Abstract

技术领域
本发明涉及束头带,且具体而言涉及具有一用于指示束头带何时恰当地绷紧的张力指示器并因而能够对佩戴者的头部施加恰当程度的压力的束头带。
背景技术
人们已知各种束头带装置。这些束头带装置包括体育运动型的束头带装置以及更复杂的束头带装置,例如那些用于固定在头上携带的装置的束头带装置。某些束头带装置用于对束头带下面的区域施加一定程度的压力。此种所施加的压力例如适用于支撑束头带佩戴者所用的医用传感器。在这些情形中,需要一种具有张力指示器的改良的束头带。
发明内容
本发明的实施例涉及一种束头带装置。在一实施例中,本发明提供一种束头带,其具有:一低拉伸性段,其尺寸适于贴绕佩戴者的头部;及一小于所述低拉伸性段的弹性段。所述弹性段具有一自由端及一附连端,其中所述弹性段在其附连端处与所述低拉伸性段附连,且所述弹性段的自由端经构造以与所述低拉伸性段形成一围绕佩戴者头部的封闭环。
在一个方面中,所述束头带还包括一可视指示器,所述可视指示器经构造以监测所述弹性段的自由端的伸展位置。所述可视指示器可为所述低拉伸性段上的一缺口、一条线或者一标记。
在一个方面中,所述束头带还包括一止挡部分,其中所述止挡部分经构造以啮合所述弹性段从而限制所述弹性段的伸缩。在一实施例中,所述止挡部分具有一开口,所述开口的宽度小于所述低拉伸性段的宽度及所述弹性段的宽度。

具体实施方式
本发明的各实施例涉及一种具有一张力指示器的束头带。此种束头带可用于支持对患者施行与健康护理有关的服务。此种服务可包括在患者的前额上置放一传感器101,例如一血氧测定传感器(例如由本发明的受让者Nellcor Puritan Bennett所制造的血氧测定传感器),如在图1中所示。典型的脉搏血氧测定计是测量两个生理参数:动脉血红蛋白的氧饱和度百分比(SpO2或sat)及脉搏率。氧饱和度可使用各种技术来加以估计。在一种常用技术中,对由光电检测器所产生的光电流进行调节及处理,以确定红色信号对红外信号的调制比之比(比率之比率)。人们已发现此种调制比与动脉氧饱和度密切相关。脉搏血氧测定计及传感器是通过对一组患者、健康志愿者、或动物在一在体内测得的动脉氧饱和度(SaO2)范围内进行调制比测量来根据经验进行校准。所观察到的关联性用于以逆向方式根据患者的调制比测量值来估计血液的氧饱和度(SpO2)。在如下美国专利案中对使用调制比来估计氧饱和度进行了说明:1998年12月29日所颁予且名称为“METHOD AND APPARATUS FOR ESTIMATINGPHYSIOLOGICAL PARAMETERS USING MODEL-BASED ADAPTIVE FILTERING”的第5,853,364号美国专利,及1990年3月27日颁予且名称为“METHOD ANDAPPARATUS FOR DETECTING OPTICAL PULSES”的第4,911,167号美国专利,且在1997年7月8日颁予且名称为“MEDICAL SENSOR WITH MODULATED ENCODINGSCHEME”的第5,645,059号美国专利中对氧饱和度与调制比之间的关系进行了进一步说明,这些美国专利的揭示内容的全文均以引用方式并入本文中。大多数脉搏血氧测定计均提取具有第一所测定饱和度或脉搏速率的体积测量信号。在名称为“StackedAdhesive Optical Sensor”且同在申请中的第10/256,245号美国专利申请案中对一种实例性的前额血氧测定传感器进行了说明,出于各种目的,该美国专利申请案的揭示内容全部以引用方式并入本文中。
施加至血氧测定传感器上的力可能是使传感器能够正确起作用的一个因素。在某些临床情形中,需要将束头带200与一前额传感器101(例如血氧测定传感器)一起使用,如在图2中所示。图2显示自传感器(未图示)伸出的引线自束头带下面向外伸出。这些临床情形包括如下情形:患者平躺,且他/她的头部接近或低于胸部高度;患者的静脉压力升高;患者发汗;患者正在剧烈运动,例如在体育锻炼过程中;以及其他其中静脉脉动可能会在血氧测定计算中引起误差的情形。在这些情形中,假如不使用束头带或者在血氧测定传感器上施加力,则静脉脉动可能会导致对波形的解译出现错误,且因此会致使对氧饱和度及脉搏率值的测定欠精确。束头带可用于对血氧测定传感器施加压力,从而减小静脉脉动的影响。当用于支撑血氧测定传感器时,传感器在前额上所施加的力的大小应大于静脉压力、但小于小动脉压力。一般而言,较佳的压力范围是使所施加压力大于静脉压力(例如3-5mm Hg)但低于毛细血管压力(例如22mm Hg)。较佳地,对于成人患者而言,此介于10mm Hg与20mm Hg之间。可使用一可调的闭合机构(例如钩圈式闭合机构)来调整根据本发明各实施例的束头带以适用于任意块头的佩戴者。另一选择为,可视佩戴者头部的总体尺寸而提供不同尺寸的束头带;例如对婴儿使用小的束头带、对儿童使用较大的束头带、对成人佩戴者则使用甚至更大的束头带。用户可对前额血氧测定传感器使用一宽广的压力范围,此视在束头带围绕佩戴者的头部放置过程中已施加至束头带上的张力大小而定。
本发明的各实施例旨在通过为护理者提供一在将束头带围绕头部放置期间指示在束头带中所需的正确张力大小的可视指示器,而缓解了护理者凭猜测进行工作的作法。所需张力与在将传感器与患者附连时由传感器所施加的压力有关。
在图3所示的一实施例中,显示一弹性束头带102处于一未拉伸位置。图中显示一非弹性织物104沿其两个边缘106附连至弹性部分102上。所述非弹性部分的其他两个边缘则不附连至所述弹性段,并因而远离所述弹性段的表面自由地向外凸伸出。所述非弹性段小于所述弹性段。所述非弹性段的尺寸确定成在所述弹性段受到拉伸时跨越所述弹性段的一部分。当所述弹性段未受到拉伸时,所述非弹性段大于其所跨越的所述弹性段的一部分。当弹性段102自其未被拉伸位置受到拉伸时,所述非弹性部分在其边缘106处与正拉伸的弹性段102一起受到拉动,直至所述边缘之间的弹性部分的长度已拉伸至等于所述非弹性部分的长度为止。所述束头带还包括将在下文中结合图4所述的闭合机构(未图示)。图5显示一显示为由用户佩戴的本发明束头带的一实施例的正视图。应注意,所述束头带可用于固定一传感器(例如在图2中所示的一应用于患者前额上的血氧测定传感器)并对所述传感器施加一压力。为清楚地说明所述张力指示器,在图5-7中未显示此一传感器。图6为一本发明束头带102的一实施例在由用户佩戴时的俯视图,图中显示其承受合适的张力。如在该图中所示,当将束头带正确地固紧时,压力指示器部分104被在弹性部分102上拉紧,因而不提供一指示需要重新固紧束头带的可视指示。相反,图7显示本发明束头带的一实施例在由用户佩戴时的俯视图,其显示所述束头带未达到合适的张力。如在图7中所示,当束头带对用户的前额施加的压力不足时,或者当束头带不够紧时,指示器104从形成一环圈的表面上凸伸出来,从而提供一所述束头带需要重新固紧的可视提示。
当束头带未受到拉伸时,在非弹性部分与弹性部分之间存在一定的松弛度。而当束头带受到拉伸时,所述非弹性带中的松弛现象消失,从而给出束头带拉伸量满足要求的可视指示。所述束头带选择成长到足以贴绕用户(或患者)的头部。所述弹性材料可由例如开孔胺基甲酸酯发泡体等任意适宜的织物形成。所述非弹性带(其短于所述弹性部分)以一小于非弹性部分的长度的间距缝至或以其他方式(例如黏合方式)附连至所述弹性束头带上。所述非弹性材料可由例如Dacron型织物等任意适宜之织物制成。
图4为本发明束头带的另一实施例的图式。图中显示一弹性束头带102处于一未受拉伸的位置。图中显示一非弹性织物104沿其两个边缘106附连至弹性部分102上。所述非弹性部分的其他两个边缘则未附连至所述弹性段上并因此远离所述弹性段的表面自由地向外凸伸出。非弹性段104小于弹性段102。所述非弹性段的尺寸确定成在所述弹性段受到拉伸时跨越所述弹性段的一部分。当所述弹性段未受到拉伸时,所述非弹性段大于其所跨越的所述弹性段的一部分。当弹性段102自其未被拉伸位置受到拉伸时,所述非弹性部分在其边缘106处与正拉伸的弹性段102一起受到拉动,直至所述边缘之间的弹性部分的长度已拉伸至等于所述非弹性部分的长度为止。
图4还显示所述非弹性部分包括一永久性的摺痕或折痕110。如在图4A中所示,此一折痕10可通过如下方式形成:使所述非弹性部分交叠以形成一折痕并随后对所述织物实施热压或热封来形成一永久性折痕或摺痕。在一实施例中,所述折痕或摺痕是在非弹性段的中间部位中形成,此使其在弹性带102回缩或松弛时以一种尖锐、倾斜的方式向外凸伸出。在使用中,已证实,所述尖锐、倾斜的摺痕或折痕充当一机械放大器,并在何时已超过最小束头带张力阈值方面提供一更明显的可视提示及更佳的灵敏度。起摺痕的张力指示器110通过以一歪斜方式远离所述弹性带向外凸伸而表现出对束头带失去张力的增强的灵敏度。从在前额处直接看去及从在前额顶上(边缘)向下看去两个角度看来,起摺痕的张力指示器110均提供更明显的可视提示。为所述具有折痕或摺痕的非弹性部分所选用的材料可类似于不起摺痕或不起折痕的非弹性材料。另外,也可使用例如聚酯网片材料等能够保持折痕或摺痕的材料。所述弹性材料可由如上文所述的材料制成,或者使用例如毛圈带等其他适宜的材料制成。
当束头带未受到拉伸时,在非弹性部分与弹性部分之间存在一定的松弛度。而当束头带受到拉伸时,所述非弹性带中的松弛现象消失,从而给出束头带拉伸量满足要求的可视指示。
图4中还显示适用于结合图3所述的实施例的闭合装置108。一个此种闭合装置是钩圈型闭合件。根据本发明实施例的束头带可使用其他闭合机构,例如卡扣、纽扣、黏合剂、销、或其组合形式,以及使用所属技术领域的技术人员所知的其他闭合机构。另一选择为,所述束头带可为一预成形的环圈而不需要使用单独的闭合机构。
上述束头带包括一传感器固定压力指示器。如在上文中所述,所述束头带可用于允许人们在患者的组织位置(例如前额等)处选取一高于静脉脉动(例如5-10mm Hg)但低于一最大值(例如30mm Hg左右)的传感器固定压力。如上文所述,此种压力指示器是使用束头带来加以固定。另一选择为,所述压力指示器可使用传感器(例如血氧测定传感器)来加以固定。压力指示器的一个实施例是如上文参照图3-4所述的张力指示器。其他压力指示构件包括足够小且足够轻到由传感器或束头带总成来含纳的压力或力量传感器。
压力指示器所提供的信息可用于帮助为传感器在患者身上的固定建立一可接受的压力窗口。也可将所述可接受的压力窗口增强为包含患者头部相对于患者心脏的高度的影响。
另外,所述使用束头带来确保传感器固定压力可接受的概念可扩展至其他患者身体部位;那些传感器固定压力可有助于提供更精确传感器读数的部位。
根据本发明的指示张力或压力的束头带的另一实施例显示于图8中。如在图8中所示,所述束头带包括一非弹性部分604及一弹性部分602。张力指示部分606也由非弹性材料制成。张力指示部分606可如结合图4所述带有摺痕或折痕或者如结合图3所述显示为不带有摺痕或不带有折痕。对闭合装置的说明及对所述弹性部分与非弹性部分如何彼此固定的说明也已在上文中提及。在该实施例中,主可拉伸部分为弹性部分602。一旦所述束头带已被拉伸到使段602被拉伸至与段606的长度相一致,所述束头带的拉伸就会受到限制。该实施例通过具有一较短的弹性部分而限制了所述束头带的伸长并因此限制了束头带可施加至用户前额或应用于用户前额上的传感器的压力范围。

所述束头带还包括一特定长度的弹性段204以在一旦被拉伸时提供一特定的弹性力,所述弹性段204在一端203处附连至包绕患者头部的低拉伸性材料(即带202)的朝外的面上。可通过将段204在203处缝合至202来实现弹性段204在203处在带202上的附连。另一选择为,可将段204以黏合方式在203处附连至202上。在另一端(自由端)205处,弹性段204构造成使用一如上文所述的闭合装置208(例如具有一VelcroTM钩材料片的非弹性材料)与一带202的一段附连。在一实施例中,弹性段的自由端205附连一低拉伸性部分或凸片206,所述低拉伸性部分或凸片206借助一闭合装置208附连带202的一段以形成一可闭合的环圈。带206以一种类似于使带穿过环圈的方式在止挡件207处滑过带202中的槽。为对包绕头部的低拉伸性材料带202施加恰当的张力并因此对皮肤施加恰当大小的压力,将弹性段204拉伸一受控的距离,并随后使用闭合装置208将其紧固至低拉伸性条带202上。在弹性段204遇到一实体止挡件时,其拉伸便受到控制。在一实施例中,所述实体止挡件是通过使弹性部分204的宽度尺寸略大于带202中止挡件207的开口来实现,且因此一旦被拉伸一定的距离,弹性部分204就会遇到一实体止挡件207。止挡件207可为带202中的一略小于弹性部分204的宽度的开口。另一选择为,所述止挡件可由一窄带来实现,所述窄带在形状及功能上类似于缝合在带202上或与带202附连的带环圈。通过拉伸弹性部分204并使用带202对其进行紧固,将弹性段204中的张力传递至包绕患者头部的整个低拉伸性条带上。此种受控的张力又会转变成施加于患者的前额皮肤上的可控压力。换句话说,通过如下方式来在所述带中获得恰当的张力并因此在患者前额上施加恰当的压力:使带202包绕患者的头部;并随后直接地拉动或通过部件206上的一拉力来拉动所述弹性段,从而使弹性段204伸长至其边缘210遇到止挡件207为止,并随后使用闭合装置208将段206的自由端紧固在带202上。
所述束头带还包括一用于监测弹性部分204的拉伸的可视指示器。在一实施例中,凸片206在弹性部分205的自由端与止挡件207之间包括一可视或指示器部分211,以便当束头带恰当地张紧时,弹性部分204受到拉伸且因而当弹性部分204抵靠在止挡件207上时所述部分211不再可见。另一选择为,束头带包括一可视指示器212(显示于图10D’及10D”中),从而能够在段204受到拉伸时对照指示器212来对弹性段204的自由端的边缘进行可视监测。尽管图中将指示器212显示为一缺口,然而其也可为一条线或者任意其他适合的标记。本文所述的束头带提供了能监测及/或控制弹性段204的拉伸的结构。弹性段的拉伸是由止挡件207来控制。止挡件207如上文所述确保使弹性段的拉伸受到限制。例如,由于弹性段的自由边缘在其完全受到拉伸时将会碰到止挡件207,因而能防止临床医生过度拉伸所述弹性段。可视指示器211或212能够实现对弹性段的拉伸量的监测。另外,能通过对照指示器212观察指示器211或者所述段的自由边缘的位置,以目视方式监测所述段的张力或拉伸的合适度。因此,例如,在已恰当地应用束头带后,预计束头带或其某些部分可能会松弛且在此种情形中所述可视指示器将显示需要使束头带重新张紧。
图10A-E为显示用于在患者的头部上放置图9所示束头带的方法的图式。为便于进行说明,假定患者或束头带佩戴者正背朝下平躺在一表面上且脸朝上。如在图10A中所示,首先将束头带放置于患者头部下面,使弹性段侧朝下且与一前额血氧测定传感器300位于同一侧上。为易于放置,较佳使所述带的长度在弹性段侧上伸出更多。接下来,如在图10B中所示,使较短的一端朝患者的前额卷起。接下来,如在图10C中所示,将弹性段侧卷到患者的前额上从而盖住所述传感器300。较佳可在束头带的弹性部分上提供一传感器设计的轮廓-在此种情形中,较佳使前额传感器的弹性带部分上的所述传感器轮廓大致对准传感器300。接下来,如在图10D中所示,拉动凸片206,直至弹性部分204到达止挡件207且无法再看到所述带的指示器或可见部分211为止。应注意,在图10C中,当所述带未受到适当拉伸时,所述凸片206在弹性部分204与止挡件207之间具有一局部可见的部分211(例如指示器部分),而当所述弹性部分受到适当拉伸时,同一凸片部分211(例如指示器部分)则在弹性部分204与止挡件207之间不再可见,如在图10D及10E中所示。另一选择为,如在图10D’-D”中所示,在将所述弹性段侧卷到患者前额上从而覆盖传感器300之后,拉动凸片206直至弹性段204到达位置标记或指示器212为止。受到适当拉伸的束头带能够在束头带中形成适当的张力并因而对前额及布置于前额与束头带之间的传感器施加足够的压力。因此,当在弹性部分204与止挡件207之间不存在可见的凸片部分211时,或者当所述弹性段与指示器212正确对准时,或者当所述弹性部分已抵靠其止挡件受到适当拉伸时,临床医生即会得到当前一正确压力正施加至佩戴者前额上的指示。
图11为图9所示束头带在以一适当张力置于患者头部上时的俯视图。由图可见,带202包绕患者头部,弹性部分204适当地张紧并通过凸片部分206与带202紧固于一起。
所属技术领域的技术人员应了解,本发明也可实施为其他具体形式,此并不背离本发明的实质特性。这些其他实施例也打算包含于由随附权利要求书所提及的本发明的范畴内。
Claims (14)
1、一种束头带,其包括:
一低拉伸性段,其尺寸适于贴绕一佩戴者的头部;及
一小于所述低拉伸性段的弹性段,所述弹性段具有一自由端及一附连端,所述弹性段在所述附连端处与所述低拉伸性段附连,所述弹性段的所述自由端经构造以与所述低拉伸性段形成一围绕一佩戴者的头部的封闭环。
2、如权利要求1所述的束头带,其进一步包括一可视指示器,所述可视指示器经构造以监测所述弹性段的所述自由端的伸展位置。
3、如权利要求2所述的束头带,其中所述可视指示器为所述低拉伸性段上的一缺口、一条线、一标记、或其一组合。
4、如权利要求1所述的束头带,其进一步包括一止挡部分,所述止挡部分经构造以啮合所述弹性段从而限制所述弹性段的伸缩。
5、如权利要求4所述的束头带,其中所述止挡部分具有一开口,所述开口具有一小于所述低拉伸性段的宽度及所述弹性段的宽度的宽度。
6、如权利要求1所述的束头带,其进一步包括一闭合机构,所述闭合机构经构造以使所述弹性部分的所述自由端与所述低拉伸性段相耦接从而紧固所述封闭环。
7、如权利要求6所述的束头带,其中所述闭合机构为一钩圈闭合件、一卡扣、一纽扣、一黏合剂、一别针、或其一组合。
8、如权利要求1所述的束头带,其进一步包括一具有一第一端及一第二端的凸片部分,其中所述凸片部分的所述第一端连接所述弹性部分的所述自由端,所述凸片部分的所述第二端经构造以与所述低拉伸性段形成一封闭环。
9、如权利要求8所述的束头带,其中所述凸片部分的弹性小于所述弹性部分。
10、如权利要求8所述的束头带,其进一步包括一止挡部分,其中所述止挡部分经构造以啮合所述弹性段从而限制所述弹性段的伸缩;及
其中所述凸片部分在其第一端与所述止挡部分之间包括一指示器部分,以使所述指示器部分在可见时指示所述束头带需要重新固紧;及当所述指示器部分不可见时,其指示张力水平适当-此对应于给予所述佩戴者的前额一处于比静脉压力高且比毛细血管压力低的范围内的压力。
11、一种用于对一患者的前额上的一血氧测定传感器施加压力的束头带,其包括:
一低拉伸性段,其尺寸适于贴绕一患者的头部;
一小于所述低拉伸性段的弹性段,所述弹性段具有一自由端及一附连端,所述弹性段在所述附连端处附连所述低拉伸性段;
一具有一第一端及一第二端的凸片部分,所述凸片部分的所述第一端与所述弹性部分的所述自由端相连,所述凸片部分的所述第二端经构造以与所述低拉伸性段形成一围绕一患者的头部的封闭环;
一可视指示器,其经构造以监测所述弹性段的所述自由端的伸展位置;
一止挡部分,所述止挡部分经构造以啮合所述弹性段从而限制所述弹性段的伸缩,所述止挡部分包括一开口,所述开口具有一小于所述低拉伸性段的宽度及所述弹性段的宽度的宽度;及
一封闭机构,其经构造以使所述凸片部分的所述第二端与所述低拉伸性段相耦接从而紧固所述封闭环。
12、如权利要求11所述的束头带,其中所述可视指示器在所述凸片部分上位于其第一端与所述止挡部分之间,以使所述指示器部分在可见时指示所述束头带需要重新固紧;且当所述指示器部分不可见时,其指示张力水平适当-此对应于给予所述患者的前额一处于比静脉压力高但比毛细血管压力低的范围内的压力。
13、如权利要求11所述的束头带,其中所述可视指示器为所述低拉伸性段上的一缺口、一条线、一标记、或其一组合。
14、如权利要求11所述的束头带,其中所述闭合机构为一钩圈闭合件、一卡扣、一纽扣、一黏合剂、一别针、或其一组合。
