Abstract
Description
The invention relates to a mounting device with a screw for Attachment of an implant for spinal stabilization with a threaded section for screwing into a vertebral body and a screw head in which a hollow ball is arranged, and an attachment for positioning and screwing a screw into the Vertebral body, which also serves as a wound spreader.
Screws of the type described in the introduction are already known become.
For example, in European Patent EP 0 641 548 B1 discloses a screw for a lumbosacral backbone support that has a partially spherical head which has a toothed, for Longitudinal axis of the screw at an angle between approximately 30 and 45 degree inclined surface and a toothing perpendicular to it Has surface arranged threaded hole. By the inclined In particular, the assembly of a side member on the surface Screws easier. However, such screws have the Disadvantage that this screw does not reach the bone surface may be screwed in, since another connecting element, which with the help one bolt is attached to the beveled surface, several Millimeters protrudes beyond the head diameter in the direction of insertion. Of further, the connecting element can only be around the axis of the Adjust the bolt.
The invention has for its object a mounting device with a screw of the type described in the introduction and an attachment To provide so that in particular a deep screwing in Screw into a vertebral body and an angular positioning of the implant attached to the screw is possible. This Task starts from a screw in the introductory designated type solved in that the screw head hollow ball has, the surface area of the screw head around can be inclined approximately between 40 ° and 80 ° degrees. The hollow sphere the screw can have teeth. Furthermore, can due to the head shape of the screw, an attachment for screwing in the Screw used as a spreader and manipulation aid could serve.
In comparison with other systems, the essay, after which Screwing the screw into the vertebral body, cannot be removed. Furthermore, the hollow ball in the screw head offers the possibility of a Connecting element, on the underside of which there is a counter ball Slits and toothed surface located to record. The Counterball with slots can be spread out by a bolt and thus secures the connecting element in the hollow sphere Screw. The connecting element is used to attach a Longitudinal rod that connects several screws and in Multi-directional inclination to the longitudinal axis of the screw attached can be. The expanding part of the connecting element in this case has a toothing distributed on the ball, so a rotation lock in the hollow ball of the screw is given. The connecting element can be screwed into the slotted part of the same are closed, thereby securing the longitudinal bar against rotation and displacement. By any adjustable inclination of the connecting element in the hollow sphere The longitudinal rod does not have to screw with a two-point connection and with a multi-segmental connection little or no bend become. This is a key benefit.
To prevent the attachment from twisting in a simple manner on the screw head To be able to attach, it is proposed that the screw head two essentially symmetrical parallel to the longitudinal axis of the screw may have arranged flattened sections. In this Context, it is also advantageous if in the area of flattened sections a preferably arcuate groove is provided, into which pins of an essay can be inserted. This defines an attachment in the screw axis.
For fastening a longitudinal rod with which several Let screws connect, it is also proposed that in Screw head a connecting element on the underside itself there is a counter ball with slots and teeth. The Counterball with slots can be spread out by a bolt and thus secures the joint screw - connecting element. The threaded bolt therefore has two tasks: First, it is fixed between connecting element and screw, on the other hand that Clamps between connecting element and rod.

Fig. 1a and b shows a screw 1 having a threaded portion 2 and a head 3. A hollow ball 4 is provided in the head 3 and serves to receive a counter ball 5 of the connecting element 8 . As an example, the surface of the screw head 3 is cut in FIG. 1a and inclined by 70 ° with respect to the longitudinal axis 6 of the screw. In order to be able to connect several screws 1 for the stabilization of the spine to an implant in the form of a rod, a counter ball 5 for fastening a connecting element 8 is arranged in the hollow ball 4 of the screw head. In the connecting element 8 , a rod (not shown), which establishes the connection to at least one further screw, can be clamped in its circular opening 9 . So that the connecting element 8 can be secured against rotation in / on the head 3 , a toothing 9 a is provided in the edge region of the opening of the hollow ball 4 . Preferably, the counter ball 5 of the connecting element 8 located in the head 3 has matching teeth 5 a. The threaded bolt 7 has a head 7 a with z. B. a hexagon socket, which can be easily screwed into the thread 8 a of the counter ball 5 with a key 7 b indicated in FIG. 4. As a result, the counter-ball 5 is spread open and the connecting element 8 is locked on the head 3 so that it cannot rotate. By screwing the threaded bolt 7 into the connecting element 8 , the leg 8 b is pressed together and the rod inserted in the opening 9 is secured against rotation. Fig. 4 shows the screw 1 with connecting member 8 and screwed stud 7 in a vertebral body 11 is arranged. The screw 1 is screwed in up to the head 3 , so that it lies even against the bone in the lower region. The attachment 20 can be fixed to the head 3 without this having to reach under the screw head. In FIGS. 5a and b, a screw 1 is shown with a bonnet 20. At the front end of the attachment 20 , a shell element 21 is attached, which partially surrounds the head 3 of the screw 1 . The shell element 21 lies against the flattened sections 12 of the screw head 3 , whereby a first anti-rotation device is created. Pins 22 , 23 , which run in corresponding grooves 15 in the head 3 of the screw 1 and in the shell element 21 , prevent movement of the attachment in a direction parallel to the flattened sections 12 . In order to prevent movement of the attachment 20 in the longitudinal direction of the screw, an incorporated pin 24 is provided which engages in a correspondingly arranged groove or recess 14 on the head 3 . As soon as the pins 22 and 23 are pulled out, however, the attachment can be pulled off the head 3 upwards.
