- Technisches Gebiet
- Die vorliegende Erfindung ist im Allgemeinen auf eine chirurgische Behandlung von Glaukom gerichtet und betrifft insbesondere eine Vorrichtung zum kontinuierlichen Aufrechterhalten der Durchgängigkeit des Schlemmschen Kanals mit einem rinnenähnlichen Dauerstent, welcher chirurgisch plaziert werden kann, um wenigstens einen Teil des Umfangs des Kanals zu durchziehen und den Abfluss von Humor aquosus dadurch zu erleichtern.
- Hintergrund der Erfindung
- Glaukom ist ein wesentliches öffentliches Gesundheitsproblem, weil Glaukom eine Hauptursache von Blindheit ist. Die Blindheit, die aus einem Glaukom resultiert, involviert sowohl zentrales als auch peripheres Sehen und hat eine große Auswirkung auf die Fähigkeit eines Einzelnen, ein unabhängiges Leben zu führen.
- Glaukom ist eine optische Neuropathie (eine Erkrankung des optischen Nervs), die normalerweise im Rahmen eines erhöhten Augeninnendrucks auftritt. Der Druck im Auge steigt, und dies ist mit Veränderungen in der Erscheinung („Schröpfen") und Funktion („blinde Flecken" im Gesichtsfeld) des optischen Nervs assoziiert. Wenn der Druck für eine Zeit, die lang genug ist, hoch genug bleibt, tritt totaler Sehverlust auf. Im Auge entwickelt sich ein hoher Druck aufgrund eines internen Flüssigkeitsungleichgewichts.
- Das Auge ist eine hohle Struktur, die eine klare Flüssigkeit enthält, die als „Humor aquosus" bezeichnet wird. Humor aquosus wird in der hinteren Augenkammer durch den Ziliarkörper mit einer Rate von etwa 2,5 Mikrolitern pro Minute gebildet. Die Flüssigkeit, welche mit einer ziemlich konstanten Rate gebildet wird, geht dann um die Linsen, durch die Öffnung der Pupille in der Iris und in die vordere Kammer des Auges. Ist die Flüssigkeit einmal in der vorderen Augenkammer, fließt sie durch zwei verschiedene Routen aus dem Auge. In der „uveoskleralen" Route sickert die Flüssigkeit zwischen Muskelfasern des Ziliarkörpers. Diese Route ist für annähernd zehn Prozent des Kammerwasserabflusses bei Menschen verantwortlich. Der primäre Weg für den Kammerwasserabfluss bei Menschen ist durch die „kanalikuläre" Route, die das Trabekelnetzwerk und den Schlemmschen Kanal involviert.
- Das Trabekelnetzwerk und der Schlemmsche Kanal sind an der Verbindung zwischen der Iris und der Sklera angeordnet. Diese Verbindung oder Ecke wird als „der Winkel" bezeichnet. Das Trabekelnetzwerk ist eine keilförmige Struktur, die um den Umfang des Auges läuft. Sie ist aus Kollagenstrahlen zusammengesetzt, die in einer dreidimensionalen siebähnlichen Struktur angeordnet sind. Die Strahlen sind mit einer Monoschicht von Zellen gesäumt, die als Trabekelzellen bezeichnet werden. Die Abstände zwischen den Kollagenstrahlen sind mit einer extrazellulären Substanz gefüllt, die durch die Trabekelzellen gebildet wird. Diese Zellen bilden ebenso Enzyme, die das extrazelluläre Material abbauen. Der Schlemmsche Kanal ist dem Trabekelnetzwerk benachbart. Die äußere Wand des Trabekelnetzwerks entspricht der inneren Wand des Schlemmschen Kanals. Der Schlemmsche Kanal ist eine schlauchförmige Struktur, die um den Umfang der Kornea läuft. Es wird vermutet, dass in menschlichen Erwachsenen der Schlemmsche Kanal durch Septen in eine Reihe autonomer Sackgassenkanäle geteilt ist.
- Die wässrige Flüssigkeit wandert durch die Zwischenräume zwischen den Trabekelstrahlen, über die innere Wand des Schlemmschen Kanals in den Kanal, durch eine Reihe von etwa 25 Sammelkanälen, die vom Schlemmschen Kanal ableiten, und in das episklerale venöse System. In einer normalen Situation ist die Kammerwasserproduktion gleich dem Kammerwasserabfluss, und der Augeninnendruck bleibt im Bereich von 15 bis 21 mmHg ziemlich konstant. Bei Glaukom ist der Widerstand durch das kanalikuläre Abflusssystem abnorm hoch.
- Es wird vermutet, dass der abnormale Widerstand in primärem Weitwinkelglaukom, welches die häufigste Form von Glaukom ist, durch die äußere Seite des Trabekelnetzwerks und die innere Wand des Schlemmschen Kanals kommt. Es wird vermutet, dass ein abnormer Metabolismus der Trabekelzellen zu einem exzessiven Aufbau von extrazellulären Materialien oder einem Aufbau von abnorm „steifen" Materialien in dieser Region führt. Eine Histopathologie von Glaukomaugen zeigt ebenso einen Kollaps des Schlemmschen Kanals. Primäres Weitwinkelglaukom ist für annähernd fünfundachtzig Prozent aller Glaukome verantwortlich. Andere Formen von Glaukom (wie zum Beispiel Engwinkelglaukom und sekundäre Glaukome) involvieren ebenso einen verminderten Abfluss durch den kanalikulären Weg, aber der erhöhte Widerstand wird durch andere Ursachen, wie zum Beispiel eine mechanische Blockierung, inflammatorische Trümmer, eine zelluläre Blockierung etc. bewirkt.
- Mit dem erhöhten Widerstand sammelt sich wässrige Flüssigkeit an, weil sie nicht schnell genug ablaufen kann. Da sich die Flüssigkeit ansammelt, steigt der Augeninnendruck (intraocular pressure, IOP) innerhalb des Auges. Der erhöhte IOP presst die Axone im optischen Nerv und kann ebenso die vaskuläre Versorgung zum optischen Nerv beeinträchtigen. Der optische Nerv trägt das Sehen vom Auge zum Gehirn. Einige optische Nerven scheinen gegenüber IOP empfindlicher zu sein als andere Augen. Während die Forschung Möglichkeiten untersucht, den Nerv vor einem erhöhten Druck zu schützen, ist der einzige gegenwärtig verfügbare therapeutische Ansatz für Glaukom die Verminderung des Augeninnendrucks.
- Die klinische Behandlung von Glaukom wird schrittweise erreicht. Eine medikamentöse Behandlung ist oft die erste Behandlungsoption. Wenn diese medikamentösen Behandlungen entweder topisch oder oral verabreicht werden, wirken sie, um entweder die Flüssigkeitsproduktion zu vermindern, oder sie wirken, um den Abfluss zu erhöhen. Gegenwärtig verfügbare medikamentöse Behandlungen haben viele ernste Nebenwirkungen, einschließend: kongestive Herzinsuffizienz, Atemnot, Hochdruck, Depression, Nierensteine, aplastische Anämie, sexuelle Dysfunktion und Tod. Die Befolgung der medikamentösen Behandlung ist ein Hauptproblem, wobei geschätzt wird, dass mehr als die Hälfte der Glaukompatienten ihre korrekten Dosierungsvorschriften nicht befolgen.
- Wenn die medikamentöse Behandlung versagt, um den Druck adäquat zu vermindern, wird oft eine Laser-Trabekuloplastie durchgeführt. Bei einer Laser-Trabekuloplastie wird thermische Energie von einem Laser auf eine Anzahl von einander nicht berührenden Stellen im Trabekelnetzwerk angewendet. Es wird vermutet, dass die Laserenergie den Metabolismus der Trabekelzellen auf gewisse Weise stimuliert und das extrazelluläre Material im Trabekelnetzwerk verändert. In annähernd achtzig Prozent der Patienten wird der Kammerwasserabfluss erhöht, und der IOP sinkt. Jedoch ist die Wirkung oft nicht langandauernd, und fünfzig Prozent der Patienten entwickeln innerhalb von fünf Jahren einen erhöhten Druck. Die Laserchirurgie ist normalerweise nicht wiederholbar. Des Weiteren ist die Laser-Trabekuloplastie keine wirksame Behandlung für primäres Weitwinkelglaukom in Patienten im Alter von weniger als fünfzig Jahren, noch ist sie für Engwinkelglaukom und viele sekundäre Glaukome wirksam.
- Wenn die Laser-Trabekuloplastie den Druck nicht ausreichend vermindert, wird eine Fistelungsoperation durchgeführt. Mit einer Fistelungsoperation wird ein Loch in die Sklera und die Winkelregion gemacht. Dieses Loch erlaubt es, dass das Kammerwasser das Auge über einen alternativen Weg verlässt.
- Das am häufigsten durchgeführte Fistelungsverfahren ist eine Trabekulektomie. Bei einer Trabekulektomie wird ein posteriorer Einschnitt in die Konjunktiva gemacht, das transparente Gewebe, welches die Sklera bedeckt. Die Konjunktiva wird nach vorne gerollt, was die Sklera am Limbus freilegt. Es wird ein Sklerahautlappen mit partieller Dicke gemacht, und es wird in der halben Dicke in die Kornea eingeschnitten. Es wird in die vordere Augenkammer unterhalb des Sklerahautlappens eingedrungen, und ein Teil der tiefen Sklera und des Trabekelnetzwerks wird ausgeschnitten. Der Sklerahautlappen wird wieder locker an seinen Platz angenäht. Der Konjunktiva-Einschnitt wird fest verschlossen. Postoperativ läuft das Kammerwasser durch das Loch unterhalb des Sklerahautlappens und sammelt sich in einem vergrößerten Raum unterhalb der Konjunktiva. Die Flüssigkeit wird dann entweder durch Blutgefäße in der Konjunktiva absorbiert oder durchquert die Konjunktiva in den Tränenfilm.
- Eine Trabekulektomie ist mit vielen Problemen verbunden. Fibroblasten, die in der Episklera vorhanden sind, proliferieren und wandern und können im Sklerahautlappen eine Narbe hinterlassen. Versagen durch Vernarben kann auftreten, insbesondere bei Kindern und jungen Erwachsenen. Von den Augen, die anfänglich eine erfolgreiche Trabekulektomie haben, versagen achtzig Prozent durch Vernarben innerhalb drei bis fünf Jahren nach dem chirurgischen Eingriff. Um eine Fibrose zu minimieren, wenden Chirurgen nun antifibrotische Mittel auf den Sklerahautlappen während des chirurgischen Eingriffs an, wie zum Beispiel Mitomycin C (MMC) und 5-Fluoruracil (5-FU). Die Verwendung dieser Mittel hat die Erfolgsrate der Trabekulektomie erhöht, hat aber ebenso die Prävalenz von Hypotonie erhöht. Hypotonie ist ein Problem, das sich entwickelt, wenn das Kammerwasser aus dem Auge zu schnell abfließt. Der Augendruck sinkt zu stark ab (normalerweise weniger als 6,0 mmHg), die Struktur des Auges kollabiert, und das Sehvermögen sinkt.
- Eine Trabekulektomie schafft einen Weg für das Kammerwasser, um zur Oberfläche des Auges zu entweichen. Gleichzeitig schafft sie einen Weg für Bakterien, die normalerweise auf der Oberfläche des Auges und der Augenlider leben, um in das Auge zu gelangen. Wenn dies passiert, kann eine Infektion des inneren Auges auftreten, die Endophthalmitis genannt wird. Eine Endophthalmitis führt oft zu permanentem und profundem Verlust des Sehvermögens. Eine Endophthalmitis kann jederzeit nach einer Trabekulektomie auftreten. Das Risiko steigt mit den dünnen Blasen, die sich nach MMC und 5-FU entwickeln. Ein anderer Faktor, der zu der Infektion beiträgt, ist die Lage der Blase. Augen, bei denen eine Trabekulektomie unten durchgeführt wurde, haben ein etwa fünffaches Risiko einer Augeninfektion als Augen, die eine obenliegende Blase haben. Daher wird eine erste Trabekulektomie entweder im nasalen oder temporalen Quadranten oben unter dem Augenlid durchgeführt.
- Zusätzlich zur Vernarbung, Hypotonie und Infektion gibt es andere Komplikationen einer Trabekulektomie. Die Blase kann tränen und zu einer profunden Hypotonie führen. Die Blase kann reizen und den normalen Tränenfilm zerstören, was zu einem verschwommenen Sehvermögen führt. Patienten mit Blasen können im Allgemeinen keine Kontaktlinsen tragen. Alle Komplikationen der Trabekulektomie kommen davon, dass Flüssigkeit vom Inneren des Auges auf die äußere Oberfläche des Auges abgeleitet wird.
- Wenn eine Trabekulektomie den Augendruck nicht erfolgreich senkt, ist der nächste chirurgische Schritt oft eine Kammerwasser-Shuntvorrichtung. Eine Kammerwasser- Shuntvorrichtung des Standes der Technik ist ein Silikonschlauch, der an einem Ende an eine Plastikplatte (Polypropylen oder anderer Kunststoff) befestigt ist. Bei einer Kammerwasser-Shuntvorrichtung wird ein Einschnitt in die Konjunktiva gemacht, was die Sklera freilegt. Die Plastikplatte wird hinten an die Oberfläche des Auges genäht, normalerweise über dem Äquator. Am Limbus wird ein Loch in der ganzen Dicke in das Auge gemacht, normalerweise mit einer Nadel. Der Schlauch wird in das Auge durch dieses Loch eingesetzt. Der äußere Teil des Schlauches wird entweder mit Spendersklera oder -perikard bedeckt. Die Konjunktiva wird zurückgelegt, und der Einschnitt wird fest geschlossen. Mit der gegenwärtigen Technologie von Kammerwasser-Shuntvorrichtungen bestehen viele Probleme, die die Vernarbung, Versagen, Hypotonie und Infektion einschließen.
- Einige Referenzen aus dem Stand der Technik für eine Glaukombehandlung richten sich auf den Schlemmschen Kanal, aber diese involvieren nicht das Plazieren von langfristigen Dauerstents. Zum Beispiel lehrt US-Patent Nr. 5,360,399 das Plazieren eines Teils eines Plastik- oder Stahlschlauches im Schlemmschen Kanal mit einer Injektion eines viskosen Materials durch den Schlauch, um das Trabekelnetzwerk hydraulisch zu zerschneiden. Nach der Injektion wird der Schlauch aus dem Kanal entfernt. Außerdem hat die '399-Vorrichtung ein Injektionsmanschettenelement mit größerem Durchmesser relativ zu dem Anteil innerhalb des Schlemmschen Kanals, welches als ein Adapter für eine Injektion und eine Spülung dient. Daher ist diese Vorrichtung für ein dauerhaftes Plazieren innerhalb des Schlemmschen Kanals nicht angepasst.
- EP A 0 898 947 beschreibt eine Stentvorrichtung, die in den Schlemmschen Kanal eingesetzt werden kann, um einen erhöhten Augeninnendruck durch Erleichterung des Abflusses zu lindern.
- WO 00/13627 als Stand der Technik gemäß Art. 54(3) und (4) EPÜ zeigt eine Stentvorrichtung mit zwei ovalen Öffnungen in der Stentwand.
- Also existiert ein Bedarf für ein physiologischeres System, um den Abfluss des Kammerwassers durch den Schlemmschen Kanal zu erhöhen.
- Erhöhen des Kammerwasserflusses direkt in den Schlemmschen Kanal würde Vernarbung minimieren, da die Winkelregion mit einer einzigen Linie von nicht-proliferierenden Trabekelzellen bevölkert ist. - Zusammenfassung der Erfindung
- Die vorliegende Erfindung stellt eine Stentvorrichtung gemäß Anspruch 1 bereit, wobei der Stent in dem Schlemmschen Kanal plaziert wird, um die Dimensionen des Kanals auszudehnen und seine Durchgängigkeit aufrechtzuerhalten. Die vorliegende Erfindung erleichtert daher den normalen physiologischen Weg zum Abfluss von Humor aquosus in und durch den Schlemmschen Kanal. Die vorliegende Erfindung ist weiterhin gerichtet auf die Bereitstellung eines permanenten Dauerstents innerhalb des Schlemmschen Kanals für eine Glaukombehandlung.

- Die vorliegende Erfindung stellt eine Humor-aquosus-Stentvorrichtung bereit, die innerhalb eines Teils des Schlemmschen Kanals des Auges als ein Dauerimplantat plaziert werden soll, um den Kanal auszudehnen und die Durchgängigkeit des Kanals aufrechtzuerhalten, wobei die Stentvorrichtung einen Körperanteil umfasst, welcher so geformt ist, dass er vollständig innerhalb des Schlemmschen Kanals aufgenommen wird, um den natürlichen Abfluss des Humor aquosus zu den Sammelgängen des Auges zu erleichtern.
- Die vorliegende Erfindung stellt ebenso Ausführungsformen eines erfindungsgemäßen Stents bereit, umfassend einen dünnen Körper aus einem biokompatiblen Material einer angepassten Länge und Form, so dass er innerhalb des Schlemmschen Kanals vollständig aufgenommen wird und um sich innerhalb eines Teils des Umfangs des Schlemmschen Kanals auszudehnen, und welcher einen Kanal darin hat, um die Passage des Humor aquosus in und durch den Schlemmschen Kanal zu den Sammelgängen zu erleichtern. Die Erfindung beabsichtigt Gestaltungen für eine Stentvorrichtung, die das Leiten des Humor aquosus durch den Schlemmschen Kanal unterstützt, indem eine Rinne bereitgestellt wird. Der Körper des Stents kann zwischen einer ersten Insertionsstellung und einer zweiten ausgedehnten Stentstellung wechseln, wenn er an der gewünschten Stelle des Kanals ist. Die Vorrichtung, die in den Schlemmschen Kanal reicht, kann aus einem flexiblen, porösen oder nicht porösen biologisch inerten Material hergestellt werden, um annähernd einem Teil des Radius', der Kurvatur und dem Durchmesser des Schlemmschen Kanals gleichzukommen. Die gesamte Vorrichtung oder Teile davon können tubulär oder nicht tubulär und fenestriert oder nicht fenestriert sein. Die Vorrichtung kann eine Größe haben, um Plazieren im gesamten Umfang oder einem Teil des Umfangs des Schlemmschen Kanals zu ermöglichen.
- Die traditionelle Glaukomlehre erklärt, dass in einem Erwachsenen der Schlemmsche Kanal durch Septen in separate Kanäle geteilt ist, was das vollständige Legen einer Naht unmöglich macht. Vorläufige Studien an menschlichen Augen von Erwachsenen aus einer Augenbank haben gezeigt, dass der Schlemmsche Kanal tatsächlich offen ist. Eine Naht kann entlang des gesamten Umfangs des Kanals gelegt werden. Es wurde bisher nicht bestimmt, dass in normalen Individuen der Schlemmsche Kanal überall in seinem Umfang offen ist, im Gegensatz dazu, dass er durch Septen in viele Kanäle mit toten Enden geteilt ist. Die vorliegende Erfindung verwendet diese Kenntnis, um eine Durchgängigkeit innerhalb des Schlemmschen Kanals mit den vorliegenden Stentvorrichtungen zu schaffen und aufrechtzuerhalten.
- Eine Vergleichsausführungsform wird in
1 veranschaulicht, in welcher die Stentvorrichtung100 in einer Seitenansicht gezeigt wird. Die Stentvorrichtung100 umfasst einen tubulären Körperanteil10 , der ein Lumen5 definiert, welcher feste tubuläre Wände haben kann oder eine Vielzahl von Fenestrierungen15 enthalten kann, die zwischen dem Lumen5 und dem Äußeren kommunizieren. Der Körperanteil hat eine vorgeformte Kurvatur mit einem Radius r, welcher den 6-mm-Radius des Schlemmschen Kanals eines adulten menschlichen Auges annähert. Der Querschnittsdurchmesser des Körperanteils10 hat eine Größe, so dass er vollständig innerhalb des Schlemmschen Kanals aufgenommen wird. Der Körperanteil10 kann entweder eine eingeschlossene tubuläre oder vielseitige Struktur sein, oder er kann eine ebene, winklige oder gekrümmte offene Struktur sein, oder eine gewisse Kombination des Obigen, wenn er an verschiedenen Seiten entlang der Gesamtheit seiner Länge quergeschnitten wird. Die Fenestrierungen15 können entlang eines beliebigen Anteils der Vorrichtung100 plaziert werden, um die Passage der Flüssigkeit dadurch zu erleichtern. - Andere Beispiele von Ausführungsformen sind in
2 –3 gezeigt.2 zeigt eine Vergleichsausführungsform des Stents, in welcher die Vorrichtung100 ein luminales tubuläres Netz in seiner Gestaltung umfasst, wieder mit einer vorgeformten Kurvatur mit einem Radius r, um den 6-mm-Radius des Schlemmschen Kanals anzunähern, und einem Querschnittsdurchmesser des Körperanteils10 , welcher eine Größe hat, so dass er vollständig innerhalb des Schlemmschen Kanals aufgenommen wird. -
3 zeigt eine Ausführungsform des erfindungsgemäßen Stents, in welcher der Körperanteil10 offen ist und entlang seiner Länge zu einem rinnenähnlichen Kanal gekrümmt ist, wieder mit einer vorgeformten Kurvatur mit einem Radius r, um den 6-mm-Radius des Schlemmschen Kanals anzunähern, und einem Querschnittsdurchmesser des Körperanteils10 , welcher eine Größe hat, so dass er vollständig innerhalb des Schlemmschen Kanals aufgenommen wird. - Da die erfindungsgemäße Vorrichtung ein Langzeitimplantat ist, kann sie aus einem Material hergestellt werden, welches unschädlich gegenüber den Geweben und Flüssigkeiten ist, mit denen es in Kontakt ist. Es ist bevorzugt, dass die Vorrichtung während ihres in situ-Sitzes nicht absorbiert, korrodiert oder anderweitig strukturell beeinträchtigt wird. Überdies ist ebenso bevorzugt, dass die Augengewebe und das Kammerwasser nicht nachteilig durch die Anwesenheit der implantierten Vorrichtung beeinflusst werden. Eine Anzahl von Materialien ist verfügbar, um die Konstruktionsspezifikationen und medizinischen Spezifikationen für die Stents zu erfüllen. In einer exemplarischen Ausführungsform der vorliegenden Erfindung wird die Stentvorrichtung
100 aus einem biologisch inerten flexiblen Material wie zum Beispiel Silikon oder ähnlichen Polymeren konstruiert. Alternative Materialien können dünnwandiges Polytetrafluorethylen, Polypropylen oder andere Polymere einschließen, sind aber nicht darauf beschränkt. Andere Metalle und Legierungen, die im Stand der Technik des Stentings bekannt sind, können ebenso verwendet werden, wie zum Beispiel rostfreier Stahl, Titan oder Nitinol. Der Stent kann ebenso mit therapeutischen Mitteln hergestellt werden, die mit der Zeit aus der Vorrichtung wandern. - In den Gestaltungen, die in
1 –5 gezeigt werden, kann der Körperanteil10 eine vorgeformte Kurve haben, um den 6,0-mm-Radius des Schlemmschen Kanals in einem menschlichen Auge anzunähern. Der Körperanteil10 kann eine ausreichende Länge haben, um sich durch eine beliebige Länge des gesamten Umfangs des Schlemmschen Kanals auszudehnen, wobei die gesamte Länge für den Körperanteil10 etwa 1,0 mm bis 40 mm oder etwa 2 mm bis 20 mm oder etwa 5 mm beträgt, um ein Plazieren durch den Schlemmschen Kanal entlang des Umfangs zu erlauben. - Der Durchmesser oder die Weite des Körperanteils
10 kann eine Größe haben, um einen inneren Durchmesser von zwischen 0,1 mm und 0,5 mm, vorzugsweise etwa 0,2 mm, und einen äußeren Durchmesser von zwischen 0,1 mm und 0,5 mm oder etwa 0,3 mm für einen tubulären oder gekrümmten Stent zu erzielen, oder eine vergleichbare maximale Weite für einen Stent mit einer vielwinkligen Gestaltung. Der Körperanteil10 kann eine Vielzahl von Fenestrierungen enthalten, um einen Flüssigkeitsaustritt zu erlauben, welche angeordnet sind, um einen Verschluss durch die benachbarten Wände des Schlemmschen Kanals zu verhindern, insbesondere in Richtung der Sammelgänge. - Die chirurgische Anatomie, die für die vorliegende Erfindung relevant ist, wird in
4 veranschaulicht. Im Allgemeinen zeigt4 die vordere Augenkammer35 , den Schlemmschen Kanal30 , die Iris40 , Kornea45 , Trabekelnetzwerk50 , Sammelkanäle55 , episklerale Venen60 , Pupille65 und Linse70 .5 veranschaulicht die chirurgische Plazierung einer Stentvorrichtung mit den relevanten anatomischen Beziehungen. Es sollte angemerkt werden, dass die erfindungsgemäße Vorrichtung so konstruiert ist, dass das Plazieren von mehreren Stents innerhalb des Schlemmschen Kanals30 dazu führen kann, dass der Schlemmsche Kanal30 entlang fast des Umfangs durchzogen wird. Der chirurgische Einschnitt in den Schlemmschen Kanal30 wird geschlossen, wobei keine direkte externe Kommunikation mit der Stentvorrichtung100 beabsichtigt wird. - Das chirurgische Verfahren, welches notwendig ist, um die Vorrichtung einzusetzen, kann alle oder einige der folgenden Schritte einschließen: Ein Konjunktiva-Einschnitt wird gemacht. Es wird dann ein Sklerahautlappen mit partieller Dicke geschaffen, und es wird in der halben Dicke in die klare Kornea eingeschnitten. Die hintere Seite des Schlemmschen Kanals wird identifiziert, und es wird hinten in den Kanal eingedrungen. Die vordere Augenkammer kann durch Injektion eines viskoelastischen oder myotischen Mittels vertieft werden. Ein Ballonkatheter, wie zum Beispiel beschrieben in
US 6 524 275 , eingereicht am 26. April 2000, kann in den Schlemmschen Kanal eingeführt und aufgebläht werden, um Teile des Schlemmschen Kanals zu dilatieren, gefolgt von einem selektiven Ablassen des Ballons und Plazieren von einem oder mehreren Stentvorrichtungen in den Schlemmschen Kanal. Alternativ können die Stentvorrichtungen direkt auf einer Ballonkathetervorrichtung eingeführt werden. Daher kann eine Vielzahl von Stentsegmenten an ausgewählten Stellen entlang des Umfangs des Schlemmschen Kanals plaziert werden. Jegliches verbleibendes Stentmaterial wird getrimmt, und der Sklerahautlappen und die Konjunktivawunde werden auf konventionelle Weise geschlossen.
Claims (6)
- Stentvorrichtung zur Verwendung im Auge, um erhöhten Augeninnendruck zu entlasten, indem der Abfluss durch den und innerhalb des Schlemmschen Kanals erleichtert wird, wobei die Vorrichtung einen dünnen Körper (
10 ) aus einem biokompatiblen Material einer angepaßten Länge und Form umfasst, so dass er innerhalb eines Teils des Schlemmschen Kanals (30 ) vollständig gehalten wird, wobei (a) der Körper (10 ) gekrümmt ist, um einen rinnenähnlichen Kanal (5 ) zu definieren, welcher über die Länge des Körpers (10 ) geöffnet ist, (b) der Körper (10 ) einen äußeren Durchmesser von etwa 0,1 mm bis 0,5 mm hat, und (c) der Körper (10 ) eine Länge von etwa 1 mm bis 40 mm hat. - Stentvorrichtung gemäß Anspruch 1, wobei der Körper (
10 ) eine Krümmung hat, welche einen Radius hat, welcher dem Radius des Schlemmschen Kanals (30 ) eines menschlichen Auges angenähert ist, wobei der Radius zwischen etwa 3 mm und 10 mm liegt. - Stentvorrichtung gemäß einem der Ansprüche 1 bis 2, dadurch gekennzeichnet, dass die Krümmung einen Radius von etwa 6 mm hat.
- Stentvorrichtung gemäß einem der Ansprüche 1 bis 2, dadurch gekennzeichnet, dass der Körper (
10 ) einen äußeren Durchmesser von etwa 0,3 mm hat. - Stentvorrichtung gemäß einem der Ansprüche 1 bis 2, dadurch gekennzeichnet, dass der Körper (
10 ) eine Länge von etwa 20 mm hat. - Stentvorrichtung gemäß einem der Ansprüche 1 bis 2, dadurch gekennzeichnet, dass der Körper (
10 ) eine Vielzahl von Fenestrierungen (15 ) darin hat, welche die Passage von Flüssigkeit in den Schlemmschen Kanal (30 ) erlauben.
