Abstract

The method involves using an X-ray system with an X-ray image recording system for the detection of digital X-ray images, and a computer arrangement for determining relevant bone contours and bone structures, especially the passage from cortical to spongy bone contour. At least one first and one second digital X-ray image are recorded by using X-ray radiation of different energy spectra, and a subtraction image is derived through digital subtraction of both images. A contour image is produced through mathematical processing of the subtraction image, to determine the bone contours under application of a computer-based contour recognition method. The contour image is used for the planning and/or selection of the artificial limb implant.

Description

The invention relates to a method for determining the planning and / or selection of prosthetic implants rele vanter bone contours and structures, in particular the transition from cortical to cancellous bone structure, using an x-ray machine with an x-ray image acquisition system for the acquisition of digital X-ray images and egg ner computing device.

The selection of prosthetic implants is based on the largest today partly on visual and mechanical measurement of x-rays film. A special care and experience must be recognized the cortical-cancellous transition in the bone shaft, for example, a femur neck become. The most exact knowledge of the proportions the bone to which the implant is to be attached, is for the planning or selection of the implant from besonde of importance, so neither during the insertion of the Implan difficulties still occur afterwards. For example in the case of a femoral neck implant, the wedge of the Implant that is inserted into the femur tube If it becomes too thick, this may already be the case during insertion cause the bone to burst. Too thin or too A short wedge can also come off the knot after it has been inserted Chen solve what a re-operation and remarkable Brings problems for the patient. The so far be Known measurement of ordinary x-rays, however a sufficiently precise determination of the proportions and especially the recognition of the cortical-cancellous transition only imprecise too.

The invention is therefore based on the problem of a method indicate by means of which the relevant bone contours can be determined with sufficient accuracy so that the einu setting prosthesis implant exactly planned or a precisely fitting can be selected.

To solve this problem, a method of the type mentioned is characterized by the following steps:

  • - Aufnahme wenigstens eines ersten und wenigstens eines zwei ten digitalen Röntgenbildes, wobei zur Aufnahme des ersten und des zweiten Bildes Röntgenstrahlung unterschiedlicher Energiespektren verwendet wird,- Inclusion of at least a first and at least one two th digital X-ray image, whereby to record the first and the second image different x-rays Energy spectra is used
  • - Erzeugung eines Subtraktionsbildes durch digitale Subtrak tion der beiden Bilder,- Generation of a subtraction image by digital subtract tion of the two pictures,
  • - Erzeugung eines Konturbildes durch rechnerische Bearbeitung des Subtraktionsbildes zur Bestimmung der Knochenkonturen un ter Verwendung eines rechnergestützten Konturerkennungsver fahrens, wobei das Konturbild für die Planung und/oder Aus wahl des Prothesenimplantats verwendet wird.- Creation of a contour image by arithmetic processing the subtraction image to determine the bone contours un ter use of a computer-aided contour recognition ver driving, the contour image for planning and / or off choice of prosthetic implant is used.

According to the invention, the contour of the examined bone and thus the relevant areas of the same through a special recording technology and image processing optimized and visualized. The two X-rays taken, the according to the invention recorded in quick succession to avoid motion artifacts, use X-ray gene radiation with different energy spectrum added. This makes the object softer or harder structures more pronounced in one picture, weaker in the other shown. Then these two digital X-rays gene images by means of an appropriate image processing direction digitally subtracted to create a subtraction image hold. In this is a clearer differentiation of the relevant structures, e.g. in the area of cortical-cancellous transition, recognizable, d. that is, the rele vant contours stand out clearly. Then that will  Subtraction image using a computerized Contour recognition process, i.e. using a suitable neten contour detection algorithm postprocessed to calculate to determine the exact bone contour and in the form of a To represent contour image. This contour image that is now due the contours are already quite clear and precise the subtraction image was created as a result reproduces these contours very precisely, is then for planning or selection of the prosthetic implant used. The one selected based on the contour image and then implanted prosthesis implant is the circumstances of the Bones adjusted extremely precisely, so that the aforementioned problems will not arise.

According to the invention, as part of the planning and / or selection of the prosthetic implant in one or the computer device filed with the contour image for determining the match comparable template-like implant images are used the. On the basis of these implant images, which outline or For reproducing existing implants, a formal and / or the size of the kno shown contour and the existing implant shape are sought, so that the selection or planning is carried out in a simple manner men can be. You can use the contour image and an implan tatbild according to the invention are displayed on a monitor, wherein the implant image is placed over the contour image or both images are displayed side by side. In particular the overlay of the two images allows a simple ver match too. The selection and / or overlay of the Im plantbilds and / or the display side by side can be done automatically by means of the computer device. In In this case, the computer device chooses to do this for example, relevant parameters shown in the contour image ten contour determined in advance, from the large number of deposited template-like implant images one or more implant pictures from which or which subsequently in superimposed or display side by side together with the  Contour image can be reproduced. That is, the computer hits here is a pre-selection regarding the relevant Im plant templates. Alternatively, the user can of course select the respective implant image yourself and for example using an operating mouse, a joy Sticks or the like. The implant template through the contour image Move overlay. Alternatively, it is further possible that the computing device makes the selection that However, overlay must be carried out by the operator himself.

To generate the various in their energy spectrum X-rays to record the first and second According to the invention, the x-ray image can be operated on an egg X-ray source serving the operating voltage supplying generator switched on different operating voltages are provided, d. that is, the x-ray source is each operated with different high voltage, the generated The x-ray pulse therefore has a different ener content. Alternatively, it is also possible to generate the different X-rays in the energy spectrum Bring filter media into the beam path, for example in the form of a copper filter that contains certain radiation components from the radiation supplied by the X-ray source filters out. In this case, for example, the first Image without a filter, the second with a filter.

In addition to the method, the invention further relates to an x-ray system comprising an x-ray image recording system with an x-ray emitting x-ray source and an x-ray receiver for recording digital x-ray images, and an image processing device, the x-ray system being suitable for carrying out the method described above. In this X-ray system according to the invention is

Fig. 1 shows an X-ray system 1 according to the invention, consisting of an X-ray image recording system 2 comprising an X-ray source 3 , which is supplied with operating voltage via a generator 4 , and an X-ray receiving device 5 in the form of a digital output signals provide the image detector. Furthermore, an image processing device 6 for processing the digital X-ray images supplied by the X-ray receiving device 5 and a computer device 7 for further processing of the images supplied by the image processing device are provided. The images determined by the image processing device 6 or the computing device 7 can be output on a monitor 8 .

As part of the operation of the x-ray system 1 for performing the method according to the invention, two x-ray images of the object O, for example the region of the thigh neck, are taken. The first x-ray image is recorded with an x-ray radiation emitted by the x-ray source 3 having a first energy spectrum, for example a first energy content, and is stored in the image processing device 6 . Subsequently, in order to avoid any movement artifacts in a very short succession, a second x-ray image with an x-ray radiation different in energy content or in the energy spectrum compared to the first x-ray radiation used is recorded, the digital image signals of this x-ray image also being stored in the image processing unit 6 . To generate or influence the x-ray radiation, either the generator 4 can be varied with regard to the operating voltage supplied to the x-ray source 3 , additionally or alternatively, filter means 9 can also be brought into the beam path during image recording, which filter out radiation components of a certain wavelength and so on Influence energy spectrum.

A subtraction image is now created in the image processing device 6 by digital subtraction of the two x-ray images obtained. Due to the different energy content of the X-rays used, harder and softer structures of the recorded object O are differently emphasized and emphasized in the two X-ray images. Due to the digital subtraction processing, the contour or structural boundaries in the area of the transition from hard to soft bone material or, for example, from the bone to the articular cartilage can be seen much more clearly than on one or the other x-ray image itself. Relevant structures such as the area of the cortical bone Cancellous transition in the area of the bone cavity can be clearly differentiated.

The subtraction image obtained is then computationally processed in the computing device 7 to generate a contour image. For this purpose, a computer-aided contour detection method using contour detection algorithms is used and the exact bone contour is determined, ie, the contour or the exact bone profile is calculated using the already very exact subtraction image, so that a more accurate contour image is obtained. This contour image is then the basis for the planning and / or selection of the prosthesis implant to be used. For this purpose, several template-like implant images are stored in the computer device 7 , which can then, if the corresponding implant is to be determined, be output together with the contour image on the monitor 8 , so that the implant image can be determined which best suits the certain bone contour fits.

In this case, the selection of one or more suitable implant images can be carried out by means of the computer device 7 itself, for which purpose the parameters, such as the diameter of the bone cavity, the radius of the joint head, etc., are determined based on the bone contour image, and these parameters are used to determine the or the if necessary, selects relevant implant images and automatically displays them on the monitor 8 , wherein these images can be superimposed or displayed side by side. Manual selection is of course also possible.

Fig. 2 shows a sectional view in the form of the upper region of a femur 10th At the joint head 11 there is the articular cartilage 12 , which is received together with the joint head 11 in the joint socket (not shown) on the pelvic bone. The bone neck 13 adjoins the joint head 11 and has a cavity 14 in the interior. In the region 15 , which is merely dashed by way of example, the solid, so-called cortical bone substance of the bone neck 13 merges into a spon giös form, which is spongy or spongy, so is therefore soft. The exact transition, where there is still solid cortical bone material, must be determined as precisely as possible, since this is relevant for the secure hold of the implant. By means of the method mentioned at the beginning, such an exact determination based on the contour of the is possible.

Such a contour image K is exemplified in Fig. 3 Darge. In the exemplary embodiment shown, this contour image determined by means of the computer device 7 shows the contours of the solid bone material of the femoral neck 10 shown in FIG. 2, ie soft parts such as cartilage 12 or the area in which cancellous bone material is present are no longer shown here. The corresponding implant of an implant image I, which is also shown in FIG. 3, can now be selected on the basis of this contour image. This implant image I shows a view of a prosthetic implant 16 , which also has a joint head 17 and a wedge shaft 18 , which is inserted into the cavity 14 of the bone shaft 13 sawn off for implantation (the joint head 13 is known to be removed). Based on the contour image, the exact shape of the implant, in particular the wedge shaft 18, can be determined, and since its adaptation to the proportions of the interior of the bone shaft 13 makes it possible, so that a secure fixation is possible.
