Definitions
- the invention relates to a device for implanting electrolytically detachable occlusion spirals in body cavities or blood vessels.
- endovascular techniques for occlusion of body cavities or vessels such as arteries, veins, fallopian tubes or vascular malformations such as. B. vascular aneurysms is known in the art.
- the occlusion coil is usually inserted and deposited into the cavity to be occluded with the help of an endovascular guide wire through a catheter.
- the separation of the occlusion helix necessary for landfilling is particularly problematic from a technical point of view, because the device required for this must be kept as small as possible in order to be able to be guided through the small diameter of the catheter to the target sites, and on the other hand it must reliably separate the Cause occlusion helix, since otherwise the occlusion helix can be unwantedly removed from the cavity to be occluded when the catheter is returned, thus causing injury and / or rupture of the cavity or vessel wall.
- EP 0 484 468 describes a device for implanting an occlusion coil based on the electrolytically corrodible design of the wire tip of the guide wire at the connection between the guide wire and the occlusion coil.
- this device uses the electrical voltage applied to the electrothrombization to the occlusion coil serving as the anode for the simultaneous removal of the wire tip and the occlusion coil located thereon, it has the disadvantage, like the aforementioned mechanical separation, that only implants are predefined Length can be separated. It is therefore generally necessary to determine the length, ie the length, before the immediate implant placement based on the size of the cavity to be occluded. H. longitudinal extension of the occlusion helix to be used is predefined.
- Another disadvantage of the electrolytic detachment of the guidewire tip is the fact that only materials which have high stability can be used to form the guidewire, so that the occlusion wire to be inserted can be safely guided through the guidewire is.
- the choice of materials for the formation of the electrolytic peeling point is therefore very limited.
- the occlusion coil and guide wire are not made from one piece, but are generally mechanically connected to one another.
- This design has the disadvantage that, in order to ensure sufficient stability in the proximal area of the guide wire and to enable the electrolytically corrosive dissolution of the wire tip in the distal area of the wire, the guide wire must be tapered towards the tip by means of complex grinding processes.
- This corrodible area of the wire tip of the guide wire at the connection point between the guide wire and occlusion helix cannot be less than a certain minimum diameter of approximately 0.05 mm to ensure sufficient stability of the connection point, since it is subject to high bending stress.
- the corrodible wire tip which represents the connection point between the occlusion coil and the guide wire is therefore quite rigid and takes a relatively long time for the electrolytically corrosive dissolution.
- DE 44 45 715 C2 describes the separation of an occlusion coil from the insertion aid, which is brought about by a laser beam which is focused on the site of the implant to be split by means of an optical fiber which is carried along.
- This device makes it possible to use the filling state of the cavity to be occluded during the procedure to separate the optimal length of the occlusion helix for filling the cavity.
- the length that is optimally suitable for filling the cavity can be separated and deposited.
- the technology required to use this device is currently still very expensive.
- the object of the invention is to provide a device which is as cost-effective as possible. cheap, effective and safe way allowed to deposit occlusion spirals in the correct length in body cavities or vessels.
- a device for implanting occlusion coils which can be removed by electrolysis in body cavities or blood vessels with a voltage source, a cathode, a catheter and an occlusion coil which can be displaced in the longitudinal direction and serves as an anode, which is characterized in that the occlusion coil is attached to is formed electrolytically corrodible from a plurality of points arranged at intervals from one another, so that one or more variably dimensioned lengths of the occlusion coil can be detached by electrolysis in contact with a body fluid.
- the invention is based on experiments by the inventors, which showed that when a current is applied to a device according to the invention, surprisingly, the electrolytically corrodible point of the occlusion coil closest to the distal end of the catheter is solved by electrolysis.
- This specificity is probably based on the fact that, on the one hand, the electrolytically corrodible sites of the occlusion filament in the catheter are isolated from the ionic medium by the catheter, that is to say they cannot be subject to electrolysis, and on the other hand, the current density due to the increasing distal resistance in the occlusion filament from proximal to decreases distally.
- the electrolytically corrodible point which is distally the first to be connected to the distal end of the catheter is therefore most strongly subjected to electrolytic processes and preferably dissolves.
- the device according to the invention for the implantation of occlusion coils like no other device known in the prior art, combines the advantages of the occlusion effectiveness with operational safety and cost-effectiveness.
- the implant length that can be determined during the implantation process prevents an occlusion spiral that is too short from being inserted into the cavity to be occluded, which leads to the formation of a thrombus that is insufficiently large for the space to be occluded.
- the device according to the invention for the implantation of occlusion coils has the advantage that favorable unit lengths of occlusion coils can be used for mass production.
- the electrolytically corrodible points of the device according to the invention constitute part of the occlusion coil itself and are also available in large numbers, they are subject to a significantly lower bending stress during the implantation process compared to the conventional, rigid, electrolytically formed connections between guide wire and occlusion device.
- This low bending stress enables the use of electrolytically corrodible points with significantly smaller diameters than in the prior art, which leads to improved and faster electrolytic detachability of the occlusion coil.
- Such small diameters of less than 0.5 mm are suitable for this, for. B. to achieve mechanical processes.
- Another advantage of the formation of the electrolytically corrodible points of the device according to the invention in the occlusion coil itself compared to the in the prior art detachment of the guidewire tip is the much larger selection of materials that can be used to form the corrodible detachment points.
- the electrolytically corrodible points of the device according to the invention located in the occlusion coil do not have to have a particularly high degree of stability, and less stable, more flexible materials can therefore also be used, provided they are corrodible and compatible with the body.
- An insertion aid designed as a guide wire can expediently adjoin the occlusion helix proximally.
- Such an embodiment has the advantage that the guidewire can be formed from materials that are less expensive than the occlusion coil, especially since it does not come into contact with the body tissue.
- the design of the guide wire is preferably determined such that it allows good control of the occlusion helix by the catheter, which contributes to better depositability.
- the insertion aid and occlusion helix are preferably connected to one another by soldering and / or adhesive and / or welding processes and / or mechanical connections. These are connection methods known in the prior art, which are distinguished by the simplicity and the stability of the connection produced in this way.
- the guide wire and occlusion helix of the device according to the invention can be designed as parts of the same wire.
- This embodiment is characterized by particular stability and can be very cost-effective since the aforementioned connection methods of the guide wire with the occlusion helix are eliminated.
- the occlusion spiral or a part thereof is designed as a micro spiral.
- This design has the advantage that an increased surface is provided for thrombosis.
- whose configurations of the occlusion coil are used, which increase the surface area of the latter for example configurations are conceivable, the distal end of which branches out.
- an embodiment of the device according to the invention is advantageous in which the occlusion helix or part thereof is under elastic prestress so that it forms turns after being released from the catheter.
- This design permits a dense and gentle filling of the cavity to be occluded without the occlusion helix having to be formed by the wall of the cavity to be occluded to form such coils, which reduces the risk of a wall rupture.
- the elastic pre-tension leads to the formation of secondary windings.
- the electrolytically non-corrodible sections of the occlusion helix can expediently contain one or more of the following materials: noble metals or noble metal alloys, corrosion-resistant ceramic materials, corrosion-resistant plastics, preferably platinum metal alloys.
- the occlusion filament of which contains one or more of the following materials at the electrolytically corrodible points: ceramic materials, plastics, non-noble metals or alloys thereof, preferably stainless steel
- the stainless steels of types AISI 301, 303 or 316 or subgroups of these types are particularly suitable.
- the ceramic materials and plastics used for the formation of the occlusion helix are electrically conductive.
- the electrolytically non-corrodible sections at the transitions to the electrolytic corrodible points selected material combinations that are suitable for forming local elements. In this way - regardless of the reduction in the diameter of the corrodible points - the electrolytic detachability of the occlusion coil is improved.
- stainless steels preferably of the types AISI 301, 304, 316 or subgroups thereof, Ti or TiNi alloys or Co-based alloys with one or more of the following noble metals, are best suited for the formation of the electrolytically corrodible points or precious metal alloys: Pt, Pt metals, Pt alloys, Au alloys or Sn alloys.
- electrolytically corrodible points which form local elements on both sides
- This embodiment of the electrolytically corrodible spots is much more corrosion-friendly and therefore corrodes much faster than electrolytically corrodible spots, which form a local element only on one side. Material combinations with the greatest possible distance in the electrochemical voltage series are therefore preferred.
- the shape of the electrolytically corrodible points is expediently designed from a functional point of view.
- the essentially straight formation of the electrolytically corrodible points has the advantage that it is technically inexpensive.
- the alignment of the essentially straight, electrolytically corrodible points in the longitudinal axis of the occlusion coil is advantageous.
- the electrolytically corrodible points of the occlusion filament can be formed, for example, by molded parts which are inserted between the electrolytically non-corrodible parts of the occlusion filament.
- This embodiment has the advantage that a particularly large number of different materials can be combined with one another for the formation of the electrolytically corrodible points and electrolytically non-corrodible sections.
- This embodiment also has the advantage that the electrolytically corrodible points and the electrolytically non-corrodible sections can be joined together in a modular and therefore technically complex manner to form occlusion coils of variable lengths. This is particularly simple if the electrolytically corrodible points and thus the molded parts forming them are essentially straight.
- the molded parts forming the electrolytically corrodible points can expediently be connected to the non-corrodible sections by soldering and / or adhesive and / or welding processes.
- the molded parts forming the electrolytically corrodible points can be mechanically attached to the electrolytically non-corrodible sections, e.g. B. by clamping or clamping, provided that the electrolytically non-corrodible sections have recesses for receiving the molded parts.
- electrolytically non-corrodible sections which are designed as micro-helixes circumscribing an inner cavity.
- the molded parts can thus be inserted and fixed in a form-fitting manner in this cavity. It may furthermore be expedient if the outer regions of the sections of the electromechanically non-corrodible sections designed as microwaves that receive the molded parts are reinforced.
- the molded parts forming the electrolytically corrodible points are pre-corroded by etching or other methods are so that their diameter tapers towards the center.
- the outer or weakened portions of the molded parts with a larger diameter are then joined to the electrolytically non-corrodible sections, for example by welding, mechanical insertion or adhesive bonding.
- the connection between electrolytically corrodible points and electrolytically non-corrodible sections is therefore very stable, whereas the diameter tapering towards the center of the molded part as a result of pre-corrosion leads to good electrolytic detachment of the occlusion coil.
- the molded parts with a partial coating of a material that is in the series of stresses above the material that forms the molded parts.
- This embodiment is particularly advantageous with regard to the electrolytic corrodibility of the electrolytically corrodible points which are located where the coating on the molded part is missing. Coatings of Zn or Sn or alloys of these metals on molded parts made of stainless steel have proven to be particularly advantageous.
- the mechanical attachment of the molded parts is particularly advantageous if the electrolytically corrodible points are to be essentially straight and are to be arranged along the longitudinal axis of the occlusion coil.
- the juxtaposition of the modules forming the occlusion helix (non-electrolytically corrodible sections and electrolytically corrodible points) is associated in this case with a particularly low technical outlay.
- a combination of the methods mentioned can also be used to further secure and stabilize the joining of the individual modules forming the occlusion helix.
- the flexibility of the occlusion helix is ensured even in the case of mechanical joining due to the choice of material and the small diameter of the molded parts forming the electrolytically corrodible points.
- the molded parts forming the electrolytically corrodible points are designed as microsystem components. These can be designed, for example, as elongated microsystem components, the diameter of which tapers towards the center.
- the microsystem components are inserted using the common methods mentioned.
- the use of such microsystem components tapering towards the center has the advantage that the regions of the largest diameter are joined to the electrolytically non-corrodible sections and thus ensure stable cohesion.
- the tapered area with a smaller diameter, however, is exposed to the surrounding medium and can easily be electrolytically corroded. In this way, particularly small diameters of the electrolytically corrodible points can be achieved.
- the tapering of the diameter of the electrolytically corrodible points toward the center is also expedient for other designs of the electrolytically corrodible points with regard to good corrodibility.
- the occlusion helix or a part thereof contains a continuous wire core made of electrolytically corrodible material, which is surrounded by a sheath of electrolytically corrosion-resistant material which is interrupted at intervals in the longitudinal axis.
- the guide wire and the core of the occlusion coil are preferably parts of the same wire.
- This embodiment is particularly cost-effective because, on the one hand, the welding, soldering or gluing process required to connect the occlusion helix and insertion aid is dispensed with and, on the other hand, the wire core consists of electrolytically corrodible material, which is generally less expensive than the electrolytically non-corrodible materials, whereas only a small amount Amounts of material which is less corrodible than the wire core have to be used for coating.
- the diameter of the occlusion helix of the device according to the invention is expediently chosen so that it ensures sufficient stability on the one hand and on the other hand the electrolytically corrodible points are well electrolytically corrodible in situ.
- embodiments with diameters of the occlusion coil between 0.01 and 0.05 mm, preferably 0.02 to 0.04 mm and particularly preferably 0.03 mm, are present at the electrolytically corrodible points.
- the electrolytically non-corrodible sections of the occlusion helix can also have larger diameters.
- the tip of the guidewire is insulated, for example, by a poorly corrodible material coating or coating with a shrinking loop, so that it is not electrolytically corrodible.
- the device according to the invention is preferably intended for use in veterinary or human medical procedures, particularly in the endovascular treatment of intracranial aneurysms and acquired or congenital arteriovenous vascular malformations and / or fistulas and / or tumor embolization by thrombosis.
- FIG. 1 shows the vertical view of a micro-coil 3 positioned in a berry neurysm with associated device in multiple magnifications
- Figure 4 is a longitudinal sectional view of the attachment of the
- 1 denotes a catheter, in particular a flexible microcatheter.
- the microcatheter 1 is used to position the occlusion coil 3, which is made of a platinum metal alloy and has electrolytically corrodible points 11 made of stainless steel and is designed as a micro-coil, with the aid of the guide wire 2 which is welded to the micro-coil 3 at the entrance of the aneurysm 6. Since this connection between guidewire 2 and micro-coil 3, which is caused by welding of a different type, is not intended for the electrolytic detachment of the micro-coil 3 and consequently does not have to be of particularly small diameter, it is designed to be particularly stable.
- the use of stainless steel and platinum for the formation of the guidewire on the one hand and the occlusion helix on the other hand is particularly advantageous, since the nickel contained in steel forms a very smooth and stable connection with the platinum during welding.
- the micro-coil 3 Due to the distal displacement of the guide aid 2 in the longitudinal axis of the microcatheter, the micro-coil 3 is inserted into the aneurysm 6, which forms secondary windings 4 due to its elastic prestress after leaving the microcatheter 1. Due to the longitudinal displaceability of the guide wire 2 and the micro coil 3 in the microcatheter 1, a length of the micro coil 3 which is individually adapted to the volume of the cavity to be filled is introduced into the latter.
- FIG. 1 shows a micro helix 3, the electrolytically corrodible point 9 closest to the distal end of the microcatheter 1 has already been electrolytically corroded.
- FIG. 1b shows an enlarged detail of FIG. 1, the electrolytically corrodible point closest to the distal end of the microcatheter 1 in the corroded state 14.
- further electrolytically corrodible points 11 located in the blood or in the microcatheter 1 are still in an intact state in front.
- the electrolytically corrodible points are adapted to the shape of the micro-turns 19 of the micro-helix.
- FIG. 1 c also shows, in a detail enlargement of FIG. 1, the electrolytically corrodible point closest to the distal end of the microcatheter 1 in the corroded state 14 for a micro coil 3 with essentially straight electrolytically corrodible points 1 which are aligned on the longitudinal axis of the micro coil 3.

- FIG. 2a shows a micro-coil 3 with non-corrodible sections 10 made of a platinum metal alloy, to which a film part 17 of 0.03 mm diameter, which is made of stainless steel and forms the electrolytically corrodible point 11, is attached by welding.
- FIG. 2 b shows a section of a micro coil 3 according to the invention with a microsystem module 16 as an electrolytically corrodible Location 11, which is inserted by a gluing process between the non-electrolytically corrodible sections 10.
- FIG. 2c shows a section of a micro-coil 3 containing a stainless steel core 12 with a diameter of 0.03 mm.
- This stainless steel core 12 is surrounded by a coating of electrolytically corrosion-resistant material 13, which is provided at regular intervals with interruptions at which the stainless steel core 12 is accessible from the outside and accordingly forms an electrolytically corrodible point 11.
- FIGS. 3a-c show, in a larger magnification than FIG. 1, a section of three different embodiments of the micro coil 3 according to the invention with electrolytically corrodible points 11 aligned in the longitudinal axis of the micro coil 3.
- FIG. 3a shows a micro-helix 3 with an essentially straight shaped part 17 made of stainless steel, which is inserted in a form-fitting manner in the interior 18 of the micro-turns 19 made of platinum wire.
- the modular joining of electrolytically non-corrodible sections 10 made of platinum wire micro indices 19 and essentially straight shaped parts 17 leads at regular intervals where the essentially straight shaped parts 17 are not surrounded by the platinum metal micro-turns 19 and are therefore accessible from the outside to form electrolytically corrodible points 11.
- the non-corrodible sections 10 of the micro coil 3, on the other hand, are formed by the platinum wire wound in micro windings 19, which closes mechanically in a form-fitting manner on both sides around the essentially straight stainless steel molded parts 17.
- FIG. 3b also shows a modular micro-coil 3 in which the micro-turns 19 of the platinum wire form-fit around the ends of a microsystem module 16 made of stainless steel and thus form the electrolytically non-corrodible sections 10, between which the exposed sections of the microsystem module 16 form the Form electrolytically corrodible point n.
- microsystem module which largely consists of an Sn alloy 22 and only contains a micro-detachment element 24 in the tapering center, which is made of stainless steel and forms an extremely small, electrolytically corrodible point.
- This advantageous form of training is extremely easy to corrode and therefore particularly easy to remove.
- FIG. 3c alternatively, the detail from a microwave coil 3 according to the invention with an essentially straight shaped part 17 made of stainless steel forming the electrolytically corrodible point 11 is shown, which is welded to the micro-turns 19 made of platinum wire forming the non-corrodible sections 10.
- the electronegativity differences between the stainless steel forming the corrodible sites 11 and the platinum metal alloy forming the non-corrodible sections 10 causes the electrolytic dissolution of the electrolytically corrodible sites 11 in an ionic medium such as blood when an electrical voltage is applied.
- FIG. 4 shows a longitudinal section through the transition between the guide wire 2 and the occlusion coil 3 in enlarged form.
- the guide wire 2 is made of stable, stainless steel and surrounded by a coating 25 which prevents the corrosion of the steel.
- This coating can either be non-conductive, in which case the electrical current for the end of the microcatheter 1 is conducted to the occlusion coil 3.
- the coating is not corrodible but is electrically conductive (for example a graphite coating), in which case the electrical current can also be conducted into the occlusion coil 3 via the guide wire 2.
- the occlusion coil 3 is formed by an arrangement of pre-corroded molded parts 17 made of stainless steel, which are mechanically and with the help of gluing in the interior space 18 of the platinum wire micro-turns 19 forming the electrolytically non-corrodible points.
- pre-corroded molded parts 17 due to their small diameter, form particularly easily corrodible spots.
