Description

The present invention relates to a device for the treatment of Varices (i.e. varicose veins) using radio frequency energy after the Preamble of claim 1.

So far, varices have been used primarily when it comes to larger diameter varices acts, surgically removed, in such a way that the treating physician cuts along each the varice is placed and the diseased vessel is removed in sections over the incisions. The problem here is that such a treatment for the patient on the one hand is very stressful, on the other hand there is a risk that due to the cuts Scars remain.

In addition, there are already devices for cutting or coagulating tissue using radio frequency energy. This is done with a rigid needle works with which, for example, the skin is pricked and a comparatively short ab a varice is treated. With varices, however, it is necessary to use the vessel to treat over longer lengths, which when using the above treatment method causes the needle to be inserted again and again. The treatment This takes place under local anesthesia, which leads to the fact that in the area of Local anesthesia can pull blood vessels together reflexively. Here there is the problem that when the needle is inserted again, it is very difficult will properly pierce the vessel.

No. 5,437,664 describes a device for the treatment of vascular defects described, which is a probe body with a bendable segment has, the direction of bending by a direction indicator also at inserted probe body is recognizable from the outside. This allows the treating doctor better lateral vascular pathways, the course of which is known reach without having to separately open and treat lateral vessel sections Need to become. A patient-friendly treatment long and wavy vessel sections is not by the present device enables.

The object of the present invention is to provide a novel device for treatment to provide varices with which one on the one hand a patient-friendly and on the other hand efficient, d. H. time-saving treatment can be made without the need for cuts.

The above object is achieved in the generic device in that Treatment instrument a probe to be inserted into the vessel is provided, the egg NEN elongated flexible probe body and one at one end Chen probe head for the exposure of the vessel to radio frequency energy. The probe is completely inserted into the diseased vessel and under high frequency exposure slowly pulled out of the vessel, causing the Ge coagulate vessel walls (weld them together). This makes it possible for vessels treat over long distances in a simple manner, without having to repeatedly must be stung. Apart from the puncture site at the insertion area, the No need for stitches or cuts. The invention ensures that the The cost of an operative intervention is completely eliminated.

The flexibility of the probe body is expediently such that the When inserting the probe into the vessel, insert the probe following the course of the vessel can be pushed.

A guide (in particular a so-called te "vein indwelling cannula") is provided, which preferably with a stitch position con trolls, d. H. a control to check whether the vessel has been pierced can be led. However, a tour is not always necessary. Without guidance the probe can be placed during surgery (e.g. crossectomy of the Venasaphenamagna).

For this purpose, the guide expediently has a needle that has an outer jacket lung is surrounded. The needle is out of the sheath after placing the guide extractable, d. H. removable.

The needle is more useful for realizing the "stitch position control" feature as a hollow needle, preferably with one end facing away from the tip provided provided cavity. Something flows when the needle is inserted into the vessel Blood into the cavity, indicating to the attending physician that he has the needle correctly placed.

To effectively seal the laid during use To achieve the probe, the diameter of the probe body is dimensioned such that it seals the vessel together with the casing.

For this purpose, the diameter of the probe body expediently corresponds essentially the diameter of the probe head.

The probe body comprises the transmission of the high-frequency energy to the probe head by means of a metal wire and a plastic sheath surrounding the metal wire. This can expediently also consist of Teflon.

To the doctor to be treated, the exact detection of the probe head within the To enable the vessel, the probe body is provided with a scale, which in the front area, d. H. the area facing the probe head is advantageously finer can be divided. The doctor to be treated can during the gradually withdrawing the probe from the variceal position of the son closely monitor the mind in terms of leadership.

In order to avoid damage to the surrounding, healthy tissue, according to ei ner further embodiment of the present invention, the temperature in the range of Probe head detectable.

The probe body consists of temperature-conductive material, at least it includes such. As an alternative to this, the probe body can also be temperature-conductive have coating.

In addition to this, the device can display a temperature and / or one via the Have temperature-activated warning sounder. It is also useful to Control circuit with automatic shutdown of the probe when a job is exceeded temperature or a working temperature range.

In a special embodiment, the metal wire is made of steel or tungsten. The probe head consists of a highly conductive metal, e.g. B. from Au, Pt or Ag or a corresponding metal alloy. It is also conceivable that the Me tall wire and probe head, especially for very thin probes made of one and the same ben material exist, the metal wire only in the region of the probe head is exposed, d. H. there is no insulation.

To avoid injuries, the probe head can be rounded the front be designed. This will result in premature perforation of the veins wall and thus prevents the probe from leaving the vein uncontrolled. This also leads to better management, i. H. an easier advancement of the probe guaranteed within the vein.

For the treatment of vessels of different sizes, it is advisable to use a probe body to provide with different diameters and / or lengths, depending on the Be may be used on the device according to the invention.

To make it easier to insert the probe into the diseased vessel, an additional che guide sleeve be useful, which previously in the vessel to be treated is brought. The guide sleeve can be inserted together with the probe be carried out in batches or in stages.

Reference number 1 in FIG. 1 shows the device according to the invention in its entirety. It comprises a flexible, elongated probe 2 consisting of a handle 17 , a z. B. made of tungsten probe body 3 and a probe head 4 arranged at the end of the probe body 3 for emitting high-frequency energy.

The probe body 3 is inserted into the diseased vessel 10 via a guide 5 . Reference numeral 11 in Fig. 1 denotes the various skin layers of the human body.

The probe is connected via a cable 14 to the high-frequency source 14 or the housing 13 of the same. The high-frequency source 14 also has a display 20 and an adjusting device 15 for delivering the high-frequency energy.

The guide comprises in FIG. 2, a hollow needle 6 having a lumen 8 which serves as a stitch position control. When the stitch is correctly placed, the cavity 8 fills with blood. The guide 5 is designed in such a way that after the sheath 7 has been placed in the skin, the needle is removed and the probe body can be inserted.

According to FIGS . 3a), b) and c), the probe body 3 has an inner tall wire 12 , a plastic surrounding the metal wire, in particular Teflon coating 9, and a temperature-conductive coating 19 . According to FIGS . 3b) and c), the temperature-conductive coating extends only over part of the circumference area of the probe body. The temperature at the probe head 4 can be "felt" and displayed on the display 20 in a simple manner via the temperature-conductive layer 19 . It is also possible to activate a warning signal (not shown) when a certain temperature is exceeded.

A control loop (not shown in the figures) is expediently provided for an automatic shutdown when a working temperature is exceeded or of a working temperature range.

According to Fig. 4, the probe head 4 has a cylindrical shape equipped with front-side rounding off, wherein according to Fig. 4 (a), the shell 7 and the probe head 4 form a uniformly extending surface, or as shown in FIG. 4 (b) to form the Son 4 denkopfes the foremost piece of the metal wire is not from the coating 9 ben. After the insertion of the probe body 3 in the guide 5 , this ensures that no blood can escape during the treatment via an intermediate space between the guide 5 and the probe body 3 .

In certain cases, especially with very thin probes, it is advisable to use the To manufacture metal wire from the same material as the probe head, d. H. this to make both parts available in one part, so to speak. The probe head will  here by the zone of the metal wire not covered by the coated zone educated.

According to a further embodiment, the probe body 3 can additionally be surrounded by a guide sleeve 21 , the guide sleeve 21 serving the purpose of stabilizing the interior of the vessel somewhat to facilitate the insertion of the probe. According to a special embodiment of the invention, the guide sleeve is more flexible than the probe 2.

For the treatment of a varix, the guide is inserted through the needle 6 through the skin 11 into the vessel to be treated 10 and the needle 6 is subsequently removed. Via the cavity 8 it can be seen that the vessel to be treated has actually been hit. The probe is then inserted through the guide 5 into the treatment vessel over the entire length of the treatment. The dimensioning of the probe head 4 and probe body 3 including the guide 5 bring about an automatic sealing of the guide 5 .

If local anesthesia is used, the puncture site with Loka is sufficient pretreat lanesthesia cream. After placing the probe you can then along the Varize a local anesthetic to be injected. Should multiple varices be treated? it is advantageous to place several probes in parallel under anesthesia and the maximum Do not exceed dose of local anesthetic.

Since the patient is anesthetized before placing the probe, it is expedient to simultaneously place a plurality of probes for the treatment of a plurality of vessels during a treatment. Once the probes are in place, the respective vessel is made as bloodless as possible (e.g. by putting it up, putting on a tourniquet, or compressing the wrap). The vessel in question is then treated under the influence of high-frequency energy (for example in the megahertz range), the vessel walls welding together, ie coagulating. During the treatment, the probe is gradually pulled out of the vessel from the most distant to the puncture point in the vessel, whereby the vessel walls are welded continuously. On the high-frequency generator itself, for example when a certain temperature is exceeded, the high-frequency energy can be changed using a setting button 15 (see FIG. 1).

Depending on the varices to be treated in each case, probe bodies 3 of different lengths or strengths can be used for each device.

At the front of the probe body 3 is the probe head 4 in the form of a flattened ball. The probe body 4 consists, for. B. made of metal wire or tungsten. However, other materials for this are also conceivable.
