- HINTERGRUND DER ERFINDUNG
- 1. Gebiet der Erfindung
- Die vorliegende Erfindung betrifft ein medizinisches Robotersystem.
- 2. Hintergrundinformationen
- Eine Verengung einer Kranzarterie kann dem Herzen Blut und Sauerstoff entziehen, die zum Lebenserhalt benötigt werden. Der Verengung kann durch medikamentöse Behandlung oder durch eine Angioplastie beseitigt werden. Bei starker Verengung wird eine aortokoronare Bypass-Operation (CABG – coronary artery bypass graft) durchgeführt, um den verengten Bereich der Arterie zu umgehen. CABG-Prozeduren werden üblicherweise dadurch ausgeführt, dass das Sternum bzw. Brustbein durchtrennt und der Brustraum geöffnet wird, um den Zugang zum Herzen zu ermöglichen. In der Arterie in der Nähe des verengten Bereichs wird ein Schnitt ausgeführt. Die innere Brustarterie wird dann durchtrennt und an der Schnittstelle an der Arterie befestigt. Die innere Brustarterie überbrückt den verengten Bereich der Arterie, um das Herz wieder mit einem vollen Blutstrom zu versorgen. Das Durchtrennen des Sternums und das Öffnen des Brustraums kann ein enormes Trauma für den Patienten erzeugen. Außerdem verlängert das gespaltene Sternum die Genesungsdauer des Patienten.
- Computer Motion aus Goleta-Kalifornien liefert unter der Marke ZEUS ein System, welches es einem Chirurgen ermöglicht, ein minimal invasives CABG-Verfahren auszuführen. Das Verfahren wird mit Instrumenten ausgeführt, die durch kleine Schnitte in die Brust des Patienten eingebracht werden. Die Instrumente werden von Roboterarmen gesteuert. Die Bewegung der Roboterarme und die Betätigung von Instrument-Endeffektoren wird von dem Chirurgen über ein Paar Griffe und ein Fußpedal gesteuert, die mit einer elektronischen Steuereinrichtung gekoppelt sind. Alternativ kann der Chirurg die Bewegung eines zur Betrachtung der inneren Organe des Patienten verwendeten Endoskops durch Sprachbefehle steuern.
- Die Griffe und ein Bildschirm sind üblicherweise in eine Konsole integriert, die von dem Chirurgen bedient wird, um die verschiedenen Roboterarme und medizinischen Instrumente eines ZEUS-Systems zu steuern. Die Verwendung eines Robotersystems zur Ausführung von Operationen erfordert ein gewisses Training. Es wäre wünschenswert, ein System bereitzustellen, welches es einem zweiten Chirurgen ermöglichen würde, einem anderen Chirurgen bei der Steuerung eines medizinischen Robotersystems zu assistieren. Der zweite Chirurg könnte einen Chirurgen, welcher lernt, ein medizinisches Verfahren mit einem ZEUS-System auszuführen, sowohl unterrichten als auch assistieren. Dies würde die Zeit deutlich verringern, die benötigt wird, um den Betrieb eines Roboter-gestützten medizinischen Systems zu erlernen.
- Das an Wilk erteilte
US-Patent Nr. 5 217 003 beschreibt ein Operationssystem, welches einem Chirurgen ermöglicht, von einem Roboter gesteuerte medizinische Instrumente über eine Telekommunikationsverbindung aus der Ferne zu bedienen. Das Wilk-System erlaubt einem Chirurgen die Roboterarme zu einer bestimmten Zeit zu bedienen. Wilk beschreibt bzw. erwägt kein System, welches es zwei verschiedenen Chirurgen ermöglicht, den gleichen Satz von Roboterarmen zu bedienen. - Das an Ichikawa et al. erteilte und an Olympus Optical Co. Ltd. übertragene
US-Patent Nr. 5 609 560 beschreibt ein System, welches es einer Bedienperson ermöglicht, mehrere verschiedene medizinische Einrichtungen über eine einzige Schnittstelle zu steuern. Das Olympus-Patent beschreibt kein System, welches es mehreren Eingabeeinrichtungen ermöglicht, eine einzige medizinische Einrichtung zu steuern. -

- Es wird auf die Zeichnungen insbesondere mit Hilfe von Bezugszeichen Bezug genommen.
1 zeigt ein System10 , welches minimal-invasive Operationen ausführen kann. Bei einer Ausführungsform wird das System10 verwendet, um eine minimal-invasive aortokoronare Bypassoperation (MI-CABG) und andere anastomotische Verfahren auszuführen. Obwohl ein Mi-CABG-Verfahren gezeigt und beschrieben ist, sollte klar sein, dass das System für andere Operationsverfahren verwendet werden kann. Beispielsweise kann das System verwendet werden, um beliebige Gefäßpaare zu nähen. Das System10 kann verwendet werden, um an einem Patienten12 , der üblicherweise auf einem Operations tisch14 liegt, ein Verfahren auszuführen. An dem Operationstisch14 sind ein erster Gelenkarm16 , ein zweiter Gelenkarm18 und ein dritter Gelenkarm20 befestigt. Die Gelenkarme16 ,18 und20 sind vorzugsweise derart an dem Tisch14 befestigt, dass die Arme in der gleichen Referenzebene wie der Patient liegen. Obwohl drei Gelenkarme dargestellt und beschrieben sind, sollte klar sein, dass das System eine beliebige Anzahl von Armen haben kann. - Der erste und zweite Gelenkarm
16 und18 weist jeweils ein chirurgisches Instrument22 bzw.24 auf, die mit den Roboterarmen26 bzw.28 gekoppelt sind. Der dritte Gelenkarm20 umfasst einen Roboterarm30 , der ein Endoskop32 hält und bewegt. Die Instrumente22 und24 sowie das Endoskop32 werden durch in die Haut des Patienten geschnittene Schnitte eingeführt. Das Endoskop weist eine Kamera34 auf, welche mit einem Fernsehmonitor36 gekoppelt ist, welcher Bilder der inneren Organe des Patienten anzeigt. - Der erste
16 , zweite18 und dritte20 Gelenkarm sind mit einer Steuereinrichtung38 gekoppelt, welche die Bewegung der Arme steuern kann. Die Steuereinrichtung38 ist mit einer Eingabeeinrichtung40 verbunden, beispielsweise einem Fußpedal, welches von einem Chirurgen bedient werden kann, um die Position des Endoskops32 zu verschieben. Die Steuereinrichtung38 enthält elektrische Schaltungen, beispielsweise einen Prozessor, um die Roboterarme26 ,28 und30 zu steuern. Der Chirurg kann einen anderen Abschnitt des Patienten betrachten, indem er eine entsprechende Taste(n) des Pedals40 drückt. Die Steuereinrichtung38 empfängt das (die) Eingangssignal(e) von dem Fußpedal40 und bewegt den Roboterarm30 und das Endoskop32 entsprechend den Eingangsbefehlen von dem Chirurgen. Der Roboterarm kann eine Einrichtung sein, die von dem Zessionar der vorliegenden Erfindung verkauft wird, Computer Motion, Inc. aus Goleta, Kalifornien, unter deren Marke AESOP. Das System wird auch beschrieben in dem an Wang et al. erteiltenUS-Patent Nr. 5 657 429 . Obwohl ein Fußpedal40 dargestellt und beschrieben ist, sollte klar sein, dass das System andere Ein gabemittel aufweisen kann, beispielsweise eine Hand-Steuereinrichtung oder eine Spracherkennungsschnittstelle. - Die Instrumente
22 und24 des ersten16 bzw. zweiten18 Gelenkarms werden von einem Paar von Master- bzw. Haupt-Griffen42 und44 gesteuert, welche von dem Chirurgen betätigt werden können. Die Griffe42 und44 und die Arme16 und18 haben eine Master-Slave-Beziehung, so dass eine Bewegung der Griffe42 und44 eine entsprechende Bewegung der chirurgischen Instrumente22 und24 bewirkt. Die Griffe42 und44 können an einem tragbaren Gehäuse46 befestigt sein. Die Griffe42 und44 sind ferner mit der Steuereinrichtung38 gekoppelt. - Die Steuereinrichtung
38 empfängt Eingangssignale von den Griffen42 und44 , berechnet eine entsprechende Bewegung der chirurgischen Instrumente und liefert Ausgangssignale zum Bewegen der Roboterarme26 und28 und der Instrumente22 und24 . Das gesamte System kann ein von Computer Motion unter der Marke ZEUS vermarktetes Produkt sein. Die Funktionsweise des Systems wird auch beschrieben in dem an Wang et al. erteilten und an Computer Motion übertragenenUS-Patent Nr. 5 762 458 . -
2 zeigt eines der chirurgischen Instrumente22 oder24 . Das Instrument22 oder24 enthält einen Endeffektor48 , der mit einer Aktuatorstange50 gekoppelt ist. Die Aktuatorstange50 ist über einen Adapter54 mit einem Motor52 gekoppelt. Der Motor52 betätigt den Endeffektor48 , indem er die Aktuatorstange50 bewegt. Die Aktuatorstange50 ist mit einem Kraftsensor56 gekoppelt, der die von dem Endeffektor48 ausgeübte Kraft erfasst. Der Kraftsensor56 liefert ein analoges Ausgangssignal, welches an die in1 gezeigte Steuereinrichtung gesendet wird. - Der Adapter
54 ist mit einer am Ende eines Roboterarms26 oder28 angeordneten Getriebeanordnung58 gekoppelt. Die Getriebeanordnung58 kann den Adapter54 und den Endeffektor48 drehen. Die Aktuatorstange50 und der Endeffektor48 können mit dem Kraftsensor56 und dem Motor52 über einen federbelasteten Hebel60 gekoppelt sein. Die Instrumente22 oder24 können die gleichen Instrumente oder ähnliche Instrumente sein, die in dem '458-Patent beschrieben sind. -
3 zeigt ein System100 , welches es zwei verschiedenen Eingabeeinrichtungen ermöglicht, eine medizinische Einrichtung bzw. ein medizinisches Gerät zu steuern. Die Eingabeeinrichtungen können eine erste Konsole102 und eine zweite Konsole104 sein. Die Konsolen102 und104 können jeweils den Bildschirm36 , die Griffe42 und44 , ein (nicht gezeigtes) Fußpedal und die Steuereinrichtung38 enthalten, die in1 gezeigt sind. Die medizinischen Einrichtungen bzw. Geräte umfassen die Roboterarme26 ,28 und30 und/oder Instrumente22 und24 , die in1 gezeigt sind. Ganz allgemein ermöglicht das System einem Chirurgen an einer der beiden Konsolen102 und104 , eine medizinische Einrichtung,22 ,24 ,26 ,28 und/oder30 zu steuern. Beispielsweise kann der Chirurg an der Konsole102 die Roboterarme26 und28 mit Hilfe einer Bewegung der Griffe42 und44 bewegen. Der Chirurg an der Konsole104 kann sich über die Eingabe von der Konsole102 hinwegsetzen und die Bewegung der Roboterarme26 und28 mit Hilfe der Bewegung der Konsolengriffe steuern. - Die Konsolen
102 und104 sind über ein Paar von Verbindungseinrichtungen108 und110 mit einem Netzwerkport106 gekoppelt. Der Netzwerkport106 kann ein Computer sein, der die notwendige Hardware und Software enthält, um Informationen über eine Kommunikationsverbindung112 in ein Kommunikationsnetzwerk114 zu senden und zu empfangen. - Die Konsolen
102 und104 , die von Computer Motion unter der Marke ZEUS geliefert werden, liefern Ausgangssignale, welche mit einem Computer inkompatibel sein können. Die Verbindungseinrichtungen108 und110 können eine Schnittstelle bereitstellen, die die Signale konditioniert, damit Signale zwischen den Konsolen102 und104 und dem Netzwerkcomputer106 übertragen und gesendet werden können. - Es sollte klar sein, dass der Computer und/oder die Konsolen
102 und104 derart aufgebaut sein können, dass das System die Verbindungseinrichtungen108 und110 nicht benötigt. Zusätzlich können die Konsolen102 und104 so aufgebaut sein, dass das System keinen separaten Netzverbindungscomputer106 benötigt. Beispielsweise können die Konsolen102 und104 so aufgebaut und/oder konfiguriert sein, dass sie Informationen direkt über das Kommunikationsnetzwerk114 senden. - Das System
100 kann einen zweiten Netzwerkport116 enthalten, der mit einer Einrichtungssteuereinrichtung(en)118 und dem Kommunikationsnetzwerk114 gekoppelt ist. Die Einrichtungssteuereinrichtung118 steuert die Roboterarme26 ,28 und30 und Instrumente22 und24 . Der zweite Netzwerkport116 kann ein Computer sein, der mit der Steuereinrichtung118 über eine Verbindungseinrichtung120 gekoppelt ist. Obwohl eine Verbindungseinrichtung120 und ein Netzwerkcomputer116 gezeigt und beschrieben sind, sollte klar sein, dass die Steuereinrichtung118 so aufgebaut und konfiguriert sein kann, dass die Einrichtung120 und/oder der Computer116 entfallen können. - Das Kommunikationsnetzwerk
114 kann ein beliebiges Kommunikationssystem sein, einschließlich des Internets und anderer Typen von Weitbereichsnetzen (WANs), Intranets, lokaler Netze (LANs), öffentlicher Fernsprechnetze (PSTN – public switched Telefone networks), digital integrierender Netze (ISDN), ist jedoch nicht auf diese beschränkt. Vorzugsweise wird eine Kommunikationsverbindung über ein faseroptisches Netzwerk aufgebaut, um die Latenzzeit in dem System zu verringern. In Abhängigkeit von der ausgewählten Kommunikationsverbindungsart können die Informationen beispielsweise gemäß dem User Datagram Protocol/Internet Protocol (UDP/IP) oder dem asynchronen Transfermodus/(ATM/AAL1)-Netzwerkprotokollen übertragen werden. Die Computer112 und116 können gemäß einem unter der Bezeichnung VxWORKS verkauften Betriebssystem arbeiten. Beispielsweise können die Computer112 und116 derart aufgebaut und konfiguriert sein, dass sie mit 100Base-T-Ethernet und/oder 155 Mbps-Faser-ATM-Systemen arbeiten können. -
4 zeigt eine Ausführungsform der "Chirurgen"-Seite des Systems. Jeder Konsole102 und104 kann ein Touchscreen- bzw. Tastbildschirm-Computer122 zugeordnet sein und ein Endoskopschnittstellencomputer124 . Der Tastbildschirmcomputer122 kann ein von Computer Motion unter der Marke HERMES verkauftes Gerät sein. Der Tastbildschirm122 ermöglicht dem Chirurgen verschiedene Funktionen und Operationen der Instrumente22 und24 zu steuern und zu verändern. Beispielsweise kann der Chirurg die Skalierung zwischen der Bewegung der Griffe42 und44 und der Bewegung der Instrumente22 und24 über eine grafische Benutzerschnittstelle (GUI) des Tastbildschirms122 verändern. Der Tastbildschirm122 kann eine weitere GUI aufweisen, welche es dem Chirurgen ermöglicht, eine Aktion wie das Schließen des Greifers eines Instruments zu initiieren. - Der Endoskopcomputer
124 kann dem Chirurgen ermöglichen, die Bewegung des Roboterarms30 und des Endoskops32 zu steuern, die in1 gezeigt sind. Der Endoskopcomputer124 kann eine Alternative oder ein Zusatz zum in1 gezeigten Fußpedal40 sein. Der Endoskopcomputer124 kann ein von Computer Motion unter der Marke SOCRATES verkauftes Gerät sein. Der Tastbildschirm122 und die Endoskopcomputer124 können mit dem Netzwerkcomputer106 über RS232-Schnittstellen gekoppelt sein. - Eine ZEUS-Konsole sendet und empfängt Informationen, welche als analoge, digitale oder Quadratur-Signale übermittelt werden. Der Netzwerkcomputer
112 kann eine analoge Eingabe/Ausgabe(I/O)-Schnittstelle126 , eine digitale I/O-Schnittstelle128 und eine Quadratur-Schnittstelle130 aufweisen, welche eine Kommunikation zwischen der Konsole102 oder104 und dem Netzwerk114 ermöglichen. Die analoge Schnittstelle126 kann beispielweise Daten senden und empfangen, die die Griffposition, Neigungsposition, Ein/Aus-Position und Fußpedalinformationen (sofern verwendet) betreffen. Die Quadratur-Signale können Rollwinkel(roll) und Gierwinkel(pan)-Positionsdaten betreffen. Die digitale I/O-Schnittstelle128 kann sich beziehen auf das drahtgebundene Sensordaten, auf Griff-Schaltflächen, Beleuchtungsvorrichtungen (LEDs) und Audio-Rückkopplung (Summer). Die Positionsdaten sind vorzugsweise absolute Positionsinformationen. Durch Verwendung absoluter Positionsinformationen können die Roboterarme noch bewegt werden, selbst wenn irgendwelche Informationen nicht erfolgreich über das Netzwerk114 übertragen werden. Wenn inkrementale Positionsinformationen bereitgestellt werden, würde ein Fehler bei der Übertragung eine Lücke in den Daten erzeugen und möglicherweise eine ungenaue Armbewegung. Der Netzwerkcomputer112 kann außerdem einen Bildschirm132 aufweisen, welcher einem Benutzer die Bedienung des Computers112 ermöglicht. -
5 zeigt eine Ausführungsform einer "Patienten"-Seite des Systems100 . Die Steuereinrichtung118 kann drei getrennte Steuereinrichtungen134 ,136 und138 enthalten. Die Steuereinrichtung134 kann Eingangsbefehle empfangen, auf der Basis der Befehle kinematische Berechnungen durchführen und Ausgangssignale ansteuern, um die Roboterarme26 und28 und die zugehörigen Instrumente22 und24 in eine gewünschte Position zu bewegen. Die Steuereinrichtung136 kann Befehle empfangen, welche verarbeitet werden, um die Instrumente22 und24 sowohl zu bewegen als auch zu betätigen. Die Steuereinrichtung138 kann Eingangsbefehle empfangen, auf der Basis der Befehle kinematische Berechnungen ausführen und Ausgangssignale ansteuern, um den Roboterarm30 und das zugehörige Endoskop32 zu bewegen. - Die Steuereinrichtungen
134 und136 können mit dem Netzwerkcomputer116 über eine digitale I/O-Schnittstelle140 und eine analoge I/O-Schnittstelle142 gekoppelt sein. Der Computer116 kann mit der Steuereinrichtung138 über eine RS232-Schnittstelle gekoppelt sein. Außerdem kann der Computer116 mit entsprechenden RS232-Ports der Steuereinrichtungen134 und136 gekoppelt sein. Die RS232-Ports der Steuereinrichtungen134 und136 können Daten empfangen, beispielsweise Bewegungsskalierung und Endeffektor-Betätigung. - Die Roboterarme und Instrumente enthalten Sensoren, Codierer usw., welche Rückkopplungsinformationen bereitstellen. Einige oder alle dieser Rückkopplungsinformationen können über das Netzwerk
114 an die Chirurgenseite des Systems gesendet werden. Beispielsweise können die analogen Rückkopplungsinformationen eine Griffrückkopplung, Neigungsrückkopplung, Ein/Aus-Rückkopplung und Fußpedal-Rückkopplung umfassen. Die digitale Rückkopplung kann eine Kabelabtastung, Schaltflächen, Beleuchtung und eine akustische Rückkopplung umfassen. Der Computer116 kann mit einem Bildschirm142 gekoppelt sein. - Die Computer
106 und116 können die Informationen für die Übertragung über das Kommunikationsnetzwerk114 paketieren. Jedes Paket enthält zwei Datentypen, Roboterdaten und RS232- Daten. Roboterdaten können Positionsinformationen der Roboter, einschließlich Eingabebefehle zum Bewegen der Roboter und eine Positionsrückkopplung von den Robotern umfassen. Die RS232-Daten können Funktionsdaten, wie Instrumentenskalierung und Betätigung umfassen. - Da das System absolute Positionsdaten sendet, können die Roboterdatenpakete außerhalb der Reihenfolge empfangen werden. Dies kann geschehen, wenn ein UDP/IP-Protokoll verwendet wird, welches eine Best-Efforts-Methode verwendet wird. Die Computer
106 und116 sind derart aufgebaut und konfiguriert, dass sie alle "spät" ankommenden Pakete mit Roboterdaten unberücksichtigt lassen. Beispielsweise kann der Computer106 Pakete 1, 2 und 3 senden. Der Computer116 kann die Pakete in der Reihenfolge 1, 3 und 2 empfangen. Der Computer116 wird das zweite Paket 2 unberücksichtigt lassen. Das Nichtberücksichtigen des Pakets statt des Anforderns einer Neuübertragung der Daten verringert die Latenzzeit des Systems. Es ist wünschenswert, die Latenzzeit zu minimieren, um einen "Echtzeit"-Betrieb des Systems zu ermöglichen. - Es ist vorteilhaft, die RS232-Informationen in strikter sequentieller Reihenfolge zu empfangen. Daher fordert der empfangende Computer eine Neuübertragung der RS232-Daten von dem sendenden Computer, wenn die Daten nicht fehlerfrei empfangen werden. RS232-Daten, beispielsweise Bewegungsskalierung und Instrumentenbetätigung müssen genau übertragen werden und dann verarbeitet werden, um sicherzustellen, dass es keinen versehentlichen Befehl gibt.
- Die Computer
106 und116 können die RS232-Daten von den verschiedenen Eingabequellen multiplexen. Die Computer106 und116 können First-In-First-Out-Warteschlangen (FIFO) zum übertragen von Informationen aufweisen. Zwischen dem Computer106 und den verschiedenen Komponenten auf der Chirurgenseite des Systems übertragene Daten können über ein Protokoll übertragen werden, das von Computer Motion unter dem Namen HERMES NETWORK PROTOCOL (HNP) angeboten wird. In ähnlicher Weise können Informationen zwischen Komponenten auf der Patientenseite des Systems gemäß HNP übertragen werden. - Zusätzlich zu den Roboter- und RS232-Daten überträgt die Patientenseite des Systems Videodaten von der Endoskopkamera
34 . Zur Verringerung der Latenzzeit in dem System kann der Computer116 die Videodaten mit den Roboter/RS232-Daten auf das Kommunikationsnetzwerk multiplexen. Die Videodaten können mit Hilfe herkömmlicher Kompressionstechniken wie JPEG usw. zur Übertragung an die Chirurgenseite des Systems komprimiert werden. - Jedes Paket
150 kann die in6 gezeigten Felder aufweisen. Das SOURCE-ID-Feld bzw. QUELLE-ID-Feld enthält Identifikationsinformationen des Eingabegeräts oder des medizinischen Geräts, von welchem die Daten stammen. Das DESTINATION-ID-Feld bzw. ZIEL-ID-Feld enthält Identifikationsinformationen, welches das Eingabegerät oder das medizinische Gerät identifizieren, welches die Daten empfangen soll. Das OPCODE-Feld bzw. OPERATIONSCODE-Feld definiert den Typ der übertragenen Befehle. Das PRIORITÄTS-Feld definiert die Priorität des Eingabegeräts. Die Prioritätsdaten können verwendet werden, um zu bestimmen, welches Eingabegerät die Steuerung über das medizinische Gerät hat. Das SEQ#-Feld liefert eine Paketsequenz-Nummer, so dass der empfangende Computer bestimmen kann, ob das Paket außerhalb der Reihenfolge ist. - Das TX-Rate-Feld bzw. Übertragungsgeschwindigkeits-Feld gibt die mittlere Rate an, mit welcher Pakete übertragen werden. Das RX-Rate-Feld bzw. Empfangsrate-Feld gibt die mittlere Geschwindigkeit an, mit welcher Pakete empfangen werden. Das RS232-ACK-Feld enthält einen Bestätigungszählwert für RS232-Daten. RS232-Daten werden üblicherweise in der Warteschlange eines Computers geführt, bis eine Bestätigung von dem empfangenden Computer empfangen wird, dass die Daten empfangen wurden.
- Das RS232-POS-Feld ist ein Zähler der gesendete RS232-Daten betrifft. Das RS232-ID-Feld ist eine Identifikation für RS232-Daten. Das RS232-MESS-SZ-Feld enthält die Paketgröße. Das RS232-PUFFER-Feld enthält die Länge des Inhalts des Pakets. Das DATEN-Feld enthält die übertragenen Daten und kann separate Unterfelder für Roboter-Daten und RS232-Daten enthal ten. CS ist ein Prüfsummenfeld, das zum Erfassen von Fehlern bei der Paketübertragung verwendet wird.
- Entweder der Computer
106 oder116 kann als Entscheidungseinheit zwischen den Eingabeeinrichtungen und den medizinischen Einrichtungen verwendet werden. Beispielsweise kann der Computer116 Daten von beiden Konsolen102 und104 empfangen. Die Informationspakete von beiden Konsolen102 und104 können Prioritätsdaten in den PRIORITÄTS-Feldern enthalten. Der Computer116 wird die Daten an die relevanten Einrichtungen (z. B. Roboter, Instrumente usw.) gemäß den Prioritätsdaten weiterleiten. Beispielsweise kann die Konsole104 eine höhere Priorität als die Konsole102 haben. Der Computer116 wird Daten zum Steuern eines Roboters von der Konsole104 unter Ausschluss von Daten von der Konsole102 weiterleiten, so dass der Chirurg bei104 die Steuerung über den Arm hat. - Bei einer alternativen Ausführungsform kann der Computer
116 so aufgebaut und konfiguriert sein, dass er Prioritäten gemäß den Daten in dem SOURCE-ID-Feld bereitstellt. Beispielsweise kann der Computer116 so programmiert sein, dass er Daten, die die SOURCE-ID von der Konsole104 haben, immer einen Vorrang bzw. eine Priorität gibt. Der Computer116 kann einen hierarchischen Baum aufweisen, welcher einer Anzahl von verschiedenen Eingabeeinrichtungen Prioritäten zuweist. - Alternativ kann der Computer
106 als Entscheidungseinheit funktionieren, die ein Screening der Daten durchführt, bevor diese über das Netzwerk114 übertragen werden. Der Computer106 kann ein Prioritätsschema aufweisen, das die Priorität immer einer der Konsolen102 oder104 zuweist. Zusätzlich oder alternativ kann eine oder mehrere der Konsolen102 und104 einen mechanischen und/oder Software-Schalter aufweisen, der betätigt werden kann, um der Konsole die Priorität zu geben. Der Schalter kann als Übersteuerungsmöglichkeit dienen, um einem Chirurgen zu ermöglichen, die Steuerung einer Prozedur zu übernehmen. - Im Betrieb führt das System zu Beginn eine Startroutine aus. Das ZEUS-System ist üblicherweise derart konfiguriert, dass es mit Daten von den Konsolen gestartet wird. Die Konso len können während der Startroutine der Roboterarme, Instrumente usw. während der Startroutine nicht in Kommunikation miteinander stehen, so dass das System die zum System-Boot benötigten Konsolendaten nicht hat. Der Computer
116 kann die fehlenden Konsoleneingangsdaten auf Standardwerte steuern. Die Standardwerte ermöglichen der Patientenseite des Systems, die Startroutine zu beenden. In ähnlicher Weise kann der Computer106 auch fehlende Eingangssignale von der Patientenseite des Systems auf Standardwerte steuern, um den Konsolen102 und/oder104 das Booten zu ermöglichen. Das Steuern fehlender Signale auf Standardwerte kann Teil eines lokalen Netzwerkmodus sein. Der lokale Modus ermöglicht einer oder mehreren Konsolen ein "Hot Plug-in" in das System ohne Abschaltung des Systems. - Wenn die Kommunikation zwischen der Chirurgen- und Patientenseite des Systems während des Betriebs unterbrochen wird, zwingt der Computer
106 die fehlenden Daten wieder auf Standardwerte. Die Standardwerte können Ruhesignalwerte sein, um einen unsicheren Betrieb des Systems zu vermeiden. Die Komponenten auf der Patientenseite werden auf dem letzten bekannten Wert gelassen, so dass die Instrumente und Arme sich nicht bewegen. - Nachdem die Startroutinen abgeschlossen wurden und die Kommunikationsverbindung aufgebaut wurde, können die Chirurgen die Konsolen bedienen. Das System ist für medizinische Verfahren sehr hilfreich, bei denen einer der Chirurgen ein Lehrer und der andere Chirurg ein Schüler ist. Die Entscheidungsfunktion des Systems ermöglicht dem Lehrer, die Steuerung von Roboterbewegung und Instrumentenbetätigung zu jeder Zeit während des Verfahrens zu übernehmen. Dies ermöglicht dem Lehrer, den Schüler über das Verfahren und/oder die Verwendung eines medizinischen Robotersystems zu unterrichten.
- Außerdem ermöglicht das System, dass ein Chirurg die eine medizinische Einrichtung steuern kann und ein anderer Chirurg die andere Einrichtung steuern kann. Beispielsweise kann ein Chirurg die Instrumente
22 und24 bewegen, während der andere Chirurg das Endoskop32 bewegt, oder ein Chirurg kann ein Instrument22 oder24 bewegen, während der andere Chirurg das andere Instrument24 oder22 bewegt. -
7 zeigt eine alternative Ausführungsform, wobei eine oder mehrere der Konsolen102 und104 eine alternative Kommunikationsverbindung160 aufweisen. Die alternative Verbindung kann ein Telekommunikationsnetzwerk sein, welches der Konsole102 ermöglicht, an einem entfernten Ort positioniert zu sein, während die Konsole104 relativ nahe bei den Roboterarmen usw. angeordnet ist. Beispielsweise kann die Konsole102 mit einem öffentlichen Telefonnetz verbunden sein, während die Konsole104 über ein LAN mit der Steuereinrichtung118 gekoppelt ist. Ein derartiges System würde eine Teleoperation mit Roboterarmen, Instrumenten usw. ermöglichen. Die Chirurgen- und Patientenseiten des Systems können mit der Verbindung160 über Netzwerkcomputer162 und164 gekoppelt sein. - Während bestimmte beispielhafte Ausführungsformen beschrieben und in den zugehörigen Zeichnungen dargestellt wurden, sollte klar sein, dass derartige Ausführungsformen nur der Veranschaulichung dienen und nicht die Breite der Erfindung beschränken sollen, und dass diese Erfindung nicht auf die gezeigten und beschriebenen Konstruktionen und Anordnungen beschränkt ist, da dem Durchschnittsfachmann zahlreiche andere Modifikationen einfallen werden, sondern nur durch den Schutzbereich der beigefügten Ansprüche.
Claims (29)
- Ein medizinisches System (
10 ,100 ), mit: einer ersten medizinischen Einrichtung; einer ersten Eingabeeinrichtung (102 ), welche die erste medizinische Einrichtung steuern kann; einer zweiten Eingabeeinrichtung (104 ), die die erste medizinische Einrichtung steuern kann, dadurch gekennzeichnet, dass die erste und die zweite Eingabeeinrichtung (102 ,104 ) jeweils derart konfiguriert sind, dass sie Informationspakete übertragen können; und dass es außerdem aufweist eine mit der ersten Eingabeeinrichtung und der zweiten Eingabeeinrichtung gekoppelte Entscheidungseinheit, wobei die Entscheidungseinheit prioritätsbezogene Informationen in den Informationspaketen verwendet, um entweder der ersten Eingabeeinrichtung oder der zweiten Eingabeeinrichtung das Steuern der ersten medizinischen Einrichtung zu ermöglichen. - Das System nach Anspruch 1, wobei die erste medizinische Einrichtung einen Roboterarm umfasst.
- Das System nach Anspruch 1, wobei die erste medizinische Einrichtung ein Instrument umfasst.
- Das System nach Anspruch 1, wobei die erste medizinische Einrichtung eine Endoskopkamera umfasst.
- Das System nach Anspruch 1, wobei die erste Eingabeeinrichtung (
102 ) und die zweite Eingabeeinrichtung (104 ) jeweils eine Konsole umfassen, die einen Griff (42 ,44 ) aufweist. - Das System nach Anspruch 5, wobei die erste Eingabeeinrichtung (
102 ) und die zweite Eingabeeinrichtung (104 ) jeweils eine Konsole umfassen, die einen Bildschirm (36 ) aufweist. - Das System nach Anspruch 1, wobei die erste Eingabeeinrichtung (
102 ) und die zweite Eingabeeinrichtung (104 ) jeweils einen Tastbildschirm (122 ) umfassen. - Das System nach Anspruch 1, wobei die prioritätsbezogenen Informationen in einem Prioritätsfeld von jedem der Informationspakete enthalten sind.
- Das System nach Anspruch 1, wobei die prioritätsbezogenen Informationen in einem Ursprungsidentifikationsfeld von jedem der Informationspakete enthalten sind.
- Das System nach Anspruch 1, wobei die Entscheidungseinheit einen Computer umfasst.
- Das System nach Anspruch 1, ferner mit einer Kommunikationsverbindung (
160 ), welche die erste und zweite Eingabeeinrichtung (102 ,104 ) mit der ersten medizinischen Einrichtung koppelt. - Das System nach Anspruch 11, ferner mit einer zweiten medizinischen Einrichtung, wobei die Entscheidungseinheit prioritätsbezogene Informationen in den Informationspaketen verwendet, um entweder der ersten Eingabeeinrichtung oder der zweiten Eingabeeinrichtung das Steuern der zweiten medizinischen Einrichtung zu ermöglichen.
- Das System nach Anspruch 12, wobei die erste medizinische Einrichtung einen Roboterarm (
26 ) umfasst, welcher ein medizinisches Instrument (22 ) bewegt, und die zweite medizinische Einrichtung einen Roboterarm (30 ) umfasst, welcher ein mit einer Kamera (34 ) gekoppeltes Endoskop (32 ) bewegt. - Das System nach Anspruch 13, ferner mit einer Kommunikationsverbindung (
160 ), welche die erste und die zweite medizinische Einrichtung mit der ersten und zweiten Eingabeein richtung koppelt, und mit einem Computer, welcher Roboterdaten von den Roboterarmen mit Videodaten von der Kamera auf die Kommunikationsverbindung multiplext. - Das System nach Anspruch 1, ferner mit Kommunikationsmitteln zum Koppeln der ersten und zweiten Eingabeeinrichtung mit der ersten medizinischen Einrichtung und zum Koppeln der ersten und zweiten Eingabeeinrichtung mit einer zweiten medizinischen Einrichtung, wobei die erste medizinische Einrichtung einen Roboterarm umfasst, welcher ein medizinisches Instrument bewegt und die zweite medizinische Einrichtung einen Roboterarm umfasst, der eine Endoskopkamera bewegt, und ferner mit Kommunikationsmitteln zum Koppeln der ersten und zweiten medizinischen Einrichtung mit der ersten und zweiten Eingabeeinrichtung, und mit Multiplexer-Mitteln zum Multiplexen von Roboterdaten von den Roboterarmen mit Videodaten von der Kamera auf die Kommunikationsmittel.
- Das System nach Anspruch 1, ferner mit: einem ersten Roboterarm (
26 ), der das erste medizinische Instrument (22 ) trägt; einem zweiten Roboterarm (28 ), der ein zweites medizinisches Instrument (24 ) trägt; einem dritten Roboterarm (30 ), der eine Endoskopkamera (34 ) hält; wobei die erste Eingabeeinrichtung den ersten, zweiten und dritten Roboterarm (26 ,28 ,30 ) steuern kann; wobei die zweite Eingabeeinrichtung den ersten, zweiten und dritten Roboterarm (26 ,28 ,30 ) steuern kann; wobei die Entscheidungseinheit mit dem ersten Roboterarm (26 ), dem zweiten Roboterarm (28 ), dem dritten Roboterarm (30 ), der ersten Eingabeeinrichtung und der zweiten Eingabeeinrichtung gekoppelt ist. - Das System nach Anspruch 16, wobei die erste und die zweite Eingabeeinrichtung jeweils ein Informationspaket mit einer Priorität an die Entscheidungseinheit senden, wobei die Priorität von der Entscheidungseinheit verwendet wird, um zu bestimmen, ob der erste und zweite Roboterarm von der ersten Eingabeeinrichtung oder von der zweiten Eingabeeinrichtung gesteuert werden sollen.
- Das System nach Anspruch 16, wobei die erste und zweite Eingabeeinrichtung jeweils ein Informationspaket mit einer Quelladresse an die Entscheidungseinheit senden, wobei die Quelladressen von der Entscheidungseinheit verwendet werden, um zu bestimmen, ob der erste und der zweite Roboterarm von der ersten Konsole oder von der zweiten Eingabeeinrichtung gesteuert werden sollen.
- Das System nach Anspruch 16, ferner mit einer mit der Entscheidungseinheit und der ersten und zweiten Eingabeeinrichtung gekoppelten Kommunikationsverbindung, wobei die Entscheidungseinheit Roboterdaten von dem ersten, zweiten und dritten Roboterarm mit Videodaten von der Kamera auf die Kommunikationsverbindung multiplext.
- Das System nach Anspruch 1, wobei die erste Eingabeeinrichtung ein erstes Informationspaket mit einer ersten Priorität und die zweite Eingabeeinrichtung ein zweites Informationspaket mit einer zweiten Priorität sendet, wobei die Entscheidungseinheit entweder das erste oder das zweite Informationspaket auf der Basis der ersten und zweiten Priorität sendet.
- Das System nach Anspruch 1, wobei die erste und die zweite Eingabeeinrichtung jeweils eine Konsole umfassen, und wobei jede Konsole ein Paar von Griffen und einen Bildschirm umfasst.
- Das System nach Anspruch 1, ferner mit einem Kommunikationsnetz, welches die Entscheidungseinheit umfasst; wobei die erste medizinische Einrichtung ein medizinisches Instrument umfasst, das von einem mit dem Kommunikationsnetz gekoppelten Roboterarm getragen wird; und wobei die erste und zweite Eingabeeinrichtung eine mit dem Kommunikationsnetz gekoppelte erste Chirurgenkonsole und eine mit dem Kommunikationsnetz gekoppelte zweite Chirurgenkonsole umfassen.
- Das System nach Anspruch 22, wobei die erste und zweite Konsole jeweils ein Informationspaket senden, wobei das Paket ein Prioritätsfeld enthält.
- Das System nach Anspruch 1, ferner mit einem die erste Eingabeeinrichtung mit einem Kommunikationsnetz koppelnden Computer, wobei der Computer ein Informationspaket an das medizinische Instrument über das Kommunikationsnetz sendet, wobei das Paket ein Prioritätsfeld und ein Roboterdaten enthaltendes Datenfeld enthält.
- Das System nach Anspruch 24, wobei das Datenfeld RS232-Daten enthält.
- Das System nach Anspruch 24, wobei die erste Eingabeeinrichtung einen Griff umfasst.
- Ein Verfahren zum Steuern einer medizinischen Einrichtung, wobei: eine einer ersten Eingabeeinrichtung (
102 ) zugeordnete Priorität mit einer einer zweiten Eingabeeinrichtung (104 ) zugeordneten Priorität verglichen wird, um zu bestimmen, ob die erste Eingabeeinrichtung oder die zweite Eingabeeinrichtung eine erste medizinische Einrichtung steuern soll; und gemäß diesem Vergleich die erste medizinische Einrichtung mit Eingaben entweder von der ersten Eingabeeinrichtung (102 ) oder der zweiten Eingabeeinrichtung (104 ) gesteuert wird. - Das Verfahren nach Anspruch 27, ferner mit einer zweiten medizinischen Einrichtung, welche einen Roboterarm umfasst, der ein mit einer Kamera gekoppeltes Endoskop steuert, wobei die erste medizinische Einrichtung einen Roboterarm umfasst, wobei Roboterdaten von den Roboterarmen mit Videodaten von der Kamera auf eine Übertragungsverbindung multiplext werden.
- Das Verfahren nach Anspruch 27, wobei ferner: Roboterdaten von einem Roboterarm übertragen werden, welcher das erste medizinische Instrument bewegt; Videodaten von einer mit einem Endoskop gekoppelten Kamera übertragen werden; und die Roboterdaten und die Videodaten auf eine Kommunikationsverbindung multiplext werden.
