Definitions
- the invention relates to a surgical lever, in particular for lifting the femur from the pelvic bone.
- Surgical levers of this type are typically used in operations with which the natural hip joint is replaced by an artificial one, in order to hold the femur after the resection of the femoral head in such a way that the femoral medullary cavity can be worked up. If the resection is performed while the hip joint is dislocated, there is also the option of lifting the femur from the pelvic bone during the resection.
- the edge of the acetabulum forms the abutment on which the surgical lever is supported.
- This is disadvantageous because, on the one hand, structures on the edge of the acetabulum can be damaged and, on the other hand, the edge of the acetabulum does not offer a secure abutment that prevents, for example, slipping sideways.
- the invention is based on the object of providing a surgical lever which avoids these disadvantages.
- the object is achieved in that a projection which is dimensioned for support in the depth of the acetabulum is arranged between the actuating handle and the engagement device. Because of its concave shape, the depth of the acetabulum provides a stable abutment for the surgical lever. Thanks to the concavity of the acetabulum, the protrusion positioned in place finds a secure hold.
- acetabulum as an abutment because of its concave shape is that the direction in which the abutment counteracts the surgical lever is not fixed.
- the direction in which the lever is supported in the acetabulum can vary depending on the situation.
- the support in the depth of the acetabulum provides security in any direction of force.
- the femur can be lifted off the pelvic bone in different directions.
- the projection of the surgical lever has a support surface which is convexly shaped in accordance with the curvature of the acetabulum. This shaping avoids local loads on the acetabulum, since the support surface acts on a larger area. It is also advantageous if several support surfaces with different radii of curvature are available, which alternatively depend on the size or radius of curvature of the acetabu- lums are used. The acetabula of different people are shaped differently and in particular have different diameters. The support surfaces with different radii of curvature enable the surgical lever to be used by as many people as possible. Either several levers with differently curved support surfaces are provided, or there are several interchangeable parts of the projection which form differently curved support surfaces.
- the support surface has an at least semi-spherical shape. This makes it particularly easy to insert it into the acetabulum and it centers itself in it.
- connection between the surgical lever and the support surface is preferably established via a cone plug connection, the cone angle being selected such that the support surface does not unintentionally come loose on the one hand and can be exchanged for another by simply removing and plugging it on.
- the part forming the support surface can be rigidly connected to the lever. When the lever is moved, the support surface slides in the acetabulum. However, it is also possible to connect this part to the lever in an articulated manner, so that the lever movement takes place on the joint while the support surface rests in the acetabulum.
- the attack device is concavely shaped in the direction of action in such a way that it can securely grip the outer shape of the bone, in particular the trochanter.
- the concave shape is suitable to prevent the trochanter from slipping off the surgical lever.
- the invention is described below with reference to the accompanying drawing based on an advantageous embodiment.
- the single figure shows a plan view of a surgical lever according to the invention when used on a hip joint.
- a human pelvic bone and part of the left femur 2 Shown are a human pelvic bone and part of the left femur 2, the femoral head of which has been resected and removed in the area of the femoral neck.
- the resection surface 4 and the acetabulum 10 are exposed.
- the femur 2 is lifted from the pelvic bone 1 with the aid of a surgical lever 3 according to the invention and is rotated inward such that the femoral medullary cavity is accessible for processing by the surgeon.
- the surgical lever 3 engages the large trochanter 8 of the femur 2.
- the surgical lever 3 comprises an actuating handle 5 and an engagement device 6.
- the actuating handle 5 and the engagement device 6 are elongate elements which are rigidly connected at their inner ends via an intermediate piece 7 which is essentially perpendicular to both.
- the attack device 6 is concavely shaped at its end by forming a bent back nose and thus adapted to the outer shape of the large trochanter 8.
- a cone 9 is arranged at the inner end of the attack device 6.
- the cone 9 forms an extension of the intermediate piece 7, displaced somewhat in the direction of the outer end of the engagement device 6.
- a conical plug connection is connected to a ball segment 11, in the sectional area of which there is a conical bore complementary to the cone 9.
- the outer surface of the spherical segment 11 is located in the acetabulum 10 and is shaped as a support surface in accordance with the curvature of the acetabulum 10.
- the cone 9 forms, together with the ball segment 11, a projection with which the surgical lever 3 is supported in the depth of the acetabulum 10.
- the ball segment 11 is held securely by the acetabulum 10.
- the ball segment 11 is also not moved out of its position by forces acting transversely to the cone 9.
- the ball segment 11 can be rotated in all directions within the acetabulum 10, so that the surgical lever can be applied both from different directions and at different angles.
- Acetabulum 10 forms an abutment over ball segment 11, which enables safe and versatile use of surgical lever 3.
