Description

The invention relates to a combined, implantable Defibrillator Pacemaker Heart Treatment System, at which is a switch in the pacemaker lead for protection electronics against high-energy defibrillation Is used.

A coupled implantable device device brillators / pacemakers exhibits a bipolar, at heart arranged pacemaker electrode tip and two surfaces electrodes on or near the heart to the heart fibrillation are arranged. The two surface electrodes can be used as a defibrillation electrode and as a conventional Electrode be formed. With such an arrangement the pacemaker's output lead path runs from the bipolar pacemaker needle electrode for conventional Chen surface electrode. The defibrillation lead wire ver runs from the defibrillation surface electrode to the conventional one electrode. One that occurs with this configuration The problem is that the pacemaker needle electrode acts as a probe during high voltage defibrillation and the high voltage that occurs during defibrillation transmits to the input of the implantable arrangement. The High voltage could destroy under normal circumstances lead of the implantable arrangement. One way to To prevent this is to put a diode between the Pacemaker needle electrode and the conventional back arrange the path, the anode of which with the pacemaker Na delelektrode and its cathode with the conventional elec trode is connected. This diode suppresses this on the  Needle electrode received high potential and protects the on order during defibrillation before high voltage. However, this diode poses another problem.

By placing the diode between the needle electrode and the conventional return path is placed, a Stromlei path from the high voltage defibrillation pads electrode, through the heart, through the pacemaker needle electrode and through the diode to conventional return path generated. This current can be up to 4 amps, which is sufficient to cut myocardial tissue to burn pacemaker needle electrode and to fer ner to increase the pacemaker thresholds.

One solution to this is to close this circuit path open by placing a large resistor in series with the needle electrode lead is placed to the current thereby limit. However, since the implantable unit is also available as Pacemaker is used in such a way great resistance the pacemaker pulse amplitude to one such a low level that the heart stimulation without a dra Matic increase in the requirements for the input energy becomes practically impossible.

A main purpose of the invention is to achieve the above explained difficulties to overcome and an implan animal paired defibrillator pacemaker ben who is able to effectively both defibrilla tion pulse as well as pacemaker pulses without that there is a risk that the implantable assembly is destroyed or that myocardial tissue due to a Current path is damaged during defibrilla tion from the pacemaker stimulation electrode tip through the Heart runs to the conventional return path.

Another object of the invention is to provide a Tighten the switch in the pacemaker lead electrode ben, which makes it possible for an implantable coupling defibrillator pacemaker unit with the irritation never perform their impedance and still any current flow through the stimulus electrode lead during an internally abge prevent given defibrillation pulse. Another The object of the invention is a switch in the Specify the lead of the pacemaker electrode, which the Current flow through the line of the pacemaker electrode due to an externally delivered defibrillation pulse borders.

The task and the advantages of the invention will be below with reference to the drawing and associated description described in more detail.

Fig. 2 shows schematically a divider arrangement which illustrates the switch in the line of the stimulation electrode.

Referring to FIG. 1 a coupled defibrillator Schrittma is light illustrated with a Defibrillationsbereich 10 cher system provided with a angeord Neten on or above the heart defibrillation electrode 12 and an on or upper half of the heart disposed conventional surface electric de 16 is connected. There is also a pacemaker area 18 which is connected via a controllable switch 22 to a pacemaker electrode tip 90 arranged on or above the heart. The switch 22 is controlled by a large number of control signals which are described in more detail below.

The defibrillation area 10 and the pacemaker area 18 are controlled by a control unit 24 . The control unit 24 can have an EKG amplifier and other EKG analysis devices with which the type of treatment to be applied is determined. The pacemaker needle electrode 20 is typically able to sense the heart activity and to deliver such measurement information to the control unit 24 .

The pacemaker area is connected to the pacemaker needle electrode 26 via a line 26 . The switch 22 is typically located in the line 26 between the pacemaker area 18 and the pacemaker needle electrode 20 .

The switch 22 is controlled by the control signals for opening or closing in such a way that a current path is optionally obtained from the pacemaker needle electrode 20 to the pacemaker area 18 .

FIG. 2 illustrates a schematic electrical block diagram in which special control signals are used to control the switch 22 . These control signals include PACE, RECHARGE, TERM, DISCHARGE, DRIVE and BLANK. As already mentioned above, the switch 22 should be open when a defibrillation pulse is delivered to the heart so that the high-energy defibrillation pulse is not picked up by the pacemaker needle electrode 20 and transmitted to the pacemaker area 18 . On the other hand, the switch 22 should be closed when a pacemaker pulse is delivered to the heart via the pacemaker needle electrode 20 . Referring to FIG. 2, designed as a MOSFET transistor Q 1 in the conduit 26 between the pacing needle electrode 20 and the pacemaker area 18 is closed. The transistor Q 1 is optionally switched on and off to close or open the line 26 .

The signals PACE and RECHARGE trigger the transistor Q 1 to conduct and in this way establish a connection path between the pacemaker area 18 and the pacemaker needle electrode 20 . Transistor Q 1 is normally off and thus line 26 is normally open. The capacitor C 1 , which is connected to the gate of the transistor Q 1 , is charged to 6 volts via the diode D 1 and the resistor R 1 . The Transi stor R 1 is connected to a negative 6 volt supply. To switch transistor Q 1 when the PACE or RECHARGE signals are high, P-channel MOSFET transistor Q 3 is triggered by NORGATE 30 to charge capacitor C 1 6 volts above ground. This forces positive 6 volts at the gate of transistor Q 1 . During pacemaker operation, the source of transistor Q 1 is negative with respect to ground. On the other hand, the source of transistor Q 1 is less than 3 volts above ground during charging. Thus, the gate-source voltage of transistor Q 1 is above its threshold when the gate of transistor Q 1 is held at positive 6 volts via capacitor C 1 , so that transistor Q 1 is turned on. With transistor Q 1 turned on , a pacemaker or recharge current can flow through line 26 to electrode line 20 .

During defibrillation, transistor Q 1 is kept off to prevent current flow on line 26 . The DRIVE and BLANK signals are high during capacitor charging and defibrillation. These signals switch the MOSFET transistor Q 2 after they have passed through several logic gates. In particular, the DRIVE signal is present via an inverter 32 at the clock input of a D-type flip-flop 34 . The output of the flip-flop 34 is fed to a NAND gate 36 . The signal BLANK is also applied to a NAND gate 36 via an inverter 38 . The output of NAND gate 36 is fed to the gate of transistor Q 2 via an inverter 40 . If the signals DRIVE and BLANK are detected, the transistor Q 2 is switched so that the gate of the transistor Q 1 will keep a diode drop below ground. When the needle electrode potential increases during normal state defibrillation, the source and drain of the transistor Q 1 become positive. The source connection will want to follow the drain connection, but if the source connection increases the diode drop of the Zener diode Z 1 over, the source connection is shorted to a diode drop over earth. Since the gate of the transistor Q 1 is shorted to earth except for a diode drop, there is not enough gate-source voltage to turn on the transistor Q 1 . Therefore, the transistor remains in the non-conductive state and no current flows through line 26 . The transistor Q 1 is laid out in such a way that it typically withstands at least 800 volts before it breaks down.

In order to reset transistor Q 1 to its normally off state, the signals TERM and DISCHARGE are detected via NAND gate 42 , which is connected to the RESET input of flip-flop 34 via an inverter 44 . The PACE signal is also connected to an input of NAND gate 42 via an inverter 48 . If the corresponding combination of the signals TERM, PACE and DISCHARGE is detected, the transistor is reset to its usual off state.

When an external defibrillation pulse is placed on the patient, the source terminal of transistor Q 1 is held over a diode drop, so that transistor Q 1 is automatically switched off for the reasons stated above. This means that during an external defi brillation the gate connection of the transistor Q 1 is a Dio denabfall below ground. When the needle electrode potential rises from normal during external defibrillation, the source follows the drain. The zener diode Z 1 keeps the source connection of the transistor Q 1 a diode drop above earth, so that the switching on of the transistor Q 1 is prevented. Under these circumstances the gate potential is kept below the switching threshold and thus the transistor Q 1 is blocked and no current flows through the pacemaker needle electrode.

With the above description is only a case game will be given and it is not intended to the inven to limit it in any way.
