Abstract

Pulse-echo imaging is performed with a catheter or endoscope (11) in which a separate coaxial line (24) serves as both a driving plunger and a current supply wire. Two plungers (8,9) are mfd. of metallic or metallised wire, tape, leaf spring or rod. The ultrasonic transducer (3) surrounded by an acoustically transparent membrane radiates into transmission fluid with translatory pivotal movement effected by pushing and pulling of the plungers in opposite directions at opposite sides of the rocker (21). USE/ADVANTAGE - For high-resolution ophthalmology or dermatology. Catheter assembled from few simple miniaturised components can operate over significantly wider range of pivot angles.

Description

The invention relates to an ultrasound probe mainly for applications in in medicine, especially in vascular diagnostics and endoscopy. The The probe can be miniaturized to a great extent and is suitable for use in spe ultrasound catheters for endoluminal ultrasound imaging (Production of ultrasound images using the probe in the vessel, parts of the interior of the vessel, parts of the vessel wall and areas au mapped outside the vessel). The diagnostic goal is Ab formation of the geometry and structure of the vessel and the recognition pathological changes. The probe is also suitable for applications gene in endoscopy. Endoscopy is an examination technique (which can also be combined with therapeutic measures can) in which an endoscope is inserted into a body cavity via a body opening to be led. Via a specific channel within the endoscope miniaturized probe described here for the purpose of ultrasound imaging and / or ultrasound tissue characterization into the respective body cavity be performed.

Since the miniaturized ultrasound probe is preferably used at relatively high ultra sound frequencies works (compared to conventional ultrasound diagnostics stik) and this fact ensures a relatively high level of detail recognition, is the probe for non-endoscopic and non-endoluminal applications gen advantageously applicable, where the high level of detail is required is (ophthalmology, dermatology). Here is the mechanical flexibility and the possibility of application for larger ultrasound applications difficult to access places an advantage.

Stand der TechnikState of the art

In practice, there are two types of mechanically operated Ultra sound catheters. A technical solution is that a rotie wave in the catheter, an ultrasound transducer or an ultrasound deflector mirror is set in a permanent rotational movement. One of them witnessed pulse-echo image is comparable to a radar system. The rotating one The sound beam is either directed radially outwards, with a plane Cross-sectional area is shown as a radial panoramic view, or the rotie The sound beam is tilted a little forward, taking the ultrasound image corresponds to the outer surface of a blunt cone. Advantages are the one here simple, mechanically uncomplicated design as a minimalized disposable product; and the systems work with revolutions that represent a real time make the pictures possible. The disadvantage of these probes is now in that in the ultrasound images lateral cross-sections of the vessel only appear such locations are detected and displayed, which the catheter tip during of an examination process has already happened. The arrangement is unable imaging forward in the direction of the catheter axis. The one in front of the catheter lying vessel areas with any curvatures there, Ver The system cannot record any gaps or closures. There is one in it certain danger or limitation in use, and there is a Need for such probes, the vascular areas lying in front of the catheter tip can map ("preliminary image").

Durch manuelle Drehung des Katheters läßt sich die Bildebene drehen (horizontal, vertikal, jede beliebige Winkelorientierung, Fig. 1).A further technical solution that has hitherto been used less in practice takes this problem into account, and a mechanical construction is presented with which it is possible to generate an axially forward-oriented sector-shaped image by pivoting the ultrasonic transducer. Here, however, the purely translatory swivel movement of the ultrasound transducer is achieved via a permanently rotating flexible shaft in the catheter and a swivel lever construction at the front end of the catheter (catheter tip) (gearbox for the direct conversion of rotation into swivel movement). The areal pulse-echo image that is generated axially towards the front is sector-shaped (2D sector scan). The catheter's elongated axis lies in the center of the fan-shaped image area. By manually rotating the catheter, the image plane can be rotated (horizontally, vertically, any angular orientation, FIG. 1).

An automated and controlled turning of the catheter in the vessel in defi ned, reproducible angular steps in the vessel to record a 3-di dimensional volume range with the possibility of data storage and mathematical reconstruction of cutting planes with other orientations (e.g. lateral sectional images) is difficult. Furthermore, with Swivel lever gearbox constructions of this type and their peripheral details in principle only a fairly limited swivel angle range is possible. In addition, the miniaturization of such mechanically complex has so far been maintained limits in structures. Manufacturing as a single-use product would be too expensive. These are major drawbacks.

Ziel dieser ErfindungAim of this invention

The aim of the invention is to implement an ultrasonic probe with essential larger swivel angle range than in the case of the known swivel lever drive constructions. Furthermore, a catheter probe is to be realized that in the area of the ultrasonic transducer from a few, easy to manufacture, un complicated and easy to miniaturize parts. More complicated mechanical mechanisms are said to be limited due to the limits of miniaturizability outside the catheter near the actuator. The Drive unit should be as complete as possible and possibly plugged in as always that can be used. The actual catheter probe, which is made of human hygienic reasons can only be used once, should be mechanical be very simple and therefore inexpensive. The need for Wed. niaturization of many and relatively complicated individual elements should be avoided the. The generation of different sectional images (2D) by manual The catheter should be turned in the vessel as before, but simplified and better sert, be possible. The automated generation of side by side Cross-sectional images should also be possible. It allows after a suitable end Evaluation of the signals also the representation of laterally oriented sectional images (so-called C-scans, orientation perpendicular to the direction of sound propagation) as well as by storing all the data obtained in this way, capturing and displaying position of 3-dimensional volume areas in the area in front of the catheter tip, depending on the processing speed, also in real time.

Required mechanical precision work that the non-reusable re probe part should be avoided if possible, if by appropriate measures and reasonable additional effort during operation (Operating mode, system control, signal processing, software) of the system  to which the ultrasound probe is operated is possible and comparable Results (in terms of image quality, frame rate, etc.).

Lösung des Problemsthe solution of the problem

The invention solves the problems outlined above and pursues the above set goals successfully in the following ways:

Via one, two or more axially reciprocating, non-rotating, shielded wires, flat springs or lines (also coaxial line (s)) located in the catheter, generally also referred to below as plungers (so-called " Push-pull elements "meant), a miniature hinge, a film hinge, a ball hinge, a film or a rocker is driven. The ultra sound transducer is located on the miniature hinge, the foil hinge, the ball hinge, the foil or the rocker ( Fig. 5). In its rest position, it is oriented forwards in the direction of the catheter axis. In operation, the beam axis swivels around the forward direction, the converter does not rotate. This mechanism can be particularly good in miniaturized form and z. B. advantageous to apply in an ultrasound catheter probe for angio-sonographic purposes. Ver various possible technical designs are shown in the figures.

An axially forward-oriented image is generated, which is recorded mechanically according to the so-called sector scan principle. The catheter's elongated axis lies in the center of the fan-shaped image area. By rotation of the catheter to the image plane (any angular orientation, Fig. 1 and Fig horizontally, vertically. 2) can be rotated.

In some operating modes, the probe can be used for multi-directional sectional imaging (including tomographic concepts). In the "drive with three (four) tappets" according to FIG. 3a and FIG. 4 rotates the beam axis controlled electronically by the forward direction of the catheter. The converter does not rotate. The ultrasound probe can automatically scan and display surfaces of any orientation and curvature ( FIG. 3). This means that all object areas lying in front of the catheter tip can be scanned using the so-called B-scan technique. The reconstruction of flat sections with an orientation perpendicular to the direction of the catheter (and to the direction of sound propagation) is also possible (so-called C-scan technique). Finally, after scanning three-dimensional volume areas, the probe allows three-dimensional imaging (3D imaging). In this parallel-displaced azimuthally offset cutting planes, according to Fig. 3b, sectional planes shown in Fig. 3c or spirally wound conical shells according to Fig. 3a, Fig. 4b and Fig. 4c are used to sample the 3D regions. Non-flat cut surfaces (e.g. cone jackets) can also be used for two-dimensional imaging, whereby these surfaces are either developed on planes or shown in their original geometry by spotting. Of particular interest is also the possibility of capturing and imaging a 3D object area in that the axis of the ultrasound beam moves continuously on an alternating opening and closing spiral path according to FIG. 4a. This form of 3D imaging takes place by scanning the volume to be displayed by means of an endless, self-contained area (so-called "cone scan") and has the advantage over the methods according to FIGS . 3a, 3b and 3c that the probe has a must perform continuous movement and that time-consuming stopping and reversing processes during probe movement are thus eliminated as part of the scanning process.

BemerkungenRemarks

The following applies to all of the following explanations for the various drives in principle paragraphs I or II as well as paragraphs III, IV and V.

  • I. Für die elektr. B. in Fig. 2, Fig. 3, Fig. 6, Fig. 7, Fig. 8, Fig. 14, Fig. 15 skizziert.I. For the electr. Lead wires are provided in the catheter body, a separate coaxial line 24 , such as. B. in Fig. 2, Fig. 3, Fig. 6, Fig. 7, Fig. 8, Fig. 14, Fig. 15.
  • II. B. in Fig. 7 skizziert.II. The coaxial line is at the same time drive plunger and electrical guide wire 8 ( 24 ), such as. B. outlined in Fig. 7.
  • III. B. in Fig. 11 Nr. 14 skizziert.III. The plungers are metallic or metallized non-metallic wires, strips, flat springs or rods of any cross-section, which at the same time drove to and can be part of the electrical supply wires. Because of the relatively high operating frequency (ultrasonic frequency) of the arrangement, a shield 10 must be present ( FIG. 10, FIG. 11). In connection with the water either an artificial coaxial line or a shielded two-wire or multi-wire line such as. B. outlined in Fig. 11 No. 14.
  • IV. The supply lines 5 and 6 located on the ultrasonic transducer 3 (see, for. Example, Fig. 7, Fig. 8, Fig. 9, Fig. 11, Fig. 14, Fig. 15) are drive arrangements described in all here available, but mostly not shown.
For reasons of simplicity, not all are always complete Functional details outlined. Usually only the parts outlined, whose special features are just described.

The rocker drive can be provided with one or more drive tappets ( FIG. 6, FIG. 7: one tappet 8 , FIG. 8: two tappets 8, 9 ), depending on the size of the swivel angle range to be realized. With corresponding mechanical properties, a coaxial line can also be used as a plunger, with the double function "drive and power supply" and with the advantage that the thin connecting wires 5 and 6 remain mechanically unloaded directly on the converter during dynamic operation of the arrangement. Then only the coaxial line 8 ( 24 ) is stressed ( FIG. 6, FIG. 7). The mounting of the tappet (or the tappets) on the rocker can be fixed or can be provided with a joint. There are also possible designs (as shown in FIGS . 5i, j) in which a rear view is possible. FIGS. 6 and FIG. 7 illustrate a special feature. If necessary, the rocker 21 can be folded away so far at a standstill that an instrumentation channel 27 is free for therapeutic measures.

The film drive can freely pivot according to FIG. 9, or be guided according to FIG. 10. In the freely pivoting arrangement, the guide bars 17 are missing. This can have the consequence that the precision of the pivoting around a fixed point is lower. Any distortion that may occur during image processing can be corrected by calibration and more signal processing measures.

Die Kombination der Elemente 8, 9, 10, 11, 12 hat also drei Funktionen:The film hinge is driving in Fig. 11, Fig. 12 and Fig. 13. Fig. 11 shows the arrangement slightly deflected to the main beam direction. The plungers 8 and 9 , shown here by metallic wires attached to the film hinge, cause the ultrasonic transducer 3 to deflect when the drive is in opposite directions. On the other hand, the plungers 8 and 9 with the very thin flexibly attached lines 5 and 6 form the front part for the electrical voltage supply of the ultrasound transducer 3 . This also includes the shield 10 with the hygiene layer 12 which isolates itself from the body, and the extruded thermoplastic tube 11 , which is between about 20 cm and 100 cm long and is commonly used in the medical field as a catheter. This catheter is used for vascular examinations, but it can be used in a modified version in the instrument channel of an endoscope for examinations in body cavities. The combination of elements 8, 9, 10, 11, 12 has three functions:

  • 1. Spannungsversorgung (Sendeimpulse u. Empfangsechos)1. Power supply (transmit pulses and receive echoes)
  • 2. mechanische Stößel gegenläufig arbeitend2. mechanical tappets working in opposite directions
  • 3. Abschirmung gegen Störstrahlung3. Shielding against interference

Fig. 13 zeigt skizziert ein Folienscharnier. The shield 10 can consist of a metal layer or of a wire braid. Fig. 13 shows outlines a film hinge.

When flat spring drive 15 in accordance with two or more end fixedly joined flat springs Fig. 14 and Fig. 30 (cross-sectional dimensions usually 0.2 mm x 2 mm or smaller) z. B. made of spring steel or other metallic or non-metallic resilient material, an ultrasonic transducer 3 is mounted. The smaller the distance a between the tappets (flat springs 30 ), the smaller the stroke of the tappets. The deflection (swivel angle) of the transducer increases with a smaller distance a. The movement force to be exerted on the tappets must then also be greater. An instrument channel can also be arranged at point d in FIG. 15.

B. Kugelge lenke verwendbar. The drive with three (four) plungers is schematically sketched in Fig. 5k and Fig. 5l. The pivot plane can rotate about the forward direction of the catheter by controlling the three (four) plungers simultaneously. Depending on the complexity of the control, almost all conceivable swivel planes can be adjusted one after the other without the catheter tube having to be rotated by hand and without rotation of the transducer ( FIG. 3, FIG. 4). This enables the automatic measurement of three-dimensional areas that lie in front of the probe. Reconstructions and representations of 3D images and sectional images transverse to the forward direction of the catheter (C-scan) are thus possible. If the plungers have a high degree of elasticity, the 3D region that can be represented can encompass almost the entire anterior hemisphere of the catheter. The attachment of the plunger can be rigid, but there are also z. B. Kugelge joints can be used.

Konstruktive Vorteile dieser ErfindungDesign advantages of this invention

The generated axially forward-oriented image, which is recorded mechanically according to the so-called sector scan principle, covers a much larger sector than is the case with the few previous inventions. The swivel angle range of this invention can be 180 °. With a special design of the probe, a limited rear view is even possible. Furthermore, a variable swiveling range sam plitude can be realized with a suitable drive. It is even possible to change the main pivoting direction via the drive, which is not possible with all previously published designs. This is achieved by arranging the mechanics for the transformation "rotary motion-pivoting motion" (conventional cure belwelle- or camshaft gear) outside the catheter ( Fig. 16). The transformation "rotary motion-swivel motion" is split and spatially separated into the transformations "rotary motion-push motion" and the "push motion-swivel motion". As a result, the large swivel range can be achieved as a goal. The transformation "push-swivel movement" is particularly trival and can be applied inexpensively and easily within the catheter tip. The transformation "rotary motion-shock movement" is located outside the catheter at the level of the drive unit. This means that only simple, easy-to-manufacture and uncomplicated components within the catheter are necessary. The elements, which are difficult to miniaturize, are located outside the catheter. The outer dimensions of the drive unit can easily be in the handy area, so that it does not have to be miniaturized here. It is sufficient to keep the design small. The now outsourced transformation mechanism can be a bit more complex. The transformation stroke can thus be freely selected. (Fig. 5A through Fig. 5l), there is also the fact that the pivot angle and even the pivoting direction can be affected also on the form and the dimensions of the film hinge or film. This results in a possible design variety that can be easily adapted to the various medical applications.
