背景技术

具体实施方式
为促进本发明原理的理解,下面将引用其所述实施例并使用明确语言对其进行说明。然而要理解这不构成对本发明范围的限制。本发明的任何改变和进一步修正、以及对本发明原理的进一步应用能由发明相关领域内的技术人员作出。
提供一种器件将植入物和其它装置插入相邻骨部分之间的空间中,以支承相邻骨部分。插入器件可用于任何种类的骨支承植入物,例如人造关节、间隔物以及融接装置。植入物可由骨材料或任何合适的生物兼容金属、塑料或其它材料制成。在一种场合下,可在脊椎手术过程中采用插入器件以将植入物插入相邻椎骨之间的脊椎盘空间中。例如,在图10、12和图15-18所述实施例中,相邻骨部分包括第一椎骨220和第二椎骨222。椎骨220、222之间包括脊椎盘空间224,脊椎盘空间224提供将植入物插入到相邻骨部分之间所需的空间。插入器件也能用于corpectomy过程以将植入物定位在一个或多个去除的或部分去除的椎骨体各侧上的相邻椎骨之间。插入器件可用多种方法靠近脊椎,包括背部、背侧、穿孔的、侧部、前侧、倾斜的和前部方法。在这些方法中可将插入器用于脊椎的各种区域,包括腰部、胸部和颈部区域。可以考虑将插入器件用于除脊椎手术过程外的手术过程中以便将植入物插入到相邻骨部分之间。
图1示出一插入器件20。插入器件20包括中间壳体22,它具有从中延伸通过并与之连接的驱动件40。驱动件40包括从中贯穿延伸的植入物固定器140。一对导向件100、120连接于壳体22并从那里向远端延伸。驱动件40和植入物固定器140的远端在导向件100、120之间的空间内延伸。驱动件40的远端与位于导向件100、120之间的扩张器60接合。植入物160位于扩张器60的前方。植入物固定器140通过扩张器60延伸并接合植入物160以便将其位置保持在导向件100、120之间。通过相对于壳体22操纵驱动件40以朝向导向件100、120的远端向前进给驱动件40,扩张器60可向远端或向前移动。
导向件100、120的近端可枢转地安装于壳体22,以便装载植入物160和使导向件100、120的远端彼此相邻地设置,以定位在脊椎盘空间内。当扩张器60在导向件100、120之间向远端推动植入物160时,导向件100、120的远端分开以将牵引力施加于相邻椎骨。由于最终牵引高度由导向件100、120远端之间的植入物160的高度确定,椎骨受到足够的牵引以容纳植入物160。
下面将结合图2-9对插入器件20组件进行更详细地说明。在图2中示出壳体22。壳体22包括从远端连接部分26向近端延伸的驱动件接合部分24。通道38在每个驱动件接合部分24和连接部分26内贯穿地延伸。连接部分26包括一对凸出于此的一对向上和向近端延伸的上指形件28、32。指形件28、32分别形成近端容纳口30、34,它们容纳并枢转地锁定导向件120(图4)至连接部分26。槽36在指形件28、32之间并沿在一对相对的下指形件之间的连接部分26的远端取向面延伸,图中仅示出该对下指形件中的一个38。下指形件与上指形件28、32相似,并可枢转地将下导向部分100锁定至连接部分26。
应当理解术语“上”、“下”表示如图所示器件中的部件在相邻骨部分之间的空间的操作方法中的取向。该器件可以转动或重新定位以使例如下指形件向上延伸而导向件100位于导向件120上方。
参阅图3,驱动件40连接于壳体22。在所述实施例中,驱动件40包括轴42和近端手柄46。手柄46包括相对的第一和第二臂48、50,它们从中央体部分47延伸以便手柄46的夹持。近端凹槽52被设置在手柄46的中央体部分47,轴42通过壳体部分22的通道38延伸至远端44。轴42的至少一部分攻有螺纹以螺纹接合于通道38的内螺纹部分。因此,驱动件40可通过转动壳体22中的轴42以沿纵向向远端和近端移动,从而使远端44向远端或近端位移。
在另一实施例中,驱动件40包括棘爪机构。可沿轴42设置棘爪杆,它在壳体22内直线地向近端移动以使导向件100、120之间的扩张器60向远端进给。可设置手持扳机类手柄、具有齿轮或工具接合端的小齿轮或其它适合的手柄以影响棘爪杆的直线移动。在手柄或壳体22内的抓紧机构可保持棘爪杆的远端定位直至被释放,这允许棘爪杆直线地和向近端地移动。
在图4中示出安装于驱动件40远端44的扩张器60。扩张器60包括具有从那里彼此沿相反方向延伸的上翼64和下翼66的中央体62,如图5所示。孔72从中间地通过中央体62延伸并且在其远端和近端形成开口。驱动件40的远端44可转动地容纳于孔72的尾部或近端开口。在所述实施例中,远端44包括圆周槽以将球塞容纳在扩张器60中。另外也能考虑其它合适的可转动连接配置,诸如C环或其它适当的连接件。根据驱动件40沿其纵轴的转动方向,扩张器60无转动地向远端或近端移动。
扩张器60还包括前端或远端壁,它向近端与中央凹陷76形成夹角。孔72开在凹陷76中。扩张器60还包括其中形成有凹槽68、70的横向侧壁。每个侧壁还包括制动结构,例如槽70中所示的制动结构74。近端凹陷的前端或远端壁60至少部分地容纳位于其前方的植入物160并当植入物160在导向件100、120之间进给时,防止其转动。
扩张器60的凹陷的远端壁和成槽的侧壁还便于使可选的适配器80接合于扩张器60。适配器80包括具有从那里向近端延伸的一对臂84、86的本体82。本体82包括与扩张器60前端或远端壁的外形匹配的外形。本体82包括适于配合在扩张器60的凹陷76内的中央偏移部分88。偏移部分88包括中央孔90,通过它植入物固定器140的远端延伸以接合植入物160,如下面所讨论的那样。臂84、86包括彼此相对延伸的凸起,诸如臂84上的凸起92。当将适配器80定位在适配器80所接合的扩张器60的前端或远端壁附近时,这两个凸起可释放地接合于扩张器60侧壁内的相邻制动结构。
当连接于扩张器60时,适配器80相对于导向件100、120通过一距离92将植入物定位在更远端,距离94至少对应于本体82的厚度和本体82和扩张器60的远端壁之间的任何间隔。这允许当扩张器60进给至导向件100、120的端部时,植入物160位于脊椎盘空间中的更远端。可以考虑将多个适配器80成组地设置以允许医师选择提供所要求偏移距离的适配器。
参阅图7,下导向件100可枢转地连接于壳体22的连接部分26。如图14所示,上导向件120可枢转地连接于壳体26的上指形件28、32。导向件的近端包括横向取向的闩,例如图14所示的闩132。闩132的端部被容纳在近端容纳口30、34相应的一个中,并靠向指形件28、32,这使导向件100保持枢转并可移动地连接于连接部分26的相对的、向下延伸的指形件。快速拆卸导向件100、120的能力使插入器件20能在手术过程完成后被清洗和除菌。它还允许导向件100、120以一组导向件的形式提供以用于共用的壳体、驱动件和植入物固定器。例如,以组的形式出现的导向件可包括各种长度、宽度或毗邻部分配置,由此医师可自手术期间进行选择。
导向件100、120从壳体22向远端延伸并界定将植入物插入相邻骨部分(例如椎骨220、222)之间的路径。如图8所示,导向件100包括从近端112向远端106延伸的本体110。本体110设有贯穿地沿本体110中轴线延伸的狭长导向槽102。导向槽102沿本体110的导向表面101和相对的外表面103开启。导向槽102从靠近近端112的位置向靠近远端106的位置延伸。导向槽102包括使下翼66增大的外端部通过的增大的近端开口108。导向槽102的剩余近端部分的尺寸被设置为可滑动地容纳下翼66的本体67,但防止翼66的增大的外端部从中通过。
同样,如图14所示,导向件120包括从近端130向远端126延伸的本体134。本体134可提供有沿本体134中轴线从中地延伸的狭长导向槽122。导向槽122沿本体134的导向表面121和相对的外表面123开启。导向槽122从靠近近端130的位置向靠近远端126的位置延伸。导向槽122包括使上翼64增大的外端部通过的增大的近端开口1128。导向槽122的剩余近端部分的尺寸被设置为可滑动地容纳上翼64的本体65,但防止翼64的增大的外端部从中通过。
导向件100可设有靠近远端106的毗邻部分104,它从外表面103凸出以接触毗邻的骨结构以限制导向件100插入到相邻骨部分之间的空间中的深度。导向件100的支承部分107从毗邻部分104向远端延伸并延伸入相邻骨部分之间的空间内,形成导向表面101和外表面103的延伸。导向件120设有从靠近远端126的外表面123凸出的毗邻部分124以与相邻的骨部分接触以将导向件120的插入深度限制在相邻骨部分之间的空间内。支承部分127从毗邻部分124向远端延伸并延伸入相邻骨部分之间的空间内,从而形成导向表面121和外表面123的延伸。
当安装于壳体22时,导向件100、120的导向表面101、121彼此朝向。支承部分107、127沿相邻骨部分的相邻表面延伸以便于植入物160插入到相邻骨部分之间的空间中。支承部分107、127还与相邻骨部分接触以将扩张力或牵引力分散到其上。可通过当植入物160和扩张器60在导向件100、120之间向远端进给时分开导向件100、120而使扩张力或牵引力作用于相邻骨部分。当植入物160位于相邻骨部分之间的空间内时,支承部分107、127还保护相邻椎骨终板。支承部分107、127可防止植入物160在进入骨结构之间的空间的入口处切入或接合于骨结构,并便于将植入物160插入相邻骨部分之间的空间中的所要求位置中。
现参阅图8,植入物固定器140如图所示地穿过驱动件40的通道54(图3)。植入物固定器140包括从远端144向在其近端的调整部分142延伸的狭长轴。调整部分142被容纳在手柄46的凹槽52中(图3)并可由医师握持以转动轴并因此使远端144接合于位于扩张器60前方的植入物160的母螺纹孔中。植入物固定器140将植入物160稳固地固定在扩张器60的前端壁或远端壁。其它实施例考虑远端144和植入物160之间的其它连接结构,包括过盈配合、咬配合、球塞和凹槽或其它合适的可释放连接结构。
下面将结合图10-18对一种利用插入器件20的过程进行说明。图10中示出连接于轴组件180远端的分离器头部170。分离器头部170包括锥形的前端鼻172以当脊椎盘空间塌陷时被插入椎骨220、222之间的脊椎盘空间224中。分离器头部170的本体向上表面174和相对的下表面(未图示)削锥。当分离器头部170被插入时,上表面和下表面将分开相邻的椎骨终板的分开高度定义为相应脊椎盘空间高度。分离器头部170还包括与轴组件180连接的近端凸缘部分176。在插入分离器头部170后,估算脊椎盘空间高度以确定是否已获得适当的脊椎盘空间高度。可考虑设置多个具有各种高度的分离器头部以顺序地将脊椎盘空间224分开。如果最后插入的分离器头部不提供所要求的脊椎盘空间高度,则选择第二分离器头部并使其配合于轴组件180以插入脊椎盘空间。重复该过程直至达到所要求的脊椎盘空间高度为止。
轴组件180的其它细节如图11所示。轴组件180包括延伸至远端连接端188的轴182。远端连接端188可移动地连接于分离器头部170以及图12所示的锉头190。在一个实施例中,远端连接端188包括外螺纹以接合凸缘部分176中的螺纹孔。其它实施例考虑使用其它连接结构,包括例如咬配合、快速联机连接器。另外还考虑将分离器头部170和锉头190一体地形成于轴组件180。手柄184被设置在轴182的近端以便插入和收回分离器头部和锉头。轴肩186围绕轴182形成在手柄184远端以提供承受来自脊椎盘空间的插入或收回分离器头部或锉头的冲击力的平台。
图12中示出锉头190,它还在近端凸缘部分196处可移动地安装于轴组件180的远端连接端188。锉头190包括锥形的前端鼻192和本体194。本体194的上表面和下表面包括粗糙面以便锉磨相邻椎骨终板的骨材料。本体194限定其上表面和下表面之间与插入到脊椎盘空间中的最末分离器头部的高度对应的高度。因此,可考虑在成组的多个锉头中提供锉头190,其高度与成组提供的分离器头部的高度对应。
在图13中示出一个槌200,它包括轴202和近端手柄204。冲击头206被设置在轴202的远端。冲击头206包括由其各侧上向远端延伸的臂208形成的U形凹槽210。臂208可定位在轴组件180的轴182周围。槌200可位于在轴肩186的近端以传递冲击插入力,或位于轴肩186的远端以传递冲击消除力。
在用分离器头部170将脊椎盘空间分开并用锉头190修整终板后,可用插入器件20将植入物160传递至脊椎盘空间。在图14中,上导向件120从下导向件100转离以便将植入物160置于扩张器60的前方。植入物固定器140随后转动以接合植入物160并使其靠向扩张器160前端地保持在位。如果需要,可将适配器80安装于扩张器60以使植入物160相对于导向件100、120和扩张器60处于更远端。驱动件140相对于壳体22完全地向近端收回或撤回,以使扩张器60对齐于增大的槽部分108、128,从而允许导向件100绕上翼64下降至图15所示的位置。
在一个实施例中,考虑从一组具有各种高度和或上表面与下表面之间具有各种夹角的植入物中选择植入物160。选择合适高度的植入物以提供与提供分开时所要求脊椎盘空间高度的分离器头部对应的高度。这样,当插入植入物时,由于其配合的指示已由最后插入的分离器头部所提供,因此它配合在脊椎盘空间中。
通过从脊椎盘空间224中除去分离器和锉磨器件,植入物160被装载入插入器件20。导向件100、120的枢转连接结构允许远端106、126彼此靠近地设置。这提供允许将支承部分107、127定位在脊椎盘空间220中的低外形结构,即使脊椎盘空间由于分开的除去而塌陷。毗邻部分104、124与相邻椎骨体接触地定位,防止导向件100、120过度插入于脊椎盘空间中。在所示实施例中,毗邻部分104、124正交地取向于导向件的中轴线,与导向件100、120对齐以使植入物插入途径沿着或平行于脊椎手术中的下垂面或冠状面。在另一实施例中,毗邻部分倾斜取向于导向件104、124中间纵轴线以便导向件100、120沿倾斜地取向于脊椎手术中的下垂面或冠状面的途径。
在图16中,通过使驱动件40在壳体22中转动、使驱动件40向远端进给,植入物160在导向件100、120之间向远端进给并因此扩张器60和植入物160沿导向件100、120进给。当植入物160和扩张器60进给时,导向件100、120彼此张开或分开。这种分开使支承部分107、127将分开力施加于椎骨终板上,将椎骨220、222分开充分距离以将植入物160容纳在它们之间。
在图17中,植入物160被固定在脊椎盘空间224中。翼64、66分别与椎骨体220、222附近的毗邻部分104、124对齐。然而,支承部分107、127被定位在所插入的植入物160和相邻的椎骨终板之间,这使得将插入器20从脊椎盘空间抽回变得困难。翼64、66分别包括沿各本体65、67向近端成锥形至各翼64、66增大的外端的远端壁。这允许翼64、66更顺应椎骨220、222在终板和椎骨体外表面之间的过渡处的解剖结构,并在植入物160位于脊椎盘空间224所要求的位置时,防止翼64、66接触椎骨体。
槽102、122延伸通过毗邻部分104、204,提供相对于导向件100、120进一步进给扩张器60的途径。在图18中,驱动件40已转动以完全将扩张器160位移至槽102、122的远端。由于翼64、66接触相邻的椎骨220、222,扩张器60无法向脊椎盘空间进给。因此,翼64、66作用于椎骨体220、222以向近端位移导向件100、120,从植入物160和相邻椎骨终板之间的空间中抽出支承部分107、227。这使得插入器件20方便地从工作点抽回,而不会造成缠绕或冲击,它们会破坏将植入物定位在脊椎盘空间中。
当将导向件100、120之间的植入物160导入相邻骨部分之间的空间时,植入物160的定位通过导向表面101、121与植入物160的接触而被控制在头部/尾部方向。导向表面101、121使植入物160与相邻骨部分之间的空间对齐。植入物160沿导向件100、120的横向定位受到植入物固定器140与植入物160的接合的控制,以使植入物160不会从导向件100、120之间滑出,在那种情况下会接触或损伤组织、神经、脉管系统或在其间空间的途径上的骨部分附近的其它组织结构。扩张器60的翼64、66延伸通过上导向件和下导向件中的槽,并且居中定位以最小化对周围组织的侵入。扩张器和导向件结构还便于将植入物快速装载和卸载于导向件之间。
本文所描述的器件通过防止植入物160和扩张器60在插入时缠绕并移出导向路径而保护插入过程中附近组织和脉管不受植入物的影响。这些器件还保护在插入时在其间插入植入物的骨结构,并且在被定位在骨结构间的空间中之后,便于将植入物取回。此外,可调整该器件以引导各种高度和各种锥形角的植入物的插入,并在相邻骨部分之间提供专门配合特定植入物的可变距离。器件包括操作空间中的低外形,便于实现在对骨结构的工作路径中的附加器件的可见与定位。这些器件可简单拆卸以清洗和使用从一组导向件中选择的导向件,提供手术过程中医师的便利性和灵活性。
本文所述的植入物可以是允许相邻骨部分融合的融接植入物。这类融接植入物可填满骨增长促进材料和/或治疗剂。本文所述的植入物还可以是可在导向件100、120之间移动的任何脊椎植入物,例如中间体间隔物、人造脊椎盘部件或装置或其它要求定位在相邻骨部分之间的植入物。

Claims (40)

1.一种用于将植入物插入相邻骨部分之间的空间内的器件,包括:
壳体;
连接于所述壳体的一对相对的导向件,所述一对导向件的每一个包括具有外表面和相对的导向表面以及在所述外表面和导向表面之间的狭长槽的本体;所述槽沿所述各个导向件延伸,所述植入物可定位在所述导向表面之间;
定位在所述成对的导向件之间的扩张器,所述扩张器包括中央体和从所述中央体延伸的一对相对的翼,各翼滑动地容纳在所述成对导向件的相应的一个的所述槽中;以及
连接于所述扩张器并可操作以向前进给所述扩张器以及位于所述扩张器前方的植入物朝向所述导向件远端的驱动件。
2.如权利要求1所述的器件,其特征在于,所述壳体包括连接部分和从所述连接部分向近端延伸的驱动件接合部分,所述驱动件接合部分和所述连接部分包括贯穿地延伸以容纳所述驱动件的通道。

可释放地连接于所述扩张器的适配器,而且所述适配器的本体部分沿所述扩张器的远端壁;以及
连接于所述扩张器并可操作以向前进给所述扩张器以及带有位于所述适配器前方的植入物的适配器朝向所述导向件远端的驱动件,所述适配器隔开在所述扩张器的远端壁的前方的植入物。
21.如权利要求20所述的器件,其特征在于,所述各对导向件包括在所述外表面和所述导向表面之间的狭长槽,所述槽沿所述各导向件的中轴线延伸。
22.如权利要求21所述的器件,其特征在于,所述扩张器包括中央体和从所述中央体延伸的一对相对的翼,所述翼可滑动地容纳在所述成对的导向件的每一个的所述槽中。
23.如权利要求20所述的器件,其特征在于,所述适配器包括从所述本体部分的相对两端向近端延伸的一对臂,所述一对臂可沿所述扩张器的侧壁定位以将所述适配器接合于所述扩张器。
24.如权利要求23所述的器件,其特征在于,每个所述臂包括在其远端的凸起,所述凸起彼此朝向地延伸并可移动地定位在所述扩张器所述侧壁中的相邻制动结构中。
25.如权利要求20所述的器件,其特征在于,所述扩张器包括在所述远端壁中向近端延伸的中央凹陷,而所述适配器本体部分包括容纳在所述凹陷中的中央近端偏移部分。
26.如权利要求25所述的器件,其特征在于,所述扩张器包括延伸通过其中的孔,并且所述适配器包括与所述扩张器的所述孔对齐的所述凹陷中的开口。
27.如权利要求26所述的器件,其特征在于,还包括延伸通过所述孔和所述开口以接合所述适配器前方的植入物的植入物固定器。
28.如权利要求27所述的器件,其特征在于,所述植入物固定器包括容纳在延伸通过所述驱动件的通道内的长轴。
29.如权利要求20所述的器件,其特征在于,所述至少一个导向件枢转地连接于所述壳体,所述至少一个导向件枢转离开另一个所述导向件,以允许所述适配器接合于所述扩张器。
30.一种将植入物插入相邻骨结构之间的空间内的方法,包括:
提供植入物插入器,包括:
壳体;
一对连接于所述壳体的相对的导向件;
位于成对导向件之间的扩张器,所述扩张器包括中央体和从所述中央体延伸的一对相对的翼,所述翼滑动地容纳在沿成对导向件的每一个的中轴线形成的槽内;
连接于所述扩张器和驱动件接合部分的驱动件;
使成对导向件的至少一个枢转离开成对导向件的另一个,以使翼移离至少一个导向件的槽;
将植入物定位在一对导向件之间和扩张器前方;以及
使至少一个导向件向另一导向件枢转,以将扩张器的翼定位在至少一个导向件的槽中。
31.如权利要求30所述的方法,其特征在于,所述槽包括增大的近端开口并枢转至少一个导向件,以将植入物定位在槽的增大的近端开口内。
32.如权利要求30所述的方法,其特征在于,还包括在定位植入物前沿所述扩张器的远端壁连接一适配器。
33.如权利要求32所述的方法,其特征在于,连接适配器包括将适配器向近端延伸的臂接合于扩张器的侧壁。
34.如权利要求32所述的方法,其特征在于,还包括将植入物接合于植入物固定器,所述植入物固定器包括向适配器远端延伸的远端、延伸通过所述驱动件的轴以及具有调整部分的近端。
35.如权利要求32所述的方法,其特征在于,还包括从一组适配器中选择适配器。
36.如权利要求30所述的方法,其特征在于,还包括将植入物接合于植入物固定器,所述植入物固定器包括向扩张器远端延伸的远端、延伸通过所述驱动件的轴以及具有调整部分的近端。
37.如权利要求30所述的方法,其特征在于,还包括:
将导向件的远端支承部分定位在所述空间中;
操作所述驱动件,以使在导向件之间的扩张器和植入物向远端的空间进给;以及
当植入物和扩张器的远端进给使导向件彼此移离时,用支承部分分开脊椎盘空间。
38.如权利要求37所述的方法,其特征在于,还包括使植入物向远端进给至所述空间,直到诸翼接触相邻的骨部分。
39.如权利要求38所述的方法,其特征在于,还包括通过操纵驱动件以将翼推抵相邻骨部分而从植入物和相邻骨部分之间的空间收回支承部分,由此使导向件相对骨部分和植入物向位移。
40.如权利要求30所述的方法,其特征在于,还包括从一组导向件中选择上导向件和下导向件。
