Description
- Gebiet der Erfindung
- Die vorliegende Erfindung betrifft allgemein Biogrenzflächenmembranen, die bei implantierbaren Geräten eingesetzt werden können, wie für die Bestimmung von Analytkonzentrationen in einer biologischen Probe, Zelltransplantationsgeräten, Arzneimittelzuführungsgeräten und elektrischen Signalgabe- oder Messgeräten. Die vorliegende Erfindung betrifft ferner Verfahren zur Bestimmung von Analytgehalten unter Benutzung implantierbarer Geräte mit diesen Membranen. Insbesondere betrifft die Erfindung neue Biogrenzflächenmembranen, Sensoren und implantierbare Geräte mit diesen Membranen sowie Verfahren zur Überwachung von Glucosegehalten in einer biologischen Flüssigkeitsprobe unter Benutzung eines implantierbaren Analytbestimmungsgeräts.
- Hintergrund der Erfindung
- Eins der am stärksten untersuchten Analyt-Messfühlergeräte ist ein implantierbarer Glucosefühler zur Feststellung von Glucosegehalten bei Patienten mit Diabetes. Trotz der wachsenden Anzahl von Personen mit diagnostizierter Diabetes und jüngsten Fortschritten auf dem Gebiet implantierbarer Glucose-Überwachungsgeräte sind die gegenwärtig benutzten Geräte nicht in der Lage, sicher und über längere Zeitspannen (z. B. Monate oder Jahre) zuverlässig Daten zu liefern (siehe z. B. Moatti-Sirat et al., Diabetologia 35:224-30 (1992)). Es gibt zwei allgemein benutzte Arten implantierbarer Glucose-Messfühlergeräte. Diese Arten sind jene, die intravaskulär implantiert werden, und jene, die in Gewebe implantiert sind.
- Bezüglich der Geräte, die im Gewebe implantiert werden können, war ein Nachteil dieser Geräte, dass sie dazu neigen, nach den ersten paar Tagen bis Wochen nach der Implantierung ihre Funktion zu verlieren. Wenigstens ein Grund dieses Funktionsverlustes wurde der Tatsache zugeschrieben, dass kein direkter Kontakt mit dem zirkulierenden Blut besteht, um eine Probe an die Spitze der Sonde des implantierten Geräts zu liefern. Wegen dieser Einschränkungen war es bisher schwierig, kontinuierliche und genaue Glucosewerte zu erhalten. Diese Information ist jedoch für Diabetespatienten äußerst wichtig, um sich zu vergewissern, ob zur richtigen Beherrschung ihrer Krankheit eine unmittelbare Korrektur nötig ist.
- Einige medizinische Geräte, darunter implantierte Analyt-Fühler, Arzneimittel-Zuführungsgeräte und Zelltransplantationsgeräte erfordern zur richtigen Funktion den Transport von Gelöstem durch die Gerät-Gewebe-Grenzfläche. Diese Geräte haben im Allgemeinen eine Membran, die hier als eine zellundurchlässige Membran bezeichnet wird, die das Gerät oder ein Teil des Geräts umhüllt, um den Zutritt entzündlicher oder immuner Wirtszellen zu empfindlichen Bereichen des Geräts zu verhindern.
- Ein Nachteil zellundurchlässiger Membranen ist der, dass sie oft eine lokale entzündliche Reaktion erregen, Fremdkörperreaktion (FBR) genannt, die lange als Einschränkung der Funktion implantierter Geräte angesehen wurde, die Transport des Gelösten erfordern. Frühere Anstrengungen, dieses Problem zu lösen, waren mit begrenztem Erfolg darauf ausgerichtet, die lokale Gefäßbildung an der Grenzfläche Gerät/Gewebe zu verstärken.
- Die FBR wurde in der Literatur eingehend beschrieben und besteht aus drei Hauptschichten, wie in
1 dargestellt ist. Die innerste, dem Gerät anliegende FBR-Schicht40 besteht allgemein aus Makrophagen und Fremdkörper-Riesenzellen41 (hier als die Sperrzellschicht bezeichnet). Diese Zellen bilden eine Monoschicht40 dicht gegenüber liegender Zellen über der gesamten Oberfläche48a einer glatten oder mikroporösen (< 1,0 μm) Membran48 . Die mittlere FBR-Schicht42 (hier als faserige Zone bezeichnet), die bezüglich des Geräts distal zu der ersten Schicht liegt, ist eine breite Zone (30–100 Mikron), die hauptsächlich aus Fibroblasten43 und faserartiger Matrix44 besteht. Die äußerste FBR-Schicht46 ist loses, körniges Bindegewebe, das neue Blutgefäße45 enthält, (hier als die vaskuläre Zone46 bezeichnet). Ein übereinstimmendes Merkmal der innersten Schichten40 und42 ist, dass sie frei von Blutgefäßen sind. Dies hat zu einer breit getragenen Vermutung geführt, dass ein schlechter Molekültransport durch die Gerät-Gewebe-Grenzfläche47 auf ein Fehlen der Gefäßbildung nahe der Grenzfläche47 zurückzuführen ist (Scharp et al., World J. Surg. 8:221-229 (1984); Colton und Avgoustiniatos J. Biomech. Eng. 113:152-170 (1991)). - Patente von Brauker et al. (US-Patent Nr. 5,741,330) und Butler et al. (US-Patent Nr. 5,913,998) beschreiben Erfindungen, die auf die Vergrößerung der Anzahl der Blutgefäße an der Implantatmembran (Brauker et al.) und das Einwachsen in der Implantatmembran an der Grenzfläche Gerät/Gewebe (Butler et al.) gerichtet sind. Das Patent von Shults et al. (US-Patent Nr. 6,001,067) beschreibt Membranen, die an der Grenzfläche Gerät/Gewebe der implantierten Glucosefühler eine Angiogenese einleiten.
2 stellt eine Situation dar, in der einige Blutgefäße45 dicht an eine Implantatmembran48 gebracht sind, aber die Primärschicht40 der Zellen an der zellundurchlässigen Membran Glucose blockiert. Diese Erscheinung wird in weiteren Einzelheiten unten beschrieben. - In den Beispielen von Brauker et al. (Supra) und Shults et al. ist beschrieben, dass Doppelschichtmembranen zellundurchlässige Schichten haben, die porös sind und an Zellen haften. Zellen können in die Zwischenräume dieser Membranen eintreten und Monoschichten auf der innersten Schicht bilden, was darauf abzielt, einen Zellzugang zu dem Inneren des implantierten Geräts (zellundurchdringliche Schichten) zu verhindern. Da die zelldurchdringlichen Schichten porös sind, können Zellen mit Pseudopodien in die Zwischenräume der Membran reichen, um an der Membran zu haften und sich auf ihr abzuflachen, wie in den
1 und2 gezeigt ist, wodurch der Molekültransport durch die Grenzfläche Membran/Gewebe blockiert wird. Die bekannte Technik bezweckt eine Verstärkung der lokalen Gefäßneubildung, um die Verfügbarkeit des Gelösten zu erhöhen. Die vorliegenden Studien zeigen jedoch, dass, wenn sich einmal die Monoschicht der Zellen (Sperrzellschicht) an der Membran gebildet hat, eine zunehmende Angiogenese nicht ausreicht, um den Transport von Molekülen, wie Glucose und Sauerstoff, durch die Grenzfläche Gerät/Gewebe zu steigern. - Ein Mechanismus der Hemmung des Transport von Gelöstem durch die Grenzfläche Gerät/Gewebe, der in der Literatur bisher nicht diskutiert wurde, ist die Bildung einer gleichmäßigen Sperre für den Analyttransport durch Zellen, die die innerste Schicht der Fremdkörperkapsel bilden. Diese Zellschicht bildet eine Monoschicht mit eng gegenüber liegenden Zellen mit dichten Verbindungen von Zelle zu Zelle. Wenn sich diese Sperrzellschicht bildet, wird sie durch verstärkte lokale Gefäßbildung im Wesentlichen nicht überwunden. Ungeachtet der Stärke der lokalen Gefäßbildung erhindert die Sperrzellschicht den Durchtritt von Molekülen, die nicht durch die Schicht diffundieren können. Dies ist wiederum in
2 dargestellt, wo Blutgefäße45 an der Membran liegen, aber der Glucosetransport ist wegen der Undurchdringlichkeit der Sperrzellschicht40 signifikant verringert. Glucose und ihre phosphorilierte Form gehen z. B. nicht leicht durch die Zellmembran, und demzufolge erreicht wenig Glucose die Implantatmembran durch die Sperrschichtzellen. - Durch die beteiligten Erfinder wurde durch histologische Prüfung explantierter Fühler bestätigt, dass der wahrscheinlichste Mechanismus für die Hemmung des molekularen Transports durch die Grenzfläche Gerät/Gewebe die Sperrzellschicht an der Membran ist. Es besteht eine starke Korrelation zwischen der gewünschten Gerätefunktion und dem Fehlen der Bildung einer Sperrzellschicht an der Grenzfläche Gerät/Gewebe. Bei den vorliegenden Untersuchungen waren Geräte, die nach histologischer Beobachtung wesentliche Sperrzellschichten haben, nach zwölf Wochen in vivo nur 41% der Zeit funktionsfähig. Dagegen waren Geräte, die keine signifikanten Sperrzellschichten hatten, nach zwölf Wochen in vivo 86% der Zeit funktionsfähig.
- Demgemäss besteht ein Bedarf an einer Membran, die die Bildung einer Sperrschicht stört und die empfindlichen Bereiche des Geräts vor entzündlicher Reaktion des Wirts schützt.
- Summarischer Abriss der Erfindung
- Die Biogrenzflächenmembranen der vorliegenden Erfindung behindern die Bildung einer hier nachfolgend als Sperrzellschicht bezeichneten Monoschicht von Zellen an der Membran, die den Transport von Sauerstoff und Glucose durch die Grenzfläche Gerät/Gewebe behindert.
- Es ist zu bemerken, dass verschiedene Biogrenzflächenmembranstrukturen (z. B. Änderungen der unten beschriebenen) nach der vorliegenden Erfindung beabsichtigt sind und innerhalb ihres Schutzumfangs liegen.
- Nach einem Aspekt der vorliegenden Erfindung ist eine Biogrenzflächenmembran für die Verwendung bei einem implantierbaren Gerät vorgesehen, mit einem zu dem implantierbaren Gerät distalen ersten Bereich, der das Einwachsen von Gewebe unterstützt und die Bildung einer Sperrzellschicht behindert, und einem zu dem implantierbaren Gerät proximalen zweiten Bereich, der gegen Zellanlagerung widerstandsfähig und für Zellen und Zellfortsätze undurchlässig ist.
- Bei einem anderen Aspekt der vorliegenden Erfindung wird eine Biogrenzflächenmembran geschaffen mit den
- Eigenschaften der Förderung des Gewebeeinwachsens in, der Behinderung der Sperrzellbildung auf oder in, des Widerstandes der Sperrzellenanlagerung an und der Blockierung der Zelleindringung in die Membran.
- Die Biogrenzflächenmembranen der vorliegenden Erfindung ermöglichen den langfristigen Schutz implantierter Zellen oder Arzneimittel sowie über lange Zeiträume eine kontinuierliche Information über z. B. Glucosegehalte eines Wirts. Aufgrund dieser Eigenschaften können die Biogrenzflächenmembranen der vorliegenden Erfindung äußerst wichtig sein bei der Behandlung von Transplantatpatienten, diabetischen Patienten und Patienten, die einer häufigen Arzneimittelbehandlung bedürfen.
- Definitionen
- Um das Verständnis der vorliegenden Erfindung zu erleichtern, wird eine Anzahl von Bezeichnungen nachfolgend definiert.
- Die Bezeichnung „Biogrenzflächenmembran" ist eine umfassende Bezeichnung und bezeichnet hier in ihrer normalen Bedeutung ohne Einschränkung eine durchlässige Membran, die als eine Gerät/Gewebe-Grenzfläche fungiert, die zwei oder mehr Bereiche aufweist. Vorzugsweise besteht die Biogrenzflächenmembran aus zwei Bereichen. Der erste Bereich unterstützt das Einwachsen von Gewebe, behindert die Bildung einer Sperrzellschicht und hat eine offenzellige Konfiguration mit Hohlräumen und einem kompakten Anteil. Der zweite Bereich ist gegen Zellanlagerung widerstandsfähig und für Zellen (z. B. Makrophagen) undurchlässig. Die Biogrenzflächenmembran besteht aus biobeständigen Werkstoffen und kann in Schichten, mit gleichmäßigen oder ungleichmäßigen Gradienten (d. h. anisotrop) oder in einer Konfiguration mit gleichmäßiger oder ungleichmäßiger Hohlraumgröße aufgebaut sein.
- Die Bezeichnung „Bereich" ist eine umfassende Bezeichnung und wird hier in ihrer gewöhnlichen Bedeutung ohne Einschränkung benutzt für Bereiche der Biogrenzflächenmembran, die Schichten, gleichmäßige oder ungleichmäßige Gradienten (z. B. anisotrop) oder als Teile der Membran vorgesehen sein können.
- Die Bezeichnung „Sperrzellschicht" ist eine umfassende Bezeichnung und wird hier in ihrer gewöhnlichen Bedeutung ohne Einschränkung für eine kohäsive Monoschicht eng gegenüber liegender Zellen (z. B. Makrophagen und Fremdkörper-Riesenzellen) benutzt, die an implantierte Membranen angelagert sein können und den Molekültransport durch die Membran behindern können.
- Die Bezeichnung „distal zu" ist eine umfassende Bezeichnung und dient hier in ihrer üblichen Bedeutung ohne Einschränkung zur Bezeichnung der räumlichen Beziehung zwischen verschiedenen Elementen im Vergleich zu einem besonderen Bezugspunkt. Einige Ausführungsformen eines Geräts haben z. B. eine Biogrenzflächenmembran mit einem Zellzerreißbereich und einem zellundurchlässigen Bereich. Wenn der Fühler als der Bezugspunkt angenommen wird und der Zellzerreißbereich weiter von dem Fühler entfernt ist, dann ist dieser Bereich distal zu dem Fühler.
- Die Bezeichnung „proximal zu" ist eine umfassende Bezeichnung und dient hier in ihrer üblichen Bedeutung ohne Einschränkung zur Bezeichnung der räumlichen Beziehung zwischen verschiedenen Elementen im Vergleich zu einem besonderen Bezugspunkt. Einige Ausführungsformen eines Geräts haben z. B. eine Biogrenzflächenmembran mit einem Zellzerreißbereich und einem zellundurchlässigen Bereich. Wenn der Fühler als der Bezugspunkt angenommen wird und der zellundurchlässige Bereich näher an dem Fühler angeordnet ist, dann ist dieser Bereich proximal zu dem Fühler.
- Die Bezeichnung „Zellfortsätze" ist eine umfassende Bezeichnung und wird hier in ihrer gewöhnlichen Bedeutung ohne Einschränkung für Pseudopodien einer Zelle benutzt.
- Die Bezeichnung „kompakte Anteile" ist eine umfassende Bezeichnung und wird hier in ihrer gewöhnlichen Bedeutung ohne Einschränkung für Material mit einer Struktur benutzt, das eine offenzellige Konfiguration haben kann oder auch nicht, aber in jedem Fall Ganzzellen daran hindert, durch das Material zu wandern oder sich in dem Material zu befinden.
- Die Bezeichnung „wesentliche Anzahl" ist eine umfassende Bezeichnung und wird hier in ihrer gewöhnlichen Bedeutung ohne Einschränkung für die Anzahl linearer Dimensionen in einem Bereich (z. B. Poren oder kompakte Anteile) benutzt, in dem mehr als 50% aller Dimensionen die angegebene Größe haben, vorzugsweise mehr als 75% und insbesondere mehr als 90% der Dimensionen die angegebene Größe haben.
- Die Bezeichnung „ko-kontinuierlich" ist eine umfassende Bezeichnung und wird hier in ihrer gewöhnlichen Bedeutung ohne Einschränkung für einen kompakten Anteil benutzt, wobei zwischen zwei beliebigen Punkten des kompakten Anteils eine ununterbrochene gekrümmte Linie in drei Dimensionen existiert.
- Die Bezeichnung „biostabil" ist eine umfassende Bezeichnung und dient hier in ihrer gewöhnlichen Bedeutung ohne Einschränkung für Materialien, die gegen den Abbau durch Prozesse, die in vivo auftreten, relativ beständig sind.
- Die Bezeichnung „Fühler" ist eine umfassende Bezeichnung und wird hier in ihrer gewöhnlichen Bedeutung ohne Einschränkung für den Bestandteil oder den Bereich eines Geräts benutzt, durch das ein Analyt quantitativ bestimmt werden kann.
- Die Bezeichnung „Analyt" ist eine umfassende Bezeichnung und wird hier in ihrer gewöhnlichen Bedeutung ohne Einschränkung für eine Substanz oder einen chemischen Bestandteil in einer biologischen Flüssigkeit (z. B. Blut oder Urin) benutzt, der analysiert werden soll. Ein bevorzugter Analyt für die Messung durch Analytbestimmungsgeräte mit Biogrenzflächenmembranen der vorliegenden Erfindung ist Glucose.
- Die Bezeichnungen „betriebsmäßig verbunden" und „betriebsmäßig verkettet" werden hier als umfassende Bezeichnungen benutzt und bedeuten in ihrem üblichen Sinn ohne Einschränkung, dass ein oder mehrere Bestandteile mit einem oder mehreren anderen Bestandteil(en) in einer Weise verkettet sind, die die Übertragung von Signalen zwischen den Bestandteilen erlaubt. Ein oder mehrere Elektroden können z. B. benutzt werden, um die Analytmenge in einer Probe festzustellen und diese Information in ein Signal umzuwandeln; das Signal kann dann zu einer elektronischen Schaltungseinrichtung übertragen werden. In diesem Fall ist die Elektrode mit der elektronischen Schaltung „betriebsmäßig verkettet".
- Die Bezeichnung „elektronische Schaltung" wird hier in der umfassenden Bedeutung benutzt und dient gewöhnlich ohne Einschränkung für Bestandteile eines Geräts, das zur Verarbeitung einer von einem Wirt erhaltenen biologischen Information erforderlich ist. Bei einem Analyt-Messgerät erhält man die biologische Information durch einen Fühler für einen besonderen Analyten in einer biologischen Flüssigkeit, wodurch Daten über die Menge jenes Analyten in der Flüssigkeit geliefert werden. US-Patente Nr. 4,757,022, 5,497,772 und 4,787,398 beschreiben geeignete elektronische Schaltungen, die bei Geräten mit der Biogrenzflächenmembran der vorliegenden Erfindung eingesetzt werden können.
- Der Satz „Element zur Bestimmung der Glucosemenge in einer biologischen Probe" wird hier als umfassender Ausdruck benutzt und bezeichnet in seinem üblichen Sinne ohne Einschränkung irgendeinen Mechanismus (z. B. enzymatisch oder nicht-enzymatisch), durch den Glucose quantitativ bestimmt werden kann. Einige Ausführungsformen der vorliegenden Erfindung benutzen z. B. eine Membran, die Glucose-Oxidase enthält, die die Umsetzung von Sauerstoff und Glucose zu Wasserstoffperoxid und Gluconat katalysiert: Glucose + O2 = Gluconat + H2O2. Da für jedes verstoffwechselte Glucosemolekül eine proportionale Änderung bei dem Mitreaktanten O2 und dem Produkt H2O2 eintritt, kann man die laufende Änderung entweder an dem Mitreaktanten oder dem Produkt überwachen, um die Glucosekonzentration zu bestimmen.
- Die Bezeichnung „Wirt" ist hier eine umfassende Bezeichnung und dient hier in ihrer gewöhnlichen Bedeutung ohne Einschränkung zur Bezeichnung von Säugern, insbesondere Menschen.
- Die hier benutzte Bezeichnung „genau" ist eine umfassende Bezeichnung und bedeutet gewöhnlich ohne Einschränkung, dass z. B. 90% der gemessenen Glucosewerte in dem „A"- und „B"-Bereich eines Clarke-Standard-Fehlergitters sind, wenn die Messungen des Fühlers mit einer Standard-Bezugsmessung verglichen werden. Es ist zu bemerken, dass wie bei jedem analytischen Gerät Eichung, Eichungsüberprüfung und Neueichung für den genausten Betrieb des Geräts erforderlich sind.
- Der Satz „kontinuierliche Glucose-Erfassung" wird hier in seiner umfassenden Bedeutung benutzt und bezeichnet gewöhnlich ohne Einschränkung die Periode, in der die Überwachung der Plasma-Glucose-Konzentration kontinuierlich durchgeführt wird, z. B. etwa alle 10 Minuten.

- Die vorliegende Erfindung betrifft allgemein neue Biogrenzflächenmembranen. Insbesondere schafft die Erfindung Biogrenzflächenmembranen, die bei implantierbaren Geräten benutzt werden können, die die Überwachung und Bestimmung von Glucosegehalten in einer biologischen Flüssigkeit erlauben, eine besonders wichtige Messung für Personen mit Diabetes.
- Obgleich die folgende Beschreibung hauptsächlich auf Glucoseüberwachungsgeräte mit Biogrenzflächenmembranen der vorliegenden Erfindung und Methoden für ihre Anwendung gerichtet ist, sind diese Biogrenzflächenmembranen nicht auf die Verwendung in Geräten beschränkt, die Glucose messen oder überwachen. Diese Biogrenzflächenmembranen können vielmehr bei verschiedenen Geräten Anwendung finden, darunter z. B. jenen, die andere in biologischen Flüssigkeiten anwesende Analyte feststellen und quantitativ bestimmen (einschließlich, jedoch nicht beschränkt auf Cholesterin, Aminosäuren und Lactat), insbesondere jene Analyte, die Substrate für Oxidaseenzyme sind [siehe z. B. US-Patent Nr.4,703,756 von Gough et al., das durch Bezugnahme hierdurch aufgenommen wird], Zelltransplantationsgeräte (US-Patent Nr. 6,015,572, 5,964,745 und 6,083,523), Arzneimittel-Zuführungsgeräte (US-Patent Nr. 5,458,631, 5,820,589 und 5,972,369) und elektrische Zuführungs- und/oder Messgeräte, wie implantierbare Impulserzeugungsherzschrittmacher (US-Patent Nr. 6,157,860, 5,782,880 und 5,207,218), EKG-Geräte (US-Patent Nr. 4,625,730 und 5,987,352) sowie elektrische Nervenstimulatoren (US-Patent Nr. 6,175,767, 6,055,456 und 4,940,065).
- Implantierbare Geräte zur Feststellung von Analytkonzentrationen in einem biologischen System können die Biogrenzflächenmembranen der vorliegenden Erfindung benutzen, um die Bildung einer Sperrzellschicht zu behindern und dadurch sicherzustellen, dass der Fühler Analytkonzentrationen empfängt, die für die in dem Gefäßsystem repräsentativ sind. Arzneimittel-Zuführungsgeräte können die Biogrenzflächenmembranen der vorliegenden Erfindung benutzen, um das in dem Gerät untergebrachte Arzneimittel vor Entzündungszellen oder Immunzellen des Wirts zu schützen, die möglicherweise das Arzneimittel schädigen oder zerstören könnten. Ferner verhindert die Biogrenzflächenmembran die Bildung einer Sperrzellschicht, die die richtige Abgabe des Arzneimittels aus dem Gerät für die Behandlung des Patienten beeinträchtigen könnte. Entsprechend könnten Zelltransplantationsgeräte die Biogrenzflächenmembranen der vorliegenden Erfindung benutzen, um die transplantierten Zellen gegen Angriff durch Entzündungs- oder Immunreaktionszellen des Wirts zu schützen, während gleichzeitig Nährstoffe sowie andere zum Überleben von den Zellen benötigte biologisch aktive Moleküle durch die Membran diffundieren können.
- Die zur Verwendung bei der Herstellung der Biogrenzflächenmembran vorgesehenen Materialien beseitigen auch den Bioabbau oder verzögern ihn signifikant. Dies ist besonders wichtig für Geräte, die Analytkonzentrationen kontinuierlich messen. Bei einem Glucose-Messgerät sind z. B. die Eletrodenoberflächen, des Glucosefühlers in Kontakt mit (oder betriebsmäßig verbunden mit) einer dünnen Elektrolytphase, die ihrerseits von einer Membran bedeckt ist, die ein Enzym, z. B. Glucoseoxidase, und ein Polymersystem enthält. Die Biogrenzflächenmembran bedeckt diese Enzymmembran und dient z. T. dazu, den Fühler vor äußeren Kräften und Faktoren zu schützen, die zum Bioabbau führen können. Durch signifikante Verzögerung des Bioabbaus an dem Fühler können über lange Zeiträume (z. B. Monate bis Jahre) genaue Daten gesammelt werden. Dementsprechend könnte der Bioabbau an der Biogrenzflächenmembran von implantierbaren Zelltransplantationsgeräten und Arzneimittel-Zuführungsgeräten Entzündungs- und Immunzellen des Wirts erlauben, in diese Geräte zu gelangen, und dadurch eine langfristige Funktion gefährden.
- Geräte und Sonden, die in subkutanes Gewebe implantiert werden, werden fast immer eine Fremdkörperumhüllung (FBC) als Teil der Reaktion des Körpers auf die Einführung von Fremdmaterial auslösen. Daher führt die Implantation eines Glucosefühlers zu einer akuten Entzündungsreaktion mit nachfolgendem Aufbau von faserigem Gewebe. Schließlich bildet sich um das Gerät eine reife FBC, die hauptsächlich ein vaskuläres Fasergewebe enthält (Shanker und Greisler, Inflammation and Biomaterials in Greco RS, Herausgeber, Implantation Biology: The Host Response und Biomedical Devices, S. 68–80, CRC Press (1994)).
- Im Allgemeinen hat die Bildung einer FBC die Sammlung zuverlässiger, fortlaufender Informationen ausgeschlossen, weil man bisher glaubte, den Fühler des implantierten Geräts in einer Umhüllung zu isolieren, die Flüssigkeit enthält, welche die Gehalte von Analyten (z. B. Glucose und Sauerstoff) in den Gefäßen des Körpers nicht nachbildete. Die Zusammensetzung einer FBC hat auch die Stabilisierung des implantierten Geräts verhindert und trägt zur Bewegungsstruktur bei, die ebenfalls unzuverlässige Resultate ergibt. Somit war es bisher Praxis der Fachleute, eine Minimierung der FBC-Bildung z. B. dadurch zu versuchen, dass eine kurzlebige Nadelgeometrie oder Fühlerbeschichtungen benutzt wurden, um die Fremdkörperreaktion zu minimieren.
- Im Gegensatz zu der herkömmlicherweise bekannten Praxis hat die Lehre der vorliegenden Erfindung erkannt, dass die FBC-Bildung der dominante Vorgang in der Umgebung einer langfristigen Implantation irgendeines Fühlers ist und so geleitet werden muss, dass die Leistungsfähigkeit des Fühlers unterstützt anstatt behindert oder blockiert wird. Es wurde beobachtet, dass während der frühen Perioden nach Implantierung eines Analyt-Fühlergeräts, insbesondere eines Glucosefühlers, die Glucosefühler gut funktionieren. Nach ein paar Tagen oder zwei oder mehr Wochen der Implantierung verlieren diese Geräte jedoch ihre Funktion. Z. B. haben US-Patent Nr. 5,791,344 und Gross et al. Performance Evaluation of the Minimed Continous Monitoring System During Patient home Use", Diabetes Technology and Therapeutics, Band 2 Nummer 1, S. 49–56, 2000, über einen Glucoseoxidase-Fühler berichtet (der zur Verwendung bei Menschen durch die Food and Drug Administration zugelassen worden war), der nach der Implantierung mehrere Tage gut funktionierte, aber nach 3 Tagen schnell die Funktion verlor. Wir haben bei unserem implantierbaren Fühler ein ähnliches Geräteverhalten beobachtet. Diese Ergebnisse lassen vermuten, dass eine ausreichende Gefäßbildung und daher Strömung von Sauerstoff und Glucose vorhanden ist, um die Funktion eines implantierten Glucosefühlers für die ersten paar Tage nach der Implantierung zu unterstützen. Die Bildung neuer Blutgefäße ist wenigstens mehrere Tage nach Implantierung für die Funktion eines in das subkutane Gewebe implantierten durch Glucoseoxidase vermittelten, elektrochemischen Fühlers eindeutig nicht nötig.
- Wir haben beobachtet, dass dieser Mangel der Fühlerfunktion nach mehreren Tagen höchstwahrscheinlich auf Zellen zurückzuführen ist, wie etwa polymorphkernige Zellen und Monocyten, die in den ersten paar Tagen nach Implantierung zu der Wundstelle wandern. Diese Zellen verbrauchen Glucose und Sauerstoff. Wenn diese Zellen in großem Überfluß vorhanden sind, können sie zu einer Verarmung an Glucose und/oder Sauerstoff führen, bevor diese die Fühlerenzymschicht erreichen können, wodurch die Empfindlichkeit des Geräts verringert oder es funktionsunfähig gemacht wird. Nach den ersten paar Tagen kann eine weitere Behinderung der Gerätefunktion auf Zellen zurückzuführen sein, die sich mit der Membran des Geräts verbinden und den Glucosetransport in das Gerät physikalisch blockieren (das sind Sperrzellen). Die Überwindung der Sperrzellblockierung würde man durch verstärkte Gefäßbildung nicht erwarten. Die vorliegende Erfindung sieht die Verwendung besonderer Biogrenzflächenmembranstrukturen vor, die die Bildung der Sperrzellschicht auf der Membranoberfläche stört. Diese Membranen können auch bei verschiedenen implantierbaren Geräten verwendet werden, (z. B. Analyt-Messgeräten, insbesondere Glucosemessgeräten, Zellimplantierungsgeräten, Arzneimittel-Zuführungsgeräten und elektrischen Signalabgabe- und Messgeräten).
- Der Fühlergrenzflächenbereich bezieht sich auf den Bereich eines Überwachungsgeräts, das für die Erfassung eines bestimmten Analyten verantwortlich ist. Z. B. bezieht sich die Fühlergrenzfläche bei einigen Ausführungsformen eines Glucoseüberwachungsgeräts auf den Bereich, wo eine biologische Probe (direkt oder nach Durchgang durch eine oder mehrere Membranen oder Schichten) mit einem Enzym (z. B. Glucoseoxidase) Kontakt hat. Der Fühlergrenzflächenbereich kann eine erfindungsgemäße Biogrenzflächenmembran mit unterschiedlichen Bereichen und/oder Schichten haben, die eine darunter liegende Enzymmembran und die Elektroden eines implantierbaren Analyt-Messgeräts bedecken und schützen können. Im Allgemeinen verhindern die erfindungsgemäßen Biogrenzflächenmembranen einen direkten Kontakt der biologischen Flüssigkeitsprobe mit dem Fühler. Die Membranen erlauben nur ausgewählten Substanzen (z. B. Analyten) der Flüssigkeit, zur Reaktion in dem immobilisierten Enzymbereich durch sie hindurchzutreten. Die Biogrenzflächenmembranen der vorliegenden Erfindung sind biostabil und verhindern die Sperrzellbildung. Die Eigenschaften dieser Biogrenzflächenmembran werden nun in weiteren Einzelheiten diskutiert.
- I. Biogrenzflächenmembran
- Die Biogrenzflächenmembran ist aus zwei oder mehr Bereichen aufgebaut. Die Membran hat unter Bezugnahme auf
3 vorzugsweise einen zu einem implantierbaren Gerät proximalen, zellundurchlässigen Bereich50 , der auch als der zweite Bereich bezeichnet wird, und einen zu einem implantierbaren Gerät distalen Zellzerreißbereich, der bei der dargestellten Ausführungsform Vliesfasern49 aufweist und auch als der erste Bereich bezeichnet wird. - A. (Erster) Sperrzell-Zerreißbereich
- Wie oben beschrieben hat der äußerste Bereich der erfindungsgemäßen Membran ein Material, das das Einwachsen von Gewebe unterstützt. Der Sperrzell-Zerreißbereich kann aus einer offenzelligen Konfiguration mit Hohlräumen und kompakten Anteilen bestehen.
4 ist z. B. eine Darstellung eines dreidimensionalen Schnitts60 eines Sperrzell-Zerreißbereichs mit kompakten Anteilen62 und Hohlräumen64 . Die Zellen können in die Hohlräume eintreten. Sie können jedoch nicht hindurchwandern oder gänzlich innerhalb der kompakten Anteile existieren. Die meisten Substanzen einschließlich z. B. Macrophagen, können durch die Hohlräume hindurchtreten. - Der offenzellige Aufbau liefert einen kokontinuierlichen kompakten Bereich, der im wesentlichen in der Gesamtheit der Membran mehr als einen Hohlraum in drei Dimensionen enthält. Ferner können die Hohlräume und Hohlraumverbindungen in Schichten mit unterschiedlichen Hohlraumdimensionen ausgebildet sein.
- Um die Dimensionen der Hohlräume und der kompakten Anteile besser zu beschreiben, kann eine zweidimensionale Ebene
66 benutzt werden (5 ), die den Sperrzell-Zerreißbereich schneidet. Eine Dimension durch einen Hohlraum64 oder einen kompakten Anteil62 lässt sich als lineare Linie beschreiben. Die Länge der linearen Linie ist der Abstand zwischen zwei Punkten, die an der Grenzfläche des Hohlraums und des kompakten Anteils liegen. So ist eine wesentliche Anzahl der Hohlräume in der kürzesten Dimension nicht kleiner als 20 Mikron und in der längsten Dimension nicht größer als 1000 Mikron. Vorzugsweise ist eine wesentliche Anzahl der Hohlräume in der kürzesten Dimension nicht kleiner als 25 Mikron und in der längsten Dimension nicht größer als 500 Mikron. - Eine wesentliche Anzahl der kürzesten Dimensionen des kompakten Anteils ist ferner nicht kleiner als 5 Mikron, und eine wesentliche Anzahl der längsten Dimensionen ist nicht größer als 2000 Mikron. Vorzugsweise ist eine wesentliche Anzahl der kürzesten Dimensionen des kompakten Anteils nicht kleiner als 10 Mikron und eine wesentliche Anzahl der längsten Dimensionen nicht größer als 1000 Mikron. Insbesondere ist eine wesentliche Anzahl der kürzesten Dimensionen nicht kleiner als 10 Mikron und eine wesentliche Anzahl der längsten Dimensionen nicht größer als 400 Mikron.
- Der kompakte Anteil kann aus Polytetrafluorethylen oder Polyethylen-Co-tetrafluorethylen bestehen. Vorzugsweise enthält der kompakte Anteil Polyurethane oder Blockcopolymere und insbesondere besteht er aus Silikon.
- Der kompakte Anteil besteht bei gewünschten Ausführungsformen aus porösem Silikon oder Vliesfasern. Vliesfasern werden vorzugsweise aus Polyester oder Polypropylen hergestellt. Beispielsweise zeigt
3 , wie Vliesfasern49 dazu dienen, die Kontinuität der Zellen zu zerreißen, so dass sie keine klassische Fremdkörperreaktion bilden können. All die Zellarten, die bei der FBR-Bildung beteiligt sind, können anwesend sein. Sie können jedoch nicht wie bei einer typischen Fremdkörperreaktion auf eine glatte Oberfläche eine geordnete, sich eng gegenüberliegende, zelluläre Monoschicht parallel zu der Oberfläche des Gewebes bilden. In diesem Beispiel schafft die 10-Mikron-Dimension eine geeignete Oberfläche für Fremdkörper-Riesenzellen, aber die Fasern sind so genähert, dass sie das Einwachsen von Blutgefäßen45 ermöglichen und fördern und in dem Biogrenzflächenbereich (3 ) Gefäße bilden. Geräte mit kleineren Fasern wurden bei früheren Erfindungen benutzt, aber diese Membranen neigen infolge der von den Zellen in den Zwischenräumen der Membranen ausgeübten Kräfte zur Abblätterung. Nach Abblätterung können Zellen Sperrschichten auf der glatten oder mikroporösen Oberfläche der Bioschutzschicht bilden, wenn sie für Zellen haftfähig ist oder Poren genügender Größe zum physikalischen Eindringen von Zellfortsätzen, aber nicht ganzer Zellen hat. - Wenn Vliesfasern als der kompakte Anteil der vorliegenden Erfindung dienen, können sie in der kürzesten Dimension größer als 5 Mikron sein. Vorzugsweise sind die Vliesfasern in der kürzesten Dimension etwa 10 Mikron und insbesondere sind sie in der kürzesten Dimension größer als oder gleich 10 Mikron.
- Die Vliesfasern können aus Polypropylen (PP), Polyvinylchlorid (PVC), Polyvinylidenfluorid (PVDF), Polybutylenterephthalat (PBT), Polymethylmethacrylat (PMMA), Polyetherketon (PEEK), Polyurethanen, cellulosischen Polymeren, Polysulfonen und ihren Blockcopolymeren, z. B. Diblock-, Triblock-Copolymeren, alternierenden, zufälligen und Pfropf-Copolymeren aufgebaut sein (Block-Copolymere werden in den US-Patenten Nr. 4,803,243 und 4,686,044 diskutiert, die hierdurch durch Bezugnahme eingefügt werden). Die Vliesfasern bestehen vorzugsweise aus Polyolefinen oder Polyestern oder Polycarbonaten oder Polytetrafluorethylen. Die Dicke des Zellzerreißbereichs ist nicht kleiner als etwa 20 Mikron und nicht größer als etwa 2000 Mikron.
- B. (Zweiter) zellundurahlässiger Bereich
- Die entzündliche Reaktion, die eine FBC auslöst und unterhält, ist mit Nachteilen bei der praktischen Erfassung von Analyten verbunden. Die Entzündung geht einher mit dem Eindringen von Entzündungsreaktionszellen (z. B. Macrophagen), die die Fähigkeit haben, über die die Grenzfläche bildenden Sperrzellschichten zu wachsen, was den Transport durch die Biogrenzflächenmembran blockieren kann. Diese Entzündungszellen können auch viele künstliche Bio materialien (von denen einige bis vor kurzem als nicht bioabbaubar angesehen wurden) bioabbauen. Wenn sie durch einen Fremdkörper aktiviert werden, degranulieren Gewebemacrophagen, wobei aus ihrem cytoplasmatischen Myeloperoxidasesystem Hypochlorit (Bleichen) und andere oxidative Spezies freigesetzt werden. Hypochlorit und andere oxidative Spezies spalten bekanntlich verschiedene Polymere. Es wird jedoch angenommen, dass Polyurethane auf Polycarbonatbasis gegen die Wirkungen dieser oxidativen Spezies resistent sind, und sie wurden als biobeständig bezeichnet. Da außerdem Hypochlorit und andere oxidierende Spezies in vivo kurzlebige Chemikalien sind, wird ein Bioabbau nicht auftreten, wenn Macrophagen in einem ausreichenden Abstand von der enzymaktiven Membran gehalten werden.
- Die vorliegende Erfindung sieht die Verwendung von zellundurchlässigen Biomaterialien von einigen Mikron Dicke oder mehr (das ist ein zellundurchlässiger Bereich) in den meisten ihrer Membranstrukturen vor. Erwünschtermaßen ist die Dicke des zellundurchlässigen Bereichs nicht kleiner als etwa 10 Mikron und nicht größer als etwa 100 Mikron. Dieser Bereich der Biogrenzflächenmembran ist für Sauerstoff durchlässig und kann für Glucose durchlässig sein oder auch nicht und ist aus biobeständigen Materialien (z. B. für einen Zeitraum von mehreren Jahren in vivo) aufgebaut, die für Wirtszellen (z. B. Macrophagen) undurchlässig sind, wie z. B. Polymergemische aus Polyurethan auf Polycarbonatbasis und PVP.
- Der innerste Bereich der erfindungsgemäßen Membran ist für Zellen nicht haftfähig (das ist der zellundurchlässige Bereich), was im Gegensatz zu den Erfindungen von Brauker et al. (siehe oben) und Shults et al. (siehe oben) steht. In diesen beiden früheren Patenten werden Beispiele angegeben, bei denen die zellundurchlässige Membran (Brauker et al.) oder die Biogrenzflächenmembran (Shults et al.) von einer als BioporeTM bekannten Membran abgeleitet sind, die als Zellkulturträger von Millipore (Bedfort, MA) verkauft wird. In Anwesenheit bestimmter extrazellulärer Matrixmoleküle und auch in vivo können viele Zellarten fest an dieser Membran haften und sie dadurch unfähig machen, als nicht haftfähiger Bereich zu dienen. Da sie ferner die Zellhaftung an der innersten Schicht der Membran erlauben, fördern sie die Bildung der Sperrzellschicht, die die Fähigkeit der Membran herabsetzt, Moleküle durch die Grenzfläche Gerät/Gewebe zu transportieren. Wenn sich diese Zellen sich vermehren, verursachen sie schließlich außerdem Druck zwischen den Membranschichten, was zu einer Abblätterung der Schichten und einem katastrophalen Versagen der Membran führt.
- Wie oben beschrieben ist der zweite Bereich bei einer Ausführungsform der erfindungsgemäßen Membran gegen Zellanlagerung resistent, für Zellen undurchlässig und vorzugsweise aus einem biobeständigen Material. Der zweite Bereich kann aus Materialien wie denen bestehen, die zuvor für den ersten Bereich als Copolymere oder Gemische mit hydrophilen Polymeren aufgeführt wurden, wie Polyvinylpyrolidon (PVP), Polyhydroxyethylmethacrylat, Polyvinylalkohol, Polyacrylsäure, Polyether, wie Polyethylenglycol, und deren Blockcopolymeren einschließlich z. B. Diblock-, Triblock-Copolymeren, alternierenden, zufälligen und Pfropf-Copolymeren (Blockcopolymere sind in den US-Patenten Nr. 4,803,243 und 4,686,044 diskutiert, die hiermit durch Bezugnahme aufgenommen werden).
- Vorzugsweise besteht der zweite Bereich aus einem Polyurethan und einem hydrophilen Polymer. Das hydrophile Polymer ist wunschgemäß Polyvinylpyrrolidon. Bei einer Ausführungsform dieses Aspekts der Erfindung ist der zweite Bereich Polyurethan mit nicht weniger als 5 Gew.-% Polyvinylpyrrolidon und nicht mehr als 45 Gew.-% Polyvinylpyrrolidon. Vorzugsweise hat der zweite Bereich nicht weniger als 20 Gew.-% Polyvinylpyrrolidon und nicht mehr als 35 Gew.-% Polyvinylpyrrolidon, insbesondere hat dieser Bereich Polyurethan mit etwa 27 Gew.-% Polyvinylpyrrolidon.
- Wie oben beschrieben besteht der zellundurchlässige Bereich bei einer erwünschten Ausführungsform aus einem Polymergemisch, das aus einem nicht-bioabbaubaren Polyurethan besteht, das Polyvinylpyrrolidon aufweist. Dies verhindert die Haftung von Zellen in vitro und in vivo und erlaubt vielen Molekülen, frei durch die Membran zu diffundieren. Dieser Bereich verhindert jedoch einen Zelleintritt in oder einen Zellkontakt mit unter der Membran liegende(n) Geräteelemente(n) und verhindert das Anhaften von Zellen und dadurch die Bildung einer Sperrzellschicht.
- II. Implantierbare Glucose-Überwachungsgeräte unter Benutzung der Biogrenzflächenmembran
- Die außergewöhnlichen Membranstrukturen der vorliegenden Erfindung können um die Fühlergrenzfläche eines implantierbaren Geräts herum benutzt werden. Diese Geräte können irgendwelche bestimmten elektronischen Komponenten (z. B. Elektroden, Schaltung usw.) enthalten. Die Membranen der vorliegenden Erfindung können tatsächlich bei praktisch jedem Überwachungsgerät eingesetzt werden, das zur Implantation geeignet ist (oder so verändert werden kann, dass eine Implantation möglich ist). Geeignete Geräte sind Analyt-Messgeräte, Zell-Transplantationsgeräte, Arzneimittel-Zuführungsgeräte, elektrische Signalabgabe- und Messgeräte und andere Geräte, wie jene die beschrieben sind in US-Patent Nr. 4,703,756 und 4,994,167 von Shults et al.; US-Patent Nr. 4,703,756 von Gough et al. und US-Patent Nr. 4,431,004 von Bessman et al., deren Inhalte hier durch Bezugnahme aufgenommen werden, sowie Bindra et al., Anal. Chem. 63:1692-96 (1991).
- Wir beziehen uns nun auf
6A , die ein Analyt-Messgerät für die Benutzung in Verbindung mit einer erfindungsgemäßen Membran zeigt. Bei dieser Ausführungsform enthält ein keramischer Körper1 und keramischer Kopf10 die Fühlerelektronik, die eine Schaltungsplatte2 einen Mikroprozessor3 , eine Batterie4 und eine Antenne5 umfasst. Ferner besitzen der keramische Körper1 und Kopf10 einen Anpassungskonusverbinder6 , der mit Epoxy abgedichtet ist. Die Elektroden werden anschließend über eine Buchse8 an die Schaltungsplatte angeschlossen. - Wie im einzelnen in
6B gezeigt ist, ragen drei Elektroden durch den keramischen Kopf10 , eine Platin-Arbeitselektrode21 , eine Platin-Gegenelektrode22 und eine Silber/Silberchlorid-Bezugselektrode20 . Jede von Ihnen ist hermetisch an den keramischen Kopf10 angelötet und ferner mit Epoxy28 befestigt. Der Fühlerbereich24 ist mit der unten beschriebenen Fühlermembran bedeckt, und der keramische Kopf10 enthält eine Rille29 , so dass die Membran mit einem O-Ring an der Stelle befestigt werden kann. -
6C zeigt eine auseinandergezogene Querschnittsansicht des in6B dargestellten Elektroden-Membran-Bereichs24 mit Einzelheiten der Fühlerspitze und der funktionellen Membranschichten. Wie in6C dargestellt, umfasst der Elektroden-Membran-Bereich die erfinderische Biogrenzflächenmembran33 und eine Fühlermembran32 . Die oberen Enden der Elektroden sind mit der Elektrolytphase30 , einer frei fließenden Flüssigkeitsphase in Kontakt. Die Elektrolytphase ist durch die Fühlermembran32 abgedeckt, die ein Enzym enthält, z. B. Glucoseoxidase. Die erfinderische Grenzflächenmembran33 bedeckt ihrerseits die Enzymmembran32 und dient teilweise dazu, den Fühler vor äußeren Kräften zu schützen, die zu einem Reißen der Fühlermembran32 infolge Beanspruchung durch die Umgebung führen kann. - III. Experimentelles
- Die folgenden Beispiele dienen zur Erläuterung von bestimmten bevorzugten Ausführungsformen und Aspekten der vorliegenden Erfindung und sollen nicht als Begrenzung ihr Erfindungsumfangs ausgelegt werden.
- In der vorhergehenden Beschreibung und der folgenden experimentellen Offenbarung werden die folgenden Abkürzungen benutzt: Eq und Eqs (Äquivalente); mEq (Milliäquivalente); M (Molar); mM (Millimolar); μM (Mikromolar); N (Normal); mol (Mole); mmol (Millimole); μmol (Mikromole); nmol (Nanomole); g (Gramm); mg (Milligramm); μg (Mikrogramm); kg (Kilogramm); L (Liter); mL (Milliliter); dL (Deziliter); μL (Mikroliter); cm (Zentimeter); mm (Millimeter); μm (Mikrometer); nm (Nanometer); h und hr (Stunden); min. - Beispiel 1
- Herstellung von Biogrenzflächenmembran mit Vliesfasern
- Der Sperrzell-Zerreißbereich kann aus einer Vlies-Polyesterfaser-Filtrationsmembran hergestellt werden. Der zellundurchlässige Bereich kann dann auf diese Bereichsschicht aufgeschichtet werden. Der zellundurchlässige Bereich wurde dadurch hergestellt, dass man etwa 706 g Dimethylacetamid (DMAC) in eine Edelstahlschale von 3 L gab, der eine Polycarbonaturethan-Lösung (1325 g, Chronoflex AR, 25% Feststoffe in DMAC und eine Viskosität von 5100 cp) und Polyvinylpyrrolidon (125 g, Plasdone K-90D) zugesetzt wurden. Die Schale wurde dann mit einem Planetenmischer mit einem schaufelartigen Rührblatt ausgestattet, und der Inhalt wurde eine Stunde bei Raumtemperatur gerührt. Diese Lösung wird dann auf den Sperrzell-Zerreißbereich durch eine mit einem Spalt von 0,006'' (152 μm) gezogene Messerkante aufgeschichtet und 24 Stunden bei 60°C getrocknet. Die Membran wird dann durch einen O-Ring mechanisch an dem Fühlergerät befestigt.
- Beispiel 2
- Herstellung einer Biogrenzflächenmembran mit porösem Silikon
- Der Sperrzellen-Zerreißbereich kann aus einer porösen Silikonfolie bestehen. Das poröse Silikon wurde von Seare Biomatrix Systems, Inc. bezogen. Der zellundurchlässige Bereich wurde dadurch hergestellt, dass man etwa 706 g Dimethylacetamid (DMAC) in eine Edelstahlschale von 3 L gab, der eine Polycarbonaturethan-Lösung (1325 g, Chronoflex AR, 25% Feststoffe in DMAC und eine Viskosität von 5100 cp) und Polyvinylpyrrolidon (125 g, Plasdone K-90D) zugesetzt wurden. Die Schale wurde dann mit einem Planetenmischer mit einem schaufelartigen Rührblatt ausgestattet, und der Inhalt wurde eine Stunde bei Raumtemperatur gerührt. Die Beschichtungslösung des zellundurchlässigen Bereichs wurde dann unter Benutzung einer Rackel über einer Walze, die auf einen Spalt von 0,012'' (305 μm) eingestellt waren, auf ein PET-Trennfutter (Douglas Hansen Co.) aufgestrichen. Dieser Film wurde dann bei 305°F (152°C) getrocknet. Der fertige Film war etwa 0,0015'' (38 μm) dick. Die Biogrenzflächenmembran wurde durch Pressen des porösen Silikons auf den gegossenen zellundurchlässigen Bereich hergestellt. Die Membran wurde dann durch einen O-Ring an dem Fühlergerät mechanisch befestigt.
- Beispiel 3
- Testmethode zur Funktion des Glucose-Messgeräts
- Die Fühlerfunktion in vivo wurde bestimmt, indem der Fühler-Ausgangswert mit Blutglucosewerten von einem externen Blutglucosemesser in Beziehung gesetzt wurde. Wir haben gefunden, dass nicht-diabetische Hunde selbst nach Aufnahme einer Nahrung mit hohem Zuckergehalt keine schnellen Änderungen der Blutglucose erfahren. So wurde eine 10%-ige Dextroselösung in einen Hund mit implantiertem Fühler infundiert. Ein zweiter Katheder wurde zwecks Blutabnahme in das gegenüber liegenden Bein gelegt. Der implantierte Sensor war unter Benutzung eines gepulsten Elektromagneten zur Übertragung in Intervallen von 30 Sekunden programmiert. Die Dextroselösung wurde in den ersten 25 Minuten mit einer Geschwindigkeit von 9,3 ml/Minute, in den nächsten 20 Minuten mit 3,5 ml/Minute und in den nächsten 20 Minuten mit 1,5 ml/Minute infundiert, und dann wurde die Infusionspumpe abgeschaltet. Die Blutglucosewerte wurden während der Dauer der Untersuchung alle 5 Minuten doppelt mit einem Blutglucosemesser (LXN Inc., San Diego, CA) gemessen. Ein Rechner sammelte die Ausgangswerte des Fühlers. Die Daten wurden dann zusammengestellt und in einer Bildschirmtabelle grafisch dargestellt, zeitlich ausgerichtet und zeitlich verschoben, bis ein optimaler R-Quadratwert erreicht war. Der R-Quadratwert lässt erkennen, wie gut der Fühler den Blutglucosewerten folgt.
- Beispiel 4
- Bewertung von Glucose-Messgeräten in vivo, die Biogrenzflächenmembranen der vorliegenden Erfindung enthalten.
- Um die Wichtigkeit einer Zellzerreißmembran zu prüfen, wurden implantierbare Glucosfühler mit den Biogrenzflächenmembranen der vorliegenden Erfindung in das subkutane Gewebe von Hunden implantiert und wöchentlich auf Glucosereaktion überwacht. Kontrollgeräte mit nur einem zellundurchlässigen Bereich („Kontrolle") wurden insbesondere mit Geräten verglichen, die einen zellundurchlässigen Bereich und einen Sperrzell-Zerreißbereich enthielten, wobei der Sperrzell-Zerreißbereich eine Vliesfaser („Vliesfasern") oder poröses Silikon („poröses Silikon") war.
- Vier Geräte von jeder Beschaffenheit wurden subkutan in das ventrale Abdomen normaler Hunde implantiert. Den Hunden wurde wöchentlich Glucose gemäß Beschreibung in Beispiel 3 infundiert, um ihre Blutglucosegehalte von etwa 120 mg/dl auf etwa 300 mg/dl zu erhöhen. Die Blutglucosewerte wurden in Intervallen von fünf Minuten mit einem LXN-Blutglucosemesser bestimmt. Die Fühlerwerte wurden in Intervallen von 0,5 Minuten übertragen. Es wurde ein Regressionsanalye zwischen Blutglucosewerten und dem am nahesten gelegenen Fühlerwert in einer Minute durchgeführt.
- Geräte, die einen R-Quadratwert von mehr als 0,5 lieferten, wurden als funktionsfähig angesehen.
7 zeigt für jede Beschaffenheit die Anzahl funktionsfähiger Geräte über den 12-Wochen-Zeitraum der Untersuchung. Beide Testgeräte zeigten während der ersten neun Wochen der Untersuchung eine bessere Leistung als die Kontrollgeräte. Alle Geräte mit porösem Silicon waren bis zur Woche 9 funktionsfähig. Zwei der vier Geräte mit Polypropylenfaser waren bis zur Woche 2 funktionsfähig, und drei von vier waren in Woche 12 funktionsfähig. Dagegen war keins der Kontrollgeräte bis Woche 10 funktionsfähig, nach der zwei für die restlichen drei Wochen funktionsfähig waren. Diese Daten zeigen klar, dass der Einsatz einer Zell-Zerreißschicht in Kombination mit einer zellundurchlässigen Schicht die Funktion von implantierbaren Glucosefühlern verbessert. - Die Beschreibung und die oben angegebenen Versuchsmaterialien sollen die vorliegende Erfindung erläutern, ohne ihren Umfang einzuschränken. Es wird dem Fachmann geläufig sein, dass Veränderungen und Modifizierungen vorgenommen werden können, ohne die Idee und den Umfang der vorliegenden Erfindung zu verlassen. Alle oben angegebenen Literaturstellen werden in ihrer Gesamtheit durch Bezugnahme in den Text aufgenommen.
Claims (24)
- Biogrenzflächenmembran für die Verwendung bei einem implantierbaren Gerät mit a) einem zu dem implantierbaren Gerät distalen ersten Bereich, der das Einwachsen von Gewebe unterstützt und die Bildung einer Sperr-Zellschicht behindert und b) einem zu dem implantierbaren Gerät proximalen zweiten Bereich, der gegen Zellanlagerung widerstandsfähig und für Zellen und Zellfortsätze undurchlässig ist.
- Biogrenzflächenmembran nach Anspruch 1, bei der der erste Bereich eine offenzellige Struktur mit Hohlräumen und einem festen Anteil aufweist.
- Biogrenzflächenmembran nach Anspruch 2, bei der die offenzellige Struktur im wesentlichen in dem gesamten Bereich in drei Dimensionen eine Tiefe von mehr als einem Hohlraum aufweist.
- Biogrenzflächenmembran nach Anspruch 2 oder 3, bei der eine wesentliche Anzahl der Hohlräume in der kürzesten Dimension nicht kleiner als 20 Mikron und in der längsten Dimension nicht größer als 1000 Mikron ist.
- Biogrenzflächenmembran nach Anspruch 2 oder 3, bei der eine wesentliche Anzahl der Hohlräume in der kürzesten Dimension nicht kleiner als 25 Mikron und in der längsten Dimension nicht größer als 500 Mikron ist.
- Biogrenzflächenmembran nach einem der Ansprüche 2 bis 5, bei der die Hohlräume und Hohlraumverbindungen in Schichten mit unterschiedlichen Hohlraumdimensionen ausgebildet sind.
- Biogrenzflächenmembran nach einem der Ansprüche 2 bis 6, bei der der feste Anteil bei einer wesentlichen Anzahl der kürzesten Dimensionen nicht weniger als 5 Mikron und bei einer wesentlichen Anzahl der längsten Dimensionen nicht mehr als 2000 Mikron hat.
- Biogrenzflächenmembran nach einem der Ansprüche 2 bis 6, bei der der feste Anteil bei einer wesentlichen Anzahl der kürzesten Dimensionen nicht weniger als 10 Mikron und bei einer wesentlichen Anzahl der längsten Dimensionen nicht mehr als 1000 Mikron hat.
- Biogrenzflächenmembran nach einem der Ansprüche 2 bis 6, bei der der feste Anteil bei einer wesentlichen Anzahl der kürzesten Dimension nicht weniger als 10 Mikron und bei einer wesentlichen Anzahl der längsten Dimensionen nicht mehr als 400 Mikron hat.
- Biogrenzflächenmembran nach einem der Ansprüche 2 bis 9, bei der der feste Anteil Silikon aufweist.
- Biogrenzflächenmembran nach einem der Ansprüche 2 bis 9, bei der der feste Anteil Polyurethane aufweist.
- Biogrenzflächenmembran nach einem der Ansprüche 2 bis 9, bei der der feste Anteil Block-Kopolymere aufweist.
- Biogrenzflächenmembran nach einem der Ansprüche 2 bis 9, bei der der feste Anteil aus einem Werkstoff hergestellt ist, der aus der aus Polytetrafluorethylen, Polyethylen-co-tetrafluorethylen, Polyolefinen, Polyestern und Polycarbonaten bestehenden Gruppe ausgewählt ist.
- Biogrenzflächenmembran nach einem der Ansprüche 2 bis 13, bei der der feste Anteil Vliesfasern aufweist.
- Biogrenzflächenmembran nach Anspruch 14, bei der der feste Anteil Vliesfasern aufweist, die in der kürzesten Dimension größer als 5 Mikron sind.
- Biogrenzflächenmembran nach Anspruch 14, bei der der feste Anteil Vliesfasern von etwa 10 Mikron in der kürzesten Dimension aufweist.
- Biogrenzflächenmembran nach Anspruch 14, bei der der feste Anteil Vliesfasern aufweist, die in der kürzesten Dimension größer als oder gleich 10 Mikron sind.
- Biogrenzflächenmembran nach Anspruch 14, bei der der feste Anteil Vliesfasern aufweist, die in der kürzesten Dimension größer als 10 Mikron sind.
- Biogrenzflächenmembran nach einem der Ansprüche 1 bis 18, bei der der zweite Bereich aus einem biobeständigen Werkstoff besteht.
- Biogrenzflächenmembran nach Anspruch 19, bei der der biobeständige Werkstoff aus einem Polyurethan und einem hydrophilen Polymeren besteht.
- Biogrenzflächenmembran nach Anspruch 19, bei der der biobeständige Werkstoff Polyvinylpyrrolidon enthaltendes Polyurethan ist.
- Biogrenzflächenmembran nach Anspruch 19, bei der der zweite Bereich Polyurethan mit nicht weniger als 5 Gew.-% Polyvinylpyrrolidon und nicht mehr als 45 Gew.-% Polyvinylpyrrolidon ist.
- Biogrenzflächenmembran nach Anspruch 19, bei der der zweite Bereich Polyurethan mit nicht weniger als 20 Gew.-% Polyvinylpyrrolidon und nicht mehr als 35 Gew.-% Polyvinylpyrrolidon ist.
- Biogrenzflächenmembran nach Anspruch 19, bei der der zweite Bereich Polyurethan mit etwa 27 Gew.-% Polyvinylpyrrolidon ist.
