GEFASSPROTHESE Die Erfindung bezieht sich auf eine Gefäßprothese aus synthetischem oder natürlichem Material zum Verschließen von Öffnungen in oder an Blutgefäßen. Gefäßprothesen können aus unterschiedlichen Materialien bestehen, wobei insbesondere synthetische Gewebe wie beispielsweise Dacron-Gewebe oder auch natürliche Materialien zum Einsatz gelangen, um schadhafte Teile eines Blutgefäßes zu sanieren. Bei der Aortendissektion kommt allerdings erschwerend hinzu, dass es sich hier um eine überaus zeitkritische Intervention handelt. Bei einer Aortendissektion ist die Wand zerschichtet und zwischen den Teilen der Wand befindet sich Blut. Es läßt sich daher nur schwer eine Blutstillung erreichen und für die Durchführung des Anschlusses an die Prothese ist in aller Regel ein Kreislaufstillstand in tiefer Hypothermie durchzuführen, woraus der Umstand resultiert, dass alle diese Maßnahmen äußerst zeitkritisch sind. Die erfindungsgemäße Gefäßprothese soll daher so ausgestattet sein, dass ein rascher Anschluss und eine entsprechend stabile Verbindung gelingt, sodass der Kreislaufstillstand wiederum aufgehoben werden kann. Zur Erzielung eines besonders raschen Verschlusses ist die erfindungsgemäße Gefäßprothese im wesentlichen dadurch gekennzeichnet, dass die Prothese an ihren mit dem zu sanierenden Blutgefäß zu verbindenden Rändern sich quer zur Ebene des Prothesenmaterials erstreckende Nadeln und/oder Haken aufweist. Dadurch, dass an den Rändern der Prothese sich quer zur Ebene des Prothesenmaterials erstreckende Nadeln und/oder Haken angeordnet sind, läßt sich eine rasche Verankerung der Prothese an den Rändern des abzudichtenden Blutgefäßes gewährleisten und bei entsprechender zusätzlicher Verwendung von üblichen Klebstoffmaterialien oder dergleichen die entsprechende mechanische Festigkeit früher erreichen, ohne dass lange und aufwändige Nähvorgänge erforderlich sind. In besonders vorteilhafter Weise kann hierbei der Rand mit Klebstoff ausgerüstet sein, wobei zusätzlich im Bereich der Nadeln bzw. Haken und des Klebstoffs Gerinnungshilfsmittel aufgebracht sein können. Insbesondere diese Gerinnungshilfsmittel bzw. blutstillenden Substanzen sind mit Rücksicht auf den Umstand, dass vor dem operativen Eingriff in
aller Regel Substanzen zur Herabsetzung der Blutgerinnung, wie beispielsweise Heparin, verabreicht wurden, von wesentlicher Bedeutung für den raschen dichten Verschluss. Die erfindungsgemäßen Gefäßprothesen sind insbesondere als Ersatz von Teilbereichen von Blutgefäßen in schlauchförmiger Ausbildung besonders vorteilhaft. Dabei ist die Ausbildung mit Vorteil so getroffen, dass der Rand bzw. die Anschlussstellen einer schlauch- förmigen Prothese als Stülpkragen ausgebildet ist/sind und über wenigstens einen Teil seiner/ihrer Axiallänge quer zur Ebene des Wandmaterials sich erstreckende Nadeln oder Haken für die Festlegung der Prothese in axialer Richtung aufweist. Wenigstens ein Stirnende einer derartigen schlauchförmigen Prothese trägt hierbei den Stülpkragen und kann einfach in ein entsprechend dissektiertes Ende eines Blutgefäßes eingeschoben werden. Der Stülpkragen stellt sich im Bereich des Anschlusses an das Blutgefäß mit im wesentlichen S-förmigem Querschnitt dar, wobei das in Richtung zum Blutgefäß offene Ende des S-förmigen Stülpkragens einander zugewandt die entsprechenden Nadeln oder Haken aufweist, sodass die Festlegung durch einfaches Zusammendrücken bzw. -zwicken dieses nach außen offenen Teils des Stülpkragens gelingt. Die Prothese kann in der Folge nach dem mechanischen Anheften des Stülpkragens weiter in das Blutgefäß eingeschoben werden, wobei auf diese Weise noch besonders hohe Aussteifungen erzielt werden können. Um die mechanische Befestigung des in der Folge durch Kleben mit der Gefäßwand verbundenen Endes der Prothese zu verbessern ist die Ausbildung mit Vorteil so getroffen, dass wenigstens ein Teil der Nadeln und/oder Haken die Gefäßwand durchsetzt. Durch Zusammenpressen der zu dem Stülpkragen umgeschlagenen Enden der Prothese gelingt es auf diese Weise, die Nadeln und/oder Haken nicht nur durch die Gefäßwand hindurchzudrücken, sondern auch mit dem jeweils zweiten umgeschlagenen Teil des Stülpkragens zu verbinden bzw. zu verankern. Mit Vorteil ist die Ausbildung so getroffen, dass die Enden einer schlauchförmigen Prothese verstärkt und/oder ausgesteift ausgebildet sind, wodurch insbesondere durch weiteres Einschieben der Prothese nach dem Festlegen des Stülpkragens die Gefahr einer Gefäßverengung verringert werden kann.
Wenn gleichzeitig bereits vorhandene Stenosen wirksam behandelt werden sollen, kann die Erfindung mit Vorteil so weitergebildet sein, dass die Enden einer schlauchförmigen Prothese Anschlussstellen für einen Stent aufweisen. In besonders einfacher Weise kann eine derartige Ausbildung so getroffen sein, dass die Enden einer schlauchförmigen Prothese mit Armierungselementen ausgestattet sind, welche nach Art eines Stents in eine stabile Durchmesserlage bringbar sind. Prinzipiell können schlauchförmige Gefäßprothesen jeweils an einem Ende mit der stülpkragenartigen Ausbildung versehen sein, wobei mehrere derartiger schlauchförmiger Prothesen mit jeweils einem Kragen miteinander verbunden werden und die erforderliche Länge angepasst werden kann. Bei definierter Einbaulänge kann aber die schlauchförmige Prothese von vornherein mit zwei Stülpkrägen ausgebildet sein, sodass der beidseitige Anschluss an das Gefäß nach der erfolgten Dissektion ermöglicht wird. Die Achsrichtung der Prothese uss hierbei nicht notwendigerweise normal auf die Ebene des Stülpkragens ausgebildet sein. Vielmehr ist es möglich, das jeweilige Ende der Prothese schräg zur Achse der Prothese verlaufen zu lassen und einen entsprechend angepaßten Stülpkragen vorzusehen. Anstelle langwieriger Nähte erfolgt erfindungsgemäß in der Regel eine Verklebung, wobei die erforderliche mechanische Festigkeit durch die sich quer zur Umfangsrichtung des schlauchförmigen Prothesenmaterials bzw. quer zur Ebene des Prothesenmaterials erstreckenden Nadeln und/oder Haken bereits zu einem frühen Zeitpunkt gewährleistet ist, bevor der Klebstoff seine volle Festigkeit erreicht hat. Auf diese Weise wird es möglich, bereits zu wesentlichen früheren Zeitpunkten die sanierten Blutgefäße wiederum unter einen entsprechenden Blutdruck zu setzen und den zuvor eingeleiteten Kreislaufstillstand wieder aufzuheben. Die Erfindung wird nachfolgend anhand eines in der Zeichnung schematisch dargestellten Ausführungsbeispiels näher erläutert. In dieser zeigen Fig.l einen teilweisen Schnitt durch ein Blutgefäß und ein Ende einer erfindungsgemäßen Gefäßprothese, Fig.2 eine vergrößerte Darstellung der Anschlussstellen
einer schlauchförmigen Prothese und insbesondere die Stülpkragenausbildung und Fig.3 eine schematische Ansicht der Art der Befestigung eines derartigen Stülpkragens nach einer Aortendissektion. In Fig.l ist mit 1 die Aortenwand schematisch dargestellt, wobei der Verlauf der Aorta insbesondere im Bereich des Aorten- bogens naturgemäß von der schematisiert geradlinig dargestellten Weise abweicht. Dies gilt insbesondere für den Bereich, in welchem an eine derartige Aorta im Aortenbogen Schlagadern angeschlossen sind, wie beispielsweise die Halsschlagader, welche in Fig.l schematisch mit 2 angedeutet ist. Nach erfolgter Aortendissektion wird eine offene Stirnfläche 3 des Blutgefäßes ausgebildet, welches aufgrund der häufig zu beobachtenden Zerschich- tung ehrlumig ausgebildet sein kann und von der idealisiert dargestellten Form in Fig.l insbesondere in der Dicke wesentlich abweichen kann. Die Gefäßprothese 4 wird nun mit dem Stülpkragenende 5 in die Aorta 1 über die Stirnflächen 3 aufgeschoben, wobei der außenliegende Falzbereich des Stülpkragens im wesentlichen S-förmig gekrümmt ausgebildet ist und an seinen beiden die Aortenwand umgreifenden Schenkeln 6 und 7 Nadeln bzw. Haken aufweist. Die Festlegung erfolgt mittels einer schematisch mit 8 angedeuteten Klemmpinzette, mit welcher die Nadeln bzw. Haken 9 des Stülpkragens in die Aortenwand eingepresst bzw. durch diese hindurchgepresst werden und am jeweils gegenüberliegenden Schenkel mechanisch eingreifen. Es erfolgt somit durch Zusammenpressen mittels einer Pinzette bzw. einem geeigneten Klemmwerkzeug 8 unmittelbar eine mechanische Verbindung des Stülpkragens 5 der Gefäßprothese 4 am Rand der dissektierten Aorta 1. Die Prothese 4 wird hierbei in Richtung des Pfeiles 10 in den Aorta eingeschoben, wobei im Bereich der Nadeln 9 zusätzlich entsprechende Klebstoffe und Gerinnungshilfsmittel aufgetragen sind, welche zu einer raschen dichten Verbindung beitragen. Der Mantel der Gefäßprothese kann hierbei, wie schematisch mit 11 angedeutet, Armierungen entsprechend einem Stent tragen, wobei durch weiteres Einschieben in Richtung des Pfeiles 10 diese Armierung an die Innenwand der Aorta gelangt und an diese durch entsprechende elastische Aufweitung oder ein entsprechendes Auf-
klappen angepresst werden können. Wenn gleichzeitig ein entsprechender Abgang, wie beispielsweise der Abgang für die Halsschlagader 2 freigehalten werden soll, ist in der Gefäßprothese 4 eine entsprechende axial sich erstreckende Durchbrechung 12 vorgesehen, welche nach dem Einschieben der Prothese in Richtung des Pfeiles 10 im Bereich der Halsschlagader 2 eine entsprechende Freistellung gewährleistet. Die Ausgestaltung des Stülpkragens ist insbesondere in den Fig. 2 und 3 noch näher erläutert. In Fig.2 ist die Wand der Prothese 4 ersichtlich, wobei die das Ende des dissektierten Gefäßes umgreifenden Schenkel 6 und 7 des Stülpkragens 5 vergrößert dargestellt sind. Es sind wiederum die Haken bzw. Nadeln 9 ersichtlich, welche von Klebstoff 13 umgeben sind. In den Klebstoff 13 können gleichzeitig blutstillende Substanzen eingearbeitet sein, sodass nach einem Zusammenpressen der freien Schenkel 6 und 7 in Richtung der Pfeile 14 und 15 zusätzlich mit der mechanischen Festlegung durch die Nadeln bzw. Haken 9 auch eine sichere Klebeverbindung bei gleichzeitiger Stillung des Blutes im Bereich der Zerschichtungen erzielt wird. In entsprechend räumlicher Darstellung zeigt Fig.3 den Anschluss einer derartigen Prothese 4 an einen Aortenbogen, wobei hier der Übersichtlichkeit halber die Prothese 4 in Umfangsrichtung aufgeschnitten dargestellt ist, um die Art des Anschlusses an die Aorta 1 besser zu verdeutlichen.
