- HINTERGRUND DER ERFINDUNG
- Die vorliegende Erfindung betrifft ein Wirbelsäuleninstrumentierungssystem unter Verwendung von länglichen Teilen, die sich entlang der Länge der Wirbelsäule erstrecken und an mehreren Wirbeln mittels Befestigungselementen, wie Knochenschrauben, befestigt sind.
- Früher wurden die Korrektur von Wirbelsäulenerkrankungen und die Behandlung von Wirbelsäulenverletzungen von hinten oder namentlich von dem Rücken des Patienten zugänglich gemacht. Anfangs wurde der Zugang von vorn mit Wirbelsäuleninstrumentierung, das heißt von vorn und seitlich des Patienten, infolge der Ungewohnheit dieses Zugangs für Wirbelsäulenchirurgen und infolge der Gefahr von verschiedenen Komplikationen, wie neurovaskulärer Verletzung oder Gefährdung des Rückenmarks, nicht bevorzugt. Jedoch suchten Chirurgen angesichts einiger berichteter Schwierigkeiten beim Richten der Korrektur von Lendenwirbelsäulen- und Lendenskoliosekrümmungen auf einen Zugang von hinten Formen der Korrektur und Stabilisierung von vorn. Eine solche Technik wurde von Dwyer in Australien während der 1960'er entwickelt, bei welcher ein Hakenschraubenkonstrukt an der konvexen Seite der Skoliosekrümmung angewendet wurde. Die Schrauben wurden mittels eines Drahtes verbunden, und die Korrektur wurde durch Aufbringen von Druckkräften in jeder instrumentierten Ebene erreicht. Die Wirbelsäulenkompression von vorn erzeugte Zugkräfte innerhalb des Drahtes, welcher seinerseits ein korrigierendes Biegemoment in jeder der Wirbelebenen erzeugte.
- Unmittelbar nach dem Erfolg von Dwyer folgte eine andere Instrumentierung von vorn. Ferner begannen Chirurgen zu erkennen, dass bestimmte Wirbelsäulenbehandlungen eher als von hinten am besten von vorn zugänglich waren. Zugänge von vorn geben direkten Zutritt zu dem Bandscheibenraum für Auslösung von vorn und Zwischenkörperfusion. Gegenwärtig umfassen übliche Indikationen für Instrumentierung von vorn: Lendenskoliose mit unzureichenden Elementen von hinten; Lendenwirbelsäulenkrümmungen mit extremer Lordose; paralytische Lendenwirbelsäulenskoliose, die eine Verschmelzung sowohl von vorn als auch von hinten erfordert; Lendenwirbelsäulentrauma, wie Berstungsbrüche; und degenerative Erkrankungen des Wirbelkörpers. Im Falle von Berstungsbrüchen ist es bekannt, dass Neurokompression aus der Richtung von vorn auftritt. Ferner wird häufig geglaubt, dass die Wundreinigung von Bruchstücken von vorn ein wirksameres Mittel zum Dekomprimieren des Wirbelsäulenkanals sei, wie gegenüber bekannten Techniken von hinten.
- Seit der anfänglichen Instrumentierung von Dwyer wurden viele Platten- und Stabsysteme von vorn entwickelt, wie die Systeme von Dunn, Kostuik-Harrington, Zielke und Kaneda. Viele dieser Systeme erlauben eine dynamische Distraktion der Wirbel, gefolgt von direkter Kompression von implantiertem Knochentransplantat, das in dem Raum der operativ entfernten Bandscheibe und nach der Dekompression der neuralen Elemente enthalten ist.
- Viele dieser Systeme von vorn können zu Komplikationen führen. Einige der bedeutenderen Probleme, die aufgetreten sind, umfassen das Fehlen der Befestigungskomponenten und ein oft hohes Eintreten von Verlust an Zurückführung oder Korrektur. Viele der Schwierigkeiten diesbezüglich können aus den wirbeln hergeleitet werden, die an dem Ende des Konstrukts instrumentiert werden, wo die Belastungen auf die Instrumentierung am größten sind. In einigen Fällen wurde eine bikortikale Hebelwirkung von Wirbelkörperschrauben festgestellt, um eine stabilere Fixierung an den Enden des Konstrukts sicherzustellen und gegen Herausfallen der Schrauben zu schützen. Es bleibt jedoch noch ein Bedarf an einer Instrumentierung von vorn, die eine adäquate Korrektur von Wirbelsäulendeformitäten schaffen kann und die leicht implantiert werden kann. Außerdem muss das System eine starke Fixierung sicherstellen, die sich über die Zeit nicht verschlechtert, woraus ein Korrekturverlust resultiert.
- In einigen Fällen wurde erkannt, dass eine Revisionsoperation notwendig ist, selbst wenn die bestmögliche chirurgische Implantation der Instrumentierung folgt. Häufige Indikationen für die Revision von Wirbelsäuleninstrumentierung umfassen die Erweiterung von bestehender Instrumentierung und der Austausch von mangelhaften Implantaten. Da die Implantatgestaltung ausgeklügelter wurde, wurden Fähigkeiten für die Revisionsoperation entwickelt, die für den Patienten relativ sicher und für Implantate unschädlich waren, besonders jene Implantate, die zum Rückhalten beabsichtigt waren.
- Bei Systemen unter Verwendung von Knochenschrauben können Revisionsoperationen den Wirbelkörper bedeutend gefährden. Außerdem können bei bestimmten Zugängen von vorn, wo stärkere Fixierung wesentlich ist, Revisionsverfahren zum Austausch mangelhafter Komponenten notwendigerweise das neue Konstrukt gefährden.
- In Anbetracht dieser Schwierigkeiten gibt es einen Bedarf für ein Wirbelsäulenfixierungssystem, das für die Revisionsoperation gut geeignet ist. Insbesondere muss das System für die Hinzufügung oder Entfernung von Komponenten durch Revision geeignet sein, ohne entweder ein bestehendes Konstrukt aufzugeben oder die Möglichkeit des Implantierens eines neuen, stabileren Konstrukts auszuschließen. Außerdem gibt es einen Bedarf für Revisionstechniken, die eine vollständige Entfernung eines Konstrukts erlauben, sobald eine Fusion eingetreten ist, ohne wiederum die Wirbelsäule oder die Implantate zu gefährden.
- Die US-A-4,987,892 beschreibt ein Wirbelsäulenfixierungssystem, bei welchem Pedikelschrauben über eine gelenkbildende Klemme mit einem länglichen Wirbelsäulenstab verbunden sind. Das System weist einen Klemmbolzen auf, um die Schrauben mit den gelenkbildenden Klemmen zu verriegeln.
- ZUSAMMENFASSUNG DER ERFINDUNG
- Gemäß der vorliegenden Erfindung ist ein System zum Fixieren der Wirbelsäule vorgesehen, aufweisend: ein längliches Teil, das konfiguriert ist, sich entlang eines Abschnitts der Länge der Wirbelsäule zu erstrecken, wobei das Teil ein erstes Ende benachbart zu einem kopfwärtigen Wirbel und ein zweites Ende benachbart zu einem fußwärtigen Wirbel aufweist; dadurch gekennzeichnet, dass das System ferner ein zylindrisches Implantat aufweist, das bemessen ist, um in dem intradiskalen Raum benachbart zu mindestens einem von dem kopfwärtigen und fußwärtigen Wirbel angeordnet zu werden, wobei das zylindrische Implantat eine innere Kammer zum Aufbewahren von Knochenwachstumsmaterial und eine Anzahl von Öffnungen definiert, die mit der inneren Kammer in Verbindung stehen, wobei das Implantat eine hintere Fläche aufweist, die von außerhalb des intradiskalen Raumes zugänglich ist, wenn das zylindrische Implantat darin angeordnet ist, wobei das Implantat ferner eine Längsachse aufweist, die sich von der hinteren Fläche weg erstreckt; ein Befestigungsteil mit der hinteren Fläche des zylindrischen Implantats verbunden ist, wobei das Befestigungsteil einen Kopfabschnitt für die Befestigung an dem länglichen Teil benachbart zu dem mindestens einen Wirbel aufweist; und ein Verbindungsteil das Befestigungsteil stabil an dem länglichen Teil in einer Lage zu der Längsachse des zylindrischen Implantats versetzt befestigt.
- Das System gemäß einer Ausführungsform verwendet ein mit Gewinde versehenes zylindrisches Fusionsimplantat, das zwischen den Endplatten der benachbarten Wirbel platziert oder in diese geschraubt ist. Das Implantat kann mit zerstückeltem Eigenknochen gefüllt sein, um die Fusion durch das Implantat hindurch und zwischen den Wirbelendplatten zu fördern. Das eine Ende des Fusionsimplantats ist seitlich angeordnet und für die Platzierung eines Befestigungsteils zugänglich. Bei einer Ausführungsform weist das freigelegte Ende eine Gewindeöffnung zum Aufnehmen eines Gewindeschaftes des Befestigungsteils auf. Gemäß einem Merkmal der vorliegenden Erfindung weist das Befestigungsteil auch einen Kopfabschnitt auf, der im Wesentlichen wie der Kopf der in die dazwischen liegenden Wirbel eingreifenden Knochenschraube konfiguriert ist. Der Kopfabschnitt definiert einen Schlitz zum Aufnehmen des Körpers einer Ringbolzenanordnung und weist ein Muster von radialer Auszackung auf. Die Kopfabschnitte der Fusionsimplantatbefestigungsteile und der Knochenschrauben sind über ein im Winkel variierbares Verbindungsteil, das einen Ringbolzen, eine Mutter und eine Keilnutscheibe aufweist, mit dem Wirbelsäulenstab in Eingriff. Das Befestigungsteil schafft einen bedeutenden Vorteil dadurch, dass es ein bekanntes Fusionsimplantat anpasst, sich wie eine Knochenschraube mit verbesserter Steifigkeit und Fixierung zu zeigen und zu verhalten.
- Ein anderes Ziel ist es, ein System zu schaffen, das aus den Vorteilen einer Bandscheibenfusionsvorrichtung, die als eine Verankerung mit dem System dient, Nutzen ziehen kann. Andere Ziele und die wesentlichen Vorteile der vorliegenden Erfindung werden bei Betrachtung der folgenden schriftlichen Beschreibung und der beigefügten Figuren deutlich.
- BESCHREIBUNG DER FIGUREN

- Für die Zwecke der Förderung des Verständnisses der Grundsätze der Erfindung wird nun auf die in den Zeichnungen dargestellten Ausführungsformen Bezug genommen, und eine besondere Sprache wird verwendet, um dieselben zu beschreiben. Es wird dennoch verständlich sein, dass keine Einschränkung des Bereichs der Erfindung dadurch beabsichtigt ist, wobei solche Abänderungen und weitere Modifikationen in der dargestellten Vorrichtung und solche weiteren Anwendungen der Grundsätze der Erfindung, wie darin erläutert, betrachtet werden, wie sie normalerweise für Fachleute auf dem technischen Gebiet, auf das sich die Erfindung bezieht, vorkommen würden.
- Mit Bezug nunmehr auf
1 werden Anfangsschritte der chirurgischen Technik erläutert, die durch die vorliegende Erfindung veranschaulicht wird. Insbesondere veranschaulicht die Erfindung die Fixierung von verschiedenen Wirbelsegmenten von vorn, die als Wirbelsäulenwirbel V1–V4 bezeichnet werden, und ihrer benachbarten Bandscheiben D1–D4. Diese Instrumentierung von vorn könnte zum Beispiel verwendet werden zum Korrigieren eines Lendenskolioseleidens, gefolgt von der Fusion der erkrankten Wirbelebenen. Anfangs werden die seitlichen Teile der Wirbelkörper durch eine thorakoabdominale Freilegung aufgedeckt. Eine Bandscheibenentfernung kann in jedem der Bandscheibenräume D1–D4 durchgeführt werden, da diese Bewegungssegmente durch die permanenten Stabilisationsimplantate beseitigt werden. - Unter Verwendung von Messschiebern wird der Abstand der Cortices der dazwischen liegenden Wirbelkörper bestimmt, um die Länge der Knochenschraube veranschlagen zu können, die seitlich in den Wirbel zu platzieren ist. In diesem Beispiel wird der dazwischen liegende Wirbel V2 mit einer Schraube
30 mit variierbarem Winkel instrumentiert. Eine bevorzugte Schraube mit variierbarem Winkel ist ausführlich im U.S. Patent Nr. 5,261,909 von Dr. Chester Sutterlin, u. a. beschrieben. Die Offenbarung dieses '909 Patents betrifft die Beschreibung der Schraube30 mit variierbarem Winkel und deren Eingriff mit einem Wirbelsäulenstab. Für die Zwecke der vorliegenden Offenbarung weist die Schraube30 mit variierbarem Winkel einen Knocheneingriffsabschnitt31 oder Gewindeschaft auf, welcher bemessen ist, um sich vorzugsweise über den Abstand zwischen den Cortices an den seitlichen Rändern des Wirbelkörpers V2 zu erstrecken. Die Schraube30 weist einen Kopfabschnitt33 auf, welcher einen Schlitz 35 zum Aufnehmen eines Ringbolzenkörpers da hindurch definiert. Der Kopfabschnitt33 weist auch eine Mehrzahl von radialen Keilnuten37 auf, die daran zum gegenseitigen Eingriff mit einer zugehörigen Scheibe ausgebildet sind, die unten erläutert wird. - Bei der dargestellten Ausführungsform werden nur vier Wirbelebenen instrumentiert. Demzufolge werden die obersten und untersten Bandscheibenräume D1 und D4 mit einer hohlen Schraube instrumentiert, die konfiguriert ist, um den Grad der Fixierung an den benachbarten Wirbeln zu erhöhen. Insbesondere weist die hohle Schraube einen ersten Abschnitt in der Weise eines zylindrischen Implantats
10 auf. Vorzugsweise ist dieses Implantat10 eine Fusionsvorrichtung, die ein Knochenwachstum zur Fusion und permanenten Fixierung erlaubt. Gemäß der bevorzugten Ausführungsform der vorliegenden Erfindung ist dieses zylindrische Implantat10 ein mit Gewinde versehenes Wirbelsäulenimplantat, wie im U.S. Patent Nr. 5,015,247 von Dr. Gary Michelson beschrieben ist. Andere Fusionsvorrichtungen werden in Erwägung gezogen, wie die Zwischenkörperfusionsvorrichtung von Dr. Kozak, die im U.S. Patent Nr. 5,397,364 beschrieben ist. Diese Vorrichtung von Kozak und andere Fusionsvorrichtungen können für die seitliche Einführung in den intradiskalen Raum zur praktischen Anwendung bei der chirurgischen Technik der vorliegenden Erfindung modifiziert werden. - Zum Zwecke der vorliegenden Erfindung definiert das zylindrische Implantat
10 eine hohle innere Kammer12 , welche mit Knochentransplantatmaterial oder Knochenstückchen gefüllt werden kann. Eine Mehrzahl von Öffnungen14 sind durch das Implantat hindurch definiert, um mit dem hohlen Inneren in Verbindung zu stehen, um ein Gewebewachstum zu ermöglichen. Eine mit Gewinde versehene Endkappe15 umschließt ein offenes Ende des Implantats, um ein Füllen des Implantats zu ermöglichen und Knochenstückchen in der hohlen inneren Kammer12 vor der Implantation aufzubewahren. Die äußere Fläche des Implantats10 weist daran geformte Knocheneingriffsgewinde16 auf, welche konfiguriert sind, in die Endplatten der benachbarten Wirbel eingeschraubt zu werden. Das zylindrische Implantat10 weist eine hintere Fläche18 auf, welche seitlich freigelegt ist, wenn das Implantat10 in den Bandscheibenraum D1 eingeschraubt ist. Diese hintere Fläche18 definiert eine Gewindeöffnung20 da hindurch. Weitere Einzelheiten der Konstruktion dieses bevorzugten zylindrischen Implantats können aus dem '247 Patent entnommen werden. - Das zylindrische Implantat
10 kann zwischen den beiden Paaren von benachbarten Wirbeln in der in dem '247 Patent offenbarten Weise implantiert werden, welche Technik in den Spalten9 –10 beschrieben ist und durch Bezugnahme einbezogen ist. Sobald die Stelle in dem Bandscheibenraum vorbereitet wurde, kann ein Einsetzstab23 , der einen Gewindezapfen25 aufweist, mit der Gewindeöffnung20 in der hinteren Fläche18 des Implantats in Eingriff gebracht werden. Ein Führungsteil27 umgibt konzentrisch den Einsetzstab, wobei der Stab speziell durch eine Bohrung29 in dem Führungsteil hindurch tritt. Der Einsetzstab23 und das Führungsteil27 werden verwendet, um jedes der zylindrischen Implantate10 in deren jeweilige intradiskale Räume D1 und D4 vorzutreiben. Wiederum sind weitere Einzelheiten der Art und Weise der praktischen Anwendung des hohlen Implantats10 ausführlicher in dem '247 Patent offenbart. Es ist verständlich, dass andere ähnliche intradiskale Implantate verwendet werden könnten, vorausgesetzt, dass das Implantat eine Gewindeöffnung in deren hinteren Fläche, wie die Gewindeöffnung20 in der hinteren Fläche18 , oder irgendein Äquivalent dazu aufweist. Zum Beispiel kann die Zwischenkörperfusionsvorrichtung des oben einbezogenen '364 Patents von Kozak modifiziert werden, um eine ähnliche Gewindeöffnung in einer seitlichen Fläche der Vorrichtung einzubeziehen. - Mit Bezug nunmehr auf
2 sind die instrumentierten Segmente zu sehen, wobei die hohlen Implantate10 vollständig in den Bandscheibenräumen D1 und D4 angeordnet sind. Gleichfalls ist die Schraube30 mit variierbarem Winkel vollständig in den Wirbelkörper V2 eingesetzt. Vorzugsweise sitzt die Schraube30 mit dem Schraubenkopf33 bündig an der Seitenfläche des Wirbels. - An den gegenüberliegenden Verankerungsenden des Konstrukts in den Bandscheibenräumen D1 und D4 ist ein Befestigungskopf
40 mit dem jeweiligen zylindrischen Implantat10 in Eingriff. Besonders weist der Befestigungskopf40 eine Basis42 auf, von welcher ein Gewindeschaft44 vorsteht. Der Gewindeschaft44 ist angepasst, um in die Gewindeöffnung20 in der hinteren Fläche18 des zylindrischen Implantats10 einzugreifen. Der Befestigungskopf40 weist einen Kopfabschnitt46 auf, welcher im Wesentlichen gleich dem Kopfabschnitt33 der oben beschriebenen Schraube30 mit variierbarem Winkel ist. Insbesondere weist der Kopfabschnitt46 einen Ringbolzenschlitz47 auf und ist mit radialen Keilnuten48 an einer Fläche des Kopfabschnitts ausgestattet. Jedes der zylindrischen Implantate10 nimmt einen entsprechenden Befestigungskopf40 auf. - Vorzugsweise werden die Kopfabschnitte
46 der Befestigungsköpfe40 und der Kopfabschnitt33 der Schraube30 mit variierbarem Winkel von dem kopfwärtigen zu dem fußwärtigen Ende der instrumentierten Segmente in einer geraden Linie gehalten. Mit dem operativen Zugang der vorliegenden Erfindung ist es nicht nötig, eine besondere Derotation oder Schraubenplatzierungstechniken zu verwenden, um eine Lordose in den instrumentierten Segmenten zu erzeugen. Stattdessen wird, wie in den nächsten in3 dargestellten Schritten des Verfahrens vorgesehen ist, ein Stab50 mit einer Krümmung C profiliert, um die Lendenlordose oder Brustkyphose in der Sagittalebene zu erzeugen. Im Allgemeinen wird der Stab50 profiliert, um mit der Skoliosekrümmung übereinzustimmen, wobei der Stab an der konkaven Seite der Krümmung angeordnet ist. - Sobald der Stab profiliert wurde, wird dieser an jedem der Kopfabschnitte
33 und46 mittels einer Keilnutscheibe52 und eines Verbindungsteils, vorzugsweise in der Form einer Ringbolzenanordnung, befestigt. Wie in3 gezeigt ist, weist die Ringbolzenanordnung einen Körper43 auf, welcher sich sowohl durch die Keilnutscheibe52 als auch durch die Schlitze35 und47 in den Kopfabschnitten33 der Schraube mit variierbarem Winkel bzw. den Befestigungskopfabschnitten46 hindurch erstreckt. Diese Einzelheiten des Ringbolzenkonstrukts sind deutlicher in4 gezeigt. Es wird auch erkannt, dass dieselbe Ringbolzenbefestigungsanordnung in dem '909 Patent beschrieben ist. - Mit Bezug auf
4 ist es zu ersehen, dass die Befestigung des Kopfabschnitts46 des Befestigungskopfes40 mit der dem Stab50 zugewandten Keilnutfläche des Kopfabschnitts erreicht wird. Die Keilnutscheibe52 ist zwischen dem Stab und der Keilnutfläche des Befestigungskopfes40 angeordnet, spezieller mit deren entsprechenden Keilnuten, die mit den Keilnuten48 ineinandergreifen. Wie ausführlicher in dem '909 Patent erläutert, ermöglichen die ineinandergreifenden Keilnuten eine Ausrichtung des Stabes50 in einer Vielzahl von Winkeln in der Sagittalebene relativ zu dem Befestigungskopf40 . Während die1 –3 zur Übersichtlichkeit die Fixierungsschrauben im Wesentlichen parallel zueinander zeigen, sind in Wirklichkeit die Kopfabschnitte33 und46 in verschiedenen Winkeln in der Frontalebene in Abhängigkeit von der korrigierten Wirbelsäulenkrümmung ausgerichtet. - Die Ringbolzenanordnung selbst weist einen Ringbolzenkörper
53 auf, welcher eine Öffnung zum Aufnehmen des Wirbelsäulenstabes50 aufweist. Ein Abschnitt des Ringbolzenkörpers53 ragt durch Keilnutscheibe52 hindurch. Von dem Körper erstreckt sich ein mit Gewinde versehener Ringbolzenschaft54 , welcher eine Mutter55 aufnimmt. Das Anziehen der Mutter55 an dem Schaft54 klemmt das gesamte Konstrukt, nämlich den Stab50 , die Keilnutscheibe52 und den Befestigungskopf40 , zusammen. Eine ähnliche Ringbolzenanordnung wird für den Eingriff der Schraube30 mit variierbarem Winkel mit dem Wirbelsäulenstab50 verwendet. - Bei diesem Schritt des Verfahrens wird jeder der Ringbolzen für den Sitz des Stabes und der Schraubenköpfe ausreichend angezogen, während noch ein Drehen des Stabes in den Öffnungen der Ringbolzenanordnungen ermöglicht wird. Ein Sechskantende
51 ist an dem Stab50 vorgesehen, so dass der Stab gedreht werden kann, während er noch an der Schraube30 mit variierbarem Winkel und den hohlen Schrauben10 befestigt wird. Ein Rollen oder Drehen des Stabes in dieser Weise versetzt die vorbestimmte Skoliosekrümmung in die Sagittalebene, um die erforderliche Lordose an den instrumentierten Wirbelsegmenten zu erzeugen. Sobald der Stab in seine genaue Position gerollt wurde, um die lordotische Krümmung zu erzeugen, kann eine segmentale Dekompression oder Kompression ausgeübt werden, um irgendeine Deformität in der Frontalebene weiter zu korrigieren. Es wurde festgestellt, dass aus der Instrumentierung der Wirbelkörper von diesem Zugang von vorn typischerweise eine vollständigere und zuverlässigere Dekompression des Wirbelsäulenkanals als mit anderen Zugängen, wie einer Instrumentierung von hinten, resultiert. Sobald die passenden Bandscheibenhöhen wieder hergestellt wurden, mit denen zusammen die genaue Krümmung in der Frontal- und Sagittalebene erreicht wurde, werden die Ringbolzen angezogen, um den Stab50 an jedem der an der Wirbelsäule angebrachten Fixierungselemente festzuklemmen. - Gemäß der bevorzugten Ausführungsform ist der Wirbelsäulenstab zu der Längsachse des zylindrischen Implantats (d. h. der Achse, die sich durch die Gewindeöffnung
20 hindurch erstreckt) versetzt. Vorzugsweise ist der Wirbelsäulenstab hinter den Kopfabschnitten positioniert, an welchen der Stab befestigt ist. Diese Platzierung ermöglicht, dass der Ringbolzenschaft und die Mutter von vorn zu dem Bauchraum hin und von empfindlicheren Bereichen weg gerichtet sind. (Es wird angemerkt, dass zur Übersichtlichkeit die in den1 –3 gezeigten Kopfabschnitte 180° von dieser bevorzugten Ausrichtung gedreht gezeigt wurden.) - Vorzugsweise ist jedes der hohlen Implantate
10 mit zerstückeltem Knochentransplantatmaterial, wie Eigentransplantat, gefüllt. Darüber hinaus kann nach Wahl des Chirurgen der übrige Bandscheibenraum auch mit zerstückeltem Knochenmaterial gefüllt werden. Ein Ziel der Verwendung des hohlen geöffneten zylindrischen Implantats10 und des Knochentransplantatmaterials ist es, eine Fusion quer über die intradiskalen Räume herbeizuführen. Sobald die Fusion eintritt, werden die zylindrischen Implantate10 , welche die Verankerung an den gegenüberliegenden Enden des Konstrukts schaffen, stabiler als andere bekannte Knochenschraubenkonstrukte an der Wirbelsäule befestigt. Es wird angenommen, dass die Verwendung des Fusionsimplantats10 mit den dazwischen liegenden Wirbelsegmenten nicht wesentlich ist, da die Belastungen in den dazwischen liegenden Abschnitten nicht so hart sind, als an den Enden der instrumentierten Wirbelsegmente. - Bei speziellen Ausführungsformen kann die Schraube
30 mit variierbarem Winkel eine 5,5, 6,5 oder 7,5 mm Spongiosaschraube sein. Die zylindrischen Implantate10 können einen Außengewindedurchmesser von zwischen 10 mm–24 mm haben. Ferner können Stäbe mit 6,4 oder 4,8 mm (1/4 oder 3–16 inch) Durchmesser zusammen mit passend bemessenen Ringbolzenanordnungen verwendet werden. Es ist natürlich verständlich, dass sowohl die Länge des spongiösen Gewindeabschnitts der Schraube mit variierbarem Winkel als auch der Durchmesser des zylindrischen Implantats10 durch die Geometrie der speziellen instrumentierten Wirbelsegmente bestimmt werden. Es wird in Erwägung gezogen, dass die vorliegende erfinderische Operationstechnik in allen Ebenen der Wirbelsäule mit passend bemessenen Implantaten verwendet werden kann. - Es wurde festgestellt, dass selbst mit der besten präoperativen Planung irgendeine Revision des Fixierungskonstrukts erforderlich sein kann. Außerdem kann ein vollständiges Versagen des Konstrukts vor der Fusion auch eine vollständige oder teilweise Revision des Implantats erfordern. In einigen Fällen erfordert die Revision ein Wegschneiden des Wirbelsäulenstabes
50 und Abschrauben jeder der Knochenschrauben, die in die Wirbel eingreifen. Mit dem(r) vorliegenden erfinderischen vorn und seitlich liegenden Zugang und Instrumentierung ist eine verbesserte Revisionstechnik und Instrumentierung verfügbar. Unter Verwendung des vorliegenden Konstrukts wird die Entfernung des Stabes extrem einfach gemacht und einfach auf den Schritt des Lösens der Ringbolzen reduziert, die an jedem der Kopfabschnitte an den verschiedenen Wirbelsegmenten befestigt sind. Die Schraube30 mit variierbarem Winkel wird durch Abschrauben der Schraube von dem Wirbelkörper V2 leicht entfernt. An dem kopfwärtigen und fußwärtigen Ende des Konstrukts werden die Befestigungsköpfe40 von ihren jeweiligen zylindrischen Implantaten10 abgeschraubt. An diesem Punkt ist das Revisionsverfahren relativ einfach. Jedoch können Schwierigkeiten beim Entfernen der zylindrischen Implantate10 aus ihren jeweiligen intradiskalen Räumen D1 und D4 auftreten. Die Entfernung der Implantate ist besonders schwieriger, wenn Knochenwachstum zwischen den Wirbelendplatten und dem hohlen Vorderen12 des Implantats eingetreten ist. - Das Entfernen dieser Fusionsimplantate
10 ist ein wichtiges Merkmal der neuen Revisionstechnik der vorliegenden Erfindung und der zugehörigen Revisionsinstrumente60 . Die Instrumente60 weisen einen Ausrichtstab62 auf, welcher ein aufgeweitetes Ende63 mit einem davon vorstehenden Gewindeschaft64 hat. Der Gewindeschaft64 ist konfiguriert, um in die Gewindeöffnung20 in der hinteren Fläche18 des zylindrischen Implantats10 einzugreifen. Bei dem Revisionsverfahren wird der Ausrichtstab62 mit dem zylindrischen Implantat10 durch den Gewindeschaft64 und die Gewindeöffnung20 in Eingriff gebracht. - Mit dem so positionierten Ausrichtstab
62 wird eine Trephine70 über den Stab geschoben. Die Trephine70 weist ein rohrförmiges Schneidende71 auf, welches eine Mehrzahl von gezackten Schneidzähnen72 definiert. Vorzugsweise sind die Schneidzähne72 von bekannten Konfigurationen zum Schneiden durch einen kortikalen Knochen hindurch, wenn das Schneidende71 in der korrekten Richtung gedreht wird. Das Schneidende71 definiert ferner eine Mehrzahl von Tiefenmarkierungen oder Ringen74 , welche benutzt werden können, um die Tiefe der Trephine70 in dem intradiskalen Raum zu bestimmen. Die Trephine70 weist eine Drehhülse76 an dem proximalen Ende der Trephine auf, welche konzentrisch um den rohrförmigen Körper des Schneidendes71 herum angeordnet ist. Das proximale Ende der Trephine definiert auch eine Antriebsnabe78 , welche konfiguriert ist, um ein herkömmliches Antriebswerkzeug oder einen geeigneten Schraubenschlüssel zum Drehen des Schneidendes aufzunehmen. Schließlich weist das nächstliegende Ende der Trephine70 eine Ausrichtstabklemme80 auf, welche zum Klemmen des proximalen Endes des Ausrichtstabes62 benutzt wird, der konzentrisch in der Trephine aufgenommen ist. - Mit Bezug auf
6 kann gesehen werden, dass der Ausrichtstab62 mit dem zylindrischen Implantat10 in Eingriff ist, das in dem Bandscheibenraum D4 angeordnet ist. Die Trephine70 wird über den Ausrichtstab62 geschoben, bis der Schneidrand72 die Wirbelkörper V3 und V4 kontaktiert. Die Drehung der Trephine bewirkt, dass die Schneidzähne72 einen Kern84 aus Knochenmaterial ausbohren, welches das zylindrische Implantat10 umgibt. Der Kern84 kann dann zusammen mit dem Implantat10 zuerst durch Zurückziehen der Trephine70 und dann durch Ziehen des Ausrichtstabes zum Entfernen der gesamten Anordnung entfernt werden. In7 ist das zylindrische Implantat10 zu sehen, das von dem Ausrichtstab62 getragen wird, wenn die gesamte Revisionsinstrumentierung60 von der Operationsstelle zurückgezogen wird. - Die Tiefenmarkierungen
74 können entweder zum Begrenzen oder Bestimmen der Einsetztiefe der Trephine verwendet werden. Bei deren Tiefenbegrenzungsfunktion ist die Tiefe des distalen Endes des zylindrischen Implantats bereits bekannt und wird speziell auf eine der Tiefenmarkierungen korreliert. Die Trephine wird dann in die Wirbel hinein geschoben, bis die passende Tiefenmarkierung zu dem Wirbelkörper ausgerichtet ist. Die Markierungen74 können auch interaktiv verwendet werden, wenn das Einsetzen der Trephine unter indirekter Betrachtung überwacht wird. Sobald es zu sehen ist, dass der Schneidrand72 der Trephine das Ende des zylindrischen Implantats passiert hat, kann die passende Tiefenmarkierung vermerkt werden. Sobald das Implantat herausgezogen wurde, wird die vermerkte Tiefenmarkierung dann benutzt, um die genaue Größe des Knochendübels zu bestimmen, der aus einer anderen Stelle herauszuziehen ist, um zurück in den intradiskalen Raum eingesetzt zu werden. - Sobald der Kern
84 des Knochens und des zylindrischen Implantats10 aus dem Bandscheibenraum herausgeschnitten wurde, kann ein genau angepasster Stöpsel oder Dübel wieder in die übrig gebliebene Bohrung eingesetzt werden. Die Trephine70 kann benutzt werden, um Knochen von dem Patienten, wie aus dem Darmbeinkamm, auszukernen, welcher dann wieder unter Verwendung derselben Trephine eingesetzt werden kann. Sobald die Trephine entfernt ist, kollabieren die benachbarten Wirbel um den neu eingesetzten Knochenstöpsel herum. Dieser Knochendübel hält die Bandscheibenhöhe bei und erlaubt wieder die Fusion an der Revisionsstelle. Darüber hinaus kann, sobald eine Fusion eintritt, die Stelle des Knochenstöpsels stark genug sein, um neue Wirbelsäulenimplantate zu tragen. - Die vorliegende Revisionstechnik gewinnt einen bedeutenden Vorteil dadurch, dass sie die Verwendung von hohlen zylindrischen Fusionsimplantaten in irgendeiner Position entlang des Konstrukts ermöglicht. Es wurde festgestellt, dass die Verwendung von Fusionsimplantaten die Steifigkeit und Festigkeit der Fixierung erhöht. Dieselbe erhöhte Steifigkeit und Festigkeit macht es schwierig oder unmöglich, normale chirurgische Revisionen vorzunehmen, ohne einen ernsthaften Schaden zu verursachen, der anderenfalls einem gesunden Wirbelkörper zugefügt werden kann. Die Verwendung der Trephine
70 gemäß der vorliegenden Technik schließt dieses Risiko aus und erlaubt ein einfaches und schnelles Entfernen des geschraubten zylindrischen Implantats, selbst wo eine Fusion eingetreten ist. - Während die Erfindung ausführlich in den Zeichnungen und der vorhergehenden Beschreibung dargestellt und beschrieben wurde, ist dieselbe als erläuternd und nicht beschränkend im Charakter zu betrachten, wobei es verständlich ist, dass nur die bevorzugten Ausführungsformen gezeigt und beschrieben wurden, und dass alle Änderungen und Modifikationen geschützt sein sollen, die in den Bereich der wie durch die beigefügten Ansprüche definierten Erfindung fallen. Zum Beispiel wird bei der bevorzugten Ausführungsform der Instrumentierungstechnik von vorn ein hohles zylindrisches Fusionsimplantat verwendet, um die Befestigungsköpfe für den Eingriff mit einem Wirbelsäulenstab zu tragen. Jedoch können andere intradiskale Implantate verschiedener Konfigurationen verwendet werden, vorausgesetzt, dass die Implantate ein Knochenwachstum erlauben. Ferner können, da ein Wirbelsäulenstab offenbart ist, andere Längsfixierungselemente, wie Wirbelsäulenplatten, beim Durchführen dieser Technik von vorn verwendet werden.
Claims (12)
- System zum Fixieren der Wirbelsäule, aufweisend: ein längliches Teil (
50 ), das konfiguriert ist, sich entlang eines Abschnitts der Länge der Wirbelsäule zu erstrecken, wobei das Teil (50 ) ein erstes Ende benachbart zu einem kopfwärtigen Wirbel und ein zweites Ende benachbart zu einem fußwärtigen Wirbel aufweist; dadurch gekennzeichnet, dass das System ferner ein zylindrisches Implantat (10 ) aufweist, das bemessen ist, um in dem intradiskalen Raum (D) benachbart zu mindestens einem von dem kopfwärtigen und fußwärtigen Wirbel (V) angeordnet zu werden, wobei das zylindrische Implantat eine innere Kammer (12 ) zum Aufbewahren von Knochenwachstumsmaterial und eine Anzahl von Öffnungen (14 ) definiert, die mit der inneren Kammer in Verbindung stehen, wobei das Implantat eine hintere Fläche (18 ) aufweist, die von außerhalb des intradiskalen Raumes zugänglich ist, wenn das zylindrische Implantat darin angeordnet ist, wobei das Implantat ferner eine Längsachse aufweist, die sich von der hinteren Fläche weg erstreckt; ein Befestigungsteil (40 ,42 ,44 ) mit der hinteren Fläche des zylindrischen Implantats verbunden ist, wobei das Befestigungsteil einen Kopfabschnitt (40 ) für die Befestigung des länglichen Teils (50 ) benachbart zu dem mindestens einen Wirbel (V) aufweist; und ein Verbindungsteil (52 –55 ) das Befestigungsteil stabil an dem länglichen Teil in einer Lage zu der Längsachse des zylindrischen Implantats versetzt befestigt. - System zum Fixieren nach Anspruch 1, wobei das zylindrische Implantat (
10 ) eine Gewindeöffnung (20 ) in der hinteren Fläche (18 ) definiert; und das Befestigungsteil (40 ,42 ,44 ) einen Gewindeschaft (44 ) für den Eingriff mit dem zylindrischen Implantat in der Gewindeöffnung aufweist. - System zum Fixieren nach Anspruch 1, wobei der Kopfabschnitt (
40 ) und das Verbindungsteil (52 –55 ) gegenseitig ineinandergreifende Keilnuten (48 ) aufweisen, um eine stabile Befestigung des zylindrischen Teils (10 ) an dem länglichen Teil (50 ) in variierbaren Winkeln zu ermöglichen. - System zum Fixieren nach Anspruch 1, ferner aufweisend eine Anzahl von Knochenschrauben (
30 ), die in den Wirbelkörper der entsprechenden Wirbel (V) eingreifen, wobei jede der Anzahl von Knochenschrauben einen Kopfabschnitt (52 ) zum Aufnehmen eines Verbindungsteils aufweist, um die Knochenschraube (30 ) stabil an dem länglichen Teil (50 ) zu befestigen. - System zum Fixieren nach einem der obigen Ansprüche, wobei das Implantat (
10 ) eine äußere Fläche mit mindestens einem Gewinde (16 ) aufweist. - System zum Fixieren nach einem der obigen Ansprüche, ferner aufweisend eine Endkappe (
15 ), die konfiguriert ist, um in die hintere Fläche (18 ) des Implantats (10 ) zusammenwirkend einzugreifen. - System zum Fixieren nach einem der obigen Ansprüche, wobei das längliche Teil (
50 ) eines von einer Stange, einem Kabel und einer Platte ist. - System zum Fixieren nach einem der obigen Ansprüche, in Kombination mit einem Einsetzinstrument (
23 ) zum Einsetzen des Implantats (10 ) in die Wirbelsäule hinein. - System zum Fixieren nach einem der obigen Ansprüche, in Kombination mit einer Trephine (
70 ) zum Erzeugen einer Bohrung in der Wirbelsäule. - System zum Fixieren nach einem der obigen Ansprüche, in Kombination mit mindestens einem von einem Transplantat und einem Dübel zum Einsetzen in die Wirbelsäule.
- System zum Fixieren nach einem der obigen Ansprüche, in Kombination mit einem Knochenwachstumsförderungsmaterial.
- System zum Fixieren nach Anspruch 11, wobei das Knochenwachstumsförderungsmaterial einen Knochen aufweist.
