当施加能量的相对相位进行调节时不再有固定焦点,这样聚焦在治疗区域内移动,从而获得加热几何形状。
通过适当的运算来确定固定焦点。然后,例如施加器为了获得该固定焦点的相对相位以一种方式(正向)调节30°,然后以其它方式(反向)调节30°,以“扫描”较大的加热/治疗区域。根据治疗区域的尺寸,扫描可聚焦在180°和90°或60°或120°之间。

优选实施例的详细说明
前列腺腺体和其微波性能的说明
前列腺腺体220为男性生殖系统的一部分,它为固体的胡桃状器官,由尿道205的第一部分包绕,紧挨在膀胱202之下,直肠210之前。前列腺癌由前列腺腺体引起,并且前列腺癌最常见的形式为公知的腺癌,意思是腺体癌。大多数前列腺癌在前列腺底部(有时指边缘区域,这里包括70%的腺状前列腺)内,靠近直肠处生长,因此这是需要重点治疗的区域。尽管手指直肠检查对检查前列腺腺体中的变硬区域或隆起很有用,但它对检查微观前列腺疾病来说不十分有用。采用经直肠施加器(外加经尿道施加器)达到前列腺的该部分是全面治疗前列腺的基础。前列腺的中央区域(最靠近膀胱)对BPH和前列腺癌疾病来说是相对免疫的。BPH主要出现在中央区域和边缘区域之间的过渡区域。

工业,科学,医学(ISM)波段902-928MHz下的微波辐射常常用于商业临床高热系统中,并且在此考虑采用初始频率波段。已知的是,前列腺为高含水量组织,因此与具有良好特性的肌肉组织类似。对于正常前列腺组织在915MHz,平均介电常数为50,平均传导率为1.3S/m。由于915MHz平面波穿过前列腺组织传播时变薄而计算出的损失近似为3dB/厘米。前列腺上皮瘤形成,也称为非典型性增生和管内发育异常是一种癌前期,与前列腺腺癌的生长有关.赘生细胞假设为一种与周围正常前列腺细胞相比含水量高的细胞,将比正常健康前列腺细胞加热得快.前列腺中正常管状组织假设处于低至中含水量范围。
采用辐射频率(微波)电磁场来治疗癌症的安全性已成为一种问题.全面的研究最近得出结论,在癌症的影响或升级和暴露于频率范围为3KHz至300GHz内的辐射频率电磁场中之间没有必然联系(L.N.Heynick,Radiofrequency Electromagnetic Fields(RFEMF)and Cancer:AComprehensive Review of the Literature Pertinent to Air Force Operations,AFRL-HE-BR-TR-1999-0145,United States Air Force Research Laboratory,Directed Energy Bioeffects Division,June 1999)。这样,基于该报道申请人认为,有明显证据表明前列腺微波治疗可安全地对包含有微观癌细胞的表面健康的前列腺腺体进行加热,这样微波治疗的结果是不再有新的癌细胞形成。
加热前列腺组织的系统
图1示出了加热前列腺组织中癌细胞和良性肿瘤的优选系统,采用适当的能量,具有适宜的微波,具有电场和温度反馈的相控阵列高热系统。为了在能量频率下可靠地对深层组织进行加热,需要采用两个或更多个由合适的相控阵列运算控制的能量施加器110,111(分别位于尿道205和直肠210内)来包围身体(前列腺凸叶(lobe)220)。能量施加器110,111可能是相干微波施加器。黑色圆圈表示焦点190,代表前列腺220的需要治疗的肿瘤或健康组织中心。
焦点190可代表前列腺的癌变疾病,包括腺癌,癌肉瘤,横纹肌肉瘤,软骨肉瘤和骨肉瘤中的一种,或为癌前期状态,包括前列腺上皮内瘤形成中的一种,和良性前列腺损伤,包括良性前列腺增生。另外,本发明的系统可治疗表面上健康的组织,以防止出现或复发癌变或良性瘤病变。
在优选实施例中,电场反馈探头175可插入到受治疗的前列腺220组织适当深度处。可以超声变换器的导引下进行电场反馈探头175的插入。用于设定输送到每个施加器110,111的初始能量相位的装置包括来自电场探头175的电场反馈信号450和具有适当算法的计算机250,以便将能量辐射聚焦到插入的电场探头175上。优选的是,电场探头175与自适应相控阵列快速加速梯度搜索算法一起使用,如Fenn的美国专利US5,810,888那样,以便将能量辐射靶向在肿瘤位置190。
另外,本发明的系统包括设定输送到每个能量施加器上的初始能量或微波功率的装置,以及监视与受治疗的前列腺相邻的尿道和直肠壁的温度的装置,以确保这些壁不会过热。用于监视尿道和直肠壁的装置可以包括非介入式地贴着尿道和直肠壁(215,216)插入的温度反馈传感器410,以监视邻近前列腺组织的尿道和直肠壁的温度。温度反馈传感器410将温度反馈信号400发送给计算机250,在此信号用于调节输送到施加大110,111的相对微波功率值,以加热聚点190处的肿瘤或组织。
优选的是,根据Sterzer的美国专利US5,007,437设计经尿道和直肠能量施加器,优选为微波施加器。经直肠施加器,具体地说可采用一个反射器或相控阵列来将微波能量选择性地直接朝向前列腺。施加器可以是非介入式施加器,例如波导,单极或双极天线,或例如为单极或双极天线的间质施加器109(见图3)。在优选实施例中,施加器可以作为一个相控阵列被相干驱动。另外,多个施加器108包围着前列腺220(见图4),可在可用于选择性加热前列腺组织的多频率阵列中非相干驱动。
优选的是,身体或前列腺220在分别包绕经尿道和经直肠施加器110,111的两个气囊112,113之间被压缩。压缩气囊112,113可由蒸馏水或去离子水膨胀。在备选方案中,压缩气囊112,113可充气膨胀,或采用其它公知的装置来膨胀气囊。压缩气囊112,113由例如乳胶等可透过微波的材料制成。除了固定前列腺组织和固定施加器的位置以外,前列腺压缩对于高热治疗来说还有一些潜在优点。采用前列腺压缩可减少实现深度微波加热所需的穿透深度,并能够减少血流,这也提高了加热组织的能力。
向前列腺组织内注射局部麻醉药物,例如利多卡因的同时还注射ephinephrine,或抗血管形成药物也可用来减少血流。在本发明的优选实施例中,前列腺压缩技术和药物治疗同时用于减少前列腺腺体中的血流,从而能够快速加热微观、恶性和良性癌细胞。这种优选方法可作为PSA值低于4时预防癌和BPH的手段来实施,和用于AUA症状指数分数低于13时预防BPH。
从前列腺的内侧和外侧压缩前列腺使前列腺腺体的表面远离微波施加器辐射器移动,这样有助于减少表浅层热聚集点。在优选实施例中,施加器可有一个流体充盈腔,该充盈腔可提高施加器与治疗组织之间的微波能量匹配。在经直肠施加器或施加器气囊内的冷却流体,例如蒸馏水或去离子水在高热治疗中有利于避免在尿道205和直肠210中的热聚集点增多,从而保护尿道和直肠壁不会过热。
在自适应相控阵列高热治疗之前,前列腺在压缩气囊112,113之间被压缩,并且单个介入式电场反馈探头175平行于微波施加器110,111的极化方向被插入到前列腺中的中央组织位置(焦点190)之中。微波施加器110,111为直线型或螺线型单极或双极天线辐射器。当采用自适应相控阵列梯度搜索算法来调节移相器,使反馈信号为最大时,电场探头175用于监视聚焦电声振幅。非介入式温度传感器410监视分别在位置184,185处的尿道和直肠壁温度,并且这些信号作为温度反馈信号400被单独传送给计算机。
温度传感器410的尖端可附着到经尿道和经直肠压缩气囊112,113的外侧,只要尖端被热隔离。可通过在温度探头和气囊的外侧表面之间安装一个薄垫(未示出)来实现热隔离,以达到包含在压缩气囊中的冷却流体的效果。本发明的双施加器自适应相控阵列与电场反馈探头一起允许移相器可以调节,这样可产生聚集的电场,允许对组织内合适深度进行聚焦加热。
优选的是,温度传感器410被无创伤地插入穿过尿道205和直肠210的开口,这样传感器410分别与尿道和直肠壁加压接触。因此,如图1所示,两个温度反馈探头传感器410分别位于尿道205和直肠210中,并产生温度反馈信号400。两个带有微波施加器110,111的微波水冷插管300,301分别定位于尿道205和直肠210中。经尿道插管300包含一个在膀胱202中充气膨胀的Foley气囊118,以便将微波施加器110相对于前列腺靶区固定在正确位置。
对于相干治疗而言,振荡器105在节点107被分开,并送给移相器120。振荡器105在优选方法中为频率近似915MHz的微波能量源。相位控制信号125将微波信号的相位控制在0至360电量度范围内。来自每个移相器120的微波信号送到微波功率放大器130中。最终的微波信号由计算机产生的控制信号135控制,其中计算机产生的控制信号设置输送到每个微波施加器的初始微波功率值。呈现相干微波功率的微波信号150由微波功率放大器130输送给两个施加器110、111,同时在每个通道中的移相器调节成使电场探头传感器175处的能量最大并且聚焦,这样在聚焦位置190处的微波功率最大。然后开始进行治疗。
在另一实施例中,用于监视前列腺温度的装置为将前列腺组织插入适当深度内的温度探头。在这种情况下,在设定输送给每个施加器的初始相对能量相位的装置聚焦到电场探头175处之后,电场探头可去除,温度探头176可插入到其位置,(从而测量)前列腺组织适当深度处的温度。
在本发明设想的另一实施例中,除了设有非介入式温度传感器与尿道和直肠壁压缩接触以外,还设有第二温度监视装置。第二温度监视装置为介入式温度探头176,它从电场探头175撤出的相同点插入到前列腺组织中。
本发明的系统和方法在将期望的总微波能量剂量输送给施加器时可以破坏所有治疗的前列腺癌、癌前期细胞和良性损伤,同时避免对前列腺正常组织造成伤害。
对于非相干治疗来说,优选在915MHz下工作的独立振荡器105可向计算机控制的两个微波功率放大器130递送,并将微波功率输送给施加器110,111。在采用单个施加器的应用中,如图2所示,优选的是单个施加器110位于尿道内。
在高热治疗过程中,测量输送给每个施加器的微波信号150和功率值作为功率反馈信号500,该信号被送给微处理器或如PC的计算机250。对功率放大器130的功率控制信号进行手动或自动调节,以控制尿道和直肠的温度和输送到前列腺组织的当量热剂量。传感器410测量尿道和直肠壁的温度,并且功率控制信号135根据感应到的温度进行调节,从而避免可能引起烧伤或起水疱。由压缩气囊112,113实现的压缩量在治疗中根据需要进行调节,以病人感觉舒适。对于相干治疗来说,每次调节前列腺压缩之后,移相器120均需重调/重聚焦,以使电场探头175接受最大功率。
根据本发明的系统,用于监视输送给微波施加器110,111的微波能量的装置在治疗期间监视输送的能量/功率,并且当期望总微波能量已由微波施加器110,111输送到前列腺时,结束治疗的装置断开输送给施加器的能量辐射。这就是说,本发明的系统和方法能够自动断开输送给前列腺的能量,从而当期望的总微波能量剂量已输送给前列腺220时完成高热治疗。在优选实施例中,总微波能量剂量在前列腺肿瘤中产生总当量热剂量,大约60分钟至400分钟之间相当于43摄氏度。输送给微波施加器的总微波能量从治疗开始即在计算机250中计算,并且在治疗期间显示于计算机监示器260上。
作为备选实施例,还设有输送到电场探头175的微波功率值的装置,用于确定何时应终止治疗。根据该实施例,采用由电场探头175接收的电场反馈信号450计算出的微波能量来控制治疗时间长短。该电场反馈信号450可用于相干和非相干治疗中。为了确定治疗的效果,在给予微波总能量剂量之前和之后采用X射线和磁共振成像之一对前列腺组织进行成像,同时对前列腺组织进行探头活检得到病理结果。
对于相干治疗,单个介入式电场探头175可替代两个分别位于尿道和直肠内固定位置的非介入式电场传感器。电场传感器插入到尿道205和直肠210的自然开口,并且固定到适于加热肿瘤和健康组织的位置。电场传感器可在该位置186,187处附着到尿道和直肠壁上,但它们不是必须与尿道和直肠壁接触。超声、X射线或其它公知的电场监测装置可用于验证(verify)合适的电场传感器位置。通过调节微波移相器120,使两个非介入式电场传感器检测到的总功率为最小(如美国专利US5,810,888所述),从而在前列腺的中央部分,或在前列腺的治疗区域产生聚焦电场。
采用该实施例,没有因插入探头带来的感染危险,没有因需要划破皮肤和插入探头的操作而使皮肤结疤的危险,也由于不使用插入的电场探头而没有使癌细胞扩散的危险。电场传感器仅固定在尿道和直肠内,这样不会穿过肿瘤床,因而减少了在外科操作过程中无意间播撒有活性的癌细胞的可能,这样降低了癌细胞在周围组织的局部复发。类似地,根据本方法,由于温度和电场传感器可放置在尿道205和直肠210内,因此当前列腺中没有限定的单个区域,例如进行微观前列腺疾病治疗时,现有的发明可以很好地发挥作用。
优选的是,相控阵列的每个通道(在节点107的两侧)包括电变微波功率放大器130(0至100W)、电变移相器120(0至360度)以及水冷微波施加器110,111。
尽管优选实施例中公开的微波能量为近似915MHz,但微波能量可在100MHz至100GHz范围内。微波能量的频率可选范围为902MHz到928MHz。实际上,较低频率的能量可用于消融或预防癌组织。
在优选实施例中,输送到每个施加器的初始微波功率在0到70瓦之间,优选在20到60瓦之间。在组织的整修治疗过程中,输送到每个施加器的微波功率可以在0-150瓦范围内调节,以便输送期望的微波能量剂量,并且避免过分加热尿道和直肠。另外,输送到两个微波施加器上的相应微波功率在治疗前和治疗过程中在-180度至180度范围内调节,以便在前列腺组织中产生聚焦场。典型的是,非相干施加器治疗与相干施加器治疗相比需要更大的微波功率。
在优选实施例中,可采用外径(OD)为0.9mm,中央导体延伸1cm的电场同轴单极电极(半刚性RG-034)作为电场探头175来测量提供给组织的电场幅度,并提供反馈信号450,该反馈信号用于确定在治疗前电移相器所需的相应相位。这种类型的同轴馈送单极探头已用于对压缩胸部模型中的线性极化电场进行精确测量(Fenn等,International Symposium onElectromagnetic Compatibility 17-19May 1994 pp.566-569)。这种线性极化电场探头插入到1.6mm OD的聚四氟乙烯插管中。热电偶探头(PhysitempInstruments,Inc.,Type T copper-constantan,包绕在一个0.6mm OD的聚四氟乙烯外鞘中)在治疗过程中可用于测量肿瘤中的局部温度。这些温度探头具有100ms的响应时间,精确度为0.1℃。也可采用光纤温度探头。
如Fenn的美国专利5,810,888所公开的那样,采用电声探头175和自适应相控阵列快速加速梯度搜索算法,将微波辐射靶向到肿瘤位置。由肿瘤中的介入式温度探头175感应到的温度可用作治疗过程中的实时反馈信号。该反馈信号可用于控制可变功率放大器的微波输出功率值,该放大器将肿瘤的聚焦温度设定和维持在43至46℃范围内。输送到相近人阵列的两个通道的功率和相位可采用数-模转换器在计算机控制下适当调节。
总微波能量剂量可用于判断所需加热时间。这就是说,申请人认识到,非介入式当量温度感应装置可替代介入式温度探头,这样总微波能量剂量可用于可靠地控制治疗持续时间。如前所述,前列腺的压缩降低了血流,这有可能消除了血流对治疗所需的微波能量的影响,并且可减少微波治疗中期望的微波能量的变化。
根据优选实施例,输送到波导施加器上以确定治疗完成的总微波能量在25千焦耳至250千焦耳之间。可消灭任何癌组织或癌前期组织的微波能量总量可近似为175千焦耳。但是,在某些情况下,所需的微波能量剂量可以是低至25千焦耳。
如申请人认识到的那样,导致厚度变薄的身体压缩对于达到预防和消灭癌细胞、癌前期细胞或良性损伤效果来说需要较少的微波能量剂量(与导致厚度增加的压缩相比)。选择输送到每个施加器上的合适初始微波能量值(P1,P2)以及选择两个施加器之间适当的微波相位来将能量聚焦到治疗区域是十分重要的。
在高热治疗过程中,需要监视尿道和直肠壁的温度,这样它们不会持续数分钟以上明显升高到高于约41摄氏度。尿道和直肠壁传感器的当量热剂量可计算出(Sapareto等,International Journal of Radiation OncologyBiology Physics,Vol.10,pp.787-800,1984),并可用作反馈信号。典型的是,需要避免输送过多的当量时间热剂量。根据本发明,在治疗过程中对输送到施加器上的个别功率(P1,P2)进行手动或计算机控制自动调节可避免尿道和直肠高温。
多普勒超声可用于在治疗前和治疗过程中测量肿瘤中和前列腺组织周围的血流,以拟定和调节微波能量剂量。例如,当前列腺被压缩和/或肿瘤被加热到治疗温度时,肿瘤血流率会降低,此时需要较少的能量。备选的是,可通过探头活检测量前列腺肿瘤组织的含水量和介电参数,用于在治疗前确定所需的微波能量剂量。例如,肿瘤中含水量高和电导率较高,则所需的微波能量剂总量降低。除了以上的变化以外,肿瘤的尺寸也影响着所需微波能量剂量。较大的肿瘤与较小肿瘤相比更难于加热,因此需要更大的微波能量剂量。初始治疗计划期间包括输送低剂量微波能量来估测肿瘤的加热能力,之后进行完全的所需微波能量剂量来完成治疗。
安全特性的临床动机(motivation)
在前列腺癌和良性前列腺增生的温热治疗中,希望在达到前列腺治疗温度的同时还保持尿道和直肠内具有较低温度。由于当微波能量辐射前列腺腺体时尿道和直肠周围的组织倾向于被加热,因此需要在保护周围健康组织的前提下来安全加热前列腺方向进行发展。
在图1-4中,在进行微波温热治疗之后,微波功率控制信号135至少为经尿道施加器控制微波功率放大器130,和/或为经直肠施加器控制微波放大器130断开,并经尿道气囊112和经直肠气囊113中的至少一个保持向前列腺腺体加压,以便减少血流,从而捕获热量来聚积额外的热剂量。这种安全实施例可用于单独加热前列腺的治疗,或者当采用温热治疗的同时还进行化学治疗或从热敏脂质体中释放化学治疗药物的情况,如Fenn的美国专利US5,810,888所描述的那样。在前列腺压缩的过程中,可采用多普勒超声成像来测量血流速率,以通过改变前列腺压缩量来验证和调节前列腺中的血流速率。
在前列腺癌的化学治疗、热敏脂质体输送化学治疗或基因治疗中,希望尽可能多地向前列腺治疗组织输送药物。以化学治疗、热敏脂质体输送化学治疗、基因治疗或其它试剂形式的所需治疗剂被注入到病人的血流中。经尿道和/或经直肠气囊插管的压缩可进行调整,这样前列腺组织中的血流可以调节。增加尿道和/或直肠中压缩气囊的压力可降低前列腺中的血流。这种降低前列腺中的血流意味着注入的治疗剂将花费更多的时间进入到前列腺组织中,潜在地增加了治疗成本。另外,如果治疗试剂需要花费更多的时间进入前列腺血管中,则治疗试剂从血管中外渗(渗透或渗漏)至前列腺组织,并达到前列腺癌细胞的可能性将增大。所以,前列腺压缩可增加输送到前列腺的治疗剂的数量,同时减少了治疗试剂释放进入正常健康周围组织内的量。
前列腺气囊压缩的期望顺序如下所述。在化学治疗、热敏脂质体输送化学治疗、基因治疗或其它试剂被注入进入病人血流中之后,由经尿道和经直肠气囊插管中的至少一个向前列腺腺体加压1分钟至10分钟。然后,经尿道和经直肠气囊中的至少一个膨胀,使正常血流在1分钟之内进入前列腺,并使治疗剂进入前列腺血流,但基本上不降低前列腺的温度。接下来,再次压缩前列腺腺体1分钟至10分钟以减少血流,从而捕获在前列腺腺体内的治疗剂。重复上面描述的气囊插管的膨胀和放气过程,直到在对前列腺进行加热的同时将期望量的治疗剂输送到前列腺中。
在优选实施例中,化学治疗剂为阿霉素(亚德里亚霉素)。在另一优选实施例中,热敏脂质体制剂如Needham的美国专利6,200,598所描述的那样。
申请人预想,多普勒超声成像可用于确定肿瘤的血流。这就是说,多普勒超声成像在前列腺压缩或不压缩的情况下可测量肿瘤的血流,并在治疗期间采用这些测量结果作为反馈,以估计对前列腺肿瘤脉管系统的任何损伤。
简化微波辐射理论
从高温治疗施加器输出的微波能量在人体的近场区域内以球面波的形式辐射,所形成的电场的振幅部分地和距施加器的距离r成反比。此外,振幅的衰减和人体组织的衰减常数α及微波在体内的穿越距离(或穿入深度)d的乘积成指数函数关系,如图1所示。电场的相位则根据相位传播常数β和距离d的乘积随着距离d线性地变化。为了简单起见,这里仅对相对的两个施加器的情况进行分析,并假设施加器以近似于平面波的方式辐射微波。从数学上讲,平面波电场和组织深度的函数关系可以以式E(d)=E0exp(-ad)exp(-iβd)来表示,式中E0为表面电场(通常由振幅和相位角来表示),i为虚数(Field and Hand,An Introduction to the Practical Aspectof Clinical Hyperthermia,Tylor & Francis,New York,pp.263,1990)。
在微波频率为915MHz时,平面波电磁能量在诸如前列腺组织等高含水量的组织中的衰减率约为每厘米3分贝。因此,单个辐射施加器所辐射的微波能量和深入深层组织的微波能量相比,有相当一部分能量被介入其间的浅层的人体组织所吸收,这就可能在浅层组织中形成热聚集点。由于表面受空气或水分冷却保护的最大深度仅为约0.25到0.5厘米,为了避免形成热聚集点,就必须引入一个相位相干的第二施加器,该第二施加器辐射和第一施加器相同振幅的微波。相位相干的第二施加器的使用,和仅使用单个施加器相比,理论上可以使输送给深层组织的功率(因此也是能量)增加到4倍(Field and hand,pp.290,1990)。
从两个或多个施加器(称为相控阵列)辐射出的电磁场的相位特征可以对传送给不同组织的能量的分布产生显著影响。均质组织中的相对比吸收率(SAR)近似地正比于电场振幅的平方|E|2。SAR和给定的时间段内的温度升高成正比。下面将对一个简化的情况进行详细描述,其中微波辐射被聚集在均质的前列腺组织的中央位点上。如Fenn等人的著作所述(Fenn等人,International Symposium on Electromagnetic Compatibility,Sendai,Japan,Vol.10,No.2,May 17-19,1994,pp.566-569,1994),多个微波信号在模型内的反射的影响可以忽略不计。
频率为915MHz的微波在均质的正常前列腺组织内(介电常数为约50,电导率为约1.3S/m)的波长为约4.5厘米,微波损失为3分贝/厘米,衰减常数α等于0.34弧度/厘米,传播常数β等于1.4弧度/厘米(或80度/厘米)。(对于厚度为2.25厘米的压缩前列腺而言,在前列腺表面处单个施加器辐射在左侧表面上的电场为E0,在中央位置(深度为1.125厘米)的电场为-i0.7E0(这里i表示90度的相位移),辐射到右侧表面的电场为-0.5E0。两个相位相干的施加器综合作用在两侧表面上电场为0.5E0,作用在中央位置上(深度为1.125厘米)的电场为-i1.6E0。因此,对于压缩前列腺来说,采用上述相干电场的平方来计算SAR,表面上的SAR明显低于中央位点的SAR,因数约为2.0。微波场发射穿过2.25厘米厚的前列腺后便发生180度的相位移,并和相位移为0度的进入组织的电场部分地抵消或平衡。由于远离中央焦点方向行进的微波的相消干涉作用,预期浅层前列腺组织的温度将比深层组织低。测量得到前列腺相对两侧表面的SAR值较低,这样能够有效地将微波能量聚集到前列腺的深处。在希望对前列腺组织的浅表或周边区域进行更强的辐射的情况下,压缩厚度可以大于2.25厘米,这样传播波相位延时更长一样,并且两个波在表面不相消,或者仅采用经尿道或经直肠施加器之一(特别是经直肠施加器)来加热前列腺。
重复上述计算,但现在针对非相干施加器,压缩的前列腺厚度为2.25厘米,在前列腺表面处单个施加器辐射在左侧表面上的电场为E0,在中央位置(深度为1.125厘米)的电场为-i0.7E0(这里i表示90度的相位移),辐射到右侧表面的电场为-0.5E0。采用独立电场的平方并叠加它们得到两个不相干的施加器综合作用在两侧表面上产生的SAR值为1.5E0 2,在中央位置上(深度为1.125厘米)为0.98E0 2。因此,对于压缩前列腺来说,采用上述不相干电场的平方来计算SAR,表面上的SAR明显高于中央位点的SAR,因数约为1.5。出于该原因,与采用相干阵列相比,采用不相干阵列更难于对前列腺组织的深部加热。但是,如前所述,对于前列腺癌治疗来说,一些前列腺癌细胞位于靠近直肠之处,因此采用不相干治疗可提供适当的加热。
根据该发明的自适应相控阵列系统使用了两个由公共振荡器105来馈送的微波通道,该系统包括两个电子调节的移相器120,以将微波能量聚集到电场反馈探头175上。本发明的自适应相控阵列系统和非自适应的相控阵列相比具有明显的优点。双通道的非自适应相控阵列理论上可以视其两个微波的相位差是180度、或是0度、或是某个中间值而对应地生成一个零电场、或是最大电场、或是中间值电场。这就是说,根据本发明,在进行治疗之前或是治疗过程中可以对输送给两个微波施加器的微波的相位在-180度到180度的范围内进行调节,以在前列腺组织内生成一个聚焦电场。
按照本发明的自适应相控阵列能够在整个组织都存在分散构造的情况下自动地使电场聚集。这样,本发明的自适应相控阵列应当能够提供比如授予Turner的美国专利US 4,589,423所述的手动调节或治疗前预先控制的相控阵列更可靠地进行深度聚集加热。另外,本发明的自适应相控阵列系统不使用介入式温度探头,这种温度探头会耗散或改变肿瘤位点的电场。
微波能量的计算
电能消耗的单位通常以千瓦小时来表示。从数学上说,一个施加器所输送的微波能量W可以以数学式
W=Δt∑Pi (1)
来表示(Vitrogan,Element of Electric and Magnetic Circuits,RinehartPress,San Francisco,pp.31-34,1971)。在上式中,Δt代表等间隔的进行微波功率测量的时间段(单位为秒),算子∑在整个治疗时间段内求和,Pi为第i个时间段内的微波功率(单位为瓦)。
微波能量W的单位为瓦-秒,此单位也称作焦耳。例如,在三个连贯的60秒的时间段内如果测得相应的微波功率分别为30瓦、50瓦、60瓦,那么在这180秒的时间内所输送的微波能量W=60(30+50+60)=8400瓦-秒=8400焦耳=8.4千焦耳。
为了更好的了解单位时间内沉积在不同厚度(以D表示)的均质前列腺组织中央位置上的由两个施加器输出的聚焦微波能量W’(‘表示prime),对相干治疗来说可作下列计算。让P1及P2分别代表两个施加器输出的微波功率。每个施加器辐射出的电场和输入给该施加器的功率的平方根成正比。假设这两个施加器发出的电场是对称的,那么这两个电场在中央聚焦位置上的相位是重合的。又假设两个施加器输出的功率相等,亦即P1=P2=P,并假设电场的辐射为平面波模型,那么中央深度上单位时间内的聚焦能量可表示为

当量热剂量的计算
相对于43℃的累计当量热剂量或总当量热剂量可由下式来累加计算(Sapareto等人,International Journal of Radiation Oncology BiologyPhysics,Vol.10,pp.787-800,1984):

式中∑为在整个治疗时间段内对一系列的温度测量值求和,T为这一系列温度测量值(T1,T2,T3,...),Δt为等间隔的进行测量的时间段(单位为秒并换算成分钟),如果T>43℃则取R等于0.5,如果T<43℃则取R等于0.25。当量热剂量的计算对于评估任何可能出现的前列腺组织,尿道或直肠的热损伤是很有用的。
等同方案
尽管已参考优选实施例对本发明进行了展示与描述,但本领域技术人员可以理解,在不脱离由所附的权利要求书限定的本发明的精神和范围内可对本发明的形式和细节进行改进。例如,尽管在此描述的高温系统用于治疗前列腺癌和良性前列腺损伤,但本发明还可用于治疗其它类型的癌,例如乳房,肝脏,肺和卵巢。除了公开的优选实施例以外,可以理解,在此公开的方法可用于对人体的其它附属物和部分例如腿和手臂以及躯干等可以用气箍(例如血压箍)或其它装置进行压缩,进行微波、辐射频率或超声热疗治疗。还可以理解的是,采用较多或较少量的阵列天线施加器,或例如移植物或经尿道或经直肠施加器的单个天线施加器也可获得类似的结果。在此描述的一些方法和技术还可用于超声高温系统中,特别是用于反馈控制中采用能量剂量。本发明的系统可用于增强辐射治疗或靶向药物输送,和/或采用热敏脂质体靶向基因输送,或者靶向基因治疗。本发明还可用于非医疗高温系统中,例如用于工业加热。
Claims (21)
1.一种治疗前列腺组织的方法,包括如下步骤:在施加微波能量、超声能量、激光能量和借助流体的能量之一之后,压缩前列腺来减少前列腺血流,以便在受治疗的前列腺组织中聚集增加的热剂量。
2.根据权利要求1的治疗前列腺的方法,其中施加微波能量、超声能量、激光能量和借助流体的能量之一由单个施加器实现,所述施加器为植入件、经尿道施加器和经直肠施加器之一。
3.根据权利要求1的治疗前列腺的方法,其中压缩步骤由膨胀经尿道插管气囊和经直肠气囊中至少之一种来完成。
4.根据权利要求3的治疗前列腺的方法,还包括如下步骤:采用来自多普勒超声速度成像系统的实时反馈来调节和验证作为经尿道和/或经直肠气囊膨胀的结果的前列腺压缩量。
5.一种安全施予温热治疗的方法,包括如下步骤:采用气囊插管和气囊箍之一来对器官和附属物中的至少一种进行压缩,以减少该器官和附属物中的血流;以及
结合压缩施以化学治疗、基因治疗、包含化学治疗的热敏脂质体、和辐射治疗之一来治疗器官和/或附属物的癌症和/或良性疾病。
6.根据权利要求5的安全施予温热治疗的方法,其中气囊插管是经尿道气囊插管和经直肠气囊插管中的至少一种。
7.根据权利要求6的安全施予温热治疗的方法,其中受治疗的器官为前列腺,并且采用气囊插管来压缩插管,并且还进一步包括如下步骤:调节前列腺压缩量,并验证作为经尿道和/或经直肠气囊插管的结果的前列腺压缩量。
8.根据权利要求7的安全施予温热治疗的方法,其中调节和验证步骤采用来自多普勒超声速度成像系统的实时反馈来完成。
9.根据权利要求5的安全施予温热治疗的方法,其中器官为前列腺,气囊插管用于压缩前列腺,并且前列腺的压缩步骤在化学治疗、基因治疗、包含化学治疗的热敏脂质体、和辐射治疗之一注入到病人血流之中,以输送治疗剂之后进行;所述压缩步骤包括膨胀气囊插管,从而在约1分钟至约10分钟之间的第一时间段内压缩前列腺腺体,然后在第一时间段压缩之后,使气囊插管回缩,使未受限的血流在少于约1分钟的第二时间段进入前列腺。
10.根据权利要求9的安全施予温热治疗的方法,其中膨胀和回缩气囊插管一直重复进行,直到所需量的治疗剂被输送到前列腺中。
11.根据权利要求5的安全施予温热治疗的方法,其中附属物被压缩,治疗癌症和/或良性疾病。
12.根据权利要求11的安全施予温热治疗的方法,其中附属物为手臂、腿或躯干之一。
13.一种有效输送化学治疗、包含化学治疗的热敏脂质体、和基因治疗的方法,包括如下步骤:对器官或附属物施以化学治疗、包含化学治疗的热敏脂质体、和基因治疗之一;然后在器官或附属物附近调整气囊插管与气囊箍之一的膨胀和回缩,以压缩器官或附属物来改变其血流。
14.根据权利要求13的有效递送化学治疗、包含化学治疗的热敏脂质体、和基因治疗的方法,还包括如下步骤:采用微波、超声、辐射频率和激光能量之一来加热器官或组织的组织,其中调整器官或附属物的压缩和热治疗一起增加了递送化学治疗、包含化学治疗的热敏脂质体、和基因治疗之一的医疗效果。
15.根据权利要求14的有效递送化学治疗、包含化学治疗的热敏脂质体、和基因治疗的方法,其中受治疗的器官为前列腺。
16.根据权利要求14的有效递送化学治疗、包含化学治疗的热敏脂质体、和基因治疗的方法,其中受治疗的附属物为手臂、腿和躯干之一。
17.一种有效地将包含化学治疗的脂质体递送到将受治疗的前列腺中的方法,包括如下步骤:随时调整气囊插管对前列腺腺体的压缩,以减少前列腺中的血流,从而降低血流,并为包含化学治疗的脂质体递送到前列腺中提供更多的时间。
18.根据权利要求16的有效地递送脂质体的方法,其中随时调整气囊插管压缩的步骤采用来自多普勒超声速度成像系统的实时反馈来调节和验证前列腺压缩量。
19.根据权利要求16的有效地递送脂质体的方法,其中压缩步骤采用膨胀经尿道插管气囊和经直肠气囊中的至少一种来完成。
20.一种对肿瘤进行温热治疗的方法,包括如下步骤:在温热治疗过程中采用或不采用压缩,采用多普勒超声成像来测量肿瘤区域的血流流速来得到反馈信息,以估计对肿瘤的任何损伤。
21.一种采用辐射前列腺对前列腺癌、癌前期或良性疾病进行早期治疗的方法,包括如下步骤:
a)确定病人的前列腺特异性抗原(PSA)水平和美国泌尿学协会(AUA)症状指数中的一个;
b)对病人的前列腺进行压缩;以及
c)当PSA值低于4.0ng/ml和AUA指数小于13之一时,采用聚焦微波能量对病人的压缩的前列腺进行加热。
