Description
Die Erfindung betrifft ein endoskopisches Nähgerät.
Bekannt ist ein Nähgerät dieser Art (EP-A1 0 482 881), bei dem eine gebo
gene Nadel im Verlauf ihres Bogens weitergedreht wird, um einen Stich zu
machen. Das Herausziehen der Nadel muß mit einer Zange erfolgen, ebenso
muß die Nadel aus dem Instrument herausgenommen werden.
Aufgabe der Erfindung ist es, ein endoskopisches Nähgerät zu schaffen, mit
dem mittels eines Fadens mehrere Stiche hintereinander gemacht werden
können.
Diese Aufgabe wird erfindungsgemäß dadurch gelöst, daß das Nähgerät
eine schraubenförmige Nadel mit einer oder mehreren Windungen aufweist,
daß am Nadelende der Faden fest und atraumatisch angebracht ist und daß
die Nadel von mindestens zwei Walzen angetrieben wird, zwischen denen sie
sich befindet.
Weitere Ausgestaltungen der Erfindung sind Gegenstand der Unteransprüche.
Die Erfindung wird nachfolgend anhand von Figuren näher erläutert.
Es zeigen:

Fig. 1 zeigt ein Nähgerät 2, das aus einem Handgriff 4, einem Schaft 6, ab
winkelbaren distalen Instrumentenkopf 7 und einem Walzenkopf 8 besteht.
Am Handgriff 4 sind ein Hebel 10 für den Antrieb der Walzen 12, 14, ein
Schieber 16 für das Öffnen und Schließen der Walzen sowie ein Drehknopf
18 für die Abwinklung des distalen Instrumentenkopfes 7 und ein weiterer
Drehknopf 20 zum Drehen des Walzenkopfes 8 vorhanden.
Weiter ist in Fig. 2a eine Nadel 22 dargestellt, die aus einer schraubenförmi
gen Wendel mit mehreren Windungen besteht. An der Nadel 22 befindet sich
ein Faden 24, der an seinem Ende einen Knoten 26 oder einen Knotenclip
26 aufweist.
Fig. 2b zeigt eine atraumatische Verbindung von Nadel und Faden, wobei der
Faden durch die Krimpstelle 23 in die Nadel eingequetscht ist. In bekannter
Weise kann der Faden auch mit der Nadel verklebt sein.
Das Nähgerät funktioniert so, daß die schraubenförmige Nadel 22 mit fest
angebrachtem Faden 24 zwischen zwei getriebenen Walzen 12, 14 einge
spannt ist und durch diese Walzen angetrieben und gedreht wird (Fig. 3a
und 3b). Dabei dreht sich die Nadel 22 und die Spitze durchdringt die zu
ver
nähenden Gewebe 21 (Fig. 5 und 6). Im weiteren Verlauf der Bewegung
schraubt sich die Nadel 22 in die zu vernähenden Gewebe 21 ein und ver
bindet diese. Durch entsprechendes Dirigieren mit dem distalen Instrumenten
kopf 7 kann der Ansatzpunkt der Spitze der Nadel 22 genau bestimmt
und auch nach einigen Stichen die Naht beendet werden, durch Herausfüh
ren der Nadelspitze aus dem Gewebe 21.
Vorteilhafterweise werden die zu vernähenden Gewebe 21 mit Zangen 50
und 52 zusammengehalten, und die Nadelspitze an der gewünschten Stelle
für den ersten Stich wie auch für die folgenden Stiche angesetzt. Es können
aber auch mehrere Stiche ohne Korrektur gemacht werden, wenn die zu ver
nähenden Gewebekanten und die Nadelachse (Walzen) die richtige Lage zu
einander haben und annähernd parallel verlaufen.
Der Faden 24 wird ebenfalls durch die beiden Walzen 12, 14, welche mit
einem weichem Kunststoff 13, 15, z. B. Silicon beschichtet sind, um die für
den Antrieb notwendige Friktion zwischen Nadel, Faden und Walze zu erzie
len, mit angetrieben und schlauft sich so durch das Gewebe 21 hindurch.
Hat die Nadel 22 die Gewebeschichten mit der gewünschten Stichzahl durch
drungen, so wird die Spitze aus dem Gewebe herausdirigiert und die Nadel
schraubt sich heraus, so daß nur noch der Faden 24 im Gewebe verbleibt.
Der Faden wird dann mit Hilfe von Zangen stramm gezogen und das Ende mit
einem Clip oder manuellem Knoten gegen Aufgehen gesichert.
Wie die Fig. 3 (a bis c) zeigen, wird die Nadel zwischen zwei Walzen 12, 14
oder drei Walzen 30, 32, 34 angetrieben, wobei die Walzen im Walzenkopf
8 befestigt sind. Der Walzenkopf kann um 180° gedreht werden, damit dann
die Nadel 22 voll im Lumen des Instrumentes liegt und dieses durch eine
Trokarhülse eingeführt werden kann. Zum Herausnehmen der Nadel 22
können die Walzen 12, 14 geöffnet werden (Fig. 4), indem sie durch Betäti
gen des Schiebers 16 scherenartig auseinander gefahren werden. Dies ist
in Fig. 4 bei 17 gestrichelt dargestellt. Der distale Instrumentenkopf 7 des
Gerätes ist abwinkelbar über den Drehknopf 18, welches eine feine Raststel
lung oder eine Verriegelung hat, die ihn in jeder Position fixiert um die Wal
zenachse in die Nahtrichtung zu bringen. Dies erleichtert das endoskopische
Arbeiten sehr. Die Schwenkbewegung der Walze 12, wird durch eine Feder
38 bewerkstelligt. Die Walze 12 ist an ihrem hinteren Ende kugelig und in
einer Kugelschale gelagert und über die Schraube 37 gehalten, so daß sie
die Schwenkbewegung ausführen kann. Ein Keil 45, welcher über die Stange
44 betätigt wird, drückt über den Stift 46 die Walze 12 nach unten gegen die
Walze 14. Die beiden Walzen sind über die Zahnräder 28 und 29 starr mit
einander gekoppelt.
Das Drehen des Walzenkopfes 8 wird über das Zahnrad 27, welches direkt
mit dem Walzenkopf verbunden ist und das Antriebszahnrad 35, welches mit
der Welle 42 verbunden ist, bewerkstelligt. Der Walzenkopf kann um 180° ge
dreht werden um wie bereits erwähnt, die Nadel mit dem Instrument zusam
men einzuführen. Die Welle 44 endet über einen flexiblen Seilzug am Schie
ber 16, die Welle 42 ist mit dem Drehknopf 20 verbunden, mit welchem der
Walzenkopf 8 gedreht werden kann.
Der Antrieb der Walzen erfolgt über die Welle 40, welche direkt mit der Walze
14 verbunden ist. Dieser Antrieb kann elektromotorisch oder auch über den
handbetätigten Hebel 10 erfolgen, wobei der Handhebel über eine Rätsche
nachgesetzt werden kann, so daß pro Griffhub eine Umdrehung oder weniger
gemacht werden kann. Eine Umdrehung bedeutet ein Stich. Der Operateur
kann dabei die Nadel gezielt und kontrolliert drehen, um die richtige Einstich
stelle für die Nadel anzugehen.
Diese Methode hat den Vorteil, daß der Arzt den Verlauf der Naht gut steuern
und kontrollieren kann und nicht einer mechanisch ablaufenden Bewegung
z. B. beim Motorantrieb willkürlich ausgesetzt ist, welcher schwierig zu steuern
ist.
Das Durchführen der endoskopischen Naht selbst ist in Fig. 5 dargestellt.
Für den Eingriff werden neben dem Nahtgerät 2 die endoskopische Optik
mit der Videokamera 51 sowie die Faßzangen 50 und 52 benötigt. Das zu
vernähende Gewebe wird durch die Faßzangen 50 und 52 gehalten, wie in
Fig. 6 detailliert dargestellt. Das Nähgerät wird mit der Nadel im Lumen durch
die Trokarhülse eingeführt. Nach dem Einführen wird der Walzenkopf 8 mit
Drehknopf 20 um 180° gedreht, die Nadel geht dabei in ihre Arbeitsstellung.
Die Walzenachse wird parallel zur Nahtrichtung gebracht, indem mit Dreh
knopf 18 die Abwicklung des Nahtgerätes 2 entsprechend eingestellt wird.
Die Nadel wird an das zu vernähende Gewebe angesetzt und mit dem Hand
hebel 10 die Drehung der Walze bzw. das Drehen der Nadel bewerkstelligt.
Mit den Zangen und dem Nahtgerät selbst kann der Nahtverlauf bestimmt wer
den.
Zur Anpassung der Naht an die medizinisch gestellt Anforderung wird es not
wendig sein, Nadeln mit verschiedenen Steigungen zu haben, um den richti
gen Stichabstand bei mehrfachen Stichen zu bekommen. Die Steigung der
Nadel soll daher zwischen 3 und 8 mm liegen. Der Stichabstand kann in
einem gewissen Bereich auch durch manuelles Verschieben des Nähgerätes
variiert werden. Die Windungszahl der Nadel soll so sein, daß die Lage der
Nadel zu den Walzen stabil ist, so daß wenigstens zwei Windungen in den
Walzen liegen. Dabei kann sich die Nadel in Richtung Walzenachse nicht
mehr verdrehen. Auch wird durch die zwei Nadelwindungen ein höheres An
triebsmoment durch Friktion auf die Nadel übertragen.
Es ist auch eine Nadel mit einer Windungszahl, die zwischen 1,25 und 1,5
liegt, einsetzbar. Diese Nadel ist dann nur im überlappenden Bereich verdre
hungssicher zur Walzenachse und ist für Einzelstiche anwendbar. Bei länge
ren Nähten ist eine Nadel mit mehreren Windungen 2 bis 5 vorteilhaft, wobei
die Windungszahl nie größer als die zu machende Stichzahl sein soll, da die
Nadel sonst mehr Einstiche macht als die gewünschte
Stichzahl.
Claims (13)
1. Endoskopisches Nähgerät, gekennzeichnet durch eine schrauben
förmige Nadel (22), mit einer oder mehreren Windungen und fest
atraumatisch angebrachtem Faden (24), wobei die Nadel (22) von
mindestens zwei Walzen (12, 14) angetrieben wird, zwischen denen
sie sich befindet.
2. Endoskopisches Nähgerät nach Anspruch 1, dadurch gekenn
zeichnet, daß der Antrieb mittels eines Motors erfolgt, welcher die
Nadel (22) umdrehungsweise oder kleineren wählbaren Schritten
ansteuerbar dreht.
3. Endoskopisches Nähgerät nach Anspruch 1, dadurch gekenn
zeichnet, daß der Antrieb über handbetätigten Hebel (10) mit einer
sich nachsetzenden Rätsche erfolgt, so daß eine Betätigung eine
ganze oder den Bruchteil einer Umdrehung der Nadel (22) bewirkt.
4. Endoskopisches Nähgerät nach Ansprüchen 1 bis 3, dadurch ge
kennzeichnet, daß sich die Walzen (12, 14) scherenartig oder
parallel schließen und öffnen über ein Betätigungsorgan (16) am
Handgriff (4) zum Einlegen oder Entnahme der Nadel (22).
5. Endoskopisches Nähgerät nach den Ansprüchen 1 bis 4, dadurch
gekennzeichnet, daß der Walzenkopf (8) zum Einführen des
Instrumentes mit eingesetzter Nadel (22) durch eine Trokarhülse
exzentrisch zur Instrumentenachse sitzt.
6. Endoskopisches Nähgerät nach Ansprüchen 1 bis 5, dadurch ge
kennzeichnet, daß der Walzenkopf (8) angetrieben ist und um min
destens 180° vom Handgriff (4) aus über Drehknopf (20) gedreht wer
den kann, um die Nadel in Arbeitsstellung zu bringen.
7. Endoskopisches Nähgerät nach Ansprüchen 1 bis 6, dadurch ge
kennzeichnet, daß der distale Instrumentenkopf (7) zum Schaft (6)
hin abgewinkelt werden kann über einen Drehknopf (18) am Instru
mentengriff und in jeder Stellung fixiert werden kann.
8. Endoskopisches Nähgerät nach Ansprüchen 1 bis 7, dadurch ge
kennzeichnet, daß die Walzen (12, 14) mit einem Belag (13, 15)
versehen sind, welcher die Haftreibung der Nadel (22) erhöht.
9. Endoskopisches Nähgerät nach Anspruch 8, dadurch gekenn
zeichnet, daß dieser Belag eine Siliconbeschichtung ist.
10. Endoskopisches Nähgerät nach Ansprüchen 1 bis 9, dadurch ge
kennzeichnet, daß die Walze (12, 14) aus hartem Material und mit
einer Riffelung versehen sind.
11. Endoskopisches Nähgerät nach Anspruch 1, dadurch gekenn
zeichnet, daß die schraubenförmige Nadel eine bis fünf Windungen
hat.
12. Endoskopisches Nähgerät nach Anspruch 1, dadurch gekenn
zeichnet, daß die Verbindung Nadel - Faden atraumatisch ist.
13. Endoskopisches Nähgerät nach Anspruch 1, dadurch gekenn
zeichnet, daß die schraubenförmige Nadel an ihrer Spitze mit
Schneiden ausgestattet ist, die ein leichtes Eindringen der Nadel
in das Gewebe ermöglichen.
