Description
- Diese Erfindung bezieht sich allgemein auf endoluminale Stützprothesen, die transluminal zugeführt und an einem Ort expandiert werden können, um eine Prothese zu schaffen, die mit der Unterstützung eines geeigneten Einführelements oder Katheters endoluminal positioniert und angeordnet wird und die derart angeordnet bleibt, um sowohl einen Gefäßdefekt zu reparieren als auch eine dauerhafte Stütze an der Stelle der Prothese bereitzustellen. In ihrer breitesten Auslegung kombiniert die Prothese vorzugsweise innerhalb einer einzigen Struktur eine rohrförmige Stützkomponente einer expandierbaren luminalen Prothese mit einer nachgiebigen Prothesenkomponente, die daran befestigt ist. Die expandierbare endoluminale Stützprothese hat eine gegabelte oder verzweigte Struktur, die aus Komponenten gebildet ist, die so entworfen sind, dass sie vorzugsweise während der Ablage oder der Reparatur und Unterstützung von Gefäßstellen bei Verzweigungsstellen oder in der Nähe davon in Bezug zueinander gegabelt positioniert werden. Vorzugsweise ist die Prothesenkomponente nachgiebig, dehnbar oder elastomer und behindert eine Expansion der rohrförmigen Stützkomponente im Wesentlichen nicht, während sie gleichzeitig eine Porosität aufweist, die ein normales Zellwachstum oder eine Gewebeinvasion von den Körpervertiefungenn nach der Implantation erleichtert.
- Elastomere Gefäßprothesen werden bekanntlich durch verschiedene Verfahren hergestellt. Enthalten sind Verfahren, die eine elektrostatische Spinntechnik wie jene umfassen, die von Annis u. a. in "An Elastomeric Vascular Prosthesis" in Trans. Am. Soc. Artif. Intern. Organs, Band XXIV, S. 209 ff. (1978) und im
US-Patent Nr. 4.323.525 US-Patent Nr. 4.459.252 . Ein weiterer Ansatz beinhaltet die Ausbildung von Poren in Polymeren durch Phaseninversionstechniken wie jene, bei denen ein solvatisiertes Polymer in ein weiteres Lösungsmittel eingetaucht wird und das Polymer koaguliert, während das Polymer-Lösungsmittel entfernt wird. Ebenso bekannt sind Spinntechniken wie jene, die imUS-Patent Nr. 4.475.972 beschrieben sind. Durch diesen Ansatz wird ein Polymer wie etwa Dorn extrudiert. Das Spinndüsen-System bewegt sich längs eines Pfades hin und her, der im Allgemeinen parallel zu der Längsachse des Dorns ist und dazu in einem gesteuerten Kantenwinkel steht. Das Ergebnis ist eine Vliesstruktur, bei der jede Faserlage mit der darunter liegenden Faserlage verbunden ist. - Ebenfalls bekannt sind Stent-Vorrichtungen, die in einem Blutgefäß oder in einem anderen Körperhohlraum oder Gefäß zur Behandlung von Verstopfungen, Stenosen, Aneurismen, Krankheit, Verletzung oder dergleichen in dem Gefäß angeordnet oder implantiert werden. Diese Stents werden in das vaskuläre System oder in ein anderes System oder in ein Körpergefäß implantiert, um zusammenfallende, teilweise verschlossene, geschwächte, kranke, verletzte oder unnormal erweiterte Abschnitte des Gefäßes zu verstärken. Hin und wieder werden Stents verwendet, um eine Erkrankung bei einer Verzweigung, Gabelung und/oder Anastomose oder in deren Nähe zu behandeln. Dadurch besteht die Gefahr, den Grad an Durchgängigkeit des Primärgefäße und/oder seinen Verzweigungen oder Gabelung zu beeinträchtigen, was als eine Folge von verschiedenen Problemen wie etwa einer Verlagerung von erkranktem Gewebe, Gefäßspasmen, einer Zergliederung mit oder ohne intimale Klappen, Thrombose und Embolie auftreten kann.
- Ein übliches Verfahren zum Implantieren eines Stents besteht darin, zuerst den Bereich des Gefäßes mit einem Ballonkatheter zu öffnen und hierauf den Stent in einer Position anzuordnen, die den erkrankten Abschnitt des Gefäßes überbrückt. Verschieden Konstruktionen und Entwürfe von Stents sind bekannt. Das
US-Patent Nr. 4.140.126 beschreibt eine Technik zum Positionieren eines länglichen zylindrischen Stents bei einem Bereich eines Aneurismas, um einen katastrophalen Ausfall der Blutgefäßwand zu verhindern, wobei der Stent ein Zylinder ist, der sich nach dem Einführen mit der Unterstützung eines Katheters zu einer implantierter Anordnung expandiert. Weitere derartige Vorrichtungen sind in denUS-Patenten Nr.4.787.899 und Nr.5.104.399 veranschaulicht. DieUS-Patente Nr. 4.503.569 und Nr.4.512.338 zeigen Federstents, die sich mit einer Änderung der Temperatur zu einer implantierten Anordnung expandieren. Es wird in einer gewickelten Konfiguration implantiert und hierauf vor Ort erwärmt, um zu bewirken, dass das Material der Feder expandiert. In die Stelle springende Stents sind imUS-Patent Nr. 4.580.568 gezeigt. DasUS-Patent Nr. 4.733.665 zeigt eine Anzahl von Stent-Konfigurationen zur Implantation mit der Unterstützung eines Ballonkatheters. DasUS-Patent Nr. 5.019.090 zeigt einen allgemein zylindrischen Stent, der aus einem Draht geformt ist, der in eine Reihe von leichten Kurven gebogen und hierauf um einen zylindrischen Dorn spiralförmig gewickelt wurde, um den Stent auszubilden. Wenn radial nach außen gerichtete Kräfte auf den Stent aufgebracht werden, wie etwa durch den Ballon eines angioplastischen Katheters, öffnen sich die scharfen Knicke und der Durchmesser des Stents vergrößert sich. DasUS-Patent Nr. 4.994.071 beschreibt einen gegabelten Stent mit mehreren Drahtschlaufen, die durch ein sich längs erstreckendes Grundgerüst und/oder durch Drahtverbindungen und Halbhaken miteinander verbunden sind. - Stents an sich können eine normale Zellinvasion häufig nicht unterstützen und sie können zu einer ungeregelten Ausbildung von Zellen im Stentmaschennetz mit einer raschen Ausbildung von zellulärer Hyperplasie führen. Prothesen alleine stellen unter bestimmten Umständen keine ausreichende Unterstützung bereit. Zum Beispiel beschreibt das kürzlich veröffentlichte Patent
US 5353747 von Jean-Pierre Dereume mit dem Titel "Luminal Grapht Endoprostheses and Manufacture Thereof" Prothesen, die die Fähigkeit haben, eine Dilatation und/oder Stützfunktionen auszuführen. Eine expandierbare rohrförmige Stützkomponente und eine elastomere Prothesenkomponente sind zu einer einzigen Vorrichtung kombiniert, bei der das Prothesenmaterial an der inneren oder der äußeren Oberfläche oder an beiden Oberflächen der expandierbaren Stützkomponente befestigt ist. Das Prothesenmaterial wird durch eine Spinntechnik hergestellt, wie jene, die imUS-Patent Nr. 4.475.972 beschrieben ist. Außerdem werden lunimale Endoprothesen mit einer expandierbaren Beschichtung auf der Oberfläche von Außenwänden der radial expandierbaren rohrförmigen Stützen in denUS-Patenten Nr. 4.739.762 und Nr.4.776.337 vorgestellt. In diesen beiden Patenten wird die Beschichtung aus dünnem, elastischen Polyurethan, aus einem Teflon-Film oder aus einem Film eines inerten biokompatiblen Materials hergestellt. A. Balko u. a. erwähnen in "Transfemoral Placement of Intraluminal Polyurethane Prosthesis for Abdominal Aortic Aneurysm" im Journal of Surgical Research, 40, S. 305–309 (1986) und in denU.S. Patenten Nr. 5.019.090 und Nr.5.092.877 die Möglichkeit, Stentmaterialien mit porösen oder texturierten Oberflächen für ein zelluläres Einwachsen oder mit nicht Thrombose erzeugenden Wirkstoffen und/oder Arzneien zu beschichten. -
EP-A-0551179 offenbart eine zweiseitige intraaortale Bypassprothese, die zwei getrennte rohrförmige Prothesen umfasst, die der Aorta intraluminal zugeführt werden können. Jede Prothese ist an einem entsprechenden expandierbaren rohrförmigen Element befestigt, das in einem radial komprimierten Zustand zugeführt und hierauf in situ expandiert werden kann, um die jeweilige Prothese an der Aorta zu befestigen, derart, dass die zwei rohrförmigen Elemente miteinander in Eingriff gelangen. -

WO95/09586 - Gemäß einem Aspekt der vorliegenden Erfindung wird eine mehrteilige verzweigte expandierbare endoluminale Prothese gemäß dem nachfolgenden Anspruch 1 geschaffen.
- Gemäß einem zweiten Aspekt der vorliegenden Erfindung wird eine verzweigte expandierbare endoluminale Stützprothese gemäß dem nachfolgenden Anspruch 15 geschaffen.
- In einem nochmals weiteren Aspekt der vorliegenden Erfindung wird ein Verfahren zur Herstellung einer verzweigten rohrförmigen Stützkomponente für eine endoluminale Prothese gemäß dem nachfolgenden Anspruch 28 geschaffen.
- Folglich werden durch die vorliegende Erfindung Prothesen geschaffen, die expandierbar und stützend sind, die sich von einem ersten Durchmesser zu einem zweiten Durchmesser ausdehnen, der größer ist als der erste Durchmesser. Wenn die expandierbare Stützprothese ihren ersten Durchmesser annimmt, hat sie eine Größe und Form, die für ein Einführen in den gewünschten Körperdurchgang geeignet ist. Das Material der Prothese ist im Wesentlichen inert und hat vorzugsweise einen hauptsächlich zylindrischen Überzug und/oder Auskleidung hauptsächlich über der äußeren und/oder inneren Oberfläche der expandierbaren Stützkomponente. Vorzugsweise ist der Überzug und/oder die Auskleidung besonders vorteilhaft, da sie nachgiebig oder elastomer und porös ist, um das gewünschte Wachstum von Gewebe dort hinein zu unterstützen, um zu einer nicht abweisenden Befestigung am Ort und zur Vermeidung einer Stenose-Entwicklung beizutragen. Die poröse Auskleidung und/oder das poröse Überzugmaterial ist nachgiebig oder elastomer genug, um eine Expansion um bis zum 2- bis 4-fachen oder mehr des unexpandierten Durchmessers zu ermöglichen. Komponenten der verzweigten oder gegabelten expandierbaren endoluminalen Stützprothese sind vorzugsweise getrennt verbringbar, derart, dass jede Komponente in Bezug auf die andere in der gewünschten verzweigten oder gegabelten Anordnung richtig positioniert ist. Eine der Komponenten hat einen Abschnitt, der wenigstens eine in Längsrichtung angeordnete Vertiefung hat, um allgemein wenigstens zwei Schenkelabschnitte zur Aufnahme von einer der anderen Komponenten zu definieren.
- Es ist eine allgemeine Aufgabe der vorliegenden Erfindung, eine verbesserte verzweigte endoluminale Prothese zu schaffen, die vor Ort expandierbar ist und im expandierten Zustand selbststützend ist.
- Eine weitere Aufgabe dieser Erfindung besteht darin, biokompatible Prothesen mit mehreren Komponenten zu schaffen, die in vivo getrennt expandierbar sind und im expandierten Zustand stützend sind.
- Eine weitere Aufgabe der vorliegenden Erfindung besteht darin, eine verbesserte expandierbare verstärkte Prothese zu schaffen, die mittels Einführelementen, Ballonkathetern oder ähnlichen Vorrichtungen zugeführt wird und die ein gutes Einwachsen von Gewebe erleichtert.
- Eine weitere Aufgabe dieser Erfindung besteht darin, eine verbesserte endoluminale Prothese zu schaffen, die erkrankte oder beschädigte Bereiche vollständig abdeckt, um luminale Reparaturen oder Behandlungen wie etwa eine Reparatur von Aneurismen durchzuführen.
- Eine weitere Aufgabe der vorliegenden Erfindung besteht darin, eine verbesserte endoluminale Prothese zu schaffen, bei der die Endoprothese im Wesentlichen in biokompatiblem, nachgiebigem Material eingeschlossen ist, das dem umgebenden Gewebe und Blut oder einem anderen Körperfluid ausgesetzt ist.
- Eine weitere Aufgabe der vorliegenden Erfindung besteht darin, eine selbstexpandierende verstärkte Prothesenvorrichtung zu schaffen, die in ihrem gestreckten und zusammengepressten Zustand aus einem rohrförmigen Element heraus zugeführt und durch ein Bewegen aus dem rohrförmigen Element heraus abgelegt wird, wobei diese Vorrichtung besonders für ein Verbringen von Komponenten geeignet ist.
- Eine weitere Aufgabe dieser Erfindung besteht darin, eine gegabelte Rumpfkomponente zu schaffen, die in einem zusammengefallenen Zustand abgelegt wird und in vivo zu einer verzweigten Vorrichtung zur Verwendung bei der Behandlung und/oder Reparatur bei verzweigten Gefäßstellen expandiert wird.
- Eine weitere Aufgabe der vorliegenden Erfindung besteht darin, eine verzweigte endoluminale Prothesenkomponente mit einer sich längs erstreckenden gefalteten Rumpfkomponente und wenigstens einer zylindrischen Zweigkomponente zu schaffen, wobei die Komponenten nach einer endoluminalen Verbringung getrennt expandiert werden und eine gegabelte Prothese ausbilden, wenn sie in Bezug zueinander positioniert und expandiert sind.
- Eine weitere Aufgabe dieser Erfindung besteht darin, ein verbessertes Verfahren zur Ausbildung einer verzweigten endoluminalen Prothese zu schaffen, die ein in Längsrichtung ausgeführtes Faltverfahren beinhaltet.
- Diese und weitere Aufgaben, Merkmale und Vorteile dieser Erfindung gehen bei einer Betrachtung der folgenden ausführlichen Beschreibung klar hervor.
- Nun folgt anhand der beigefügten Zeichnung von Verfahren zur Umsetzung der vorliegenden Erfindung eine lediglich beispielhafte Beschreibung.

13 ist eine Stirnansicht der in12 gezeigten Struktur. - Eine gegabelte Endoprothese oder eine expandierbare Stützprothese ist eine Prothese, bei der die einzelnen Komponenten jeweils expandierbare Stützprothesenelemente sind. Diese getrennten Komponenten sind in
1 bis4 veranschaulicht, die außerdem ihre getrennte Ablage in Bezug aufeinander in einem Aortarumpf veranschaulichen. Das Gleiche wird in Verbindung mit der Behandlung eines Aneurismas wie etwa eines abdominalen iliakalen Aorta-Aneurismas gezeigt. Die Vorrichtung enthält eine Rumpfkomponente101 , die im veranschaulichten Gebrauch so gebildet ist, dass sie sich von unterhalb der renalen Arterien zu einer Stelle zwischen der proximalen Einschnürung des Aneurismas und der iliakalen Aorta-Vergabelung erstreckt. Selbstverständlich kann diese Rumpfkomponente auch kürzer sein, derart, dass sie kurz unterhalb der proximalen Einschnürung des Aneurismas endet, beispielsweise von einer Länge, die in einer Vertiefung oder Falte124 endet. Zusätzlich ist die gegabelte, expandierbare Stützprothesenkomponente dieser Ausführungsform selbstexpandierend und wird mittels eines Einführelements abgelegt, das zusammengedrückte, expandierbare Stützprothesenkomponenten enthält. - Genauer gesagt, wird zunächst in Bezug auf
1 zuerst ein Führungsdraht102 gemäß bekannten Verfahren eingeführt, um das Aneurisma103 zu durchqueren. Nun wird ein Einführelement, das allgemein mit104 bezeichnet ist, mit der darin befindlichen Rumpfkomponente in einem radial zusammengedrückten Zustand über den Führungsdraht102 eingeführt. Das Einführelement wird derart manövriert, dass es genau wie gewünscht positioniert wird, in diesem Fall bei einer Stelle distal vom distalen Ende des Aneurismas. Hierauf wird das Hüllenrohr105 des Einführelements durch ein Gleiten in die proximale Richtung zurückgezogen, während der verbleibende Teil des Einführelements104 an der Stelle bleibt. Wenn das Hüllenrohr zurückgezogen ist, expandiert der Rumpf101 , wobei er gegebenenfalls die abgelegte oder implantierte Position erreicht, die in2 gezeigt ist. Zu diesem Zeitpunkt ist der distale Abschnitt106 des Rumpfes in der Blutgefäßwand gut verankert und geeignet abgelegt. -
3 zeigt ein Einführelement, das allgemein mit107 bezeichnet wird, mit einer unabhängigen rohrförmigen expandierbaren Stützprothesenschenkel-Komponente108 (4 ), die darin radial zusammengedrückt ist. Bei dieser veranschaulichten Ausführungsform ist diese Schenkelkomponente eine iliakale Komponente der gegabelten Stützprothese, die in dem Körpergefäß zusammengebaut wird. Das Einführelement107 wird weiterbewegt, bis diese iliakale Komponente in einen Schenkel109 der bereits abgelegten Rumpfkomponente101 bewegt worden ist. Diese Positionierung ist in4 veranschaulicht. Es wird festgestellt, dass sich die rohrförmige iliakale Stützprothesenkomponente108 von weit innerhalb des Schenkels109 bis zu einer Stelle proximal zum Aneurisma in der iliakalen Arterie110 erstreckt. - In einem vorherigen Schritt wurde ein Führungsdraht durch das entsprechende Gefäß zur iliakalen Arterie
112 durchgeführt, bis er das Aneurisma103 durchquert hat, wobei er durch den anderen Schenkel113 der abgelegten Rumpfkomponente101 verläuft. Wenn das Einführelement für die zuvor radial zusammengedrückte iliakale Komponente115 entfernt worden ist, wird die Komponente115 radial expandiert und abgelegt. Folglich ist die gesamte gegabelte Endoprothese oder expandierbare Stützprothese gemäß dieser Ausführungsform vollständig abgelegt und zusammengebaut, wie es in4 gezeigt ist und wie es allgemein in12 und13 dargestellt ist. - Es wird angemerkt, dass es nicht erforderlich ist, die Rumpfkomponente
101 und die rohrförmigen Komponenten108 ,115 tatsächlich aneinander zu befestigen. Mit anderen Worten werden diese Komponenten im Allgemeinen teleskopisch in Bezug zueinander positioniert. Diese teleskopische Eigenschaft ermöglicht ein gewisses Gleiten zwischen der Rumpfkomponente und den rohrförmigen Schenkelkomponenten, wodurch eine teleskopische Verbindung mit Funktionen wie die eines Gleitlagers bereitgestellt wird. Selbstverständlich ist es im Allgemeinen wünschenswert, Abschnitte der gegabelten Endoprothese in dem gesunden Gefäßwandgewebe fest zu verankern. Dies kann durch die Festigkeit des Gebindes der Stützprothese oder durch das Durchführen von Maßnahmen zur Verbesserung der Festigkeit des Gebindes an ihren Enden oder durch Bereitstellen von Greifstrukturen wie etwa Haken, Widerhaken, ausgestellte Enden und dergleichen erzielt werden. Während eines pochenden Blutstroms und möglicherweise während eines Trainings der Person, bei der die Endoprothese implantiert ist, wirken auf die Endoprothese Spannung und Dehnkräfte. In Strukturen ohne eine teleskopische Verbindung oder irgendwelche anderen Mittel zur Entlastung der durch diese Spannung entwickelten Belastung kann ein beträchtlicher Betrag an Belastung auf die Verankerungsstellen und/oder die Befestigungskomponenten wirken, wodurch möglicherweise ein Lösen der Verankerungsstellen oder ein Zerbrechen von Befestigungskomponenten möglich wird. -
5 ,6 ,7 und8 veranschaulichen ferner eine Rumpfkomponente101 . Sie enthält einen gemeinsamen Rumpfabschnitt118 und einen verzweigten Ab schnitt, der allgemein mit115 bezeichnet wird. Der verzweigte Abschnitt enthält die Schenkel109 und113 . Bei dieser Ausführungsform befindet sich ein weiterer gemeinsamer Rumpfabschnitt120 gegenüber dem anderen gemeinsamen Rumpfabschnitt118 und erweitert sich von dem verzweigten Abschnitt119 . Folglich weist die Gesamtkonfiguration der Rumpfkomponente eine doppellumige Länge auf, die sich zwischen zwei einlumigen Längen befindet. Der gemeinsame Rumpfabschnitt118 kann beispielsweise in der Aorta-Arterie positioniert werden, der verzweigte Abschnitt119 stellt eine gegabelte Struktur bereit, um den Blutstrom in die zwei iliakalen Arterien zu lenken, und der weitere gemeinsame Rumpfabschnitt120 erleichtert eine Ablage der Schenkelkomponenten in den verzweigten Abschnitt119 , wobei er als eine Art Trichter für jeden Führungsdraht, jedes Einführelement und jede zusammengezogene Schenkelkomponente dient. - Die Rumpfkomponente
101 umfasst einen Stent oder eine rohrförmige Stützkomponente121 . Ebenfalls umfasst ist eine Auskleidung, die allgemein mit122 bezeichnet wird. Eine weitere Auskleidung123 befindet sich vorzugsweise innerhalb der Auskleidung122 . Die Auskleidungen122 ,123 sind innerhalb der Stentkomponente121 befestigt, um eine richtige Porosität für eine Endoprothese bereitzustellen. - Die Rumpfkomponente
101 enthält eine oder mehrere Vertiefungen wie etwa die Vertiefung124 und die Vertiefung125 . Eine dritte, eine vierte oder weitere Vertiefungen können abhängig von dem gewünschten Verzweigungsgrad bereitgestellt werden. Selbstverständlich werden eine oder mehrere rohrförmige expandierbare Stützschenkelprothesenkomponenten bereitgestellt, um in die verzweigten Vertiefungen zu gleiten, die folglich durch die Vertiefung bzw. die Vertiefungen definiert sind. Bei der veranschaulichten Ausführungsform gelangt eine derartige Schenkelkomponente108 gleitend mit einer Öffnung126 des Rumpfkomponentenschenkels109 in Eingriff, während eine zweite Schenkelkomponente115 gleitend und sich erweiternd in die Öffnung127 der Schenkelkomponente115 passt. - Die beschriebene Prothese wird abgelegt, um rohrförmige Körper wie etwa Blutgefäße, Tracheen, Ureteren und dergleichen zu ersetzten oder zu reparieren, wobei sie mehr als ein Leitung aufnimmt, um einen Strom zu weiteren Zweigen des rohrförmigen Körpers umzuleiten. Dies ermöglicht eine Reparatur eines gegabelten Bereichs, der mit der Verwendung einer einlumigen Vorrichtung oder von mehreren einzelnen einlumigen Vorrichtungen schwierig zu reparieren ist. Sie ist zur Reparatur von beschädigten verzweigten Leitungen oder umgekehrt zur Reparatur von Leitungen geeignet, die in einem einzigen Zweig zusammenlaufen.
- Eine bevorzugte Verwendung für die hier besprochenen gegabelten Endoluminalprothesen ist die Einführung in ein verzweigtes Blutgefäß. Entsprechend ist sie üblicherweise geeignet für die Verwendung bei der koronaren Vaskulatur (der rechten, der linken gemeinsamen, der linken vorderen absteigenden und der zirkumflexen Koronar-Arterien und ihrer Verzweigungen) und bei der peripheren Vaskulatur (Zweige der Karotidarterie, Aortaarterie, Femoralarterie, der Popliteal-Arterie und dergleichen). Diese gegabelten Vorrichtungen sind auch für eine Implantation in andere verzweigende Gefäße wie etwa im Magen-Darm-System, im tracheobronchialen System, im Gallensystem und im urogenitalen System geeignet.
- Selbstverständlich dehnen und/oder unterstützen die expandierbaren Stützprothesen gemäß der vorliegenden Erfindung Blutgefäßverletzungen und weitere Defekte oder erkrankte Bereiche einschließlich bei den Stellen von vaskulären Gabelungen, Zweigen und/oder Anastomosen oder in deren Nähe. Die expandierbare Stützprothese ist eine integrale Struktur, die die expandierbare Stützkomponente in die Wand oder in die Wände der elastomeren Prothese einbaut. Überzüge und/oder Auskleidungen, die die Prothese bilden, koppeln mit Körperkomponenten, die eine normale Zellinvasion ohne Stenose oder rezidive Stenose erleichtern, wenn die Prothese in ihrer expandierten, stützenden Ausrichtung ist. Das Prothesenmaterial ist inert und biokompatibel. Die expandierbare Stützprothese kann durch die Einwirkung von radial nach außen gerichteten Kräften, die bereitgestellt werden, indem die Endoprothese mit einem Ballonkatheter expandiert wird, von einer Einsetz-Konfiguration mit kleinerem Durchmesser zu einer Implantationskonfiguration mit größerem Durchmesser expandiert werden, wobei ein Auswurfrohr verwendet wird, das es ermöglicht, dass eine an die Stelle springende Struktur von dem Ende eines Katheters in ihrer expandierten Konfiguration abgelegt wird, oder beispielsweise durch Verwendung einer Stützkomponente, die aus bestimmten Legierungen hergestellt wird, die Wärmeübergangseigenschaften zeigen, durch die sie expandieren, wenn sie erwärmt werden.
- Zusätzlich zu den hier veranschaulichten Stützkomponenten-Strukturen umfassen Stützstrukturen andere mit Federeigenschaften und jene mit einer Wicklung mit in Umfangsrichtung ausgerichteten Fingern wie jene, die im Gianturco-US-Patent Nr.
4.800.882 gezeigt sind. DieUS-Patente Nr. 5.061.275 , Nr.5.219.355 und Nr.5.336.500 beziehen sich auf expandierende oder selbstexpandierende endoluminale Vorrichtungen. Üblicherweise konzentrieren sich diese Vorrichtungen auf die Verwendung einer metallischen Struktur, die Expansionseigenschaften weitergibt. DieUS-Patent Nr. 4.994.071 und Nr.5.360.443 beschreiben gegabelte Vorrichtungen, die expandierbare metallische Stentstrukturen und textile Materialien verwenden, die eine Verzweigung eines Fluidstroms ermöglichen. Im Allgemeinen können Materialien dieser Patente bei der Konstruktion von Komponenten der vorliegenden Erfindung verwendet werden. - Genauer ist die rohrförmige Stützkomponente vorzugsweise ein geflochtener rohrförmiger Stentkörper, der aus einer metallischen Legierung oder irgend einem anderen Material hergestellt ist, das flexibel ist, während es steif und elastisch ist, wenn es geflochten ist. Federartige Metalle sind üblicherweise bevorzugt, etwa rostfreier Stahl, Titan, rostfreie Stahllegierungen, Kobalt-Chrom-Legierungen einschließlich Legierungen wie Elgiloy®, Phynox® und Conichrom®. Thermische Übergangs-Legierungen oder Legierungen mit Memory-Funktion wie etwa Nickel-Titan-Legierungen einschließlich Nitinol® sind ebenfalls geeignet. Schmiedbare Metalle einschließlich Tantal sind für eine Struktur, die nicht selbstexpandierend ist, besonders geeignet.
- Als Materialien für die Auskleidung bzw. die Auskleidungen kommen üblicherweise Polymermaterialien in der Form einer Membran oder eines gewebeartigen Materials in Frage, deren Aufgabe darin besteht, die Porosität des Stents für ein richtiges Einwachsen in Gewebe und für eine Fluiddichtigkeit zu verringern. Beispielhafte Polymermaterialien umfassen Polyester wie etwa Polyethylen-Terephthalat, Polyolefine wie etwa Polypropylen oder elastomere Materialien wie etwa Polyurethan oder Silikongummi. Kombinationen aus diesen Materialien sind ebenfalls möglich. Bei einer besonders bevorzugten Anordnung wird die Auskleidung
112 , die die rohrförmige Stützkomponente121 belegt, aus einem doppellagigen Trikot-Polyester-Maschennetz hergestellt, üblicherweise aus einem Dacron®-Materialtyp, während die Innenauskleidung aus einem Polyurethan wie etwa Corethan®-Polymer-Polycarbonaturethan der Corvita Corporation hergestellt wird. Bei einer besonders bevorzugten Ausführungsform wird eine dünne Beschichtung oder Überzug aus Polymer über den geflochtenen Drähten der rohrförmigen Stützkomponente bereitgestellt. Ein diesbezüglich bevorzugtes Polymer ist Polycarbonaturethan. - Zusätzlich können verschiedene Oberflächenbehandlungen angewendet werden, damit die Oberflächen der expandierbaren Stützprothese biokompatibler bleiben. Enthalten sind die Verwendung von Pyrokohlenstoff, Hydrogelen und dergleichen. Die Oberflächenbehandlungen können auch für das Auswaschen oder eine Demobilisierung von Arzneien wie etwa Heparin, Anti-Blutplättchen-Wirkstoffen, Anti-Blutplättchen-abgeleiteteten Wachstumsfaktoren, Antibiotika, Steroiden und dergleichen sorgen. Zusätzlich kann die Beschichtung und/oder die Auskleidung mit Arzneien durchsetzt sein wie jene, die hier besprochen wurden, sowie mit lytischen Wirkstoffen, um eine lokale Arzneitherapie bereitzustellen.
- Es wird angemerkt, dass die Vertiefung bzw. die Vertiefungen wie die Vertiefungen
124 ,125 in Längsrichtung angeordnet sind und im Allgemeinen wenigstens zwei Schenkelabschnitte definieren, die je einen Durchmesser haben, der kleiner ist als der Durchmesser des Hauptkörpers. Jede Vertiefung hat eine innere Oberfläche in Längsrichtung wie eine Längskante128 ,129 . Diese Kanten können miteinander in Kontakt sein. Wenn gewünscht, können diese Kanten aneinander befestigt werden, etwa mit (nicht gezeigten) Nähten, Klebstoffen, Drähten, Klipsen oder dergleichen. Eine oder zwei derartige Vertiefungen oder Falten erzeugen je nach Wunsch eine asymmetrische oder symmetrische Gabelung. Bei einem weiteren beispielhaften Ansatz bilden drei Vertiefungen eine dreifach verzweigte Vorrichtung. Zusätzliche Falten können bereitgestellt werden, soweit dies durch die Flechtdrahtnetzdichte und den Durchmesser erlaubt ist. - Unabhängig von der Anzahl von Vertiefungen verringert die Verformung des geflochtenen rohrförmigen Körpers die Querschnittsfläche von jener des Hauptrumpfkörpers zu jener des jeweiligen verzweigten Bereichs. Die Gesamtquerschnittsfläche der verzweigenden rohrförmigen Körper sollte etwa 40 % oder mehr von der Querschnittsfläche des Hauptrumpfkörpers betragen. Vorzugsweise sollte dieser Bereich größer als etwa 70 % sein, um jegliche erheblichen Druckunterschiede längs der Vorrichtung zu vermeiden, wenn diese verbracht und implantiert ist. Bei einem typischen Menschen wird die Querschnittsfläche der Abdominal- Aorta beispielsweise um lediglich etwa 20 % verringert, wenn sie in die gemeinsamen iliakalen Arterien mündet.

9 veranschaulicht eine Befestigung, die zur Verwendung bei der Ausbildung der Vertiefung oder der Vertiefungen als dauerhafte Verformungen des geflochtenen Maschennetzzylinders, der die rohrförmige Stützkomponente darstellt, geeignet ist. Die Befestigung131 in der veranschaulichten Konfiguration wird verwendet, um einen symmetrischen gegabelten Entwurf zu formen. Der geflochtene Zylinder ist in Längsrichtung zusammengedrückt und über dem Dorn132 angeordnet, wobei diese Anordnung allgemein in10 gezeigt ist. Endkappen133 ,134 verriegeln die rohrförmige Stützkomponente121 in ihrem zusammengedrückten Zustand. Diese wird hierauf in der Befestigung131 angeordnet, wie es allgemein in9 gezeigt ist. Schlitze135 sind parallel zu der Längsachse und an gegenüberliegenden Seiten positioniert. Dies ermöglicht ein Gleiten von Flügel136 ,137 in die Befestigung131 und folglich in einen Eingriff mit der rohrförmigen Stützkomponente. Kanten138 ,139 der Flügel gelangen hierauf in Eingriff und falten die rohrförmige Stützkomponente121 zwischen den Kanten138 ,139 und den Vertiefungen141 ,142 des Dorns132 . - Selbstverständlich kann die Länge der Flügelkanten
138 ,139 geändert werden, um eine gewünschte Länge der Verformung am Hauptkörper der Rumpfkomponente zu erzeugen. Zusätzlich können derart geformte Verzweigungsbereiche mit verschiedenen Größen hergestellt werden, indem die Größe der einzelnen zylindrischen Komponenten des Dorns132 geändert wird, so dass sie nicht völlig gleich zu den in9 gezeigten Komponenten sind. Ein Dornzylinder mit größerer Größe hat die Ausbildung eines größeren Rumpfkomponentenschenkels109 ,113 zur Folge. Dies beinhaltet üblicherweise auch eine Verschiebung der Stelle der Schlitze135 , derart, dass die Ebene zum Einführen der Flügel auf die Vertiefungen ausgerichtet ist. Entsprechend wird die dreifach verzweigte Anordnung durch einen Dreikomponentendorn und drei Schlitze und Flügel erzielt, die um einen Winkel von 120° zueinander versetzt sind. In ähnlicher Weise würde eine vierfach verzweigte Struktur vier entsprechende Merkmale enthalten, die um 90° zueinander versetzt sind. - Bei einer bevorzugten Anordnung wird die derart verformte geflochtene rohrförmige Stützkomponente chemisch und mittels Wärme bearbeitet, um den gewünschten Durchmesser und die mechanischen Eigenschaften des Hauptkörpers zu erreichen. Wenn dieser flexible metallische Stent mit angepasster Form derart vorbereitet ist, wird er vorzugsweise ausgekleidet, wie es bereits hier besprochen wurde. Es wird angemerkt, dass das veranschaulichte rohrförmige geflochtene Maschennetz eine Haupt-Querschnittsfläche und eine auswärts gerichtete Aufweitung an beiden Enden hat. Die geflochtene Struktur wird vorteilhaft durch die zackige Struktur der Flügelkanten

- Dieses Beispiel veranschaulicht die Ausbildung einer verzweigten expandierbaren endoluminalen Stützprothese mit einem expandierten Innendurchmesser von 10 mm, die gegabelt ist, um zwei endoluminale Stützprothesenschenkel mit einem Durchmesser von 5 bis 7 mm aufzunehmen. Eine vliesartige Polycarbonat-Urethan-Auskleidung (Corethan®) wurde im Allgemeinen in Übereinstimmung mit dem
US-Patent Nr. 4.475.972 gesponnen, indem sie über einen Dorn gewickelt wurde. In diesem Beispiel bestand die Auskleidung aus etwa 400 Lagen Fasern. Eine gegabelte geflochtene Maschenrohr-Stützkomponente, die in einer wie in9 veranschaulichten Befestigung hergestellt worden war, wurde unter Verwendung einer verdünnten Lösung aus Polycarbonaturethan mit einem Härtegrad und einem Schmelzpunkt, der niedriger war als jener, der zum Verspinnen der Auskleidung verwendet wurde, sprühbeschichtet. Diese konnte mit warmer Luft getrocknet werden. Mehrere Sprühschichten ermöglichten die Ausbildung einer Klebeschicht. - Die zuvor hergestellte Polycarbonaturethanauskleidung wurde in der Länge zugeschnitten und in dem mit Klebstoff beschichteten, gegabelten geflochtenen Maschennetz angeordnet und so eingesetzt, dass sie genau in das gegabelte geflochtene Maschennetz passt. Ein Dorn mit einer Form ähnlicher der inneren Konfiguration des gegabelten Maschennetzes wurde von einem Ende eingesetzt, um als eine Stütze zu dienen. Ein Schrumpfrohr wurde über Abschnitte dieser Anordnung gezogen. Diese Anordnung wurde auf den Schmelzpunkt des Polycarbonat-Urethan-Klebstoffs erwärmt, wodurch das Schrumpfrohr unter Erwärmung schrumpft und das geflochtene Maschennetz gegen die Auskleidung drückt, die durch den geformten Dorn gestützt wird. Nach dem Abkühlen wurde das Schrumpfrohr entfernt und der Dorn herausgezogen, und so die vollständige Rumpfkomponente, wie sie hier beschrieben wurde, zurückgelassen.
- Die zwei endoluminalen rohrförmigen expandierbaren Stützprothesenschenkel-Komponenten werden gemäß einem ähnlichen Verfahren hergestellt, was wegen der zylindrischen Form dieser Komponenten einfacher ist.
- Beispiel II
- Das Verfahren von Beispiel I wird im Wesentlichen wiederholt, abgesehen davon, dass die Auskleidung ein Doppeltrikot-Polyestermaschennetz ist. In einer ähnlichen Anordnung wurde eine Rumpfkomponente der gleichen Struktur ausgebildet, abgesehen davon, dass vor dem Einführen des Stützdorns eine zweite, innere Auskleidung aus Polycarbonat-Urethan positioniert wird, um eine doppelt ausgekleidete verzweigte Komponente bereitzustellen.
- Beispiel III
- Prinzipiell wird den Verfahren von Beispiel I und von Beispiel II gefolgt, abgesehen davon, dass hier der expandierte Innendurchmesser der Rumpfkomponente 25 mm beträgt und dass die zylindrischen endoluminalen Schenkelprothesen einen Durchmesser von 12–15 mm aufweisen.
Claims (38)
- Mehrteilige verzweigte expandierbare endoluminale Stützprothese, umfassend: eine Vielzahl von expandierbaren geflochtenen endoluminalen Stützkomponenten (
101 ,108 ,115 ), die an einer ausgewählten Stelle in einem Körpergefäß individuell abgelegt werden können, wobei jede der endoluminalen Stützkomponenten der Prothese für endoluminale Einführung radial komprimierbar und zur Ablage an einem gewünschten Ort in einem Körpergefäß radial expandierbar ist; wobei eine der expandierbaren endoluminalen Stützkomponenten eine geflochtene Rumpfkomponente (101 ) ist, wobei die geflochtene Rumpfkomponente im Allgemeinen röhrenförmig ist und einen ersten Abschnitt (118 ) mit einem gegebenen Durchmesser aufweist; wobei die geflochtene Rumpfkomponente einen zweiten Abschnitt (119 ) mit einem gekräuselten Abschnitt mit einer in Längsrichtung angeordneten Vertiefung (124 ), die allgemein mindestens zwei Schenkelabschnitte (109 ,113 ), die jeweils einen Durchmesser besitzen, der kleiner ist als der vorgegebene Durchmesser, definiert, aufweist, und wobei jeder Schenkelabschnitt eine Schenkelöffnung (126 ,127 ) aufweist; wobei mindestens eine weitere der expandierbaren endoluminalen Stützkomponenten eine allgemein zylinderförmige Stützschenkelkomponente (108 ,115 ) ist; und wobei die allgemein zylinderförmige Stützschenkelkomponente in einem der Schenkelabschnitte der Rumpfkomponente positioniert ist, wenn die Schenkelkomponente und die Rumpfkomponente im Körpergefäße abgelegt sind. - Endoluminale Stützprothese nach Anspruch 1, worin die allgemein zylinderförmige Stützschenkelkomponente (
108 ,115 ) einen Endabschnitt aufweist, der nach der Ablage in der Schenkelöffnung (126 ,127 ) der Rumpfkomponente positioniert ist. - Mehrteilige verzweigte expandierbare endoluminale Stützprothese nach Anspruch 1, worin die allgemein zylinderförmige Stützschenkelkomponente (
108 ,115 ) einen Endabschnitt aufweist, der nach der Ablage teleskopisch in der Schenkelöffnung (126 ,127 ) der Rumpfkomponente positioniert ist. - Mehrteilige verzweigte expandierbare endoluminale Stützprothese nach Anspruch 1, Anspruch 2 oder Anspruch
- worin die Rumpfkomponente (
101 ) eine Vielzahl von Vertiefungen (124 ,125 ) aufweist, wobei jede Vertiefung eine innere Längsfläche aufweist, wobei die inneren Längsflächen die Schenkelabschnitte (109 ,113 ) der Rumpfkomponente definieren. - Mehrteilige verzweigte expandierbare endoluminale Stützprothese nach einem der vorhergehenden Ansprüche, worin die Schenkelabschnitte (
109 ,113 ) die allgemein zylinderförmigen Schenkelkomponenten (108 ,115 ) aufnehmen können. - Endoluminale Stützprothese nach einem der vorhergehenden Ansprüche, worin die Vielzahl der expandierbaren endoluminalen Stützkomponenten selbstexpandierend ist.
- Endoluminale Stützprothese nach einem der Ansprüche 1–5, worin die Vielzahl der expandierbaren endoluminalen Stützkomponenten von einer radial expandierbaren Vorrichtung abgelegt werden kann.
- Endoluminale Stützprothese nach einem der vorhergehenden Ansprüche, worin die allgemein zylinderförmige Stützkomponente (
108 ,115 ) ein allgemein zylinderförmiges Stützelement und eine daran befestigte allgemein zylinderförmige Auskleidung aufweist. - Endoluminale Stützprothese nach einem der vorhergehenden Ansprüche, worin die Rumpfkomponente (
101 ) eine Rumpfauskleidung (122 ,123 ) mit einer nachgiebigen Wand aus einem im Wesentlichen inerten biokompatiblen Material aufweist, wobei die Rumpfauskleidung an der Innenseite der Rumpfkomponente befestigt ist, wobei die nachgiebige Wand eine Durchmessergröße aufweist, die mit der Rumpfkomponente expandiert. - Endoluminale Stützprothese nach Anspruch 8, worin die Auskleidung der allgemein zylinderförmigen Stützschenkelkomponente (
108 ,115 ) eine nachgiebige Wand aus einem im Wesentlichen inerten biokompatiblen Material ist, wobei die nachgiebige Wand mindestens an die Innenseite des allgemein zylinderförmigen Stützelementes der Schenkelkomponente (108 ,115 ) angelegt ist. - Endoluminale Stützprothese nach einem der vorhergehenden Ansprüche, worin die allgemein zylinderförmige Stützschenkelkomponente (
108 ,115 ) nach der Ablage teleskopisch gleitend in einem der Schenkelabschnitte (109 ,113 ) der Rumpfkomponente positioniert ist. - Endoluminale Stützprothese nach Anspruch 8, worin die Auskleidung der Schenkelkomponente (
108 ,115 ) eine nachgiebige Wand aus einem porösen Material ist, das eine Struktur ergibt, die normale Zellinvasion aus dem Körpergefäß in es hinein gestattet, wenn die Prothese im Gefäß implantiert ist. - Endoluminale Stützprothese nach Anspruch 12, worin das poröse Material mit einer dünnen Schicht aus Silikonkautschuk beschichtet ist.
- Endoluminale Stützprothese nach einem der vorhergehenden Ansprüche, worin die allgemein zylinderförmigen röhrenförmigen Stützkomponenten (
108 ,115 ) Drahtstrange umfassen, die als schneidende, miteinander integrale Längsteile ausgebildet sind, die Öffnungen zwischen sich definieren, um ein geflochtenes maschenförmiges röhrenförmiges Stützelement zu bilden. - Verzweigte expandierbare endoluminale Stützprothese, umfassend: eine geflochtene endoluminale Stützprothesenkomponente, die an einer ausgewählten Stelle in einem Körpergefäß individuell abgelegt werden kann, wobei die endoluminale Stützprothesenkomponente radial komprimierbar und radial expandierbar ist; wobei die expandierbare endoluminale Stützkomponente eine geflochtene Rumpfkomponente (
101 ) ist, die allgemein röhrenförmig ist und einen ersten Abschnitt (118 ) mit einem gegebenen Durchmesser aufweist; wobei die geflochtene Rumpfkomponente einen zweiten Abschnitt (119 ) mit einem gekräuselten Abschnitt mit einer in Längsrichtung angeordneten Vertiefung (124 ) aufweist, die allgemein mindestens zwei Schenkelabschnitte (109 ,113 ) definiert, die jeweils einen Durchmesser besitzen, der kleiner ist als der vorgegebene Durchmesser, und wobei jeder Schenkelabschnitt eine Schenkelöffnung (126 ,127 ) aufweist. - Endoluminale Stützprothese nach einem der Ansprüche 1 bis 15, worin die Rumpfkomponente eine Vielzahl von Vertiefungen (
124 ,125 ) aufweist, wobei jede Vertiefung eine innere Längsfläche aufweist, wobei die inneren Längsflächen ineinander greifen und dadurch die Schenkelabschnitte (109 ,113 ) als allgemein zylinderförmig definieren. - Endoluminale Stützprothese nach Anspruch 16, worin die inneren Längsflächen Längsränder sind, die aneinander befestigt sind.
- Endoluminale Stützprothese nach Anspruch 15, worin die Rumpfkomponente (
101 ) eine poröse Polymerwand (122 ) aus einem im Wesentlichen inerten biokompatiblen Material aufweist, wobei die nachgiebige poröse Wand mindestens an die Innenseite der röhrenförmigen Stützkomponente angelegt ist und wobei die Polymerwand eine Längsfalte aufweist, die an der Vertiefung (124 ) der Rumpfkomponente anliegt. - Endoluminale Stützprothese nach Anspruch 1 oder Anspruch 15, worin die Rumpfkomponente (
101 ) einen dritten Abschnitt (120 ) aufweist, dessen Durchmesser dem vorgegebenen Durchmesser des ersten Abschnitts (118 ) ähnelt, und wobei der zweite Abschnitt (119 ) zwischen dem ersten Abschnitt und dem dritten Abschnitt angeordnet ist. - Endoluminale Stützprothese nach Anspruch 19, worin der erste Abschnitt (
118 ) und der dritte Abschnitt (120 ) jeweils allgemein zylinderförmig sind. - Endoluminale Stützprothese nach Anspruch 12 oder Anspruch 18, worin das poröse Material der nachgiebigen Wand ein elastomeres Polymer ist.
- Endoluminale Stützprothese nach einem der Ansprüche 12, 18, oder 21, worin das poröse Material der nachgiebigen Wand ein Polycarbonaturethan ist.
- Endoluminale Stützprothese nach Anspruch 9 oder Anspruch 18, worin die Rumpfwand eine innere Auskleidung (
123 ) und eine Zwischenauskleidung (122 ) zwischen der inneren Auskleidung und der röhrenförmigen Stützkomponente aufweist. - Endoluminale Stützprothese nach einem der vorhergehenden Ansprüche, worin die Rumpfkomponente eine Vielzahl von Drahtsträngen mit offenen Bereichen dazwischen zur Bildung eines geflochtenen Maschennetzes aufweist.
- Endoluminale Stützprothese nach Anspruch 24, worin die Drahtstränge eine Vielzahl von einzelnen Strängen aufweisen, die in Längsrichtung nebeneinander und in Berührung miteinander in einer geflochtenen, sich überkreuzenden Konfiguration liegen.
- Endoluminale Stützprothese nach einem der vorhergehenden Ansprüche, worin die endoluminale Prothese einer Oberflächenbehandlung mit einem nachgiebigen klebstoffartigen Polymer unterworfen wird.
- Endoluminale Stützprothese nach Anspruch 26, worin das nachgiebige klebstoffartige Polymer ein Polycarbonaturethan ist.
- Verfahren zur Herstellung einer geflochtenen verzweigten röhrenförmigen Stützkomponente für eine endoluminale Prothese, umfassend: Bildung einer expandierbaren röhrenförmigen endoluminalen Prothesenstützkomponente, wobei die Komponente eine Rumpfkomponente (
101 ) ist, gekennzeichnet durch Kräuselung mindestens eines Längsabschnitts (119 ) der röhrenförmigen Stützkomponente zur Bildung mindestens einer in Längsrichtung angeordneten Vertiefung (124 ) darin, um mindestens zwei Schenkelabschnitte (109 ,113 ) zu bilden, die jeweils einen Durchmesser aufweisen, der geringer ist als der Durchmesser der Rumpfkomponente, und wobei jeder Schenkelabschnitt eine Schenkelöffnung (126 ,127 ) aufweist. - Verfahren nach Anspruch 28, worin eine in Längsrichtung angeordnete Vertiefung (
124 ) gebildet wird, um einen doppellumigen Abschnitt (119 ) der röhrenförmigen Stützkomponente zu bilden. - Verfahren nach Anspruch 28, worin zwei in Längsrichtung angeordnete Vertiefungen gebildet werden, um einen dreilumigen Abschnitt der röhrenförmigen Stützkomponente zu bilden.
- Verfahren nach Anspruch 28, worin zwei in Längsrichtung angeordnete Vertiefungen (
124 ,125 ) gebildet werden, um einen doppellumigen Abschnitt der röhrenförmigen Stützkomponente zu bilden. - Verfahren nach Anspruch 28, worin drei in Längsrichtung angeordnete Vertiefungen gebildet werden, um einen vierlumigen Abschnitt der röhrenförmigen Stützkomponente zu bilden.
- Verfahren nach einem der Ansprüche 28–32, worin die röhrenförmige Stützkomponente ein zylinderförmiges geflochtenes Maschenrohr aufweist, das durch Flechten von Metallsträngen zu einem zylinderförmigen Element hergestellt wird.
- Verfahren nach einem der Ansprüche 28–33, ferner umfassend den Schritt der Befestigung einer Klebeschicht auf der Innenseite der gekräuselten röhrenförmigen Stützkomponente.
- Verfahren nach Anspruch 34, ferner umfassend den Schritt der Einführung eines nachgiebigen Polymerrohrs in die gekräuselte röhrenförmige Stützkomponente in Eingriff mit der Klebeschicht.
- Verfahren nach Anspruch 35, ferner umfassend den Schritt des Aufbringens von Drucks zur Fixierung des nachgiebigen Polymerrohrs auf der Innenseite der gekräuselten röhrenförmigen Stützkomponente und zur Bildung einer Falte in dem nachgiebigen Polymerrohr, die sich allgemein an die Form und Größe der Vertiefung der gekräuselten röhrenförmigen Stützkomponente anpasst.
- Verfahren nach einem der Ansprüche 28–36, worin der Kräuselschritt mindestens zwei Schenkelabschnitte (
109 ,113 ) definiert, wobei jeder Schenkelabschnitt eine Schenkelöffnung (126 ,127 ) definiert, deren Innendurchmesser kleiner ist als der der röhrenförmigen Stützkomponente vor dem Kräuselschritt; und ferner umfassend den Schritt der Bildung einer allgemein zylinderförmigen Stützschenkelkomponente (108 ,115 ), die ein röhrenförmiges Stützelement mit einem Außendurchmesser aufweist, der kleiner ist als der Innendurchmesser einer Schenkelöffnung (126 ,127 ) der gekräuselten röhrenförmigen Stützkomponente.
