Description
- GEBIET DER ERFINDUNG
- Die vorliegende Erfindung betrifft das Gebiet der kontrazeptiven und Sterilisationsvorrichtungen und insbesondere reversible kontrazeptive Vorrichtungen.
- HINTERGRUND DER ERFINDUNG
- Die herkömmlichen Strategien zur Empfängnisverhütung fallen im Allgemeinen in drei Kategorien: physikalische Barrieren, Arzneimittel und Operation. Jede Methode hat bestimmte Vorteile, aber es gibt auch diverse Nachteile. Barrieren, wie z.B. Kondome und Diaphragmen können versagen, indem sie reißen und verrutschen. Medikamentöse Strategien, wie z.B. die Pille und NorplantTM, bei denen die Hormonkonzentrationen künstlich kontrolliert werden, sind mit bekannten und unbekannten Nebenwirkungen bei längerer Anwendung behaftet. Operative Verfahren, wie z.B. Tubenligatur und Vasektomie, sind schließlich kostspielig und mit den Risiken einer Operation behaftet, und sie sind häufig nicht reversibel. Deshalb besteht ein Bedarf nach einer sicheren, wirksamen Methode zur Empfängnisverhütung, insbesondere nach einer nichtchirurgischen Methode, die reversibel ist.

US-A-5474089 beschreibt eine Blockiervorrichtung, die nichtchirurgisch in einen Gang eines Reproduktionssystems eingeführt werden kann. Die Vorrichtung umfasst ein Federsystem, das im Wesentlichen zylinderförmig ist und eine bekannte Spiralstruktur formt, und eine Membran, die sich ausdehnt und die Barriere zur Blockierung eines Gangs bildet. - ZUSAMMENFASSUNG DER ERFINDUNG
- Die vorliegende Erfindung betrifft eine kontrazeptive Vorrichtung wie in den Ansprüchen beschrieben. Insbesondere betrifft die vorliegende Erfindung ein kontrazeptives oder Sterilisationssystem zum Verschließen eines Reproduktionstrakts oder Lumens zur Verhinderung des Durchgangs von Reproduktionsfähigen Zellen durch den Trakt oder das Lumen. Die Erfindung umfasst ein Verschlusselement, das sich im Körperlumen von einer ersten Konfiguration, die zur Einführung in das Körperlumen geeignet ist, zu einer zweiten größeren Konfiguration ausdehnen kann, um die Fixierung des expandierten Verschlusselements zumindest an einem Teil einer Wand, die das Reproduktionskörperlumen definiert, zu erleichtern. Die Erfindung umfasst ferner ein Mittel zur Erleichterung der Fixierung des expandierten Verschlusselements an der Wand des Körperlumens und ein Mittel zur Kontraktion des expandierten Verschlusselements und des am Verschlusselement fixierten Wandteils, um das Reproduktionskörperlumen ausreichen zu verschließen, um den Durchgang von Reproduktionszellen dort hindurch zu verhindern.
- Eine zurzeit bevorzugte Ausführungsform der Erfindung umfasst ein reversibles kontrazeptives System, das entweder zum Verschließen der Eileiter eines weiblichen Patienten, des Vas deferens eines männlichen Patienten oder eines anderen Reproduktionstrakts verwendet werden kann. Ein Schlüsselmerkmal des kontrazeptiven Systems ist ein Verschlusselement, das zunächst in einem expandierten Zustand an der den Reproduktionstrakt definierenden Wand fixiert ist und dann auf kleinere Querdurchschnittsabmessungen kollabiert wird, um Kollabieren des fixierten Abschnitts der Wand zu verursachen und damit den Gefäßgang zu blockieren, um die Passage von Reproduktionszellen zu verhindern. Das Verschlusselement kann mit einer beliebigen Anzahl geeigneter Mittel wieder geöffnet werden. Beispielsweise durch Kollabieren des Verschlusselements um einen Pfropf oder einen Mandrin, der an Ort und Stelle bleiben kann, um den Durchgang wirksam zu blockieren, bis der Patient den Eingriff rückgängig machen will. Der Pfropf kann mit einem geeigneten Mittel entfernt werden, beispielsweise mit herkömmlichen laparoskopischen oder anderen Instrumenten zum Wiederöffnen des Durchgangs. Mit Hilfe eines Ballondilatationskatheters kann die Öffnung nach der Entfernung des Pfropfes weiter vergrößert werden. Bei anderen Wegen zur Wiederöffnung des Reproduktionslumens bleibt der proximale Abschnitt des Verschlusselements beim Kollabieren des Elements offen, so dass ein expandierbares Element, wie z.B. ein Ballon auf einem Katheter, eingeführt und expandiert werden kann. Durch eine Reihe von Expansionen und stufenweises Vorschieben kann der gesamte Durchgang wieder geöffnet werden.
- Vorzugsweise umfasst das Verschlusselement ein röhrenförmiges Element aus einer Formgedächtnislegierung mit einer primären Konfiguration, die in den Querabmessungen relativ klein ist, um die Einführung des Elements in das gewünschte Körperlumen zu erleichtern. Sobald es eingeführt ist, wird das Verschlusselement zu einer zweiten Konfiguration expandiert, deren Querabmessungen in etwa dem Körperlumen entsprechen oder etwas größer sind, so dass das Verschlusselement an der das Körperlumen definierenden Wand fixiert werden kann. Wenn das offene Gerüst des Verschlusselements im Körperlumen expandiert ist, wird das Verschlusselement durch Endothelialisierung durch die offene Konstruktion an der das Körperlumen definierenden Wand fixiert. Durch Erwärmung des aus einer Formgedächtnislegierung bestehenden Verschlusselements auf eine Temperatur bei oder oberhalb der Übergangstemperatur des Formgedächtnismaterials, nimmt es eine erinnerte geschlossene oder kollabierte Konfiguration an, durch die sich die am Verschlusselement fixierte Wand nach unten schließt, so dass der Durchgang verschlossen wird. Das Verschlusselement kann mit geeigneten Mitteln, wie z.B. einem herkömmlichen Ballonkatheter ähnlich dem, der zur Ablage von Stents, Aortenprothesen und verschiedenen Arten von Prothesen verwendet wird, an der gewünschten Stelle im Körperlumen abgelegt werden.
- In einer zurzeit bevorzugten Ausführungsform besitzt das Verschlusselement ein offenes Gerüst, so dass Endothelgewebe durch die Öffnungen einwachsen und so das Verschlusselement und die Wand des Körperlumens miteinander verbinden kann. Die Oberfläche des Verschlusselements kann zur Förderung der Endothelialisierung behandelt werden.
- Nach Implantation des Verschlusselements im Körperlumen und ausreichender Endothelialisierung zu seiner Fixierung an der Körperwand (was eine Woche oder mehr dauern kann) kann es durch Erwärmung auf eine Temperatur bei oder über der Übergangstemperatur des Formgedächtnismaterials aktiviert werden, damit es wieder seine erinnerte eingeengte Form annehmen kann. Da das Verschlusselement durch die Endothelialisierung an der Wand des Körperlumens fixiert wurde, führt Kontraktion des Verschlusselements zu seiner erinnerten kollabierten Gestalt dazu, dass die das Körperlumen definierende Wand zusammen mit dem Verschlusselement kollabiert und so den Durchgang wirksam blockiert.
- Der Pfropf befindet sich im Inneren des Verschlusselements vor der Wärmeaktivierung, so dass das Verschlusselement zur Blockierung des Lumens auf den Pfropf kollabiert.
- Das Verschlusselement kann auf der Außenseite eines Ballons eines Dilatationsballonkatheters in der ersten Konfiguration mit kleinen Querabmessungen befestigt und dann in die Region des zu verschließenden Reproduktionslumens eingeführt und darin positioniert werden. Der Ballon wird aufgeblasen, um das Verschlusselement zu expandieren, wobei vorzugsweise der Außendurchmesser etwas größer ist als die Innenabmessungen des Reproduktionslumens, an dem fixiert wird. Das Verschlusselement bleibt bis zur Erwärmung auf eine Temperatur bei oder über seiner Übergangstemperatur von Martensit zu Austenit, bei der es in den kollabierten Zustand zurückkehrt, in der offenen Konfiguration. Wenn das Verschlusselement um einen Pfropf kollabiert wird, kann der Pfropf zur Wiedereröffnung des Durchgangs herausgezogen werden, wenn der Patient wieder fruchtbar sein will.
- Die vorliegende Erfindung sorgt für wirksame Sterilisation oder Kontrazeption für Männer und Frauen und lässt sich leicht rückgängig machen, was ein wichtiger Aspekt ist. Darüber hinaus sind für die Implantation und Aktivierung des Verschlusselements und die anschließende Wiederherstellung der Gefäßdurchgängigkeit leicht anzuwendende minimal invasive Vorrichtungen, wie z.B. Katheter, Führungsdrähte, Führungskatheter und dergleichen, notwendig. Diese und andere Vorteile der Erfindung gehen aus der nachfolgenden detaillierten Beschreibung der Erfindung in Verbindung mit den beiliegenden beispielhaften Zeichnungen deutlich hervor.
- KURZE BESCHREIBUNG DER ZEICHNUNGEN

1 zeigt einen Katheter10 , der sich für die Umsetzung der Erfindung eignet und der einen länglichen Schaft12 mit einem Inflationslumen14 umfasst, das in Flüssigkeitsverbindung mit einem aufblasbaren Element16 steht, das auf einem distalen Abschnitt des Katheterschafts und des Adapters18 befestigt ist. Das Verschlusselement20 , ein sich selbst stützendes Metallelement aus einem Formgedächtnismaterial, passt sich eng an den Durchmesser des nicht aufgeblasenen aufblasbaren Elements16 an, um die Einführung in das gewünschte Körperlumen zu erleichtern. Das Verschlusselement20 ist so geformt, dass es eine erinnerte kollabierte Konfiguration mit relativ kleinen Querabmessungen aufweist. Das Verschlusselement20 kann verformt werden, um die Befestigung auf dem aufblasbaren Element16 zu erleichtern, und es wird durch das aufblasbare Element zu einer offenen expandierten Konfiguration im Körperlumen expandiert. Beim Erhitzen auf eine Übergangstemperatur kehrt es wieder zu seiner erinnerten Konfiguration zurück. Das Verschlusselement20 hat eine offene gitterartige Struktur, die die Endothelialisierung erleichtert, wodurch das Verschlusselement an der das Körperlumen definierenden Wand befestigt wird. Vorzugsweise kann das Verschlusselement20 auf einen expandierten Durchmesser verformt werden, der vorzugsweise gleich groß wie oder etwas größer ist als die Abmessungen des Körperlumens, in dem das Verschlusselement angeordnet werden soll. Zur Anordnung in den Eileitern einer Patientin sollten die expandierten Querabmessungen bei ca. 0,1 mm bis ca. 5 mm liegen. - Das erfindungsgemäße Verschlusselement kann eine Vielzahl von geeigneten Konfigurationen aufweisen, wie sie schematisch in
2 und3 gezeigt sind.2 zeigt das Verschlusselement26 in einer offenen Konfiguration und3 ist eine Konfiguration mit relativ kleiner Abmessung zum Einführen und Vorschieben in das Körperlumen des Patienten. Das Verschlusselement26 kann aus einem Stück hypodermischen Schlauch mit Formgedächtnis hergestellt werden. In2 und3 ist das Verschlusselement26 eine Spule28 aus Formgedächtnisdraht oder -band. In1 und4 umfasst das Verschlusselement20 eine Reihe von geschlossenen sinusförmigen Ringen aus Formgedächtnisdraht oder -band und ist auf einem aufblasbaren Element16 des Katheters10 befestigt. - Durch Aufblasen des aufblasbaren Elements
16 wird das Verschlusselement20 in einem Reproduktionstrakt38 bis zu einer offenen Konfiguration mit relativ großem Durchmesser wie in5 gezeigt expandiert. - In jeder dieser Ausführungsformen sollte das Formgedächtnismaterial des Verschlusselements eine Übergangstemperatur aufweisen, die ausreichend über der normalen Schwankung der menschlichen Körpertemperatur liegt, um versehentliche Aktivierung zu verhindern, wodurch das Verschlusselement vorzeitig kollabieren könnte. Andererseits sollte die Übergangstemperatur so hoch sein, dass thermische Aktivierung des Verschlusselements nicht zu unerwünschten Wärmeschäden am umgebenden Gewebe führt. Das Formgedächtnismaterial ist vorzugsweise eine Nickel-Titan-Legierung mit Formgedächtnis, wie z.B. NITINOL, und weist vorzugsweise eine Übergangstemperatur zwischen ca. 43°C und ca. 70°C auf.
- In jeder der oben aufgeführten Ausführungsformen können bestimmte konventionelle Verbesserungen vorgenommen werden. Beispielsweise kann die Oberfläche des Gerüsts des Verschlusselements so ausgestaltet sein, dass Endothelwachstum begünstigt wird. Bei solchen Modifikationen wird im Allgemeinen das Verschlusselement mit einem offenen Gerüst versehen, um das Endothelwachstum in und um das Element zu fördern und so sicherzustellen, dass es fest an der Wand des Körperlumens befestigt ist. Geeignete Oberflächentechniken umfassen EDM, Laserbohren, Fotoätzen, Sintern und dergleichen. Darüber hinaus kann die Vergrößerung der Oberfläche des Verschlusselements auch für stärkere Adhäsion des Endothelgewebes sorgen. Geeignete Oberflächenbehandlungen umfassen Plasmaätzen, Sandblasen, maschinelle Bearbeitung und andere Behandlungen zum Aufrauen der Oberfläche. In anderen Ausführungsformen kann das Formgedächtnismaterial zum Fördern der Endothelialisierung beschichtet oder geimpft sein. Beispielsweise kann die Verschlussvorrichtung mit einem Polymerüberzug versehen sein, der mit einem Arzneimittel, einem Enzym oder einem Protein imprägniert ist, um das Wachstum von Endothelgewebe auszulösen oder zu fördern. In einer anderen Verbesserung könnte das Verschlusselement mit Kupfer plattiert sein oder anderweitig Kupfer enthalten, um eine Entzündungsantwort im Gewebe der das Körperlumen definierenden Wand auszulösen, die weiter zur Obstruktion des Lumens beiträgt. Andere entzündliche Materialien können ebenfalls geeignet sein. Beispielsweise könnte das Verschlusselement radioaktiv sein oder Alpha-, Beta- oder Gammateilchen emittieren.
- Die Umsetzung der Erfindung umfasst die folgenden allgemeinen Schritte. Ein Verschlusselement
20 mit einer relativ kleinen Querabmessung ist auf der Außenseite des Ballons16 des in1 gezeigten Katheters10 befestigt. Der Katheter10 wird unter fluoroskopischer oder endoskopischer Visualisierung vorgeschoben, bis das Verschlusselement20 in einem der Eileiter34 der Patientin angeordnet ist, wie in6 gezeigt ist. Aufblasflüssigkeit wird durch den Adapter18 eingeführt, um das aufblasbare Element16 aufzublasen. Wie in5 –6 gezeigt, wird durch Aufblasen des aufblasbaren Elements16 das Verschlusselement20 zu einer offenen Konfiguration expandiert und es setzt sich im Körperlumen38 fest. Der Katheter10 wird entfernt, so dass das expandierte Verschlusselement20 im Körperlumen38 implantiert bleibt, wie in8 zu sehen ist. Ein weiteres expandierbares Element wird in den Eileiter der Patientin eingeführt und darin auf die gleiche Weise expandiert. Alternativ kann das Verschlusselement expandiert und im Vas deferens36 eines männlichen Patienten wie in7 gezeigt angeordnet werden, um mit demselben Verfahren für Kontrazeption beim Mann zu sorgen. - Über einen Zeitraum von einer Woche oder mehr proliferieren die das Lumen auskleidenden Endothelzellen und wachsen um das offene Gerüst des Verschlusselements
20 wie in8 und9 gezeigt, so dass die das Körperlumen38 definierende Wand an dem expandierten Verschlusselement20 fixiert wird. Wenn das expandierte Verschlusselement20 im Reproduktionstrakt38 des Patienten ausreichend endothelialisiert ist, wird es durch Wärme aktiviert, damit es wieder seine erinnerte kollabierte Konfiguration annimmt. Das Verschlusselement kann mit mehreren Mitteln aktiviert werden, einschließlich erwärmte Flüssigkeit, HF-Energie, Laserenergie oder andere geeignete Energiequellen. Ein geeignetes Aktivierungssystem ist in10 gezeigt, wo das distale Ende des Katheters40 neben dem Verschlusselement20 positioniert ist, Kochsalzlösung etwas oberhalb der Übergangstemperatur zum Baden des Verschlusselements20 eingeführt wird, wodurch seine Temperatur auf den Übergangspunkt oder höher steigt, so dass das Verschlusselement20 zu seiner geschlossenen Konfiguration mit dem verringerten Durchmesser kollabiert. Die Endothelgewebeschicht, die sich in der Struktur des Verschlusselements bildet, hilft, das Lumen zu blockieren und zu verschließen, um die Passage von Reproduktionszellen, Eiern oder Spermazellen zu verhindern. - In
12 ist ein Pfropf42 im Verschlusselement20 im expandierten Zustand positioniert, so dass bei Aktivierung das Verschlusselement20 auf den Pfropf42 kollabiert und das Lumen38 blockiert. Der Pfropf besteht vorzugsweise aus einem inerten Material, wie z.B. einem Fluorpolymer, z.B. PTFE. Andere geeignete Materialien sind Polyethylen hoher Dichte und Silikonkautschuk. Auch mehrere Abwandlungen des Pfropfes sind geeignet. Der Pfropf könnte beispielsweise als Arzneimittelabgabevorrichtung verwendet werden, ähnlich wie NorplantTM. Der Pfropf könnte auch zur Auslösung einer Entzündungsantwort wie oben beschrieben verwendet werden, um den Verschluss des Lumens zu verstärken. Bei solchen Ausführungsformen hat der Pfropf42 vorzugsweise einen Außendurchmesser von ca. 0,25 mm bis ca. 4 mm. Der Pfropf42 weist Löcher auf, die helfen, zumindest einen Teil der natürlichen Auskleidung des Reproduktionstraktes zu erhalten. - Der Verschluss des Lumens kann einfach durch Entfernen des Pfropfes
42 rückgängig gemacht werden. Wenn ein größerer Durchgang gewünscht wird als der Durchgang, der nach Entfernen des Pfropfes42 zurückbleibt, kann ein Ballonkatheter im Körperlumen vorgeschoben werden, bis der Ballon sich in dem Lumen befindet, das nach Entfernen des Pfropfes zurückbleibt, und der Ballon auf dem Katheter wird dann aufgeblasen, um das Verschlusselement20 zu expandieren und es in seine offene Konfiguration zu verformen. Bei der Aktivierung des expandierten Verschlusselements zur kollabierten Konfiguration kann es wünschenswert sein, das proximale Ende des Verschlusselements etwas offen oder in einem expandierten Zustand zu halten, um die Einführung des Dilatationsballons auf einem Katheter zur leichteren Öffnung des Körperlumens zu erleichtern. Wie in11 gezeigt kann der zur Aktivierung des Verschlusselements verwendete Katheter40 im proximalen Ende des Verschlusselements positioniert werden, so dass das proximale Ende nicht völlig in seine geschlossene Konfiguration zurückkehren kann. Der Reproduktionstrakt könnte sich anschließend schließen, wenn wieder Kontrazeption gewünscht wird, indem das Verschlusselement20 erhitzt und so seine Umwandlung in die kollabierte Konfiguration aktiviert wird. - In erfindungsgemäßen Ausführungsformen, in denen der Pfropf
42 zur Anwendung kommt, eignen sich auch verschiedene andere Strategien zur Umkehrung des Verschlusses. Beispielsweise kann der Pfropf42 einfach entfernt werden, so dass die Durchgängigkeit des Lumens38 wieder hergestellt wird. Alternativ kann der Pfropf42 hohl sein und einen entfernbaren Kern aufweisen (nicht gezeigt). Dieser Kern kann aus einem weicheren Material wie z.B. Silikon bestehen oder ein Gewinde aufweisen, um seine Entfernung zu erleichtern. Analog kann auch der Pfropf selbst ein Gewinde aufweisen, so dass bei der Entfernung eine Drehbewegung ausgeübt wird, wodurch die Belastung des Gewebes, in dem sich das Verschlusselement befindet, minimiert wird. - In anderen Ausführungsformen können mechanische, klebende oder andere Mittel zur Fixierung des expandierten Verschlusselements
20 an der den Reproduktionstrakt38 definierenden Wand verwendet werden. Beispielsweise können die Mittel zur Fixierung eines Stents oder einer Prothese an einer Aorten- oder Arterienwand wie inUS-Patent Nr. 4.140.126 ;US-Patent Nr. 4.562.596 ;US-Patent Nr. 4.577.631 ;US-Patent Nr. 4.787.899 ;US-Patent Nr. 5.104.399 ;US-Patent Nr. 5.167.614 ;US-Patent Nr. 5.275.622 ;US-Patent Nr. 5.456.713 ; undUS-Patent Nr. 5.489.295 beschrieben mit der vorliegenden Erfindung zur Verbindung der den Reproduktionstrakt definierenden Wand mit dem expandierbaren Element verwendet werden. - Verschiedene Modifikationen und Verbesserungen sind an der vorliegenden Erfindung möglich. Beispielsweise kann ein mechanisches expandierbares Element, wie in
US-Patent Nr. 4.585.000 beschrieben, zum Expandieren des expandierbaren Elements im Reproduktionstrakt verwendet werden, um in die Wand des Trakts einzugreifen.
Claims (20)
- Kontrazeptive Vorrichtung, die eine röhrenförmige Konstruktion (
26 ) mit einer Längsachse, die zumindest teilweise so konfiguriert ist, dass sie radial um die Längsachse in einem Lumen (38 ) des Reproduktionssystems einer Patientin aus einer ersten röhrenförmigen Konfiguration in eine zweite gehaltene röhrenförmige Konfiguration mit größeren Querabmessungen als die erste röhrenförmige Konfiguration expandiert werden kann, und einen das Lumen blockierenden Pfropf (42 ) in der expandierbaren röhrenförmigen Konstruktion umfasst, wobei die röhrenförmige Konstruktion eine Spirale mit einer ersten zylinderförmigen Spiralkonfiguration und einer zweiten, radial expandierten, zylinderförmigen gehaltenen Spiralkonfiguration aufweist, die nicht wesentlich größer ist als die Querabmessungen des Reproduktionslumens (38 ) der Patientin, dadurch gekennzeichnet, dass die röhrenförmige Konstruktion ein offenes Gerüst in der zweiten gehaltenen expandierten Konfiguration aufweist, um die Fixierung zumindest eines Teils der röhrenförmigen Konstruktion (26 ) über die Länge eines Wandabschnitts, der zumindest teilweise das Lumen (38 ) des Reproduktionssystems einer Patientin definiert, und das Einwachsen von Gewebezellen durch die Länge zu erleichtern, wobei der in der röhrenförmigen Konstruktion (26 ) angeordnete Pfropf (42 ) Löcher aufweist, die dazu beitragen, zumindest einen Teil der natürlichen Auskleidung des Reproduktionslumens (38 ) zu erhalten. - Kontrazeptive Vorrichtung nach Anspruch 1, worin die Spiralkonfiguration in der expandierten Konfiguration voneinander beabstandete Windungen über die Länge aufweist.
- Kontrazeptive Vorrichtung nach Anspruch 1, worin die röhrenförmige Konstruktion (
26 ) mindestens ein offenes Ende aufweist. - Kontrazeptive Vorrichtung nach Anspruch 3, worin das offene Ende ein proximales Ende ist.
- Kontrazeptive Vorrichtung nach Anspruch 1, worin die röhrenförmige Konstruktion so konfiguriert ist, dass sie das Einwachsen von Gewebe fördert.
- Kontrazeptive Vorrichtung nach Anspruch 1, worin der Pfropf ein Entzündungsmaterial aufweist, das eine Entzündungsantwort hervorrufen kann.
- Kontrazeptive Vorrichtung nach Anspruch 1, worin der Pfropf eine Außengewindekonfiguration aufweist.
- Kontrazeptive Vorrichtung nach Anspruch 1, worin der Pfropf (
40 ) ein hohles Rohr und einen entfernbaren Kern umfasst. - Kontrazeptive Vorrichtung nach Anspruch 2, worin der Pfropf (
42 ) ein festes zylindrisches Element im röhrenförmigen Körper ist. - Kontrazeptive Vorrichtung nach Anspruch 1, worin der Pfropf zur Auslösung einer Entzündungsantwort verwendet wird.
- Kontrazeptive Vorrichtung nach Anspruch 1, worin die röhrenförmige Konstruktion zumindest teilweise so konfiguriert ist, dass sie die zweite expandierte röhrenförmige Konfiguration über eine für das Einwachsen von Gewebe ausreichende Zeit im Wesentlichen hält, um zumindest einen expandierten Teil der röhrenförmigen Konstruktion zu epithelialisieren.
- Kontrazeptive Vorrichtung nach Anspruch 1, worin zumindest ein Teil der röhrenförmigen Konstruktion so konfiguriert ist, dass er in der expandierten zweiten Konfiguration äußere Querabmessungen aufweist, die im Wesentlichen den inneren Querabmessungen des Reproduktionslumens der Patientin an der Expansionsstelle im Reproduktionslumen entsprechen.
- Kontrazeptive Vorrichtung nach Anspruch 1, worin die röhrenförmige Konstruktion so konfiguriert ist, dass sie sich im Wesentlichen an die inneren Querabmessungen des Eileiters der Patientin in der zweiten röhrenförmigen Konfiguration anpasst und zumindest teilweise die zweite röhrenförmige Konfiguration über eine für das Einwachsen von Gewebe ausreichende Zeit im Wesentlichen hält, um zumindest einen expandierten Teil der röhrenförmigen Konstruktion zu epithelialisieren.
- Kontrazeptive Vorrichtung nach Anspruch 1, mit einem abgebbaren Kontrazeptivum.
- Kontrazeptive Vorrichtung nach Anspruch 1, worin die röhrenförmige Konstruktion zumindest teilweise aus einer Nickel-Titan-Legierung besteht.
- Kontrazeptive Vorrichtung nach Anspruch 1, worin die röhrenförmige Konstruktion zumindest teilweise selbst-expandierend ist.
- Kontrazeptive Vorrichtung nach Anspruch 1, worin zumindest ein Teil der röhrenförmigen Konfiguration so konfiguriert ist, dass er in der zweiten expandierten Konfiguration im Reproduktionslumen der Patientin über einen Zeitraum von mindestens sieben Tagen gehalten wird.
- Kontrazeptive Vorrichtung nach Anspruch 1, worin zumindest ein Teil der röhrenförmigen Konstruktion so konfiguriert ist, dass er sich von der zweiten Konfiguration zu einer erinnerten Konfiguration mit kleineren Querabmessungen als die zweite Konfiguration zusammenzieht, um das Reproduktionslumen der Patientin noch weiter zu verschließen.
- Kontrazeptive Vorrichtung nach Anspruch 1, worin die röhrenförmige Konstruktion so konfiguriert ist, dass sie das Einwachsen von Gewebe auslöst, das die röhrenförmige Konstruktion in der zweiten expandierten Konfiguration im Reproduktionslumen der Patienten epithelialisiert, um das Reproduktionslumen im Wesentlichen zu verschließen.
- Kontrazeptive Vorrichtung nach Anspruch 1, worin die expandierte röhrenförmige Konstruktion so konfiguriert ist, dass sie an einem Teil einer Wand, die das Reproduktionslumen der Patientin formt, durch Epithelialisierung fixiert wird.
