Abstract
Description
The present invention relates to a method for Manufacture of a medical breathing mask after the Preamble of claim 1 and a medical Breathing mask according to the preamble of claim 12.
Such breathing masks are used, for example, in patients who are life-threatening breathing disorders of sleep apnea are ill and in connection with nasal overpressure Respirators applied. This will make the patient adequately supplied with air during the sleep phase, so that brief respiratory arrests are prevented. Since the Air supply takes place under possibly considerable overpressure, it is important that the breathing mask is tight and tight on the nose and the surrounding areas of the patient's face.
Dies bedeutet u. a. einen erheblichen Kostenaufwand. Known breathing masks of the type mentioned are made-up Masks, d. that is, a limited number differ large and anatomically preformed masks are available. Naturally, this means that these masks never fit exactly, even though with considerable effort on the Areas of the patient's face must be pressed. This in turn means that the patient is not only the significant Train the straps must endure, but also that not inconsiderable pressure marks on the patient's face occur. There are leaks between the breathing mask and the relevant parts of the face on which the breathing mask lies on, the overpressure air supply can Respiratory air escape, which on the one hand causes pressure loss means and on the other hand especially in the area of the eyes is particularly disadvantageous in this respect, since there is the danger there is conjunctivitis in the eyes. Another the disadvantage in the medical field is that the dead space volume between the assembled breathing mask and the facial areas of the patient in question is large, which affects the response behavior of the overpressure Respirator or the response time in the absence of Breathing the patient and providing ventilation adversely affects. As a rule, therefore ready-made masks for patients with mask problems be changed frequently because they are either on any one Let the air escape or painful pressure points let arise. This means u. a. a significant one Expense. Only then do ready-made masks close tightly when put on with a certain pressure will. That means the pressure is not even can be distributed. In the area of the corner of the eye is the Print not as transferable, i. H. the pressure is weaker so that air / oxygen can easily escape. At the nose root or on the upper lip becomes the most pressure transferred, d. H. there is the pressure on the tissue at strongest d. H. pressure points often occur. This high contact pressure causes in connection with the shape of the Masenkrandes in patients who have dentures in the upper jaw or wear a full denture that this or this can be mobilized or easily pushed away. About that In addition, the ready-made mask is so large that it the view is obstructed.
The object of the present invention is therefore a To create breathing mask of the type mentioned, the is individually adapted and in particular a method for Manufacture of a customized breathing mask create with the corresponding in a very simple manner Breathing mask can be created that mentioned Avoids disadvantages and still with acceptable Effort can be made.
To solve this problem, a method for Manufacture of a medical breathing mask of the type mentioned the features specified in claim 1 and at a medical breathing mask of the type mentioned in claim 12 specified features provided.
The breathing mask according to the invention is not only for patients with sleep apnea syndrome but on all patients who have to be ventilated for a longer period of time or e.g. B. need hourly oxygen supply applicable. Due to the measures according to the invention, a perfect and The breathing mask is comfortably seated on the patient Breathing mask itself is lighter and more delicate and one has individual border. Surrender this way also with the necessary for positive pressure ventilation No pressure points on the patient's face. Through the optimal sealing of the breathing mask on the face no air or oxygen can escape from the patient in the area of the inner corner of the eye the danger of Conjunctivitis of the eyes. By the individual adjustment is a much smaller one Dead space volume inside the breathing mask reached, which leads to medical advantage leads that the response time of the Ventilator for changes in patient behavior can be reduced to a minimum. If the breathing mask at the Used positive pressure ventilation, it is sufficient to prevent the Escape of air or oxygen from the Fasteners in normal, holding the breathing mask Way to tighten, so that thanks to the edge Ensures tightness and thus increases comfort becomes. There are no pressure points. Should still be on air escape or a pressure point result, the breathing mask can at any time with the relevant soft plastic also retrospectively to be relined. Such post-processing, which may be necessary in particular if Face of the patient changes due to Weight changes are easy to do. At the mask according to the invention can thus possibly problems occurring individually on the existing mask be corrected e.g. B. by relining the silicone edge and in further exemplary embodiments, by moving of wire mounts, by changing the angle between mask base and connector u. the like There are no contact pressure problems because the mask is everywhere evenly applied, the pressure is transmitted evenly and the patient with a smaller contact pressure gets along. This also allows patients with dentures or a full denture in the upper jaw is no problem more. Since the breathing mask according to the invention is now a has a much more delicate shape, there are none Impairment of the field of vision more, so that the patient can read during ventilation.
15 vorgesehen. With the features according to claim 2 or 13 it is achieved that also the closing edge of the breathing mask in an individual way is adjusted, which is an additional increase in comfort and the tightness results. The are expediently Features according to claim 3 or 14 and / or 4 or 15 intended.
16 vorgesehen. To ensure an adequate and stable connection between the to achieve soft plastic and the raw mask, the features according to claim 5 or 16 are provided.
According to the features of one or more of claims 6 to 9 or 17 to 20 are different variants for manufacturing or attaching the connector for the air supply the raw mask realized.
A preferred embodiment in terms of manufacture of the model of the nose area results from the characteristics according to claim 10. Instead of an individually manufactured Impression tray could also be a made-up Impression trays are suitable, which are then slightly oversized must be, but can still be used without problems, however the individual spoon a little more precise; therefore he will especially in patients with difficult anatomical Conditions.
For a good hold of the breathing mask on the patient's head it is appropriate to the mounting brackets according to the To provide features according to claim 11 or 21. This is moreover a very simple and durable Type of attachment, which can also be easily moved if necessary.
Figs. 1 and 2 show an individually adapted to the relevant patient medical breathing mask 10 to both the pressure-term ventilation in under the sleep apnea syndrome patients suffering used can also be applied in such patients as that need to be ventilated for a longer period or z . B. need hourly oxygen supply. An essential component of the breathing mask 10 is a basic mask shape or raw mask 11 made of a deformable plastic, which represents the individual negative shape of the facial area of a patient containing the nose and its surrounding parts. The raw mask 11 is provided on the inside of its peripheral edge 12 with a layer 13 made of a soft plastic, preferably silicone. This silicone layer 13 , which is initially applied in a constant thickness and width, is also individually adapted to the shape of the aforementioned parts of the face of the patient concerned. The silicone layer 13 also covers the outer edge 22 of the peripheral edge 12 of the raw mask 11 . Before the soft silicone layer 13 is applied, the raw mask 11 is provided with perforations 14 along its circumferential edge 12 , so that the silicone layer 13 penetrates the raw mask 11 via knobs 15 for better adhesion.
In the area of the front end of the nose section 16 of the raw mask 11 , a connector 17 is formed in the form of a tubular extension. In the connecting piece 17 , which is integral with the raw mask 11 , a connector tube piece (not shown) or a hose directly can be inserted and held in place. The connector 17 is arranged in an area which is directly opposite the openings of the patient's nose.
Fig. 1 also shows a schematic representation of mounting brackets 19 for fastening extending around the patient's head bands, hoods or the like. The mounting brackets 19 have the form of wire stirrups 20, which are held by a plastic adhesive 21 permanently attached to the mask blank. 11
The medical breathing mask 10 shown in FIGS. 1 and 2 is produced in the following way:
First the nose and the surrounding parts of the face, such as the forehead, corner of the eye, cheekbones and upper lip of the patient in question are covered with several wax plates, each about 3 mm in diameter, the wax plates having previously been softened in a warm water bath. These wax plates serve as placeholders. A soft first plastic plate is placed over these wax plates and adjusted so that the wax plates are completely covered. This first plastic plate consists of a light-curing plastic, which is cured by means of a polymerization lamp after being deformed over the nose and the surrounding areas. In this way, an individually adapted nasal impression spoon is obtained, which represents the individual impression of the nose and the surrounding areas of the patient concerned. The hardened impression tray is perforated over its entire extent using a milling machine, so that it looks like a large-hole sieve.
An impression material, for example made of alginate, is then used or another impression material, such as silicone, Polyether rubber or impression plaster made. The example Stirred impression material is in the aforementioned individual impression trays introduced or on the Evenly distributed on the inside, with the impression material for mechanical connection in the perforation holes of the Impression tray penetrates. Before that on its inside prepared impression trays on the face area of the concerned patient is pressed, the nostrils the patient is padded with cotton wool so that, on the one hand, none Impression mass can get into the nostrils and others not the nostrils due to the impression material can be compressed. During the impression taking the Patient lie still and spread evenly through the mouth breathe that is not covered by the impression tray. After a Setting time of the impression material on the face area of the impression tray applied to the patient concerned is the Imprint acceptance completed. The setting time depends on impression material or impression material 2 to 6 used Minutes.
To produce the (positive) facial part model 25 of the patient, as shown in FIG. 3, the impression spoon completed by the impression material is poured out with a suitable material, preferably plaster. The set plaster model 25 is ground back to the necessary size so that it only represents the parts relevant for the breathing mask 10 . The plaster model 25 is at least partially somewhat larger than the original part of the patient's face due to the padding, in order to create corresponding free spaces, particularly in the area of the nostrils.
The plaster model 25 is placed in a vacuum thermoforming film device. A second plastic sheet with a thickness in the range of 2 to 4 mm is placed over the plaster model. In the thermoforming film device, this second plastic plate is heated until it becomes soft and is then deep-drawn over the plaster model 25 by means of a vacuum. After cooling, the deep-drawn second plastic plate is deformed into the basic mask shape or the raw mask 11 , which is then removed from the plaster model 25 . The raw mask 11 is cut back to its later dimensions or milled.
The peripheral edge 12 of the basic mask shape 11 , which negatively depicts the surrounding parts of the nose and the bridge of the nose, is provided with the perforations 14 . In addition, the inside of the peripheral edge 12 is coated with an adhesion promoter. It goes without saying that it is also possible to provide only one of the two, namely either the perforations 14 or the adhesion promoter, which serve to connect the silicone layer 13 mentioned to the raw mask 11 .
The soft plastic is then mixed in the form of the soft silicone mentioned and placed on the inside of the basic mask shape 11 along the peripheral edge 12 provided with the perforations 14 . The amount or thickness of the silicone layer 13 is arbitrary and depends on the individual or anatomical circumstances of the patient. E.g. the silicone layer 13 is about 1 to 2 cm wide and about 2 to 4 mm thick. After the silicone layer 13 has been applied evenly to the inside of the circumferential edge 12 , the raw mask 11 with the silicone layer 13 is placed on the patient lying still and held in place by hand until the soft silicone has hardened. This fixation takes about 8 to 10 minutes. After the soft silicone has hardened, the raw mask 11 provided with the layer 13 is removed and the excess silicone is cut off with a pair of scissors onto a narrow overhang over the edge 22 of the peripheral edge 12 .
The connection piece 17 in question for the supply of air or oxygen via a hose, not shown, from an overpressure respirator, also not shown, can be produced in two ways.
A variant consists in providing the raw mask 11 in the area of the front part, ie in front of the nostrils, with a single large bore and either inserting a short tube piece 18 into the bore ( FIG. 1) or placing it in front of the bore and with the help to firmly connect a plastic adhesive to the raw mask 11 . A cold polymer can be used as an adhesive. Another variant consists in supplementing the aforementioned plaster model in the area of the nostrils with a corresponding cylindrical negative part such that this part is firmly connected to the plaster model 25 . Then the plaster model 25 is placed with the cylindrical part in the vacuum thermoforming film device and covered with the thermoforming film, so that after the vacuum thermoforming process, the connector 17 is in one piece with the raw mask 11 in the form of a short pipe section. Any other connecting parts, such as swivel, rotary connections or the like, assembled hose connecting parts can be connected to the pipe piece 18 or the connecting piece 17 integrated in one piece with the raw mask 11 .
It is essential that either the connector or Pipe piece or the other connecting parts with a Exhaust air opening for the exhaled air is provided. Also the other connectors, if they can not are insertable, using a cold polymer with the Connection piece or pipe piece are connected.
To complete the breathing mask 10 , the fastening brackets 19 are used, which are designed, for example, as wire brackets 20 and are connected to the raw mask 11 by means of an adhesive, for example a cold polymer. Auxiliary devices holding the breathing mask 10 on the patient's head can be connected to these wire brackets 20 .
The materials used in the respiratory mask 10 , such as the thermoformable plastic film, the soft silicone, the cold polymer and the wire are materials that have been used intraorally in dentistry for years and are very well tolerated.
The individually adapted breathing mask 10 lies with the help of the silicone layer 13 close to the parts of the face or the bridge of the nose and yet leaves enough space for movement, in particular the nostrils, without a dead space volume which adversely affects ventilation.
The silicone layer 13 can be subsequently changed or supplemented when the face shape of a patient changes, for example when losing or gaining weight, by applying an additional silicone layer or by replacing the previous silicone layer 13 with a new one.
