Description
- Die vorliegende Erfindung betrifft einen distalen Zentrierer für den Schaft einer intramedullären Prothese, wie beispielsweise ein Oberschenkelimplantat. Insbesondere betrifft die Erfindung einen distalen Zentrierer mit vier Elementen, die sich davon in einer Richtung im Wesentlichen senkrecht zum Schaft nach außen erstrecken, wobei benachbarte Rippen in der proximal distalen Richtung versetzt zueinander sind.
- BESCHREIBUNG DES STANDES DER TECHNIK
- Distale Zentrierer werden verwendet, um den Schaft eines zementierbaren prothetischen orthopädischen Implantats in einer neutralen Position im Markkanal eines Knochens anzuordnen. Dadurch ist es möglich, einen gleichmäßigen Knochenzementmantel zwischen dem Implantat und dem Knochen zu bilden.
- Allgemein sind Zentrierer nach dem Stand der Technik als sich verjüngende Ringe mit Rippen um deren Umfang, die sich radial davon nach außen erstrecken, gebildet. Der Zentrierer wird über der Spitze des Schaftes angeordnet und entlang eines sich verjüngenden Abschnitts des Schaftes geführt. Üblicherweise erfolgt die Fixierung des Schaftes durch Bilden einer passenden Verjüngung an der Innenseite des Zentrierers entsprechend der Verjüngung des Schaftes.
- Zentrierer, wie z. B. in der Britischen Patentanmeldung 2 104 391 gezeigt, werden verwendet, um den distalen Schaft im Oberschenkelkanal zu zentrieren. Eine weitere Zentriervorrichtung unter Verwendung von sich vertikal erstreckenden Elementen ist in der US-Patentschrift 3,793,650 gezeigt, die auf die Erfinder der US-Patentanmeldung 2 104 391 ausgestellt ist. Beim „Exeter-Universal-Hip"-System ist eine Manschette aus Polyethylen gezeigt, die derjenigen aus der US- Patentschrift 3,793,650 darin ähnlich ist, dass die Federelemente vertikal verlaufen. Man beachte, dass diese Abstandhalter nicht nur dazu dienen, das distale Ende der Prothese zu zentrieren, sondern auch verhindern, dass der Schaft tatsächlich in die Wand des Markkanals eingreift.
- Die EP-A-0 496 089 beschreibt eine Oberschenkelkopf-Schaftprothese, bei der an einigen wenigen Stellen am Umfang des Schaftes Noppenreihen gebildet sind. Diese können in einer proximal distalen Richtung entlang der Längsachse des Schaftes seitlich versetzt sein.
- Ein Nachteil bei einigen der Vorrichtungen nach dem Stand der Technik besteht darin, dass sie bei der Verwendung von Zement dazu neigen, Hohlräume und Luftblasen im Zement dahinter zu bilden, wenn sie nach unten geschoben werden; diese Hohlräume und Luftblasen bilden sich hinter den Rippen, d. h. zwischen den Rippen und dem Metallschaft der Prothese. Dies sollte vermieden werden, da Schwachstellen und Risse innerhalb des Zementmantels entstehen können.
- Manschetten aus Polymethyl-Methacrylat sind bereits bekannt und wurden von Leo. A. Whiteside et al. in einem im Juni 1988 unter dem Titel „The Effects of the Collar on Total Hip Femoral Component Subsidence" veröffentlichten Artikel beschrieben und als „Whiteside Total Hip System" verkauft. Diese Vorrichtung ist jedoch lediglich eine zylindrische Manschette, die den distalen Schaft nur in einer präzise gefrästen Markhöhle zentralisieren kann.
- Der Aufbau der vorliegenden Erfindung weist vier Rippen auf, die in gleichen Abständen um den Umfang des Ringkörpers des Zentrierers angeordnet sind. Dadurch wird die Zentriereigenschaft des Zentrierers in einem nicht-kreisförmigen Markkanal verbessert. Obwohl bei einer größeren Anzahl von Rippen die Zentrierfähigkeit des Zentrierers verbessert ist, kann bei einem derartigen Aufbau der Zementfluss um die Rippen infolge des Einsetzens des Schaftes mit dem daran befestigten Zentrierer in die Höhle abreißen.
- Zusätzlich zu den Rippen, die in gleichen Abständen angeordnet sind, sind gegenüberliegende Paare der vier Rippen in der proximal distalen Richtung, d. h. der Längsrichtung zueinander versetzt angeordnet. Durch diesen Versatz der Rippen wird die Ausbreitung etwaiger Unstetigkeiten im frischen Zement über eine einzelne Ebene hinaus begünstigt. Die Wahrscheinlichkeit der Rissfortplanzung durch den Zementmantel in einer Ebene ist dadurch geringer. Es wurde festgestellt, dass der Winkel zwischen der Längsachse des Schaftes und der Vorderkante jeder der befestigten Zentrierrippen 135° oder mehr betragen sollte, so dass die Anzahl der durch das Einsetzen des Teils entstehenden Zementfehler aufgrund der optimierten Strömungsdynamik verringert ist. Es wurde ebenso festgestellt, dass der Winkel zwischen der Hinterkante jeder Rippe und der Längsachse des Oberschenkelteils weniger als 90° betragen sollte, um Fehler im Mantel zu reduzieren. Da die Rippenpaare zueinander versetzt angeordnet sind und der Ring von konstanter Länge ist, nimmt der sich verjüngende Ring eine wellenähnliche Form an. Folglich bleibt im Querschnitt in Längsrichtung betrachtet die proximal distale Abmessung des Rings konstant, wodurch der wellenförmige Aufbau entsteht. Es wurde festgestellt, dass der Abstand zwischen distalen und proximalen Enden des Zentrierers maximal 10 mm betragen sollte. Oberhalb 10 mm ist es schwierig, den Zementfluss oberhalb und unterhalb des Rings während des Einsetzens in den Markkanal wiederzuverbinden. Dies ist insbesondere der Fall, wenn der Zentrierer bei einem Schaft mit darin gebildeten Rillen oder Vertiefungen verwendet wird, durch die der Zement oberhalb und unterhalb des Zentrierers fließen kann.
- ZUSAMMENFASSUNG DER ERFINDUNG
- Es ist eine Aufgabe der Erfindung, einen Zentrierer zu schaffen, bei dem die Fähigkeit, einen Schaft bei Verwendung von Zement in einer Markhöhle zu zentrieren, verbessert ist.
- Es ist eine weitere Aufgabe der Erfindung, einen Zentrierer zu schaffen, der zuverlässig einen hohlraumfreien Mantel im Bereich des Zentrierers und der Spitze des Schaftes erzeugt.
- Diese und weitere Aufgaben werden gelöst durch einen Zentrierer in Form eines ringförmigen hohlen Körpers mit einer Innenfläche zur Aufnahme des Schaftes eines Implantats. Der Zentrierer hat wenigstens drei, vorzugsweise vier Rippen, die um die Außenfläche des ringförmigen Körpers herum angeordnet sind, wobei sich die Rippen von der Fläche in Richtung im Wesentlichen senkrecht zur Längsachse des Schaftes nach außen erstrecken. Wenigstens eine der Rippen ist von den benachbarten Rippen in proximal distaler Richtung versetzt angeordnet. Üblicherweise sind bei Verwendung von vier Rippen die Rippen in gleichen Abständen um den ringförmigen Körper herum angeordnet, wobei sich gegenüberliegende Paare im selben proximal distalen Abstand, jedoch versetzt vom benachbarten gegenüberliegenden Paar befinden.
- Die proximalen und distalen Endflächen des ringförmigen Körpers sind in einem vorbestimmten Abstand voneinander entfernt angebracht. Die Rippen haben eine Grundfläche benachbart zum ringförmigen Körper, die sich über einen wesentlichen Teil des vorbestimmten Abstands hinweg erstreckt, wobei die Endflächen wellenförmig verlaufen, um die Versetzung der benachbarten Rippen in proximal distaler Richtung auszugleichen. Folglich ist der Abstand zwischen den proximalen und den distalen Endflächen des Rings im 360°-Verlauf um den Umfang des ringförmigen Körpers konstant.
- Der ringförmige Körper verjüngt sich in dessen Längsrichtung in Richtung zur Spitze des Schaftes, wobei die gegenüberliegenden Rippen der vier Rippen an denselben proximal distalen Stellen am ringförmigen Körper angeordnet sind. Darüber hinaus nimmt die Länge der Rippen in proximal distaler Richtung nach außen in einer Richtung im Wesentlichen senkrecht zur Längsachse ab, wobei die Rippen mit der Außenfläche des Zentrierers in Richtung zur Spitze des Schaftes einen Winkel größer 135° und mit der Hinterkante des ringförmigen Körpers des Zentrierers, d. h. mit dem von der Schaftspitze entfernten Ende, einen Winkel kleinerer als 90° bilden.
- Diese und weitere Aufgaben und Vorteile der vorliegenden Erfindung ergeben sich aus der folgenden Beschreibung der beigefügten Zeichnungen, die verschiedene Ausführungsformen der Erfindung zeigen. Selbstverständlich dienen die Zeichnungen der Darstellung und nicht als Definition der Erfindung.
- KURZBESCHREIBUNG DER ZEICHNUNGEN
- In den Zeichnungen, in denen in sämtlichen Ansichten gleiche Teile mit den gleichen Bezugszeichen angegeben sind, ist

8 eine Vorderansicht des in4 gezeigten Zentrierers. - BESCHREIBUNG DER BEVORZUGTEN AUSFÜHRUNGSFORM

1 zeigt ein Oberschenkelteil beispielhaft als Implantatschaft, wobei ein Zentrierer der vorliegenden Erfindung allgemein mit dem Bezugszeichen10 angegeben ist. Bei der bevorzugten Ausführungsform hat das Oberschenkelteil12 einen sich verjüngenden distalen Abschnitt14 , der an der Spitze16 endet. Der Abschnitt14 ist passend zu einer sich verjüngenden Innenfläche15 des Zentrierers10 gebildet. -
2 zeigt einen Zentrierer gezeigt, der am distalen Ende14 des Oberschenkelteils12 angebracht ist. Dies wird durch das Führen der Innenfläche15 des Zentrierers10 über die distale Spitze16 des Oberschenkelteils12 erreicht bis die passenden Verjüngungen ineinander greifen.2 zeigt darüber hinaus perspektivisch einen Oberschenkelhals18 . Normalerweise würde man den Markkanal des Oberschenkelhalses ausfräsen oder in einer anderen Weise vorbereiten, um das Oberschenkelteil, das mit PMMA-Knochenzement implantiert wird, aufzunehmen. -
3 zeigt ist eine Ansicht im Schnitt des Zentrierers in unmittelbarer Nähe zur distalen Stelle des Zentrierers am distalen Schaft14 . Obwohl der distale Schaft14 kreisförmig gezeigt ist, kann er ohne weiteres rechteckförmig sein mit vier sich verjüngenden Seiten, wobei der Zentrierer10 auch einen rechteckförmigen Innenquerschnitt hat. Bei der bevorzugten Ausführungsform hat der Zentrierer10 einen ringförmigen Körper20 , der in Form des distalen Schaftes14 gebildet ist. Bei der bevorzugten Ausführungsform hat der Zentriererkörper20 vier Rippen22 , die sich davon radial nach außen erstrecken. Ein Aufbau mit drei oder mehr als vier Rippen ist ebenfalls möglich. -
4 zeigt den ringförmigen Körper20 des Zentrierers10 mit einer proximalen Endfläche24 und einer distalen Endfläche26 . Bei der bevorzugten Ausführungsform ist der Abstand zwischen der proximalen Endfläche24 und der distalen Endfläche26 konstant und beträgt in etwa 10 mm oder weniger. Wie in den4 und6 zu sehen ist, sind benachbarte Rippen in der proximal distalen Richtung zueinander versetzt.7 zeigt, dass gegenüberliegende Rippen bei der bevorzugten Ausführungsform an derselben proximal distalen Stelle angeordnet sind. Alternativ können alle vier Rippen an unterschiedlichen proximal distalen Stellen angeordnet sein. - Wieder Bezug nehmend auf die
4 bis8 ist bei der bevorzugten Ausführungsform ersichtlich, dass die Vorderkante28 , d. h. die distale Kante jeder Rippe22 einen Winkel β größer als 135° mit den Körper20 des Zentrierers10 bildet. Zusätzlich bildet bei der bevorzugten Ausführungsform die Hinterkante, d. h. die proximale Kante30 jeder Rippe22 einen Winkel α von weniger als 90° mit dem Körper20 . - Der Zentrierer der vorliegenden Erfindung ist vorzugsweise durch Spritzgießen eines Polymethyl-Methacrylat-Polymers hergestellt, wobei das Polymer mit der Knochenzementzusammensetzung, die in den Knochenkanal angeordnet wird, identisch ist. Diese Zusammensetzung kann gleich derjenigen sein, die von Howmedica Inc. für das Simplex P® Knochenzementmaterial verwendet wird, das im Handel erhältlich ist. Wie zuvor angegeben, besteht das bevorzugte Herstellungsverfahren für den Zentrierer darin, ihn durch Spritzgießen in einer Metallgießform herzustellen, die entsprechend den Merkmalen des beabsichtigten Zentrierers hergestellt ist. Es können mehrere Gießformen verwendet werden, um Zentrierer mit verschiedenen Abmessungen für die Verwendung mit einer Reihe von Implantaten und Markkanaldurchmessern herzustellen. Die Lage der Zentriererrippen und deren Form werden zunächst aus Metall hergestellt und anschließend wird das PMMA-Material bei einer Temperatur oberhalb dessen Schmelzpunkt eingespritzt.
Claims (8)
- Zentrierer (
10 ) für den Schaft (14 ) eines prothetischen orthopädischen Implantats (12 ), das für das Implantieren in eine Markhöhle eines Knochens (18 ) geeignet ist, wobei der Zentrierer Folgendes umfasst: – einen hohlen ringförmigen Körper (20 ), der eine Innenfläche zum Aufnehmen des Schafts des Implantats aufweist und – mindestens drei, um eine Außenfläche des ringförmigen Körpers (20 ) herum in Abständen angebrachte Rippen (22 ), wobei die Rippen sich von der Fläche in einer Richtung nach außen erstrecken, die etwa senkrecht zu einer sich durch den ringförmigen Körper in einer proximal distalen Richtung erstreckenden Längsachse verläuft, wobei jede der Rippen in einer Ebene liegt, die die Längsachse enthält, dadurch gekennzeichnet, dass jede der Rippen (22 ) von den benachbarten Rippen in der proximal distalen Richtung versetzt ist. - Zentrierer nach Anspruch 1, bei dem die Rippen (
22 ) in gleichen Abständen um den ringförmigen Körper (20 ) herum angebracht sind. - Zentrierer nach Anspruch 1 oder 2, bei dem der Körper (
20 ) proximate und distale Endflächen (24 ,26 ) aufweist, die in vorbestimmtem Abstand voneinander entfernt angebracht sind, bei dem die Rippen (22 ) neben dem ringförmigen Körper eine Grundfläche aufweisen, die sich über einen wesentlichen Teil des vorbestimmten Abstands hinweg erstreckt, und bei dem die Endflächen (24 ,26 ) auf eine Art und Weise gewellt sind, um die Versetzung der benachbarten Rippen (22 ) in proximal distaler Richtung auszugleichen, um den vorbestimmten Abstand um den ringförmigen Körper (20 ) herum etwa konstant zu halten. - Zentrierer nach einem der Ansprüche 1 bis 3, der vier Rippen (
22 ) aufweist, wobei gegenüberliegende Rippen der vier Rippen an der gleichen proximal distalen Stelle des ringförmigen Körpers gelegen sind. - Zentrierer nach einem der Ansprüche 1 bis 4, bei dem die Querschnittslänge der Rippen (
22 ) in proximal distaler Richtung beim nach außen Bewegen in einer Richtung, die zur Längsachse etwa senkrecht ist, abnimmt. - Zentrierer nach einem der Ansprüche 1 bis 5, bei dem sich die Innenfläche des ringförmigen Körpers (
20 ) der Längsachse entlang in proximal distaler Richtung nach innen verjüngt. - Zentrierer nach einem der vorhergehenden Ansprüche, bei dem der Zentrierer aus Polymethylmethacrylat besteht.
- Orthopädisches Implantat zum Implantieren im Markkanal eines Knochens, umfassend: – einen Schaft (
14 ) für das Implantieren im Markkanal (18 ) und – einen Zentrierer (10 ) wie in einem der Ansprüche 1 bis 7 definiert, der entfernbar in ein Ende des Schaftes eingreift.
