Description
Die vorliegende Erfindung betrifft medizinische Prothesen zur Inplantation in einen Körper, genauer
gesagt Prothesen, die mit Hilfe des Einwachsens des Knochens in die Prothese im Körper fixiert werden.

Medizinische Vorrichtungen, wie Knochenplatten, Nägel,
Stifte, Schrauben und Gelenkprothesen, werden seit vielen Jahren in die Skelette von Menschen und Tieren
implantiert, um die Teile eines gebrochenen Knochens miteinander zu verbinden oder um fehlende oder beschädigte
Skeletteile zu ersetzen. Es ist dabei oft beabsichtigt, daß diese Teile zu einem permanenten
Teil des Körpers werden. In solchen Fällen ist es wichtig, daß die Teile fest und permanent am Knochen-
Es ist seit mehr als 10 Jahren bekannt, daß eine feste permanente Fixierung durch die Verwendung einer porösen Fläche am gesamten Teil oder an einem Abschnitt
des Teiles erreicht werden kann, wenn die mittlere Porengröße etwa 50 pm überschreitet. Dabei stellen
die älteren Verfahren der Knochenfixierung, in erster
Linie mit Hilfe von Reibungspassung oder mit Methy1-methacrylat,
noch die am häufigsten verwendeten Verfahren zur Fixierung dar, und zwar trotz der Tatsache,
daß das Lösen von mittels Reibung und Zement fixierten Prothesenteilen im Laufe der Zeit ein signifikantes
medizinisches Problem darstellt. Dies ist darauf zurückzuführen, daß zur Erzielung einer guten Fixierung
mit Hilfe des Einwachsens des Knochens der Patient
keine Kraft oder Last auf das Skelett im Implantat aufbringen darf, bis das Knochenwachstum aufgetreten
ist, während bei einer Verbindung über Reibung oder Zement nahezu sofort eine Belastung stattfinden kann.

In der nachfolgenden Beschreibung bedeuten die Worte
bioabsorbierbar und resorbierbar, daß die Substanz, auf die sich diese Worte beziehen, abgebaut, absorbiert oder in anderer Weise von der Chemie des
Wirtkörpers in Mengen entfernt wird, die ausreichen, um den physikalischen Aufbau des Abschnittes der Vorrichtung,
die bioabsorbierbar ist, innerhalb einer Zeitspanne zu verändern, die geringer ist als die
Zeitspanne, die zur vollständigen Genesung von Knochen erforderlich ist. Es versteht sich, daß sämtliche
Materialien, sogar Stahl, von der Chemie des Körpers geringfügig absorbiert werden; derartige
geringfügige Absorptionen, die den physikalischen Aufbau der bioabsorbierbaren Vorrichtung über die Zeitspanne
der Knochenheilung nicht verändern, sind jedoch nicht in den Begriffen bioabsorbierbar und
resorbierbar eingeschlossen. Es versteht sich ferner, daß, obwohl ein Material bioabsorbierbar ist,
eine geringfügige Menge desselben im Körper über längere Zeitspannen zurückbleiben kann.
Ein Verfahren zum Fördern eines früheren Knocheneinwachsens
in poröse Flächen von Implantaten besteht im Beschichten von porösen Metallfasern mit
Hydroxyapatit durch Eintauchen der Faser in einen Wasserschlamm von Hydroxyapatit und Trocknen.
Hydroxyapatit wird nicht im allgemeinen als
resorbierbar im menschlichen Körper angesehen. Es wurde festgestellt, daß Hydroxyapatit ein schnelleres Einwachsen
des Knochens in die porösen Metallflächen über eine Zeitspanne von bis zu vier Wochen nach der Implantation
fördert, daß der erreichte Effekt jedoch nur kurzzeitig war, da die Menge des Knochengewebes innerhalb
der Poren nach dieser Periode von vier Wochen abfiel. (P. Ducheyne et al "Effect of Hydroxyapatite
Impregnation on Skeletal Bonding of Porous Coated
Vol. 14, 225-237 (1980) ). Zusätzlich zu der Tatsache,
daß das gesteigerte Einwachsen nicht permanent war, wurde festgestellt, daß Hydroxyapatit mehr das Faserwachstum
als das Knochengewebewachstum an der
zu einem Lösen der Prothese führen kann. Darüberhinaus wird der Hydroxyapatitüberzug in dem vorstehend genannten
Artikel als relativ zerbrechlich bezeichnet und kann bei einer normalen Handhabung für kommerziell
erhältliche Produkte leicht brechen, so daß dies zu keiner weitgestreuten kommerziellen Verwendung geführt
hat.
Die nachfolgenden US-Patente betreffen Aspekte der «liegenden Erfindung. In der US-PS 3 605 123 ist
das Plasmaspritzverfahren einer metallischen porösen Fläche auf eine Prothese beschrieben. In den
US-PS · en 3 892 648 und 3 892 649 wird die galvanische Abscheidung von Knochengewebe und Collagen auf Implantate
oder in ein Kunststoffsieb auf Implantaten, um hierdurch die Befestigung des Knochengewebes am Implantat anzuregen,
erläutert.
Die US-PS 3 919 723 betrifft das Einbetten von Calzium-
und Phosphatatomen in die Oberfläche einee keramischen Iaplantates durch Erhitzen des Implantates und Einbetten
desselben in eine Schmelze von Calziumphosphat. Es ist dabei speziell darauf hingewiesen, daß Temperaturen
über 1.300° C nicht verwendet werden sollten, da sich das Calziumphosphat bei höheren Temperaturen
zersetzt. Diese Zersetzung bei höheren Temperaturen ist charakteristisch für Materialien auf der Basis
von Calziumphosphat (s.hierzu E.Hayek und H. Newesely,
Inorganic Synthesis 7 (1963) 63).
In der US-PS 4 202 055 ist die Kombination eines bioabsorbieren, bioaktiven Calziumphosphatmaterials
mit einem Polymerisat auf der Oberfläche einer Prothese, um hierdurch ein Einwachsen des Knochens in das
Polymerisat zu bewirken, beschrieben. Die US-PS1 en 4 365 357 und 4 373 217 erläutern die Kombination
eines absorbierbaren Tricalziumphosphatmaterials mit Knochenzement, um ein Knochenwachstum in den Zement
zu erzielen. Jedes dieser Patente bezieht sich auf die Einarbeitung des absorbierbaren Materials
in das Material, aus dem die Fläche besteht, und schlägt vor,daß die poröse Fläche durch Absorption
des absorbierbaren Materials erzeugt wird, wodurch Poren in der Fläche an der Stelle des Leerraumes
verbleiben, der durch die Absorption des absorbierbaren Materiales erzeugt wird. Keines dieser Patente
schlägt die Beschichtung einer bereits porenhaltigen Fläche mit einem absorbierbaren Material, um dadurch
das Knochenwachstum zu fördern, vor.
In der US-PS 4 338 926 wird die Hinzufügung einer 0,1 bis 1 mm dicken bioabsorbierbaren Schicht auf
eine Implantatfläche beschrieben. Hierdurch will man ein Lösen des Implantates erzeugen, wenn das Material
absorbiert wird. Die Prothese, auf der die absorbierbare Schicht angeordnet wird, ist porenfrei, da man
ein Knocheneinwachsen verhindern will.
Der Erfindung liegt die Aufgabe zugrunde, eine Prothese zu schaffen, die zu einem schnellen und permanenten
Knocheneinwachsen in eine poröse Fläche führt, so daß auf diese Weise eine frühe, feste und permanente
Die Erfindung bezweckt ferner die Schaffung einer Prothese und eines Verfahrens zur Herstellung der
Prothese, durch die bzw. durch das die Nachteile der Prothesen des Standes der Technik, mit denen eine permanente
Fixierung am Knochen erzielt werden soll, beseitigt werden.
Die vorstehend genannte Aufgabe wird durch ein zur Befestigung am Knochenskelett dienendes Teil gelöst, das
ein Basiselement mit einem porösen Bereich auf dessen Oberfläche und einen bioabsorbierbaren Oberzug auf
mindestens einem Abschnitt des porösen Bereiches umfaßt, dessen Material das permanente Knocheneinwachsen
in den porösen Bereich fördert.
Es wurde festgestellt, daß sich das Basiselement nicht löst, wenn der Oberzug der porösen Fläche absorbiert
wird, wie dies im Stand der Technik vorgeschlagen wird. Vielmehr bewirkt das absorbierbare Material in einer
nicht ganz verständlichen Weise ein schnelleres und stärkeres Knocheneinwachsen in die poröse Fläche, die
es abdeckt. Darüberhinaus wird das Einwachsen permanent
angeregt und stellt ein Knochenmaterial anstelle eines faserigen, schwachen Gewebes dar, das bei Hydroxyapatit-Oberzügen
auftritt.

Erfindungsgemäß wird ferner ein Verfahren zur Herstellung
eines Prothesenteiles zur Verwendung als Körperimplantat vorgeschlagen, daß die nachfolgenden Schritte
umfaßt; Herstellen eines Basiselementes, das mit einem porösen Oberflächenbereich versehen ist, auf dem eine
Knochenbefestigung gewünscht wird; Herstellen eines Materiales, das nach dem Erhitzen auf eine hohe Temperatur
in einen Zustand abkühlt, in dem es bioabsorbierbar ist; Erhitzen des Materiales auf eine Temperatur
über 1.350° C und Aufbringen desselben auf mindestens
einen Abschnitt des porösen Oberflächenbereiches des Basiselementes. Vorzugsweise umfaßt das Material mindestens
eine Substanz, die aus der aus Hydroxyapatit und /3-Tricalciumphosphat bestehenden Gruppe ausgewählt
ist, wobei das Material nach dem Erhitzen und Abkühlen in primäres <rt -Tricalciumphosphat umgewandelt
wird. Es wurde festgestellt, daß die Hochtemperaturbehandlung des Oberzuges zu einem Oberzug führt, der
sowohl bioabsorbierbar ist als auch eine größere Adhäsionskraft besitzt als die Oberzüge des Standes
der Technik. Es wurde ferner festgestellt, daß der Oberzug in ausreichender Weise anhaftet, so daß die
Teile verpackt, sterilisiert und allgemein für Implantationszwecke gehandhabt werden können, ohne sich
vom Basiselement zu lösen.
Erfindungsgemäß wird somit ein lange bestehendes
Problem des Standes der Technik gelöst. Die Erfindung
wird nachfolgend anhand von Ausführungsbeispielen in Verbindung mit der Zeichnung im einzelnen erläutert.

Figur 5 die zeichnerische Wiedergabe einer Fotografie eines Abschnittes wie in
Figur 4, jedoch 948-fach vergrößert, wobei nur ein Abschnitt von einem der Metalldrähte dargestellt ist, der die
poröse Fläche und den Oberzug des Drahtes bildet.
In Figur 1 ist ein prothetisches Teil dargestellt, das als Körperimplantat verwendet werden soll. Bei dem
speziellen gezeigten Teil handelt es sich um eine Hüftprothese. Derartige Prothesen umfassen üblicherweise
einen glatten, kugelförmigen Kopf 10, der eine Oberfläche, eines Hüftgelenks bildet und der sich in einer
Hüftpfanne drehen kann, einen Hals 14 und ein Basiselement oder Schaft 15. Der Schaft 15 dient zur Verankerung
im Femur des Menschen oder Tieres. Um die Fixierung des Schaftes 15 im Femur zu verbessern, ist
ein Bereich 16 der Oberfläche des Schaftes 15 porös.
Bei der Ausführungsform der Figur 1 umfaßt der poröse Bereich 16 des Schaftes eine Matte, die sich aus
komprimierten kurzen Metallfasern zusammensetzt, wie beispielsweise in der US-PS 3 906 550 beschrieben.
Erfindungsgemäß ist dieses Metallfasersieb 17 mit ei-.
nem bioabsorbierbaren Material beschichtet, wie nachfolgend
im einzelnen beschrieben wird.
Figur 2 zeigt eine zweite beispielhafte Ausführungsform der Erfindung. Das zu implantierende Teil 20
ist eine Acetabular-Hüftprothese, die üblicherweise
als Hüftbecher bezeichnet wird. Dieser Hüftbecher 20 besitzt eine glatte, sphärische Oberfläche 21, die
die andere Oberfläche eines Hüftgelenks bildet und die sphärische Fläche 10 der Prothese der Figur 1
aufnehmen soll. Die Außenfläche 23 des Hüftbechers 20 soll in das Acetabulum implantiert werden und ist
durch Plasmaspritzen mit einer porösen metallischen Oberflächenschicht 24 bedeckt. Das entsprechende
Plasmaspritzverfahren ist in der US-PS 3 605 123 beschrieben. Erfindungsgemäß umfaßt die poröse Schicht
24 ein bioabsorbierbares Material auf mindestens einem
Abschnitt seiner Fläche, das das permanente Knocheneinwachsen
in den porösen Bereich fördert. Der Oberzug wird nachfolgend im Detail beschrieben.
Ein anderer häufig eingesetzter poröser Oberzug, bei
dem die Erfindung Anwendung finden kann, ist ein gesinterter
Oberzug aus Metallpartikeln.
Die vorstehend beschriebenen Typen von porösen Flächen dienen nur als Beispiele. Es versteht sich, daß irgendeiner
aus einer Vielzahl von Typen von porösen Flächen, die für die Fixierung von in den Körper implantierten
Teilen verwendet werden, bei der Erfindung Anwendung
finden kann.

In Figur 3 ist eine Gitterstruktur mit poröser Oberfläche, wie beispielsweise die Matte 17, etwa 20-fach
vergrößert dargestellt. Die Gitterstruktur 17 besteht aus kurzen Metallfasern 18, die komprimiert sind, und
ist mit einem bioabsorbierbaren Material 19 beschichtet, bei dem es sich bei der dargestellten Ausführungsform
primär um °<-Tricalciumphosphat handelt.
Ein Abschnitt der Gitterstruktur 17, der durchgeschnitten und mittels eines Rasterelektronenmikroskops
untersucht worden ist, ist in Figur 4 dargestellt. Die Oberfläche der Gitterstruktur (die aus dem Mikrobild
der Figur 3 herausweisende Richtung) zeigt zur unteren rechten Ecke der Figur 4, Das Mikrobild des
entsprechenden Abschnittes ist 63,5-fach vergrößert.
Typische Schnitte durch die Drähte sind bei 31 und dargestellt. Die Verlängerungen der Drahtabschnitte,
wie bei 32 gezeigt, sind auf die Tatsache zurückzuführen, daß die Schnittebene mit dem Drahtdurchmesser
einen schiefen Winkel bildet. Partikel, die lose erscheinen, wie bei 33 gezeigt, sind tatsächlich nicht
lose, sondern stellen nur einen Partikel dar, der außerhalb der Schnittebene verbunden ist. Die Flächen
34, 35, 37 der Drähte sind mit dem <*-Tricalciumphosphat
33, 36 und 38 beschichtet. Bei der dargestellten Ausführungsform beträgt die maximale Oberzugsdicke
etwa 30 um, mit einer durchschnittlichen Dicke von 20 um an den Außenflächen, wie bei 37, der
Außendrähte der Gitterstruktur. Der Oberzug dringt etwa 0,020" (500 pm) in die Gitterstruktur ein, wobei die
Kontinuität des Oberzuges von der Auienseite zur Innenseite hin abnimmt. Figur 5 zeigt eine andere Ansicht
der Ausführungsform der Figur 4, die 948-fach vergrößert ist.

Die in den Figuren 1, 3, 4 und 5 dargestellten beschichteten
Fasermetallteile werden in der folgenden Weise hergestellt. Im Handel erhältlicher reiner
Draht wird zu Kissen geformt auf der Stelle der Prothese gesintert, wie in der US-PS 3 906 550 beschrieben. Die
Gitterstruktur wird dann mit einem Material plasmabespritzt, das nach dem Spritzen und Abkühlen bioabsorbierbar
ist. In entsprechender Weise können die in Figur 2 dargestellten beschichteten Teile 20 hergestellt
werden, indem zuerst die Metallfläche auf das Basiselement (unter der Fläche 24) plasmagespritzt wird,
und dann auf die entstehende poröse Fläche ein Oberzug
einer Substanz plasmagespritzt wird, die zu einem bioabsorbierbaren Material abkühlt.
Nach dem vorstehend beschriebenen Verfahren wurden Metallzylinder hergestellt. Bei dem verwendeten Draht
handelte es sich um ein im Handel erhältliches reines Titan mit einem Durchmesser von etwa 0,254 mm, das in
Längen von etwa 25,4 mm zerschnitten wurde. Das Gitter besaß eine Dicke von etwa 2 mm und wurde auf einen
mittleren Titaastab gesintert, der einen Gewindeabschnitt besaß, um eine Befestigung an einer
mechanischen Testvorrichtung zu ermöglichen. Die Zylinder besaßen einen Durchmesser von 9,5 mm und eine
Länge von 50 mm. Die Fasergitterzylinder wurden dann mit einem Material, das etwa 50 t Hydroxyapatit und
50 % β-Tricalciumphosphat enthielt, plasmabespritzt. Dabei fand das übliche Plasmaspritzverfahren Anwendung,
wie es in der US-PS 3 605 123 beschrieben ist. Die nachfolgende Analyse des plasmabespritzten Materials
durch Röntgenstrahlbeugung ergab, daß sich das
50/50 Hydroxyapatit - P -Tricalciuraphosphat-Material
primär in Λ -Tricalciumphosphat umgewandelt hatte. Es verblieb etwas /^-xricaiciumphosphat und Hydroxyapatit.
Die Beugungsanalysendaten zeigten ferner einige Linien, die noch nicht identifiziert wurden,
von denen jedoch angenommen wird, daß es sich um Hochtemperatur-Calciumphosphat handelt. Die Beugungslinienbreite
zeigte an, daß die Kristallit-Größe geringer war als die bei einer herkömmlichen Keramik.
Die Zylinder wurden zusammen mit ähnlichen Zylindern, die mit einer Reihe von anderen Materialien behandelt

worden waren, nämlich in der Literatur als osteogenisch
beschriebenen Materialien, wie beispielsweise β -Tricalciuraphosphat,
entmineralisiertem Knochenpulver und autogenem Knochen und Mark,in die Oberschenkelknochen
von Hunden implantiert. Nach einer Zeitdauer von zwei Wochen bis sechs Wochen wurden die Tiere getötet,
und die Proben wurden aus den Oberschenkelknochen herausgezogen. Die erfindungsgemäß hergestellten Proben
besaßen die höchste Festigkeit gegenüber Herausziehen.
Dies zeigt offensichtlich die stärkste Fixierung an und wird allgemein als verbessertes Knocheneinwachsen
interpretiert. Die erfindungsgemäß ausgebildeten Proben konnten hinsichtlich Verpackung und Implantation in
den Tieren ohne jegliche Qualitätsverschlechterung normal gehandhabt werden.
Es wird angenommen, daß die Umwandlung des Hydroapatit/
ß-Tricalciumphosphat-Materials in oC -Tricalciumphosphat
auf die Tatsache zurückzuführen ist, daß das Material durch das Plasmaspritzverfahren Umwandlungsenergie erhält. Aufgrund dieser Tatsache können auch
andere Verfahren zur Aufbringung des Materials, die Energie für die Umwandlung zur Verfügung stellen,
eingesetzt werden, um den Oberzug aufzubringen, wie
beispielsweise Bedampfungsverfahren, Elektrophorese,
elektrostatische Sprühverfahren etc.
Vorstehend wurden neuartige Teile zur Implantation in den menschlichen Körper und das Verfahren zur Herstellung
dieser Teile beschrieben, wodurch sich eine verbesserte Fixierung und ein verbessertes Knocheneinwachsen
erreichen läßt und die zahlreiche andere
Merkmale und Vorteile aufweisen. Obwohl sich die vorstehende Beschreibung der Erfindung auf einige spezielle
Ausführungsformen bezog, versteht es sich, daß nunmehr aufgrund der offenbarten Vorteile der Beschichtung einer
porösen Fläche einer Prothese mit einem bioabsorbierbaren Material zahlreiche Modifikationen und Abweichungen
der speziellen beschriebenen Ausführungsformen durchgeführt werden können, ohne vom erfinderischen
Konzept abiuweichen. Beispielsweise kann die beschichtete poröse Fläche in Kombination
mit vielen Arten von anderen implantierbaren Fixatoren als beschrieben verwendet werden, beispielsweise mit
Knieprothesen, Knochenplatten, Intramedularstäben etc. Feter können auch andere bioabsorbierbare Materialien,
wie Calciumpyrophosphate oder Polymilchsäuren, verwendet
werden. Es können auch andere Dicken als vorstehend beschrieben Anwendung finden. Das Basiselement
der Prothese und/oder die poröse Schicht können auch aus Metallen bestehen, beispielsweise Kobalt-Chrom-Stahllegierungen,
Edelstahl etc. und anderen Materialien, die für die Implantation im Körper geeignet sind.

Claims (12)
- Patentansprüche, Ij. Verfahren zur Herstellung eines Prothesenteiles zur Verwendung als Körperimplantat, gekennzeichnet durch die nachfolgenden Schritte:Herstellen eines Basiselementes, das einen Bereich mit einer porösen Oberfläche aufweist, auf dem eine Knochenbefestigung gewünscht wird;Herstellen eines Oberzugsmateriales; undAufbringen des Materiales auf mindestens einen Abschnitt des Bereiches der porösen Oberfläche, wobei ausreichend Energie zur Verfügung gestellt wird, um das Material in einen Zustand zu Überführen, in demes bioabsorbierbar ist. .-,
- 2. Verfahren nach Anspruch 1, dadurch gekennzeichnet, daß ein Material zur Verfügung gestellt wird, das sich nach dem Erhitzen auf eine hohe Temperatur auf einen Zustand abkühlt, in dem es bioabsorbierbar ist, und daß das Material auf eine Temperatur erhitzt wird, die größer ist als 1.350° C, und auf mindestens einen Abschnitt des Bereiches der porösen Oberfläche aufgebracht wird.
- 3. Verfahren nach Anspruch 2,dadurch gekennzeichnet, daß das Material durch Plasmaspritzen auf die Oberfläche des Basiselementes aufgebracht wird.
- 4. Verfahren nach Anspruch 1,dadurch gekennzeichnet, daß das Material auf mindestens einen Abschnitt des Bereiches der porösen Oberfläche aufgedampft wird.
- 5. Verfahren nach Anspruch 1, dadurch g e kennzeichn et, daß das Material auf mindestens einen Abschnitt des Bereiches der porösen Oberfläche elektrostatisch aufgesprüht wird.
- 6. Verfahren nach Anspruch I1 d a durch gekennzeichnet, daß das Material Calcium und Phosphor enthält.
- 7. Verfahren nach Anspruch 6,da durc h gekennzeich net, daß das Material Hydroxyapatit und /Ϊ-Tricalciumphosphat enthält und sich nach Aufnahme der zur Verfügung gestellten Energie primär in *-Tricalciumphosphat umwandelt.
- 8. Verfahren nach Anspruch 1,dadurch gekennzeichnet, daß das Material Pyrophosphate enthält.
- 9. Das Anhaften von Knochengewebe an sich förderndes Teil,gekenn ζ eic hnet d urch ein Basiselement, das auf seiner Oberfläche einen porösen Bereich besitzt, der auf mindestens einem Abschnitt mit einem bioabsorbierbaren Material beschichtet ist, das das permanente Knochenwachstum in den porösen Bereich hinein fördert.
- 10. Teil nach Anspruch 5,dadurch gekennzeichnet, daß das bioabsorbierbare Material*t -Tricalciumphosphat umfaßt.
- 11. Knochenimplantat, d adurch gekennzeichnet, daß es eine poröse Metallische Oberflächenschicht und ein mit der Oberflächenschicht verschmolzenes bioabsorbierbares Material umfaßt.
- 12. Knochenimplantat nach Anspruch 11, dadurchgekennzeichnet, daß das bioabsorbier· bare Material o<-Tricalciumphosphat umfaßt.
