Description
- Die vorliegende Erfindung betrifft ein intervertebrales Implantat, das vor allem zum Fixieren eines Knochenspans zwischen zwei Wirbeln bestimmt ist, und insbesondere ein als Cage bezeichnetes Körbchen, das die Knochenspäne zu enthalten vermag.
- Anwendungsgebiet der Erfindung ist insbesondere die chirurgische Behandlung von degenerierten Bandscheiben.
- Diese Behandlung umfasst allgemein einen operativen Eingriff, durch den zwei aneinandergrenzende Wirbel miteinander fixiert werden sollen, und zwar durch Einfügen von Knochenspänen, durch die diese nach Anwachsen des Transplantats fest miteinander verbunden sind. Angesichts der durch die beiden Wirbel auf die Knochenspäne ausgeübten mechanischen Belastungen besteht jedoch insbesondere die Gefahr, dass diese abgestoßen oder zerstört werden.
- Zum Beheben dieses Nachteils werden die Knochenspäne bekannterweise mit Hilfe von sogenannten Cages fixiert, deren oberes und unteres Ende, an deren Rändern die Wirbel aufliegen, offen ist, um den Kontakt zwischen den Knochenspänen und der oberen Wirbelplatte bzw. der unteren Wirbelplatte herzustellen.
- Das Fixieren von zwei Wirbeln miteinander durch ein Knochenelement hat den Vorteil, der auf diese Weise erzielten Einheit gleichmäßige mechanische Eigenschaften zu verleihen und dem Ganze vor allem eine elastische Verformbarkeit zu geben, die durch deren Elastizitätsmodul gekennzeichnet ist.
- Die allgemein verwendeten Cages sind jedoch aus Titanlegierung, und der Elastizitätsmodul dieser Legierungen ist mindestens zehnmal so groß wie der Elastizitätsmodul des Wirbelknochens. Da die Intervertebralcages dauerhaft zwischen den Wirbeln verbleiben, entsteht durch sie folglich eine Ungleichmäßigkeit der mechanischen Eigenschaften in der durch die beiden zusammengewachsenen Wirbel gebildeten Einheit.
- Um eine gleichmäßige elastische Verformung zu bewahren, wurde die Verwendung von Cages aus makromolekularen Werkstoffen vorgeschlagen, deren Elastizitätsmodule etwa denen des Knochens entsprechen. Die allgemein ringförmigen Cages aus Kunststoff sind jedoch empfindlicher als Cages aus Titanlegierung, und ihr Einsetzen zwischen den Wirbeln unter Aufwendung von Kraft kann zum Brechen ihrer Ränder führen. Durch Verbreitern ihrer Ränder wird das für die Knochenspäne verfügbare Volumen verringert und somit auch die Bindungskraft zwischen den beiden Wirbeln. Da sie nicht so hart wie die Cages aus Titanlegierung sind, dringen ihre Ränder zudem weniger stark in die Wirbelplatten ein, so dass ihre Fixierung bezüglich der Wirbel weniger gut gewährleistet ist.
- Das Patent
US-A-5,192,327 beschreibt ein intervertebrales Implantat gemäß dem Oberbegriff von Patentanspruch 1. - Das Patent
FR-A-2 742 653 - Eine Aufgabe der vorliegenden Erfindung besteht darin, ein intervertebrales Implantat aus makromolekularem Werkstoff vom Typ eines Cages bereitzustellen, das eine obere Öffnung und eine untere Öffnung gleicher Größe wie die der Cages aus Titanlegierung aufweist, um eine im Wesentlichen gleiche Transplantationsoberfläche beizubehalten, und dessen Halt bezüglich der Wirbel in feststehender Position verbessert ist.
- Hierzu schlägt die Erfindung ein intervertebrales Implantat vor, das dadurch gekennzeichnet ist, dass wenigstens ein Abschnitt des zweiten unteren und oberen Rands aus dem durch den ersten unteren und oberen Rand des ersten Teils gebildeten Raum übersteht und eine gekrümmte Form hat, und dessen Enden den ersten unteren bzw. oberen Rand des ersten Teils derart verbinden, dass die zweiten Ränder die bikonkave Form des intervertebralen Zwischenraums beachten und zusätzliche Auflagepunkte des Implantats an den Wirbelplatten in deren Mittelteil bilden.
- Ein Merkmal des intervertebralen Implantats beruht demnach auf der Verstärkung des ersten, allgemein ringförmigen Teils durch einen zweiten, langgezogenen Teil, der in der vorbestimmten Einsetzrichtung des Implantats zwischen den Wirbeln angeordnet ist. Trotz der während des Einklemmens zwischen den Wirbeln auf den ersten Teil ausgeübten Belastungen, die diesen entlang seiner Mittelebene derart zu verformen versuchen, dass die im Wesentlichen gegenüberliegenden Abschnitte einander angenähert werden, behält der zweite Teil seine allgemein ringförmige Form bei. Die obere und die untere Oberflächenöffnung, die durch den zweiten Teil halbiert wird, hat die gleiche Größe wie die Oberflächen eines intervertebralen Implantats aus Titanlegierung, wodurch eine im Wesentlichen identische Transplantationsoberfläche erzielt wird.
- Der zweite untere und obere Rand steht aus dem durch den ersten unteren und oberen Rand des ersten Teils gebildeten Raum solchermaßen über, dass das Implantat nicht nur durch den Kontakt der Ränder des ersten Teils mit den Wirbelplatten zwischen den Wirbeln gehalten wird, sondern auch durch den zweiten langgezogene Teil, der an den bikonkav geformten Wirbelplatten anliegt.
- Der zweite untere und obere Rand des zweiten Teils hat ferner eine gekrümmte Form, und ihre Ränder verbinden jeweils den ersten unteren bzw. den ersten oberen Rand des ersten Teils. Die Form des zweiten Teils stimmt somit im Wesentlichen mit der Form des zwischen den Wirbeln befindlichen verfügbaren Raums überein.
- Gemäß einer ersten Ausführungsform der Erfindung sind der erste Teil und der zweite Teil derart miteinander verbunden, dass sie ein einziges Stück bilden, so dass sie miteinander vollkommen fest verbunden sind.
- Gemäß einer bevorzugten Ausführungsform der Erfindung ist der erste Teil entlang der Hauptachse des zweiten Teils seitlich derart durchbohrt, dass ein Sackloch ausgebildet ist, das sich in dem zweiten Teil fortsetzt, wobei das Sackloch ein Werkzeug zum Einführen aufzunehmen vermag.
- Wie nachstehend noch näher erläutert wird, erfolgt das Einsetzen des Implantats zwischen zwei aneinandergrenzenden Wirbeln gemäß dieser bevorzugten Ausführungsform unter Aufwendung von Kraft, und die Einsetzrichtung muss notwendigerweise vollkommen kontrolliert erfolgen. Um einen konstanten Stoßwinkel beizubehalten, wird das Werkzeug, das an das Implantat angesetzt wird, in ein Sackloch eingeführt, das sich in dem zweiten Teil fortsetzt. Das an das Implantat angesetzte Ende des Werkzeugs ist somit während der Stoßbewegung vom Implantat umschlossen, und es besteht keine Gefahr, dass durch dieses ein in der Umgebung des Eingriffs befindliches Gewebe durchbohrt wird.
- Vorteilhafterweise weisen die unteren und oberen Ränder des ersten und des zweiten Teils Kerben auf, die zum Bilden von Verankerungspunkten in den Wirbel geeignet sind.
- Der spitze Abschnitt der Kerben dringt somit in die Wand der Wirbelplatten ein. Das Implantat wird demnach zwischen den Wirbeln ohne jedes mögliche seitliche Spiel gehalten, was für das Anwachsen der Knochspäne von Vorteil ist.
- Vorzugsweise weist der erste, allgemein ringförmige Teil wenigstens eine Symmetrieachse auf, und vorteilhafterweise verbindet der zweite Teil die beiden gegenüberliegenden Abschnitte des ersten Teils entlang der Symmetrieachse.
- Diese Ausbildung erlaubt es, das Implantat vollständig der Form der Wirbel und insbesondere der Wirbelkörper anzupassen. Da der zweite Teil entlang der Symmetrieachse angeordnet ist, werden die während des Einklemmens auf das Implantat ausgeübten Beanspruchungen gleichmäßig verteilt.
- Gemäß einer anderen bevorzugten Ausführungsform der Erfindung verbindet der zweite Teil die beiden gegenüberliegenden Abschnitte des ersten Teils in einer Richtung, die mit der Symmetrieachse einen Winkel zwischen 0° und 90° bildet.
- Das Einsetzen des Implantats zwischen den Wirbeln lässt sich somit leichter bewerkstelligen, wenn die Stelle, an der es eingesetzt werden soll, einen Eingriff symmetrisch zur Wirbelsäule nicht zulässt. Das Implantat muss nämlich stets entlang der Achse des zweiten Teils eingeführt werden, damit der erste Teil nicht bricht, und manche Stellen der Wirbelsäule sind nur von der Seite zugänglich. In diesem Fall ist der zweite Teil bezüglich der Symmetrieachse des Implantats schräg angeordnet.
- Der erste Teil weist vorzugsweise im Wesentlichen die allgemeine Form eines Halbkreises auf.
- Gemäß einer anderen bevorzugten Ausführungsform der Erfindung ist das Stück aus einem makromolekularen Werkstoff geformt. Es wird kostengünstig hergestellt. Vorzugsweise ist das Stück aus Polyetheretherketon, das leicht formbar ist und dem Knochen verwandte elastische Eigenschaften aufweist.
- Gemäß einer zweiten Ausführungsform der Erfindung umfasst der zweite Teil ferner Verankerungsmittel, die jeweils in den zweiten oberen Rand und in den zweiten unteren Rand münden. Beim Einsetzen des Implantats zwischen den Wirbeln dringen die Verankerungsmittel somit derart kräftig in die Oberfläche der Wirbelplatten ein, dass das Implantat bezüglich der Wirbel translatorisch vollkommen fixiert ist und bezüglich diesen nicht verrutschen kann.
- Gemäß einer speziellen Ausführungsform sind die Verankerungsmittel aus einem Verankerungsstück mit zwei Enden gebildet, und weist der zweite Teil eine Aussparung auf, die derart in den zweiten unteren und oberen Rand mündet, dass das in die Aussparung eingeführte Verankerungsstück den zweiten Teil vollständig durchquert, wobei das erste Ende in den zweiten oberen Rand mündet und das zweite Ende in den zweiten unteren Rand des zweiten Teils mündet.
- Das Verankerungsstück vermag somit in den zweiten Teil während dessen Einsetzens zwischen den Wirbeln eingeführt zu werden, sofern dieses unbedingt erforderlich ist. Da die Verankerungsmittel einstückig ausgebildet sind, wird ihre Steifigkeit zudem dadurch erhöht, und es erfolgt keine Verformung während des Einführens.
- Vorteilhafterweise ist das Verankerungsstück im Wesentlichen trapezförmig, wobei seine beiden Enden durch die beiden nicht aufeinanderfolgenden und zueinander nicht parallelen Seiten gebildet sind. Es weist vorzugsweise eine Mittelebene auf, und ist in den zweiten Teil derart eingeführt, dass die Hauptachse des zweiten Teils die beiden Grundlinien des Trapezes schneidet, und dass seine Mittelebene im Wesentlichen lotrecht zur Mittelebene des ersten Teils ist. Durch die Aussparung, die im Wesentlichen die Form eines rechteckigen Parallelepipeds hat, stimmen die Wände des Verankerungsstücks mit den Wänden der Aussparung überein.
- Eine der Grundlinien des Trapezes bildet vorzugsweise mit den beiden anderen zueinander nicht parallelen Seiten zwei spitze Winkel, und das Verankerungsstück ist in den zweiten Teil derart eingeführt, dass das Implantat von der größten Grundlinie aus in Richtung der kleinsten Grundlinie des Trapezes eingeführt wird. Das Verankerungsstück weist demnach einen an jedem Ende durch den spitzen Winkel gebildeten vorstehenden Punkt auf, der tiefer in den Wirbelkörper eingebettet zu werden vermag und mit diesem noch fester verbunden ist. Das Verankerungsstück bildet ferner, wenn es zwischen den Wirbeln eingefügt ist, ein Stück eines Pfeils, wenn das Implantat von der größten Grundlinie des Trapezes aus in Richtung der kleinsten Grundlinie eingesetzt wird. Das Verschieben des Verankerungsstücks in entgegengesetzter Richtung ist somit durch das Einbetten der vorstehenden Punkte in die Wirbelplatten unmöglich. Vorzugsweise ist das Verankerungsstück aus Titanlegierung ausgeführt, und seine Steifigkeit erlaubt eine bessere Verankerung.
- Weitere Besonderheiten und Vorteile der Erfindung ergeben sich unter Bezugnahme auf die beigefügten Zeichnungen aus der nachfolgenden Beschreibung von speziellen Ausführungsformen der Erfindung, die rein informativen Charakter haben, jedoch nicht einschränkend sind. Es zeigen:

1 zeigt das intervertebrale Implantat eingefügt zwischen zwei aneinandergrenzenden Wirbeln. Zur Beschreibung des Implantats gemäß einer ersten Ausführungs form der Erfindung wird auch auf2 verwiesen, in der dieses vorzugsweise zwischen Lendenwirbeln eingefügt ist. - Das intervertebrale Implantat weist einen in
1 im Profil dargestellten ersten Teil10 auf. Die vordere Seite Ra der Wirbelsäule liegt gegenüber der vorderen Wand des Körpers und die hintere Seite Rp der Wirbelsäule liegt gegenüber der hinteren Wand des Körpers. - Das intervertebrale Implantat ist zum Fixieren von Knochenspänen zwischen zwei Wirbeln bestimmt, um die Bandscheibe zu ersetzen. In
1 ist die Bandscheibe DIV1 intakt zwischen den Wirbeln V2 und V3 dargestellt. - Der sich längs der Wirbelsäule entlang der Achse R erstreckende Wirbelkanal enthält das Rückenmark und aufgrund dessen Empfindlichkeit erfolgt das Einsetzen des Implantats zwischen zwei Wirbeln verständlicherweise vorzugsweise zunächst von vorne. Dies ist insbesondere bei den meisten Eingriffen bei zervikalen Bandscheibenvorfällen der Fall.
- In
2 , in der das intervertebrale Implantat im Profil dargestellt ist, ist der erste, allgemein ringförmige Teil10 erkennbar, der eine Aussparung11 bildet, die durch einen zweiten, entlang einer Achse A langgezogenen Teil16 halbiert ist, wobei der zweite Teil16 zwei gegenüberliegende Abschnitte des ersten Teils10 miteinander verbindet. Der erste obere Rand12 des ersten Teils10 liegt im Wesentlichen in einer Ebene, die zu der durch den ersten unteren Rand14 des ersten Teils10 definierten Ebene parallel ist. Jeder der ersten Ränder12 ,14 ist nach Einsetzen des Implantats zwischen den Wirbeln mit der jeweiligen Wirbelplatte in Kontakt. - Die Aussparungen
11 sind zur Aufnahme der Knochenspäne bestimmt, die im oberen Teil mit der unteren Wirbelplatte des oberen Wirbels und im unteren Teil mit der oberen Wirbelplatte des unteren Wirbels in Kontakt sind. Die Transplantate befinden sich somit zwischen den Wirbeln und sind derart fixiert, dass dem Transplantat ein Anwachsen und eine fest Verbindung möglich ist. Bei diesen Transplantaten handelt es sich allgemein um am Darmbeinkamm entnommene Kortikalisspäne, deren Dicke im Wesentlichen dem intervertebralen Zwischenraum entspricht. Es versteht sich, dass die intervertebralen Implantate dauerhaft dort verbleiben und Bestandteil der Verbindung sind, wenn die Knochenverbindung zwischen den Wirbeln hergestellt ist. - Damit das Implantat nach Einsetzen zwischen den Wirbeln so stabil wie möglich ist, weist der zweite Teil
16 , der die beiden gegenüberliegenden Ränder des ersten Teils10 verbindet, einen zweiten unteren18 und einen zweiten oberen20 gekrümmten Rand auf, die aus dem ersten Teil10 überstehen. Diese Geometrie erlaubt es, die bikonkave Form des intervertebralen Zwischenraums zu beachten und zusätzliche Auflagepunkte des Implantats an den Wirbelplatten in deren Mittelteil zu bilden. - Vor dem Einfügen des mit den Knochenspänen versehenen Implantats wird die beschädigte Bandscheibe in der Weise entfernt, dass das Einsetzen des Implantats möglich ist. Danach wird das Implantat in den intervertebralen Zwischenraum in einer vorbestimmten Richtung eingesetzt.
- Diese vorbestimmte Richtung hängt von der Stelle der Wirbelsäule, an der der Eingriff vorgenommen wird, und der therapeutischen Indikation ab.
- Das in
2 dargestellte intervertebrale Implantat ist dazu bestimmt, an der Vorderseite der Wirbelsäule eingesetzt zu werden. Es weist ein Sackloch22 auf, das in die Wand des ersten ringförmigen Teils10 gebohrt ist und sich in dem zweiten Teil16 entlang dessen Hauptachse A fortsetzt, die in die Symmetrieachse S des ersten Teils10 übergeht. Das Ende des Einsetzwerkzeugs wird in das Sackloch22 eingeführt und kommt in dem zweiten Versteifungsteil16 derart zur Anlage, dass das Implantat beim Einsetzen in den intervertebralen Zwischenraum gehalten und geführt wird. - Ein Merkmal der Erfindung beruht nämlich auf der Einlage des zweiten langgezogenen Teils
16 in dem ersten Teil, damit dieses den Beanspruchungen standhalten kann, denen es während des Einklemmens ausgesetzt ist. Um eine optimale Verformungsfestigkeit zu erlangen, müssen die Stoßbewegungen entlang der Achse A des zweiten Teils16 erfolgen. Das Sackloch22 hält folglich das Stoßwerkzeug in eine bezüglich des Implantats konstante Richtung entlang der Achse A dieses Teils. Dadurch, dass das Ende des Einsetzwerkzeugs zum Teil mit dem Implantat fest verbunden ist, lässt sich der Weg des Implantats zwischen den Wirbelkörpern kontrollieren. - Die Fixierung des Implantats zwischen den Wirbeln hängt von den Reibungskräften ab, die an der Verbindungsstelle zwischen den Rändern
12 ,14 ,18 und20 des ersten Teils10 und des zweiten Teils16 und den Wirbelplatten auftreten. Um diese Rei bungskräfte zu erhöhen, weisen die Ränder12 ,14 ,18 und20 Kerben auf, die Verankerungspunkte in den Wirbelplatten bilden. - Gemäß einer speziellen Ausführungsform sind die Kerben entgegen der Einsetzrichtung schräg ausgebildet, so dass die Reibungskräfte des Implantats an den Wirbeln in der Einsetzrichtung geringer sind als die in entgegengesetzter Richtung auftretenden Reibungskräfte. Das Implantat ist somit entgegen der Einsetzrichtung festgeklemmt.
- Der erste ringförmige Teil
10 aus2 weist eine Symmetrieachse S auf, die in die Symmetrielängsachse des zweiten Teils16 übergeht, wodurch ein zur vorderen Seite der Wirbelsäule senkrechtes Einsetzen möglich ist. Der erste Teil10 hat im Wesentlichen die allgemeine Form eines Halbkreises, so dass er im Wesentlichen der Form der Wirbelplatten entspricht. Die durch das Implantat gebildete Oberfläche des Transplantats ist demnach optimal. - Gemäß einer anderen speziellen Ausführungsform der Erfindung, die in
3 dargestellt ist, verläuft der zweite Teil16 bezüglich der Symmetrieachse S des ersten Teils10 schräg. Das den Seitenrand des ersten Teils10 durchbohrende Sackloch22 setzt sich ebenfalls in dem zweiten Teil16 entlang dessen Achse A fort, die bezüglich der Symmetrieachse S schräg verläuft. - Diese Ausbildung erlaubt es, das Implantat schräg zur Vorderseite der Wirbelsäule einzusetzen, was bei einigen schwer zugänglichen intervertebralen Zwischenräumen erforderlich sein kann.
- Wie in
3 dargestellt, bilden der zweite langgezogene Teil mit der Achse A und die Symmetrieachse S des ersten Teils einen Winkel von im Wesentlichen 45°. Der zweite Teil kann jedoch so angeordnet sein, dass er mit der Symmetrieachse S einen Winkel zwischen 0° und 90° bildet. - Gemäß eine bevorzugten Ausführungsform ist das Implantat aus einem makromolekularen Werkstoff hergestellt, der Verformungseigenschaften aufweist, die denen des Knochens ähnlich sind und genauer gesagt zwischen denen der Kortikalis und denen der Spongiosa liegen.
- Der Elastizitätsmodul oder Young's Modulus liegt bei der Spongiosa bei etwa 0,1 GPa und bei der Kortikalis bei etwa 12 GPa. Der makromolekulare Werkstoff wird daher aus den synthetischen Werkstoffen gewählt, die einen Elastizitätsmodul zwischen 0,1 und 12 GPa aufweisen.
- Der Werkstoff weist vorzugsweise einen Elastizitätsmodul zwischen 1 und 6 GPa auf, der somit einen guten Kompromiss darstellt und, wenn er zwischen zwei Wirbeln eingesetzt ist, für keinerlei Ungleichmäßigkeit hinsichtlich den elastischen mechanischen Eigenschaften der aus den Wirbeln und dem Implantat gebildeten Einheit sorgt.
- Vorteilhafterweise ist das Implantat einstückig aus Polyetheretherketon formgegossen. Dieser Werkstoff weist einen Elastizitätsmodul von 3,5 GPa auf.
- Die Tatsache, das Implantat formgießen zu können, wirkt sich auf die Kosten günstig aus und erlaubt die Herstellung von Intervertebralcages zu einem Preis, der unter den Fertigungskosten von Cages aus Titanlegierung liegt.
- Die offene Oberfläche des Implantats, die der Summe der Oberflächen entspricht, die durch den Schnittpunkt einer Mittelebene des Implantats und der beiden Aufnahmen definiert sind, und durch die Gesamtfläche dividiert wird, die durch den Schnittpunkt der Mittelebene mit den Außenlinien des ersten Teils definiert ist, entspricht gemäß einer weiteren speziellen Ausführungsform einer Verhältniszahl zwischen 0,75 und 0,95.
- Nachstehend wird nun eine weitere spezielle Ausführungsform der Erfindung mit Bezug auf
3 ,4 und5 beschrieben, bei der das Implantat vorzugsweise zwischen den Halswirbeln eingesetzt ist. - In
4 ist ein Implantat erkennbar, das einen ersten Teil10 und einen zweiten Teil16 umfasst, der zwei gegenüberliegende Ränder des ersten Teils10 verbindet. Der zweite Teil16 weist analog zu dem in2 dargestellten zweiten Teil einen zweiten unteren Rand18 und einen zweiten oberen Rand20 auf. Er ist jedoch mit einem Verankerungsstück40 versehen, das vorzugsweise aus Titanlegierung gefertigt ist, um biokompatibel und äußerst haltbar zu sein. Das Verankerungsstück40 weist ein erstes Ende42 auf, das in den zweiten oberen Rand20 mündet, sowie ein zweites Ende44 , das in den zweiten unteren Rand18 mündet. - Hierzu weist der zweite Teil
16 eine Aussparung46 allgemein in Form eines rechteckigen Parallelepipeds auf, das in den zweiten unteren Rand18 und in den zweiten oberen Rand20 mündet und dessen vier durch die Innenwände der Aussparung46 gebildeten Kanten im Wesentlichen senkrecht zur Mittelebene P des ersten Teils10 sind. Auf diese Weise vermag das Verankerungsstück40 , das geringfügig größer als die Aussparung46 ist, die Aussparung46 unter Aufwendung von Kraft vollständig zu durchqueren. Es wird somit bezüglich des zweiten Teils16 durch Reibung gegen die Innenwände der Aussparung46 ortsfest gehalten, die im Wesentlichen voneinander abgerückt sind. - Die Abmessungen des Verankerungsstücks
40 zwischen den beiden Enden42 und44 sind natürlich größer als die der Aussparung46 , so dass die beiden Enden42 und44 an jedem der zweiten Ränder18 ,20 überstehen. -
5 zeigt das Verankerungsstück40 in perspektivischer Ansicht. Es ist trapezförmig und weist zwei gegenüberliegende, zueinander parallele erste Ränder48 und50 , welche die Grundlinien des Trapezes bilden, und zwei gegenüberliegende zweite Ränder52 und54 auf, die mit der Symmetrieachse T des Verankerungsstücks40 einen identischen Winkel bilden. Das Verankerungsstück40 weist demnach zwei identische spitzwinklige Ecken56 und58 auf, die in die Oberfläche der Wirbelplatten einzudringen vermögen. - Die beiden zweiten gegenüberliegenden Ränder
52 und54 haben ferner ein spitzes Profil, um noch leichter in die Wirbelplatten eindringen zu können. - Das Verankerungsstück
40 definiert eine Mittelebene Pa und ist, wie in4 dargestellt, derart in die Aussparung46 eingefügt, dass die Mittelebene Pa lotrecht zur Mittelebene P des ersten Teils10 ist. Es ist ferner solchermaßen eingefügt, dass die Richtung F zum Einführen des Implantats in den intervertebralen Zwischenraum einer Richtung entspricht, die vom ersten Rand50 mit größeren Abmessungen zu dem gegenüberliegenden Rand48 mit kleineren Abmessungen verläuft. Diese Anordnung verhindert verständlicherweise, dass das Verankerungsstück das Einsetzen des Implantats in den intervertebralen Zwischenraum behindert, wenn die Enden42 und44 an den Wirbelplatten der beiden Wirbel anliegen. Die beiden Ecken56 und58 des Verankerungsstücks, die beim Einsetzen des Implantats in die Wirbelplatten eindringen, bilden ebenfalls verständlicherweise Mittel, um das Verankerungsstück40 und somit das Implantat insbesondere entgegen der Einsetzrichtung kräftig festzuklemmen. Das Implantat wird somit zwischen den beiden Wirbeln wirksamer gehalten. -
6 zeigt das Implantat von vorne, versehen mit dem Verankerungsstück40 , dessen erstes Ende42 über dem ersten Teil10 und dessen zweites Ende44 darunter sichtbar ist. - Bei der Ausführungsform, bei der das Implantat allgemein zwischen Halswirbeln eingesetzt ist, ist der erste Teil
10 bezüglich seiner Mittelebene P nicht genau symmetrisch. Wie in6 dargestellt, weist der erste untere Rand14 an den Seitenteilen des ersten Teils10 gebogene Abschnitte60 auf, wobei zwischen der Tangentialebene Ptg des ersten Teils10 und dem ersten unteren Rand14 Freiräume62 erkennbar sind. Der erste obere Rand des ersten Teils weist ferner einen gewölbten Abschnitt64 auf, an dem der zweite Teil16 mit dem ersten Teil10 verbunden ist. - Durch diese Ausbildung stimmt das intervertebrale Implantat vollkommen mit der Geometrie des intervertebralen Zwischenraums, insbesondere an den Halswirbeln, überein.
- Angesichts dieser Asymmetrie muss das Implantat in einer vorbestimmten Richtung eingesetzt werden. Hierzu vermag ein in der Symmetrieachse S liegendes Sackhauptloch
66 , ein Werkzeug aufzunehmen, und ein Richtungsloch68 erlaubt es, das Implantat in der vorbestimmten Richtung auszurichten. - Gemäß einem weiteren Gegenstand betrifft die Erfindung eine Intervertebralimplantat-Zange, die zum Einsetzen bzw. Entfernen des Verankerungsstücks
40 in der Aussparung46 vorgesehen ist. Die Zange wird mit Bezug auf7 und8 beschrieben. - Die Implantat-Zange weist eine erste Backe
70 auf, die das Implantat aufzunehmen vermag, das aus dem ersten Teil10 gebildet ist, in dem der zweite Teil16 zwei gegenüberliegende Ränder miteinander verbindet, wobei das Verankerungsstück40 in den zweiten Teil16 eingesetzt wird. Die erste Backe70 weist eine in8 dargestellte mittige Aussparung72 auf, die das Hindurchführen des Verankerungsstücks40 erlaubt, wenn dieses durch die Aussparung46 des zweiten Teils16 durchgesteckt wird. Die Zange weist eine zweite Backe74 auf, die dazu bestimmt ist, an dem Verankerungsstück40 anzuliegen, um dieses einzuführen, wenn sich die Backen70 und74 einander annähern. Hierzu ist jede Backe70 ,74 durch einen Schenkel verlängert, wobei die Schenkel solchermaßen miteinander verbunden sind, dass sie bezüglich einander geschwenkt werden können. - Ist das Verankerungsstück in dem zweiten Teil
16 bereits vormontiert, kann dieses ferner mit Hilfe der Zange unter Aufwendung von Kraft einfach entfernt werden, wenn für den Eingriff das Einsetzen eines mit einem Verankerungsstück versehenen Implantats nicht erforderlich ist.
Claims (16)
- Intervertebrales Implantat, das geeignet ist, wenigstens einen Knochenspan zu enthalten und in einer vorbestimmten Richtung in den intervertebralen Zwischenraum eingeführt zu werden, welches umfasst: – einen ersten, allgemein ringförmigen Teil (
10 ), der eine Aussparung (11 ) umgibt und einen ersten oberen Rand (12 ) und einen ersten unteren Rand (14 ) aufweist, und – einen zweiten, entlang seiner Hauptachse (A) langgezogenen Teil (16 ), der einen zweiten unteren Rand (18 ) und einen zweiten oberen Rand (20 ) aufweist und in der vorbestimmten Richtung zwei gegenüberliegende Abschnitte des ersten Teils (10 ) miteinander verbindet und hierbei die Aussparung (11 ) in zwei Aufnahmen unterteilt, die den Knochenspan zu enthalten vermögen, dadurch gekennzeichnet, dass wenigstens ein Abschnitt des zweiten unteren und oberen Rands aus dem durch den ersten unteren (14 ) und oberen (12 ) Rand des ersten Teils (10 ) gebildeten Raum übersteht und eine gekrümmte Form hat, und dessen Enden den ersten unteren (14 ) bzw. oberen (12 ) Rand des ersten Teils (10 ) derart verbinden, dass die zweiten Ränder die bikonkave Form des intervertebralen Zwischenraums beachten und zusätzliche Auflagepunkte des Implantats an den Wirbelplatten in deren Mittelteil bilden. - Intervertebrales Implantat nach Anspruch 1, dadurch gekennzeichnet, dass der erste Teil (
10 ) und der zweite Teil (16 ) derart miteinander verbunden sind, dass sie ein einziges Stück bilden. - Intervertebrales Implantat nach einem der Ansprüche 1 oder 2, dadurch gekennzeichnet, dass der erste Teil (
10 ) entlang der Hauptachse (A) des zweiten Teils seitlich derart durchbohrt ist, dass ein Sackloch (22 ) ausgebildet ist, das sich in dem zweiten Teil (16 ) fortsetzt, wobei das Sackloch (22 ) ein Werkzeug zum Einführen aufzunehmen vermag. - Intervertebrales Implantat nach einem der Ansprüche 1 bis 3, dadurch gekennzeichnet, dass die unteren und oberen Ränder (
12 ,14 ,18 ,20 ) des ersten und des zweiten Teils Kerben aufweisen, die zum Bilden von Verankerungspunkten in den Wirbel geeignet sind. - Intervertebrales Implantat nach einem der Ansprüche 1 bis 4, dadurch gekennzeichnet, dass der erste, allgemein ringförmige Teil (
10 ) wenigstens eine Symmetrieachse (S) aufweist. - Intervertebrales Implantat nach Anspruch 5, dadurch gekennzeichnet, dass der zweite Teil (
16 ) die beiden gegenüberliegenden Abschnitte des ersten Teils (10 ) entlang der Symmetrieachse (S) verbindet. - Intervertebrales Implantat nach Anspruch 5, dadurch gekennzeichnet, dass der zweite Teil (
16 ) die beiden gegenüberliegenden Abschnitte des ersten Teils (10 ) in einer Richtung verbindet, die mit der Symmetrieachse (S) einen Winkel zwischen 0° und 90° bildet. - Intervertebrales Implantat nach einem der Ansprüche 5 bis 7, dadurch gekennzeichnet, dass der erste Teil (
10 ) im Wesentlichen die allgemeine Form eines Halbkreises aufweist. - Intervertebrales Implantat nach einem der Ansprüche 1 bis 8, dadurch gekennzeichnet, dass der erste Teil (
10 ) und der zweite Teil (16 ) aus einem makromolekularen Werkstoff einstückig geformt sind. - Intervertebrales Implantat nach einem der Ansprüche 1 bis 9, dadurch gekennzeichnet, dass der erste Teil (
10 ) und der zweite Teil (16 ) aus Polyetheretherketon sind. - Intervertebrales Implantat nach einem der Ansprüche 1 bis 10, dadurch gekennzeichnet, dass der zweite Teil ferner Verankerungsmittel umfasst, die jeweils in den zweiten oberen Rand und in den zweiten unteren Rand münden.
- Intervertebrales Implantat nach Anspruch 11, dadurch gekennzeichnet, dass die Verankerungsmittel aus einem Verankerungsstück mit zwei Enden gebildet sind, und dass der zweite Teil eine Aussparung aufweist, die derart in den zweiten unteren und oberen Rand mündet, dass das in die Aussparung eingeführte Verankerungsstück den zweiten Teil vollständig durchquert, wobei das erste Ende in den zweiten oberen Rand mündet und das zweite Ende in den zweiten unteren Rand des zweiten Teils mündet.
- Intervertebrales Implantat nach Anspruch 12, dadurch gekennzeichnet, dass das Verankerungsstück im Wesentlichen trapezförmig ist, wobei seine beiden Enden durch die beiden nicht aufeinanderfolgenden und zueinander nicht parallelen Seiten gebildet sind.
- Intervertebrales Implantat nach Anspruch 13, dadurch gekennzeichnet, dass das Verankerungsstück eine Mittelebene aufweist, und dass es in den zweiten Teil derart eingeführt ist, dass die Hauptachse (A) des zweiten Teils die beiden Grundlinien des Trapezes schneidet, und dass seine Mittelebene im Wesentlichen lotrecht zur Mittelebene des ersten Teils ist.
- Intervertebrales Implantat nach einem der Ansprüche 13 oder 14, dadurch gekennzeichnet, dass eine der Grundlinien des Trapezes mit den beiden anderen zueinander nicht parallelen Seiten zwei spitze Winkel bildet, und dass das Verankerungsstück in den zweiten Teil derart eingeführt ist, dass das Implantat von der größten Grundlinie aus in Richtung der kleinsten Grundlinie des Trapezes eingeführt wird.
- Intervertebrales Implantat nach einem der Ansprüche 12 bis 15, dadurch gekennzeichnet, dass das Verankerungsstück aus Titanlegierung ausgeführt ist.
