Description
- Die vorliegende Erfindung beschreibt ein bioabsorbierbares, aus Schichten bestehendes chirurgisches Implantat, das zwei Bestandteile aufweist. Ein Bestandteil ist eine feste Platte aus einem bioabsorbierbaren Polymer, der andere Bestandteil ist ein aus bioabsorbierbaren Fasern hergestelltes Flachmaterial. Diese Implantate lenken und fördern die Knochenheilung und schützen die Bindegewebe unterhalb des heilenden Knochens. Diese Implantate eignen sich besonders für die Kranioplastik.
- Die gelenkte Knochenregeneration mit Hilfe von Implantaten hat eine lange Geschichte, insbesondere auf dem Gebiet der Kranioplastik, auf dem ein großes Bedürfnis besteht, eine Schädigung des Gehirns durch Abdecken von Löchern und anderen Defekten im Schädelknochen zu verhindern. Die zur Erzielung einer gelenkten Knochenregeneration eingesetzten Materialien, wie sie in der Kranioplastik Verwendung finden, müssen mehreren Kriterien genügen. Sie müssen zum Beispiel eine gute Biokompatibilität sowie hohe mechanische Festigkeit besitzen. Sie sollten darüber hinaus auch keine Knochenerosion verursachen.
- In der Vergangenheit wurden Knochengewebetransplantate beispielsweise als Allotransplantate aus Hundeknochen, menschlichen Knochen, entkalkten Knochen, Pericranium sowie als Autotransplantate aus der Tibia, Rippen und der Crista iliaca hergestellt; vergleiche Zeiss Index and History of Plastic Surgery 900 BC – 1863 AD, Baltimore, Williams & Wilkins, 1977, Band 1, Seiten 51 bis 52; Chase, S.W., Herndon, C.H., The Fate of Autogenous and Homogenous Bone Grafts: A Historical Review, Journal of Bone Joint Surgery 37A, 1955, Seiten 809 bis 841; Prolo, D.J., Cranial Defects and Cranioplasty, in Wilkins, R.H., Rengachary, S.S. Mit der Verwendung von Knochengewebstransplantaten sind allerdings Probleme verbunden. Wenn patienteneigener Knochen als Transplantat verwendet wird, muss der Chirurg eine zusätzliche, traumatische Operation vornehmen, um die Knochenprobe zu gewinnen. Wenn das Knochentransplantat von einer anderen Person oder von einem Tierknochen verwendet wird, besteht die Möglichkeit von viralen Kontaminationen oder immunologischen Problemen, auch wenn das Transplantat behandelt wird, um es mit dem Gewebe des Patienten verträglich zu machen.
- Ferner wurden künstlich hergestellte biostabile Materialien für kranioplastische Anwendungen untersucht, wie etwa Cellulosefasern, Aluminium, Gold, Titan, rostfreier Stahl, Polymethylmethacrylat (PMMA), Polyethylen und Silicone, vergleiche Habal, M.B., Leake, D.L., Maniscako, J.E., A New Method for Reconstruction of Major Defects in the Cranial Vault, Surgery Neurology 6, 1976, Seiten 137 bis 138; Karvounis, P.C., Chiu, J., Sabin, H., The Use of Prefabricated Polyethylene Plate for Cranioplasty, Journal of Trauma 10, 1970, Seiten 249 bis 254, sowie Black, S.P.W., Reconstruction of the Supraorbital Ridge Using Aluminum, Surgery Neurology 9, 1978, Seiten 121 bis 128. Die klinische Anwendung der meisten dieser Materialien wurde aber aufgrund gravierender Gewebsreaktionen verworfen. Ferner sind biostabile Implantate für die Kranioplastik bei Kindern besonders schlecht geeignet, da ein biostabiles Implantat den noch nicht voll entwickelten Schädelknochen am Wachstum zur Erwachsenengröße hindert, weshalb das Implantat in einer zweiten chirurgischen Prozedur entfernt werden muss.
- Zahlreiche der mit biostabilen Materialien verbundenen Probleme können mit Implantaten gelöst werden, die aus bioabsorbierbaren Polymeren hergestellt sind, die weniger entzündliche Reaktionen hervorrufen. Die bioabsorbierbaren Implantate eignen sich auch für Kinder, da diese Implantate vollständig resorbiert werden und die Abbauprodukte über Stoffwechselwege aus dem Körper verschwinden. Darüber hinaus können diese Materialien so gewählt werden, dass sie schnell genug abgebaut werden, sodass das Wachstum des kindlichen Schädels nicht behindert wird, wodurch eine zweite Operation nicht mehr erforderlich wird. Auch bei bioabsorbierbaren Platten besteht allerdings ein Bedürfnis nach Erzielung einer schnelleren Knochenregeneration und Heilung.
- WO 92/10218 A1 offenbart einen implantierbaren bioabsorbierbaren Gegenstand zur Trennung und Regeneration von Gewebe an einer Gewebedefektstelle. Der Gegenstand weist eine Fasermatrix auf, die als Schicht an einer Oberfläche eines als Zellbarriere wirkenden Flachmaterials angebracht ist. Nach der Implantation erlaubt der Gegenstand das Einwachsen von Gewebe in die Fasermatrixseite; das an der Gewebedefektstelle zu regenerierende Gewebe wird durch das die Zellbarriere bildende Flachmaterial vom einwachsenden Gewebe getrennt.
- Der in WO 92/10218 offenbarte Gegenstand ist primär für periodontale Anwendungen sowie zur Nervenreparatur vorgesehen. Obgleich die Verwendung des Gegenstands zur Reparatur von Knochendefekten und zur Knochenregeneration kurz erwähnt ist, ist ausgeführt, dass zu den speziellen Anwendungen enchondrale Knochen, intermembranöse oder runde Knochen sowie Röhrenknochen gehören. Der spezielle Fall der Schädelreparatur ist nicht erwähnt. Dieses Dokument enthält ferner keine Offenbarung einer speziellen Porengröße oder eines speziellen Bereichs von Porengrößen für die Faserschicht.
- WO 98/07384 A1 beschreibt ein resorbierbares, flexibles Implantat in Form eines durchgehenden, makroporösen Flachmaterials, das dazu geeignet ist, biologische Gewebsdefekte wie etwa Knochendefekte vor der Interposition von benachbarten Bindegeweben während der In-vivo-Reparatur zu schützen. Das makroporöse Flachmaterial besitzt Poren mit Durchmessern von 20 bis 3000 μm. Die Vorrichtung von WO 98/07384 A1 weist keine steife bioabsorbierbare Platte auf. Ferner ist, wie in Verbindung mit der vorliegenden Erfindung festgestellt wurde, dieser Porengrößenbereich im Hinblick auf eine maximale Förderung des Knochenzellwachstums erheblich zu groß.
- KURZE ZUSAMMENFASSUNG DER ERFINDUNG
- Es ist somit eine Aufgabe der vorliegenden Erfindung, ein Implantat, insbesondere zur Kranioplastik, anzugeben, das bioabsorbierbar ist und zugleich ausreichende Festigkeit besitzt, um weiches Gewebe, wie etwa das Gehirn, während der Heilungsdauer zu schützen.
- Eine weitere Aufgabe der vorliegenden Erfindung besteht darin, ein Implantat, insbesondere für die Kranioplastik, anzugeben, das leicht angepasst oder geformt und über einen Defekt in einem Knochen, wie etwa dem Cranium, aufgebracht werden kann.
- Eine weitere Aufgabe der vorliegenden Erfindung besteht darin, ein Implantat, insbesondere zur Kranioplastik, anzugeben, das eine schnelle Knochenregeneration fördert, wodurch die Heilungsdauer verkürzt wird.
- Eine weitere Aufgabe der vorliegenden Erfindung besteht darin, ein Implantat, insbesondere für die Kranioplastik, anzugeben, das schnell genug abgebaut wird, sodass das natürliche Wachstum des reparierten Knochens bei Kindern nicht eingeschränkt wird.
- Diese und andere Aufgaben werden durch die vorliegende Erfindung gelöst, die ein steifes und zähes, aber auch leicht zu formendes, aus Schichten aufgebautes bioabsorbierbares Implantat zur gelenkten Knochengewebsregeneration umfasst, das als bioabsorbierbares chirurgisches Implantat zur Kranioplastik verwendet werden kann. Das in der vorliegenden Anmeldung mit mehr Details beschriebene Implantat weist zwei Bestandteile auf. Ein Bestandteil ist eine feste Platte aus einem bioabsorbierbaren Polymer; der andere Bestandteil ist eine Faserlage, die typischerweise aus bioabsorbierbaren Fasern besteht. Diese Implantate wiesen eine Oberflächenstruktur auf, die auf einer Seite das Knochenwachstum fördert und Gewebereizung auf der anderen Seite verhindert. Die Implantate der vorliegenden Erfindung fördern so die Knochenheilung und schützen die weichen Gewebe unterhalb des heilenden Knochens und um das Implantat herum.
- KURZE BESCHREIBUNG DER ZEICHNUNGEN
- Die vorliegende Erfindung wird in Verbindung mit der beigefügten Zeichnung näher beschrieben, wobei
1 eine schematische Querschnittsansicht einer Ausführungsform des Implantats der vorliegenden Erfindung darstellt, das einen Schädeldefekt abdeckt. - DETAILLIERTE BESCHREIBUNG DER ERFINDUNG
- Die vorliegende Erfindung bezieht sich auf ein bioabsorbierbares, das Knochenwachstum lenkendes Implantat, wie etwa ein Implantat zur Kranioplastik, das am Knochen haftet, das Wachstum und die Heilung des Knochens fördert und weiche Gewebe, wie etwa das Gehirn, unterhalb des Implantats schützt. Das Implantat wird hergestellt, indem vollständig oder teilweise zusammengefügt werden:
(a) eine feste und steife bioabsorbierbare Platte und (b) eine flexible und poröse bioabsorbierbare Faserlage. Diese Bestandteile werden als (a) Plattenlage und (b) Faserlage bezeichnet. Die beiden Lagen können aus einem bioabsorbierbaren Homopolymer, einem bioabsorbierbaren Copolymer, einem bioabsorbierbaren Polymerblend oder einem bioabsorbierbaren Kompositmaterial auf Polymerbasis bestehen. Diese Lagen können je nach der jeweiligen besonderen Anwendung aus dem gleichen Material oder aus unterschiedlichen Materialien bestehen. Für das Implantat der vorliegenden Erfindung verwendbare biologisch abbaubare Polymere sind z.B. in WO 96/41596 aufgelistet. -
1 veranschaulicht eine Ausführungsform der vorliegenden Erfindung; das Implantat kann über den Defekt7 in einem Knochen4 , etwa dem Cranium, implantiert werden, um die Gewebe6 innerhalb oder unterhalb des Defekts7 , wie etwa Gewebe innerhalb des Schädels, vor Schädigung zu schützen. Das Implantat1 umfasst zwei Lagen, die steife Plattenlage2 und die Faserlage3 . - Die Plattenlage
2 gibt die Form und die Größe des Implantats vor und besitzt genügende Festigkeit und Steifigkeit, um als Schutzschild für das Gewebe6 wirken zu können. Es kann speziell geschnitten und geformt werden, damit es den Defekt7 und den umgebenden Knochen4 leicht und bündig abdeckt. Bei einer bevorzugten Ausführungsform der vorliegenden Erfindung ist die Oberseite der Plattenlage2 glatt, um eine Reizung von umgebendem Gewebe zu verhindern, die zu einer nachteiligen Reaktion beim Patienten führen könnte. - Die Plattenlage
2 kann aus einem beliebigen herkömmlichen biologisch abbaubaren Material unter Anwendung von in der Kunststofftechnologie bekannten Verfahren hergestellt werden, wozu die Extrusion, das Spritzgießen und/oder die Verformung in festem Zustand oder das Verpressen zur gewünschten Form mit oder ohne Anwendung von Wärme gehören. Es ist ferner auch möglich, die Plattenlage durch mechanische Bearbeitung in die gewünschte Form zu bringen. Auch ist es möglich, eine Kombination von Techniken anzuwenden, zum Beispiel die mechanische Bearbeitung, um ein gekrümmtes Implantat aus einem extrudierten Flachmaterial zu formen. - Die Dicke der Plattenlage
2 liegt allgemein im Bereich von etwa 0,05 bis etwa 3 mm und vorzugsweise im Bereich von etwa 0,2 bis etwa 1,5 mm. Es ist bevorzugt, die Plattenlage2 so dünn wie möglich zu machen, wobei noch ausreichende Steifigkeit erhalten bleibt, um das weiche Gewebe6 adäquat zu schützen. Die Dicke und die anderen Abmessungen der Plattenlage2 (und des Implantats1 ) hängen von der Größe des abzudeckenden Defekts7 sowie der Krümmung des umgebenden Knochens4 ab und können daher in erheblichem Maße variieren. - Die Faserlage
3 ist unterhalb der Plattenlage2 und oberhalb des Knochens4 und des Defekts7 angeordnet. Es wurde in überraschender Weise festgestellt, dass die Faserlage3 das Knochenwachstum dadurch lenkt und fördert, dass eine poröse Oberfläche zur Verfügung gestellt wird, längs deren und in die hinein das Knochengewebe wachsen und sich ausbreiten kann. Die Faserlage3 befindet sich daher auf der Seite des Implantats, das zum Knochen4 und zum Defekt7 hin liegt. Die Porengrößen in der Faserstruktur werden so kontrolliert, dass sie das Knochenwachstum begünstigen. - Die günstige Porengröße zur Förderung des Knochenwachstums längs der Faserlage
3 der vorliegenden Erfindung ergab sich zu 50 bis 400 μm Porendurchmesser. Typischerweise sind Knochenzellen nicht befähigt, leicht in Poren einzuwachsen, die kleiner sind als etwa 30 μm. Bei Porengrößen über etwa 1000 μm ist das Knochenwachstum langsamer, da dann weniger physikalische Struktur vorliegt, an die sich die Knochenzellen während der Regeneration anheften können. Wenn die Porengröße etwa 50 bis etwa 400 μm beträgt, wird das Knochenwachstum am besten gefördert. - Zur Aufrechterhaltung der Porosität der Faserlage während der Herstellung des Implantats
1 werden in einer bevorzugten Ausführungsform der vorliegenden Erfindung die Plattenlage2 und die Faserlage3 lediglich teilweise oder nur lose aneinander befestigt. Hierdurch verbleibt ein hoher Porositätsgrad in der Oberflächenstruktur der Faserschicht3 , was eine rasche Knochenregeneration fördert. Die obere Oberfläche der Faserlage3 sollte mit der Plattenlage2 in Kontakt sein, jedoch nicht vollständig mit der Oberfläche der Plattenlage2 vereinigt sein, damit so die Faserlage3 porös bleibt. - Die poröse Struktur der Faserlage kann aus biologisch abbaubaren Fasern unter Anwendung beliebiger aus der mechanischen Textiltechnologie und der Kunststofftechnologie bekannter Verfahren hergestellt werden. Die Dicke der Fasern kann von etwa 1 bis etwa 200 μm variieren. Bei einer bevorzugten Ausführungsform der Erfindung liegt die Faserdicke im Bereich von etwa 5 bis etwa 150 μm.
- Für den Faserbestandteil dieses Implantats geeignete Strukturen können zum Beispiel ein Stoff, ein Schmalgewebe, ein gestricktes oder gewirktes Material, ein durch Weben hergestelltes Material, ein Geflecht oder ein Faservlies sein. In jedem Fall sollte die Struktur porös mit einer Porengröße von etwa 50 bis etwa 400 μm sein. Der Faserbestandteil kann unter Verwendung eines Typs von Fasern, zum Beispiel von Polyglykolidfasern oder Polylactidfasern, hergestellt werden. Es ist ferner auch möglich, die Faserlage, je nach der speziellen Anwendung und den angestrebten physikalischen Eigenschaften des Implantats, unter Verwendung von zwei oder mehr unterschiedlichen Typen von Fasern herzustellen.
- Bei einer bevorzugten Ausführungsform der vorliegenden Erfindung besteht die Faserlage
3 aus einem biologisch abbaubaren Polymer, das schneller abgebaut wird als das für die Plattenlage2 verwendete Polymer. Somit wird die Faserlage3 vor der Plattenlage2 abgebaut, wodurch der Knochen eine dichtere Struktur entwickelt und sich an der Oberfläche der Platte anheften kann, bevor die Platte zerfällt. Der Plattenbestandteil verbleibt und ergibt die gewünschte Festigkeit und Form und schützt den Defekt, während Festigkeit und Dichte des sich regenerierenden Knochens zunehmen. Gleichzeitig kann, allerdings in einem langsameren Prozess, Knochen auch die andere Seite des Implantats durch Wachsen auf seiner oberen Fläche bedecken, ausgehend von den Rändern der Platte. Unabhängig davon wird das Implantat schließlich resorbiert, nachdem es durch neuen Knochen und/oder neues Bindegewebe ersetzt wurde. Die Resorptionsprodukte verschwinden auf Stoffwechselwegen aus dem Körper. Am Ende ist der Knochendefekt durch eigenen regenerierten Knochen des Patienten bedeckt oder ausgefüllt. - Das Implantat
1 kann, je nach der Implantationsstelle und der Größe des Implantats, nach verschiedenen auf diesem Gebiet bekannten Befestigungstechniken am Knochen4 befestigt werden, etwa durch bioabsorbierbare Nähte, bioabsorbierbare Klammern, Miniklammern oder Mikroklammern, oder mit bioabsorbierbaren Schrauben. In1 ist das Implantat1 mit kleinen bioabsorbierbaren Schrauben5 am Knochen4 befestigt. - Die Lagen des Implantats
1 können z.B. durch Verschweißen miteinander verbunden werden, wie in einer Patentanmeldung von Paasimaa, S., Kellomäki, M., und Törmälä, P., mit dem Titel "A Bioabsorbable 2-Dimensional Multi-Layer Composite Device and its Manufacturing Method" beschrieben ist, die gleichzeitig eingereicht wurde, oder die Lagen können durch Verkleben, durch Heißpressen, durch Ultraschallschweißen oder Schweißen durch andere Verfahren verbunden werden. Die Lagen des Implantats1 können vor der Implantation getrennt bleiben und während der Operation durch gleichzeitiges Vernähen zur Abdeckung des Schädeldefekts miteinander verbunden werden. Sie können auch dadurch miteinander verbunden werden, dass sie unter Verwendung von biologisch abbaubaren Klammern, Miniklammern, Mikroklammern oder Minischrauben am Knochen befestigt werden. Diese Verfahren können auch dazu herangezogen werden, das Implantat1 mit anderen vorher verbundenen Komponenten zu verbinden. - Das Implantat
1 kann verschiedene Additive und Modifizierungsmittel enthalten, welche die Verarbeitbarkeit des Polymers verbessern, wie zum Beispiel Weichmacher und Antioxidationsmittel. Die Bestandteile des Implantats können auch ein oder mehrere bioaktive, das Knochenwachstum stimulierende oder pharmazeutisch wirksame Mittel enthalten, wie zum Beispiel Antibiotika, Wachstumshormone oder Antikoagulantien. Ferner kann auch ein beliebiges biokeramisches Material oder bioaktives Glas (z.B. in Form eines Pulvers, von Schuppen oder Fasern) als Additiv eingesetzt werden, da festgestellt wurde, dass hierdurch die Knochenheilung beschleunigt wird. Typische Beispiele für solche biokeramischen Materialien und bioaktiven Gläser, die sich für die vorliegende Erfindung eignen, sind Hydroxylapatit, Tricalciumphosphat und andere Calciumphosphate, Bioglass® (erhältlich von Research Center, University of Florida, Gainsville, Fla., USA), Ceravital®, Aluminiumoxid, Zirconiumoxid, bioaktives Gel-Glas und andere bioaktive Gläser. - Nach einer besonders vorteilhaften Ausführungsform der vorliegenden Erfindung wird die Faserlage in einem Gel oder einer Paste eingebettet, die Knochenwachstumsfaktor(en), wie NOVOS (hergestellt und vermarktet von Stryker Biotech, Natic, MA, USA), das osteokonduktives Knochencollagen vom Typ I und osteogenes Protein
1 enthält, beinhalten. Diese Wachstumsfaktoren induzieren und stimulieren ferner das Knochenwachstum unter der Kranioplastik, wodurch die Knochenbildung und die Heilung intensiviert werden. - Nach der obigen Beschreibung der vorliegenden Erfindung und bestimmter spezieller Ausführungsformen davon ist für Fachleute unmittelbar klar, dass zahlreiche Abwandlungen und Modifizierungen der vorliegenden Erfindung möglich sind, ohne dass ihr Rahmen verlassen wird. Die nachstehenden nicht einschränkenden Beispiele erläutern verschiedene Ausführungen der vorliegenden Erfindung noch weiter.
- Beispiel 1.
- Die Reparatur eines 10 × 10 mm großen Defekts im Schädel von ausgewachsenen Kaninchen aus Neuseeland wurde unter Anwendung der nachstehenden fünf Methoden (A bis E) durchgeführt, um die Geschwindigkeiten der Knochenregeneration bei jeder dieser Methoden zu vergleichen.
- A. Die Weichgewebe wurden über dem Defekt geschlossen.
- B. Durch Extrudieren von Poly-D,L-lactid (D/L-Verhältnis 96/4) wurde eine Polylactid-Folie einer Dicke von 0,4 mm hergestellt; daraus wurde ein Stück von 15 × 15 mm Größe herausgeschnitten; die Ecken wurden abgerundet, und die Platte wurde zu der gewünschten konvexen Form gebogen. Die Platte wurde mit Stichen mit DEXON-Nahtmaterial (erhältlich von Davis & Geck, USA), die sich in das umgebende Periost erstreckten, über dem Defekt fixiert; die Weichgewebe wurden darüber geschlossen.
- C. Ein Stück einer 0,15 mm dicken Polyglykolid-Membran mit einer Faseroberfläche (Biofix®, vertrieben und hergestellt von Bionx Implants Ltd., Tampere, Finnland) wurde zu der Form der oben zu Methode B beschriebenen Platte geschnitten und über dem Defekt angeordnet, wobei seine Faserseite zum Knochen und zum Defekt hin lag. Die oben zu Methode B beschriebene Platte wurde oben auf den Defekt und die Membran aufgelegt. Die Membran und die Platte wurden mit Stichen mit DEXON-Nahtmaterial, die sich in das umgebende Periost erstreckten, in ihrer Position fixiert; die Weichgewebe wurden darüber geschlossen.
- D. Ein Stück eines Faserflachmaterials, das aus Polyglykolid-Fasern und Fasern aus bioaktivem Glas (Zusammensetzung Na2O 6 Mol-%; K2O 7,9 Mol-%; MgO 7,7 Mol-%; CaO 22,1 Mol-%, P2O5 1,7 Mol-% und SiO2 54,6 Mol-%) bestand, wurde zu der oben zu Methode C beschriebenen Platte geschnitten und über dem Defekt angeordnet, wobei eine Platte (wie oben zu Methode B beschrieben) darauf angeordnet wurde. Die Platte und das Flachmaterial wurden mit Stichen mit DEXON-Nahtmaterial, die in das umgebende Periost reichten, in ihrer Position fixiert; die Weichgewebe wurden über der Platte geschlossen.
- E. 50 μg rekombinanter Wachstumsfaktor (rTGF-β 1, rekombinanter transgener Wachstumsfaktor, erhältlich und bezogen von Helsinki University, Dept. of Orthopedics and Traumatology) wurden mechanisch in ein steriles 85/ 15 (Gew.-%/Gew.-%)-Gemisch eingemischt, das aus Oligo-L-lactat und einem Copolymer aus ε-Caprolacton und D,L-Lactid (Verhältnis D/L 60/40) bestand. Die Paste wurde auf die Faseroberfläche einer Membran wie oben zu Methode C beschrieben aufgestrichen, worauf die Membran mit der den Wachstumsfaktor und den Träger aus dem Polymerblend enthaltenden Oberfläche dem Knochen (und dem Defekt) zugekehrt in Position gebracht wurde. Eine Platte der zur obigen Methode B beschriebenen Art wurde daraufgelegt, wonach die Membran und die Platte mit Stichen mit DEXON-Nahtmaterial, die in das umgebende Periost reichten, in ihrer Position fixiert wurden; die Gewebe wurden über der Platte geschlossen.
- Jede dieser Prozeduren wurde dreimal durchgeführt; die Tiere wurden in allen Fällen nach 12 Wochen getötet.
- In Versuchsreihe A trat kein Knochenwachstum auf. Im Fall der Versuchsreihe B hatte Knochenwachstum bis auf 40 % der Defektfläche stattgefunden, jedoch war der Defekt noch teilweise in der Mitte mit Bindegewebe gefüllt. Die Versuchsreihen C, D und E, bei denen Implantate gemäß der vorliegenden Erfindung verwendet worden waren, zeigten allerdings eine vollkommene Bedeckung des Defekts durch Knochen, obgleich dieser in Versuchsreihe C in der Mitte etwa 50 % dünner war als an den Rändern und in Versuchsreihe D die Mitte etwa 40 % dünner war als an den Rändern. In Versuchsreihe E war der neue Knochen in der Mitte des Defekts lediglich 30 % dünner als an den Rändern. Daher ist es mit den Implantaten der vorliegenden Erfindung möglich, die Geschwindigkeit der Schädelknochenregeneration, die bisher mit biologisch abbaubaren Implantaten nach dem Stand der Technik erzielt wurde, stark zu erhöhen.
- Beispiel 2.
- Die Reparatur eines 10 × 10 mm großen Defekts im Schädel von ausgewachsenen Kaninchen aus Neuseeland wurde unter Anwendung der nachstehenden zwei Methoden durchgeführt, um die Geschwindigkeit der Knochenregeneration für diese Verfahren miteinander zu vergleichen.
- A.
Durch Extrudieren von Poly(orthoester) (ein steifes Copolymer aus
Diketenacetal und steifen und flexiblen Diolen im Molverhältnis 60:40,
Herstellung wie beschrieben in Heller, J., Poly(ortho esters), Advances
in Polymer Science 107 (1993) 41-92, wobei die gesamte Offenbarung
dieser Publikation hier in Bezug genommen wird) wurde eine steife
Platte in einer Dicke von 0,5 mm hergestellt und zu Stücken einer
Größe von 15 × 15 mm
zerschnitten. Die Ecken der Platte wurden abgerundet, und die Stücke wurden
durch Erwärmen
zu der gewünschten
konvexen Form gebogen. Wie in
1 dargestellt ist, wurde die Platte unter Verwendung von Poly(orthoester)-Ministiftschrauben an dem den Defekt umgebenden Knochen befestigt; die weichen Gewebe wurden über der Platte geschlossen. - B. Die innere Oberfläche der oben zu Methode A beschriebenen Platte wurde mit einem Lösungsmittel befeuchtet, das die Oberfläche der Platte klebrig machte, worauf gebrochene Poly(orthoester)-Fasern aufgestreut wurden, so dass sie daran hafteten und die Oberfläche uneben und porös machten.
- Jede Versuchsreihe (A und B) umfasste 12 Tiere, von denen 4 nach 3 Wochen, 4 nach 24 Wochen und 4 nach 48 Wochen getötet wurden. Am Versuchsende wurden in keiner der Versuchsreihen entzündliche Reaktionen im weichen Gewebe festgestellt. In Versuchsreihe A wurde nach 3 Wochen kein neues Knochengewebe festgestellt, jedoch waren in Versuchsreihe B 10 % der Fläche des Defekts mit neuem Knochen bedeckt. Nach 24 Wochen war in Versuchsreihe A eine Bedeckung von 90 % und in Versuchsreihe B eine vollständige Bedeckung erzielt, während nach 48 Wochen die Unterseite des Implantats in beiden Versuchsreihen vollständig in Knochen umgewandelt war und sich auch auf der oberen Oberfläche etwas Knochen gebildet hatte. Wie in
1 dargestellt ist, war die Platte unter Verwendung von Ministiftschrauben aus Poly(orthoester) (gleiches Copolymer wie oben zu Methode A beschrieben) an dem den Defekt umgebenden Knochen befestigt.
Claims (10)
- Kranialimplantat (
1 ), das zumindest teilweise bioabsorbierbar ist und aufweist: – eine steife Plattenlage (2 ) aus bioabsorbierbarem Material mit einer Oberseite und einer Unterseite und – eine Faserlage (3 ) aus bioabsorbierbarem Material, die in zumindest teilweisem Kontakt mit der Unterseite der steifen Plattenlage (2 ) steht, – wobei das Material der Faserlage (3 ) schneller bioabsorbierbar ist als das Material der steifen Plattenlage (2 ), dadurch gekennzeichnet, dass die Faserlage (3 ) Poren mit einem Durchmesser von 50 bis 400 μm enthält. - Kranialimplantat nach Anspruch 1, dadurch gekennzeichnet, dass die steife Plattenlage (
2 ) ein bioabsorbierbares Homopolymer, Copolymer, Polymerblend oder Polymer-Kompositmaterial enthält. - Kranialimplantat nach Anspruch 1 oder 2, dadurch gekennzeichnet, dass die Faserlage (
3 ) ein bioabsorbierbares Homopolymer, Copolymer, Polymerblend oder Polymer-Kompositmaterial enthält. - Kranialimplantat nach einem oder mehreren der Ansprüche 1 bis 3, dadurch gekennzeichnet, dass die steife Plattenlage (
2 ) und die Faserlage (3 ) aus dem gleichen Material bestehen. - Kranialimplantat nach einem oder mehreren der Ansprüche 1 bis 4, dadurch gekennzeichnet, dass das Implantat (
1 ) ferner biokeramisches Glas enthält. - Kranialimplantat nach einem oder mehreren der Ansprüche 1 bis 5, dadurch gekennzeichnet, dass das Implantat (
1 ) ferner ein bioaktives Mittel enthält. - Kranialimplantat nach einem der oder mehreren der Ansprüche 1 bis 6, dadurch gekennzeichnet, dass die steife Plattenlage (
2 ) und die Faserlage (3 ) lose aneinander befestigt sind. - Kranialimplantat nach einem oder mehreren der Ansprüche 1 bis 7, dadurch gekennzeichnet, dass die steife Plattenlage (
2 ) und die Faserlage (3 ) durch Verschweißen miteinander verbunden sind. - Kranialimplantat nach einem oder mehreren der Ansprüche 1 bis 7, dadurch gekennzeichnet, dass die steife Plattenlage (
2 ) und die Faserlage (3 ) durch Verkleben miteinander verbunden sind. - Kranialimplantat nach einem oder mehreren der Ansprüche 1 bis 7, dadurch gekennzeichnet, dass die steife Plattenlage (
2 ) und die Faserlage (3 ) durch Heißpressen miteinander verbunden sind.
