発明の開示
本発明はかかる従来技術の問題点を解消するために、 つぎのような構成を採用する。
すなわち、 本発明は、 バルーンが弾性糸と該弾性糸よ り 自由長の大きい非弾性糸からなる複合糸で捕強されて いることを特徵とするバルーン付カテーテルである。

芯鞘糸の作り方と しては、 非弾性糸をオーバ一フィ 一 ドしながら交撚または流体交絡する方法、 特にウ レタ ン 糸に合成繊維の摞糸を螺旋状に巻いた芯鞘糸がコンパク 卜である点で好ま しい。 非弾性糸は加工糸である方が、 伸縮性や弾性フィ ルムとの接着性等に優れていて好ま し い。 加工糸の中でも伸縮性に優れる仮撚加工糸が良い。 加工糸でない場合は弾性糸との分離を起こ しゃすい欠点 があるが、 それも撚数を十分大き くするなどの対策によ り該欠点を回避することは可能である。
バルーンは弾性フィ ルム/複合糸の筒状体 Z弾性フィ ルムの 3層構造であるこ とが望ま しい。 すなわち、 内面 の弾性フィ ルムは複合糸による補強効果を生かす作用を 有し、 外側の弾性フィ ルムは血管内の通過性を良く する 作用を有する。 本発明に係るバルーンは大抵のバルーン 付カテーテルに適用できるが、 特に、 互いに同心的に摺 動可能な 2重チューブの内チューブ (内チューブは中実 の心棒でも良い) に一端が、 外チューブに他端が固定さ れているようなバルー ン付カテーテルではさ らに優れた 効果が期待できる。 すなわち、 かかるバルーン付カテー テルでは両チューブを摺動させてバルーンを軸方向に伸 長させることにより、 血管や心臓内腔への挿入時や排出 時においてバルー ンの外径をさ らに小さ く できるのであ る。 内チューブの剛性が小さ く てバル一ンを伸脹できな い場合には、 該内チューブ先端にス ト ッパーを設けて該 内チューブ内に揷入可能な心棒で押すようにすることに より効果的にバル一ンを軸方向に伸長させることができ る。 外チューブの剛性が不足の場合には外チューブの先 端部に弾性率の高い繊維を固定して他端まで導いておく と良い。
弾性フィ ルムは伸縮性が大き く表面が滑かな薄膜で、 先端側の厚みが後端側より薄く製作されるこ とが多い。 その素材には、 ポ リ ウ レタ ンやゴムが用いられ、 特にゴ ムは初期弾性率が小さく、 本発明の弾性フィ ルム用素材 と しては優れた材料である。
弾性フィ ルムの厚みは 0 . 1〜 0 . 4 m mの範囲が好 ま しい。 またこの厚みは、 内層、 外層と もに同じ厚さで ある必要はない。
また、 先端側と後端側の厚みの差は 5〜 2 0 %位でよ く、 たとえば筒状の弾性フ ィ ルムを製作する際に薄くす るか、 また簡単な方法と しては、 製作した筒状の弾性フ イ ルムの該当部分を膨脹させて、 残留伸びを固定させて 膜厚を薄く するなどの方法で得られる。
複合糸の筒状体は、 編物、 織物、 組紐、 大きな綾角で 筒状に巻いたものなどが用いられるが、 中でも伸縮性の ある筒編構造のものが好ま しい。
瓢箪型バルーンとする場合は、 たとえば中程に弾性体 のパン ド等を、 ゴム糊などで接着すれば良い。
加工糸の芯糸となる弾性糸としては伸縮性を有する繊 維であれば別に制約されない。 たとえば天然ゴムや合成 ゴムなどのゴムならびにポリ ウレタ ンの単糸や撚糸が適 当であり、 巻きつける合成繊維の糸はナイ ロ ン、 ポリエ ステルやテフロンなどの強力糸の他、 ポリイ ミ ドゃポリ エチレンなどの高強力糸の単糸でも良いが、 撚糸が撓ゃ かで好ま しい。 これらの糸の太さは作用部位や目的に合 わせて要求される耐圧性が異なるので特に限定されない が、 芯糸となる弾性糸は 1 0〜 5 0 D、 巻き付ける強力 糸や高強力糸は 3 0〜 1 5 0 Dの範囲で選ばれることが 多い。
次に、 外側の弾性フィ ルムを複合糸の筒状体に接着し、 複合糸の筒状体の内側になる弾性フィ ルムとの間にはシ リ コーンオイル等を塗布しておけば、 膨脹、 収縮によつ てずれたりせず、 均等な膨脹が得らる。 バルーンの長さ は自由にできるが、 好ま しく は 1 5〜 7 0 m mの範囲が 適当である場合が多い。
バルーン両端や先端およびチューブの接続部分は接着 剤で表面を滑かに仕上げるのが好ま しい。
同軸 (同心) に組合わせるチューブの軟かい内チュー ブに用いる素材と しては、 たとえば塩化ビニルゃポリ ウ レタ ンなどが好ま しく使われる。 このチューブの長さは
5〜 2 5 c mが好ま しく 、 直径はセルディ ンガ法で挿入 するために 1 4 F rより小さいものが望ま しい。
また、 トルク性のあるやや硬い外チューブは、 たとえ ばポリエチレン、 テフロン、 ポリ プロ ピレンあるいはナ ィ ロ ンのような合成樹脂製チューブが使われる。 これら のチューブの他に、 たとえば塩化ビニルの可塑剤を溶媒 で抽出したチューブや、 放射線照射で架橋させて硬化さ せたチューブなどの硬いチューブも使用できる。 この外 チューブの直径は前述の理由で 1 4 F r より小さいのが 望ま しい。 また、 長さについては患者の年齢、 体格ゃ揷 入部から治療する場所までの距離によって異なるので規 定するものではないが、 好ま しく は全長 5 0〜 2 0 0 c mの範囲のものが使用される。 なお、 治療部位によっては、 柔らかい内チューブも外 チューブと同じように、 やや硬いチューブにしたものが 剛体の揷入を容易にする作用を有するので、 好ま しく適 用される場合がある。
また、 これらのチューブに硫酸バリ ウムやビスマスの 化合物あるいはタ ングステンのような金属粉末の造影剤 を混入させておく と、 血管内の位置をレン トゲン線照射 により知る助けとなり好ま しい。
さ らに柔らかい内チューブに埋設したり沿わせる糸や 編物の素材は、 細く て強力なものが望ま しく、 たとえば 上記の糸はいずれも使用できるが、 ポリアミ ド糸はその 中で最も優れている。
さ らに、 内チューブと外チューブの間に設置する空気 抜きチューブは钦かいチューブの撓ゃかさを妨げないこ とが必要で、 厚みが薄く て直径が小さい、 たとえばポリ イ ミ ドゃテフロ ンからなるチューブあるいはポリエチレ ンチューブなどが用いられる。
内チューブに挿入し、 バル一ンを伸ばす剛体と しては、 たとえばステンレスのパイプや細い棒が用いられ得る。
またバルーンを膨脹させる流体としては、 たとえば生 理食塩水や炭酸ガスあるいは造影液などが用いられる。 これらの流体は、 注射筒等により、 その一定量を圧入す る方法で用いられる。
本発明のバルーン付カテーテルは、 伸縮能を大き く し た複合糸の筒状体を用いたことにより、 伸縮性が大き く、 かつ耐圧性と均一な膨脹能を有するという特徴を有する。 本発明によれば、 大口径のバルーンでも細長く 小さ く して血管内に挿入でき、 さ らにチューブ先端部が血管内 壁を擦って傷つけることがなく、 血管内や心臓内をガイ ドワイャゃバルーンの浮遊性を利用して治療部位に送り 込むことができる。
したがって治療に要する時間が短く、 術後の管理も簡 単で治癒も早く なり、 しかもバルーンによる血管塞部の 拡張や心臓弁裂開術を非手術的に簡単容易に治療する方 法と して提供し得たものである。 また、 本発明に用いる バルーンは、 各種の大きさのものを別々に製造すること により、 さ らに簡単に製造することができ、 歩留りがよ く コス トの低いバルーン付カテーテルを提供する こ とが できる。
(実施例)
つぎに実施例を挙げ本発明を更に説明する。
実施例 1
2 0 Dのポ リ ウ レタ ン糸 (弾性糸 : "オペロ ン" ) に 7 0 D - 2 4 f のポ リ エステル糸 (非弾性糸) からなる 仮撚加工糸を、 該ウ レタ ン糸に対して 3 0 0 %オーバー フィ 一 ドしながら 4 0 0 Tノ mで交撚して複合糸を製造 した。
この自由長の比を S — Sカーブから読みとつたら、 0 . 2 6であった。 この複合糸を使用してバル一ンを作成した。
すなわち、 外チューブに 1 3. 5 F rで長さ 5 6 0 m mの造影剤を含有するポ リエチレンチューブと、 D O P 4 5部を含有する 1 3. 5 F rで長さ 1 3 0 mmの塩化 ビニルチューブを、 直径 3. 5 mmのステンレスパイプ を間にして継ぎ、 塩化ビニルチューブの先端に直径 3 m m、 長さ 6 mmのステンレスパイプを装着する。 この時、 塩化ビニルチューブの中空側に 4 0 0 Dポリイ ミ ドの撚 糸を沿わせ、 両端のステンレスパイプに固定しておく。 内チューブには D 0 P 4 5部を含有する 6. 6 F rの塩 化ビニルチューブの先端に、 直径 1. 6 mm、 長さ 7 m mにコルセッティ ングで、 内側に凸部を持たせたステン レスパイプを装着し、 後端に 1 4 G長さ 7 0 mmの針基 付きステンレスパイプを揷入、 内チューブおよび外チュ ーブの継ぎおよび挿入したステンレスパイプを、 7 0〃 mナイ ロ ン糸で縛り固定した。 この内チューブを Wコネ クタに通し、 さらに外チューブに挿入した後、 該外チュ ーブを Wコネクタに固定し、 チューブ部分を作成した。
次にバルーンの内側弾性フィ ルムと して、 厚さ 0. 2 5 mm、 長さ 2 5 mmのゴムを、 内チューブ、 外チュー ブの各先端に 7 0 Ai mのナイロン糸で縛って固定し、 表 面にシ リ コ ンオイルを少量塗布し、 別に前述の複合糸を、 直径 2 O mmに円周に 5 0本の編み針を並べた編み機で 筒編みした編物の 3 O mmの中程に、 厚さ 0. 2 5 mm、 長さ 7 m mのゴムをゴム糊で張りつけてバン ドとし、 さ らにその外側に、 先端側の厚みが 0. 2 5 mmで、 後端 側の厚みが 0. 3 m mであるゴムチューブを貼り付けた ものを覆せて、 全長が 2 5 mmになるように合せて、 そ の両端を 7 0 〃 mのナイ ロ ン糸で縛り固定した。 そして 余分の編物やゴムは切り取り、 バルーン部分を完成させ

次に直径 0. 9 mmのポリ イ ミ ドチューブを、 Wコネ クタの残りの出入口の一つから、 外チューブと内チュー ブの間の流体輸送路をバルー ンの 1 0 mm近く まで挿入 した後、 後部に捕強用ポ リ エチ レ ンチューブを wコネク 夕の中まで被覆した。 その後端に二方活栓を取り付け、 出入口の隙間を接着材で埋めた後、 最後にバルー ンの両 端、 先端およびチューブの継ぎを、 エポキシ樹脂で表面 を滑らかに仕上げて、 本発明のバルーン付カテーテルを 完成させた。
このバルーン付カテーテルに注射筒を取り付け、 空気 抜きチューブより空気を抜きながら水に置き換えたあと、 約 7 mlの水を注入した。 このバル一ンは先端のみが約 2 3 mmに膨脹した。 また、 さ らに注入量を 2 3 mlにする と約 2 8 m mの円筒状になった。
次にこのバルーンから水を抜いて見たが、 皺を生ずる こ となく表面は滑らかに収縮した。 次に内チューブに 直径 1. 1 9 mmのステン レスチューブを揷入し、 内チ ュ一ブの先端を押し、 パルーンを 2 5 mmから 5 5 nim に引き伸ばしたところ、 バルー ンの直径は約 7 m mであ つたものが 4 . 5 m mとなり、 ステンレスチューブを引 き戻すと、 バルーンは元の直径と長さに戻った。
実施例 2
実施例 1の同様のバルーン付カテーテルを僧帽弁交連 裂開術の臨床に供した。 すなわち心房中隔穿刺法を用い て、 特製のガイ ドワイヤ'を経皮的に、 股静脈より右心房、 心房中隔を通して左心房に揷入した。 つぎに 1 4 F rの ダイ レー夕をこのガイ ドワイヤに沿わせて進め、 股静脈 穿刺部および心房中隔穿刺部を拡大した。 続いて本発明 のバルーン付カテーテルを右心房より心房中隔を通して、 左心房内に挿入し、 バルーン先端のみを直径 1 O m m程 度に炭酸ガスで膨脹させ、 スワ ンガンツカテーテルの如 く血流に乗せて弁口に流しこみ、 左心室内に揷入し、 さ らに希釈造影剤でバルーンの先端側を膨脹させ、 軽く 引 き寄せて裂開すべき弁口に当てた。 この位置でさ らにバ ル一ンを膨脹させ、 く びれたバルーンで弁口を挟み、 そ のまま膨脹させたところ、 バルーンは滑脱することなく 弁口を拡大した。
バルーンの膨脹、 収縮の 5秒を含む裂開術は 1時間で、 副作用も生じず、 入院から退院までの所用日数は 3 日で、 血管の露出、 切開を伴なう裂開術の場合にく らベて、 半 分以下の時間で済んだ。
実施例 3
実施例 1を同様にしてバルーン付カテーテルを製造し た。 ただし弾性糸と しての "オペロ ン" 糸は 3 0 Dであ り、 ポリエステル糸のオーバーフィ ー ド率を 2 7 0 %、 撚数を 4 0 O TZmに変更した。
このバルーン付カテーテルは、 見掛上複合糸の構成糸 の自由長の比は 0. 2 8 となり、 バルー ンの最大膨脹径 は 3 0 mm、 最小収縮径は 4〜 5 mmの、 皺の発生しな い優れた伸縮性能を有するものであった。
産業伏の利用可能性
本発明に係るバルー ン付カテーテルは狭窄あるいは閉 塞した血管等に収縮したバルーンを潜り込ませバルーン に流体を圧入、 膨脹するバルーンで拡張し狭窄部或いは 閉塞部を正常化する治療方法にひろく利用するこ とがで きる。 特に、 老化や石灰化あるいはリ ュウマチ熱の後遺 症で狭窄している心臓弁の弁切開術や人工弁の弁置換手 術に代わる非手術的な経皮的弁裂開術に広く利用するこ とができる。

Claims

言青求 の 範 囲
(1) パル一ンが弾性糸と該弾性糸より 自由長の大きい 非弾性糸からなる複合糸で補強されていることを特徴と するバル一ン付カテーテル。
(2) 弾性糸と非弾性糸の自由長の比が 0 . 1 5〜 0 . 5であることを特徵とする請求項 (1 )に記載のバルーン付 力テ——テノレ。
(3) 弾性糸と非弾性糸の自由長の比が 0 . 2〜 0 . 3 5であることを特徵とする請求項 (1 )に記載のバルーン付 カテーテル。
(4) 該複合糸が弾性糸を芯、 非弾性糸を鞘とする芯鞘 糸であることを特徵とする請求項 (1 )に記載のバルーン付 力テ一テノレ。
(5) 該芯鞘糸がゥレタ ン糸に合成繊維の撚糸を螺旋状 に巻いた芯鞘糸であることを特徵とする請求項 Π )に記載 のバルーン付カテーテル。
(6) 非弾性糸が仮撚加工糸であることを特徵とする請 求項 (1 )に記載のバルーン付カテーテル。
(7) バル一ンが弾性糸と該弾性糸より 自由長の大きい 非弾性糸からなる複合糸の編織物で捕強されていること を特徵とするバル一ン付カテーテル。
(8) 該編織物が編物であることを特徵とする請求項 (7) に記載のパルーン付きカテーテル。
(9) バル一ンが弾性フィ ルム Z複合糸の筒状体 Z弾性 フィ ルムからなる 3層構造であることを特徵とする請求 項 (1 )に記載のバル一ン付カテーテル。
(D 複合糸の筒状体が弾性フィ ルムの一方又は両方と 接着されていることを特徴とする請求項 (9)に記載のバル 一ン付カテ一テノレ。
(11) バル一ンが互いに同心的に摺動可能な 2重チュ一 ブの内チューブに一端が、 外チューブに他端が固定され ていることを特徴とする請求項 (1 )に記載のバルー ン付力 テーテノレ。
(12) 該内チューブ内に挿入可能な心棒と該内チューブ 先端に該心棒のス ト ッパーを有することを特徴とする請 求項 (11)に記載のバルー ン付カテーテル。
