Die Erfindung betrifft eine Feststoffkomponente für einen Knochenzement nach dem Oberbegriff des
Patentanspruchs 1. jo
Für die Verankerung von Implantaten oder Gelenkendoprothesen im Knochen sind bisher zwei verschiedene
Wege beschriften worden. Auf der einen Seite werden Prothesen mit Hilfe von sogenannten Knochenzementen
— das sind im wesentlichen aus Kunststoff bestehende, eine im Knochen aushärtende Komponente
enthaltende Mischungen (DE-OS 20 22 117) - in den Knochen einzementiert, während auf der anderen Seite
die im Knochen zu verankernden Teile der Prothese mit einer porösen Oberfläche versehen werden, in deren
Poren Knochengewebe im Laufe der Zeit hineinwächst. Beide Arten von Prothesenverankerung haben gewisse
Nachteile.
So tritt bei der Aushärtung der Knochenzemente eine beträchtliche Temperaturerhöhung auf, die die unmittelbar
an den Zement anliegenden Gewebeschichten schädigen und zu Nekrosen führen. Das Einwachsen von
Knochengewebe in eine poröse Prothesenoberfläche erfordert andererseits eine zumindest wochenlange
Fixierung und Ruhigstellung der Prothese relativ zum sie umgebenden Gewebe. Eine derartige Ruhigstellung
ist in der Praxis nur sehr schwer zu verwirklichen und kann wegen der erzwungenen Ruhe schwerwiegende
Folgen für das Allgemeinbefinden des davon betroffenen Patienten haben.
Der Erfindung liegt die Aufgabe zugrunde, eine Feststoffkomponente nach dem Oberbegriff des Patentanspruchs
zu schaffen, die sowohl zu einer Erniedrigung der beim Aushärten des Knochenzements auftretenden
Temperatur als auch zur Ausbildung einer porösen Struktur des Knochenzements führt.
Diese Aufgabe wird entsprechend dem kennzeichnenden Teil des Patentanspruchs gelöst.
Unter »hochviskos« im Zusammenhang mit dem zu bildenden Gel werden Viskositäten verstanden, die h>
302 00OmPa · s (= cPs) und mehr betragen. Derartig hohe Viskositäten sind erforderlich, um eine Entmischung
der wasserlöslichen und wasserunlöslichen Dispers^onskomponente zu verhindern.
Die Anwesenheit eines wässrigen Gels während der Reaktion der Komponenten eines Knochenzements
bisher üblicher Zusammensetzung bewirkt eine beträchtliche und für die Verhütung der geschilderten
Nekrosen entscheidende Erniedrigung der beim Aushärten auftretenden Maximaltemperatur. Weiterhin löst
sich von dem gebildeten Knochenzement nach der Operation die wässrige Gelkomponente in den körpereigenen
Flüssigkeiten auf. Dadurch entsteht in dem Knochenzementbett, das das Implantat umgibt, ein Netz
von offenen, miteinander kommunizierenden Puren, in die — da die nekrotische Schädigung des umgebenden
Gewebes wegen der niedrigen, höchstens erreichten Reaktionstemperatur bei der Polymerisationsreaktion
fehlt — verhältnismäßig sehr rasch Knochengewebe einsproßt. Damit wird aber die durch einwachsendes
Gewebe besonders intensive Verankerung des Zementbettes im Knochen erreicht, ohne daß eine längere
Ruhigstellung des Implantats erforderlich ist; durch das Einzementieren des Implantats wird jedoch auch von
Anfang an eine feste, primäre Verankerung erzielt.
Der Grad der erhaltenen Porosität nach dem Lösen des Gels läßt sich durch Änderung des relativen
Gelbildneranteils in der Feststoffkomponente variieren. Es hat sich dabei als vorteilhaft erwiesen, wenn der
Gelbildner einen derartigen Mengenanteil hat, daß in der aus der Feststoff- und Flüssigkeitskomponente
gebildeten Dispersion ein Netz zusammenhängender Gel-Fäden vorhanden ist; dies läßt sich mit Reaktionsmischungen erreichen, die 3 bis 50Gew.-% Gel
enthalten.
Die mechanischen Festigkeiten eines mit einer derartigen Feststoffkomponente hergestellten ausgehärteten,
gewollt porösen Knochenzementbettes sind gegenüber denjenigen eines massiven Zementkörpers
bekannter Art, der infolge von Gas- und Flüssigkeitseinschlüssen aus eine gewisse, unvermeidbare Porosität
hat, erniedrigt; daher ist es besonders vorteilhaft, eine Feststoffkomponente zu wählen, bei der ohne den Gel
bildenden Stoff bei Aushärtung des Knochenzements diese unvermeidbare, eigene Porosität möglichst gering
ist.
Da weitere Stoffe, beispielsweise die Knochenbildung fördernde Substanzen, wie Calciumphosphat, und/oder
Pufferlösungen zur Einhaltung eines bestimmten ph-Wertes des Gels und/oder Antibiotika sowie andere
Pharmazeutika, beispielsweise zur Verbesserung der Antisepsis nach der Operation, wegen der zusammenhängenden
Gel-Fäden in der Reaktionsmischung praktisch aus dem gesamten Volumen des Zementbettes
herausgelöst und wirksam werden können, sind für eine vergleichbare Wirkung wesentlich geringere Mengen
erforderlich als bisher, wo derartige Stoffe beispielsweise Bestandteil der Feststoffkomponenten sind und nur
ein an der Oberfläche liegender Anteil wirksam werden kann.
Im folgenden wird die Erfindung anhand eines Ausführungsbeispiels näher erläutert.
Hierbei dient als pulverförmiger wasserunlöslicher Anteil der Feststoffkomponente eine Mischung, die
90Gew.-Teile Polymethylmethacrylat (PMMA) als
Füllstoff, 10 Gew.-Teile Zirkoniumoxid als Röntgenkontrastmittel
und 2 Gew.-Teile Benzoylperoxid als Polymerisationskatalysator enthält, sowie als Flüssigkeitskomponente eine solche, die aus einem aushärtbarem
Monomer, 85 Gew.-Teilen Methylmethacrylal (MMA) und 15 Gew.-Teilen n-Butyimethacrylat, sowie einem
Polymerisationsbeschleuniger, 2 Gew.-Teilen N-Dimethyl-p-toluidin,
besteht
Als Gelbildner werden 6 Teile Carboxymethylcellulose (CMC) (Substitutionsgrad bei der Carboxylierung der
Alkoholreste 1,18; pH-Wert einer 0,5%igen, wässrigen
Lösung 6,8; Viskosität, gemessen mit dem Kapillar-Viskosimeter, einer l%igen wäßrigen Lösung bei Umgebungstemperatur
39OmPa - s (= cPs) verwendet die mit 20 Teilen des bekannten Pulveranteils der Feststoffkomponente
gemischt werden.
Zur Untersuchung der Eigenschaften, wie z. B. Porositätsgrad, Druckfestigkeit Ausbildung von untereinander
verbundenen Gel-Fäden in der Mischung — was beispielsweise durch Messungen der elektrischen
Leitfähigkeit ermittelt worden ist — und ähnliches, werden 15 Teile der Mischung aus dem bekannten
Pulveranteil der Feststoffkomponenten und dem CMC-Pulver und 15 Teile einer Flüssigkeitskomponente
zugemischt die flüssiges, monomeres Methylmethacrylat enthält wobei bis zu 5 Gew.-% der Fiüssigkomponente
ein tertiäres Amin und 10 bis 45Gew.-% mindestens ein einwertiger, monofunktioneiler Methacrylsäureester
aus der Gruppe mit gesättigten, aliphatischen Alkoholkomponenten mit 2 bis 6 C-Atomen im
Alkoholrest sind. Als letztes werden 14 Teile des zur Gelbildung erforderlichen Wassers zugegeben und
eingemischt.
In dieser Mischung erfolgen nun gleichzeitig die Aushärtung des ICnochenzements und die Gelbildung
aus CMC und Wasser. Die beschriebene Mischung, die einen Knochenzement mit einem Gel-Anteil von 40%
darstellt, wobei das Gel 30% CMC enthält dient zur Durchführung der angedeuteten Untersuchungen, wobei
vor den Versuchen mit oder an dem porösen Zementskelett das Gel mit Wasser ausgewaschen
worden ist
Die bei den Versuchen erhaltenen Aushärtetemperaturen sind, wie bereits erwähnt stark erniedrigt
während die Aushärtezeiten, die bekanntlich definiert sind als die Zeitspanne vom Zusammenmischen der
flüssigen und der pulverförmigen Komponente des Knochenzements bis zum Erreichen der Temperaturmaximums,
gegenüber denjenigen des Knochenzements ohne Gelanteil praktisch unverändert bleiben. Der
Porositätsgrad der Mischung ändert sich selbstverständlich mit dem Gel-Anteil, wobei mit steigendem
Gel-Anteil die Porosität die bei den Untersuchungen um einen etwa konstanten Anteil von 7 bis 10% höher
liegt als der prozentuale Gel-Anteil in der Mischung, zu- bzw. die mechanische Festigkeit abnimmt. Der Porendurchmesser
liegt in einem Bereich von etwa 50 bis 150 μηι; der für biologische Zwecke gut geeignet ist
Durch Änderung der Korngröße und/oder der Form der einzelnen Partikel der pulverförmigen CMC ist es u. U.
möglich, die Porendurchmesser im Knochenzementgerüst zu vaiiieren.
Claims (1)
- Patentanspruch:Feststoffkomponente für einen Knochenzement, die mindestens ein pulverförmiges Polymethylmethacrylat als Füllstoft, mindestens einen pulverförmigen Polymerisationskatalysator und gegebenenfalls ein Röntgenkontrastmittel enthält und zur Umsetzung mit einer Flüssigkomponente, die mindestens ein monomeres Methylmethacryiat und wenigstens einen Polymerisationsbeschleuniger enthält, be- u> stimmt ist, dadurch gekennzeichnet, daß die Feststoffkomponente weiterhin einen zur Bildung eines biologisch verträglichen, in Wasser bzw. Körperflüssigkeiten löslichen, hochviskosen Gels befähigten Stoff aus der Gruppe Polyvinylalkohol, Polyvinylmethyläther, Polyvinylpyrrolidon, polyacrylsaurem Salz und wasserlöslichem Celluloseäther, insbesondere Carboxymethylcellulose, enthält, wobei der Anteil des zur Gelbildung befähigten Stoffes in der Feststoffkomponente so bemessen ist, daß die Reaktionsmischung 3 bis 5 Gew.-% an Gel mit einem Gehalt von 3 bis 6 Gew.-% an GelDildner aufweist.
