L'invention concerne un dispositif d'ancrage d'une instrumentation rachidienne sur une vertèbre comprenant au moins un support (55, 55a, 55b) portant des moyens (5a, 5b, 6a, 6b, 7a, 7b) de couplage de l'instrumentation à ce support et des moyens (35, 61, 65, 71) d'ancrage du support (55, 55a, 55b) par rapport à la vertèbre, caractérisé en ce que le support (55, 55a, 55b) comporte une face convexe (56) épousant et venant en appui au contact d'une portion au moins de la surface concave de l'arc postérieur de la vertèbre d'un côté au moins de l'apophyse épineuse (58), et en ce que les moyens d'ancrage (35, 61, 65, 71) associent rigidement au moins deux portions distinctes du support (55, 55a, 55b) à au moins deux parties distinctes correspondantes de la vertèbre.
Description
DISPOSITIF D'ANCRAGE D'UNE INSTRUMENTATION RACHIDIENNE SUR UNE VERTEBRE.
L'invention concerne un dispositif d'ancrage d'une instrumentation rachidienne sur une vertèbre.
Les dispositifs implantés d'ostéosynthèse rachidienne connus sont fixés par rapport aux différente? vertèbres par des éléments d'ancrage. On connaît déjà des éléments d'ancrage qui sont constitués ou qui comportent des vis intrapédiculaires. Ces vis intrapédiculaires sont largement utilisées, sous diverses formes, avec différents types de dispositifs d'ostéosynthèse (Cotrel-Dubousset, Roy- Camille, ... ) .
On connaît également des éléments d'ancrage sous forme de crochets à extrémités fixes recourbées prenant appui sur les apophyses articulaires, ou sur la lame ou sur une apophyse transverse de la vertèbre. Le crochet est maintenu contre l'os et tenu par l'instrumentation rachidienne.
On connaît également des dispositifs d'ancrage sous forme de fils sous-laminaires (dispositifs d'ostéosynthèse de Luque) . Tous ces éléments d'ancrage connus présentent des inconvénients au regard de la fiabilité et de la longévité et/ou de la résistance de l'ancrage ainsi réalisé d'autant qu'ils réalisent un ancrage unique dans chacune des gouttières paravertebrales. En effet, tous ces éléments d'ancrage sont essentiellement constitués d'un élément métallique dur et rigide prenant appui sur une seule zone localisée d'un côté de la vertèbre. Or, ces éléments d'ancrage sont destinés à transmettre des contraintes extrêmement importantes engendrées par l'instrumentation sur les différentes vertèbres. De ce fait, un des risques les plus importants limitant la longévité et la fiabilité des instrumentations rachidiennes provient du fait que les éléments d'ancrage peuvent se
desceller ou rompre l'os en cas de contraintes mécaniques trop importantes. En particulier, les crochets et les vis induisent une forte discontinuité entre l'instrumentation et la vertèbre du point de vue des caractéristiques mécaniques. Ainsi, la zone localisée de surface en contact entre le dispositif d'ancrage et la portion osseuse de vertèbre est soumise à de fortes sollicitations susceptibles d'engendrer une ostéolyse secondaire.
Par ailleurs, les fils sous-laminaires présentent l'inconvénient majeur d'engendrer des risques neurologiques importants, et ne sont pas utilisables avec toutes les catégories d'instrumentation rachidienne.
Le brevet américain US-A-4 836 196 décrit un dispositif espaceur composé d'un matériau plastique permettant de diminuer les contraintes transmises entre une vis engagée dans la partie osseuse de la vertèbre et l'instrumentation. Un tel dispositif n'est néanmoins pas utilisable avec la majorité des instrumentations rachidiennes connues, e induit une souplesse dans le couplage entre l'instrumentation et le dispositif d'ancrage gui peut se révéler néfaste.
La demande de brevet français FR-A-2 689 750 décrit un dispositif dOstéosynthèse rachidienne _ comprenant des crochets et/ou des vis intrapédiculaires et des plaques de' couplage de l'instrumentation qui sont ancrés dans les vertèbres grâce à une vis intrapédiculaire. Là encore, l'ancrage réalisé par une seule vis intrapédiculaire s'avère insuffisant.
L'invention vise donc à pallier les inconvénients des dispositifs d'ancrage connus.
L'invention vise donc de façon générale à proposer un dispositif d'ancrage pour une instrumentation rachidienne implantée telle qu'un dispositif d'ostéosynthèse rachidienne ou une orthèse vertébrale implantée dynamique, offrant une fiabilité, une longévité et une résistance supérieures à celles des éléments d'ancrage connus. L'invention vise aussi à proposer un dispositif d'ancrage vertébral qui respecte les appareils
articulaire et ligamentaire postérieurs vertébraux, condition indispensable à l'insertion d'un matériel destiné à corriger et à conserver les mouvements physiologiques de la colonne vertébrale. L'invention vise également à proposer un tel dispositif d'ancrage qui peut être utilisé et adapté à la plupart des instrumentations rachidiennes.
1 Pour ce faire, l'invention concerne un dispositif d'ancrage d'une instrumentation rachidienne sur une vertèbre, comprenant au moins un support portant des moyens de couplage de l'instrumentation à ce support et des moyens d'ancrage du support par rapport à la vertèbre, caractérisé en ce que le support comporte une face convexe épousant et venant en appui au contact d'une portion au moins de la surface concave de l'arc postérieur de la vertèbre d'un côté au moins de l'apophyse épineuse, et en ce que les moyens d'ancrage associent rigidement au moins deux portions distinctes du support à au moins deux parties distinctes correspondantes de la vertèbre. Selon l'invention, ce dispositif comporte au moins un support dont la face convexe s'étend en regard de l'apophyse épineuse et/ou d'au moins une apophyse transverse et/ou d'au moins un pédicule de la vertèbre, et des moyens d'ancrage de ce support respectivement sur l'apophyse épineuse et/ou au moins une apophyse transverse et/ou au moins un pédicule de la vertèbre.
Selon l'invention, chaque support est formé d'une plaque portant des moyens de couplage et recouvrant une portion de la surface concave de 1 ' arc postérieur de la vertèbre d'un côté au moins de l'apophyse épineuse.
Un dispositif selon l'invention peut comporter deux supports, un de chaque côté de l'apophyse épineuse de la vertèbre, ou un seul support s'étendant d'un seul côté ou des deux côtés de l'apophyse épineuse. Lorsque le dispositif de l'invention comporte deux supports, ce dispositif comporte des moyens de couplage de l'instrumentation portés par un seul ou par chacun des deux supports. Avantageusement et selon l'invention, les deux
supports sont associés ensemble à la même apophyse épineuse par des moyens d'ancrage communs. L'apophyse épineuse, dont la corticale est entièrement respectée, est ainsi étayée et réalise donc l'appui et l'union des deux supports bilatéraux. L'ensemble de l'arc vertébral postérieur sert d'ancrage, y compris pour une instrumentation unilatérale.
Selon l'invention, les moyens d'ancrage de chaque support comportent au moins un crochet articulé au suppo t autour d'un axe, ce crochet portant une vis de serrage engagée dans un taraudage du crochet, cette vis prenant appui directement ou indirectement sur le support ou sur un autre crochet pour faire pivoter le crochet autour de son axe dans le sens du serrage de l'extrémité libre du crochet sur la partie correspondante de la vertèbre. Avantageusement et selon l'invention, les moyens d'ancrage comportent au moins une pince de serrage formée de deux crochets articulés au même axe ou à des axes parallèles et dont les extrémités libres en regard se serrent l'une vers l'autre. Selon l'invention, les moyens d'ancrage de chaque support comportent au moins une bride de serrage d'une extrémité du support sur une apophyse vertébrale, notamment sur une apophyse épineuse et/ou transverse.
Par ailleurs, selon l'invention, la face convexe d'un support est formée d'un métal poreux et avantageusement recouverte d'une couche d'hydroxyapatite.
L'invention concerne également un dispositif d'ancrage comportant en combinaison tout ou partie des caractéristiques mentionnées ci-dessus ou ci- après.
D'autres caractéristiques et avantages de l'invention ressortiront de la description détaillée qui suit en référence aux dessins annexés dans lesquels :

La figure 1 représente un exemple d'orthèse vertébrale implantée qui permet d'effectuer et de maintenir une correction de la position relative de cinq vertèbres dorsales pour le traitement d'une déformation de type scoliotique. Avant la pose de l'orthèse, les vertèbres
présentent une courbure à convexité orientée vers la droite (figure 10). L'orthèse comprend un dispositif d'ancrage 1 fixé sur la vertèbre supérieure extrême, un dispositif d'ancrage 2 fixé sur la vertèbre extrême inférieure, et un dispositif d'ancrage 3 fixé sur la vertèbre médiane située au centre de la courbure originelle de la scoliose. Ainsi, l'orthèse instrumente cinq vertèbres successives et est ancrée sur trois vertèbres.
L'orthèse comporte en outre des tiges 4a, 4b s'étendant latéralement de chaque côté des apophyses épineuses, à savoir une tige gauche 4a placée dans la concavité de la déformation à corriger, et une tige droite 4b placée du côté de la convexité de la déformation à corriger. Chaque tige 4a, 4b est une tige courbe, souple et élastique en flexion réalisée en matériau biocompatible tel qu'un alliage métallique (acier inoxydable ou titane) et/ou en matériau composite. Chaque tige 4a, 4b est associée aux éléments d'ancrage 1, 2, 3 des vertèbres par des moyens 5a, 5b, 6a, 6b, 7a, 7b de couplage. Les moyens 5a de couplage de la tige gauche
4a au dispositif d'ancrage 1 supérieur de la vertèbre supérieure instrumentée comportent une sphère 8 montée librement rotative et enfermée dans un logement sphérique 9 d'un cyliηdre 10 solidaire du dispositif d'ancrage 1 de façon à former une rotule de liaison. La sphère 8 est percée d'un alésage cylindrique 11 traversé par la tige 4a, et dans lequel cette tige 4a peut coulisser en translation axiale longitudinale selon la direction verticale. La largeur de l'alésage 11 correspond à la largeur de la tige 4a et la sphère 8 est engagée dans le logement 9 sans possibilité de mouvements relatifs en translation horizontale, et notamment selon les directions latérale et antéropostérieure de la vertèbre. De la sorte, les moyens 5a de couplage ainsi réalisés, interdisent, après la pose de la tige 4a, tout mouvement relatif en translation horizontale de la tige 4a par rapport aux éléments d'ancrage 1 et à la vertèbre correspondante. Par contre, ces moyens 5a de couplage autorisent, après la pose de la
tige, un mouvement relatif selon quatre autres degrés de liberté : un coulissement en translation longitudinale relative selon un axe vertical de la tige 4a et trois degrés de liberté de rotation de la sphère 8 par rapport au cylindre 10, à savoir une rotation relative autour d'un axe perpendiculaire à un plan frontal, une rotation relative autour d'un axe perpendiculaire à un plan sagittal, et une rotation relative autour d'un axe vertical. ,De préférence, toutes les rotations sont effectuées par le glissement en rotation de la sphère 8 dans le logement sphéri-que 9. Par contre, la translation selon l'axe vertical intervient au contact de la tige 4a avec l'alésage 11 de la sphère 8. L'alésage 11 est cylindrique de révolution. Et comme représenté à la figure 4, la tige 4a est pourvue de nervures 12 régulièrement réparties autour de son axe et s'étendant le long de la tige 4a en saillie par rapport à sa face extérieure pour venir au contact le long de la face intérieure de l'alésage 11. Dans l'exemple représenté, on a prévu trois nervures de section droite transversale triangulaire. De la sorte les frottements en contact sont diminués. Les matériaux constitutifs de la sphère 8 et du cylindre 10 sont choisis pour permettre les rotations de cette sphère 8 dans le logement 9 comme indiqué ci-dessus. Par exemple, la sphère 8 est en alliage métallique de grande qualité de surface ou en céramique et le cylindre 10 est constitué d'un bloc de matière synthétique telle que du polyéthylène ou autre. La sphère 8 et/ou le cylindre 10 peuvent être constitués d'une matière autolubrifiante ou comporter un revêtement en cette matière. Le cylindre 10 comporte des ouvertures supérieure 14 et inférieure 15 permettant le passage de la tige 4a et dont les dimensions en largeur sont plus importantes que celles de la tige 4a pour autoriser les mouvements d'inclinaison de la tige 4a par rapport à l'axe du cylindre sous l'effet des mouvements de rotation sus¬ mentionnés. De préférence, les dimensions des ouvertures 14 et 15 sont telles qu'elles autorisent une amplitude d'inclinaison d'au moins 45° de la tige 4a par rapport à
l'axe vertical du cylindre 10.
La sphère 8 est engagée dans le logement sphérique 9 lors de la pose de la tige 4a. Pour ce faire, une couronne 16 filetée peut être montée à une extrémité supérieure (ou, en variante non représentée, inférieure) du cylindre 10 qui comporte un logement de réception de cette couronne 16 muni d'un taraudage correspondant (figures 3 et 4). La couronne 16 comporte un perçage axial qui détermine l'ouverture supérieure 14 du cylindre 10 débouchant dans le logement sphérique 9.. Les dimensions du logement de réception de la couronne 16 sont définies pour permettre 1' insertion de la sphère 8 par le haut ou par le bas dans le logement sphérique 9. La face inférieure de la couronne
16 a une forme concave en portion de sphère de sorte qu'elle vient en prolongement de la face interne sphérique du logement 9 en enfermant la sphère 8 dans ce logement 9
(figure 3). En variante (figure 1), le cylindre est formé d'un bloc de matière synthétique et l'ouverture 14 supérieure (ou inférieure) a un diamètre légèrement inférieur à celui de la sphère 8 qui peut être engagée en force dans le logement 9 à travers cette ouverture 14 qui retient ensuite la sphère 8 dans le logement 9.
Avant d'introduire la sphère 8 dans le logement 9, la tige 4a est introduite à l'intérieur du S* cylindre 10. Pour ce faire, ce cylindre 10 est pourvu sur toute sa hauteur d'une fente 17 communiquant avec le logement 9 et dont la largeur correspond au plus grand diamètre envisagé pour la tige 4a. Cette fente 17 peut être placée sur le côté intérieur du cylindre 10 en regard des apophyses épineuses, comme représenté sur la figure 4, ou du côté opposé, ou même du côté postérieur. De préférence, cette fente 17 est néanmoins ménagée dans une portion du cylindre 10 qui subit le moins de sollicitations dans la direction radiale horizontale. Pour le montage de l'ensemble, on engage la sphère 8 autour de la tige 4a en introduisant cette tige 4a à travers l'alésage 11, on introduit la tige 4a dans le logement 9 à travers la fente
17 en maintenant la sphère 8 au-dessus ou au-dessous du
cylindre 10, puis on engage la sphère 8 dans le logement 9. Le cas échéant, on visse ensuite la couronne 16 dans le logement correspondant du cylindre 10. Cette couronne 16 aura été préalablement engagée autour de la tige 4a au-dessus (ou au-dessous) de la sphère 8. Le diamètre des tiges 4a, 4b adaptables aux mêmes dispositifs d'ancrage 1,
2, 3 peut ainsi varier, l'adaptation étant réalisée en - utilisant des sphères 8 dont l'alésage 11 correspond au diamètre des tiges 4a, 4b. Les moyens 6a de couplage de la tige 4a par rapport au dispositif d'ancrage inférieur 2 de la vertèbre extrême inférieure sont identiques aux moyens 5a de couplage de cette tige 4a au dispositif d'ancrage supérieur 1 de la vertèbre extrême supérieure préalablement décrits. Par contre, les moyens 7a de couplage de la tige 4a au dispositif d'ancrage médian 3 de la vertèbre médiane sont des moyens d'association rigides interdisant tout mouvement relatif de la tige 4a par rapport au dispositif d'ancrage

Pour ce faire, ces moyens 7a de couplage sont constitués d'un cylindre 18 monté solidaire du dispositif d'ancrage 3 et pourvu d'un alésage cylindrique 19 sur toute sa hauteur traversé par la tige 4a. Cet alésage 19 est similaire à l'alésage 11 des moyens 5a de couplage préalablement décrits, ιet a donc des formes et des dimensions correspondant à celles de la tige 4a. De plus, le cylindre 18 porte une ou plusieurs vis 20 de blocage transversal de la tige 4a. La vis 20 est engagée dans un taraudage correspondant du cylindre 18 qui débouche dans l'alésage 19. En serrant la vis 20, celle-ci appuie sur la tige 4a et la bloque en translation par rapport à l'alésage 19. Pour favoriser ce blocage, la tige 4a peut être pourvue d'une ou plusieurs rainures périphériques horizontales. Egalement le cylindre 18 est pourvu d'une fente sur toute sa hauteur pour le montage de la tige 4a. La tige 4b placée à droite des apophyses épineuses (figure 1) est associée aux dispositifs d'ancrage 1, 2, 3 par des moyens de couplage 5b, 6b, 7b identiques aux moyens de couplage 5a, 6a, 7a préalablement décrits
pour la tige de gauche 4a.
L'orthèse représentée à la figure 1 comporte en outre des ressorts 21a, 21b, 22a, 22b, 23a, 23b, 24a, 24b agissant sur les dispositifs d'ancrage d'au moins une vertèbre. Dans le mode de réalisation de la figure 1, l'orthèse comporte pour chaque tige 4a, 4b, un ressort à spires 21a, 21b interposé entre les dispositifs d'ancrage 1, 3 de la vertèbre supérieure et de la vertèbre médiane, un ressort à spires 22a, 22b interposé entre les dispositifs d'ancrage 2, 3 de la vertèbre inférieure et de la vertèbre médiane, un ressort à spires 23a, 23b interposé entre le dispositif d'ancrage 1 de la vertèbre supérieure et une extrémité libre 25a, 25b supérieure de la tige 4a, 4b et un ressort à spires 24a, 24b interposé entre le dispositif d'ancrage 2 de la vertèbre inférieure et l'extrémité libre inférieure 26a, 26b de la tige 4a, 4b. Chaque ressort à spires 21a, 21b, 22a, 22b est interposé entre les dispositifs d'ancrage 1, 3 et 3, 2 de deux vertèbres distinctes et a une extrémité associée au dispositif d'ancrage 1 ou 3 d'une vertèbre et l'autre extrémité associée au dispositif d'ancrage 3 ou 2 d'une autre vertèbre. Les ressorts 21a, 22a, 23a, 24a entourent la tige 4a placée du côté concave de la malformation à corriger et sont des ressorts de compression. Les ressorts 21b, 22b, 23b, 24b entourent la tige 4b placée du côté convexe de la déformation à corriger et sont des ressorts de traction. Chaque ressort peut également être utilisé en torsion et associé à des moyens de fixation en torsion de ses extrémités. La figure 2 représente une vue de profil d'une variante de réalisation de l'orthèse précédemment décrite. Comme on le voit, le cintrage de la tige 4a permet de rétablir et de maintenir la cyphose physiologique.
Les figures 3 à 9 représentent divers modes de réalisation des dispositifs d'ancrage 1, 2, 3- Selon l'invention, les dispositifs d'ancrage 1, 2, 3 de chaque vertèbre comportent au moins une plaque 55 ayant une face convexe 56 antérieure venant en appui au contact de l'arc
postérieur vertébral et notamment au moins contre la lame vertébrale 57 et/ou d'un côté au moins de l'apophyse épineuse 58. La forme de cette face convexe 56 correspond donc à celle de la face postérieure de la lame vertébrale 57. La face convexe 56 peut être constituée de métal poreux de type "pointe de diamant" et est avantageusement recouverte d'hydroxyapatite afin de permettre une meilleure cohésion avec l'os. Les moyens de couplage sont portés par une plaque 55 des dispositifs d'ancrage. Plus précisément, les cylindres 10, 18, solidaires des dispositifs d'ancrage sont supportés par une plaque 55 en regard de l'extrémité transverse de la lame 57, c'est-à-dire au voisinage de l'apophyse transverse 59.
Selon l'invention, les dispositifs d'ancrage 1, 2, 3 comportent des moyens d'ancrage en au moins deux portions distinctes de chaque vertèbre. Ainsi, chaque plaque 55 des dispositifs d'ancrage s'étend en regard du pédicule 60 de la vertèbre, et peut être fixée à la vertèbre sur au moins un pédicule 60 par une vis intrapédiculaire 61 à tête fraisée engagée dans un perçage 62 correspondant de la plaque 55.
Chaque plaque 55 des dispositifs d'ancrage s'étend aussi avantageusement en regard des faces latérales de l'apophyse épineuse 58, de chaque côté, et comporte également des moyens d'ancrage sur l'apophyse épineuse 58. Pour ce faire, l'extrémité 63 de la plaque 55 venant en regard de l'apophyse épineuse 58 comporte un renflement 64 en saillie dans le plan frontal et dans la direction horizontale de façon à former un épaulement de retenue d'un cerclage (qui peut être constitué par un collier autobloquant métallique ou en matériau synthétique) ou d'une bride 65 entourant l'apophyse épineuse 58 et serrant l'extrémité 63 de la plaque 55. La bride 65 peut être constituée d'un cadre 66 entourant l'apophyse épineuse et l'extrémité 63 de la plaque 55, d'une vis 67 à axe vertical engagée à travers un taraudage vertical du cadre 66, et dont l'extrémité libre est munie d'un patin 68 prenant appui sur la face supérieure de l'apophyse épineuse 58.
Ainsi, le cadre 66 est bloqué par rapport à l'apophyse épineuse. La bride 65 comporte également une vis 69 à axe horizontal frontal engagée dans un taraudage horizontal du cadre 66, et dont l'extrémité libre est munie d'un patin 70 qui prend appui sur l'extrémité 63 de la plaque 55 pour la serrer latéralement contre l'apophyse épineuse 58. Après serrage de la vis 69, l'épaule ent de retenue formé par le renflement 64 bloque la bride 65 en translation horizontale par rapport à l'extrémité libre 63 de la plaque 55, le patin 70 venant contre ce renflement 64. La bride 65 associe ensemble les deux plaques 55a, 55b et constitue ainsi des moyens d'ancrage communs de ces deux plaques 55a, 55b à l'apophyse épineuse 58 de la vertèbre.
Egalement, chaque plaque 55 des dispositifs d'ancrage s'étend avantageusement en regard d'au moins une apophyse transverse 59 de la vertèbre, et comporte des moyens d'ancrage sur une apophyse transverse 59. Ces moyens d'ancrage peuvent être formés par au moins un crochet 71 (plaque 55a de gauche sur la figure 3) articulé à l'extrémité transverse 72 de la plaque 55a autour d'un axe horizontal 74 et prenant appui sur la face supérieure et/ou sur la face inférieure de l'apophyse transverse 59. L'extrémité transverse 72 de la plaque 55a présente donc, pour chaque crochet 71, une chape 73 dans laquelle le crochet 71 est articulé autour de son axe 74. Chaque crochet 71 est muni à son extrémité opposée à l'apophyse transverse, d'une vis verticale 75 engagée dans un taraudage du crochet 71 et prenant appui sur une surface horizontale du fond de la chape 73 de la plaque 55a pour serrer ce crochet 71 contre la surface correspondante de l'apophyse transverse 59.
Sur la figure 3, les moyens d'ancrage de la plaque 55b de droite comportent une pince 35 de serrage formée de deux crochets 36, 37 articulés par rapport à la plaque 55b autour de deux axes 48, 49 parallèles horizontaux. En variante non représentée, l'ancrage sur l'apophyse transverse peut être réalisé par un collier autobloquant de serrage (similaire à celui utilisé sur
l'apophyse épineuse) métallique ou de matériau synthétique. Chaque crochet 36, 37 est similaire au crochet 71 sus- décrit et est monté dans une chape 50, 51 de la plaque 5bb avec une vis verticale 53, 54 de serrage prenant appui sur une surface horizontale du fond de la chape 50, 51 pour serrer ce crochet 46, 47 contre la surface correspondante respectivement supérieure et inférieure de l'apophyse transverse 59. Les extrémités libres 46, 47 respectives des deux crochets 36, 37 s'étendent en regard l'une de l'autre en opposition et se serrent l'une vers l'autre sous l'effet des vis 53, 54 pour emprisonner l'apophyse transverse 59. A la place des deux vis 53, 54, on peut prévoir un tendeur à deux pas de vis inversés reliant les deux crochets l'un à l'autre et qui est accessible par le haut ou par le bas pour serrer ou desserrer les deux crochets 36, 37. Une telle pince peut être prévue aussi pour la plaque 55a de gauche.
Comme on le voit sur les figures 3 et 4, les dispositifs d'ancrage peuvent comporter deux plaques 55a, 55b, une de chaque côté de l'apophyse épineuse (comme représenté sur les figures 1 ou 3), et ce que l'orthèse comporte deux tiges (figure 1) ou une seule tige (figure 3) d'un seul côté de l'apophyse épineuse. En variante, une seule plaque 55 peut être prévue. La figure 5 représente une variante de réalisation dans laquelle la plaque 55 s'étend dans les deux gouttières paravertebrales, la vertèbre ne comportant pas d'apophyse épineuse. La plaque 55 est ancrée sur les deux apophyses transverses 59 et par les vis intrapédiculaires 61. La plaque 55 forme un pont recouvrant l'arc vertébral postérieur et protégeant le canal médullaire.
Les figures 6 et 7 représentent une autre variante dans laquelle les plaques 55 sont ancrées uniquement par les vis intrapédiculaires 61 et par la bride 65 sur l'apophyse épineuse 58.
La figure 8 représente une autre forme de réalisation plus spécifiquement destinée à la vertèbre
dorsale D12 qui, en général, ne comporte pas d'apophyse transverse. Chaque plaque 55 épouse néanmoins la forme du moignon de l'apophyse transverse sur lequel elle est fixée par une vis oblique 76 orientée en direction du pédicule. Les plaques 55 sont aussi fixées par des vis intrapédiculaires 61 et une bride 65 de serrage sur l'apophyse épineuse 58.
1 Dans le cas de la vertèbre sacrée SI (figure 9) qui ne présente pas d'apophyse transverse mais un aileron sacré, on utilise une plaque 55 d'un seul tenant s'étendant des deux côtés de l'apophyse épineuse en formant un pont au-dessus de cette apophyse épineuse. La plaque 55 peut être ancrée par deux crochets inférieurs 77 similaires aux crochets 71 des apophyses transverses préalablement décrits, et qui s'appuient sur les bords supérieurs des deux premiers trous sacrés- Chaque crochet 77 est serré par une vis de serrage 79 engagée dans un taraudage du crochet et prenant appui sur une face verticale de la plaque 55. La plaque 55 est fixée sur la vertèbre SI par deux vis intra- osseuses 61 passant par un orifice 62 prévu dans la plaque et dirigées dans le corps vertébral de SI vers son plateau vertébral supérieur. Deux autres vis (non représentées) peuvent stabiliser la plaque 55 en la fixant latéralement à l'aileron skcré. Comme on le voit sur les figures 10 et 11, les dispositifs d'ancrage 1, 2, 3 selon l'invention permettent non seulement de réaliser l'ancrage de l'orthèse sur les vertèbres, mais également d'effectuer la réduction de la déformation scoliotique correspondante, et immédiatement après, la pose de l'orthèse.
Pour ce faire, chaque dispositif d'ancrage 1, 2, 3 est pourvu, de chaque côté de l'apophyse épineuse 58, d'un perçage 78 ménagé à travers une extension horizontale de la plaque 55 qui porte les moyens de couplage des tiges 4a, 4b. Chaque perçage 78 est ménagé de préférence au voisinage de l'apophyse transverse 59 et du côté antérieur et latéral d'un cylindre 10, 18 de couplage. Les perçages 78 à axe vertical permettent l'introduction de
tétons d'extrémité des branches 80 de pinces 81a, 81b ancillaires. Une pince 81a est placée du côté gauche, c'est-à-dire de la concavité de la déformation à réduire. Une pince 81b est placée au contraire du côté droit, c'est- à-dire du côté de la convexité de la déformation à réduire. Les pinces prennent appui sur les dispositifs d'ancrage 1, 2, 3. En manoeuvrant les poignées 82 des pinces ancillaires 81a, 81b, on obtient la réduction de la déformation jusqu'à la position représentée à la figure 11. Ensuite, on peut associer les tiges 4a, 4b et les ressorts, c'est-à-dire l'instrumentation constituée de l'orthèse, aux différents dispositifs d'ancrage 1, 2, 3 alors que les pinces 81a, 81b maintiennent la colonne vertébrale dans sa position corrigée. Une fois l'instrumentation associée aux dispositifs d'ancrage 1, 2, 3, on peut désolidariser les pinces 81a, 81b des dispositifs d'ancrage 1, 2 , 3.
Bien que l'invention a été décrite en référence à une orthèse vertébrale implantée dynamique de réduction d'une scoliose, les dispositifs d'ancrage selon l'invention peuvent être utilisés pour toute autre instrumentation rachidienne dans laquelle les mêmes problèmes se posent, à savoir une orthèse lombaire ou sacro-lombaire de réduction des efforts intervertébraux, une orthèse cervicale, un dispositif d'ostéosynthèse rachidienne, par exemple du type Cotrel-Dubousset, ou autre.
Claims
REVENDICATIONS 1/ - Dispositif d'ancrage d'une instrumentation rachidienne sur une vertèbre comprenant au moins un support (55, 55a, 55b) portant des moyens (5a, 5b, 6a, 6b, 7a, 7b) de couplage de l'instrumentation à ce support et des moyens (35, 61, 65, 71, 76, 77) d'ancrage du support (55, 55a, 55b) par rapport à la vertèbre, caractérisé en ce que le support (55, 55a, 55b) comporte une face convexe (56) épousant et venant en appui au contact d'une portion au moins de la surface concave de l'arc postérieur de la vertèbre d'un côté au moins de l'apophyse épineuse (58), et en ce que les moyens d'ancrage (35, 61, 65, 71, 76, 77) associent rigidement au moins deux portions distinctes du support (55, 55a, 55b) à au moins deux parties distinctes correspondantes de la vertèbre.
2/ - Dispositif selon la revendication 1 , caractérisé en ce qu'il comporte au moins un support (55, 55a, 55b) dont la face convexe s'étend en regard de l'apophyse épineuse (58) de la vertèbre, et en ce qu'il comporte des moyens (65) d'ancrage de ce support (55, 55a, 55b) sur l'apophyse épineuse (58) de la vertèbre.
3/ - Dispositif selon l'une des revendications 1 et 2, caractérisé en ce qu'il comporte au moins un support (55, 55a, 55b) dont la face convexe s'étend en regard d'au moins une apophyse transverse (59) de la vertèbre, et en ce qu'il comporte des moyens (71) d'ancrage de ce support (55, 55a, 55b) sur au moins une apophyse transverse (59) de la vertèbre.
4/ - Dispositif selon l'une des revendications 1 à 3, caractérisé en ce qu'il comporte au moins un support (55, 55a, 55b) dont la face convexe s'étend en regard d'au moins un pédicule (60) de la vertèbre, et en ce qu'il comporte des moyens (61) d'ancrage de ce support (55, 55a, 55b) par rapport à au moins un pédicule (60) de la vertèbre.
5/ - Dispositif selon l'une des revendications 1 à 4, caractérisé en ce que chaque support (55, 55a, 55b) est formé d'une plaque (55, 55a, 55b)
portant des moyens (5a, 5b, 6a, 6b, 7a, 7b) de couplage et recouvrant une portion de la surface concave de l'arc postérieur de la vertèbre d'un côté au moins de l'apophyse épineuse (58). 6/ - Dispositif selon l'une des revendications 1 à 5, caractérisé en ce qu'il comporte deux supports (55a, 55b), un de chaque côté de l'apophyse épineuse (58).
7/ - Dispositif selon la revendication 6, caractérisé en ce qu'un seul des deux supports (55a, 55b) porte des moyens (5a, 5b, 6a, 6b, 7a, 7b) de couplage de 1' instrumentation.
8/ - Dispositif selon la revendication 6, caractérisé en ce que chacun des deux supports (55a, 55b) porte des moyens (5a, 5b, 6a, 6b, 7a, 7b) de couplage de l'instrumentation.
9/ - Dispositif selon l'une des revendications 6 à 8, caractérisé en ce que les deux supports (55a, 55b) sont associés ensemble à l'apophyse épineuse par des moyens (65) d'ancrage communs.
10/ - Dispositif selon l'une des revendications 1 à 9, caractérisé en ce que les moyens d'ancrage d'un support (55, 55a, 55b) comportent au moins un crochet î-(36, 37, 71, 77) articulé au support (55, 55a, 55b) autour d'un axe (48, 49, 74), ce crochet (36, 37, 71, 77) portant une vis (67, 75) de serrage engagée dans un taraudage du crochet (36, 37, 71, 77), cette vis (67, 75) prenant appui directement ou indirectement sur le support (55, 55a, 55b) ou sur un autre crochet (37, 36) pour faire pivoter le crochet (36, 37, 71, 77) autour de son axe (48, 49, 74) dans le sens du serrage de son extrémité libre sur la partie correspondante de la vertèbre.
11/ - Dispositif selon la revendication 10, caractérisé en ce que les moyens d'ancrage d'un support (55, 55a, 55b) comportent au moins une pince (35) de serrage formée de deux crochets (36, 37) et dont les extrémités libres (46, 47) en regard se serrent l'une vers 1 ' autre.
12/ - Dispositif selon l'une des revendications 1 à 11, caractérisé en ce que les moyens d'ancrage d'un support (55, 55a, 55b) comportent au moins une bride (65) de serrage d'une extrémité (63) du support (55, 55a, 55b) sur une apophyse vertébrale (58) ou (59).
13/ - Dispositif selon l'une des revendications 1 à 12, caractérisé en ce que la face convexe (56) d'un support (55, 55a, 55b) est formée de métal poreux. 14/ - Dispositif selon l'une des revendications 1 à 13, caractérisé en ce que la face convexe (56) d'un support (55, 55a, 55b) est recouverte d'une couche d'hydroxyapatite.
