Description

The invention relates to a device according to the preamble of claim 1.

With surgical fracture care in the maxillofacial area, as well as in the hand and foot area, the tendency can be clearly established that smaller and smaller implants are preferred. The reason for this is the generally growing understanding of the biomechanical foundations of osteosynthesis. In the field of maxillofacial fracture treatment, thanks to the miniaturization of implants, more consideration can be given to the cosmetic result of an osteosynthesis.

In the field of hand surgery, movement restrictions in the finger area can be avoided. Osteosynthesis implants in the finger area come under the tendons. With an implant with a large cross-section, the tendon can no longer be stretched to the maximum.

The dimensions of the so-called mini implants (screws and plates) are in the range of 0.8 mm - 2.0 mm. This miniaturization has caused problems in the area of packaging, storage and handling during the surgical procedure. The handling in the operating room has become special in the maxillofacial area, proven to be difficult. Depending on the degree of difficulty of the fracture or correction, up to 40 bone screws are inserted. These screws have to be taken individually by the surgical nurse from a so-called screw rake, checked for their length, placed on a screwdriver and handed over to the surgeon. The latter, in turn, has to insert it through the osteosynthesis plate into the predrilled screw hole. During the screw transition and when trying to screw in the screw, it often falls from the screwdriver, into the wound or into the surgical cover. Trying to find a lost screw, with its dimensions (diameter 0.8 mm x 4.0 mm), is too time-consuming and would increase the operating time. Since the loss of screws due to handling in the operating room, packaging and sterilization is quite common, the hospital incurs unnecessary costs.

Another problem with handling mini screws is described below. After the surgeon has selected the appropriate osteosynthesis plate for the respective fracture, he positions the plate over the fracture gap. Then he drills the core hole for the screw (diameter 0.5 - 1.5 mm) through one of the plate holes. After the drilling process, he takes over the necessary screw from the surgical nurse and turns it through the plate into the bone. The problem is often finding the core hole in the bone, since the surface of the bone is covered with blood or soft tissue and the plate can slide on the smooth surface of the bone.

Hier will die Erfindung Abhilfe schaffen. The invention seeks to remedy this. The invention has for its object to provide a device for the continuous presentation and application of osteosynthetic fixation elements, in particular bone screws. Another object is to provide packaging for such osteosynthetic fixation elements, which is used simultaneously for storage in the operating room and as a dispenser for the surgeon.

The invention achieves the stated object with a device which has the features of claim 1 and a rotary drum for this device which has the features of claim 9.

The advantages achieved by the invention are essentially to be seen in the fact that, thanks to the device according to the invention, an efficient tool is created for the surgeon, with which the stockpiling for osteosynthetic fixation elements, in particular for bone screws, the intraoperative administration and application of such fixation elements is extremely facilitated. In addition to the presentation of the fixation elements, the device according to the invention also serves as a drilling jig and guide for the screwdriver with which the fixation elements, in particular bone screws, are screwed in.

The device shown in FIGS. 1-5 is similar in structure to a drum turret. The drum device 3 consists of a drum housing 6, 7 having a rotating hub 5 for receiving a rotary drum 10 which can be rotated about the axis 2 of the rotary hub 5, with a plurality of through bores 4 which are arranged peripherally and run parallel to the axis 2 and serve as storage chambers for bone screws 1.
The drum housing 6,7 in turn consists of two plates 6 and 7 connected by the rotating hub 5, the drum housing 6,7 or the plates 6 and 7 not being able to be rotated on the rotating hub 5.
The upper plate 6, which is firmly connected to a handle 9, is provided with an access opening 14 for introducing an instrument 21 and 22 shown in FIGS. 6 and 7, the center of the access opening 14 being at the same distance from the axis 2 as the centers of FIG peripherally arranged through holes 4 running parallel to axis 2 and serving as storage chambers for bone screws 1.
The lower plate 7, which is firmly connected to the rotary hub 5, is provided with an outlet opening 13 for the bone screws 1, the center of the outlet opening 13 being at the same distance from the axis 2 as the centers of the bores 4.

The lower plate 7 of the drum housing 6.7 also has a protective sleeve 11 through which the core hole and the setting of the bone screws 1 can be carried out. The distance between the axis 12 of the tissue protection sleeve 11 and the axis 2 corresponds to the distance between the screw and drill holes 4, 18 in the rotary drum 10 and its center axis 2. The upper plate 6 has, in addition to the handle 9, a drill sleeve 15 which is in contact with the holes 4 and 18 and the axis 12 of the tissue protection sleeve 11 coincide. In addition, the upper plate has a sprung Bolt or ball that can engage in the indexings 17 of the rotary drum 10. As a result, the bores 4 and 18 of the rotary drum 10 can be easily aligned with the tissue protection sleeve 11 and the drill sleeve 15.

To assemble the device according to the invention, the rotary hub 5, which is firmly connected to the lower plate 7, is passed through the hollow hub of the rotary drum 10 and an opening (not shown in the drawing) in the upper plate 6 and with the nut 8 in a certain manner, the rotatability of the between the two Plates 6,7 rotatably mounted rotary drum 10 ensuring distance fixed.

The actual packaging of the bone screws 1, or screw-like fixation elements, is thus the rotary drum 10 itself, which has circularly arranged bores 4 and 18. There are two different drilling diameters that alternate with each other. The larger bores 4 serve for receiving the bone screws 1, the smaller bores 18 for guiding the drill 21, for the core hole drilling in the bone, and the screwdriver 22 for fastening the bone screw 1 in the bone. A circular indexing 17 on the drum top 19 allows a precisely defined positioning of the desired bore 4 or 18 within the drum housing 6,7.

The rotary drum 10 is easily exchangeable and also serves as packaging for the bone screws 1. Depending on requirements, the rotary drum can be in a sterile packaging offered, or it can only be sterilized in the hospital. Instead of replacing the rotary drum 10 after its exhaustion with a new full rotary drum 10, it can also be refilled with bone screws 1 via an additional filling hole 20 provided in the upper plate 6. The center of the filling hole 20 must be at the same distance from the axis 2 as the centers of the holes 4 due to the design.

The intraoperative handling of the device according to the invention is described below:
The rotary drum 10 filled with bone screws 1 is pushed into the drum housing 6, 7 without having to handle the individual bone screws 1. The surgeon now uses the device according to the invention initially as a drill sleeve in order to drill the core hole for the bone screws 1 to be placed in the bone. The drill 21 is guided exactly through the drill sleeve 15 of the upper plate 6, through one of the smaller bores 18 of the rotary drum 10 and through the tissue protection sleeve 11. The length of the individual guide elements 15, 11 are matched to the length of the bore. This allows the drilling depth in the bone to be adjusted to the screw length. The tip of the tissue protection sleeve 11 is adapted to the screw holes in the bone plate, as a result of which the bone plate is pressed onto the bone and can therefore no longer slide off. The surgeon now drills the core hole in the bone. After removing the drill 21, the surgeon rotates the rotary drum 10 by an index position 17 by means of its peripheral edge. As a result, one of the ends up in the rotary drum 10 located bone screws 10, in the working channel formed by the elements 13 and 11. Depending on the nature of the bore 4 in which the bone screw 1 is located, the bone screw 1 falls through the tissue protection sleeve 11 onto the bone surface, or it has to be pushed with a screwdriver 22 through the tissue protection sleeve 11 onto the bone surface. The diameter of the screwdriver 22 is adapted to the head diameter of the bone screw 1, so that it is guided over its entire length and necessarily engages in the drive of the bone screw 1 (cross recess, Allen key, etc.). Since the device according to the invention does not have to be removed during the entire working process, the bone screw 1 can be screwed into the bone without looking for the pilot hole.
After the bone screw 1 has been screwed in completely, the screwdriver 22 is removed and the rotary drum 10 is rotated further by an index position 17. As a result, there is again a smaller hole 18 in the rotary drum 10 in the working channel 13, 11, as a result of which the next core hole can be drilled. The entire process is repeated until all screw holes in the plate are covered with bone screws 1.
