Abstract

The device described makes it possible to atomize medicaments to be administered by inhalation in patients with an intubation probe or a tracheotomy cannula. The device comprises a hollow piece (1) of which one cylindrical end (2) fits perfectly onto the connection of the intubation probe or the tracheotomy cannula and the other cylindrical end (3) fits perfectly onto the T-piece of the respirator. The main piece also comprises a lateral orifice (4) for introducing the valve of the vial containing the product to be atomized. A duct (6) prolongs the natural orifice (4), bends at right angles at the center of the piece (1) and extends towards the end (2). The valve of the vial can be blocked by a stop (5) located at the junction of the duct (6) and the lateral orifice (4). A flexible tube (8) attached to the free end (7) of the duct (8) penetrates the intubation probe or the tracheotomy cannula as far as their end in the bronchial tree. The flexible tube may be removable. The piece (1) comprises a rough concave part (9) diametrically opposite the lateral orifice which can be firmly gripped by the index finger of the user in order to press on the valve. The device may be sterile and for single use. Its use is recommended for all medicaments administered by inhalation in patients with an intubation probe or a tracheotomy cannula.

Description

Device for administering drugs by vaporization in intubated or tracheotomized patients.
The present invention relates to a device for administering drugs by vaporization in intubated or tracheotomized patients. There are an increasing number of drugs packaged for inhalation administration. The local pulmonary route is indeed preferable for certain products, in particular those which have pulmonary pharmacological effects. The product administered by inhalation may be vaporized by a continuous or metering valve with a gas propellant (this is most often the case) or by a pump. However, it is very difficult to administer these drugs by spraying in patients with an endotracheal prosthesis (intubation probe or tracheostomy tube) because the majority of the drug remains adherent to the prosthesis and the pusher does not adapt to the prosthesis. - The device according to the invention overcomes this drawback. It should be recalled that the intubation probe or the tracheostomy tube can be connected to a respirator by means of a connection called a T-piece. The device according to the invention is a pusher that can be used for continuous or metering valves or for pumps. This push-piece comprises a hollow part having two terminal orifices and a lateral orifice. The first terminal orifice is said to be distal. It is cylindrical and adapts perfectly to the connection of the intubation probe or the tracheostomy tube. The second terminal orifice is said to be proximal. It is cylindrical and adapts perfectly to the T-piece of the respirator. The lateral opening allows the introduction of the valve of the bottle containing the product to be vaporized. It adapts perfectly to the valve. It is extended by a bent pipe at right angles to the center of the room. This conduit com¬ carries at its departure from the lateral orifice a stop on which the valve of the bottle is blocked allowing the valve to sink into the bottle and release the vaporized product. After the right angle bend, the conduit goes to the distal orifice. It stops before or at the distal orifice. At its end is fixed a small caliber tube, hose, that enters the intubation probe or tracheotomy cannula e. The length of this tube is such that when the distal orifice of the tappet is adapted to the connection of the intubation probe or the tracheostomy tube, the free end of the tubing comes exactly at the free end of the airway tube or tracheostomy tube. It is recommended that the internal diameter of the conduit is equal to the internal diameter of the sou¬ pope at the level where these two elements fit together and that the internal diameter of the conduit is equal to the internal diameter of the pipe which sinks into the son¬ intubation or in the tracheostomy tube at the level where these two pieces fit together.
If the patient is mechanically ventilated using a ventilator, the user begins by disconnecting the T-piece from the connection of the intubation probe or the tracheostomy tube, introduces the push-up tube into the probe intubation or the tracheostomy cannula, adapts the distal end of the tappet to the connection of the intubation probe or the tracheostomy cannula and the proximal end to the T-piece then places the vial by pushing in its valve in the side opening. : At the start of the breathing time, it exerts pressure: the valve locks on the stop, sinks into the bottle and causes the release of the propellant gas and the medication if it is a medication packaged with a gaseous propellant or spraying the medicament if it is a medicament vaporized by a mechanical pump. Thus, the medicament comes out vaporized at the free end of the tube at the free end of the intubation probe or of the tracheostomy tube directly into the patient's bronchial tree.
If the patient breathes spontaneously, the user introduces the tube into the probe or the cannula, adapts the distal end of the plunger to the connection of the intubation probe or the tracheostomy tube j the patient thus breathes freely through the pusher by the proximal end thereof. In the same way as for a patient ventilated by a ventilator, the user adapts the bottle to the lateral opening. At the start of the patient's spontaneous inspiration, he puts pressure, vaporizing the medication directly into the patient's bronchial tree.
According to a preferred embodiment, the case comprises at the level diametrically opposite to the lateral orifice a concave portion roughly, offering the index finger of the user a solid grip for pressing the push-button while the other fingers hold the bottle. . 5 According to a variant, a nozzle allowing better vaporization can be welded to the free end of the pipe; its diameter must allow it to pass easily inside the intubation probes or tracheostomy tubes.
According to another variant, the thin pipe which fits on the bent pipe at a right angle is eliminated. The medication is then vaporized at the entrance to the intubation tube or the tracheostomy tube.
The characteristics and advantages of the invention will emerge from their description from the description which follows by way of example with reference to the accompanying drawings. Figure 1 shows in longitudinal section an embodiment of the device.

The distal end (2), cylindrical, adapts very precisely to the connection of the intubation probe or the tracheostomy tube. The cylindrical proximal end (3) adapts very precisely to the T-connector on the respirator. The lateral opening (4) allows the introduction of the valve of the bottle containing the product to be vaporized; it locks against the stop (5). A conduit (6) extends the lateral orifice; it is bent at right angles to the center of the part (1) and goes towards the distal end (2). At its free end (7) is fixed a flexible pipe (8) intended to sink into the intubation probe or the tracheostomy tube. Its length is calculated to arrive at the free end of the intubation probe or the tracheotomy tube. A rough concave part (9) diametrically opposite the lateral opening allows the index finger to have a solid grip for applying pressure to the valve. The internal diameters of the pipe (8) and the conduit (6) are equal at the distal end (7) of the conduit (6).
The valve (13) of the bottle containing the medicine to be vaporized is adapted to the lateral orifice (4) and locked against the stop (5). The internal diameters of the conduit (6) and the valve (13) are equal to the level where these two elements fit together. The ex- free end of the flexible hose (8) arrives at the free end (14) of the intubation probe (13) allowing the medicament to be vaporized directly in contact with the patient's bronchial tree.
The different sizes of the device allow it to adapt to the different sizes of intubation probe and tracheostomy tube fittings, the different dimensions of the T-pieces and the different lengths of intubation probe or tracheostomy tube.
