Abstract
Description
Gegenstand der ErfindungSubject of the invention
The invention relates to a device for cleaning abscess cavities in humans or animals.
An abscess cavity is an accumulation of pus in a cavity in the body which is not preformed but formed by an inflammatory tissue fusion. Abscess cavities can in principle arise anywhere on the human or animal body.
The application of the invention presented here was carried out in patients with a pelvic abscess as a complication of a surgical procedure on the rectum.
The use of the device is also possible at other locations with poorly accessible and delayed healing Abzesshöhlen such as an abscess in Retroperitoneum with pancreaticocutaneous fistula in the pancreas or a fistula system in osteomyelitis in the context of bone infection on the trunk or the extremities.
Based on the own clinical experience with the pelvic abscesses, the application of the device for the treatment of abscess oils will be explained in the following.
Stand der TechnikState of the art
After removal of intestinal sections z. As after cancer, circulatory disorders in the rectum or severe inflammation, it is necessary to reunite the healthy bowel ends (attachment of an anastomosis). This is done either by stapling or by hand suture, in about 10% of patients it comes as a complication to a so-called anastomotic insufficiency, d. H. a bursting of the intestinal suture with suppuration in the pelvis or the free abdominal cavity. Of these, only about 1400 patients / year are affected in Germany alone. Escaping from the hole in the anastomosis intestinal secretions in the abdominal cavity or pelvis to severe inflammation. In severe inflammation in the abdominal cavity, the patient then has to be operated on again. Renewed surgical measures, drainage and purely medical therapies are rarely successful or associated with a high mortality rate. With limited inflammation only in the pelvis, the further therapy can often be performed endoscopically under the protection of an upstream artificial intestinal outlet.
The traditional treatment of anastomotic leakage includes procedures such as drainage, the administration of broad spectrum antibiotics, parenteral nutrition, as well as the creation of a protective artificial intestinal outlet. The diagnosis of anastomotic leakage is usually made endoscopically. Previous therapy and state of the art are divided into:
a) optisch kontrollierte Spülung durch flexibles Endoskop oder starres Proktoskopa) Optically controlled irrigation through flexible endoscope or rigid proctoscope
B. Metronidazol). For many decades, the gold standard for treating anastomotic leakage has been the programmed endoscopic irrigation of the pelvic abscess. Either this is done flexibly endoscopically with a gastroscope or rigidly with a proctoscope. As a rule, saline solution is used as flushing medium. Some doctors additionally rinse the abscess cavities with an antibiotic (eg metronidazole).
b) blinde Spülung transanal über herkömmlichen Katheter (z. B. Blasenkatheter)b) blind irrigation via conventional catheter (eg bladder catheter)
Here, a tube is placed blind in the rectum and rinsed the region.
c) endoskopische Vakuumtherapie mit dem Endo-Sponge-Systemc) endoscopic vacuum therapy with the endo-sponge system
In addition to the irrigation of the abscess cavities via the endoscope, another therapy option has been available since 2006: Endoscopic Vacuum Therapy (VAC) developed by Weidenhagen and Grützner. Here, a large-pored polyurethane sponge is endoscopically placed in the abscess cavity in the pelvis. The sponge is connected via a hose and an external vacuum bottle. After opening the vacuum bottle, the abscess cavity is cleared by the continuous suction. Two to three times a week, the sponge in the pelvis and the vacuum bottle must be changed. During the time of VAC treatment, the patient is permanently affected by the drainage tube from the anus and attached to the vacuum bottle. With the VAC therapy a much faster cleaning of the abscess cavity succeeds compared to the sole irrigation.
Apparatus and method for rectal cleaning, in particular for patients who have to undergo bowel surgery (
The proctoscope is inserted using an opturator, the opturator is withdrawn, and a stopper with a seal is inserted into the distal end of the proctoscope. A catheter is delivered through the sealant and into the patient. Liquid passes through an outlet port into a receptacle provided by the user

e) Sanierung durch erneute operative Revisione) Rehabilitation through renewed operational revision
Here, the inflammation in the pelvis is to be definitely supplied by another surgical intervention. For this purpose, various approaches are known.
Kritik des Standes der TechnikCriticism of the prior art
a) optisch kontrollierte Spülung durch flexibles Endoskop oder starres Proktoskopa) Optically controlled irrigation through flexible endoscope or rigid proctoscope
This has been the gold standard of insufficiency therapy in surgery for decades. The disadvantage here is that this therapy can only be performed by the experienced doctor. As a rule, the patient or his spouse can not independently endoscopy and rinse at home. The procedure can only be performed in the clinic. Thus, the outpatient treatment also incurs high costs due to the arrival of the patient over months and thus also loss of time, medical work time, nursing assistance, material consumption and reprocessing of the endoscopes.
b) blinde Spülung transanal über herkömmlichen Katheter (z. B. Blasenkatheter)b) blind irrigation via conventional catheter (eg bladder catheter)
With blind advancement of the rinsing catheter in months of repeated use, the reproducibility of the catheter layer is not given. Even with blind placement of the conventional catheter directly in the cavity, the patient alone at home would fill the abscess cavity excessively without feedback until the excess irrigating fluid passes from the sphincter to the bed next to the catheter.
This is hygienic and would significantly reduce the cooperation of the patient. If the patient rinses blindly only with a small flush volume at home, he does not achieve adequate cleaning and healing of the cavity. Even with external marking of the height of the leak, a single-lumen catheter has the disadvantage that it could only be flushed or drained. The uncontrolled blind feed could lead to a penetration of only eienge mm thick abscess membrane with the catheter into the free abdominal cavity and thus to the injury of intestinal structures or larger blood vessels.
A continuous flushing with simultaneous drainage through the few cm behind the flushing hole drainage holes directly in the abscess cavity are thus not given.
The blind conditioner is a pelvic abscess cavity is dangerous and not very effective and therefore has after the introduction of flexible endoscopy no more importance in the developed world.
c) endoskopische Vakuumtherapie mit dem Endo-Sponge-Systemc) endoscopic vacuum therapy with the endo-sponge system
For the VAC therapy as therapy can be performed only endoscopically by the doctor. This results again in the course of high costs due to repeated application. With VAC therapy, the abscess cavity is significantly faster than the optically controlled endoscopic irrigation. After the abscess cavity has been cleared, many weeks and months pass until the abscess cavity is completely healed. Premature termination of VAC therapy causes recurrent severe inflammation in the pelvic recurrent pelvis in many patients. The remaining anal sphincter prevents leakage of inflammatory secretions from the intestine and pelvis. Especially with already chronic abscess cavities (in our experience older than 12 months) or an inoperable local recurrence of the cancer in the cave is no longer to expect a healing of the cave. During the time of VAC treatment, the patient is off above the drainage tube The anus permanently impaired and bound to the vacuum bottle. During VAC therapy, the patient continuously has the troublesome drainage tube in the anus, which can lead to local pressure sores or bleeding from haemorrhoidal plexus, and is "tied up" to the vacuum bottle for 24 hours a day, limiting his mobility for weeks to months.
All of these systems are aimed at complete defecation in intact intestine. The rigid distal end with mushroom or pistol grip like insertion part in blind placement in the rectum could open the ready torn intestinal suture on blind insertion or seal the lateral hole in the intestinal suture to the pelvic abscess with the large, rigid insertion part and only one via the central rinsing attachment Cleansing of healthy, located above the intestinal suture reach large intestine. The pelvic abscess would be in these rigid systems can not be cleaned, since opening the irrigation and opening of the drainage holes are many inches apart and can not be placed simultaneously blindly in the abscess cavity by the doctor under view or the patient. The systems described above are widely known and widely used in daily use for their respective applications. None of these rectal irrigation systems is used to treat pelvic abscesses after anastomotic leak because of these obstacles.
e) operative Sanierung der Insuffizienze) surgical rehabilitation of insufficiency
First solution: under the protection of an upstream plasticizer, the abscess cavity is rinsed under mild endoscopy until complete healing under endoscopic control ("classic endoscopic irrigation treatment"). Some clinics also favor the rinse on the upstream Kunstafter.
Second solution: irrigation was carried out via the intraoperative pelvic drainage until complete healing of the leak after weeks or months. Here is a dressing change daily to make the drainage. The drainage leads to a disturbance of patient comfort in daily life. There is a risk of premature drainage loss when loosening the retaining thread.
Third solution: the complete removal of the rectum by surgery in the sense of an extirpation. Thus, the patient loses the possibility of a later return displacement of the artificial intestinal exit, which then becomes a permanent solution for him. It again results in a significant limited quality of life through the permanent artificial intestinal output.
The final cure can be delayed by months or become impossible. A rapid return of the temporary artificial intestinal outlet is then no longer possible. As a last resort in these patients, the last piece of rectum with the sphincter must be completely removed in another operation. These patients are then dependent on a permanent artificial colon exit.
The renewed procedure takes place via an abdominal or pelvic floor train in already inflamed area. The occlusion of the hole by mowing or the complete removal of the lower rectum with discharge of the upper colon as a permanent artificial bowel outlet are at high risk for a renewed bursting of the intestinal suture or re-inflammation in the former area of the rectum.
Aufgabe der ErfindungObject of the invention
It is the object of the invention to develop a device for cleaning abscess cavities in humans or animals, which can be applied by natural and / or pathogenic and / or artificially created access routes to the abscess cavity by a physician and / or the patient himself.
Erfindungsgemäße Lösung der AufgabeInventive solution of the problem

multiple use 2 , (11 ) - - Closed drainage bag for single use (
8th ) (see picture) - - Connection to fixed suction system (eg use in the intensive care unit)
- Open drain in open container as desired by some patients
In our application we chose the open bag with drain plug for multiple use.
Alternatively, a catheter system without variable locking with prefabricated stop lengths (eg 2-20 cm in length) would also be possible here. The fixed stop of the end position in the body is easy palpable.
The required height for the external stop (g) must be determined individually for each patient by a parallel examination with a horizontal irrigation catheter and a flexible endoscope. Depending on the surgical procedure, the hole in the intestinal suture can be between 1-12 cm away from the outer sphincter. The mass of the leaks is between 2-6 cm height. Intestinal sutures that are above 12 cm can hardly be supplied with this flushing system. For surgery involving a rectal stump with a hole that has appeared to be a complication, the irrigation system can easily be placed through the intestinal hole in pelvic abscesses that are much larger than 12 cm in length.
