Description
- Die vorliegende Erfindung betrifft allgemein Kunstglieder bzw. Prothesen zur Implantierung in Körperlumen und insbesondere einen dehnbaren Stent.
- Transplantatprothesen werden häufig in Blutgefäße implantiert, und zwar speziell in die Aorta oder andere Arterien, die der Bildung von Aneurysmen und/oder schweren arteriosklerotischen Erkrankungen, bei denen vielfache Stenosen auftreten können, unterliegen. Beispielsweise kann sich ein Aortenaneurysma bei einem Patienten etwa in der Bauchschlagader oder an der aortoiliakalen Gabelung entwickeln und eine Behandlung erforderlich machen, bevor die Gefäßwand reißt. Zur Reparatur eines Blutgefäßes, das durch eine solche Erkrankung geschädigt ist, wird im allgemeinen ein Verfahren durchgeführt, bei dem eine Transplantatprothese verwendet wird.
- Es sind zahlreiche Transplantatprothesen vorgeschlagen worden, die ein rohrförmiges Transplantat aufweisen, das an einem Stent angebracht ist. Das rohrförmige Transplantat kann eine biokompatible poröse oder nichtporöse rohrförmige Struktur sein, an der eine Stentstruktur wie etwa ein Drahtnetz angebracht sein kann. Die Stentstruktur kann vorgespannt sein, um eine erweiterte Konfiguration anzunehmen, die einer Zielbehandlungsstelle entspricht, kann aber in einem kontrahierten Zustand eingegrenzt sein, um die Einführung in das Gefäßsystem eines Patienten zu erleichtern. Die Transplantatprothese kann in dem kontrahierten Zustand perkutan eingeführt, bis zu einer Behandlungsstelle innerhalb eines Blutgefäßes vorwärtsbewegt und losgelassen werden, um den erweiterten Zustand anzunehmen und die Behandlungsstelle zu reparieren und/oder zu umgehen.
- Ein bei solchen Prothesen häufig auftretendes Problem besteht darin, das rohrförmige Transplantat an der Behandlungsstelle wirksam zu befestigen. Die losgelassene Transplantatprothese kann eventuell nicht in ausreichendem Eingriff mit der Gefäßwand in der Nachbarschaft der Behandlungsstelle sein, was eventuell dazu führt, daß sich die Transplantatprothese nach der Implantierung bewegt, wodurch die beschädigte Gefäßwand exponiert werden könnte. Plastisch verformbare aufweitbare Stentstrukturen können vorgesehen werden, um eine etwas direktere Kontrolle des Eingriffs zwischen der Transplantatprothese und der Gefäßwand zu versuchen. Solche aufweitbaren Strukturen erfordern aber eventuell den Einsatz eines Ballons oder eines anderen aufweitbaren Elements, um die Stentstruktur in den vergrößerten Zustand aufzuweiten, was die Gefahr einer ungleichmäßigen Aufweitung der Stentstruktur und/oder ein Reißen des Ballons in sich birgt.
- Außer plastisch verformbaren Stents sind Stentstrukturen aus gewickelten Flächenkörpern vorgeschlagen worden. Stents aus gewickelten Flächenkörpern können eine verbesserte Verankerung innerhalb des Blutgefäßes bieten, weil die Größe des völlig aufgeweiteten Stents präziser kontrolliert werden kann. Ein Stent aus gewickeltem Flächenkörper kann jedoch quer zu seiner Längsachse im wesentlichen steif sein, was potentiell zu einer weniger flexiblen Transplantatprothese führt, die eventuell nicht effektiv in eine gewundene Anatomie implantiert werden kann.
- Es besteht daher ein Bedarf für ein verbessertes Stent-Transplantat, das verbesserte Flexibilität bieten kann und doch gleichzeitig eine starke Verankerung in einem Blutgefäß ermöglicht.
- Die vorliegende Erfindung richtet sich auf einen dehnbaren Stent gemäß der Definition in Anspruch 1.

4A und4B sind eine End- bzw. eine Seitenansicht eines Stents mit einem Antiknicksegment; und -
5A und5B sind eine Seiten- bzw. eine Perspektivansicht eines Stents mit dehnbaren Enden. - In den Zeichnungen zeigt
1 ein Stent-Transplantat10 , das ein rohrförmiges Transplantat12 , ein Außenskelett14 sowie einen ersten und einen zweiten Stent16 ,18 aus gewickeltem Flächenkörper aufweist. Das rohrförmige Transplantat12 hat ein erstes und eines zweites Ende20 ,22 , die zwischen sich eine Längsachse24 definieren, und eine Außenumfangswand26 , die einen Umfang28 und darin ein Lumen30 definiert. Das rohrförmige Transplantat12 kann aus vielen verschiedenen biokompatiblen Materialien gebildet sein, bevorzugt aus einem Polymermaterial wie Polyester, Polytetrafluorethylen, Dacron, Teflon und Polyurethan. - Das Außenskelett
14 ist an der Umfangswand26 angebracht und weist eine Vielzahl von Serpentinenelementen32 auf. Das Außenskelett14 kann aus vielen verschiedenen halbsteifen Materialien geformt sein, bevorzugt aus einem biokompatiblen metallischen Material wie Nitinol oder rostfreiem Stahl. Das Material kann elastisch verformbar sein, kann Formgedächtnis-Eigenschaften haben und/oder plastisch verformbar sein, wie nachstehend noch im einzelnen beschrieben wird, um die Gelenkigkeit des Stent-Transplantats10 und/oder das Kollabieren und/oder die Aufweitung des Außenskeletts14 zwischen einem kontrahierten Zustand und einem vergrößerten Zustand zu erleichtern. Das Außenskelett14 kann aus flachem Flächenkörpermaterial gebildet sein, in dem die einzelnen Serpentinenelemente32 durch Ätzen, Stanzen aus dem Flächenkörpermaterial oder auf andere Weise gebildet sind. Alternativ kann das Außenskelett14 aus drahtähnlichen Materialien gebildet sein, beispielsweise durch Formen jedes Serpentinenelements32 aus einer einzigen Drahtlitze. - Das Außenskelett
14 kann an der Außenseite der Umfangswand26 oder an der Innenseite der Umfangswand26 angebracht oder alternativ in die Umfangswand26 eingebettet sein, wobei der Ausdruck ”Außenskelett” jede dieser Stellen einschließen und nicht auf eine Stelle gegenüber einer anderen beschränkt sein soll. Das Außenskelett14 kann mit mechanischen Befestigungsmitteln wie etwa Nahtmaterial, Drähten, Heftklammern und dergleichen, mit einem Klebstoff oder durch ein Bondverfahren wie etwa Thermobonden, chemisches Bonden oder Ultraschallbonden befestigt werden. - Jedes Serpentinenelement
32 erstreckt sich sowohl ”umfangsmäßig” als auch ”axial” entlang mindestens einem Bereich der Umfangswand26 . ”Umfangsmäßig” bezieht sich auf jedes Serpentinenelement32 , das sich auf eine Weise erstreckt, die die Umfangswand26 im allgemeinen umgibt, was bevorzugt kreisförmig oder elliptisch sein kann, z. B. im allgemeinen um den Umfang oder einen anderen Umkreis der Umfangswand26 herum, während sich ”axial” auf das Serpentinenelement32 bezieht, das sich entlang der Umfangswand26 im allgemeinen parallel zu der Längsachse24 erstreckt. Somit definiert jedes Serpentinenelement32 eine allgemeine ”Zickzack”-Gestalt, die beispielsweise aus abrupten ”Z”- und/oder gerundeten ”U”-förmigen Elementen besteht, die integral miteinander verbunden sind. - In den
1 und2 sind die Serpentinenelemente14 durch eine Vielzahl von Zickzackelementen definiert, die im allgemeinen gerade axiale Zonen32a und gekrümmte Umfangszonen32b aufweisen, die integral miteinander ausgebildet sind und sich im wesentlichen umfangsmäßig um die Umfangswand26 herum erstrecken. Somit bilden die Serpentinenelemente32 ein vielzelliges Außenskelett14 , das die Beweglichkeit zwischen benachbarten Serpentinenelementen32 erleichtern kann, wenn das Stent-Transplantat10 in bezug auf die Längsachse24 im wesentlichen quer gerichtet wird. - Bei einer Ausführungsform sind die Serpentinenelemente
32 durch Verbinderelemente34 miteinander verbunden, die sich bevorzugt im wesentlichen axial zwischen benachbarten Serpentinenelementen32 erstrecken. Die Verbinderelemente34 können geformt sein, sie können geätzt oder ausgestanzt sein, wenn die Serpentinenelemente aus einem flachen Flächenkörper gebildet sind, oder die Verbinderelemente34 können Drahtlitzen sein, die an den Serpentinenelementen32 auf eine herkömmliche Weise ange bracht sind. Alternativ können die Serpentinenelemente32 separate Strukturen sein, die einzeln an der Umfangswand26 des rohrförmigen Transplantats12 angebracht sind. - Die Stents
16 ,18 aus gewickeltem Flächenkörper können an den jeweiligen Enden20 ,22 des rohrförmigen Transplantats, bevorzugt an der Innenseite der Umfangswand26 , angebracht sein, obwohl die Stents16 ,18 aus gewickeltem Flächenkörper alternativ als von dem rohrförmigen Transplantat12 separate Komponenten vorgesehen sein können. Die Stents16 ,18 aus gewickeltem Flächenkörper können sich automatisch aufweiten, sind aber bevorzugt mechanisch aufweitbar, beispielsweise sind sie schrittweise auf größere Durchmesser aufweitbar, indem beispielsweise ein Ballon oder ein anderes aufweitbares Element (nicht gezeigt) verwendet wird. - Die Stents
16 ,18 aus gewickeltem Flächenkörper haben eine dehnbare Konstruktion, ein dehnbares knickfestes Segment und/oder ein dehnbares Kronenende. Beispielsweise zeigt3 einen vollständig dehnbaren Stent410 aus gewickeltem Flächenkörper, der aus einer im wesentlichen flachen Netzstruktur412 gebildet ist, die einzelne elastische Netzelemente420 definiert und Zähne414 entlang einem Seitenrand416 davon hat, die in den Netzelementen420 aufzunehmen sind. Die Netzstruktur412 kann gerollt oder gewickelt werden, um eine Längsachse418 und einen Umfang oder Umkreis (nicht gezeigt) in einer Ebene zu definieren, die zu der Längsachse418 im wesentlichen senkrecht ist. Die Netzstruktur412 kann aus rostfreiem Stahl bestehen. - Bei der bevorzugten Ausführungsform ist die Netzstruktur
412 jedoch aus Nitinol oder einem gleichartigen Formgedächtnis-Material gebildet, das beispielsweise poliert und/oder wärmebehandelt wurde. In einem spannungsfreien Zustand, z. B. in der austenitischen Phase, definieren die Netzelemente420 bevorzugt einen ”gedehnten” Zustand, d. h. sie sind um den Umfang der Netzstruktur412 herum erweitert, so daß die Netzstruktur412 vorgespannt ist, um eine größere Größe anzunehmen, z. B. im wesentlichen gleichartig mit dem Querschnitt eines Blutgefäßes, in das der Stent410 zu implantieren ist. Die Netzelemente420 können eine ”nichtgedehnte” Konfiguration annehmen, d. h. sie können um den Umfang der Netzstruktur412 herum komprimiert werden, so daß die Netzstruktur412 eine erheblich verringerte Größe annimmt. Dies kann erreicht werden durch Umwandlung des Nitinolmaterials der Netzstruktur412 in eine martensitische Phase, beispielsweise beim Abkühlen nach einer Wärmebehandlung. Der Stent410 kann dann zu einem verringerten Versandprofil gerollt und/oder kollabiert werden, um an einem Stent-Transplantat angebracht zu werden, wie oben beschrieben wurde. - Wenn der Stent
410 in ein Blutgefäß implantiert wird, kann sich die Netzstruktur412 ausdehnen oder in ihren spannungsfreien Zustand, z. B. die austenitische Phase, zurückkehren und sich aufweiten, um mit der Gefäßwand in Eingriff zu gelangen. Wenn von dem Blutgefäß ein radialer Druck auf den Stent410 aufgebracht wird, können die Netzelemente420 um den Umfang herum komprimiert werden, wodurch sich der Stent410 wieder aufwickeln kann und die Gefahr eines Knickens des Stents410 , die auftreten kann, wenn ein herkömmlicher Stent aus gewickeltem Flächenkörper erheblichen radialen Kompressionskräften ausgesetzt wird, im wesentlichen beseitigt ist. - Die
4A und4B zeigen eine andere Ausführungsform eines Stents510 aus gewickeltem Flächenkörper, der ein dehnbares knickfestes Segment512 hat, das aus einer Netzstruktur gebildet ist, die an einem gewickelten Flächenkörperbereich514 angebracht ist. Der gewickelte Flächenkörperbereich514 weist Zähne516 entlang einem Seitenrand518 auf und kann so gerollt oder gewickelt sein, daß er überlappte innere und äußere Längsabschnitte524 ,526 , eine Längsachse520 und einen Außenumfang522 definiert, so daß sich das knickfeste Segment512 in Axialrichtung, d. h. im wesentlichen parallel zu der Längsachse520 erstreckt. Ebenso wie bei der vorhergehenden Ausführungsform kann das knickfeste Segment512 aus Nitinol gebildet sein, das wärmebehandelt und gedehnt werden kann und danach abkühlt und ungedehnt ist. Das axial orientierte knickfeste Segment512 erleichtert das Wiederaufrollen des gesamten Stents510 , wenn er radialen Kompressionskräften ausgesetzt ist, indem Netzelemente524 vorgesehen sind, die um den Umfang522 herum zusammengedrückt werden können, wie oben beschrieben wird. Somit kann der Stent510 die Vorzüge sowohl eines Stents aus gewickeltem Flächenkörper, der um seinen Umfang herum im allgemeinen nichtkompressibel ist, als auch einer drehnbaren Stentstruktur in sich vereinen. - Die
5A und5B zeigen eine andere Ausführungsform eines Stents610 , der ein knickfestes Segment oder ”Kronenende”616 an einem Ende614 eines gewickelten Flächenkörperbereichs612 aufweist. Der gewickelte Flächenkörperbereich612 und das knickfeste Segment616 weisen Zähne618a ,618b entlang einem Seitenrand620 davon auf und können so gerollt sein, daß eine Längsachse622 und ein Umkreis624 definiert sind. Das knickfeste Segment616 ist bevorzugt poliert, zu einer gewünschten Gestalt wärmebehandelt, abgekühlt und ungedehnt und dann zu seinem kollabierten und gerollten Versandprofil gewickelt. Nach dem Implantieren können Netzelemente626 in dem knickfesten Segment616 ähnlich wie die oben beschriebenen Ausführungsbeispiele komprimiert werden, wenn der Stent610 radialen Kompressionskräften ausgesetzt wird, so daß die Enden des Stents610 sich konisch verjüngen. Alternativ kann das Ende628 des knickfesten Segments616 nach außen aufgeweitet sein (nicht gezeigt), um sich dadurch unter radial kompressiven Kräften teilweise wieder aufzuwickeln, so daß der Stent beim Implantieren in ein Blutgefäß eine im wesentlichen gleichmäßige Größe annimmt. - Die Stents
16 ,18 aus gewickelten Flächenkörper weisen ferner nach außen orientierte Haken oder Dorne (nicht gezeigt) zur besseren Verankerung des Stent-Transplantats10 in einer Körperpassage auf. Gerinnungsförderndes Material (nicht gezeigt) kann an den äußeren Oberflächen der Stents16 ,18 aus gewickeltem Flächenkörper oder alternativ an den Enden20 ,22 des rohrförmigen Transplantats12 vorgesehen sein, um den dichten Abschluß an der Wand der Körperpassage zu verstärken.
Claims (10)
- Dehnbarer Stent (
510 ), der folgendes aufweist: einen gewickelten Flächenkörper, der einen inneren und einen äußeren Längsabschnitt (524 ,526 ) aufweist, die einander überlappen, und der zwischen einer ersten Größe und ein oder mehreren größeren Größen radial aufweitbar ist, wobei der gewickelte Flächenkörper einen Umfang (522 ) in einer Ebene definiert, die zu einer Längsachse (520 ) davon im wesentlichen senkrecht ist; eine Vielzahl von Zähnen (516 ), die sich von dem inneren Längsabschnitt (524 ) erstrecken, um mit Öffnungen in dem äußeren Längsabschnitt (526 ) in Eingriff zu gelangen, um den gewickelten Flächenkörper in der einen oder den mehreren größeren Größen selektiv festzulegen; und einen dehnbaren Bereich (512 ), der eine Vielzahl von elastischen Netzelementen aufweist, die sich entlang einem Bereich des gewickelten Flächenkörpers erstrecken, wobei die Netzelemente in eine gedehnte Konfiguration vorgespannt sind, die um den Umfang des gewickelten Flächenkörpers herum eine größere Größe annimmt, wobei die Netzelemente zu einer ungedehnten Konfiguration kompressibel sind, die um den Umfang des gewickelten Flächenkörpers herum eine kleinere Größe als die gedehnte Konfiguration definiert, wenn der Stent in Radialrichtung wirkenden Druckkräften ausgesetzt ist. - Stent (
510 ) nach Anspruch 1, dadurch gekennzeichnet, daß sich der dehnbare Bereich (512 ) in Längsrichtung zwischen dem ersten und dem zweiten Ende des gewickelten Flächenkörpers (510 ) erstreckt. - Stent nach Anspruch 1, dadurch gekennzeichnet, daß der dehnbare Bereich an einem Ende des gewickelten Flächenkörpers (
610 ) eine Hülse (616 ) aufweist. - Stent (
510 ) nach Anspruch 1, dadurch gekennzeichnet, daß der gewickelte Flächenkörper Nitinol aufweist. - Stent (
510 ) nach Anspruch 4, dadurch gekennzeichnet, daß der gewickelte Flächenkörper so vorgespannt ist, daß er eine zweite Größe annimmt, die größer als die erste Grö ße ist, wenn das Nitinol in seiner austenitischen Phase ist, wodurch die Netzelemente in die gedehnte Konfiguration vorgespannt werden. - Stent (
510 ) nach Anspruch 4, dadurch gekennzeichnet, daß der gewickelte Flächenkörper (510 ) zu einer dritten Größe kompressibel ist, die kleiner als die zweite Größe ist, wenn das Nitinol in einer martensitischen Phase ist, wodurch die Netzelement die ungedehnte Konfiguration definieren. - Stent (
510 ) nach Anspruch 1, dadurch gekennzeichnet, daß der gewickelte Flächenkörper einen ungedehnten Bereich aufweist, der um den Umfang herum allgemein nicht kompressibel ist. - Stent (
410 ) nach Anspruch 1, dadurch gekennzeichnet, daß der gesamte Stent (410 ) aus elastischen Netzelementen (412 ) besteht. - Stent nach Anspruch 8, dadurch gekennzeichnet, daß der Stent (
410 ) aus Nitinol besteht, wobei die Netzelemente (412 ) so kompressibel sind, daß sie den ungedehnten Zustand definieren, wenn das Nitinol in einer martensitischen Phase ist, und wobei die Netzelemente (412 ) in einer austenitischen Phase in den gedehnten Zustand vorgespannt werden. - Stent nach Anspruch 9, dadurch gekennzeichnet, daß der Stent so konfiguriert ist, daß er in einem Blutgefäß die austenitische Phase annimmt.
